背景技术
糖尿病性胃轻瘫是糖尿病的一种常见慢性合并症之一,糖尿病胃轻瘫是糖尿病胃肠植物神经病变常见的症状,约见于50%的糖尿病患者。临床表现为慢性胃炎、胃驰缓与胃潴留。典型症状为腹胀、早饱、厌食、嗳气、恶心、呕吐、体重减轻,餐后症状加重。糖尿病胃轻瘫由于胃排空延迟可致胃潴留,反复发作,可致胃石形成;并发食管下括约肌压力减低时可致胃到食管反流,形成反流性食管炎;各类症状反复性发生可引起小肠和结肠排空异常,导致腹痛、便秘、腹泻等症状。目前常采用西沙必利治疗,但该西药治疗短期疗效满意,但长期应用,则胃动力作用反而减弱。申请人依据中医理论研究,认为该病属于中医“消渴”“痞满”范畴。糖尿病胃轻瘫是糖尿病发展到中后期阶段常见并发症之一,此阶段脾气损伤,气堂失运,燥热灼津,津亏气耗,久病必虚,故此时病机表现为气虚为主。脾胃虚弱、气机升降失调,致饮食水谷运化失职,精微物质的化生和气、血、精液的生成、输布、代谢发生障碍,故病机以脾气虚弱、运化无力为本,气滞、血瘀、痰浊、食积、湿热等引起胃失和降为标。

上述的一种治疗糖尿病胃轻瘫的中药,其原料组份的重量配比为:太子参18~22g、黄芪38~42g、山药13~17g、白术13~17g、乌贼骨13~17g、煅瓦楞子13~17g、刺猬皮13~17g、肉豆蔻7~11g、洋金花0.3~0.7g、枳实10~14g、苏梗10~14g、木香10~14g、吴茱萸1~5g、枳壳3~5g、厚朴4~6g、佛手3~5g、川楝子4~6g、合欢皮3~5g、夜交藤4~6g、柴胡3~5g、香附4~6g、莱菔子4~6g、生姜3~5g、半夏3~5g、地黄1~5g、火麻仁2~3g。

在上述的一种治疗糖尿病胃轻瘫的中药中:黄芪、太子参、山药、白术益气健胃以治脾虚之本,配合枳实一升一降,升清降浊,健脾消痞。太子参甘凉滋润,取其益气养阴,润胃而不滞气之性。黄芪总甙具有促进胃肠道吸收功能的作用;枳实对胃肠道平滑肌有一定兴奋作用;肉豆蔻辛温,归脾胃大肠经。温中行气,涩肠止泻。现代药理研究其具有调节肠胃功能健胃、镇静作用。《本草汇言》谓其“为和平中正之品,运宿食而不伤,非若枳实、莱菔子之有损真气也;下滞气而不峻,非若香附、大腹皮之有泄真气也,止泄泻而不涩,非若诃子、罂粟壳之有兜塞掩伏二闭邪气也。”实验也表明,肉豆蔻所含挥发油可刺激胃肠道局部,少量内服可增加胃液分泌,刺激胃壁蠕动,从而增进食欲,帮助消化,并有轻微发酵作用,若大量使用则对胃肠道有抑制作用;洋金花辛温、归心肺脾经,止痛解痉,所含东莨菪碱和阿托品为M-胆碱缓释阻断剂,抑制兴奋中枢,镇痛作用强,相当于西药胃肠动力药物;乌贼骨归脾肾经制酸止痛《现代实用中药》:“为制酸药”;煅瓦楞子归肝胃经,煅用可治胃痛吐酸,有制酸止痛的功效,《山东中草药手册》:“制酸止痛,治溃疡病”;刺猬皮苦能泄降,有化瘀止痛作用,治疗气滞血瘀之胃脘疼痛,《中药大辞典》:“烧灰酒服治胃逆,又煮汁服止反胃”;苏梗、木香和降胃气,木香除了加快胃对钡剂的排空外,并能促进内源性胃动素的释放;不同阶段糖尿病胃轻瘫均有微循环障碍,运用活血化瘀中药可以改善微循环、降低血粘度、产生治疗效果,治诸经之瘀血,故活血祛瘀,能改善糖尿病慢性血管、神经病变;吴茱萸温胃降逆开郁结。枳壳、厚朴辅助治疗胃胀满;佛手、川楝子辅助治疗胃疼痛;合欢皮、夜交藤辅助治疗失眠多梦;柴胡、香附辅助治疗肝郁气滞;莱菔子辅助治疗早饱厌食;生姜、半夏辅助治疗恶心呕吐;地黄、火麻仁辅助治疗口干便秘。以上药物结合,起到健脾益气,活血祛瘀、理气止痛、和胃降逆之功效。本病从中医整体现象出发,辨证施治,有独特的疗效,同时中医从多系统、多器官、多靶点有效调节机体的功能紊乱,发挥防治疾病、提高患者生活质量的作用。经研究观察显示,本中药配方优于西药西沙必利,且副作用少,运用灵活,适合长期治疗,值得推广应用。
具体实施方式
实施例1,一种治疗糖尿病胃轻瘫的中药,其原料组份的重量配比为:太子参18~22g、黄芪38~42g、山药13~17g、白术13~17g、乌贼骨13~17g、煅瓦楞子13~17g、刺猬皮13~17g、肉豆蔻7~11g、洋金花0.3~0.7g、枳实10~14g、苏梗10~14g、木香10~14g、吴茱萸1~5g、枳壳3~5g、厚朴4~6g、佛手3~5g、川楝子4~6g、合欢皮3~5g、夜交藤4~6g、柴胡3~5g、香附4~6g、莱菔子4~6g、生姜3~5g、半夏3~5g、地黄1~5g、火麻仁2~3g。
制备及使用方法:每日1剂,水煎2次取汁350~450ml,分2次饭后服用。
实施例2,一种治疗糖尿病胃轻瘫的中药,其原料组份的重量配比为:太子参20g、黄芪40g、山药15g、白术15g、乌贼骨15g、煅瓦楞子15g、刺猬皮15g、肉豆蔻9g、洋金花0.5g、枳实12g、苏梗12g、木香12g、吴茱萸3g、枳壳4g、厚朴5g、佛手4g、川楝子5g、合欢皮4g、夜交藤5g、柴胡4g、香附5g、莱菔子5g、生姜4g、半夏4g、地黄3g、火麻仁2g。
制备及使用方法:每日1剂,水煎2次取汁400ml,分2次饭后服用。

实施例4,一种治疗糖尿病胃轻瘫的中药,其原料组份的重量配比为:太子参22g、黄芪42g、山药17g、白术17g、乌贼骨17g、煅瓦楞子17g、刺猬皮17g、肉豆蔻11g、洋金花0.7g、枳实14g、苏梗14g、木香14g、吴茱萸5g、枳壳5g、厚朴6g、佛手5g、川楝子6g、合欢皮5g、夜交藤6g、柴胡5g、香附6g、莱菔子6g、生姜5g、半夏5g、地黄5g、火麻仁3g。
制备及使用方法:每日1剂,水煎2次取汁450ml,分2次饭后服用。
效果验证:糖尿病胃轻瘫是糖尿病的一种常见慢性合并症之一,是糖尿病自主神经病变之一,约见于50%的糖尿病患者,临床发现食道、胃、小肠、大肠等消化道运动功能障碍。表现为恶心、呕吐、饱胀、腹痛、腹胀、腹泻、便秘等症状。西医治疗短期疗效满意,但长期应用,则胃动力作用反而减弱。发明人于2012年1月~2014年6月采用本发明一种治疗糖尿病胃轻瘫的中药治疗92例糖尿病胃轻瘫疗效满意,现报道如下。
1. 临床资料
1.1 一般资料:全部病例均选自我院门诊及住院的2型糖尿病患者,按照随机分组将184例患者分为治疗组92例,对照组92例。治疗组中,男34例,女58例;糖尿病病史5~20年;单纯应用磺脲类、双胍类药物治疗者80例,合用胰岛素治疗者12例;合并胃轻瘫病史6个月~3年。对照组中,男32例,女60例;糖尿病病史6~19年;单纯应用磺脲类、双胍类药物治疗者78例,合用胰岛素治疗者14例;合并胃轻瘫病史5个月~2年10个月。两组患者在年龄、性别、病情轻重、病程方面相似,经统计学处理,差异无显著性意义(P>0.05),具有可比性。
1.2 诊断标准:参照《内科学》(2013年3月第8版),①1次或2次以上空腹血糖(FPG)≥7.0mmol/L;②1次或2次以上餐后2h血糖(2hppG)≥11.1mmol/L,尿糖多为阳性,半数以上伴有口干、多饮、多尿、消谷善肌、乏力、体重减轻等症状。确诊为2型糖尿病,同时符合《百度百科糖尿病性胃轻瘫》诊断依据拟定:①糖尿病病程5年以上;②食后饱胀、恶心、厌食、上腹不适;③胃部饱胀、振水声阳性;④X线钡餐检查胃蠕动收缩力减弱,排空迟缓;⑤无溃疡及幽门梗阻;⑥排除有明显重要器官并发症者。
2.治疗方法
2.1治疗组:①采用本发明一种治疗糖尿病胃轻瘫的中药:其原料组份的重量配比为:太子参20g、黄芪40g、山药15g、白术15g、乌贼骨15g、煅瓦楞子15g、刺猬皮15g、肉豆蔻9g、洋金花0.5g、枳实12g、苏梗12g、木香12g、吴茱萸3g、枳壳4g、厚朴5g、佛手4g、川楝子5g、合欢皮4g、夜交藤5g、柴胡4g、香附5g、莱菔子5g、生姜4g、半夏4g、地黄3g、火麻仁2g。每日1剂,水煎2次取汁400ml,分2次饭后服用。
2.2对照组:口服西沙必利(国药准字H20020345浙江京新药业股份有限公司),每次5mg,每日3次,餐前30分钟服用。
两组病例均在原控制血糖及饮食的基础治疗原发病。1个月为1个疗程,1个疗程后评定疗效。
3. 疗效观察
3.1 疗效标准:治愈:临床症状完全消失,X线钡餐检查排空正常;有效:临床症状明显减轻或消失,X线钡餐检查仍为排空延迟;无效:临床症状改善不明显,X线钡餐检查表示大量储留或排空延迟。

1.一种治疗糖尿病胃轻瘫的中药,其特征在于,其原料组份的重量配比为:太子参18~22g、黄芪38~42g、山药13~17g、白术13~17g、乌贼骨13~17g、煅瓦楞子13~17g、刺猬皮13~17g、肉豆蔻7~11g、洋金花0.3~0.7g、枳实10~14g、苏梗10~14g、木香10~14g、吴茱萸1~5g、枳壳3~5g、厚朴4~6g、佛手3~5g、川楝子4~6g、合欢皮3~5g、夜交藤4~6g、柴胡3~5g、香附4~6g、莱菔子4~6g、生姜3~5g、半夏3~5g、地黄1~5g、火麻仁2~3g。
