Abstract

本专利申请要求申请日期为2007年12月21日、申请号为11/963,441的美国专利申请(简称′441申请)的优先权。在此通过引用并入′441申请,如同在本文中将其完全阐述一般。
技术领域
本发明涉及医用导管组件,尤其涉及利用临近导管轴远端部的远端尖部的偏转拉环使可偏转的导管轴弯曲并使远端尖部往所需方向移动的医用导管组件。
背景技术
用于多种医学疾病诊断或治疗的医用导管组件在世界各地的医疗设施中很常用。它们一般包括一根可插入接受诊断或治疗的患者的一条合适静脉或动脉内并沿着静脉或动脉延伸至所需部位的可偏转导管轴;一个支撑导管轴近端部的手持开动器;一个连接到导管轴远端部并包括一个专用于相应诊断或治疗的尖端元件的远端尖部;以及一个包括一个位于导管轴远端部附近的拉环和一些拉线的拉环组件,拉线从拉环上延伸穿过导管轴回到手持开动器,在手动操作手持开动器后用于倾斜或摆动拉线并因此拉动拉线,即,使该带有远端尖部的导管轴的一个远端部分向所需方向偏转。
消融导管组件是一类例如在治疗心功能不全时用于消融组织的医用导管组件。其可以是灌注型(不但排出消融能量,还排出消融液体)或非灌注型(排出消融能量但不排出消融液体)。远端尖部将包括一个尖部电极作为专用尖部元件,能源将连接至其手持开动器上,为尖部电极提供能量。在灌注型电极组件中,有一个液体歧管连接到尖部电极上或与其成一个整体,并有一个液体源连接到其手持开动器上,以向该处供应消融液体。无论是哪一种情况,远端尖部均可包括一个安装轴,其与附近的可偏转导管轴的远端部合作以连接到该处。
现已发现此种医学导管组件,包括灌注型或非灌注型消融导管组件,由于以拉线反复倾斜或摆动手持开动器,拉环会向手持开动器方向(离开远端尖部的方向)缓慢移动,因此导管组件的操作会随时间推移而受到影响。另外,由于拉线与拉环之间钎焊或焊接接头的疲劳失效,导致拉线与拉环组件的拉环分离,会发生医用导管组件故障。
因此,本发明的一个目标是提供一种医用导管组件(包含灌注型或非灌注型导管组件),其构造可以防止拉环往手持开动器方向(离开远端尖部的方向)缓慢移动。
本发明的另一目标是提供一种医用导管组件(包含灌注型或非灌注型导管组件),其构造可以减少拉线与拉环之间接头上的应力,从而减少因拉环组件故障而导致医用导管组件故障。
发明内容
这些目标和其它目标可由一种医用导管组件而达成,其中远端尖部构造成包含用于拉线组件的拉线的导沟,使拉线初始从与其连接的拉环上伸出往尖端元件(对于灌注型和非灌注型消融导管组件的情况,其为一个尖部电极)方向延伸,然后向导管轴方向环路而回并通过导沟穿过导管轴到达手持开动器。这将形成远端尖部与拉环联锁。这种构造将使拉环组件的拉环随着操作(拉动)拉线向尖端元件方向移动。此外,也使施加到连接拉线与拉环的钎焊或焊接接头上的应力分散(减小),从而使拉环组件的故障仅将发生在拉线本身的拉伸应力极限附近。

具体实施方式
图1所示为依据本发明优选实施例的灌注型消融导管系统10。其包括连接能量源130与液体源140的灌注型消融导管组件12。
灌注型消融导管组件12包括一个导管20、一个支撑导管20近端部的手持开动器120、一个连接至导管远端部的远端尖部30和一个拉环组件50。
如图2和3所示,远端尖部30包括一个尖部电极31、一个液体歧管33和一个安装轴38。液体歧管33可用胶粘剂连接至尖部电极上(在另一实施例中液体歧管和尖部电极可以是一个整体)。安装轴38与液体歧管33成一个整体,并伸入导管轴21的中空内部23。其直径小于液体歧管(其形状为圆柱形)的直径,从而在液体歧管的后表面上形成一个外侧环状凸缘34。安装轴界定了消融液体的一条中心轴向通路39,消融液体由导管轴中的液体输送管(图中未示出)供应。液体歧管33界定了一条中心轴向通路35,其为中心轴向通路39的延伸,并界定了从中心轴向通路35上伸出至在其外围间隔分布的孔口37的输送通道36(在另一实施例中仅采用通向一个孔口的一条输送通道)。输送至安装轴中的轴向通路39的液体将流向轴向通路35,然后经输送通道36到达孔口37,在远端尖部周围排出。尖部电极31包括一个将液体从轴向通路35输送至其远端部的通道32。其可由铂或其它众所周知的材料制成。
导沟41和42设置在安装轴38中径向相对的位置上。导沟41包括一个弯曲部分41a和一个直线部分41b。弯曲部分41a在靠近液体歧管33的安装轴外表面上有一个入口,直线部分41b在安装轴自由端40处有一个出口。导沟42具有相应的部分42a和42b。拉环组件的拉线分别延伸穿过这些导沟。
拉环组件50包括一个拉环51和拉线52及53,拉线通过钎焊或焊接接头54连接至拉环内表面上的径向相对位置。拉线沿长度方向至少有一部分是平的,特别是在其远端部处是平的,其它部分则为圆形。其它构形也是可能的。拉环51位于导管轴21的远端22与液体歧管的外侧环状凸缘34之间,而拉线从拉环向液体歧管方向延伸,然后分别环路而回进入并穿过导沟41和42至手持开动器120。在使用导管组件期间以手持开动器拉动拉线52、53将使拉环倾斜或摆动,从而使导管轴21弯曲,同时也拉动拉环51使其与液体歧管33的外侧环状凸缘34接触。
现在来看图4所示的实施例,远端尖部60包括一个专用尖端元件61(在消融导管组件中为尖部电极)及一个具有远端部分63和近端部分67的安装轴62。远端部分67包括位于其外表面上的倒钩68以夹持导管轴21a的远端部,还包括位于径向相对位置上的轴向槽道(导沟)69、70。倒钩可用具有各种不同构形的表面突出部替代。远端部分63包括一个具有在其内穿过的轴向导沟65和66的环状凸缘64,导沟65和66与轴向槽69和70对齐。拉环组件71的拉环72位于导管轴21a远端部与尖端元件61的外侧环状凸缘61a之间,而连接至其内表面径向相对位置上的拉线73和74则往尖部电极方向延伸,然后环路而回穿过各个导沟65、69和66、70至手持开动器。
在图5所示的实施例中,压缩环100将导管轴21b的一个远端部分紧压在安装轴82的一个外表面之上,安装轴与尖端元件81成一个整体,而拉环组件91的拉线93和94则往远端尖部80的尖端元件81方向延伸,然后环路而回往导管轴方向并穿过安装轴外表面上的相应轴向槽(导沟)95、96。压缩环100具有大致矩形的横断面。
在与图5类似的图6所示的实施例中,压缩环110界定了一个朝内面向安装轴的大体为平的表面111和一个大体为凸面的面朝外的表面112。
尽管已给出本发明各种实施例的详细说明,但仍可在不脱离本发明范围的情况下对其进行改动。例如,拉环组件可包括两条以上绕其圆周连接至拉环的拉线,而在远端尖部中设置相应的导沟,先将各条拉线导向远端尖部的尖端元件,然后环路而回至手持开动器。而且,图5和图6中所示的压缩环可具有不同于这些具体描述的形状。

Claims (22)

1.一种医用导管组件,包括:
一个界定一个远端部和一个中空内部的可偏转导管轴;
一个位于所述导管轴的所述远端部处并包括一个尖端元件和一个伸入所述导管轴的中空内部的安装轴的远端尖部;以及
一个包含一个拉环和连接至其上的拉线的拉环组件,所述拉环位于所述安装轴的周围;
所述远端尖部设有导沟,所述拉线通过导沟被分别引导,初始从所述拉环向所述尖端元件延伸,然后环路而回至并穿过所述导管轴,使得拉动所述拉线可引起所述拉环使所述导管轴弯曲并使所述拉环往所述尖端元件方向移动。
2.如权利要求1的医用导管组件,其中所述安装轴包括一个具有在其内穿过的轴向延伸通路的向外延伸的环状凸缘,通路包含所述导沟。
3.如权利要求2的医用导管,其中所述安装轴包括一个接至所述尖端元件的远端部分和一个伸入所述导管轴的近端部分,其中所述向外延伸的环状凸缘与所述远端部分为一整体,并且其中所述近端部分包括表面突出部以夹持所述导管轴。
4.如权利要求3的医用导管组件,其中所述表面突出部包含倒钩。
5.如权利要求4的医用导管组件,其中轴向槽与轴向通路对齐设置在所述倒钩内并包含所述导沟。
6.如权利要求1的医用导管组件,包括一个压缩环,将所述导管轴的一个远端部分紧压在所述安装轴的一个外表面之上。
7.如权利要求6的医用导管组件,其中所述安装轴的所述外表面包括位于其内的轴向槽,轴向槽包含所述导沟。
8.如权利要求7的医用导管组件,其中所述压缩环具有大体为矩形的横截面。
9.如权利要求7的医用导管组件,其中所述压缩环具有一个向内面朝所述安装轴的大体为平的表面和一个大体为凸面的面朝外的表面。
10.如权利要求1的医用导管组件,其中所述尖端元件界定外侧环状凸缘,所述拉环位于所述导管轴和所述外侧环状凸缘之间。
11.如权利要求1的医用导管组件,其中所述远端尖部包括两条位于其径向相对侧上的导沟。
12.如权利要求11的医用导管组件,其中所述拉环组件包括两条连接至所述拉环上径向相对位置的拉线。
13.如权利要求1的医用导管组件,其中每条拉线沿其长度方向至少有一部分是平的。
14.如权利要求1的医用导管组件,其中每条拉线都接至所述拉环的内表面上。
15.如权利要求14的医用导管组件,其中每条拉线都通过焊接接至拉环上。
16.一种消融导管组件,其包括:
一根界定一个远端部和一个中空内部的可偏转导管轴;
一个位于所述导管轴的所述远端处并包括一个尖部电极和一个伸入所述导管轴的安装轴的远端尖部;以及
一个包含一个拉环和连接至其上的拉线的拉环组件,所述拉环位于所述安装轴的周围;
所述远端尖部设有多条导沟,所述拉线通过导沟被分别引导,初始从所述拉环向所述尖部电极延伸,然后环路而回至并穿过所述导管轴,使得拉动所述拉线引起所述拉环使所述导管轴弯曲将会引起所述拉环与所述尖部电极联锁。
17.如权利要求16的消融导管组件,其中所述远端尖部包括一个位于所述尖部电极和所述安装轴之间的液体歧管,而且其中所述每条导沟在所述安装轴中都包括一个弯曲部分和一个直线部分。
18.如权利要求17的消融导管组件,包括两条位于所述远端尖部的径向相对侧上的导沟,而且其中所述拉环组件包括两条连接至所述拉环上径向相对位置上的拉线。
19.如权利要求17的消融导管组件,其中所述安装轴和所述尖部电极为一个整体。
20.如权利要求16的消融导管组件,其中每条拉线沿其长度方向至少有一部分是直的。
21.如权利要求16的消融导管组件,其中每条拉线被接至所述拉环的内表面上。
