Abstract

Description
本发明涉及一种装置,特别是涉及用于腹腔镜手术,也被称为“锁孔手术”的装置。
本发明的目的是提供一种简单的手动操作的装置或仪器,该装置或仪器具有使用的人体工程学正确属性和关于更有效利用外科医生的肌肉用于操纵该仪器的扳机或促动器的技术方案,它给外科医生提供了对仪器的改进控制。
虽然在1910年就第一次对人类执行腹腔镜检查,但是直到1987年腹腔镜技术的使用才开始腾飞。从那以后在使用和手术操作领域中得到了快速地发展。然而,腹腔镜仪器的发展很少有关人体工程学的改进。科学测量结果证明与开放式手术相比外科医生要花费高达十倍的精力在腹腔镜下执行相同的操作。
由美国专利5480409,5893878,5383888,5792165,5976121,5488441,5735873和5868784以及由WO 9724072及其它文献中已公知大量的腹腔镜仪器的设计。虽然已公知的仪器在设计和功能上有很大的不同,但是已公知的仪器具有以下共同特点,即它们由包括一个或多个可移动部分的把手、其中尤其是由“扳机”或促动器构成,该“扳机”或促动器可由用户例如外科医生进行操纵以控制工具,所谓的受动器,该受动器连接到管状元件或工具杆的悬臂端部部分上,管状元件或工具杆的另一端部部分连接到把手上。
US 5792165公开了一种展现出关于操纵受动器有很大灵活性的仪器,其具有三种自由度:旋转,枢转和夹紧。另外可从该仪器的管状体上连接和去除不同的受动器。在US 5792165中公开的该仪器还设有集成的马达和部分控制受动器动作的微处理器。
US 5383888公开了一种展现出基本上与US 5792165的仪器具有相同功能的仪器。
US 5976121公开了一种用于操作连接内窥镜的仪器的把手,其中借助于杠杆来张开/闭合该仪器端部处的呈一对剪刀形式的工具。
关于前面提到的现有技术存在若干缺点。
其中一个缺点涉及在绝大多数上面提到的现有技术中仪器把手的独特设计在人体工程学上是不利的,这是因为该仪器不能提供弯曲手掌的工作位置和/或必须移动手指来操作该仪器,而除了食指,即所谓的食指外其它手指不得不用于操纵该仪器的主要功能。这使得外科医生的手部很容易发生很小的不可控的移动。这些移动导致仪器的操作端部部分产生相对较大且所不希望的移动。这一不利设计的结果在于试图抵消上面提到的不希望的移动,除其它因素外,与开放式手术相比外科医生要花费高达十倍的精力在腹腔镜下执行相同的操作。
涉及多数上面提到的仪器的另一实质性的缺点在于它们在技术上非常复杂,这使得仪器制造上非常昂贵。从而,在很大程度上,试图重复使用该仪器若干次。虽然理论上仪器能100%地消毒,但是除了别的以外由Fengler,Pahlke,Bisson和Kraas在Krankenhaus Moabit,Lehrkrankenhaus der Humboldt Universitaetzu Berlin的手术部进行的“The Clinical suitability of laparoscopic instrumentation.Aprospective clinical study of function and hygiene”研究显示在清洁后,相对大量的仪器中含有血液制品的残余物,这表现出患者被传染的潜在风险。这可导致患者患上严重的疾病,并且在最坏的情况下死亡。
对于腹腔镜手术而言,外科医生在很大程度上操作仪器的受动器,例如处于近封闭位置的抓紧器。为了将该受动器从正常情况下全张开的初始位置处移动到将近全闭合的所需工作位置,外科医生不得不移动仪器的促动器相对较长的距离。类似的,当受动器由一对剪刀构成时,例如在剪刀处于接近闭合位置时需要在剪刀上施加最大的力,因为它正常情况下在这一位置上剪切是有效的。在用于腹腔镜手术的已公知仪器中,由于施加外力的结果使促动器移动与受动器移动之间的关系从全张开位置到全闭合位置基本上是恒定的。这意味着用户不得不施加正像从受动器全张开位置的通常“无用”范围到受动器的“使用范围”或活动范围的力,这在很大程度上导致0%-近似50%的区域张开。这意味着相对较差地使用操纵促动器的肌肉群。除了上面提到的缺点,在其它因素中,控制US 5735873,US 5868784和5976121所公开的仪器必须使用多个手指以操纵受动器。当该仪器用于任何时间长度及困难的工作位置时,这表现出尤其显著的缺点,其中手本身被迫处于异常和部分扭曲的工作位置。这样可导致仪器的操作者例如外科医生,误用其肌肉组织,从而快速疲劳并且使手臂和肩膀紧张。
本发明的目标在于克服或至少减少前面提到的专利文献所表现出的与现有技术相关的一个或多个缺点,并且特别是克服或至少减少涉及使用肌肉来操纵促动器的缺点。同时本发明的一个目的在于提供一种仪器,该仪器不仅具有非常简单的结构而且其中仪器的主要元件部分可由例如,但不限于塑料材料制造。这导致相对低的生产成本,从而可像一次性物品一样来保护仪器的使用。而且,这样就会消除由于仪器清洁的不彻底而带来的传染问题。
根据本发明通过在下面说明书和随后的权利要求书中所限定的技术特征来实现该目的。
一方面,本发明由用于腹腔镜手术的仪器构成,包括设有促动器的把手,该促动器被设置成经连杆实现操纵置于管状元件第一端部部分的受动器,所述管状元件在其第二端部部分延伸到仪器的把手内并连接一部分连杆组,该连杆组由剪刀连杆构成,其在第一端部部分借助于第一旋转连接部并通过杠杆连接把手,该杠杆在第二端部部分连接到第四旋转连接部中的把手,借助于第三旋转连接部将剪刀连杆在其第二端部部分连接到杠杆的第一端部部分。这样,将移动和力从促动器传递到受动器的连杆组将使促动器围绕所述第一旋转连接部的旋转与受动器钳口或张角之间的关系基本上呈现出双曲线关系,如当促动器处于或临近其初始位置或非启动位置时,连杆组使得促动器和受动器之间的传动比相对较大,而同时受动器处于或临近其全张开位置,并且随促动器旋入把手,传动比减小。这样就使得促动器越接近其完全启动的位置,即在腹腔镜手术中最常用的位置,外科医生或其它用户就能越来越好地控制受动器。相应地,当促动器旋入把手时,施加到促动器上的力的效果将非线性且大体上以双曲线的方式增加到受动器上。这表示施加到促动器上的力基本上与受动器的相对移动量成反比。
腹腔镜操纵可能持续相对较长的时间。因此,非常重要的是仪器具有就操作者的手,既相对于像促动器等功能设备的定位又相对于其大小而言最佳地进行调整的可能性。因此,在优选实施例中,根据本发明的仪器设有把手,该把手设有至少一个能够就仪器操作者的手的大小来调整把手的可调部分。在一个实施例中,该仪器的至少一个可调部分被放置在把手的后部。
在手术中常常需要灼烧组织或血管。在优选实施例中,该仪器设有本身已公知的用于连接电源的电子连接装置,该电源通过导线向受动器供电用于执行单极切割。
在下文中描述了优选实施例的一个非限定性示例,其体现在附图中,其中:

管状元件7可围绕其纵轴旋转。借助于设置在把手2上部中的旋轮15来控制该旋转,将管状元件7牢固地连接到旋轮15上,并且将受动器9可旋转牢固地连接到管状元件7上。通过使旋轮15旋转,所述管状元件7和受动器9相对于把手2可以其本身已公知的方式进行旋转。
在图1和2中,促动器3被卸载掉外力并处于其未启动的位置。通过呈弹簧35形式的偏压元件促使促动器3进入这一位置,该弹簧35连接在连杆组元件6和一部分把手2之间从而使得当在促动器3上施加压力时,该偏压力经连杆组元件5进行传递,造成其围绕轴4旋入其未启动位置。当促动器3未启动时,受动器9处于其完全张开位置。本领域的技术人员将会意识到在可选实施例中,当促动器3处于其未启动位置时,受动器9可处于其完全闭合位置。
在一个可选实施例(图中未示出)中,该仪器不设有偏压元件。在这一可选实施例中,当作用在促动器3上的外力停止时,促动器3将基本保持在其当力停止时的位置。
连杆组包括由一部分促动器3形成的连杆元件3’(在图中以虚线示出)和将连杆元件3’连接到连杆元件6上的连杆元件5。促动器3的连杆元件3’可围绕连接把手2的轴4旋转。该连杆元件3’的旋转限于从一部分把手2的内表面凸出的止动设备30与例如一部分把手2上的促动器3的一个端部部分的支座之间。
连杆元件5在第一端部部分可旋转地连接到旋转连接部40中的连杆元件3’的端部部分上,并且在第二端部部分可旋转地连接到旋转连接部44中的连杆元件6的更低端部部分上。在其第二端部部分处,连杆元件6可旋转地连接到旋转连接部46中的把手2上。
将本身已公知类型的管状元件7连接到连杆元件6的上部,并且启动受动器9以响应连杆元件围绕旋转连接部46进行的钟摆运动。
当促动器3为非启动时,连杆元件3’和5分别与在旋转连接部4和旋转连接部44之间的直线L形成角度α0和β0,见图6。
当外力施加到促动器3上时,连杆元件3’和5将朝着线L向上旋转,直到促动器3移到支承在机械止动设备30处。在这一位置,随着促动器完全启动,连杆元件3’和5还有旋转连接部40将优选与线L重合。在图3和4中,促动器3分别显示为处于部分启动和近乎完全启动的位置。相应地,受动器9分别处于部分闭合和基本上闭合的位置。当促动器3受到由图中未示出的手指施加的外力时,并且当连杆元件5和3’的角度α0和β0分别减少到角度α和β时,旋转连接部44将移动距离ΔL。这一移动距离ΔL造成连杆元件6围绕旋转连接部46旋转,并且同时连同管状元件7,其连接一部分连杆元件6的端部部分产生移动,从而使得受动器9以这样的方式移动从而减小钳口或开口。旋转连接部44从促动器3处于非启动位置直到促动器3处于启动位置的移动距离ΔL可由下面的公式简单地表达:

仪器1设有可调后部8,所述可调后部在图1-4中示出了它最凸出的位置。借助于锁定构件41将后部8锁定在适当的位置,该锁定构件由在其第一端部部分可旋转地连接到旋转连接部43中的一部分把手2上的锁定元件42构成。锁定元件42的第二端部部分设有卡爪构件47,所述卡爪构件被设置成完全抓握越过与卡爪构件47互补的若干(所示为四个)止块48,其被设置在后部8的内底部并从那里凸出。以这样的方式借助于弹簧构件50偏压锁定元件42,使得卡爪构件47将会推挤越过止块48。在对后部8的所需调整中,可调按钮52受到与来自弹簧构件50的力反作用的力,从而使卡爪构件47与止块48脱开接合。当卡爪构件47与止块48脱开接合时,借助于从后部8的一部分内表面凸出并且在其自由端部部分对从把手2一部分内表面凸出的相反元件53施加压力的偏压元件51,将后部8偏压到最外面的位置。
在附图中后部8被显示为可旋转地连接到设置在把手2顶部的旋转连接部55中的把手2上。本领域的技术人员将会意识到也可将用于可调后部8的旋转连接部定位到其它地方,例如定位在把手2的底部中,并且可调后部6可设有若干旋转连接部并具有不同于附图中所示的其它结构。
在可选实施例(图中未示出)中,仪器的把手可设有同样在把手2的腹部和/或一个或两个侧部进行调节的可能性。
在图5中,可调后部8基本上移入把手2中,从而使得把手2采取其近乎最小的凸出位置,并且促动器3和后部8之间的距离将可能是最小的。把手2的后部8可固定在图5所示位置与例如图1所示位置间的许多中间位置处。为了实现把手2对外科医生手的大小的最佳可能的适应性,这种调节的可能性是重要的,从而用户的舒适感将是最理想的。
在附图中,可调后部8的更低端部部分设有其本身已公知的用于电能的连接设备60。连接设备60的目的是能够连接电源,电能经导线62传送到受动器9,从而使得可执行与手术有关的单极切割。也可将该连接设备60设置在仪器1所示位置之外的其它位置。
本领域的技术人员将会意识到促动器3在本代表性实施例中被显示为具有所谓的开放手指夹(finger-grip),其可设有环状或闭合的手指夹。
在可选实施例(图中未示出)中,该仪器设有棘齿机构,所述棘齿机构由棘齿条和基本上互补的卡爪构成,该棘齿机构被设置用于选择性地将促动器3锁定在所需的位置上。在优选的实施例中,棘齿条形成连杆组6的整体部分。该卡爪被设置成借助于设置于一部分把手2内的连接开关而选择性地接合或脱离棘齿条,并且其经柔性元件连接该卡爪,该柔性元件被设置成对卡爪施加偏压力。连接开关和促动器3优选地设置成彼此独立操纵。
Claims (7)
1.一种用在腹腔镜手术中的仪器(1),包括设有促动器(3)的把手(2),该促动器(3)被设置成经连杆组(3’,5,6)有效操纵放置在管状元件(7)第一端部部分上的受动器(9),所述管状元件(7)在其第二端部部分延伸进仪器(1)的把手(2)并连接一部分连杆组(3’,5,6),其特征在于,连杆组(3’,5,6)由剪刀连杆(3’,5)形成,该剪刀连杆(3’,5)在第一端部部分借助于第一旋转连接部(4)并通过杠杆(6)连接到把手(2),该杠杆(6)在第二端部部分连接到第四旋转连接部(46)中的把手(2)上,剪刀连杆(3’,5)在其第二端部部分借助于第三旋转连接部(44)连接到杠杆(6)的第一端部部分。
2.根据权利要求1所述的仪器,其特征在于,所述管状元件(7)连接到在第三旋转连接部(44)和第四旋转连接部(46)之间部分中的杠杆(6)上。
3.根据权利要求2所述的仪器,其特征在于,所述管状元件(7)连接到位于更靠近第四旋转连接部而非第三旋转连接部(44)部分中的杠杆(6)上。
4.根据前述权利要求中任一项所述的仪器,其特征在于,所述把手(2)设有至少一个可调部分(8),使得把手(2)能就不同用户的需要进行尺寸调节。
5.根据权利要求4所述的仪器,其特征在于,所述至少一个可调部分之一被设置在把手(2)的后部(8)内。
6.根据前述权利要求中任一项所述的仪器,其特征在于,所述仪器(1)设有被设置成将促动器(3)偏压到最有可能的凸出位置上的偏压元件(35)。
7.根据权利要求6所述的仪器,其特征在于,所述偏压元件由弹簧(35)形成。
