近年来,重组疫苗、特别是重组活疫苗的研发取得了进展,其中载体编码和表达目的外源抗原。其中,基于重组病毒的载体已显示很有前途,并在新疫苗的开发中发挥重要作用。已对许多病毒研究了其表达来自外源病原体或肿瘤组织的蛋白质的能力,以及在体内诱导针对这些抗原的特异性免疫应答的能力。这些基于基因的疫苗一般能刺激高效的体液和细胞免疫应答,且病毒载体可能对于递送抗原编码基因以及促进和增强抗原呈递而言都是有效的策略。为了作为疫苗载体,理想的病毒载体应该是安全的,并能向免疫系统高效呈递所需的病原体特异性抗原。此外,载体系统必须满足使其能够大规模生产的标准。因此,目前已经出现了一些病毒疫苗载体,它们根据所提出的应用均具有相对的优点和局限性(重组病毒疫苗的综述参阅如Harrop和Carroll,2006,Front Biosci.,11,804-817;Yokoyama等,1997,J Vet Med Sci.,59,311-322)。
在二十世纪九十年代初期观察到质粒DNA载体可在体内直接转染动物细胞之后,已经进行了大量的研究来开发通过直接向动物中引入编码抗原的DNA而基于使用DNA质粒来诱导免疫应答的免疫接种技术。这些通称为DNA免疫接种的技术目前已经用于在大量疾病模型中引发保护性免疫应答。DNA疫苗的综述参阅Reyes-Sandoval和Ertl,2001(CurrentMolecular Medicine,1,217-243)。
然而,在疫苗领域的一个一般性问题是鉴定在接种的个体中诱导足够强的免疫应答以保护和/或治疗免于感染和疾病的手段,从而延长患有致命性疾病例如癌症的患者的存活。
因此,近年来已进行了大量工作来发现通过刺激免疫系统的某些关键方面来发挥作用的新药物化合物,用于提高疫苗诱导的免疫应答。多数这些化合物(称为免疫应答调节剂(IRM)或佐剂)似乎都通过基础免疫系统机制,经由Toll样受体(TLR)诱导多种重要细胞因子(例如干扰素、白介素、肿瘤坏死因子等。参阅如Schiller等,2006,Exp Dermatol.,15,331-341)的生物合成发挥作用。这些化合物已显示刺激迅速释放某些T细胞、树突细胞、或单核细胞/巨噬细胞衍生的细胞因子,还能刺激淋巴细胞功能,例如B细胞分泌在IRM化合物的抗病毒及抗肿瘤活性中发挥重要作用的抗体。
备选地,已经提出了免疫接种策略,其中多数是基于引发-加强免疫接种的方案。根据这些“引发-加强”的免疫接种方案,首先通过对患者施用引发组合物对免疫系统进行诱导,然后通过施用加强的第二组合物进行加强(参阅如EP1411974或US20030191076)。
此外,在医疗保健背景中已显示一种治疗可能仅在特定组的患者中有效。因此希望为医生提供使他们能够定制最佳的个性化患者疗法的工具和方法,即在适当的时间对适当的患者指示适当的疗法以提供更高的治疗成功率,监控对治疗的反应,提高药物效力和安全性,消除对患者的不必要的治疗(对患者不合适的疗法),减少患者的不必要的毒性和副作用,降低患者和保险公司的不必要或危险的无效药物的费用,并提高患者的生活质量,最终使癌症成为可控的疾病,并根据情况跟进测定。
在这一点上,文献提出了多种工具和方法,例如:
-药物遗传学,其包括根据遗传差异对药物的个体反应的研究。这些反应涉及药物如何在任意给定个体中发挥功能,其如何被代谢,其毒性和剂量要求。随着人类基因组计划的发展,药物遗传学已扩大为药物基因组学。药物基因组学超越了药物遗传学,可能在药物发现和开发、靶发现和确认和临床试验中发挥作用。
-也可在预测医学领域应用代谢组学(Metabolomics)。与局限于遗传因子的药物遗传学不同,药物代谢组学能够不但基于遗传因素而且基于非遗传因素,例如在患者体内的其他药物,患者目前的健康状态等等预测个体对药物的反应。
-生物标志物在治疗学的临床发展中具有越来越重要的作用。生物标志物可为正常生物学过程、疾病过程或对治疗干预的药理学反应的指示剂。其作用范围从对患者群体分组以帮助鉴定应答者和非应答者,以测定治疗的效力。生物标志物可作为有价值的工具用于进行更好的决策以减少药物开发的费用和使治疗更快地应用于最适当的患者群体。
本发明提供了使用生物学标志物(生物标志物(biomarker))预测涉及对患者施用免疫原性组合物(即免疫疗法治疗)的治疗效力的材料和方法,所述生物标志物已确定是和预期临床结果相关的基本可信的信号。在用所述免疫原性组合物治疗之前,生物标志物存在于从患者处得到的生物样品中。在治疗开始以前预测治疗的临床效果的能力使临床医生和患者能够鉴定无效的治疗,对相关治疗过程进行知情决策,包括是否放弃或允许实施备选的疗法。
本申请人现已鉴定出新的工具和免疫接种策略。更具体地,本发明涉及白细胞介素10/干扰素γ(IL10/IFNγ)的比率作为预测患者是否易于产生通过施用免疫原性组合物的预防性或者治疗性应答,优选地免疫应答的生物标志物的用途。备选地,根据本发明,能够将干扰素γ/白细胞介素10(IFNγ/IL10)比率用作生物标志物,但是在那种情况下应该改变结论。
Van den Boogaardt等,2006(Transplantation,82,844-848)已经显示了干扰素γ/白细胞介素10(IFNγ/IL10)比率是区分对于肾脏移植非排异和排异患者的有价值的工具。
Jamal等,2007,(Tuberculosis,87,279-287)已经显示在肺和肺外结核中,干扰素γ/白细胞介素10(IFNγ/IL10)比率和疾病严重度等级之间有直接的关系。
相似地,Gomes-Silva等,2007,(Clinical and ExperimentalImmunology,149,440-444)表明高IFNγ和低IL10与黏膜利什曼病的严重度相关联。

根据另一个实施方案,本发明涉及通过施用免疫原性组合物而在患者中诱导免疫应答(即所产生的免疫应答)用于治疗人类疾病的方法,其中所述患者选自由具有低IL10/IFNγ比率的患者组成的患者群且其中所述产生的免疫应答是先天免疫应答。先天免疫应答是针对病原体的身体最初免疫防御且由多种细胞包括抗原呈递细胞或者“APC”引发。这些细胞表达识别外源来源的分子(例如,细菌和病毒核酸、蛋白质、碳水组合物)的表面和胞质受体。在检测到这些信号后,树突状细胞和巨噬细胞引发了防御性应答,包括细胞因子(包括干扰素、TNF-α和IL-12)和吸引细胞如未成熟的树突状细胞、巨噬细胞、NK细胞和粒细胞到攻击部位的趋化因子的释放。因此,当身体在产生适应性应答的同时,先天免疫应答赋予了非特异性保护。

根据另一个具体实施方案,本发明的所述异源核苷酸序列编码HPV抗原的全部或一部分,所述HPV抗原选自HPV的E6早期编码区、HPV的E7早期编码区及其衍生物或组合。
本发明重组载体所编码的HPV抗原选自HPV E6多肽、HPV E7多肽,或者同时有HPV E6多肽和HPV E7多肽。本发明包括使用与p53之结合发生改变或至少显著降低的任何HPV E6,和/或使用与Rb之结合发生改变或至少显著降低的任何HPV E7多肽(Munger等,1989,EMBO J.8,4099-4105;Crook等,1991,Cell 67,547-556;Heck等,1992,Proc.Natl.Acad.Sci.USA 89,4442-4446;Phelps等,1992,J.Virol.66,2148-2427)。适用于本发明目的的一种非癌基因HPV-16E6变体缺失了位于约118位至约122位(+1代表天然HPV-16E6多肽的第一个甲硫氨酸残基)的一个或多个氨基酸残基,尤其优选完全缺失118至122位残基(CPEEK)。适用于本发明目的的一种非癌HPV-16E7变体缺失了位于约21位至约26位(+1代表天然HPV-16E7多肽的第一个氨基酸)的一个或多个氨基酸残基,尤其优选完全缺失21至26位残基(DLYCYE)。根据一个优选的实施方案,用于本发明的一种或多种HPV-16早期多肽经进一步修饰,以改善I类MHC和/或II类MHC呈递,和/或刺激抗HPV免疫。HPV E6和E7多肽是核蛋白,之前已显示膜呈递可以改善其治疗效力(参阅如WO99/03885)。因此,将至少一种HPV早期多肽修饰成锚着在细胞膜上是可取的。膜锚着可通过在该HPV早期多肽中掺入膜锚着序列(如果天然多肽中缺少该序列,则掺入分泌序列,即信号肽)来容易地实现。膜锚着序列和分泌序列为本领域已知。简言之,分泌序列存在于位于膜上或分泌的多肽的N端,并起始其通过内质网(ER)。它们通常含有15至35个基本为疏水性的氨基酸,其后通过位于ER上的特异性内肽酶除去,得到成熟多肽。膜锚着序列通常为高疏水性质,并用于将多肽锚定在细胞膜上(参阅如Branden和Tooze,1991,Introduction to Protein Structure p.202-214,NY Garland)。
可用于本发明上下文的膜锚着序列及分泌序列的选择十分广泛。它们可得自任何包含它的膜锚着多肽和/或分泌多肽(例如细胞或病毒多肽),例如狂犬病糖蛋白、HIV病毒包膜糖蛋白和/或麻疹病毒F蛋白,或者可以是合成的。插入本发明所用早期HPV-16多肽中的膜锚着序列和/或分泌序列可具有共同或不同的来源。优选插入分泌序列的位点是翻译起始密码子的N端下游,膜锚着序列的则为C端,例如紧接终止密码子之后的上游。
本发明使用的HPV E6多肽优选地通过插入麻疹F蛋白的分泌和膜锚着信号来进行修饰。任选地或与其组合地,本发明使用的HPV E7多肽优选地通过插入狂犬病糖蛋白的分泌和膜锚着信号来进行修饰。

在本发明融合蛋白的这些特定部分中,每个NS多肽可独立地是天然的或修饰的。例如,NS4A-NS3部分中包含的NS4A多肽可以是天然的,而NS3多肽包含下述修饰中至少一种。
必要时,用于本发明的核酸分子可以进行优化,以在特定宿主细胞或生物(例如人宿主细胞或生物)中提供靶标抗原(如HPV早期多肽)的高水平表达。密码子优化一般通过将哺乳动物宿主细胞中较少使用的一个或多个“天然”(例如HPV)密码子替换为一个或多个更频繁使用的编码相同氨基酸的密码子来进行。这可通过常规诱变或通过化学合成技术(例如产生合成核酸)来实现。没有必要替换对应于较少使用密码子的所有天然密码子,因为即使部分替换也可实现提高的表达。此外,可以产生与严格遵从优化的密码子使用的一些偏离,用于适应限制性位点的引入。
本文使用的术语“重组载体”指病毒及非病毒的载体,包括染色体外(如附加体)、多拷贝和整合型载体(即,整合进宿主染色体中)。本发明中特别重要的是用于基因治疗的载体(即,能将核酸递送至宿主生物的载体)以及用于多种表达系统的表达载体。合适的非病毒载体包括质粒,例如pREP4,pCEP4(Invitrogene),pCI(Promega),pCDM8(Seed,1987,Nature329,840),pVAX和pgWiz(Gene Therapy System Inc;Himoudi等,2002,J.Virol.76,12735-12746)。合适的病毒载体可来自多种不同病毒(例如逆转录病毒、腺病毒、AAV、痘病毒、疱疹病毒、麻疹病毒、泡沫病毒等。本文使用的术语“病毒载体”包括载体DNA/RNA以及由其产生的病毒颗粒。病毒载体可以具有复制能力,或者可以在遗传上使其失去能力,从而成为复制缺陷的或复制受损的。本文使用的术语“具有复制能力”包括复制选择性及条件性复制的病毒载体,它们被改造成在特定的宿主细胞(如肿瘤细胞)中更好地复制或选择性复制。
在一个方面中,用于本发明的重组载体是重组腺病毒载体(综述参阅“Adenoviral vectors for gene therapy”,2002,D.Curiel和J.Douglas编辑,Academic Press)。它可来自多种人或动物来源,并且从腺病毒血清型1至51的任何血清型均可使用。特别优选的是人腺病毒2(Ad2)、5(Ad5)、6(Ad6)、11(Ad11)、24(Ad24)和35(Ad35)。这些腺病毒可得自美国典型培养物保藏中心(ATCC,Rockville,Md.),并且已有大量文献描述其序列、组织及产生方法,使得本领域技术人员能够应用它们(参阅如US 6,133,028;US6,110,735;WO 02/40665;WO 00/50573;EP 1016711;Vogels等,2003,J.Virol.77,8263-8271)。
用于本发明的腺病毒可以具有复制能力。具有复制能力的腺病毒载体的多种实例易于为本领域技术人员获得(参阅如Hernandez-Alcoceba等,2000,Human Gene Ther.11,2009-2024;Nemunaitis等,2001,Gene Ther.8,746-759;Alemany等,2000,Nature Biotechnology 18,723-727)。例如,可通过以下方法从野生型腺病毒基因组中改造它们:缺失E1A CR2结构域(参阅如WO00/24408)和/或将天然E1和/或E4启动子替换为组织、肿瘤或细胞状态特异性启动子(参阅如US 5,998,205,WO99/25860,US5,698,443,WO00/46355,WO00/15820和WO01/36650)。
备选地,用于本发明的腺病毒载体可以是复制缺陷型的(参阅如WO94/28152;Lusky等,1998,J.Virol 72,2022-2032)。优选的复制缺陷型腺病毒载体为E1缺陷型(参阅如US 6,136,594和US 6,013,638),E1缺失大致从459位延伸至3328位,或大致从459位延伸至3510位(人5型腺病毒的序列公开于GenBank登记号M73260以及Chroboczek等,1992,Virol.186,280-285)。可以通过缺失腺病毒基因组的其他部分(非必需E3区或其他必需E2、E4区的全部或一部分)来进一步提高克隆容量。可以如Chartier等(1996,J.Virol.70,4805-4810)所述通过同源重组在腺病毒载体的任何位置插入核酸。例如,可将编码HPV-16 E6多肽的核酸替换E1区插入,并将编码HPV-16E7多肽的核酸替换E3区插入,或反之亦然。

根据特定的实施方案,所述病毒可以是具有复制能力的痘病毒,特别地可以具有复制能力的痘苗病毒。在WO9531105(例如产品JX594、VVTK-GMCSF或JX963、VV TK-VGF-GMCSF)、WO0073479、WO2009/065547、WO2009/065546中提供这些病毒的实例。
在痘病毒基因组中插入核酸及表达所需相关调节元件的基本技术描述于本领域技术人员可获得的大量文献中(Paul等,2002,Cancer gene Ther.9,470-477;Piccini等,1987,Methods of Enzymology 153,545-563;US4,769,330;US 4,772,848;US 4,603,112;US 5,100,587和US 5,179,993)。一般通过病毒基因组与带有待插入核酸的质粒之间所存在重叠序列(即,期望的插入位点)之间的同源重组来进行。
编码本发明抗原的核酸优选地插入痘病毒基因组的非必需基因座中,以使重组痘病毒仍能生存和仍有感染性。非必需区是非编码的基因间区域,或者是其失活或缺失不显著损害病毒的生长、复制或感染的任何基因。还可以设想插入必需的病毒基因座中,只要缺陷功能在病毒颗粒的产生过程中以反式提供即可,例如使用辅助细胞来提供,其带有对应于痘病毒基因组中所缺失序列的补充序列。
在使用哥本哈根痘苗病毒时,抗原编码核酸优选插入胸苷激酶基因(tk)中(Hruby等,1983,Proc.Natl.Acad.Sci USA 80,3411-3415;Weir等,1983,J.Virol.46,530-537)。然而,其他插入位点也是合适的,例如在血凝素基因中(Guo等,1989,J.Virol.63,4189-4198)、在K1L基因座中、在u基因中(Zhou等,1990,J.Gen.Virol.71,2185-2190)或者痘苗病毒基因组的左侧末端,这里已经在文献中报道了多种自发或改造的缺失(Altenburger等,1989,Archives Virol.105,15-27;Moss等1981,J.Virol.40,387-395;Panicali等,1981,J.Virol.37,1000-1010;Perkus等,1989,J.Virol.63,3829-3836;Perkus等,1990,Virol.179,276-286;Perkus等,1991,Virol.180,406-410)。
在使用MVA时,抗原编码核酸可插入已鉴定的缺失I至VII中的任一个或插入D4R基因座,但插入缺失II或III中是优选的(Meyer等,1991,J.Gen.Virol.72,1031-1038;Sutter等,1994,Vaccine 12,1032-1040)。
使用禽痘病毒时,尽管可以考虑插入胸苷激酶基因中,但抗原编码核酸优选导入ORF 7和9之间的基因间区域(参阅如EP 314569和US5,180,675)。

根据一个最优选的实施方案,用于本发明的重组载体编码MUC1抗原的全部或一部分以及以上所列的至少一种细胞因子,且优选白介素,特别是IL2。优选地,用于本发明的重组载体是编码MUC1抗原的全部或一部分以及以上所列的至少一种细胞因子,且优选白介素,特别是IL2的MVA。
包含上述重组病毒载体的感染性病毒颗粒可通过常规方法产生。示例性方法包括以下步骤:
a.将病毒载体引入合适的细胞系中,
b.在适当条件下培养所述细胞系,以允许产生所述感染性病毒颗粒,
c.从所述细胞系的培养物中回收所产生的感染性病毒颗粒,和
d.任选地纯化所回收的感染性病毒颗粒。
适用于增殖腺病毒载体的细胞为例如293细胞、PERC6细胞、HER96细胞或者WO 94/28152,WO 97/00326,US 6,127,175中公开的细胞。
适用于增殖痘病毒载体的细胞为禽类细胞,最优选由得自受精卵的鸡胚制备的原代鸡胚成纤维细胞(CEF)。
感染性病毒颗粒可从培养物上清液中回收或者在裂解后从细胞中回收(例如通过化学方法、冻融、渗透压休克、机械休克、超声处理等)。病毒颗粒可通过连续的斑点纯化轮次来分离,接着使用本领域的技术纯化(层析法、在氯化铯或蔗糖梯度上超速离心)。
如果需要,根据本发明的治疗患者的人类疾病的方法或用途(即通过施用包含至少一种抗原的免疫原性组合物)可以与一种或多种常规的治疗方式(例如放射、化学疗法和/或外科手术)在所选择的患者中联合实施。多种治疗方法的使用为所选择的患者提供了基于更宽的干预。在一个实施方案中,根据本发明的治疗患者的人类疾病的方法或用途可以先于或后于外科干预。在另一个实施方案中,它可以先于或后于放射治疗(例如γ放射)。本领域技术人员可以轻易地制定合适的可以使用的放射治疗方案和参数(参阅如Perez和Brady,1992,Principles and Practice of radiationOncology,第二版,JB Lippincott Co;使用的合适的修改和改变对本领域技术人员来说是显而易见的。又在另一个实施方案中,本发明的方法或用途与使用一种或多种药物的化学疗法(例如常规用于治疗或预防病毒感染、病毒相关的病理病症、癌症等的药物)相关。

本发明还涉及改善细胞毒性药物(即化学治疗剂)或者放射治疗的细胞毒性效应的方法,所述方法包括用根据本发明的免疫原性组合物共治疗在由具有低IL10/IFNγ比率的患者组成的患者群中选择的患者。
在另一个实施方案中,本发明的方法或免疫原性组合物的用途根据引发加强治疗方式来实施,所述引发加强治疗方式包括顺序地施用一种或多种引发组合物和一种或多种加强组合物。引发组合物和加强组合物一般使用不同的运载体,其包含或编码至少一种共同的抗原结构域。首先施用引发免疫原性组合物给宿主生物,然后在1天至12月的不同期间后,将加强免疫原性组合物施用给同一宿主生物。本发明的方法可以包括1次至10次顺序地施用引发组合物,然后是1次至10次顺序地施用加强组合物。根据需要,注射间隔是大约1周至6月。此外,引发组合物和加强组合物可以通过相同的施用途径或者通过不同的施用途径在相同的部位或在其它的部位施用。
根据具体的实施方案,预期的临床益处是独立于所表明的对疫苗的免疫应答。

实施例1:
免疫原性组合物,著名的疫苗TG4010,与标准化学疗法联合用于治疗非小细胞肺癌(NSCLC)患者。
TG4010是既表达IL2又表达肿瘤相关抗原MUC1的重组经修饰病毒Ankara(MVA)(参阅Rochlitz等,2003,J.Gene Med.,5,690-699)。
使148名患者随机接受:
-仅化学疗法(每3周的第1天顺铂75mg/m2以及第1天和第8天吉西他滨1250mg/m2,直到6个循环)(研究组2)或者
-与TG4010一起的化学疗法(研究组1)。
每6周评价肿瘤(WHO标准)。终点是在第6月时的疾病无恶化的生存(PFS),以及总存活旨在治疗分析。
在第1天(治疗第一天)治疗前采集血液样品并立即运往中心免疫实验室,在此处分离血浆并且冷冻储存直到分析。
