Abstract

技术领域
本发明涉及提高琼脂糖包被的含有分泌细胞的琼脂糖大珠(macrobead)的质量和数量的方法。这是通过使用下文所述的思科姆金(Seakem Gold)琼脂糖实现的。
背景技术
现在已确定,胰岛置换疗法是一种治疗多种疾病患者的可行方法。其中包括接受上腹部脏器摘除术的癌症患者(Tzakis,等,Lancet,336:402-405(1990));胰腺炎(Clayton,等,Transplantation,76:92-98(2003);Farney,等,Surgery,110:427-437(1991);Fontes,等,Transplant Proc,24:2809(1992);Obenholzer,等,Transplantation,69:1115-1123(2000);Robertson,等,Diabetes,50:47-50(2001))和胰岛素依赖性患者,在此胰岛移植是一种治疗选择(Goss,等,Transplantation,74:1761-1766(2002);Ricordi,等,Transplantation,75:1524-1527(2003);Ryan,等,Diabetes,50:710-719(2001);Shapiro,等,N.Engl.J.Med,343:230-238(2000))。

制备含有分泌细胞和/或细胞器如癌细胞、胰岛等的大珠是公知的。参阅如美国专利No.6,818,230;6,808,705;RE 38,027;6,303,151;6,224,912;5,888,497,和5,643,569,以及已公开的美国专利申请2005/0096561,所有这些均通过引用并入本文。
在这些专利文献中使用琼脂糖包裹生物材料,之后将所得的结构再包裹一层琼脂糖。
熟悉琼脂糖的技术人员会了解,此材料有许多可用的类型和种类。一种这样的琼脂糖思科姆金描述于美国专利No.4,983,268,其公开内容通过引用并入本文。
目前需要上文列出的专利和申请中首先描述的材料的改进形式。现在发现,思科姆金琼脂糖得到的产品出人意料地优于现有技术产品。

优选实施方案
实施例1
根据Gazda等的公开专利申请2006/0121445中所述方法分离胰岛,该申请公开于2006年6月8日,序列号11/273,737,其通过引用整体并入本文。但是,应该注意,其它分离胰岛的方法也是可能的,并且可以使用,因为本发明不依赖于特定的分离方法。
在分离和评价之后,进一步处理合适的胰腺。剪下腺体上的脂肪和结缔组织,然后将16g不锈钢钝头针插入主胰管。在30℃下以50ml/mm的速率输注含有浓度为1.5-2.0g/l的胶原蛋白酶P的HBSS溶液,以为每克胰腺提供2ml溶液。
接下来,在30℃下,用500ml HBSS和2%PS与200ml胶原蛋白酶溶液一起浸没胰腺。使用39℃的外循环水将该器官缓慢升温到37℃,并保持消化温度为36~37℃。当该器官看起来已解离、并对手施加的压力的抵抗力很小时(约10~20分钟总时间之后,在达到37℃之后大约5~10分钟),终止消化。
收集消化物并离心,吸去上清,将所得沉淀悬浮在10%PS和器官保存溶液中。接着,在50ml试管中,以密度梯度为1.105、1.095、1.085、1.05g/cm3的不连续Ficoll、HBSS加2%PS纯化胰岛。4℃下以650g离心试管,收集含有胰岛的层,并用添加10%PS的HBSS溶液洗三遍,其后借助解剖显微镜将它们从非胰岛组织中手工纯化出来。重悬这些胰岛,取两份0.5ml的样品用于计数胰岛产量。
所测试的10个胰腺的平均产量为130,000EIN,平均每克消化后的组织为1,101EIN。所有样本的纯度均高于90%。其中9个器官的胰岛生存力高于89%。
实施例2
在分离胰岛之后,测定多种参数,包括纯度和生存力,如上文所述。
通过用DTZ对约500EIN的胰岛染色10分钟来测定纯度,然后使用解剖显微镜和数码相机进行标准图像分析。
通过用二乙酸荧光素(FDA)和溴化乙锭(EB)染色样品来测定生存力。详细地说,将约500EIN加入1ml RPMI、10%PS、1%抗生素/抗霉剂(“A/A”)中。接着加入由10mg FDA和1ml丙酮制成的FDA染色剂20μl、由30μl EB和1ml PBS制成的EB 200μl。将胰岛在黑暗中染色7分钟,然后在荧光显微镜下观察10-50个胰岛的随机样品并拍照,以使用标准图像分析来测定其生存力。
还通过将约500EIN置于酸醇提取液(7.2ml 1N盐酸,400ml100%变性乙醇)中来测量胰岛的胰岛素含量。将所述样品保存在-20℃,并实施胰岛素RIA。

实施例3
本实施例及其后的实施例将阐明如上所述鉴定为有用并已分离的胰岛是否能用于大珠中。
将纯化的胰岛重悬在RPMI 1640+10%PS+1%A/A中,至体积为2000EIN/ml。将这些胰岛平均分配在管中,使得每管包含2000EIN的1ml悬液。
经重力沉降后,去除上清液,向每个样品中加入0.5ml 50℃的在极限必需培养基加2.5%HEPES缓冲液中制备的1.5%思科姆金琼脂糖并均匀搅拌。接着,将悬液在无菌矿物油下排出,形成四个具有光滑表面和相同胰岛分布的珠。
取出大珠,洗两遍(RPMI+5%PS+1%A/A)。将这些大珠在加湿的5%二氧化碳气氛中在同样的溶液中培养5~7天,之后用RPMI+1%A/A洗三遍,接着施加第二琼脂糖包被层。为此,用移液器将0.5ml 60℃的MEM加HEPES缓冲液中的5%琼脂糖转移到无菌塑料勺中,将每个大珠滚动3~5次以产生均匀的第二琼脂糖包衣。在转移进无菌矿物油以产生光滑表面之后,取出大珠,用RPMI+2.5%PS+1%A/A洗两遍,在37℃下在加湿的5%二氧化碳加空气中培养。当然也可采用其他制备所述珠的方法。
包含被包裹胰岛的大珠经测定可以保持生存力超过六个月,在此期间,放射性免疫测定显示它们能继续产生高水平的胰岛素。
实施例4
本实施例描述了证明猪胰岛能在体内发挥功能的实验。
采用7~9周龄的雄性非肥胖型糖尿病CB17-PrKdc<scid>/J小鼠。适应一周后,动物接受诱导糖尿病的275mg/kg链脲霉素。9天后,当其血糖水平平均超过480mg/dl时,开始进行胰岛素疗法。
在施用链脲霉素后的34~35天,动物接受大约1000EIN的猪胰岛,所述胰岛按照Bowen,等,Aust.J.Exp.Biol.Med.Sci.,58:441-447(1980)中的方法在血块中移植,该文献通过参考并入本文中。简言之,将胰岛从悬液中沉淀下来,吸去培养基。然后,从动物体内取约5~10μl血,加入胰岛中,并使之凝结。用等体积的氯胺酮(167mg/dl)、甲苯噻嗪(33mg/ml)和盐水麻醉受体动物。以0.5ml/100g的剂量皮下施用所述混合物。然后,在左肋切开一个小口以暴露肾,利用解剖显微镜在肾囊上切开一个小口,将肾囊与肾分离开,将胰岛/血块置于囊下,缝合切口,使动物复原。
在移植38~39天后对动物施行肾切除术。简言之,在麻醉之后,暴露带有移植物的肾,结扎肾血管,摘除每个动物的肾。5天之后处死动物,收集胰腺用于在组织学上证实胰岛β细胞完全破坏。
将组织样品在10%中性缓冲的甲醛溶液中放置24小时,然后转移到70%乙醇中。
接下来,用石蜡包埋组织,用苏木精和伊红染色5μm的切片。使用标准方法对胰腺和带移植物的肾的切片针对含有胰岛素和胰高血糖素的细胞进行染色,然后进行研究。
所有小鼠均在胰岛移植后血糖恢复正常。在肾切除术后,所有小鼠在四天内均变成高血糖。
实施例5
此实施例描述了设计用于确定琼脂糖-琼脂糖包被的珠(由不同琼脂糖制成)在受体动物中引起组织反应(即炎症)程度的实验。
使用两种大鼠株系,即Wistar和Sprague Dawley大鼠。总共测试了29只大鼠(14只Wistar,15只Sprague Dawley)。使用每个株系的3只大鼠测试本发明思科姆金琼脂糖-琼脂糖包被的珠。使用每个株系的3只大鼠测试琼脂糖类型FMC HGT(P)和Amresco。使用2只Wistar大鼠以及4只Sprague Dawley大鼠测试Sigma HV琼脂糖。最后,使用3只Wistar大鼠以及2只Sprague Dawley大鼠测试Sigma LV琼脂糖珠。
将每种类型共60个空珠植入到大鼠的腹膜腔中,两只除外。两只Sprague Dawley大鼠接受54或59个Sigma HV琼脂糖珠。观察大鼠3个月,然后处死。取出各个器官以研究炎症(脾、肝、肾、骨骼肌、胰腺和肠系膜)。
使用美国兽医病理学会(the American College of VeterinaryPathologists)认可的标准通过炎症来评价组织。所述评价使用6级尺度来评价炎症,基本如下:
0=正常
1=低度(少于10%的炎症)
2=轻度(10-25%的炎症)
3=中度(25-50%的炎症)
4=显著(50-75%的炎症)
5=重度(多于75%的炎症)
评价每个动物的组织,然后取平均值。结果如下:

很明显,本发明的琼脂糖-琼脂糖包被的珠炎性最低。
对狗进行了额外的炎症研究,其比较了本发明的珠和FMCHGT(P)琼脂糖(FMC HGTP琼脂糖)制成和用它包被的珠。同样,本发明的琼脂糖-琼脂糖包被的珠炎性更低。
在另一研究中,狗接受本发明的琼脂糖-琼脂糖包被的珠,其含有猪胰岛。在2.5年后处死所述动物,只观察到低度的炎症。相比于未移植的对照动物,腹膜和肠系膜看起来非常正常。
实施例6
在这些实验中,将体外培养的含有胰岛的思科姆金琼脂糖大珠与对照(空)大珠进行比较。
如上文实施例3中所述,制备包裹在思科姆(Seakem)琼脂糖中的胰岛。
使用12只雄性自发性糖尿病BB大鼠作为受试动物。所有的动物均为10-15周龄,并已显示出临床糖尿病迹象3-16天。
在到达后20-21天,用以2.2-2.3ml/kg体重的剂量皮下施用的氯胺酮/甲苯噻嗪/NaCl麻醉所述BB大鼠。麻醉之后,使所述动物接受含有胰岛的大珠植入物,其剂量等价于每日所需胰岛素的1.0倍,或者接受与此数量相当的空大珠。
移植之后,观察动物,每天记录临床观察,包括一般状况(好、良或差)、体重、血糖、尿糖、和尿酮。在本研究全程中收集血清样品,以用于测定胰岛素、胰高血糖素和猪C肽。使用放射性免疫测定测量这些参数。还实施了腹膜内葡萄糖耐量测试。
移植97天后,对所述动物进行完整的尸体剖检。麻醉动物,放血,暴露腹膜腔。
在研究的97天全程中,6只接受了含胰岛的思科姆琼脂糖珠的动物均不需要施用胰岛素。而接受空大珠的动物则相反。在研究开始后两天开始对这些动物施用外源胰岛素,因为血糖水平升至300-500mg/dl。这些对照动物中的两只在研究的第三天死亡,推测是由于胰岛素缺乏。
相比于对照,在接受了含胰岛的思科姆琼脂糖珠的动物中平均每日血糖水平显著更低。同时,六只测试动物甚至在没有进行胰岛素治疗时也显示出非常窄的每日血糖偏差范围。
一个月后,这些接受了胰岛的动物变成中度高血糖,但是显示出有限的血糖波动(约100mg/dl)。而对照则相反,其显示出剧烈的变化,约400-500mg/dl,即便是施用了2-3U/天的外源胰岛素。
开始时,对所有动物实施腹膜内葡萄糖耐量测试,以证实I型糖尿病的临床诊断。在移植8天和90天后也进行了此测试。移植前5天,对葡萄糖负荷的应答不明显,但是在移植后第8天,含胰岛大珠的受者显示出对葡萄糖负荷的明显应答,即血糖开始时升高,然后回复到正常血糖。高血糖在接受空大珠的动物中也没有被抑制,即便是同时进行了如上文所述的胰岛素治疗。
移植后90天进行另一次腹膜内葡萄糖耐量测试。接受了胰岛大珠的大鼠再次重建了基线血糖,但是起始血糖水平明显更高,即约400mg/dl。接受了空大珠的大鼠不能重建基线血糖。
对猪C肽的测定在移植之前的研究动物或在空大珠受者中未检测到该肽。相反,在植入了胰岛大珠的大鼠血清中常规检测到所述肽,在移植后21天的平均水平为0.8800.249ng/ml,在研究终止时为0.6620.160ng/dl。
还检测了研究动物的糖尿、酮尿和对施用碳酸氢盐的需要。在移植前没有显著差异;但是,在移植之后,胰岛大珠的受者糖尿(56个样本中的37个,相对于81个中的67个)和酮尿(64个样本中的20个,相对于54个中的32个)的发生显著较少。对碳酸氢盐治疗的需要也显著降低(2个治疗,相对于26个)。
实施例7
接下来的实验证明包入了胰岛的思科姆琼脂糖大珠可在体外长期培养,并仍然保持其功能。
在这些实验中,使用满足与上文实施例6相同标准的12只的一组糖尿病BB大鼠,还使用了23只7周龄Wistar-Furth大鼠。此第二组大鼠作为正常对照。
通过尾静脉对这些Wistar-Furth大鼠中的5只注射65mg/kg链脲霉素,以诱导糖尿病。当观察到两次连续的血糖读数>500mg/dl时,大鼠开始接受胰岛素治疗,如下文所述。
在到达后20-21天,使用肌内施用的0.1ml/100g剂量的氯胺酮/(60mg/ml)、甲苯噻嗪(6mg/ml)/布托啡诺(3mg/ml)来麻醉动物。
麻醉后,所有BB大鼠接受含有胰岛的思科姆琼脂糖珠,如上文所述。所述大鼠分成3组,每组4只,接受体外培养了9周、40周或67周的大珠。所施用的大珠的量相当于动物每日胰岛素需求的1.0倍。
五只Wistar-Furth大鼠接受体外培养了7.8-11.5周的大珠,剂量与BB大鼠相同。即每只Wistar-Furth大鼠约45-49个大珠,每只BB大鼠56-60个。
在实验过程中,大鼠平均增重约75g。因此,在第一次植入后第97天对BB大鼠进行补充植入。大鼠植入前的平均胰岛素需求是0.0083U胰岛素/g体重。由此值计算出需要另外的17个含胰岛大珠,以每24小时产生39.19mU胰岛素。如下文所说明的,由于在第二次植入之前从每只大鼠中移出了4个珠,因此施用培养了19周的21个大珠。Wistar-Furth大鼠未接受第二次植入。
在实施例7中实施的多种测定在此处也使用相同方法实施。
在移植后201-202天,也如上文所述的进行完整的尸体剖检。
图1中显示平均每日血糖水平。植入后,所有BB大鼠中恢复正常血糖(100-200mg/dl)约一个月。此后,BB大鼠发生中度高血糖(200-400mg/dl),这一情况在研究的剩余时间内一直持续。中度高血糖的产生和达到最大体重同时发生。在研究的剩余时间内体重保持一致,而血糖水平在300-400mg/dl之间波动。在接受含胰岛思科姆金大珠的3组大鼠之间没有观察到平均每日血糖水平的差异,不管珠的体外培养时间是多少。
已诱导出糖尿病的Wistar-Furth大鼠也显示约一个月的正常血糖,之后观察到中度高血糖。
应该注意,如上文所述,研究的第97天对BB大鼠中进行了第二次植入。这一第二次植入未影响每日血糖值。
还测定了猪C肽,并在所有的3组BB大鼠中都检测到了。在实验最初88天中,平均猪C肽水平从0.6-0.9ng/ml降至0.2-0.4ng/ml。在第116天,在第二次植入之后,该肽的水平提高到平均为0.3-0.7ng/ml,尸体剖检时在腹膜液中观察到了40倍的提高。
在研究全程中对所有接受胰岛大珠植入的大鼠进行葡萄糖负荷操作。经过不同培养时间的大珠在移植后应答葡萄糖负荷的能力没有观察到差异。具体而言,所有BB大鼠的血糖水平约为初始值100-200mg/dl的两倍。12只动物中的10只在120分钟内回复到基线血糖。此应答与正常Wistar-Furth动物中所观察到的相似。
所有的研究动物最终确实都变为中度高血糖,但是移植后第105天的葡萄糖负荷显示血糖最初升高,然后回复到基线血糖。在移植后第200天,施用葡萄糖之后只是在基线血糖上略有增加,然后就回复到基线血糖。
与上文Transplantation中Jain等的工作相比,这些研究的结果显示,含有胰岛的思科姆金琼脂糖珠出人意料的优于Jain等所报道的。例如,在Jain等的报道中,40%的受试动物在第200天之前死亡,而使用本发明大珠的死亡率是0。另外,本文所实现的结果是使用Jain等所报道的珠量的一半实现的。另外,在本文未详细说明的结果中,在尸体剖检之后测定了所回收的大珠的胰岛素产生水平,其显著高于Jain等所报道的尸体剖检之后所回收大珠的水平。
实施例8
进行实验以比较本发明的珠与FMC HGT(P)琼脂糖制成和包被的珠的强度。
在这些测试中,将珠单个置于压迫设备中,该设备具有上板和下板。上板以12英寸/分钟的速率向下移动,压迫珠直至其破裂。以lbf测定压力(最大压缩时)。
对于HGT(P),最大压力范围是0.714lbf至3.183lbf,平均为1.958,标准差为0.5444。对于本发明的产品,其范围是2.322lbf至6.418lbf,平均为4.282,标准差为1.096。
显然,本发明的珠比其它琼脂糖-琼脂糖包被的珠更坚固。
前述实施例描述了本发明的多个特征,其涉及含有分泌细胞的被琼脂糖包被的琼脂糖大珠,其中所使用的琼脂糖是思科姆金琼脂糖。
如本文所述的术语“大珠”指基本为球形的结构,其直径为约4至约10-12mm,最优选直径为约6至约8mm。第二琼脂糖层优选厚度为约0.05至约5mm,更优选厚度为约0.5mm至约5mm,更优选约1.0至约3mm,最优选厚度为约1.0至约2.0mm。第二琼脂糖层可以但不必须是思科姆金琼脂糖。
“大珠”作为优选结构使用;但是,任何包裹分泌细胞的固体琼脂糖结构(优选包被第二琼脂糖层)均是本发明的特征。
所述分泌细胞可有所不同。可以包裹任何产生所需分泌产物的细胞或细胞器。胰岛、癌细胞和干细胞是可这样使用的材料类型示例。每个珠可含有不同数目的细胞器,例如,对于胰岛为约50个至约5000个胰岛,更优选约100个至约2500个胰岛,更优选约250个至约1000个,最优选约475个至约550个胰岛。特别优选约500个胰岛。
本发明的其它方面对于本领域技术人员来说是很明显的,不需要进一步说明。
所使用的术语和表述用于描述而非限制,使用这些术语和表述并不旨在排除任何所示及所述特征的等价形式或其部分,应该理解,在本发明的范围内可能有多种改变。
Claims (38)
1.一种生产包被有琼脂糖的思科姆金琼脂糖分泌细胞珠的方法,其包括:
(a)将分泌细胞悬浮在思科姆金琼脂糖中,
(b)由步骤(a)的悬浮分泌细胞形成珠,
(c)在润湿空气中孵育步骤(b)的珠,
(d)使用琼脂糖包被步骤(c)的珠,以形成包被有琼脂糖的思科姆金琼脂糖分泌细胞珠。
2.权利要求1的方法,其还包括在5%琼脂糖中滚动步骤(c)的珠,使所述滚动的珠与矿物油接触以及洗涤所述滚动的珠,以形成所述包被有琼脂糖的思科姆金琼脂糖分泌细胞大珠。
