这里讨论的公开物,只是其公开先于本申请的申请日。但这并不是表示说许可本发明无权由于那些在先发明提前这种公开的日期。更进一步,所提供的公开物日期可能和实际公开的日期不同,其可能需要分别确认。
虽然本发明主要描述其在糖尿病医治中的应用,特别是在胰岛素配送以及葡萄糖浓度测量方面的内容,应该明白的是本发明不仅仅局限于此,还可以被应用到任何药物配送应用中,尤其是当药物配送方式为自我管理的情况。其它应用包括有,比如荷尔蒙增长管理,哮喘药物治疗,血液稀释剂等。
现在参照附图,图1阐明了本发明的系统2的示意性表示,其包括储存/运载箱或容器10,用于提供各种患者使用的设备和元件,进行对葡萄糖水平的自我监测和对胰岛素的自我管理。特别地,所述系统还包括葡萄糖测量计11,一个或更多药物配送设备12和13,例如胰岛素注射笔或者胰岛素注射器。测量计11可以是任何一种具有所述功能的合适尺寸的测量计。所述药物配送设备可以具有相同或不同类型。本领域普通技术人员将会辨认出可包括在箱10中的任何数目的药物配送设备。
箱10可以由柔软、易弯的材料制成,包括皮革、乙烯基、塑料或者其它为本领域普通技术人员所已知的合适材料。可替换的是,箱10可以用刚性的材料制成,例如注射模制塑料。箱10的闭合可以通过拉链、搭扣、拴钩、挂钩或者其它合适的安全装置。测量计11可以通过吊钩和绳扣(例如维可牢尼龙搭扣)、绳圈、黏合剂或者其它合适装置将其紧固在箱10内,以便用户可以容易地从箱10中取出测量计。箱10可以配置成附于其它包含相同或相似设备和供应的组合箱或辅助箱上11。此类辅助箱有助于在旅行中确保能随时取到供应品和用于自治的药品。
可选地,箱10可以进一步地包括隔室或者容器,用于容纳各种用于存取或采集血液或体液样本的设备和元件,例如切割机构14,柳叶刀19和针24。箱10可以进一步地包括诸如测试带18的设备,用于将存取或采集的血液或体液样本传送和提供至测量计11内。
箱10配置成为在置于箱10中的某些元件之间提供通讯。例如,箱10可以包括检测装置21,用于检测药物配送设备12和/或13是否在箱10中或者从中移出。检测设备21可以是开关或者传感器,例如机械、磁性、浓度、电容开关、电或者光传感器,诸如红外线传感器。
检测设备21可如图所示进行配置,或者将其嵌入提供有诸如弹簧固定结构的紧固装置的托架、空腔或者支撑台中,用于将药物配送设备夹持在箱10内。例如,红外线发射器和检测器可以分开地安装在笔托架的相对侧上。如果注射笔在托架中,则发射器发射出的红外束会因为受阻而不被红外检测器检测到。当注射笔不在托架中时,红外光线将被检测器所检测,信号被发送给箱10或者测量计11用于指示。
可替换地,如图2所示,所述检测装置可被配置为用于检测设备12和/或13与箱10或测量计11之间的邻近程度。例如,所述检测装置可以包括可揭除地贴于或附于所述药物配送设备12上的邻近度标签25或26,以及测量计11上所提供的相应的邻近度标签27。所述邻近度标签可以是具有粘性背面的磁性标签,或者其底板(bay)能够粘贴在药物配送笔12上的其它传感器类型。测量计11具有相应电子线路,用于检测胰岛素配送笔12相对于测量计11的位置。因此,胰岛素配送笔12的移动可以通过测量计11进行检测和监视。例如,当注射装置被移动了一个特定的距离,比如离开测量计6英寸或者更多时,测量计11将会被触发。本实施例的一个优点正是在于药物配送笔12不一定要贴在箱10上,而与此同时,使用药物配送笔12的始点仍然可被监测。
所述药物配送设备检测装置还可以包括射频发生装置(RF),其组合在药物配送笔10内,或者置于所述笔的笔帽下,当笔帽从所述笔上取走时,射频信号被发送到带有射频接收装置的箱10或测量计11上。该动作被记录下来,并与自我监测信息相互作用(以图形绘出)。本实施例减少了要求患者完成激活动作的步骤,而且同样地,减弱了对所描述的治疗计划的依赖性。
以所述检测装置21的任一种配置形式,所述开关或传感器可以进行通讯地耦合在测量计11和/或微处理器,以及装在箱体本身中的记忆芯片等,其中测量计11和所述微处理器/记忆芯片也被电耦合,例如通过一系列的通讯端口或者电连接23。可选择地,测量计11可以集成在箱10中,两者共同构成个人数字处理设备(PDA)。
所述药物注射器预置需要进行配送的药物,另一个传感器被用于自动确定从注射器中分配出的药物量。通过该方法,解除了患者进行日志记载和信息记录的负担。
箱10可以进一步装配有测量计检测器(未示出),用于检测测量计11在箱10中和从箱10中的移出,并且同样地,用于确定患者是否在服用胰岛素之前进行了葡萄糖的测量。通过该方法,可以发送给用户适当的提示和询问。所述测量计检测器可以是单杆机械开关,其安装在其中带有测量计11的箱10部分中。
箱10可以进一步包括传感器(未示出),用于检测箱盖是打开还是闭合。可以使用两个柔性的磁性带将合适的传感器安装在箱盖的背面。当箱关闭时,触发该系统进入到“睡眠”模式以省电。
箱10可以进一步包括能够使箱、测量计和/或PDA以及箱10外部的设备或系统16之间进行通讯的装置。所述外部设备16的例子包括手机、PDA、计算机、手表、寻呼机、打印机,或其它任何能够接收和/或传输无线通信的设备。为产生上述效果,无线通信设备15集成在所述箱内,将数据传输到外部设备16并且可能接收来自这些外部设备16的数据。更进一步地,无线通信设备15可以具有与测量计11和/或药物配送设备12、13交换数据的能力。无线通信设备15可以通过诸如射频(RF)信号(例如,蓝牙链接)发送信息。
在使用中,系统2可以配置成根据图3所示的流程图给出的一个或更多的流程进行操作。至于如图3所示的流程30,当药物配送设备12、13从箱10中移出或者移动了一个特定的距离(步骤32),检测器或传感器21向测量计11发送或者传输信号用于指示药物配送设备已被移出,并且优选地,创建(crating)该事件发生的时间标记记录。这又触发测量计11开启,并且向用户显示画面(通过显示器22)和/或给出声音(通过麦克风,未示出)的提示或者询问(步骤34)。例如,用户可能会被询问或者被提示进行葡萄糖测试或输入药物配送信息。如果用户通过测量计11进行了葡萄糖测量,则测量计11提示给用户需要注射胰岛素的建议剂量,其建议剂量基于卫生保健从业者(HCP)输入的算法和/或其它由用户预先或当前输入到测量计中的参数而得(步骤36)。用户可以根据所述建议剂量,也可以不根据所述建议剂量,自行使用药物配送设备12进行药物配服。用户可以根据实时情况选择用药剂量,而不采用上述的建议剂量。例如可能发生这样的情况,当糖尿病患者预期或者典型的碳水化合物摄取量发生了变化,比如,在吃饭的时候,患者发现所摄取的碳水化合物多或者少于他/她在饭前的预期摄入量。在其它时候,用户可能没有选择进行葡萄糖测量,但仍然进行了一定剂量胰岛素的用药。这种情况当患者的血糖水平在任何给定的时间都可预知的情况下可能发生,例如饭后。如果不进行葡萄糖测量,测量计11将会询问用户是否进行胰岛素的用药,如果用户选择是,那么要求用户输入药物剂量信息并进行确认(步骤38)。为了便于用户输入药物剂量信息,所述系统可以配置为初始状态显示一个预设的数字读取,例如,一单位的胰岛素,优选地,用户可以通过测量计11或者PDA上的上下按键调整所述的单位值。一旦输入了值,测量计11将剂量数据存储到测量计的存储器中。随后,所述剂量数据将会自动地或者被用户提示地,通过无线通信设备15传输到外部设备16。当药物配送设备12、13放回箱10中时,测量计11可被触发而关闭。
如图3所示的流程图40,系统2和测量计11可以配置为通过用户激活开关进行打开和关闭(步骤42),并且允许用户进行多种功能(步骤44),例如检查可用的系统存储器,下载数据、算法或程序,发送和输入数据,以及定制或格式化测量计或PDA。如流程图50所示,测量计11也可以配置为通过将测试带18插入测量计11而启动(步骤52),这又触发类似流程图30给出的流程步骤(步骤54-64)。
图4是本发明的葡萄糖监测系统的示意图,所述系统包括有胰岛素笔100,笔帽110和葡萄糖测量计120。其中胰岛素笔100和笔帽110的外壳由耐久IM塑料制成,用于抵抗耗损和裂缝,以及各种普通的家居清洁剂(漂白剂和酒精)和使用者的化学物(手的湿气)。胰岛素笔100包括射频收发单元(以下称为射频Tx/Rx单元130),本地微控制器140,电源150和事件开关160。胰岛素笔帽110可以与胰岛素笔100压合或者螺纹配合。胰岛素笔帽110被设置为当其从胰岛素笔100上移出时,启动事件开关160。本地微控制器140还可以包括存储元件。另外,本地微控制器还可以控制无线电发送、定时、电源管理、事件存储和本地电子线路(例如事件开关160)。事件开关160可以是机械开关,当其被激活时关闭电路。当胰岛素笔帽110从胰岛素笔100上移出的时候,产生激活。在激活过程中,可以由胰岛素笔100发射射频信号到葡萄糖测量计120。事件开关160的激活可以被本地微控制器140记录并存储下来。本领域技术人员能够知晓许多通过用户的动作(例如压力释放)激活开关的例子。电源150为射频Tx/Rx单元130、本地微控制器140和事件开关160提供电能。电源150通过可集成也可不集成在胰岛素笔帽110中的接触,与各种元件电耦合。
射频Tx/Rx单元130可以包括收发器,例如Chipcon CC1000收发器和使用频率为433MHz FM频带的环线天线。一个例子是,合适的电源150选用Silver Oxide RayOVac 357。一个非局限的例子是,事件开关160可以选用松下公司的按钮开关EVQ-PAC04M。
在一个可替换的实施例中,减少了胰岛素笔100的电子元件,射频Tx/Rx单元130和本地微控制器140可以结合成一个单独封装件,其中所用的存储器是外部EEPROM。非局限的例子是,一体化射频Tx/Rx单元130和本地微控制器140包括有德克萨斯仪器混合信号处理器(MSP)或者Chipcon CC1010。
在另一个可替换的实施例中,减少了胰岛素笔100的能量消耗,本地微控制器140和电源150可以被射频识别标签(RFID)代替。为了与RFID进行信息传输,葡萄糖测量计120需要足够强的RF信号发生装置以为RFID供能。此外,胰岛素笔帽110仍然需要原位能量发生器取代电源150。当胰岛素笔帽110从笔100上移出时,该移出的动作用于产生供应给胰岛素笔100的能量。
葡萄糖测量计120也可以包括电子日志和射频收发器装置(与射频Rx/Tx单元130相类似)。在一个实施例中,葡萄糖测量计120可类似于市场上可买到的LifeScan One测量计。通过射频协议进行葡萄糖测量计120与胰岛素笔100的通讯。葡萄糖测量计作为“主端”,而胰岛素笔100中的电子器件作为“从端”。葡萄糖测量计120周期性地提示胰岛素笔100用于确定事件是否已经被记录,例如配送笔从箱中移出或者笔帽从笔上移出。如果事件已经被所述从端记录,则发送消息至葡萄糖测量计120,指示事件的发生及该事件发生的时间。葡萄糖测量计120记录该事件,并可以询问患者进一步的信息和给出通知。
在本发明的一个实施例中,用户可以将胰岛素笔笔帽110从胰岛素笔100上移出。这样操作之后,胰岛素笔100发送射频信号至葡萄糖测量计120指示用户将开始胰岛素注射。然后,葡萄糖测量计120在液晶显示器(LCD)屏幕上指示用户其最临近的餐前和餐后的血糖水平。葡萄糖测量计120提示用户确保这些值正确,如果不正确,葡萄糖测量计120允许用户对这些值做相应地调整。在本发明的一个实施例中,葡萄糖测量计120可以进行餐前追加胰岛素(bolus)算法的计算,在葡萄糖测量计120的液晶显示屏上显示使用胰岛素笔100注射的推荐胰岛素剂量。根据这个情况,用户可以注射所推荐胰岛素剂量,或者注射不同的剂量。葡萄糖测量计120可以提示用户输入实际注射的胰岛素的剂量,用于列出胰岛素笔100中可取用的剩余胰岛素的目录。当胰岛素笔中的胰岛素量较少时(即剩余10%),葡萄糖测量计120可以提示用户根据需要,获取附加的胰岛素笔。在本发明的另一个实施例中,葡萄糖测量计120可以提示用户在事件开关被激活之后,输入其它类型的信息,例如早餐、午餐、晚餐、零食、酒精、碳水化合物、脂肪、卡路里、蛋白质、药物、药丸、胰岛素、泵送餐前追加胰岛素(pump bolus)、健康记录、酮、糖化血红蛋白(HBA1C)、微蛋白、胆固醇、血压以及运动量。
本发明的一个优点在于,用户无需手动记录使用胰岛素笔100进行胰岛素注射的时间。这简化了用户对数据收集的操作,更有效地管理疾病。本发明进一步的优点在于,起动开关160发送无线信号到葡萄糖测量计120,提示用户进行一系列的步骤,从而促进用户对其糖尿病疗法的顺应性。葡萄糖测量计120可以不间断地显示最近的和最相关的葡萄糖浓度,提示用户更新的血糖浓度,根据现有血糖测量结果推荐合适的胰岛素注射剂量,或者提示用户输入其它相关数据。
在本发明的一个可替换实施例中,胰岛素笔100可以被装有药物(诸如Glucotrol的药物)的“药瓶”代替。在所述实施例中,所述“药瓶”包括Tx/Rx单元130,本地微控制器140,电源150和事件开关160。此外,然后必须用药瓶帽取代胰岛素笔帽110。
本发明还提供一个用于实践所述方法的工具箱。该工具箱可以进一步地包括记录在CD-ROM或者类似载体上的软件程序,其中这些程序可以下载到所述测量计、PDA和/或与系统一同使用的外部数据设备。最后,该工具箱可以进一步包括本发明设备和系统的使用说明书。这些说明书可以出现在包装、标签嵌入物或工具箱内的容器中的一个或多个上,或者也可以通过CD-ROM或类似载体提供。
前述仅仅阐述了本发明的基本原理。本领域熟练技术人员可以使用本发明的基本原理,根据本发明的实质和范围,设计出各种各样在本申请中没有明确描述的组合。更进一步地,本申请中叙述的所有例子和条件用语,都是为了帮助读者理解本发明的基本原理和发明者在促进本领域发展中贡献的概念,并且应当理解为不局限于所述明确列举的例子和条件。此外,本申请中的所有关于原理、特征以及本发明的实施例和具体示例,都包括了具有类似结构和功能的等效物。另外,所述的等效物包括了现有已知的和将来可能发展出的等效物,也就是说,能够实现相同功能的发展出的元件,而与结构无关。因此,本发明的范围不局限于本申请中记载的示例性实施例。相反,本发明的范围和精神根据所附权利要求而体现。
Claims (10)
1.一种药物配送系统,包括:
测量计;和
药物配送设备,被配置成将药物配送给用户,所述药物配送设备包括:
本地微控制器,其包括存储器单元;
射频收发器,连接至所述本地微控制器以便将数据发送至所述测量计,以及
事件开关,耦合至所述本地微控制器以使得一旦所述开关被激活,所述药物配送设备与所述测量计通信以使所述测量计指示所述用户最近的血糖测量,并基于卫生保健从业者HCP输入的算法和/或其它由用户预先或当前输入到所述测量计的存储器中的参数提供药物剂量建议。
2.一种药物配送系统,包括:
测量计;和
药物配送设备,被配置成将药物配送给用户,所述药物配送设备包括:
本地微控制器,其包括存储器单元;
射频收发器,连接至所述本地微控制器以便将数据发送至所述测量计,以及
事件开关,耦合至所述本地微控制器以使得一旦所述开关被激活,所述药物配送设备与所述测量计通信以使所述测量计指示所述用户最近的血糖测量,执行餐前追加胰岛素计算,以及指示推荐的药物水平。
3.如权利要求1和2中任意一项所述的药物配送系统,进一步包括显示屏,被配置成显示所述用户最近的血糖测量。
4.如权利要求3所述的药物配送系统,其中,所述最近的血糖测量包括最近的餐前和餐后的血糖水平中的一个。
5.如权利要求1和2中任意一项所述的药物配送系统,其中,所述本地微控制器被配置成记录所述事件开关激活的日期和时间。
6.如权利要求1和2中任意一项所述的药物配送系统,其中,所述测量计被配置成在所述事件开关被激活之后提示所述用户输入其它类型的信息。
7.如权利要求6所述的药物配送系统,其中,所述其它类型的信息包括早餐、午餐、晚餐、零食、酒精、碳水化合物、脂肪、卡路里、蛋白质、药物、胰岛素、酮、HbA1c、微蛋白、胆固醇、血压或运动量。
8.如权利要求1和2中任意一项所述的药物配送系统,其中,所述事件开关包括从以下组中选出的开关,所述组包括:机械开关、磁开关、浓度开关、电容开关、电传感器、光传感器。
9.如权利要求1和2中任意一项所述的药物配送系统,其中,所述药物配送设备包括胰岛素笔。
10.如权利要求1和2中任意一项所述的药物配送系统,其中,所述系统还包括微处理器设备,所述微处理器设备包括从以下组中选出的设备,所述组包括:手机、个人数字助理、计算机、手表或寻呼机。
