Abstract

发明背景
光在医学诊断中的用途近来已受到重视(参见例如BiomedicalPhotonics Handbook(Editor:T.Vo-Dinh),CRC Press)。正在实验测试多种诊断方法以在各种医疗学科,例如内镜检查术、乳房X线照相术、外科或妇科中应用。为此目标,将染料供给组织作为用于荧光诊断和成像的外源造影剂,并且这里尤其是最大吸收和最大荧光光谱范围在700-900nm范围内(组织的诊断窗口)的荧光染料已经被用于体内成像。这种波长的光子较少被组织吸收,因此在吸收过程(主要通过氧合血红蛋白和去氧血红蛋白)终结光传输前渗入组织几厘米。此外,被引入所述组织中的荧光染料能够产生吸收,但以更长波长荧光辐射的形式发射所吸收的能量。可以通过光谱分离检测这种荧光辐射,从而可能定位染料并与染料所结合的分子结构相关联(这方面还参见Licha,K.(2002)Contrast Agents for Optical Imaging(Review).In:Topics in Current Chemistry-Contrast Agents II(Editor:W.Krause),Volume 222,Springer Heidelberg,pp.1-31.)。

为了获得患病结构和健康组织之间的诊断上的显著差异,所给予的染料必须在所述两种组织类型之间产生尽可能高的浓度差。这可以基于肿瘤生理学或形态学性质(血供、分布动力学、延迟消除、血管结构)以及基于肿瘤和血管细胞或相邻组织的分子性质来实现。对于疾病特异性结构的分子标记而言,可以使用由荧光染料和靶亲合(target-affine)分子如蛋白质、肽或抗体组成的缀合物。在注射后,这些缀合物的某些部分结合到分子靶结构如受体、细胞表面结构或基质蛋白上,而未结合的部分在体液中被稀释或代谢或排泄出体外。以此方式,可能导致更高的浓度差,从而在进行荧光诊断时导致更大的图像对比(高信噪比)。

所述血管发生特异性结合成分与如下结构结合,所述结构优先存在或仅存在于微转移中,位于新形成的微血管中或其附近,或者所述结构在微血管系统结构生长之前或生长过程中存在。这些分子结构在例如WO96/01653,Alessi P等人(2004)以及Nanda A和St Croix B.(2004)中得到综述。如上所述,形成微转移的细胞和小的增生性病变的类似细胞表达血管发生因子和抗血管发生因子,只要血管发生抑制剂抵抗血管发生因子的作用,其就导致血管发生的抑制。一旦血管发生因子的作用占优,它们就导致血管发生的开始。因此,参与调节血管发生的两种结构,即血管发生激活物和血管发生抑制剂都可以是本发明意义内的血管发生特异性结合成分。血管发生激活物包括但不限于如下分子结构,例如ED-B纤连蛋白(ED-BF)、endoglin(CD105)(Burrows FJ等人(1995)Clin.Cancer Res.1:1623-1634)、VEGF家族成员、血管内皮生长因子(VEGFR)、NRP-1、Ang1、Thie2、PDGF-BB和受体、TGF-β1、TGF-β受体、FGF、HGF、MCP-1、整联蛋白(αvβ3、αvβ5、α5β1)、VE-钙粘着蛋白、PECAM(CD31)、Ephrins、纤溶酶原激活物、MMP、PAI-1、NOS、COX-2、AC133、趋化因子或Id1/Id3。血管发生抑制剂包括但不限于如下分子结构,例如VEGFR-1、Ang2、TSP-1、TSP-2、血管生长抑素和相关的纤溶酶原kringles、内皮生长抑素(胶原XVII片段)、血管抑素、血小板因子4、TIMP、MMP抑制剂、PEX、Meth-1、Meth-2、IFN-α、IFN-β、IFN-γ、IP-10、IL-4、IL-12、IL-18、促乳素(M,16K)、VEGI、SPARC的片段、骨桥蛋白片段或Maspin(CarmelietP和Jain RK(2000)Nature 407:249-257;Yancopoulos GD等人(2000)Nature407:242-248;Bergers G.和Benjamin LE(2002)Nature Reviews Cancer 3:401-410;Hendrix MJC等人(2002)Nature Reviews Cancer 3:411-421)。在优选的实施方案中,所述血管发生特异性结合成分包括ED-BF、VEGFR或endoglin。其中ED-BF是特别优选的靶结构。ED-BF是纤连蛋白的剪接变体,也称为肿瘤胚胎(oncofoetal)纤连蛋白,其在血管发生过程中,在新长成的微血管结构中特异性地形成。
与这些结构结合的成分优选为肽(具有2-50个氨基酸残基的氨基酸链)、蛋白质(具有多于50个氨基酸残基的氨基酸链)、核酸、小分子或糖。
优选的蛋白质或肽是受体的配体,它们优先在或仅在微转移和/或新近血管化或正在血管化的结构中表达,尤其是血管内皮生长因子(VEGF)和抗体,包括人抗体、人源化抗体和嵌合抗体;包括抗体结合结构域的片段如Fv、Fab、Fab′、F(ab′)2、Fabc、Facb;单链抗体如单链Fvs(scFvs);和二价抗体(diabodies)。

本领域公知核酸可以具有特异性结合性质,因此,所述血管发生特异性结合成分也可以是核酸。优选地,这些核酸包括DNA、RNA、适体和PNA,其中特别优选适体。识别特异性结合适体的方法在本领域中是公知的,并且描述于例如WO 93/24508 A1、WO 94/08050 A1、WO 95/07364 A1、WO 96/27605 A1和WO 96/34875 A1中。本文特别参考这些文件中公开的方法,所述方法可以用于识别可用于本发明的血管发生特异性结合适体。在本发明的用途中所用的优选适体特异性识别ED-BF、endoglin或VEGFR。
随着小分子,即分子量低于1.000g/mol,优选低于500g/mol的非肽非核酸化合物的高通量筛选的出现,已经可能识别具有特定结合性质的小分子。这些小分子同样可以用作根据本发明可用的缀合物的一个成分。优选的小分子为2,2-二苯基乙胺,已经确定它与ED-BF特异性结合(Scheuermann J.(2002)Isolation of binding molecules to the EDB domain offibronectin,a marker of angiogenesis.Dissertation submitted to Swiss FederalInst,of Technology,Zurich)。

可以用于本发明的特别优选的吲哚三碳菁染料选自下面表1所列式(XVIII)至(XXXVI)的染料,表1显示在偶联至B之前的染料结构,并且包含马来酰亚胺(顺丁烯二酰亚胺)或溴乙酰偶联基团,所述偶联基团促进偶联至含有硫醇基的血管发生特异性结合成分。在所得缀合物中,如果偶联基团是离去基团,则在一些实施方案中各自的偶联基团将不再存在,或者其仅有一部分会保留在所述缀合物中,称为偶联基团的残余部分。本领域技术人员将明白,代替下面描述的马来酰亚胺(顺丁烯二酰亚胺)或溴乙酰偶联基团,其它基团可以在以下结构中取代,包括例如氯乙酰基、碘乙酰基、氯乙酰氨基、溴乙酰氨基、碘乙酰氨基、氯烷基、溴烷基、碘烷基、吡啶基二硫化物和乙烯基磺酰胺,以实现与B的偶联反应。

可以用于偶联至血管发生特异性结合成分的优选染料:
通过花青染料中部的R5偶联至B的花青染料显示特别好的量子产率,并且一旦偶联至血管发生特异性结合成分,所述量子产率令人惊讶地很少降低或者甚至不降低。因此,在本发明中,使用实施方案(i)至(vi)的花青染料和结构(XVIII)至(XXXVI)的特定花青染料是特别优选的。
用于偶联各花青染料和各血管发生特异性结合成分的适当方法主要取决于所述血管发生特异性结合成分的化学性质。现有技术中已知多种残基或基团,它们是天然存在的,或者可以被引入各种血管发生特异性结合成分中,所述残基或基团例如-NH2、-COOH、-SH、-OH等。然后这些基团可以与连接于所述花青染料的基团形成共价键,后者对另一基团显示良好的反应性,导致该两种成分的偶联。当然也可能逆转该顺序,即将该可反应的基团连接到所述花青染料上,而将该反应性基团连接到所述血管发生特异性结合成分上。基于这种教导,本领域技术人员能够为每对花青染料和血管发生特异性结合成分选择合适的反应性和可反应基团。
许多蛋白质或肽包含硫醇基如半胱氨酸残基的硫醇基,或者能被修饰以包含硫醇基。因此,如果用于本发明中的缀合物包含肽或蛋白质和花青染料,则优选上述花青染料在偶联至所述蛋白或肽之前包含硫醇基反应性偶联基团。硫醇基反应性官能团在本领域中是公知的,并且包括例如马来酰亚胺(顺丁烯二酰亚胺)、氯乙酰基、溴乙酰基、碘乙酰基、氯乙酰氨基、溴乙酰氨基、碘乙酰氨基、氯烷基、溴烷基、碘烷基、吡啶基二硫化物和乙烯基磺酰胺。因此,在优选的实施方案中,在连接至B之前,上述实施方案中的R3或R10′表示这种偶联基团。如果R3是连接于B的连接基,则所述连接基在偶联之前包含这种偶联基团。

在优选的实施方案中,小的增生性病变是与血管发生相关的原发性肿瘤;初癌状态;发育不良;化生;由于例如银屑病、银屑病关节炎、类风湿性关节炎的自身免疫疾病或感染的炎性病变;子宫内膜异位;微病变,优选皮肤微病变和/或眼病。在本发明的优选用途中,将所述缀合物用于体内诊断上述疾病和/或微转移。

优选地,根据本发明的用途检测的微转移为医原性微转移、血源性微转移、空腔微转移、管腔内微转移、淋巴微转移、局部微转移和/或区域微转移。
所诊断的微转移优选源自原发性肿瘤,包括但不限于胃肠道或结肠直肠道的恶性瘤(malignomas)(例如癌、肉瘤)、肝癌、胰腺癌、肾癌、膀胱癌、前列腺癌、子宫内膜癌、卵巢癌、睾丸癌、黑素瘤、口腔粘膜发育不良(dysplastic oral mucosa)、侵入性口腔癌(invasive oral cancer)、小细胞和非小细胞肺癌;乳房肿瘤,例如激素依赖性乳癌、非激素依赖性乳癌;过渡型和扁平细胞癌;神经恶性肿瘤,包括成神经细胞瘤、神经胶质瘤、星形细胞瘤、骨肉瘤;软组织肉瘤;hemangioamas和内分泌肿瘤。根据本发明的用途可检测的小的原发性肿瘤优选为上述肿瘤中的一种。在特别优选的实施方案中,所述小的原发性肿瘤或所述微转移为乳房肿瘤,特别是激素依赖性乳癌或非激素依赖性乳癌。

化生是受控细胞生长的形式,其中一种类型的成熟细胞或完全分化的细胞取代另一种类型的成熟细胞。根据本发明可检测的化生疾病优选选自特发性骨髓外化生、大汗腺化生(apocrine metaplasia)、非典型化生(atypicalmetaplasia)、自身实质性(autoparenchymatous)化生、结缔组织化生、上皮化生、肠化生、化生性贫血、化生性骨化、化生性息肉、骨髓外化生、原发性骨髓外化生、继发性骨髓外化生、鳞状化生、羊膜鳞状化生、症状性(symptomatic)骨髓外化生和再生性(regenerative)化生。
根据本发明可检测的眼病优选选自沙眼、早产儿视网膜病变、糖尿病性视网膜病变、新生血管性青光眼和年龄相关性黄斑变性。

对所述缀合物的剂量没有特别的限制,只要所述剂量能够检测最终要诊断的位置即可。根据要使用的化合物的种类、年龄、体重和靶器官或组织等对其进行适当调整。通常剂量为0.002-100mg/kg体重,优选0.005-10mg/kg体重,更优选0.01-2mg/kg体重,且最优选0.02-1mg/kg体重。
按照已知方法实施本发明的荧光成像方法,病期可以为每个要给予的缀合物适当地确定每个参数如激发波长和要检测的荧光波长,以获得最佳的成像和分辨率。从给予所述缀合物到通过所述荧光成像方法确定病变的时间消耗取决于所述缀合物和给药靶而变。例如,当将所述缀合物用于肿瘤成像时,流逝时间通常将在给药后约2至120小时的范围内,优选为给药后约2至约10小时。当流逝时间太短时,荧光太强以至于不能清楚地分辨血管发生组织和非血管发生组织(低信噪比)。用于所述荧光成像方法的装置在本领域中是公知的,并且描述于例如EP 0 868 143、EP 1 146 811A1、EP 1 408 824 A2、EP 1 409 995 A1和EP 1 410 330 A2。
如上所述,本发明还可以与手术操作联合使用,因此可以用外科显微镜、显微镜、放大镜等进行荧光的检测。这些装置可以用于多种手术操作中,包括切开操作和内窥镜操作。还可能将本发明与通常用于常规筛查癌症的装置和操作如结肠镜检查术和胃镜检查术联合使用。

a)(3-叔丁氧羰基-丙基)-三苯基-溴化鏻
在-40℃下,将50g(0.30mol)4-溴丁酸在400ml THF中与187g(0.89mol)三氟乙酸酐逐滴混合。在-40℃下搅拌30分钟后,在1小时内滴加400ml叔丁醇/30ml THF。在室温下搅拌16小时后,将反应混合物倾入冰冷的碳酸钠溶液中,用乙醚将水相萃取三次,用硫酸钠干燥有机相,并蒸发浓缩。将残余物在真空中蒸馏(沸点72℃/0.9mbar;收率:41g)。通过将41g(0.18mol)中间产物、44.6g(0.17mol)三苯基膦和32.5g(0.36mol)碳酸氢钠在250ml乙腈中加热回流20小时来进行形成膦盐的反应。将反应混合物过滤,蒸发浓缩,并通过与乙醚一起搅拌使残余物结晶。收率:58.5g(相对于4-溴丁酸为理论值的40%)白色固体。

一般规程:在每种情况下,将5mg(0.074μmol)BSA(Sigma Company)在5ml磷酸盐缓冲液(0.1 M Na2HPO4/NaH2PO4,pH6.8)中的溶液与0.74μmol实施例1-16的标题化合物(0.5mg/ml在PBS中的贮备液)混合,并在25℃下温育30分钟。通过凝胶色谱法纯化缀合物(柱:Sephadex G50,直径1.5cm,Pharmacia,洗脱剂:PBS)。
实施例21:用实施例17-19的标题化合物标记BSA

一般规程:将0.3ml AP39在PBS中的溶液(浓度:0.93mg二聚体/ml)与60μl三(羧乙基)膦(TCEP)在PBS中的溶液(2.8mg/ml)混合,并在25℃和氮气下温育1小时。通过在NAP-5柱上进行凝胶过滤来分离过量的TCET(洗脱剂:50mmol硼酸盐缓冲液,pH8.5)。通过光度法测定所获得的AP39单体(OD280nm=1.4)量为230-250μg(体积0.5-0.6ml)。将溶液与0.06μmol实施例17-19的标题化合物(0.5mg/ml在PBS中的贮备液)混合,并在25℃下温育4小时。通过在NAP-5柱上进行凝胶色谱法纯化缀合物(洗脱剂:PBS/10%甘油)。通过亲和色谱法(ED-B-纤连蛋白树脂)测定缀合物溶液的免疫活性(J.Immunol.Meth.1999,231,239)。所得缀合物的免疫活性>75%(在缀合前AP39的免疫活性>95%)。
实施例24:不同染料结构和AP39的靶特异性缀合物的光物理性质和免疫活性(ELISA)
通过光度法并基于短波吸收肩峰(约690-710nm)消光系数为75000 Lmol-1cm-1确定生物分子负载程度(染料对生物分子摩尔比);用抗体吸收(抗CD105 IgG)为OD280nm=1.4进行计算。用SPEX fluorolog(校正灯和检测器的波长依赖性灵敏度)相对于吲哚花青绿确定荧光量子产率(在DMSO中Q=0.13,J Chem.Eng.Data 1977,22,379,Bioconjugate Chem.2001,12,44)。
通过ELISA测量免疫活性,并描述与靶(ED-B-纤连蛋白)结合的生物分子相对于在与实施例1-19的样品染料缀合前未标记生物分子AP39的百分比。结果总结于下面表2中。

本实施例描述针对靶CD105(endoglin)的不同生物分子类型(全长IgG抗体)。
染料合成:用于缀合至抗体的染料是3,3-二甲基-2-{7-[3,3-二甲基-5-磺酸-1-(2-磺酸乙基)-3H-吲哚鎓-2-基]-4-(6-羧基-4-氧杂己基)庚-2,4,6-三烯-1-亚基}-1-(2-磺酸乙基)-2,3-二氢-1H-吲哚-5-磺酸三钠,内盐(实施例10c)。通过在室温下,在二甲基甲酰胺中,使该染料与5当量N-羟基琥珀酰亚胺、4当量N,N′-二环己基碳二亚胺反应5h而将其转化为相应的N-羟基琥珀酰亚胺酯。在用乙醚沉淀后,将粗产物直接用于与抗CD105抗体缀合。
标记反应:用0.17μmol上述N-羟基琥珀酰亚胺酯(0.2mg/mL在蒸馏水中的贮备液)处理1ml抗体抗CD105 IgG在磷酸盐缓冲盐水(pH7.4)中的溶液(浓度为1mg/mL),并在25℃想温育5小时。通过凝胶色谱法进行纯化(NAP10即用型脱盐柱,洗脱剂:PBS),得到10μmol/ml染料浓度/1.4μmol/ml抗体浓度的溶液。
如上所述测量理化性质,并示于下面表3中。

实施例26:带有Capan-1肿瘤的裸鼠中的微转移成像
将在培养物中亚铺满(subconfluently)生长的Capan-1肿瘤细胞胰蛋白酶化,离心并再悬浮于PBS中。在用锥虫蓝染色并计算细胞浓度后,将细胞悬浮液的浓度设置为3×107ml。将所述细胞悬浮液在冰上冷却直到使用。麻醉三只雌性裸鼠(NMRI裸鼠,体重24-25g),在腹部切口后,将30μl(1×106细胞/动物)细胞悬浮液囊下接种到每只动物的胰腺中。每只动物接受0.05μmol/kg体重(1.3mg/kg体重)的包括实施例10,即结构如式XXVII所示的花青染料的物质,其已经按照实施例22的方法被缀合到EB-DF抗体AP39上。在可触知清楚的肿瘤生长的时间点(在肿瘤细胞植入后约12至14周)静脉内给予该物质。在给予物质后6小时处死动物,并用强化CCD相机在体外拍摄包含微转移的肠系膜的荧光信号。通过用波长为740nm的近红外光照射肠系膜来激发所述物质的荧光,用激光二极管(0.5W输出)产生所述近红外光。数字化地贮存荧光图像。然后,用低倍率显微镜(Stemi2000-C,Fa.Carl Zeis)评价微转移的大小。从0.5至2.0mm直径范围的微转移和从更大的肠系膜转移接收荧光信号,并使其与显微评价相对应。基于实施例在图1中显示了染料缀合物的效力。
实施例27:裸鼠中小的子宫内膜异位病变的体外成像
通过手术在4只NMRI裸鼠中诱导子宫内膜异位。用腹膜内注射赛拉嗪/氯胺酮(体积比2∶10,1ml/kg体重)将所述小鼠麻醉。通过2cm中线切口打开腹部,并将两个人子宫内膜异位组织样品(样品大小约1mm3)固定到腹腔每侧上的腹膜上。在诱导子宫内膜异位后9天,在所有小鼠中进行成像。为了成像,2只小鼠静脉内接受0.05μmol/kg体重(1.3mg/kg b.w.)的实施例20的缀合物(AP39+结构如式XXVII所示的实施例10的标题化合物)。其它两只小鼠接受从BSA和3,3-二甲基-2-{7-[3,3-二甲基-5-磺酸-1-(2-磺酸乙基)-3-H-吲哚鎓-2-基]-4-(6-羧基-4-氧杂己基)庚-2,4,6-三烯-1-亚基}-1-(2-磺酸乙基)-2,3-二氢-1H-吲哚-5-磺酸三钠,内盐(实施例10c)合成的对照缀合物。在给予物质后24小时处死所有动物,并用强化CCD相机体外拍摄包含子宫内膜异位病变的腹膜荧光信号。通过用波长为740nm的近红外光照射包含子宫内膜异位病变的肠系膜来激发所述物质的荧光,用激光二极管(0.5W输出)产生所述近红外光。数字化地贮存荧光图像。在用实施例20的缀合物(AP39+结构如式XXVII所示的实施例10的标题化合物)处理的两只小鼠的子宫内膜异位病变中都观察到了清楚的荧光信号增强,而在用对照物质处理的小鼠中并未观察到。包含病变的荧光的大小小于2mm。基于代表性实施例在图2中显示了染料缀合物的效力。
实施例28:裸鼠中皮肤的自发性微小病变的体内成像
在两只NMRI裸鼠中观察多个自发性微小皮肤病变。每只小鼠静脉内接受0.05μmol/kg体重(1.3mg/kg b.w.)的实施例20的缀合物(AP39+结构如式XXVII所示的实施例10的标题化合物)。在给予物质后6小时,在麻醉的小鼠中进行成像。用吸入麻醉剂异氟烷(Isofluran Curamed,CuramedPharma GmbH,Karlsruhe,Germany)诱导短时间麻醉。通过二极管激光器激发所述物质的荧光(激发波长742nm),并用强化CCD相机检测荧光。数字化地保存荧光图像。然后,用低倍率显微镜(Stemi 2000-C,Fa.Carl Zeis)评价微小病变的大小。从小至小于<1mm的微小病变接收到了荧光信号。基于代表性实施例在图3中显示了所述染料缀合物的效力。
