背景技术
糖尿病肾病临床高发，但目前尚无理想的防治药物；现在国内外防治糖尿病肾病的措施主要有：低蛋白饮食，戒烟，降糖药物及胰岛素控制血糖，他汀类降脂药，血管紧张素转化酶抑制剂(ACEI)，血管紧张素II受体阻断药(ARB)类控制血压，现有西药虽然有一定的肾脏保护作用，但其肝毒性，肾毒性亦不容忽视，而且这些措施不能对因治疗，治疗作用单一，缺乏对血压，血脂，血糖的联合治疗，远期疗效不佳；基因治疗、干细胞移植还不成熟且费用昂贵，尚在实验和临床试用阶段。目前为止，国内及国际临床上尚没有理想的药物能应用于糖尿病肾病的治疗。

本发明的中药组合物，临床上用于糖尿病肾病的防治，用于肝肾阴虚、脾肾气虚，兼有水湿、湿热、瘀血，正虚邪实并重，症见腰背肢体酸疼沉重、肌瘦无力、纳后腹胀、畏寒肢冷、手足浮肿、大便常溏、舌苔白滑或腻见上述症候者。
本发明的中药组合物，其降糖、降脂、改善血循环、减少蛋白尿，临床疗效确切；作用机理清楚，已证实本发明的药物可以降低一氧化氮合酶的表达，从而减少一氧化氮的生成，改善早期的高灌注、高滤过状态，对糖尿病肾病有显著的预防和治疗作用。
本发明的治疗糖尿病肾病的中药组合物的制备方法，可以采用以下方法进行制备，或者采用其它方法制备。
A.称取处方中的原料，置于多功能提取罐中，加适量水，浸泡30分钟；继续加 水加热煎煮2次(第一次加8倍量水，第二次加5倍量水)，每次煎煮2小时，过滤，合并滤液；
B.将滤液静置24小时，取上清液，加热浓缩至相对密度1.10(60℃测)，浓缩清膏过80目筛，喷雾干燥(进风温度160～175℃，出风温度85～95℃)；
C.将所得干浸膏粉用振荡筛整理，过80目筛；另将过筛所得残料用高效粉碎机粉碎，仍用振荡筛整理，过80目筛，将过筛所得药粉合并，用三维混合机混合30分钟，分装成3g/袋，贴签，用环氧乙烷消毒5小时，即得。
用法与用量：口服，一日3次，每次2袋。
本发明产品与现有技术产品相比具有以下特点：
(1)治疗效果显著，毒性实验表明，无毒副作用。
(2)成本低，用量少，价格低廉。
具体实施方式
实施例
1)按重量比例取牡丹皮1份、泽泻1份、山茱萸2份、熟大黄2份、枸杞子2份、云苓1份、肉桂1份、山药4份、猪苓2份、马齿苋4份、地黄2份、黄芪6份、红花1份、桃仁1份、路路通2份，共15味药置于多功能提取罐中，加适量水，浸泡30分钟；继续加水加热煎煮2次(第一次加8倍量水，第二次加5倍量水)，每次煎煮2小时，过滤，合并滤液；
2)将滤液静置24小时，取上清液，加热浓缩至相对密度1.10(60℃测)，浓缩清膏过80目筛，喷雾干燥(进风温度160～175℃，出风温度85～95℃)；

取上述实施例制得的药物，进行以下动物实验。
1实验动物
健康纯种Wistar大白鼠共120只，雌雄各半，体重230±20g，由山东大学医学院实验动物中心提供。
2实验饲料
饲料组成为：22％脂肪、52％蔗糖、10％蛋白质以及维生素、纤维素和盐等，热量为4.8Cal/g。
3实验药品
组合物颗粒剂：每袋3克，由本发明实施例提供。
糖适平(Gliquidon)：30mg/片，北京第六制药厂与德国勃林格宜格翰国际公司合作生产。批号：011204
洛汀新(Lotensin)：10mg/片，由北京汽巴-嘉基制药有限公司提供。批号：011600
4糖尿病肾病模型大鼠复制方法
购回大鼠后，适应性喂养一周，随机分为正常对照组30只，实验组90只，实验组于造模前禁食10小时，将链脲佐菌素溶于0.1mmol/L，PH4.4的枸橼酸缓冲液中，采用STZ50mg/kg一次性腹腔注射，同时喂养高脂高糖半合成高热量饲料，2周后测定空腹血糖，血糖值超过16.65mmol/L者，确定为模型形成。
5动物分组及饲养
将实验组大鼠90只随机分为3组，每组30只，分别为：糖尿病模型组，西药组(糖适平和洛汀新组)，中药组(组合物颗粒剂组)，和正常对照组共为4组，处理方法：
正常组：喂正常饲料，生理盐水。
模型组：喂正常饲料，生理盐水。
西药组：糖适平和洛汀新分别研磨溶于生理盐水中灌胃，前者剂量为30mg/kg/d，每日一次；后者6mg/kg/d，每日一次，喂正常饲料。
中药组：灌服组合物颗粒剂，剂量为1.5g/kg/d，每日一次，喂正常饲料。
6疗程：8周为一个疗程。
7指标测定
空腹血糖、血脂、尿微量白蛋白、血浆内皮素、一氧化氮合酶。
测定一氧化氮合成酶(iNOS)：(1)提取RNA：大鼠腹腔麻醉后，用冷生理盐水灌洗肾脏，待肾脏苍白后即刻取肾，提取细胞总RNA，用紫外分光光度计(DC-7型，Bechman公司)鉴定RNA纯度和量。(2)逆转录：采用逆转录试剂盒，取所提取部分RNA作为cDNA模板，以Oligo(dT)15为引物，按试剂盒操作程序进行实验。(3)将逆转录产物进行扩增，并经琼脂糖凝胶电泳，进行吸光度扫描。
血浆内皮素(ET)：(1)取动脉血2ml，放入含乙二胺四乙酸和抑肽酶的预冷试管中，混匀，离心，取上清液于-20℃保存待测。(2)采血后剖腹取肾，称重后取肾皮质置于预冷的冰醋中研磨，100℃水浴中10分钟，匀浆，离心，取上清液-20℃保存，按ET放免药盒测定含量。
8统计学处理方法
所有结果采用(x±s)表示，数据统计采用SAS统计学软件进行分析和t检验。
实验结果
(一)对大鼠尿微量白蛋白的影响
表1各组大鼠治疗前后尿微量白蛋白排泄率(ug/min)的变化
*与治疗前比较P＜0.05，**与治疗前比较P＜0.01，☆中药组与西药组相比较P＞0.05，△与对照组相比，P＜0.01。
由表1可以看出，模型形成后，各组尿微量白蛋白的排泄率明显增高，与正常组比较有显著性差异(P＜0.01)，说明造模成功。治疗8周后，中药组和西药组与治疗前比较均有显著性差异(P＜0.05)，表明中药组和西药组均可减少尿微量白蛋白，但两者比较无显著差异(P＞0.05)。
(二)对大鼠血糖和糖基化血红蛋白的影响
表2各组大鼠治疗前后血糖(mmol/L)的变化
*与治疗前比较P＜0.05，**与治疗前比较P＜0.01，☆中药组与西药组相比较P＞0.05；
从表2可以看出，中药组治疗后与治疗前相比，血糖有明显降低，统计学有显著差异(P＜0.05)；西药组治疗后与治疗前比较血糖有非常显著差异(P＜0.01)，说明中药组和西药组对糖代谢均有改善作用，中药组与西药组两者之间比较均无显著差异(P＞0.05)。
(三)对大鼠血脂的影响
表3各组大鼠治疗前后胆固醇(mmol/L)、甘油三酯(mmol/L)的变化
*与治疗前比较P＜0.05，☆中药组与西药组比较P＜0.05，▲西药组与治疗前相比较P＞0.05。
从表3可以看出，治疗8周后，中药组与治疗前以及模型组比较均有明显降低，统计学处理有显著性差异(P＜0.05)，西药组与治疗前以及模型组相比血脂无显著改变(P＞0.05)，说明中药组能改善糖尿病大鼠血脂水平，而西药则无此作用，两组间相比较有也有显著性差异(P＜0.05)。
(四)对大鼠血浆内皮素的影响
表4各组大鼠治疗前后血浆内皮素水平的的变化(pg/ml)
*与治疗前比较P＜0.05；☆中药组与西药组比较P＞0.05.
由表4可以看出，模型组大鼠ET水平随着病程延长而逐渐增高，与治疗前相比，有显著性差异(P＜0.05)；而中药组与西药组ET均较治疗前下降，统计学处理有显著差异(P＜0.05)，中药组与西药组两组之间相比，疗效无显著差异(P＞0.05)。
毒性实验
1试验材料
1.1动物 昆明种小鼠，体重18-20g，雌雄各半。山东大学医学院实验动物中心提供。
1.2受试药物 组合物颗粒剂，本发明实施例提供。实验时加适量蒸馏水，制备成相当于生药4.5g/ml。
2方法与结果
每毫升相当生药4.5g，按小鼠最大灌胃容量(0.4ml/10g)计算剂量为180g/kg，对小鼠进行一次性灌胃，给药后常规饲养，连续观察7天，20只小鼠7天内无一死亡及中毒。按公斤体重计算，相当于成人剂量(相当每日生药0.48g/kg)的370倍；按动物系数折算，相当于70kg成人剂量的40倍，结果表明本组合物颗粒剂口服的安全性很大，小鼠口服给药的LD50大于180g/kg。
临床实验结果：
46例糖尿病肾病患者随机分为2组，治疗组口服8周，每日3次，每次2袋(规格3g/袋，本发明实施例提供)。测定治疗前后血糖、24h尿白蛋白排泄率、血脂、血浆内皮素。结果治疗组较对照组血糖、24h尿白蛋白排泄率、血脂、血浆内皮素显著降低(P均＜0.05)。说明该组合物颗粒剂可以用于糖尿病肾病的防治。
Claims (1)
1.一种治疗糖尿病肾病的中药组合物，其特征在于是由以下重量份的原料制成：
牡丹皮0.8-1.2份、泽泻0.8-1.3份、山茱萸1.5-2.4份、熟大黄1.6-2.6份、枸杞子1.5-2.5份、云苓0.7-1.4份、肉桂0.5-1.5份、山药3.4-4.6份、猪苓1.6-2.2份、马齿苋3.6-4.4份、地黄1.5-2.5份、黄芪5.3-6.4份、红花0.7-1.4份、桃仁0.7-1.3份、路路通1.6-2.5份。
