Abstract
本发明属于中医药领域,具体涉及一种改善甲状腺癌术后气血亏虚证的中药组合物。该中药组合物由手掌参、龙眼、玉竹、当归、扶芳藤、水红花子、蛇莓、灯芯草、白术、甘草组成,本发明紧紧围绕“虚者补之,结者散之”为原则,具有益气养阴、活血化瘀、软坚散结、扶正解毒的功效,可显著改善甲状腺癌术后患者气血亏虚证的诸多临床症状。

技术领域
本发明属于中医药领域,具体涉及一种改善甲状腺癌术后气血亏虚证的中药组合物。
背景技术
甲状腺癌在内分泌肿瘤和头颈肿瘤中均占第一位,约占整个人体实体肿瘤的1%,病理类型主要分为乳头状癌、滤泡状癌、未分化癌、髓样癌等,主要临床表现为颈部甲状腺肿块、疼痛、声音嘶哑及吞咽困难等。近年来统计表明甲状腺癌的发病率有增高的趋势,已经成为常见的恶性肿瘤之一。
甲状腺癌在中医古籍中并无明确特指的病名,但离不开中医“瘿瘤”的范畴。中医学认为甲状腺癌多因情志不畅、肝郁气滞、痰湿凝聚所致。瘀血阻络气滞痰凝则瘿肿如石,故称“石瘿”。发生机制为在正虚的基础上,气郁痰凝血瘀聚结颈前,日久蕴结成毒所致,但病久常因郁久化火,灼伤阴津,病程日久,耗伤阴血,气血双亏,则由实转虚,以气虚阴虚的病变多见。如《外科正宗·瘿瘤论》认为:“夫人生瘿瘤之症,非阴阳正气结肿,乃五脏瘀血,浊气,痰滞而成”。
中医药治疗甲状腺癌可贯穿整个治疗过程,有着独特的优势和潜力。基于辩证论治、整体观念原则基础上,临床上对于甲状腺癌的治疗多采用辨证与辨病相结合的治疗方式。大多数甲状腺癌治疗首选外科手术,并结合临床具体情况进行放疗、化疗、内分泌治疗等。患者预后一般,局部复发、远处转移及并发症和西医治疗带来的副反应均给患者带来许多痛苦。并且甲状腺癌术后,患者气、血、津液大量耗伤,多出现神疲乏力、自汗盗汗、口干、声音嘶哑、颈部胀满不适、二便失调等症状。
发明人根据自己多年的中医临床经验,依据《中医肿瘤治疗学》和《中医临床诊疗术语国家标准(证候部分)》,将甲状腺癌术后患者证型分为三类,肝郁痰阻证、气血亏虚证、心肝肾阴虚证。
肝郁痰阻证,常见症状为目眩头晕、口干、乏力、多梦、情志抑郁、肢体倦怠、心烦易怒、胸胁不舒、舌红、苔白、脉细、薄苔、脉沉。
气血亏虚证,症状为神疲乏力、多梦、肢体倦怠、大便干结、口干、心悸、失眠、心慌气短、声音嘶哑、自汗、苔白、薄苔、脉细、舌红、齿痕。
心肝肾阴虚证,症状为腰酸、口干、乏力、心烦易怒、多梦、肢体倦怠、盗汗、眼干目涩、喜太息、心悸、舌红、苔白、脉细、薄苔。
三类患者辨证虽各有不同,但均出现虚实夹杂之候,实证以肝郁痰阻为主,虚证以气虚、阴虚为主。痰瘀互结是瘿病日久,痰凝不化,血行不畅、血脉瘀阻所致。
目前,越来越多的证据表明中医药可以有效防止和延缓甲状腺癌的复发与转移,改善患者术后症状,加速术后康复。因此,中医药治疗成为甲状腺癌综合治疗方案中必不可少的一部分。
发明内容
发明人根据自己多年的中医临床经验,总结传统验方和医案的治疗经验,潜心研究提供了一种改善甲状腺癌术后气血亏虚证的中药组合物,可有效改善甲状腺癌术后多种临床症状,加速术后康复,提高患者生活质量,防止和延缓甲状腺癌的复发和转移。
为达到上述目的,本发明采用如下技术方案:
一种改善甲状腺癌术后气血亏虚证的中药组合物,其特征在于是由以下药用原料药制成:手掌参、龙眼、玉竹、当归、扶芳藤、水红花子、蛇莓、灯芯草、白术、甘草。
本发明各组分的具体药理研究现状如下:
手掌参:甘,平。归肺,脾,胃经。补益气血,生津止渴。治肺虚咳喘,虚劳消瘦,神经衰弱,久泻,失血,带下,乳少,慢性肝炎。《四川常用中草药》:补血益气,生津止渴。治肺虚咳喘,虚痨羸瘦,缺乳汁。《陕西中草药》:滋补强壮,补脾润肺,安神镇惊,益气止痛。治身体虚弱,神经衰弱,劳伤,咳嗽,气喘。
龙眼:甘,平,温。归心,脾,胃经。补心脾,益气血,健脾胃,养肌肉。治思虑伤脾,头昏,失眠,心悸怔忡,虚赢,病后或产后体虚,及由于脾虚所致之下血失血症。
玉竹:甘,微寒。归肺、胃经。功能主治:养阴润燥,生津止渴。用于肺胃阴伤,燥热咳嗽,咽干口渴,内热消渴。
当归:甘、辛,温。归肝、心、脾经。功能主治:补血活血,调经止痛,润肠通便。用于血虚萎黄,眩晕心悸,月经不调,经闭痛经,虚寒腹痛,肠燥便秘,风湿痹痛,跌扑损伤,痈疽疮疡。酒当归活血通经。用于经闭痛经,风湿痹痛,跌扑损伤。
扶芳藤:苦、甘,温。归肝;脾;肾经。功能主治:散瘀止血,舒筋活络。用于咯血,月经不调,功能性子宫出血,风湿性关节痛;外用治跌打损伤,骨折,创伤出血。
水红花子:咸,微寒。归肝、胃经。功能主治:散血消症,消积止痛。用于症瘕痞块,瘿瘤肿痛,食积不消,胃脘胀痛。
蛇莓:甘、酸,寒。有小毒。功能主治:清热解毒,散瘀消肿。用于感冒发热,咳嗽,小儿高热惊风,咽喉肿痛,白喉,黄疸型肝炎,细菌性痢疾,阿米巴痢疾,月经过多;外用治腮腺炎,毒蛇咬伤,眼结膜炎,疔疮肿毒,带状疱疹,湿疹。
灯笼草:苦,凉。功能主治:清热解毒,消炎利水。主治感冒发热,腮腺炎,支气管炎,急性肾盂肾炎,睾丸炎,泡疹,疖疮,疝气痛。
白术:苦、甘,温。归脾、胃经。功能主治:健脾益气,燥湿利水,止汗,安胎。用于脾虚食少,腹胀泄泻,痰饮眩悸,水肿,自汗,胎动不安。土白术健脾,和胃,安胎。用于脾虚食少,泄泻便溏,胎动不安。
甘草:甘,平。归心、肺、脾、胃经。功能主治:补脾益气,清热解毒,祛痰止咳,缓急止痛,调和诸药。用于脾胃虚弱,倦怠乏力,心悸气短,咳嗽痰多,脘腹、四肢挛急疼痛,痈肿疮毒,缓解药物毒性、烈性。
方解:手掌参、龙眼肉补益气血、扶正固本;手掌参、玉竹合用生津止渴、滋阴润燥;当归、扶芳藤补血散瘀、舒筋活血;水红花子、扶芳藤散瘀消积;蛇莓与灯芯草、甘草合用增强清热解毒之能;白术、甘草补脾健脾、燥湿祛痰,本方紧紧围绕“虚者补之,结者散之”为原则,拟定“益气养阴、活血化瘀、软坚散结、扶正解毒”的基本治则。
本发明中药组合物在中药组分选择上以补益气血、生津养阴、活血化瘀、化痰散结、清热解毒为主要标准,同时本发明中药组合物组合后具有显著改善甲状腺癌术后气血亏虚证的诸多临床症状。本发明对上述诸味中药的重量份数进行了优选,优选的条件是中药配伍使用后药物对甲状腺癌术后气血亏虚证的诸多临床症状的改善效果。
作为本发明的一个优选,本发明所述的改善甲状腺癌术后气血亏虚证的中药组合物由以下重量份的药用原料药制成:手掌参15-25份、龙眼20-30份、玉竹15-25份、当归10-20份、扶芳藤25-35份、水红花子11-17份、蛇莓8-15份、灯芯草30-40份、白术11-15份、甘草5-9份。
作为本发明的进一步优选,本发明所述的改善甲状腺癌术后气血亏虚证的中药组合物由以下重量份的药用原料药制成:手掌参17-23份、龙眼23-28份、玉竹20-25份、当归13-18份、扶芳藤25-30份、水红花子12-16份、蛇莓11-15份、灯芯草33-38份、白术11-15份、甘草5-9份。
作为本发明的更进一步优选,本发明所述的改善甲状腺癌术后气血亏虚证的中药组合物由以下重量份的药用原料药制成:手掌参19份、龙眼24份、玉竹22份、当归16份、扶芳藤27份、水红花子15份、蛇莓13份、灯芯草36份、白术14份、甘草7份。
为了更好地表达本发明的中药组合物,本发明的中药组合物可以制备成临床上常用的剂型,其主要包含下述步骤:
(1)取处方量的水红花子、蛇莓加所述药材总重量的3-6倍量乙醇提取二次,每次1-2小时,合并提取液,滤过,滤液回收乙醇,减压浓缩;
(2)取处方量的手掌参、龙眼、玉竹、当归、扶芳藤、灯芯草、白术、甘草加入所述药材总重量的5-10倍量水煎煮二次,第一次煎煮2-3小时,第二次煎煮1-2小时,合并煎液,滤过,滤液减压浓缩,加入乙醇使得溶液的含乙醇量为50-70%,溶液于-5℃-0℃放置12-24小时,滤过,滤液回收乙醇,浓缩,
(3)合并以上二种稠膏,加入淀粉,混匀,干燥,粉碎成细粉,混匀,过筛,制备得到临床上常用中药药物剂型。
临床上常用中药药物剂型比如,丸剂、片剂、颗粒剂、口服液、胶囊剂、膏剂、乳剂、咀嚼剂当中的任何一种。
优选地,本发明中药组合物按照常规制备工艺制备成颗粒剂、片剂或胶囊剂。
所述药物剂型中加入了药物赋形剂。优选地,所述的赋形剂是调味剂、分散剂、粘合剂、增稠剂、润滑剂、稀释剂当中的一种或几种。
以下通过临床观察试验来进一步阐述本发明药物组合物的效果。
临床观察试验
1.一般资料:60例甲状腺癌术后患者,来自2012年5月-2013年7月我院内分泌科,采用随机数字表法随机分为治疗组和对照组,各30例。治疗组,男6例,女24例;平均年龄30.6±12.4岁;对照组男5例,女25例;平均年龄30.8±12.1岁;;两组预计生存期>3个月,两组在性别、年龄方面的差异无统计学意义(P>0.05),具有可比性。
2.诊断标准:经术前B超、甲状腺核素扫描或针吸活组织检查确诊,并经术后病理证实诊断为甲状腺癌;伴或不伴有甲状腺功能异常;中医辨证为气血亏虚证,临床表现为面色苍白,或淡白,或萎黄无华,无光泽,神疲乏力,少气懒言,自汗,头晕目眩,言语音低,心慌气短,脱发,口唇、眼睑、爪甲色淡,肌肤干燥,头发干枯,掉发,发黄,甚则眉毛、腋毛、阴毛亦同时稀少脱落,形体消瘦,平时易于感冒,两目干涩,或肢体肌肉麻木、闰动、蠕动,饮食不佳,失眠,手心偏热或者出汗或者手冰冷,妇女月经量少,色淡或月经延期不至,甚至闭经,颈部手术部位肿胀不适,疤痕色深,长时间无消退,舌体瘦薄,或者舌面有裂纹,舌淡或淡胖有齿痕,苔少,脉细无力或芤。
3.病例排除标准:妊娠、哺乳期妇女、肝功能异常者;合并有心血管、肝、肾和造血系统等严重原发性疾病,精神病患者;接受其他中药、糖皮质激素及其它免疫抑制剂治疗者;近三月内参加其他药物临床试验者。
4.治疗方法:
对照组:给予左甲状腺素钠片治疗,根据甲状腺功能调整剂量。
治疗组:在对照组治疗的基础上加用本发明实施例1制备的胶囊治疗,一日两次,每次4-6粒。
两组均以1月为1疗程,3个疗程结束后进行疗效观察。
5.评价指标
5.1实验室指标:采用血清游离三碘甲状腺原氨酸(FT3)、血清游离甲状腺素(FT4)、血清促甲状腺激素(TSH)及尿碘中位数(MUI),别在治疗前、及治疗后2个月采集。
5.2中医症状评分:1:症状无或消失;2分:症状轻微,偶有;3分:症状明显影响生活;4分:症状严重影响生活。分别在治疗前、及治疗后2个记录。
5.3统计学分析采用spss16.0统计软件进行分析,计数资料两样本率的比较采用X2检验,计量资料以表示,治疗前后比较用配对t检验,组间比较采用t检验,P<0.05表示差异有统计学意义。
6.结果
6.1两组治疗前后证候积分比较,见表1。两组治疗后证候积分均显著改善,与治疗前比较,差异均有显著性意义(P<0.05,P<0.01);两组治疗后比较,差异也有显著性意义(P<0.05)。

6.2两组甲状腺功能治疗前后的变化:
治疗后两组FT3、FT4均明显升高、TSH、明显下降,差异有统计学意义(P<0.05);治疗后比较两组组间甲状腺功能变化,FT3、FT4均上升,TSH、MUI均下降,治疗组甲状腺功能变化幅度较对照组略大,TSH、MUI组间比较差异有统计学意义(P<0.05),见表2。

综上,本发明的中药组合物,可有效多种临床症状,加速术后康复,提高患者生活质量。本发明还请求保护上述中药组合物在制备改善甲状腺癌术后气血亏虚证的中药药物中的用途。
本发明的有益效果:
1.本发明中药组合物可显著改善甲状腺癌术后患者的气血亏虚证的多种症状,加速术后康复,提高患者生活质量;
2.本发明药物组合物为常规剂型服用、携带方便,可长期保存,避免了长期服用中药汤剂煎药麻烦、单次服用量大、患者服用依从相差的缺点。
具体实施方式

实施例1
处方:手掌参19份、龙眼24份、玉竹22份、当归16份、扶芳藤27份、水红花子15份、蛇莓13份、灯芯草36份、白术14份、甘草7份。
制备方法:取处方量的水红花子、蛇莓加所述药材总重量的3倍量乙醇提取二次,每次1小时,合并提取液,滤过,滤液回收乙醇,减压浓缩;取处方量的手掌参、龙眼、玉竹、当归、扶芳藤、灯芯草、白术、甘草加入所述药材总重量的6倍量水煎煮二次,第一次煎煮2.5小时,第二次煎煮1小时,合并煎液,滤过,滤液减压浓缩,加入乙醇使得溶液的含乙醇量为55%,溶液于0℃放置16小时,滤过,滤液回收乙醇,浓缩;合并以上二种稠膏,加入淀粉,混匀,干燥,粉碎成细粉,混匀,过筛,用常规制剂方法制成胶囊剂100份。
实施例2
处方:手掌参17份、龙眼23份、玉竹20份、当归13份、扶芳藤2份、水红花子12份、蛇莓11份、灯芯草33份、白术11份、甘草5份。
制备方法:取处方量的水红花子、蛇莓加所述药材总重量的5倍量乙醇提取二次,每次1.5小时,合并提取液,滤过,滤液回收乙醇,减压浓缩;取处方量的手掌参、龙眼、玉竹、当归、扶芳藤、灯芯草、白术、甘草加入所述药材总重量的8倍量水煎煮二次,第一次煎煮2小时,第二次煎煮1小时,合并煎液,滤过,滤液减压浓缩,加入乙醇使得溶液的含乙醇量为65%,溶液于-5℃放置12小时,滤过,滤液回收乙醇,浓缩;合并以上二种稠膏,加入淀粉,混匀,干燥,粉碎成细粉,混匀,过筛,用常规制剂方法制成胶囊剂。
实施例3
处方:手掌参23份、龙眼28份、玉竹25份、当归18份、扶芳藤30份、水红花子16份、蛇莓15份、灯芯草38份、白术15份、甘草9份。
制备方法:取处方量的水红花子、蛇莓加所述药材总重量的5倍量乙醇提取二次,每次2小时,合并提取液,滤过,滤液回收乙醇,减压浓缩;取处方量的手掌参、龙眼、玉竹、当归、扶芳藤、灯芯草、白术、甘草加入所述药材总重量的10倍量水煎煮二次,第一次煎煮3小时,第二次煎煮1小时,合并煎液,滤过,滤液减压浓缩,加入乙醇使得溶液的含乙醇量为60%,溶液于-2℃放置24小时,滤过,滤液回收乙醇,浓缩;合并以上二种稠膏,加入淀粉,混匀,干燥,粉碎成细粉,混匀,过筛,用常规制剂方法制成胶囊剂。
实施例4
处方:手掌参21份、龙眼25份、玉竹23份、当归16份、扶芳藤26份、水红花子14份、蛇莓12份、灯芯草34.5份、白术13.5份、甘草8份
制备方法:参照实施例1的操作进行。
实施例5
处方:手掌参15份、龙眼20份、玉竹15份、当归10份、扶芳藤25份、水红花子11份、蛇莓8份、灯芯草30份、白术11份、甘草5份。
制备方法:参照实施例1的操作进行。
实施例6
处方:手掌参25份、龙眼30份、玉竹125份、当归20份、扶芳藤35份、水红花子17份、蛇莓15份、灯芯草40份、白术15份、甘草9份。
制备方法:取处方量的水红花子、蛇莓加所述药材总重量的4倍量乙醇提取二次,每次2小时,合并提取液,滤过,滤液回收乙醇,减压浓缩;取处方量的手掌参、龙眼、玉竹、当归、扶芳藤、灯芯草、白术、甘草加入所述药材总重量的7倍量水煎煮二次,第一次煎煮2.5小时,第二次煎煮1.5小时,合并煎液,滤过,滤液减压浓缩,加入乙醇使得溶液的含乙醇量为65%,溶液于-5℃放置18小时,滤过,滤液回收乙醇,浓缩;合并以上二种稠膏,加入淀粉,混匀,干燥,粉碎成细粉,混匀,过筛,用常规制剂方法制成片剂。
实施例7
处方:手掌参18份、龙眼25份、玉竹17份、当归15份、扶芳藤30份、水红花子13份、蛇莓11份、灯芯草34份、白术12份、甘草7份。
制备方法:参照实施例6的操作进行。
实施例8
处方:手掌参20份、龙眼22份、玉竹20份、当归15份、扶芳藤32份、水红花子15份、蛇莓14份、灯芯草38份、白术15份、甘草8份。
制备方法:取处方量的水红花子、蛇莓加所述药材总重量的4倍量乙醇提取二次,每次2小时,合并提取液,滤过,滤液回收乙醇,减压浓缩;取处方量的手掌参、龙眼、玉竹、当归、扶芳藤、灯芯草、白术、甘草加入所述药材总重量的7倍量水煎煮二次,第一次煎煮2.5小时,第二次煎煮1.5小时,合并煎液,滤过,滤液减压浓缩,加入乙醇使得溶液的含乙醇量为65%,溶液于-5℃放置18小时,滤过,滤液回收乙醇,浓缩;合并以上二种稠膏,加入淀粉,混匀,干燥,粉碎成细粉,混匀,过筛,用常规制剂方法制成颗粒剂。
Claims (7)
1.一种改善甲状腺癌术后气血亏虚证的中药组合物,其特征在于是由以下重量份的药用原料药制成:手掌参15-25份、龙眼20-30份、玉竹15-25份、当归10-20份、扶芳藤25-35份、水红花子11-17份、蛇莓8-15份、灯芯草30-40份、白术11-15份、甘草5-9份。
2.如权利要求1所述的中药组合物,其特征在于是由以下重量份的药用原料药制成:手掌参17-23份、龙眼23-28份、玉竹20-25份、当归13-18份、扶芳藤25-30份、水红花子12-16份、蛇莓11-15份、灯芯草33-38份、白术11-15份、甘草5-9份。
3.如权利要求1或2所述的中药组合物,其特征在于是由以下重量份的药用原料药制成:手掌参19份、龙眼24份、玉竹22份、当归16份、扶芳藤27份、水红花子15份、蛇莓13份、灯芯草36份、白术14份、甘草7份。
4.一种如权利要求1-3所述中药组合物的制备方法,其特征在于它包括以下步骤:
(1)取处方量的水红花子、蛇莓加所述药材总重量的3-6倍量乙醇提取二次,每次1-2小时,合并提取液,滤过,滤液回收乙醇,减压浓缩;
(2)取处方量的手掌参、龙眼、玉竹、当归、扶芳藤、灯芯草、白术、甘草加入所述药材总重量的5-10倍量水煎煮二次,第一次煎煮2-3小时,第二次煎煮1-2小时,合并煎液,滤过,滤液减压浓缩,加入乙醇使得溶液的含乙醇量为50-70%,溶液于-5℃-0℃放置12-24小时,滤过,滤液回收乙醇,浓缩;
(3)合并以上二种稠膏,加入淀粉,混匀,干燥,粉碎成细粉,混匀,过筛,制备得到临床上常用中药药物剂型。
5.如权利要求4所述的制备方法,其特征在于所述药物剂型是丸剂、片剂、颗粒剂、口服液、胶囊剂、膏剂、乳剂、咀嚼剂当中的任何一种。
6.如权利要求5所述的制备方法,其特征在于所述药物剂型中加入了药物赋形剂。
7.如权利要求6所述的制备方法,其特征在于所述的赋形剂是调味剂、分散剂、粘合剂、增稠剂、润滑剂、稀释剂当中的一种或多种。
