背景技术
恶性神经胶质瘤是最常见的原发性脑部肿瘤,并且与非常差的预后有关(Wrensch等,2002)。据推测,神经胶质瘤源自于内源性神经胶质祖细胞或神经干细胞(Canoll和Goldman,2008),它们与所述内源性神经胶质祖细胞或神经干细胞都具有沿着白质纤维束(whiter matter tract)和血管周围空间和软膜下空间转移的能力(Louis,2006)。因此,恶性神经胶质瘤是高度浸润性肿瘤,通过手术将它们予以完全切除是不可行的。观察到80%的恶性神经胶质瘤在2cm至3cm的初始肿瘤块内复发,由此使在充分治疗浸润性肿瘤细胞处的常规治疗方式的局限性受到高度关注(Hess等,1194)。
单纯疱疹病毒(HSV-1)是一种大型天然嗜神经双链DNA病毒,该病毒正在被积极开发成有用的复制选择性(溶瘤)和复制缺陷型基因治疗载体(Bowers等,2003)。到目前为止,两个复制选择性病毒构建体已经达到了恶性神经胶质瘤患者的临床试验阶段(Rampling等,2000;Markert等,2000;Papanastassiou等,2002;Harrow等,2004)。这些病毒被命名为G207和HSV1716,它们在γι34.5基因的两个拷贝中都具有无效突变。这个基因的产物在增强HSV-1感染神经元并克服宿主细胞对病毒感染的反应的能力中是关键的(He等,1997)。此外,γι34.5基因的无效突变赋予这些载体在肿瘤细胞中选择性复制的能力(Shah等,2003)。

根据定义,Ⅳ级神经胶质瘤由组织坏死面积来表征(World HealthOrganisation(世界卫生组织),2007年)。结果,利用能够在活的恶性神经胶质瘤细胞内复制的复制选择性病毒载体对坏死的原发性肿瘤块的直接接种不太可能有效地治疗原发性肿瘤块,或者更重要的是,不太可能有效地治疗浸润性肿瘤细胞。另外,原发性肿瘤块往往适合进行手术切除,使得肿瘤内注射复制选择性载体变得不必要。事实上,Harrow等(2004)对在经历恶性神经胶质瘤复发或者新诊断出的恶性神经胶质瘤的切除术的患者中的HSV1716外周肿瘤注射进行了1/II期研究。该研究证明了这种方法是安全的,尽管对于这种方法而言有效性显然是关键的,但是必须对病毒的分布进行优化以利于尽可能多的浸润性肿瘤细胞的转导。
对流增强输送法(convection enhanced delivery,CED)涉及使用细导管和受精确控制的输注速度以利用整体流(bulk-flow)使治疗剂直接分布到脑部细胞外空间中,这可能沿着与神经胶质瘤细胞能够转移相同的胞外路径进行。与依赖于输注来实现充分的药物分布的药物输送技术(如卡莫司汀浸渍可生物降解性聚合物)不同,采用CED可以将药物均匀地分布在脑部的大的体积上,而不管治疗剂的分子尺寸如何(Morrison等,1994)。因此,这是一个将病毒载体介导的基因治疗应用于患有恶性神经胶质瘤的患者的脑部的理想技术。
HSV-1载体的直径为120nm至300nm(Jacobs等,1999),而平均而言,脑部细胞外空间的直径为38nm至64nm(Thorne和Nicholson,2006)。显然,这有可能使通过CED施用基于HSV-1的载体无法实现。因此,在这项研究中,已经检查了通过CED将复制选择性HSV-1病毒构建体分布在灰质和白质中所进行的分布,并且也评价了增强病毒载体分布的各种策略。
然而,除了上述的临床试验(6-9),HSV载体已通过立体定位注射到正常小鼠(17-19)、大鼠(20-26)和灵长动物脑部(20-28)、高分级神经胶质瘤的动物模型(29-35)、VII型粘多糖病(36)、GM2神经节苷脂贮积症(37)和帕金森氏病(37-39)中来进行施用,以及通过CED施用到神经胶质瘤大鼠模型(40)中来进行施用。鉴于这些大量的研究,令人惊讶的是,迄今为止还没有尝试对将这些载体直接输送到脑部中所进行的输送进行系统性的评价和优化。因此,在这项研究中,已经检查了由CED将复制选择性HSV-1病毒构建体在灰质和白质中的分布,并且评价了增强病毒载体分布的各种策略。
发明内容
根据所述研究,本发明人已经鉴定了可用于优化将治疗剂输送至白质尤其是通过对流增强输送法将治疗剂输送至白质中所进行的输送的组合物和化合物。这对于基因治疗载体的输送是特别有用的。
本发明的第一方面提供了一种药学组合物,所述药学组合物包含治疗剂和白蛋白或其功能有效性片段。
所述组合物优选包含用于治疗神经系统疾病的治疗剂。所述治疗剂优选用于治疗脑部或者脊髓的疾病,尤其是用于治疗脑部的疾病。所述疾病可以是癌症,特别是白质的癌症,尤其是神经胶质瘤。或者,所述疾病可以是任何适当的其他神经系统疾病,特别是白质疾病,例如多发性硬化症。
所述治疗剂可以是用于治疗神经系统疾病的任何试剂,例如基因治疗剂,特别是基因治疗载体。或者,所述治疗剂可以是一种药学试剂,例如神经营养因子,特别是神经胶质细胞源性神经营养因子(GDNF);抗体或其片段;免疫抑制剂;免疫调节剂,特别是芬戈莫德;细胞因子,尤其是干扰素,例如干扰素1α或β;或抗炎药。
基因治疗载体是能够被用来将遗传物质导入到细胞中的任意载体。基因治疗,如所熟知的那样,是使用遗传物质来调节个体细胞中的基因或者将遗传物质添加到个体细胞中的基因以治疗疾病。要被导入的遗传物质可以是任何适当的遗传物质,包括DNA和RNA。所述遗传物质可以以任何已知的方式,例如基因置换、基因敲除、促生存基因治疗和细胞自杀疗法而被用来治疗疾病。
所述基因治疗载体可以是适于将基因治疗应用于受试者的任何载体,包括例如病毒载体。可以使用任何合适的病毒载体,如单纯疱疹病毒载体,特别是HSV-1;腺病毒载体或慢病毒载体。优选的是,所述病毒载体是大型载体,直径至少为l00nm。进一步优选的是,所述载体结合到肝素结合受体。
所述组合物另外包含白蛋白或它们的功能性片段。如前文所述,所述组合物可被用于基因治疗。白蛋白首先被包含在所述组合物中以打开细胞之间的空间,从而能够使治疗剂,尤其是大型病毒载体,更容易地在细胞之间移动。白蛋白还阻断细胞上的肝素受体,细胞上的肝素受体是低特异性结合受体,其结合至各种蛋白,包括肝素、白蛋白HSV载体。通过利用白蛋白阻断肝素受体,减少了HSV与受体的结合,从而增加了可用于转导的HSV比例。白蛋白可被另一种肝素受体结合剂,如肝素本身所代替,但是白蛋白是优选的。白蛋白的一种功能性片段是以至少75%的作为全长蛋白的结合效力结合至肝素受体的片段。
所述组合物优选还包含脑脊液(CSF),尤其是人造脑脊液。人造脑脊液在本领域中是公知的,是模拟天然CSF,特别是在其盐含量方面模拟天然CSF的一种流体。优选的是,所述组合物包含浓度类似于天然CSF中所见到的浓度的NaCl,也就是说,所述浓度优选为天然CSF中所见到的浓度的15%以内,更优选在10%以内。优选的是,所述组合物包含浓度类似于在天然CSF中所见到的浓度的NaHCO3,也就是说,所述浓度优选为天然CSF中所见到的浓度的15%以内,更优选在10%以内。优选的是,所述组合物包含浓度类似于在天然CSF中所见到的浓度的KCl,也就是说,所述浓度优选为天然CSF中所见到的浓度的15%以内,更优选在10%以内。优选的是,所述组合物包含浓度类似于在天然CSF中所见到的浓度的NaH2PO4,也就是说,所述浓度优选为天然CSF中所见到的浓度的15%以内,更优选在10%以内。优选的是,所述组合物包含浓度类似于在天然CSF中所见到的浓度的MgCl2,也就是说,所述浓度优选为天然CSF中所见到的浓度的15%以内,更优选在10%以内。优选的是,所述组合物包含浓度类似于在天然CSF中所见到的浓度的葡萄糖,也就是说,所述浓度优选为天然CSF中所见到的浓度的15%以内,更优选在10%以内。或者,人造CSF可以省去葡萄糖,以减少细菌在用以将所述组合物施用于受试者的任意导管中生长的可能性。
所述组合物可以进一步包含其他活性剂。本发明的药学组合物还可以包含任何药学上可接受的载体、佐剂或赋形剂。可以用于所述药学组合物的所述药学上可接受的载体、佐剂和赋形剂包括,但不限于:离子交换剂、氧化铝、硬脂酸铝、卵磷脂、血清蛋白(如人血清白蛋白)、缓冲物质(如磷酸盐)、甘氨酸、山梨酸、山梨酸钾、饱和植物脂肪酸的部分甘油酯混合物、水、盐或电解质(如硫酸鱼精蛋白)、磷酸氢二钠、磷酸氢钾、氯化钠、锌盐、胶体二氧化硅、三硅酸镁、聚乙烯吡咯烷酮、纤维素基物质、聚乙二醇、羧甲基纤维素钠、聚丙烯酸酯、蜡、聚乙烯-聚氧丙烯嵌段聚合物、聚乙二醇和羊毛脂。
本发明的药学组合物可以通过任何适当的途径施用,但是优选通过注射施用,特别是通过神经导管(neurocatheter)施用,尤其是通过对流增强输送法施用。所述药学组合物可以含有任何常规的非毒性的药学上可接受的载体、佐剂或赋形剂。
所述药学组合物可以是无菌可注射制剂的形式,例如为无菌可注射水性或油性悬浮液。这种悬浮液可以根据本领域中已知的技术,使用合适的分散剂或润湿剂(诸如Tween 80)和悬浮剂配制。所述无菌可注射制剂也可以是在无毒的肠胃外可接受的稀释剂或溶剂中的无菌可注射溶液或悬浮液,例如,作为在1,3-丁二醇中的溶液。其中,可以采用的可接受的赋形剂和溶剂是甘露糖醇、水、林格氏溶液(Ringer’s solution)和等渗氯化钠溶液。此外,无菌的固定油通常被用作溶剂或悬浮介质。为了这个目的,任何无味的固定油,包括合成的单甘油酯或二甘油酯,也可以采用。脂肪酸,如油酸及其甘油酯衍生物可用于可注射制剂中,如天然的药学上可接受的油,例如橄榄油或蓖麻油,尤其是它们的聚氧乙基化形式。这些油溶液或悬浮液也可含有长链醇稀释剂或分散剂,如Ph.Helv或类似的醇。

优选的是,所述白蛋白在等渗溶液中,特别是在人造CSF的溶液中。人造CSF优选如在本发明的第一方面中所定义的那样。
优选的是,所述受试者为哺乳动物,优选为灵长类动物,特别是人类。所述受试者优选患有癌症,特别是患有脑部癌症,尤其是患有神经胶质瘤。
优选的是,所述组合物、白蛋白和基因治疗载体用于施用至脑部,特别是通过输注而施用于脑部,最优选的是通过细导管施用于脑部。特别是,所述组合物、白蛋白和基因治疗载体用于通过对流增强输送法进行施用。

在采用1%BSA对组织进行预输注之后,以低速率(0.5μ1/分钟)、高速率(2.5μ1/分钟)和肝素一起输注HSV-1。在24小时、48小时和72小时时,EGFP阳性细胞的数量:0.5μ1/分钟(p=0.012),2.5μ1/分钟(p=0.013)(a)和EGFP阳性细胞的分布体积:0.5μ1/分钟(p=0.019),2.5μ1/分钟(p=0.011)(b)。在输注到纹状体中之后24小时、48小时和72小时时,EGFP阳性细胞的数量(c)和EGFP阳性细胞的分布体积(d)。P值用于与标准缓冲液中的载体输注进行比较。

序言
恶性神经胶质瘤是最常见的原发性脑部肿瘤并且几乎总是不可治愈。其中关键的原因包括这些肿瘤的高度浸润性、固有的化学抗性以及与在没有造成毒性的情况下实现脑部化学疗法的治疗浓度有关的难度。利用对流增强输送法实现溶瘤病毒载体的脑实质内直接施用是一个有希望的新的治疗策略。但是没有任何证据表明,大到像HSV-1这样的溶瘤病毒可以通过CED施用。在这项研究中,详细评价了在小型(大鼠)和大型(猪)的动物模型的灰质和白质中施用HSV-1病毒载体的能力。
使用与CED兼容的输注参数将表达EGFP报告基因的基于HSV-1的载体输注到大鼠的纹状体和胼胝体以及猪的辐射冠中。使用体视学方法确定每次输注后被转导的细胞的分布体积和数量。使用免疫组织化学法来确定载体的转导向性并评价是否有免疫细胞浸润到脑部中。评价了改善载体分布的策略,包括使用高流速输注、联合输注肝素或使用等渗白蛋白溶液对组织进行预输注。
以低输注速度(0.5μ1/分钟)和高输注速度(2.5μ1/分钟)将HSV-1输注到大鼠灰质和白质中都导致严重的组织损伤和可忽略的细胞转导。为使非特异性载体结合最小化而以低浓度肝素联合输注导致严重出血。使用等渗白蛋白溶液对组织进行预输注促进白质中的广泛的载体分布和细胞转导,但是不能改善灰质中的载体分布。在猪脑部中采用这种方法导致广泛的载体分布和星形细胞的大量转导,激活的小胶质细胞位于远离输注部位的皮层和血管周围空间。在大鼠脑部中,EGFP表达在载体施用后48小时后达到峰值,并且与ED1阳性小胶质细胞和CD8阳性T细胞的浸润为特征的强烈免疫反应有关。
基于HSV-1的病毒载体直接输注到脑部中导致最小的载体分布、可以忽略的细胞转导和严重的损伤。载体施用前的等渗溶液的组织预输注是用于实现广泛的载体分布的实用且高效的技术,在采用基于HSV-1的病毒载体的当前和未来的临床试验中应予以采用。
方法
载体
在ICP4和ICP27基因中带有无效突变并且在CMV启动子的调控下表达增强的绿色荧光蛋白(EGFP)的HSV-1病毒构建体由Biovex友情提供。
载体输注
所有程序均按照UK Home Office animal velfare regulations进行并且得到Home Office的适当许可
大鼠输注装置和程序
由外部直径为220μm且内部直径为150μm的多段熔融二氧化硅构造急性输注套管。这些段熔融二氧化硅经由内部制造(made in-house)的连接器件连接至10μl的Hamilton注射器,所述连接器件用来在Hamilton注射器的腔室与熔融二氧化硅的内腔之间形成密封,并且引导熔融二氧化硅套管从Hamilton注射器通过硬脑膜进入脑部。经由这个连接器件与所述套管附接的Hamilton注射器然后安装在附接到立体定位框架(Stoelting Co,Wood Dale,ILUSA)的输注泵(World Precision Instruments Inc.,Sarasota,FL,USA)中,其中大鼠被固定住。为了插入套管,在所述立体定位框架中降低整个泵/注射器/连接器/套管结构直至达到目标深度。
雄性Wistar大鼠(B&K,英国)分组圈养并在实验程序之前允许它们适应环境。雄性大鼠体重225g至275g,采用腹膜内剂量的氯胺酮和甲苯噻嗪进行麻醉,并放置在立体定位框架(Stoelting Co,Wood Dale,IL USA)中。在眉间和枕部之间形成线性切口并使颅骨外露。在前囱侧部2.75mm且在前部0.5mm形成直径为约2mm的钻孔,并且在靶向纹状体的情况下套管插入至硬脑膜下方5mm的深度,而在靶向胼胝体的情况下套管插入至2mm的深度。所有套管被插入到脑部之前都采用载体预先引发(pre-prime)。进行每次尝试以确保输注套管中没有存在空气泡。所有载体输注都是4μl,1×107pfu/ml浓度。根据所使用的输注参数将动物进一步分为4个组(表1)。通过将病毒输注液与2μl的5000IU/ml肝素(10IU肝素)混合来实现肝素联合输注。在无菌盐水中混合牛血清白蛋白(BSA;Sigma,UK)。通过将4μl等渗的1%BSA输注到纹状体和胼胝体中来实现组织的BSA预输注,然后马上进行载体输注。输注结束后,套管原位保留5分钟,然后以1mm/分钟的速度移除。然后使用4/0Vicryl封闭伤口,施用肌肉内注射剂量的丁丙诺啡(30μg/kg)),并使用腹膜内剂量(0.1mg/kg)的盐酸阿替美唑(Antisedan;200μg/kg;Pfizer,Kent,UK)解除麻醉。
在每个组内,在24小时、48小时、72小时或96小时的时间点,在深度全身麻醉下采用100ml磷酸盐缓冲液然后再用100ml的4%多聚甲醛(pH7.4)灌注固定来处死动物。然后将大脑从颅骨中取出,并放置在4%的多聚甲醛(pH7.4)中48小时,然后在切片之前于30%的蔗糖中冷冻保护。
猪输注装置和程序
对一头体重为45kg的Landrace大白猪施用肌肉内剂量的氯胺酮(0.1mg/kg体重)。然后使用异氟醚(2-5%)诱导并维持全身麻醉,并且使用有囊气管导管(cuffed endotracheal tube)对动物实施管插法。使用放置在耳静脉中的套管形成静脉通道并以250ml/小时的速率输注生理盐水。
使用定制的装置实现猪头固定,所述装置具有双侧MRI兼容性双边颧螺丝、可成型的腭托盘和吻部带(snout strap)。所有材料完全是MR兼容性的,以防止形成矫作物图像。在牢固地固定猪头部后,将基准弧放在了动物的头部。然后将Flex-L圈附接至头部的侧面,将动物转移到1.5T MRI扫描仪(Intera,Phillips)。使用Pathfinder(Prosurgics,UK)立体定位机械手和有关软件进行立体定位手术计划和程序。简而言之,这种立体定位手臂的作用如下。使用放置在动物头部上固定位置的基准球阵列对猪进行成像。在手术室,使用精确放置在相同位置的光学反射器球代替所述基准球。使用位于所述机械手的下侧的照相机对所述光学反射器球进行可视化。所述光学反射器球和MRI基准由计划软件(planning software)自动共寄存。所述软件只允许来自单一平面的MR图像可视化。结果,由于冠状图像(coronal image)便于最好地观察平面化套管轨道,因此将它们用于所有的手术计划。为了实施手术程序,将设计来适应于钻孔形成和套管输送工具的一系列末端效应器(end-effector)放置在机械手上。
形成用于载体输注的敏感输送套管(acute delivery cannula),所述套管包括熔融二氧化硅管(外部直径为220μm,内部直径为150μm),所述熔融二氧化硅管沿着其远端长度由刚性氧化锆管支撑。为了方便在输注病毒之前采用1%BSA对组织进行预输注,形成由熔融二氧化硅衬里的三通连接器,使得两个注射器可以直接附接至敏感套管的熔融二氧化硅管(图1)。这确保了套管能够被插入到辐射冠,然后在不需要移除所述套管以再次加载病毒的情况下进行BSA和病毒的输注。按如下方式将80μl载体(1×107pfu/ml)输注到每个半球的辐射冠中:
左半球:将套管插入到还差8mm才到达标靶的位置并以1μl/分钟预输注1%BSA2分钟、以2.5μl/分钟预输注2分钟,然后以5μ1/分钟预输注7分钟(BSA总体积为42μ1)。然后以5μ1/分钟的速率输注载体8分钟(载体总体积为40μ1)。然后将套管插入至标靶,并且再次以1μl/分钟预输注1%BSA2分钟、以2.5μl/分钟预输注2分钟,然后以5μ1/分钟预输注4分钟(BSA总体积为42μ1)。然后以5μ1/分钟的速率输注载体8分钟(载体总体积为40μ1)。
右半球:将套管插入至标靶中,并以1μl/分钟预输注1%BSA2分钟、以2.5μl/分钟预输注2分钟,然后以5μ1/分钟预输注7分钟(BSA总体积为42μ1)。然后立即以5μ1/分钟的速率输注80μl的载体16分钟(载体总体积为40μ1)。
输注完成后,套管保持就位10分钟,然后手动缓慢撤出。在伤口闭合前,来自钻孔的脑脊液渗漏和套管轨道采用Cerebond密封。然后将动物转移回到MRI扫描仪并进行T2加权成像以确认套管已经被准确插入至标靶中。让动物恢复28天的时间,然后在最终麻醉下通过灌注固定处死动物,并获取脑部进行组织学分析。。
组织学
使用Leica CM1850低温恒温器(Leica Microsystems,德国)将大鼠大脑切除35μm后的冠状切片(coronal section)。使用Leica SM2500显微切片机将猪大脑切成100μm的冠状切片。对选定的切片进行免疫组织化学。简而言之,所有用于免疫组织化学的溶液都是在磷酸盐缓冲盐水(PBS)中配制。将自由浮动的PFA固定切片在PBS中洗涤3次,15分钟,在3%的过氧化氢中温育以除去内源性过氧化物(peroxidise)活性。然后将切片于PBS中洗涤3次,15分钟,然后于室温在在封闭溶液(10%正常山羊或驴血清)中封闭1小时。然后将切片从封闭溶液中直接转移到在封闭溶液适当稀释的一抗中,并温育过夜。使用以下一抗:小鼠抗NeuN(1:300;Chemicon,英国)、兔抗GFAP(1:200,Chemicon)、小鼠抗-ED1(1:100;Serotec,英国)、小鼠抗CD4(Ox38)(1:300;Serotec,英国)和小鼠抗CD8(ox8)(1:300;Serotec,英国)。三次PBS洗涤后,接着将切片与二抗于室温温育至少2小时。对于荧光免疫组织化学,使用物种特异性二抗(Cy3标记)(1:200;Jackson实验室,CA,美国)。在PBS洗涤后,将切片固定在明胶包埋的载玻片上的Vectashield(Vectorlabs,CA,美国)中并封片,然后进行荧光成像。
成像
使用Leica DM5500显微镜(Leica Mircrosystems,德国)和数码相机(Microbrightfield,美国)进行荧光成像。使用市售软件(Stereoinvestigator,Microbrightfield)在免疫染色切片上进行体视学计数。简而言之,在代表性组织切片上进行群体估计,以确定计数的帧大小、计数帧编号和数量,并进行必要的组织切片分离以获得准确的细胞计数,误差的Gundersen(m=1)误差系数小于0.1。然后使用这些参数,采用Optical Fractionator探针对一系列以等距间隔开的切片进行细胞计数。通过描绘每个切片上的EGFP表达细胞的外部边缘周围的轮廓线计算被转导的细胞的分布体积。从这些轮廓线排除纹状体外部的被转导细胞,以确保只计算被转导细胞的纹状体内的分布体积。从进一步分析排除与载体明显渗漏到脑室系统中的渗漏有关的输注。使用Fractionator探针,在对靠近针头轨道的选定组织切片上,确定每次输注的病毒分布体积中载体细胞向性和激活的小胶质细胞和CD4以及CD8阳性T淋巴细胞的密度。
统计分析
使用Tukey检验结合方差分析(ANOVA),以确定在与不同输注参数有关的载体分布和细胞转导方面是否存在显著差异。
结果
大鼠纹状体和白质中的HSV输注
以0.5μl/分钟的流速进行的大鼠纹状体和白质中的HSV-1输注与严重组织损伤和靠近输注部位近端处的细胞数量可忽略的转导有关(图2)。在试图改善被转导细胞的分布体积和数量时,以较高的流速(2.5μl/分钟)重复输注以提高套管尖端所达到的压力。然而,观察到严重组织损伤的类似图案,细胞转导水平低,并且病毒性转导的细胞分布少。
据推测,与HSV输注有关的严重组织损伤和病毒性转导细胞的分布差是由紧挨套管尖端周围的组织中的病毒颗粒聚集造成的,或者是因为相对较大的病毒颗粒(直径为约250μm)被迫使进入窄的胞外空间的缘故。因此,设计两种策略来检验这些推测。试图使套管尖端周围的大量病毒结合最小化时,在肝素溶液中重复输注。尽管与以0.5μl/分钟和2.5μl/分钟输注载体(在标准缓冲液中)相比,在肝素存在的情况下被转导细胞的数量和病毒性转导细胞的分布体积增加了,但是这种增加不具有统计学显著性(图3a和b)。相反,使用1%牛血清白蛋白(BSA)的等渗溶液对白质进行预输注,然后立即进行病毒输注,导致被转导细胞的数量和被转导细胞的分布体积的显著增加,两者都是在输注后48小时时达到峰值(图3a和b)。与以0.5μl/分钟和2.5μl/分钟进行的载体输注(在标注缓冲液中)相比,这种效果对于被转导细胞的数量(分别为p=0.012和p=0.013)和被转导细胞的分布体积(分别为p=0.019和p=0.011)都是具有统计学显著性的。这种效果在纹状体中没有被观察到。图4显示了与HSV和肝素联合输注有关的出血,因此这种方式在临床上是不可行的。
成功HSV输注后的白质中的免疫细胞浸润
使用1%BSA对白质进行的预输注使细胞转导广泛分布而没有显著的输注相关的组织损伤(图5a)。EGFP阳性细胞的数量和EGFP阳性细胞的分布体积在48小时时达到峰值。然而,白质中HSV-1病毒颗粒的广泛分布确实导致ED1阳性小胶质细胞(图5b)和CD8阳性T细胞(图5c)的快速浸润,这种浸润在24小时和72小时之间增加(图5d)。
白质中HSV-1的转导向性
使用1%BSA对组织进行预输注,然后使HSV-1在白质中有效地分布,导致GFAP阳性星形细胞转导的广泛分布(图6a、b和c)。虽然没有包括在白质中转导细胞的数量和转导细胞的分布体积的分析中,但是细胞还转导至皮层中。从形态学上来看,这些细胞中的大部分细胞是星形细胞,不过这些细胞中的一些细胞与神经元标记NEUN共定位(图6d、e和f)。虽然转导至白质中的细胞中的大部分细胞与GFAP共定位,但是显著比例的ED1阳性激活小胶质细胞也被转导(在图6中的图表)。在24小时和72小时之间的星形细胞或小胶质细胞转导的百分比没有显着的差异。
猪的HSV输注
确定使用1%的BSA对组织进行预输注能够使大鼠白质中的HSV-1载体的广泛分布之后,在体积大得多的猪模型大脑上对这种方法的实用性进行评价。沿着辐射冠内的导管轨迹的两个部位处对左半球进行输注,而在右半球中,在辐射冠中的单个部位处输注全部病毒溶液。让动物恢复2天,证明在这段时间并没有表现出异常的神经系统病征。
在左侧,近端输注与套管尖端损坏不相关。与此相反,在远端标靶部位输注导致少量出血,直径为约3mm(图7,中间图)。尽管有这种损伤,但是在整个辐射冠和额叶皮质中的远端部位、胼胝体和脑岛中,观察到EGFP阳性细胞的广泛分布(图7a-g)。此外,发现EGFP阳性细胞定位在距离输注部位非常远的血管周围空间,特别是在豆纹动脉的附近。除了局限于血管周围空间的EGFP阳性细胞之外,在距离输注部位长达15mm的脑实质中观察到转导细胞。输注部位周围的T2加权MR图像(图7i-k)清楚地显示了延伸通过辐射冠以及此外的出血区的高信号的证据。
在右半球的输注与导管尖端处的损伤不相关(图8,中间图)。在辐射冠和皮质以及远至胼胝体和颞叶皮层中观察到EGFP阳性细胞的广泛分布(图8a-f、h)。如在左半球所见到的那样,EGFP阳性细胞存在于豆纹动脉分支周围的血管周围空间中(图8g)。
输注的T1加权MR图像显示辐射冠中大面积的高信号。从组织学上来说,所有高信号区域与病毒转导组织区域相对应。然而,虽然EGFP表达细胞在远离高信号区域的边界之外的位置也存在,不过可能已经由于输注载体而不是CED的轴突或者血管周围运输而导致远离输注部位的细胞的转导。
图9示出了在猪辐射冠中由HSV-1转导细胞引起的免疫反应。表达EGFP的转导细胞与ED1共定位(图9a-c),这说明存在CD68表达的小胶质细胞,并且此外还有GFAP(图9d-f)。
讨论
高分级神经胶质瘤是高度浸润性肿瘤。虽然主要的肿瘤块往往可以采用手术和/或放射性疗法进行有效的治疗,但是破坏浸润性肿瘤细胞在技术上具有挑战性。对于为证明有效而进行的载体介导溶瘤,首先要求对于初始载体施用要尽可能多地转导肿瘤细胞。然而,由于这些肿瘤包含大量的坏死区域,直接肿瘤内载体输送往往是无效的,因此不是本研究的焦点。这反应在以前为展示高分级神经胶质瘤的治疗中的有效性的令人信服的证据所进行的临床试验的失败中(7-9)。实际上,更近的研究已经集中在将复制选择性HSV载体施用到肿瘤周围组织中(6)。由于通过CED来实现大脑中的载体的广泛分布是可能的,因此对于临床实践中的溶瘤性复制选择性载体的输送而言是最合理的方法。然而,曾有怀疑直径在120nm至300nm(Jacobs等人,1999)之间的基于HSV的载体是否能够输注通过直径不大于80nm(Thorne和Nicholson,2006)的大脑细胞外空间。因此,在这项研究中,详细地检测了通过CED将复制选择性HSV-1载体施用至正常大脑中的可行性。尽管已经有大量的临床前研究明显涉及基于HSV的载体的直接颅内施用(17、18、20-24、26-41),但是这次研究是首次报道采用适当的输注参数同时评价在白质和灰质中HSV-1载体的分布性质并且评价改善载体分布的策略的研究。
考虑到大脑细胞外空间和HSV-1颗粒的直径之间的尺寸差异,令人吃惊的是,以0.5μ1/分钟进行的载体输注导致白质和灰质中严重的组织损伤,并有最小量的载体渗透到组织中。鉴于这种损伤,曾推测以较高的流速以及由此导致的较高的压力进行输注可能会通过扩展大脑细胞外空间而改善载体分布。然而,观察到类似水平的组织损伤,并且载体分布差。这种组织损伤与发现在肿瘤内注射HSV1716的两个患者的大脑中的输注部位处有囊性空洞的Rampling等(2000)进行的死后观察吻合。
类肝素硫酸蛋白聚糖已知作为HSV-1的细胞表面受体(WuDunn和Spier,1989年;Shieh等,1992年),还有很多其他病毒,包括II型重组腺相关病毒(rAAV2)。事实上,已经证明与肝素的联合输注改善了成年大鼠的纹状体中的rAAV2的分布,推测是与类肝素硫酸蛋白聚糖的病毒结合的竞争性拮抗作用的缘故(Nguyen等,2001)。此外,Mastakov等(2002a)证明,浓度为500IU/ml至5000IU/ml的肝素联合输注显著地改善了报道基因转移的分布,不过较高的浓度与致死性颅内出血有关。然而迄今为止,尚未见HSV-1和肝素的联合输注的报道。
在这项研究中,5000IU/ml浓度的HSV联合输注导致EGFP表达的分布略有增加,不过动物确实出现在临床上检测不到而是在灰质和白质中的严重的脑出血。考虑到与在没有肝素的情况下的HSV-1输注有关的分布广泛的损伤,与肝素的联合输注导致这种严重出血是不意外的。因此,这种方法在临床实践中不像具有重大价值。
有大量的研究评价了具有与HSV-1相似的尺寸的纳米颗粒的基于CED的分布性质。Chen等(2005)和Mackay等(2005年)证明了直径为200nm的聚苯乙烯纳米颗粒和脂质体只渗透到大鼠纹状体中短的距离。然而,有趣的是,Chen等(2005)证明,使用白蛋白涂布的纳米颗粒以屏蔽疏水性颗粒表面显著地改善它们的分布。此外,Neeves等(2007)证明,用等渗盐水对纹状体进行预输注显著地改善了直径为53nm的聚苯乙烯纳米颗粒在大鼠纹状体中的分布。这种效果大于以前预输注透明质酸酶来降解大脑胞外基质或者预输注高渗甘露醇来渗透性地扩大细胞外空间所观察到的情况。
在这项研究中,合并使用白蛋白的前处理来屏蔽非特异性结合并实现大脑胞外空间的等渗扩大便于施用复制选择性的HSV-1载体。这种方法便于大鼠白质和猪大脑中载体的广泛分布,不过在大鼠灰质中没有成功。尽管如此,这些结果清楚表明,使用适当的组织预输注,在白质内实现非常广泛的载体分布是可能的。使用猪模型的主要优点是这些动物具有比很多灵长类动物大的大且多脑回的大脑,并且能够容纳大体积的输注,药物输注套管的规模能够直接转入人类研究。实际上,本发明人打算将来在临床试验中使用这种套管系统。
在白质而不是灰质中分布HSV的能力很可能与白质较高的弹性因而其具有较大的容量来容纳所输注的体积有关(Bobo等,1994)。事实上,在大脑胞外空间的实验性诱导扩张存在的情况下通过少突胶质细胞方法已经证明保持有髓轴突的微结构定取向。结果,虽然对白质预输注等渗白蛋白溶液使载体能够大量分布,但是可能不可行的是,使用这种方法充分地靶向已经在灰质结构中出现或者已经浸润到灰质结构中的肿瘤细胞。然而有趣的是,有清楚的证据证明在载体输注后猪的皮层中的转导细胞进入到辐射冠中。鉴于HSV不能分布通过大鼠中的灰质,似乎通过来自输注部位的载体的轴突或者血管周围运输导致使皮层细胞转导发生。由于HSV不太可能能够有效地转导白质中的有髓轴突,并且鉴于所观测到的血管周围空间中的转导细胞分布,这两种机制中的后一种机制是最有可能的。
白质中载体的成功分布导致敏感的免疫反应,其特征是激活的小胶质细胞和CD8阳性T细胞的快速浸润。然而,有一些证据表明,ED1也是一种用于缺血性和创伤性脑损伤中的中性粒细胞的标记(41),因此这些浸润性细胞的真实身份可能值得进一步检查。在治疗脑肿瘤的情况中,抗HSV CD8反应的诱导可能具有增强肿瘤细胞杀灭的额外优点(Todo等,1999)。不过,虽然没有动物出现可检测的神经学缺损,但是大脑中利用HSV-1载体的细胞转导的广泛分布的长期后果以及所导致的强烈的免疫反应需要进行仔细的评价。大量载体从免疫相对优先的大脑逃逸并进入到系统循环中结果形成适应性免疫反应的可能性更加凸显这种必要性。实际上,这可以通过与套管植入有关的组织损伤、输注有关的损伤或者载体沿着血管周围排出进行调节。这是特别重要的,因为尽管抗原呈递树突细胞并不存在于没有感染的大脑中,但是树突细胞存在于血管周围空间和CSF中,在其中血管周围空间排尽(McMahon等,2006)。
病毒载体泄漏到CSF中和免疫反应的形成的潜在风险凸显了临床可视化载体分布的重要性。因此,已经开发了很多Tl和T2造影剂作为载体分布的的替代标记(Szerlip等,2007;Fiandaca等,2008)。然而,载体与替代标记的联合输注显著增加了在临床上将基因治疗载体施用到脑部中的复杂性和调控障碍。在这项研究中,进行T2加权MR成像以尝试可视化输注相关的水肿。这些图像清楚地显示在输注附近的高信号区域,这些区域在组织学上与白质中转导细胞的位置对应。不过细胞转导明显超出这种高信号的边缘。然而,鉴于加大通过CED直接获得的分布的载体的明显的血管周围运输和可能的轴突运输,使用替代标记或MR成像将提供HSV载体介导转导细胞的最终分布的现实预测是非常不可能的。
总之,来自本研究的证据表明,HSV-1的载体太大,以至于无法有效地通过CED进行分布,除非对标靶组织进行预输注以扩张脑部胞外空间。这一发现在解释涉及使用基于HSV病毒载体治疗脑部肿瘤患者的临床试验的结果中具有重要的含意。实际上,该数据表明,在这些试验中采用的输注方法将很可能导致可忽略的载体分布。然而,在未来临床试验中利用这一发现具有通过使转导肿瘤细胞的数量最大化从而使治疗效果的可能性最大化来改善患者结局的潜能。

5.根据权利要求1至3中任一项所述的组合物,其中,所述组合物用于治疗癌症,特别是脑部癌症,尤其是神经胶质瘤。
6.一种用于治疗癌症,特别是脑部癌症,尤其是神经胶质瘤的方法,所述方法包括将权利要求1至3中任一项所述的组合物施用于需要的受试者。
7.一种白蛋白,所述白蛋白用于神经胶质瘤的治疗。
8.一种白蛋白,所述白蛋白用于引发细胞或者组织,尤其白质、星形细胞或者神经胶质瘤细胞,以接收基因治疗载体。
9.一种白蛋白,所述白蛋白用于改善基因载体通过细胞或者组织,尤其是通过白质、星形细胞或者神经胶质瘤细胞的转导。
10.根据权利要求7至9中任一项所述的白蛋白,其中,所述白蛋白在要施用基因治疗载体的受试者中使用。
11.根据权利要求7至10中任一项所述的白蛋白,其中,所述白蛋白为等渗溶液和/或人造CSF。
12.一种基因治疗载体,所述基因治疗载体用于神经胶质瘤的治疗。
13.根据权利要求12所述的基因治疗载体,其中,所述基因治疗应用于已经施用白蛋白的受试者。
14.一种用于改善基因治疗载体的转导的方法,所述方法包括向受试者施用白蛋白。
15.一种用于治疗神经胶质瘤的方法,所述方法包括将白蛋白和基因治疗载体施用于受试者。
16.根据权利要求15所述的方法,其中,所述基因治疗载体在施用白蛋白之后施用。
17.根据权利要求6、15或16所述的方法,其中,所述白蛋白和/或基因治疗载体通过输注尤其是经由细导管进行施用。
18.根据权利要求17所述的方法,其中,所述白蛋白和/或基因治疗载体通过对流增强输送法施用。
