背景技术
巨核细胞(MK)分化是以顺序的步骤为特征的连续过程。MK倍性通过核内有丝分裂增加,同时细胞大小平行增大。储存细胞器和质膜的合成增加,导致分隔膜的形成。胞质成熟与MK大小的显著增加相关。最后,成熟MK经历微管参与的完全细胞骨架重组,来诱导相当于前血小板(proplatelet)的伪足伸长(Patel等人,2005a;Richardson等人,2005)。血小板从这些包含所有血小板细胞器的前血小板的尖端释放。
循环血小板通过粘附于血管壁和通过将第一层粘附血小板聚集到内皮下基质参与血栓形成(Ruggeri,2003)。血管性血友病因子(Von Willebrandfactor)(VWF)是内衬于脉管系统中的内皮细胞的主要组分。VWF是能够在高剪切率(>1000s-1)条件中抑留循环血小板的唯一蛋白。系留的血小板通过GPIb(又名CD42b)与VWF A1结构域的结合转位到固定基质VWF上(Huizinga等人,2002)。在血小板的瞬时系留过程中进行性发生的αIIbβ3整联蛋白(又名CD41a/CD61)的活化包括GPIb-介导的信号传导并使得αIIbβ3结合VWF的RGD序列。巨血小板综合症(BSS)是以严重的血小板减少和巨血小板为特征的出血性障碍(Nurden,2005)。这是由于GPIb-IX-V复合物,特别是包含VWF结合位点的GPIbα亚基,的数量上或性质上的异常。在BSS病人的骨髓中发现正常数目的MK,提示在该情况中观察到的巨血小板减少症与MK的缺陷血小板形成有关。
由于不是经常在骨髓隙中观察到MK破碎成血小板,MK需要迁移入骨髓毛细管中(Tavassoli和Aoki,1981)。血小板与前血小板脱离已在缺少流动条件的情况下描述。有趣的是,从在促血小板生成素(TPO)存在的情况下培养的MK脱落的血小板产量远低于在最佳成熟条件中可能预期的产量(Norol等人,1998)。血小板脱落的许多步骤仍然是难以捉摸的。特别是从来没有证明剪切力对血小板形成的作用。VWF受体GPIb和αIIbβ3都是在成熟过程中在MK表面上表达(Debili等人,1990)。少数成熟MK已经在血液循环中被识别(Pedersen,1978;Tavassoli和Aoki,1981),其中它们暴露于在内皮细胞的内腔侧上的VWF。

发明详述
定义:
如本发明所使用的,术语“血小板”表示参与导致形成血凝块的初级止血(primary hemostasis)的细胞机制的由细胞衍生的无核胞质体。
如本发明所使用的,术语“前血小板”表示可导致血小板形成的巨核细胞或其片段的任何结构形式,例如胞质连接的血小板样颗粒。所述结构形式包括但不限于具有长的胞质延伸、突出或伪足的细胞,所述延伸、突出或伪足包含包括在各形成阶段的血小板体的膨大,例如,节结、水泡等。
如本发明所使用的,术语“巨核细胞”表示负责产生正常止血所需的血小板的骨髓细胞。巨核细胞源自骨髓中的造血干细胞前体细胞。巨核细胞生成的原始信号是促血小板生成素或TPO。TPO对于诱导骨髓中的祖细胞向最终巨核细胞表型的分化是必需的。用于巨核细胞分化的其他分子信号包括GM-CSF、IL-3、IL-6、IL-11和促红细胞生成素。
如本发明所使用的,术语“成熟巨核细胞”表示以稳定方式表达GPIb和αIIbβ3表面标志物的巨核细胞的群体。如本发明所使用的,表面标志物的“稳定方式表达”表示在细胞群体中,至少70%的细胞表达这些表面标志物。
如本发明所使用的,术语“剪切率”表示用于表征层流的参数。在管中,层流具有抛物线流速特征,相邻流动层的渐增的流速在腔轴处达到最大值。在相邻层中径向速率增加率定义为剪切率。在平行板腔中,剪切率由公式(γ)给出,并对应于流动物料中的速度梯度。用于剪切率的SI计量单位是s-1,表示为“倒易秒”或“倒数秒”。剪切率的公式(γ)是:

其中γ是剪切率(以s-1计),Q是流速(ml/s),a=狭缝宽度(cm),b=狭缝高度(cm)和f(k)是该系统的物理参数的函数。例如,使用来自马斯特里赫特仪表(Maastricht Instrumentation)的流体室(描述在(Legendre等人,2006)中),对于剪切率>100s-1,f(k)等于1.03。因此,可以通过控制流速和腔室的狭缝高度来调节剪切率。

根据本发明,VWF因子可以是重组VWF或天然VWF。在以其天然形式存在时,其可被纯化或可被包含在含有其他组分的组合物中(例如在细胞外基质中)。

用于制备血小板的方法和装置:
本发明涉及用于制备血小板的体外方法,所述方法包括使成熟巨核细胞的悬浮液在涂覆有血管性血友病因子的固相上经历剪切率至少为600s-1的流动。
在优选的实施方式中,该流动的剪切率至少为800s-1、优选至少为1000s-1、优选至少为1200s-1、优选至少为1400s-1、更优选至少为1600s-1、至少为1800s-1或者至少为2000s-1。
典型地,该流动的剪切率不超过血小板在体内可能经受的生理剪切率。典型地,所述剪切率不超过6000s-1、优选不超过4000s-1、更优选不超过3000s-1。
在一个实施方式中,成熟巨核细胞的悬浮液呈现0.5×106到4×106/mL,优选至少1×106/mL,优选至少2×106细胞/mL,更优选至少4×106/mL的细胞浓度。
典型地,成熟巨核细胞的悬浮液包含悬浮在适当的细胞培养基中的巨核细胞群体。在一个实施方式中,所述细胞培养基是Iscove's ModifiedDulbecco's培养基(IMDM)。
成熟巨核细胞的悬浮液可以在从血液样品或者骨髓样品分离后获得。或者,可在适当条件下使用稳定的巨核细胞细胞系(例如DAMI(Greenberg等人,1988),Meg-01(Isakari等人,2009),UT7/TPO(Barroga等人,2008))的悬浮液。根据本发明,成熟巨核细胞悬浮液经历600s-1的剪切率,持续足以产生血小板的处理时间。典型地,所述处理时间可以为10分钟到2小时,典型地为15分钟到1小时,更优选地为15分钟到30分钟。在优选的实施方式中,所述处理时间为约20分钟。
在具体的实施方式中,固相通过与血管性血友病因子或者其片段或变体或类似物的溶液一起孵育而涂覆。
用于涂覆固相的VWF的浓度通常为5到100μg/mL。在优选的实施方式中,VWF的浓度是20μg/mL。
在一个实施方式中,该固相涂覆有选自下组的片段或者变体或者类似物:VWF的52/48-kDa胰蛋白酶片段、金黄色葡萄球菌V-8蛋白酶消化的VWF、用于治疗用途的VWF浓缩物和重组野生型VWF或引起2N型血管性血友病的VWF突变体。
用于由成熟巨核细胞制备血小板的方法可以在流体室中进行,其中所述流体室包括涂覆有VWF或者其片段或变体或类似物的底壁,其中所述其片段、变体或类似物结合GPIb。

在进一步的实施方式中,用于制备血小板的流体室是在(Mekrache等人,2002)和(Legendre等人,2006)中描述的腔室。
在另一个实施方式中,所述流体室是微流体流动系统例如玻璃微毛细管(Kauskot等人,2007),或微流体生物芯片和流量传感器(Robinson等人,2008;Williams等人,2002),或任何其他类型的微流体流动系统(例如参见Conant等人,2009)。
有利地,适合于本发明的一些流体室允许在无菌条件下工作(例如参见Williams等人,2002和Conant等人,2009)。在优选的实施方式中,所述流体室是无菌的。
在进一步的实施方式中,剪切率由于流体室中的电泵而获得。
血小板和药物组合物:
作为另一目的,本发明涉及由上述方法可获得的血小板。

而且,本发明血小板和本发明药物组合物可用于治疗血小板计数减少症障碍,特别是血小板减少症和血小板病。例如,由本发明方法可获得的血小板可以以有效量输入患有血小板生成障碍的受试者中。
在优选的实施方式中,血小板计数减少障碍可选自自身免疫性血小板减少症、与生成减少有关的血小板减少症(中心起源)或与任何原因的破坏增加有关的血小板减少症(外周起源)。
本发明的相关方面涉及用于治疗患有血小板计数减少症障碍的受试者的方法,所述方法包括将有效量的本发明血小板(或其群体或其药物组合物)给药于受试者的步骤。
在本发明的情况中,如本文所使用的术语“治疗”或“疗法”意指目的在于延迟或防止病理发作、在于逆转、缓解、抑制、延缓或停止病理症状的发展、加重或恶化、在于导致病理症状的改善和/或在于治愈病状的方法。
如本文所使用的,术语“有效量”意指足以实现预定目的的本发明血小板(或其群体或其药物组合物)的任何量。
有效剂量和给药方案可以基于受试者的病理性质通过良好的医疗实践容易地确定,并取决于许多因素,包括但不限于:病理症状的程度和所关注的组织或器官的损伤或退化程度,以及受试者的特征(例如,年龄、体重、性别、总体健康等)。
为了治疗,本发明的血小板和药物组合物可以通过静脉内给药而全身性施用。给药剂量和次数可以由本领域技术人员以已知方式优化。
而且,本发明的血小板可用于诊断目的。它们可作为用于血小板功能标准化的正常对照。
血小板功能测试要求来自正常个体和感染个体的天然功能性状态下的新鲜血液血小板。血管性血友病是出血性疾病的最常见的单一原因。另外,高达50%的皮肤粘膜出血性疾病由血小板功能障碍所引起。血小板功能测试的标准化要求实验室应该对所进行的每批血小板功能测试进行正常对照。然而,通过静脉穿刺的连续常规血液采样引起几种健康担忧和伦理问题。血小板功能的实验室评估包括透光率集合度测定(light transmittance aggregation)和ATP释放试验、糖蛋白试验、电子显微术、促凝血功能测试、遗传测试(Pai和Hayward,2009)。
因此本发明涉及用于诊断血小板障碍的方法,包括使用本发明的血小板以标准化血小板功能的步骤。
通常,本发明的血小板可以由上述的巨核细胞细胞系获得。然后它们用于在体外诊断测试中标准化血小板功能。换言之,它们在测定血小板功能的体外诊断试验中用作阳性对照。
通过以下实施例、附图和表格进一步说明本发明。

实施例
材料和方法
蛋白质
VWF由Laboratoire francais du Fractionnement et des Biotechnologies(Lille,法国)赠送。纤维蛋白原来自Hyphen BioMed(Neuville-sur-Oise,法国)。人VWF和纤维蛋白原经纯化,且分别耗竭杂质纤维连接蛋白及纤维蛋白原和VWF(Legendre等人,2006)。马腱胶原蛋白来自Nycomed(Munich,Germany),且人纤维连接蛋白来自VWR(Fontenay-sous-Bois,法国)。野生型和突变的(V1316M2B型)重组VWF(rVWF)如另外(Ajzenberg等人,2000)所描述的获得。
抗体
单克隆抗体(MoAb)713,其在瑞斯托菌素的存在下阻断VWF与GPIbα的结合,由JP Girma博士(INSERM Unit770,Le法国)惠赠(Ajzenberg等人,2000)。多克隆抗糖萼素抗体,包含VWF结合位点的GPIbα胞外域,由MC Berndt博士(Monash University,Melboume,Australia)惠赠(Cramer等人,1991)。藻红蛋白(PE)偶联的抗-CD62P(P-选择蛋白)、异硫氰酸荧光素(FITC)-偶联的抗-CD41a(αIIb)、抗-CD42b(GPIbα)和非免疫抗体来自Beckman Coulter(Villepinte,法国)。抗-α和β-微管蛋白MoAb来自Amersham(Orsay,法国)。抗-β3整联蛋白P37MoAb由Gonzalez-Rodriguez博士(CSIC,Madrid,Spain)赠送(Calvete等人,1991)。抗-αIIbβ3Fab(C7E3)阿昔单抗来自Lilly(Suresnes,法国)。
人巨核细胞
用于该研究的人MK由从脐带血或骨髓分离的前体细胞培养。人骨髓样品(在臀部手术期间收获)和脐带血样品在经我们的学院伦理委员会(Assistance Publique desde Paris)同意后并按照赫尔辛基宣言(Declaration ofHelsinki)声明获得。人脐带血和骨髓CD34+细胞通过免疫磁性技术(StemCell Technologies,Grenoble,法国)如先前所述(Fichelson等人,1999)进行分离。细胞在补充有15%BIT9500血清替代物(StemCellTechnologies)和20nmol/L TPO肽激动剂AF13948(Genosys-Sigma,SaintQuentin Fallavier,法国)的Iscove′s Modified Dulbecco′s培养基(IMDM)(Gibco-Invitrogen,Cergy-Pontoise,法国)中生长。在培养的第一天一次性加入10ng/mL干细胞因子(SCF)(Amgen,Neuilly-sur-Seine,法国)。在TPO存在下培养10到16天的MK完全成熟,并且准备用于血小板生物发生(Patel等人,2005a)。通过流式细胞计显示细胞表达GPIb和αIIbβ3(60-70%阳性细胞)。对于剪切实验,在第10天和第16天之间使用细胞。
人血小板
从在之前的两周时间内没有摄取任何药物的健康个体中获得血液。将血液吸入pH5.8的15%(v/v)的酸性柠檬酸葡萄糖(acid citrate dextrose)(ACD)中。在三磷酸腺苷双磷酸酶(1U/mL)(Sigma)和ACD(1mL用于40mL)存在下从分离的富含血小板的血浆(PRP)制备经洗涤的血小板(Ajzenberg等人,2000)。简言之,在洗涤后,血小板再悬浮在含有0.15%牛血清蛋白(BSA)的pH7.5的Hepes缓冲液(10mmol/L Hepes(N-[2-羟乙基]哌嗪-N′-[乙磺酸])、136mmol/L NaCl、2.7mmol/L KCl和2mmol/L MgCl2)中。用电子颗粒计数器(Model Z1,Coulter Electronics,Margency,法国)计数血小板,且浓度调节到1.5×108血小板/mI。
小鼠巨核细胞和血小板
使用7-10周大的野生型C57BL/6J小鼠。动物饲养和处置遵循制度规定。巨核细胞扩增并从冲洗骨髓中分离。简言之,免疫磁性纯化的Lin细胞在存在TPO-肽的情况下在适当的巨核细胞分化培养基中生长。培养6天后,将成熟巨核细胞引入小型化流体室(Conant等人,2009)中并在固定的小鼠重组VWF(Marx等人,2008)上以高剪切率灌流。
灌流研究
使用由Maastricht Instrumentation获得的先前公开的流体室进行灌流研究(Legendre等人,2006)。流体室由雕刻在有机玻璃(plexiglas)块中的0.05mm高、29mm长和5mm宽的矩形空腔组成。该室的底部内衬有盖玻片,该盖玻片在4℃下用在Tris-缓冲盐水(TBS,25mmol/L Tris-HCl,pH7.4,150mmol/L NaCl)中稀释的VWF(20μg/mL)涂覆过夜。在一些实验中,测试另一种纯化的蛋白(纤维连接蛋白、纤维蛋白原、胶原蛋白或2B型-rVWF)。将在IMDM悬浮液(2mL)中的浓度为0.8-1×106细胞/mL的MK吸入5mL气密的玻璃微量注射器(Exmine,Ito Corporation,Fuji,日本)中,所述微量注射器通过延伸装置(extension set)(Steritex,Codan,Lensahn,德国)与室连接。通过电动泵施加225μL/min的流速,从而产生1800s-1的剪切率。细胞灌流10min,然后在相同剪切率下IMDM灌流10min。所有的实验在利用Minitüb加热系统(Minitüb Abfüll und Labortechnik,Tiefenbach,德国)维持的37℃下进行。在一些实验中,细胞或盖玻片在灌流前用抗体预孵育10min。在灌流结束时,用冰冷的甲醇固定盖玻片5min,用蒸馏水洗涤,干燥并染色。在室的出口处收集流出物用于进一步的研究,包括凝血酶活化。
视频显微镜系统
灌流室置于倒置显微镜(Axiovert135,Zeiss,德国)的夹片台上。用CCD照相机(Sony,Tokyo,日本)显现细胞,使用20×Hoffman调制对比度物镜。用连接到视频定时器(video timer)(VTG, Tokyo,日本)上的数字图像记录仪(Replay Software,Microvision Instruments,Evry,法国)进行连续记录。用Histolab或Videomet量化软件(Microvision Instruments)分析图像帧。
细胞活化研究和流式细胞分析。
收集流出细胞悬浮液,并在0.5mmol/L的EDTA存在下以1800rpm离心12min,沉淀再悬浮在IMDM中。等分试样在不存在或存在凝血酶(0.5U/mL)的情况下在37℃下孵育10min。悬浮液(100μL)中的细胞在室温下与FITC-抗-CD62P和PE-抗-CD41a(各5μL)或非免疫偶联的对照抗体一起孵育20min,然后加入300uL PBS。然后用FACSorr流式细胞计(Becton-Dickinson,Le Pont-de-Claix,法国)分析细胞;用血小板区域上的设置获得10000个结果,所述设置通过其在由未刺激的洗涤血小板在独立实验中确定的直角散射(right-angle scatter)(SSC)和前向角散射(FSC)图上的特征曲线确认。
共焦免疫荧光
将未激活的和凝血酶激活的细胞的等分试样(150μL)加入在静态条件下孵育30min的纤维蛋白原涂覆的玻璃Lab-Tek室载玻片上(Nunc,Rochester,NY)。除去非粘附细胞,并用PBS洗涤载玻片三次,然后用在PBS中的2%仲甲醛(Carlo Erba,Val de Reuil,法国)固定30分钟,并在4℃储存。对于免疫荧光染色,用0.1%Triton X-100的PBS溶液渗透细胞5min,然后用抗-β3P37MoAb(10μg/mL的PBS溶液)孵育30min,洗涤并用20μg/mL的与AlexaFluor488(Molecular Probes,Eugene,OR)偶联的二级抗体一起孵育。阴性对照使用来自小鼠腹水的相同蛋白浓度的纯化IgG。对于肌动蛋白标记,渗透的细胞在室温下用含有3%BSA的50μL的30nmol/L AlexaFluor546鬼笔环肽(Molecular Probes)的PBS溶液孵育30min。然后洗涤盖玻片两次并封固在含有TOTO3和Vectashield(Vector Laboratories,Peterborough,UK)的封片溶液(mounting solution)中。用Leica TCS SP2AOBS共焦显微镜分析AlexaFluor488/AlexaFluor546染色。
电子显微镜检查
流出细胞悬浮液直接在灌流室的出口处收集在戊二醛固定液(在0.1M、pH7.4的磷酸盐缓冲液中的最终浓度为1.5%)中,在通过各种基质后,然后如先前描述(Cramer等人,1997)的进行处理以用于电子显微镜检查。一些样品首先如上由凝血酶激活并进一步处理用于电子显微镜检查。在Jeol10-11电子显微镜(Jeol Ltd,Tokyo,日本)上进行检测。
统计学分析
我们使用Student's不配对t检验进行统计分析;P值<0.05被认为显著。误差棒代表平均值的标准误差(SEM)。
结果
MK与VWF的动力学相互作用
脐血成熟MK以不同的切变率在涂覆有纯化VWF的表面上灌流。在1800s-1下,MK形成与VWF的瞬时相互作用,从而减慢在VWF上滚动的MK的速度并逐渐导致深刻的形态变化,其最终导致血小板从MK脱落。血小板形成的不同阶段概述于图1A中。该过程从大致球形的MK开始,然后细胞外缘变形和伪足在粘附的MK上变得可见(阶段2)。然后在细胞体后面和/或前面发生沿着流动组织化的细胞质延伸(阶段3)。具有成串珠外观特征的中间膨大开始显现。延伸以21μm/min的速度迅速发生,从而比在静态条件下报告的高25倍(Patel等人,2005b)。有趣的是,破碎不仅发生在这些延伸的末端,还发生在膨大之间,特别在较薄的收缩段中,从而释放不同大小的碎片,且最经常是几个珠仍然连在一起,但是没有主细胞体(图1A)。最后,这些碎片进一步破碎成在稍后时间点期间清晰可见的更小的颗粒,为血小板的大小(图1A)。前血小板破碎在高剪切率条件下在15-20min内迅速发生。使用脐血源的MK或骨髓源的MK获得了相似的结果。相反,在不存在流动或在较低剪切率(<1000s-1)的情况下,在MK与VWF接触的20min时间内没有观察到前血小板和血小板释放。未成熟MK(培养少于9天)的剪切暴露不会发生转位和破碎。
与MK受体相互作用的VWF在血小板形成中的特异性
定义四个细胞种类以量化剪切暴露期间的形态变化:1)转位的MK;2)早期变形MK,丧失球形;3)晚期变形MK,具有前血小板延伸;和4)前血小板破碎和血小板释放(图1B,图a)。延伸细胞和前血小板和血小板的比例逐渐增加,在20min时达到细胞的27.6±4.9%。这种增加反映转位的细胞从5min时的28.2±6.5%降低到20min的3.6±1.6%,以及早期变形MK的降低(图1B,图b)。当测定暴露于高剪切率的骨髓MK的分布时,获得类似于用脐血MK描述的结果(图1B,图c)。在存在针对糖萼素或VWF的GPIb结合域的抗体的情况下,MK转位和后续步骤(包括前血小板形成和血小板释放)完全停止(图1B,图d和e),说明VWF-GPIb相互作用的至关重要。在αIIbβ3抑制剂阿昔单抗存在下,不具有前血小板的细胞(未变形和早期变形的MK)的比例随时间保持恒定,而前血小板延伸强烈减少,且血小板的形成几乎完全消除(图1B,图f)。这种前血小板形成的无效是由于与VWF表面的接触不良(数据未示出)。
由MK暴露于高剪切率诱导的血小板形成的表征
和在静态条件下在VWF上孵育细胞20min得到的对照(其中MK的形态相对于与VWF接触前的细胞保持不变)相比,施加于成熟MK的剪切力诱导特异性的形态变化(图2)。相反,MK灌流20min后涂覆有VWF的盖玻片的固定后染色显示,MK在其尖端延伸长的线状伪足,且沿轴形成珠状血小板样尖突起以及较大细胞质碎片(图2)。前血小板延伸需要对于α和β1微管蛋白染色的重叠微管的滑动(Patel等人,2005b)。我们发现MK延伸在与肌动蛋白不同的区域中由抗微管蛋白抗体染色,证实这些延伸显示与由Italiano等人(Italiano等人,2003;Italiano等人,1999;Patel等人,2005a;Patel等人,2005b)的工作在前血小板形成MK中确认的类似的染色;微管蛋白标记跟随延伸的MK中的前血小板轴并在球形MK上、前血小板上和血小板上显示环形图案(图3B)。微管在剪切诱导的前血小板形成中的重要性由诺考达唑(nocodazole)(微管装配抑制剂)的预孵育证实,其完全阻止流动条件下的MK延伸和前血小板形成(数据未显示)。为证明诺考达唑不是作用于细胞悬浮液中的预形成的血小板,在延伸后加入微管抑制剂;实时可见的效应显示,当在该过程发生之后添加时,诺考达唑可以逆转剪切诱导的延伸。逆转的特异性由微管蛋白染色证实。有趣的是,加入诺考达唑25min后,在前血小板和血小板中观察到微管蛋白不再组织化,从而表明延伸过程不同于系留(图3C)。事实上,膜系留是在流动影响下从小的、局部化粘附接触延伸的动态结构,其由VWF-GP Ib相互作用诱导,但它们不显示任何微管蛋白染色(Dopheide等人,2002)。
电子显微镜检查显示从细胞核心延伸的、含有平行微管束的长的细胞质轴,且有时被胞质细胞器充满(图4,图a)。具有致密异染色质的核叶位于细胞的一极,偶尔观察到裸核(图4,图b)。没有细胞核的大的细胞质碎片大致是球状、哑铃状或延伸有细长的末端(图4,图c和d)。也可见血小板样碎片(图4,图e)。
高剪切率和VWF加速前血小板形成
最近表明,粘性蛋白参与前血小板形成的调节,特别是在静态条件下将MK平铺在VWF上后16小时发生的前血小板形成(Balduini等人,2008)。如图5A所示,我们获得了类似的发现。相反,在高剪切率存在下,该过程非常迅速地发生,因为在20min内,70%的脐血MK和80%的骨髓MK形成前血小板。该作用取决于剪切率的水平(600-2400s-1)以及细胞浓度(0.5-2×106/mL)。内皮细胞含有高分子量的VWF多聚体,其以低速率组成型释放并在炎性状态下增加(Rondaij等人,2006)。因此我们评估从内皮细胞基质中释放的VWF是否支持MK粘附和前血小板形成。MK滚动并粘附在未激活HUVEC上,但前血小板形成发生在激活的HUVEC上(图5B)。血小板形成也发生在位于内皮细胞层之下且在去除这些内皮细胞时呈现的内皮细胞基质上。另外,我们研究了在涂覆有突变型2B-rVWF的表面上灌流的MK的前血小板形成。我们选择V1316M置换(一个明确的突变),其特征在于VWF与血小板GPIb的结合增强,具有温和的血小板减少症和巨血小板(Ajzenberg等人,2000;Miura等人,2000)。我们发现在2B型-rVWF上MK转位的平均速度(4.6±0.3μm/s)远远低于在野生型(wt)-rVWF上的平均速度(33.6±2.5μm/s,p<0.0001)。通过MK暴露于高剪切率而形成前血小板的所有步骤在2B-rVWF上比在wt-rVWF上慢,导致血小板生成减少以及前血小板集聚增加(未显示数据)。最后,为进一步确立粘性表面在调节前血小板形成中的特异性,我们调查了在高剪切率下由MK在纤维蛋白原、胶原蛋白或纤维连接蛋白上灌流的前血小板形成水平。在这些表面上没有形成前血小板;唯一的变化是在纤维蛋白原上轻微的早期MK变形(图5C)。
由MK剪切暴露产生的血小板是功能性的
αIIbβ3整联蛋白介导的血小板对纤维蛋白原的粘附在不存在活化的情况下发生,并在凝血酶激活该受体时增强。为证实由MK剪切暴露产生的血小板在结构上和功能上类似于血液血小板,在对于纤维蛋白原的静态粘附试验中比较流通中收集的细胞和分离的血液血小板(Mazharian等人,2007)。在不存在凝血酶的情况下细胞牢固地粘附于纤维蛋白原,说明αIIbβ3在剪切诱导的血小板和前血小板中是功能性的。非激活成分显示弥漫的肌动蛋白染色和αIIbβ3膜定化。凝血酶活化后,肌动蛋白微丝在有核和无核成分中重新组织化,显示板状伪足(lamellipod)和丝状伪足形成,以及整联蛋白膜染色(图6)。这些由剪切暴露产生的MK源的血小板显示与在未暴露于剪切的分离试验中制备的洗涤血液血小板类似的细胞骨架组织(图6)。
剪切暴露细胞的流式细胞计分析根据其大小显示三种CD41阳性细胞群。较小尺寸的群体与血小板重叠,而中间群体对应于较大的成分,包括前血小板和裸核(图7)。因此,与从外周血液分离的血小板比较,由暴露于剪切生成的血小板群体包括较高比例的中间尺寸的血小板。剪切的MK属于与未剪切的MK相同的大尺寸群体。响应于凝血酶刺激,在剪切产生的血小板的表面上观察到P-选择蛋白(CD62P)的上调(11.8±1.8%,与未活化血小板的6.2±0.9%比较,p=0.0005)。与分离血液血小板的相似性由超结构数据证实(图8)。不存在凝血酶时,血小板样碎片显示平滑表面、若干分散在细胞质中的α粒以及离散的表面连接微管系统(SCCS)(图8)。凝血酶活化后,这些细胞显示典型的激活血小板的形态变化,即球形、表面伪足、SCCS的集中的和扩大的潴泡内的致密材料、无细胞质粒形成以及中央的微纤丝束(图8)。最后,为证实血小板大小的成分由剪切暴露产生,且不是简单地在成熟MK的悬浮液中产生,通过裸核的存在比较在流体室中灌流前和后的流出物。暴露于剪切的细胞的非粘附流通比在静态条件下的细胞(缺乏裸核)包含更多的裸核(未显示数据)。
小鼠巨核细胞和血小板
使小鼠成熟巨核细胞在小鼠VWF上暴露于高剪切率导致有效的血小板形成。在一系列实验中,从6-8只动物中提取骨髓源的细胞使得生成至少6-12×106个成熟巨核细胞。这些细胞在90分钟内暴露于1800s-1的剪切率,并完全转化为与血小板尺寸相当的小成分。由共焦免疫荧光表征小鼠血小板显示了血小板尺寸的成分中特定的微管蛋白环。在凝集计中通过标准技术确认响应于这些血小板的激动剂的聚集特性。
将本发明血小板注射给血小板减少症小鼠挽救出血表型
该实验旨在通过灌注如上所述通过高剪切力产生的供体小鼠血小板挽救血小板减少症受体小鼠的出血表型。
受体小鼠经由尾部静脉注射通过来自汇集的供体小鼠的骨髓成熟巨核细胞的剪切暴露获得的血小板。血小板减少症小鼠通过施用低浓度的抗GPIIb-IIIa抗体(Nieswandt等人,2000)获得。在典型实验中,我们通过注射2-3次抗-GPIIb抗体使得循环血小板减少80%。在给血小板减少症小鼠注射前,荧光标记这些血小板以用于检测和清除研究。测定输注小鼠的血小板计数、自发出血和出血时间的改正。这些循环的碎片是活性血小板,因为它们增加血小板数目并在诱导出血的体内模型中改正失血。我们使用导致限于炎症区域的大出血的刺激性接触性皮炎模型(Goerge等人,2008)。在施用剪切产生的血小板之前和之后比较血小板减少症小鼠的皮肤出血。也通过体内荧光显微术利用氯化铁损伤的血栓症模型体内评估新形成血小板的功能(Denis等人,1998)。该方法提供血栓形成过程的不同阶段的详细信息,包括单血小板粘附、血栓生长的速率、血栓稳定性、栓子尺寸和堵塞血管的时间(Denis and Wagner,2007)。我们比较了对血小板减少症小鼠施用血小板之前和之后的实时血栓形成和血小板在血栓中的结合。
结论
综上所述,在流动条件下成熟MK与VWF的相互作用促进人/小鼠前血小板形成的发现代表了了解血小板形成方面的重大突破,因此这可以发生在毛细血管或小动脉中。具有血小板生成减少的疾病的调查将极大地有利于更好地理解血小板从巨核细胞的脱落。最后,本发明人的结果提供了一种用于诊断或治疗目的的制备血小板的方法。

Claims (8)
1.用于制备血小板的体外方法,包括使成熟巨核细胞的悬浮液在涂覆有血管性血友病因子(VWF)或其结合GPIb的片段或变体或类似物的固相上经历剪切率至少为600s-1的流动。
2.根据权利要求1的方法,其中所述VWF的片段或变体或类似物选自下组:VWF的52/48-kDa胰蛋白酶片段;金黄色葡萄球菌V-8蛋白酶消化的VWF;用于治疗用途的VWF浓缩物和引起2N型血管性血友病的VWF突变体。
3.通过权利要求1或2定义的方法可获得的血小板。
4.根据权利要求3的血小板,其用于治疗。
5.根据权利要求3或4的血小板,用于治疗血小板计数减少障碍。
6.根据权利要求5的血小板,其中所述血小板计数减少障碍选自下组:自身免疫性血小板减少症、与生成减少有关的血小板减少症和与血小板破坏的增加有关的血小板减少症。
7.根据权利要求3的血小板用于诊断目的的用途。
8.用于制备血小板的装置,包括流体室,所述流体室包括涂覆有VWF或其结合GPIb的片段或变体或类似物的固相。
