Abstract
本发明涉及中医成药。尤其是一种治疗中晚期肝癌的中药水丸制剂。本发明采用如下中药组方配伍:人参、黄芪、灵芝、龙葵、猪苓、女贞子、白芷、白花蛇舌草、半枝莲、茯苓、蚤休、蟾蜍、壁虎、枸杞、白术、鳖甲、水红花子、鹿角胶、全蝎共十九味中药。使用本发明对症治疗,可提高疗效,改善生活质量,延长生存期,本发明可起到保护肝细胞,减轻化疗毒副作用,提高免疫功能,抑杀癌瘤细胞作用。
Description
技术领域:本发明涉及中医成药,一种治疗中晚期肝癌的中药水丸制剂。
背景技术:肝癌起病隐匿,不易早期发现,一经有了自觉症状,病程已是中晚期,往往失去了治疗时机。同时,肝癌的病因病机复杂,病程进展快,极具侵袭性,明确诊断时已出现转移,很难治疗。
发明内容:对于那些术后复发,再切除再复发,以复发与转移的预防等问题,对于那些不能接受手术治疗,介入治疗或放化疗的中晚期肝癌患者,以及术后体质的恢复,放化疗后毒副反应的治疗等问题,西医则显得束手无策,这时中医的对症治疗则有极大优势。因此,本发明采用如下中药组方配伍(以下简称“龙鳖灵芝丸”):人参、黄芪、灵芝、龙葵、猪苓、女贞子、白芷、白花蛇舌草、半枝莲、茯苓、蚤休、蟾蜍、壁虎、枸杞、白术、鳖甲、水红花子、鹿角胶、全蝎共十九味中药。本方中人参、黄芪、灵芝、枸杞提高机体抗病能力,提高机体免疫功能来防治肿瘤,本发明以免疫治疗为主要手段,枸杞、猪苓所含多糖能诱导LKA细胞活性,能使巨噬细胞NK生成增加。女贞子、龙葵不但能抗肿瘤,而且治疗白细胞减少症。白花蛇舌草、半枝莲、蚤休清热解毒,抗癌抑癌,现代药理证实其尚有调整免疫功能。灵芝益气强状,养心安神,主治虚劳赢弱,并有促进T淋巴细胞生成作用,同时也能促进巨噬细胞生成作用,鳖甲坚阴散结,消癥化积,蜈蚣、全蝎熄风镇痉,攻毒散结,通络止痛,其走窜之力最速,内而脏腑外而经络。凡气血凝聚之处皆能开之,性有微毒,而转善解毒。凡一切疮疡诸毒皆能消之。蟾蜍、壁虎拔毒散肿,破癥结,利水湿,化痰凝,消癌肿。全方共奏扶正祛邪,抗癌消瘤之功。
单纯化疗对肝癌疗效很差,尤其对于有肝硬变背景的中晚期肝癌患者,对化疗耐受量小,疗效难以提高。在化疗时联合使用本发明龙鳖灵芝丸对症治疗。可提高疗效,改善生活质量。延长生存期,本发明可起到保护肝细胞,减轻化疗毒副反应,提高免疫功能,协助抑杀癌瘤细胞的作用。
具体实施方式:本发明为一种动物药与中草药制备剂,下列十九味中药材按重量比组成:蟾蜍6g、壁虎6g、蜈蚣6g、全蝎6g、鳖甲15g、鹿角胶10g、人参20g、黄芪40g、灵芝30g、枸杞20g、猪苓20g、女贞子30g、白花蛇舌草30g、蚤休20g、龙葵30g、半枝莲15g、白术30g、茯苓20g、水红花子15g。
制备方法与使用:蟾蜍、壁虎、蜈蚣、全蝎、鳖甲、鹿角胶晒干或烘干后碾碎成粉末。人参、黄芪、灵芝、枸杞、猪苓、女贞子、白花蛇舌草、蚤休、龙葵、半枝莲、白术、茯苓、水红花子共入锅用水煮熬2小时,滤取药汁与动物类药粉搅拌均匀后蜥蜴干或烘干。第2次,第3次各加水煮熬1小时,分别滤取药汁与药粉搅拌均匀后晒干或烘干。
将经过3次药汁搅拌均匀晒干或烘干后的药粉进一步研细过100目筛,水泛为丸,如绿豆大,晒干或烘干即制备成“龙鳖灵芝丸”。
成人每服3克,日服3次,温开水送下,小儿老年体弱者酌减。
本实施例适用于中晚期肝癌治疗。
典型病例:
赵某某,男,75岁。该患者因右肋不适隐痛胀痛,时而频作。于2000年10月经某大医院做彩超检查,示肝内占位性病变,又经CT扫描诊断为肝癌,后因尿少双下肢浮肿,邀余诊治。
初诊,右肋下胀痛,疲乏无力,食欲不振,嗳气,下肢微肿,尿少,腹胀,脉弦细,苔薄黄,肝右肋下约3cm质硬。中医辩证:气滞血瘀,夹湿毒内蕴。
治法:疏肝理气,活血化瘀,佐以利湿清毒。
方药:1、投以“龙鳖灵芝丸”3克/次,3次/日。
2、辅以“紫胡12克,白芍20克,红花15克,桃仁20克,莪术15克,薏苡仁20克,茯苓皮20克,丹参20克,厚卜20克”以上方煎剂服用,日二次,早晚分服,每日一剂。
以上方为主加减服用半年多,病情稳定,双下肢浮肿消失,且尿量正常,三年后随访该老人仍健在。
孙某某,男,50岁,于2000年8月感觉腹胀,右肋下痛疼,伴食少,消瘦,日渐加重,在民间服用偏方治疗一年未效,于2002年6月来诊,所持某医院B超示肝右叶肝癌声像,肝核素扫描示肝右叶占位。HBSAg阳性,AFP阳性。查体见:腹胀,右肋下及腰背部痛疼2周,伴食少纳呆,倦怠嗜卧,头晕目眩,舌质淡红,舌苔薄白,脉弦细,右肋下肝大5cm,剑下4cm质硬,综上,中医辩证:正虚邪盛,邪毒乘虚而入,致肝脏功能失调,气滞血瘀,痰结湿聚、阻塞经络,不通则痛,瘀久则成积块。辩证:肝郁脾虚型。治法:养血舒肝,益气健脾,消肿散结。方药:(1)当归15克,白芍20克,柴胡15克,白术20克,干姜15克,大枣7枚,三棱15克,文术15克,薄荷10克,甘草10克,日一剂,早晚分服。(2)龙鳖灵芝丸3克/次,3次/日。
以上方剂连服120剂,症状明显好转,肝右肋下1cm,剑下7.3cm,右肋下及腰背部痛疼减轻。随访存活5年后辞世,带瘤生存5年。
伍某,男,40岁,于2005年11月在某医院诊为肝癌,行肝局部切除加静脉化疗。病理诊断为肝梁状细胞癌,II期伴肝硬化,手术后右肋隐痛,胃纳不佳,时有腹胀,舌苔厚腻,脉弦细。中医辩证:脾虚湿盛,湿浊困脾。治法:益气健脾,清热利湿。方药:(1)党参20克,佛手20克,八月扎20克,白术30克,茯苓20克,陈皮15克,枳壳10克,苦参20克,薏苡仁20克,厚卜20克,土茯苓30克,仙鹤草15克,败酱草20克,田基黄15克,莪术15克,日一剂,早晚分服。(2)龙鳖灵芝丸3克/次,3次/日。
该患长期间歇服药,至今已存活5年,2010年经CT、B超复查,未见肝癌复发。
曲某某,男,48岁,患者因右上腹部肿块伴刺痛。于2000年12月到当地市立医院同位素扫描发现肝内占位病变,即转来就诊,查体:上腹部隆起,肝上界右锁骨中线第6肋间,肋下13.5cm,剑下6cm,质硬,肝表面触诊结节状,舌红绛,苔黄花剥,脉弦数,AFP阴性,谷氨酰转肽酶224u超声波示肝区可见丛状波,诊断为原发性肝癌。中医辩症:肝瘀脾虚。治法:健脾理气,化瘀软坚。方药:(1)龙鳖灵芝丸内服,3克/次,3次/日。(2)丹参15克、苏木15克、牡蛎15克、鼠妇15克、茯苓15克、香附20克、柴胡15克、陈皮15克、穿山甲10克,桃仁10克,沉香末3克(冲服)上方日一剂,早晚分服。治疗二月后,肝区刺痛消失,肝脏无增大。遂只服龙鳖灵芝丸,以固后效。翠年后,经某医院复查,肿块大小基本无变化。因病人一般情况良好,患者本人及家属认为不是肝癌,遂停止服药,同年年底,终因旧病复发病情恶化不幸辞世。
本发明“龙鳖灵芝丸”通过26例中晚期肝癌患者对症加减服用,有效者24例,总有效率93%,远期随访26例患者生存一年以内者6例,占23%,1至3年的10例,占38%,3至5年以上的8例,占30%。
Claims (1)
1.本发明一种治疗中晚期肝癌的中药水丸制剂。其特征在于组方配伍和各中药重量:蟾蜍6g、壁虎6g、蜈蚣6g、全蝎6g、鳖甲15g、鹿角胶10g、人参20g、黄芪40g、灵芝30g、枸杞20g、猪苓20g、女贞子30g、白花蛇舌草30g、蚤休20g、龙葵30g、半枝莲15g、白术30g、茯苓20g、水红花子15g。
