Abstract

Description
本发明涉及用于药物持续释放的组合物及其应用。
脱乙酰壳多糖为在自然界中极易获得的天然物质。它与其它聚合物相比是廉价的,无毒性,生物可降解并且生物相容的。脱乙酰壳多糖的药物应用极为广泛。科学和医学文献中列举了脱乙酰壳多糖数以百计的工业、医学和膳食应用,参见J.Karlsen和O.Skaugrud,Exicipient Properties of chitosan,Manufacturing Chemist,62,p.18,1991,和I.Orienti,K.Aieda,C.Ponti,E.Gianasi,V.Zecchi,Progesterone Loaded chitosan Microspheres,Effect oftriethylene glycol glutarate linked to the chitosan moleculeon drug release,S.T.P.Pharm.Sci.,6,p.424-429,1996。认为脱乙酰壳多糖在通过口服途径摄取时,因缺乏脱乙酰壳多糖酶而无法被人消化,但脱乙酰壳多糖酶存在于某些细菌中。
作为递药载体,将脱乙酰壳多糖视为21世纪的药物载体。制药工业已广泛检验了它在控释递药系统中的潜能。脱乙酰壳多糖在控释递药系统中的应用的目的在于制备包括以动力学方式控制药物释放的微粒或大分子系统的固体剂型,以便使得释放更依赖于药物制剂而非药物的物理化学特性。实例如下:脱乙酰壳多糖直接压制片;附着胃壁的脱乙酰壳多糖微球;与乙酐和戊二醛交联的脱乙酰壳多糖在暴露于溶出介质时形成海绵状结构;和制备成控释药物载体的脱乙酰壳多糖珠。
固体药物组合物的给药对某些患者而言有时是不方便的。固体剂型不能给予儿童、婴儿和新生儿,因为与液体剂型相比存在吞咽固体剂型的困难。此外,某些成年人不能吞咽片剂,另外与固体剂型相比,以液体剂型可以给予更大量的药物剂量,尤其是对于高强度的药物。
因此,本发明的目的在于克服现有技术的缺陷并且提供用于药物持续释放的组合物,其可以容易并且可靠地给予患者。
通过用于使药物持续释放的组合物实现这一目的,该组合物包含:a)溶于pH低于约4.0的溶剂中的脱乙酰壳多糖,其中脱乙酰壳多糖选自在约3.0-约7.5的pH范围内沉淀的脱乙酰壳多糖;和b)至少一种可溶于上述脱乙酰壳多糖溶液的药物化合物。
优选脱乙酰壳多糖选自在5.0-7.5,优选6.0-7.5,更优选6.5-7.5,且最优选7.0-7.5的pH范围内沉淀的脱乙酰壳多糖。
更优选溶剂为0.1M HCl和/或水。
在一个优选的实施方案中,药物为双氯芬酸、布洛芬钠、丙戊酸钠和/或诸如此类。
可以提供组合物,其中脱乙酰壳多糖具有约500-约400,000,优选约500-约100,000的分子量。
优选药物在液体组合物中的存在量为约0.1-10%w/v,更优选0.1-2%w/v。
另外优选脱乙酰壳多糖在液体组合物中的存在量为约1-15%w/v,更优选1-10%w/v,最优选1-5%w/v。
本发明的组合物可用于药物持续释放。
优选通过口服给予所述的液体组合物。
令人意外地发现,使用本发明的液体组合物可以保护胃抵抗酸性药物诸如双氯芬酸钠或布洛芬钠的刺激副作用,因为药物释放主要持续到肠道中,在其中pH稍高。迄今为止,双氯芬酸钠或布洛芬钠似乎不溶于酸性介质。然而,当用于本发明的液体组合物中时,它转变成可溶于酸性介质(pH<5.0),只要将其与相应的脱乙酰壳多糖聚合物溶液相组合。
随着pH增加,由于按照pH脱乙酰壳多糖沉淀而导致药物缓慢释放。人消化道的pH在1.0-7.8之间变化,其中胃为酸性的(pH在1.0-3.0左右)并且小肠具有约5.50-7.8的pH。
使用本发明的液体组合物,药物的释放可以靶向至胃肠道中的特定区域,例如十二指肠和结肠。据推定持续释放特别取决于与药物一起使用的脱乙酰壳多糖的分子量。
此外,观察到可以通过经向本发明组合物中添加粘膜粘着剂和/或建立粘度的聚合物改变溶液从胃中的清除率而获得药物在胃中保留时间的改善。

实施例
脱乙酰壳多糖寡糖的分离方法
为了制备本发明用于药物持续释放的液体组合物,脱乙酰壳多糖部分(或几个部分)必须由商购脱乙酰壳多糖混合物制备。
A)将25g脱乙酰壳多糖寡糖混合物溶于500ml蒸馏水或0.1MHCl并且使用6.0M HCl调节至pH为1.0。脱乙酰壳多糖寡糖混合物可以为包含具有例如不超过2,000,不超过3,000,不超过5,000,不超过10,000,不超过50,000,10,000-100,000,不超过100,000或250,000-400,000分子量的脱乙酰壳多糖的混合物。然后使用0.45um醋酸纤维素滤膜过滤每种脱乙酰壳多糖溶液。
B)对于上述制备的每种脱乙酰壳多糖溶液(各自含有具有不同分子量的脱乙酰壳多糖),如下在不同pH值下收集沉淀:
1.使用1.0M氢氧化钠(NaOH)将过滤溶液的pH调节至pH3.0,过滤并收集沉淀;
2.使用1.0M氢氧化钠(NaOH)将步骤1中获得的过滤溶液的pH调节至pH4.0,过滤并收集沉淀;
3.使用1.0M NaOH将步骤2中获得的过滤溶液的pH调节至pH5.0,过滤并收集沉淀;
4.使用1.0M NaOH将步骤3中获得的过滤溶液的pH调节至pH5.5,过滤并收集沉淀;
5.使用1.0M NaOH将步骤4中获得的过滤溶液的pH调节至pH6.0,过滤并收集沉淀;
6.使用1.0M NaOH将步骤5中获得的过滤溶液的pH调节至pH6.5,过滤并收集沉淀;
7.使用1.0M NaOH将步骤6中获得的过滤溶液的pH调节至pH7.0,过滤并收集沉淀;和
8.使用1.0M NaOH将步骤7中获得的过滤溶液的pH调节至pH7.5,过滤并收集沉淀。
C)通过将每种沉淀溶于0.1M HCl纯化上述沉淀。然后加入1MNaOH以便将pH增加至用于沉淀每种制备溶液的pH。过滤并且收集沉淀。将该步骤重复两次。
D)风干上述制备的沉淀,研磨并且作为粉末收集。
将上述在5.0-8.0pH范围内获得的脱乙酰壳多糖沉淀中的一些与具有不同分子量的脱乙酰壳多糖用于进一步评价,并且使用所述脱乙酰壳多糖部分和不同的药物制备用于药物持续释放的溶液。
1.使用双氯芬酸钠作为模型药物的体外药物释放试验。
实施例1
风干在步骤B3中在pH5.0从脱乙酰壳多糖混合物中收集的沉淀(分子量=不超过100,000),研磨并且收集为粉末。将2.0g该粉末溶于20ml 0.1M HCl。使用6.0M HCl将该溶液的pH调节至2.0。所得脱乙酰壳多糖浓度为10%w/v。
此外,制备下列溶液用于进一步评价:
溶液(1):将1.667ml 15mg/ml双氯芬酸钠在丙二醇中的溶液加入到20ml上述制备的脱乙酰壳多糖寡糖溶液中。
溶液(2):将1.667ml 15mg/ml双氯芬酸钠在丙二醇中的溶液加入到20ml蒸馏水中。
溶液(3):将1.667ml蒸馏水加入到20ml上述制备的脱乙酰壳多糖寡糖溶液中[用作溶液1的空白]。
溶液(4):使用0.1M HCl溶液并且与其它溶液类似地改变其pH[用作溶液2的空白]。
将制备的4种溶液各自加入到600ml 0.1M HCl中并且使用USP桨式仪器以50rpm的速度在37℃的温度下搅拌。如下逐步加入0.2M磷酸三钠以便缓慢增加pH值:
1.600ml的0.1M HCl(pH1.20);
2.向上述溶液中加入105.0ml 0.2M磷酸三钠以便得到pH2.92;
3.再加入6.0ml 0.2M磷酸三钠以便得到pH4.25;
4.再加入3.0ml 0.2M磷酸三钠以便得到pH5.20;
5.再加入6.75ml 0.2M磷酸三钠以便得到pH6.06。
将从按照步骤B3制备的脱乙酰壳多糖部分(分子量不超过100,000)的药物释放百分比概括如下。通过标准溶出法测定药物释放,该方法对本领域技术人员而言是公知的,例如,参见M.Sheu,H.Chou,C.Kao,C.Liu,T.Sokotoski,Dissolution of Diclofenac Sodiumfrom Matrix Tablets,Int.J.Pharm.,85,pp.57-63,1992。

1.600ml的0.1M HCl(pH1.20);
2.向上述溶液中加入105.0ml 0.2M磷酸三钠以便得到pH2.92;
3.再加入6.0ml 0.2M磷酸三钠以便得到pH4.25;
4.再加入3.0ml 0.2M磷酸三钠以便得到pH5.20;
5.再加入6.75ml 0.2M磷酸三钠以便得到pH6.06;
6.再加入45.0ml 0.2M磷酸三钠以便得到pH6.88。

1.600ml的0.1M HCl(pH1.20);
2.向上述溶液中加入105.0ml 0.2M磷酸三钠以便得到pH2.92;
3.再加入6.0ml 0.2M磷酸三钠以便得到pH4.25;

脱乙酰壳多糖溶液中的药物负载量为25mg双氯芬酸钠/22ml。该剂量看起来是现实的,因为商品双氯芬酸钠剂量为12.5mg-150mg。
2.对布洛芬钠脱乙酰壳多糖缀合物的体内研究
布洛芬钠为用于证实体内持续释放构思的另一种模型药物。评价不同分子量(Mw<3000,<5000和<10000)的脱乙酰壳多糖对布洛芬释放的控制。每种持续释放脱乙酰壳多糖缀合物溶液包含布洛芬钠与脱乙酰壳多糖的复合物。溶液的最终pH为6.60。缀合物口服溶液的剂量为30mg布洛芬/kg家兔。以10mg布洛芬/kg家兔剂量给予参比速释口服溶液。以指定的时间间隔抽取血样,以3500rpm离心10分钟并且在分析前置于-20℃的冷藏箱中。使用HPLC法分析样品。研发了通过HPLC测定布洛芬浓度的简便快速的方法。将萘普生用作内标。流动相:水∶乙腈(40∶60),然后使用H3PO4调节pH至2.4。柱:Waters,Symmetry,5u,C18,和150*4.6mm。内标储备溶液:将5mg萘普生溶于50ml MeOH。注射环:50ul。流速:1ml/分钟。波长:220nm。
样品制备:将100uL血浆样品转入试管,加入10uL内标储备溶液,加入0.25ml 1M HCl,振摇30秒,加入5mL(85∶15)(己烷∶异丙醇),通过涡旋振摇1分钟,以3000rpm离心10分钟,将4ml有机层转入新的试管,使用空气浴蒸发有机溶剂,并且用1ml流动相重构。评价了该方法的特异性,其表现出对布洛芬峰没有干扰。布洛芬回收率为85-90%。校准曲线在0.5-10ug/ml浓度范围为线性。
脱乙酰壳多糖缀合物溶液包含12mg布洛芬/ml。对每只家兔给予的药物剂量为30mg/kg。
速释水溶液包含浓度为4mg布洛芬/ml的布洛芬钠。作为参比物给予的药物剂量为10mg/kg。该剂量(10mg/kg家兔)等同于对体重为60kg的人受试者给予600mg布洛芬(通常的成人速释剂量为200-600mg,每天给予3次)。在持续释放制剂中,配方应包含总每日剂量并且给予一次,即30mg/kg。
这证明持续释放溶液的剂量高于速释溶液3倍是正当的。持续释放的优点之一在于通过每天一次给予剂量减少了给药频率。
药物从速释溶液中的释放极快地发生,达到最大值并且也极为快速地下降,附图5-7。
附图5-7显示布洛芬从缀合物样品中的释放曲线为pH依赖性的。当药物通过不同pH的特定部分时,一些药物得到释放。在第一部分中,释放的药物在一定程度上等于或少于布洛芬速释溶液释放的药物。
例如,在CH MWt<3000的持续释放溶液中,基于Cmax比,在第一阶段达到约70%的速释药物血浆水平。而在CH MWt<5000和CH MWt<10000的持续释放溶液中,在第一阶段分别达到约45%和86%的速释药物血浆水平。这一结果清楚地表明30mg/kg持续释放剂量未被快速吸收。在复合物通过整个GIT时,取决于肠内发生的pH改变和脱乙酰壳多糖与该药物的缀合物的分子量,小部分药物被递送。
上述结果表明,布洛芬在作为脱乙酰壳多糖缀合物溶液给予时,通过胃肠道时表现出持续释放。

Claims (9)
1.用于药物持续释放的组合物,包含:
a)溶于pH低于约4.0的溶剂中的脱乙酰壳多糖,其中脱乙酰壳多糖选自在约3.0-约7.5的pH范围内沉淀的脱乙酰壳多糖;和
b)至少一种可溶于上述脱乙酰壳多糖溶液的药物化合物。
2.权利要求1的组合物,其中脱乙酰壳多糖选自在5.0-7.5,优选6.0-7.5,更优选6.5-7.5,且最优选7.0-7.5的pH范围内沉淀的脱乙酰壳多糖。
3.权利要求1或2的组合物,其中溶剂为0.1M HCl和/或水。
4.上述权利要求中任一项的组合物,其中药物为双氯芬酸钠、布洛芬钠、丙戊酸钠和/或诸如此类。
5.上述权利要求中任一项的组合物,其中脱乙酰壳多糖具有约500-约400,000,优选约500-约100,000的分子量。
6.上述权利要求中任一项的组合物,其中药物在组合物中的存在量为约0.1-10%w/v,优选0.1-2%w/v。
7.上述权利要求中任一项的组合物,其中脱乙酰壳多糖在组合物中的存在量为约1-15%w/v,优选1-10%w/v,更优选1-5%w/v。
8.权利要求1-7中任一项的组合物在药物持续释放中的应用。
9.权利要求8的应用,其中通过口服给予所述的组合物。
