Abstract

发明背景
肿瘤-宿主相互作用是肿瘤发展的复杂特征并且是抗癌治疗的日益重要的靶标(Tímár等人,″Molecular pathology of tumor metastasisIII.Target array and combinatorial therapies″Pathol Oncol Res 9:49(2003))。癌细胞、免疫效应细胞、炎症细胞、肿瘤脉管系统和基质之间的细胞相互作用显示出复杂的多方面的相互作用。该复杂的相互作用网络的各个成员的功能的治疗调节可以导致更大的肿瘤控制。

如Tímár等人报导的,将白细胞白介素(LI)注射液沿肿瘤外周给药,在2周内每周3次,以约20IU到1600IU或者特别以400IU或者800IU给药,或者进一步每周5次,以约20IU到1600IU或者以400IU或者800IU给药,其中IU代表世界卫生组织首次国际标准关于人IL-2,86/504(World Health Organization IS International Standardfor Human IL-2,86/504)的国际单位。在常规治疗前,将剂量作为第一线治疗给药晚期原发性OSCC患者并在浸润性免疫细胞而非炎症细胞中诱导肿瘤内的改变。如通过增加的CD25+表达所证明的,迁移到癌细胞巢中的T细胞群体和浸润性CD8+T细胞被激活。
尽管在科学文献中通常所认可的,CD8+细胞毒性T细胞是肿瘤破坏所需的,但是OSCC中赘生细胞上I类HLA表达的缺少似乎排除了在OSCC中CD8+细胞的靶定。因此,对OSCC的成功的免疫应答可能需要逆转低的肿瘤内CD4/CD8比率。

因此,在癌细胞巢中抗原呈递细胞(树突细胞)和炎症细胞,特别在嗜中性粒细胞的密度和定位需要特定调节。还需要肿瘤免疫细胞浸润液组成的偏移以逆转肿瘤内CD4/CD8比率和进入肿瘤的单核细胞浸润液结合给药LI以增加对残留的肿瘤细胞的放射疗法的敏化。
还需要诱导肿瘤细胞进入细胞周期的方法,所述细胞周期选自(细胞周期的不同期)G1、S、G2和M,其中该新方法可以与化学疗法、免疫疗法和放射疗法协同应用,并通过改变CD4/CD8比率和进入肿瘤的单核细胞浸润液。
一般而言,还需要预敏化癌肿瘤以及需要由IL-1β与IL-2、TNF-α与IL-2、IFN-γ与IL-2和GM-CSF与IL-2以特定比率构成的无血清并且无促分裂原的混合物,其通过改变CD4/CD8比率和进入肿瘤的单核细胞浸润液出乎预料地证明超过已知组合物或方法的功效。
发明概述
本发明部分地基于用由IL-1β与IL-2、TNF-α与IL-2、IFN-γ与IL-2和GM-CSF与IL-2以特定比率组成的无血清并且无促分裂原的白细胞白介素注射液(LI)混合物改变CD4/CD8比率和进入肿瘤的单核细胞浸润液。本发明还使得开发用作药物或者作为佐剂与治疗性癌症治疗如化学疗法、免疫疗法和放射疗法结合使用的组合物。
在本发明的实施方案中,公开了用无血清并且无促分裂原的细胞因子混合物改变免疫系统的赘生物或者疾病中CD4/CD8比率的方法。该方法提供了用于与放射疗法或者肿瘤细胞杀伤的其他物理方式结合来治疗癌症的预敏化步骤。还预期诱导肿瘤细胞进入选自(细胞周期的不同阶段)G1、S、G2和M的易损的细胞周期阶段的方法。本发明不限于任何特定类型的癌症并且可以包括任何类型的癌症。
特定应用包括在肿瘤周围给药总日剂量的一半的白细胞白介素注射液(LI)和沿淋巴外周给药另外一半的总日剂量,五(5)次每周,为期三(3)周。总日剂量为800IU/mL IL-2,其中IU代表世界卫生组织首次国际标准关于人IL-2,86/504的国际单位。半总剂量是肿瘤外周剂量。因此,将400IU沿肿瘤外周注射到肿瘤块的四个不同部位。将四分之一(1/4)的肿瘤外周剂量注射到每个肿瘤部位使得每个部位接受大约100IU/mL IL-2。将LI皮内给药于可见的/可触知的肿瘤块的周围边缘。将400IU的总剂量的剩余一半沿淋巴管外周同侧顺序给药于相同的肿瘤部位。沿淋巴管外周注射给药于注射的肿瘤块同侧的颈部淋巴管链区域中的后下颌区域。
本发明的另一实施方案包括白细胞白介素注射液(LI),其具有如下特定比率的细胞因子与白介素2(IL-2):IL-1β与IL-2的比率为0.4-1.5,优选0.7+/-0.1(IL-1β/IL-2)、TNF-α与IL-2的比率为3.2-10.9,优选9.5+/-1.8(TNF-α/IL-2)、IFN-γ与IL-2比率为1.5-10.9,优选6.0+/-1.1(IFN-γ/IL-2),和GM-CSF与IL-2的比率为2.2-4.8,优选4.0+/-0.5(GM-CSF/IL-2)。

发明详述和优选实施方案
本发明一般涉及预敏化癌症的方法和由IL-1β与IL-2、TNF-α与IL-2、IFN-γ与IL-2和GM-CSF与IL-2以特定比率组成的无血清并且无促分裂原的混合物。一种此类新颖的细胞因子混合物是白细胞白介素注射液(LI),其已经证明具有免疫调节能力。癌症患者中免疫抑制的临床重要性出乎意料地影响在治疗性治疗尤其在肿瘤细胞进入细胞周期阶段前预敏化癌症的方法。
此外,每周5次,持续三周沿肿瘤外周给药400IU/mL白细胞白介素注射液(LI)和沿淋巴管外周给药400IU/mL改变了CD4/CD8比率和进入肿瘤的单核细胞浸润液。通过向肿瘤块周围四个不同部位注射相当于400IU/mL IL-2的肿瘤外周剂量。四个部位的每一个用全部400IU/mL 肿瘤外周剂量的四分之一(1/4)注射,使得每个部位接受约大约100IU/mL的IL-2。相当于800IU/mL IL-2的总剂量的剩余一半在沿淋巴管外周同侧顺序给药于肿瘤部位并且在相同的部位。淋巴管外周注射液给药于注射的肿瘤块同侧的颈部淋巴管链区域中的下颌后区域。IU代表世界卫生组织首次国际标准关于人IL-2,86/504的国际单位。

用表征方案对白细胞白介素注射液(LI)进行测试并且该注射液不含有下面的细胞因子和其他生物活性小分子:IL-4、IL-7、和IL-15、TfR、sICAM、PDGF-AB、IFN-α、EPO、LTC4、TGF-β2、FGF basic、血管生成素、sE-选择蛋白、SCF和LIF。LI仅含有痕量(仅高于该测定法的检测水平)的IL-12和LTB4。
在所述生产方法中,将单核细胞通过分级梯度离心与从人供者“血沉棕黄层”分离并与PHA一起培养以增强从培养的供者白细胞产生和分泌IL-2和其他细胞因子,如美国专利5,093,479、4,390,623、4,388,309、4,406,830、4,661,447、4,681,844和4,464,355描述,将它们都引入本文作为参考。随后,无菌收获培养物上清液,将其澄清,并进行商业化病毒排阻方法。然后将上清液进一步通过超速离心和微滤浓缩约10倍。

定义
IL-2-白介素2(IL-2):由CD4+辅助T淋巴细胞合成的15.5kD的糖蛋白(以前称作T细胞生长因子)。IL-2对产生它的CD4+T淋巴细胞和免疫系统的其他细胞(包括B淋巴细胞、CD8+T淋巴细胞、NK[自然杀伤细胞]细胞和其他的)具有自分泌效应。
1L-Iβ-白介素1β(1L-Iβ):由活化的单核吞噬细胞合成的17kD细胞因子,其以游离形式在循环系统中存在并且介导炎症应答。它作用于CD4+T淋巴细胞以帮助促进它们的增殖,并且作为生长和分化因子作用于B淋巴细胞。它还诱导单核吞噬细胞的IL-6合成。

CD25+细胞-CD25是单链糖蛋白,通常称作白介素-2-受体(IL-2R)或者Tac-抗原的α链,其具有55kDa的分子量并且存在于活化的T细胞和B细胞以及活化的巨噬细胞上。它作为IL2的受体。CD25抗原与IL-2R的β链一起形成IL-2的高亲和性受体复合物。
CTLL-2(细胞系)-从C57B1/6小鼠得到的小鼠细胞毒性T淋巴细胞的细胞系。该T细胞系依赖于外源IL-2进行生长和增殖。

手术前给药LI导致细胞周期阶段中肿瘤细胞的增加,而不像从现有技术预测的那样增加更快生长和更快复发的肿瘤的风险。LI还改变CD4/CD8比率和进入肿瘤的单核细胞浸润液。诱导肿瘤细胞进入细胞周期的能力是LI特有的并且可以是由于该试验药中存在的不同细胞因子的协同作用和这些细胞因子对宿主免疫系统和肿瘤细胞的差别作用。从LI的依赖剂量的给药也看到典范转移(paradigm shift),改变CD4/CD8比率和进入肿瘤的单核细胞浸润液。
患者

入选标准
在LI治疗组中包括患有以前未治疗的头和颈癌(口腔鳞状细胞癌,OSCC)的21名患者。入选标准是18岁或者以上,患有组织学上确证的头和颈的鳞状细胞癌,没有已知的转移疾病并且预期存活大于6个月,并且以前没有对OSCC进行治疗或者进行任何其他免疫治疗。排除怀孕、以前对LI给药部位进行了放射治疗、存在十二指肠溃疡或者胃溃疡的患者、和哮喘患者。
可评估的患者
总共选择了41名患者,39名是可评估的。LI治疗组由21名患者组成,19名患者可评估。LI治疗组患者中的两名随后证实患有(1)腭垂的未分化癌和(2)口底部的腺癌。给予这两名患者全程治疗,包括给予试验药,但是他们不是组织病理学/免疫组织化学分析的部分,因为他们不满足患有口腔鳞状细胞癌的标准。对照组由20名患者组成,活组织检查证实了口腔鳞状细胞癌,肿瘤位于相同的口腔区域并且肿瘤和疾病阶段与试验药(LI)治疗组的相似。
治疗方案
以下面的方式进行白细胞白介素注射液(LI)的给药:在三周内将一半的(400IU)总日剂量(800IU/mL,按照IL-2)沿肿瘤外周注射(1/4的肿瘤外周剂量[约100IU]在肿瘤块周围的四个部位的每一个),另一半(400IU,以IL-2表示)的总日剂量在淋巴管周同侧给药于肿瘤部位(顺序地并且相同部位),每周5次,为期三周。将LI皮内给药于可见的/可触诊的肿瘤块的圆周边缘。沿淋巴管外周注射给药于所注射的肿瘤块同侧的颈动脉淋巴链的下颌区后部。
在第一次LI给药前三天,给予环磷酰胺的单次静脉内输注,注射300mg/m2。自己口服(与食物一起)吲哚美辛(25mg),每天三次,总的日剂量为75mg,在环磷酰胺给药后3天开始,并且直到手术前24小时。在环磷酰胺给药后3天开始自己给药硫酸锌(500mg,按照元素锌)和多种维生素补充物,每天一次,直到手术前24小时。
所有患者(LI治疗的和对照患者)在最后的LI剂量后1-2周都接受手术,然后在手术后1-4周进行放射疗法。LI预治疗没有不利地影响该患者群体中的伤口愈合。

包括在该试验前,对患者进行了总体评估。还检查他们的医疗史。一旦参加完全的体检,就进行血液学、血液化学诊断检查、胸部心脏放射线照相术和心电图。进行原发肿瘤部位的可能的成像以得到基线图像。对于所有患者,都通过测量两个主要垂直直径对他们的原发肿瘤进行基线双二维测量。在每个后来的就诊时检查患者并使用合格的调查表调查生命质量(例如,痛苦水平、舌灵活程度,等等)。在每次就诊时治疗医生评估毒性。
白细胞白介素注射液(LI)
从美国红十字会(US Red Cross)得到的所选人外周血单核细胞通过FDA要求的所有输血测试后,用促分裂原培养后制备白细胞白介素注射液(LI)。没有允许首次供者。然后将无血清的培养物上清液无菌收获,澄清,进行商业病毒排阻过程,离心,并微滤。向浓缩物加入人血清白蛋白注射液(USP)并将所得溶液缓冲到生理pH,达到目标IL-2浓度,并进行二次微滤。将所配制的药物溶液无菌分配到无菌血清型小瓶中并通过其IL-2含量标记。通过使用细胞毒性T淋巴样(CTLL-2)、IL-2依赖的细胞系通过放射标记的胸苷(体外)的掺入测量产物效价。通过ELISA进一步测试或者通过五种标记细胞因子:IL-2、IL-1β、GM-CSF、IFN-γ和TNF-α的存在来测试最终可注射试剂。将LI进一步进行质量控制测试以检查无菌性、细菌内毒素、pH和总蛋白质浓度和进行其他物理-化学测试。
在硼硅玻璃血清小瓶中提供冷冻的白细胞白介素注射液(LI),小瓶含有2.2mL药物,标签声明为IL-2(400IU/ml),用于肿瘤外周、肿瘤内、淋巴管外周或者皮下给药。将LI进行质量控制测试以检查身份、无菌性、细菌内毒素、pH和总蛋白质浓度。检查每个小瓶的微粒污染和外观。该制剂的总蛋白质含量为3mg/mL,其中提供的物质是无菌且无致热原的。将LI的过期日期指定为当药物在-20℃时,从生产日期开始24个月。
环磷酰胺
提供环磷酰胺注射液USP(Bristol-Myers-Squibb,UK),其为无菌粉剂,每100mg环磷酰胺含有45mg氯化钠,75mg甘露醇或者约82mg碳酸氢钠,在静脉内输注前进行重构。
吲哚美辛
吲哚美辛USP(Sanofi-Synthelabo,France)以25mg片剂提供,与食物一起自己进行口服给药。
硫酸锌和多种维生素
由诊所为每名患者提供硫酸锌(50mg元素锌,R.P.SchererCorporation,Clearwater,Florida,USA)和OTC多种维生素用于自己给药。
病理学
一种病理学方案描述了手术切除的样品的制备和固定和总量、目视和显微镜检查、组织学和免疫生物化学步骤,如本文描述。对可疑的损伤的切除活组织检查诊断口腔损害。将癌症分类为T2-3N0-2M0。仅选择满足所有其他标准的T2-3N0-2M0用于免疫疗法。在LI治疗方案结束时且在患者的肿瘤切除方案之前通过二维/三维测量评估肿瘤应答。将切除的组织置于装有盐水缓冲的福尔马林的预先标记的容器中,过夜固定,然后石蜡包埋并制备薄切片用于苏木精-伊红(H&E)染色、免疫组织化学和肿瘤应答的病理学评估。
组织学
对三种不同的肿瘤区:表面(区1)、中心(区2)和肿瘤基质界面(区3)进行组织病理学分析。从HE染色的切片确定组织学和AJCC分级和坏死肿瘤细胞的发生。通过如下两种方法确定肿瘤中肿瘤上皮组分相对于基质的百分数:根据Mallory染色的结缔组织(毛状体染色)和(2)通过ImagePro分析软件(MediaCibernetics,Silver Spring,MD)测量切片的癌细胞巢(肿瘤上皮)的面积。用DAKO(Glostrup,Denmark)的泛-细胞角蛋白(pan-cytokeratine)抗体来检测含有切除的组织的载玻片标记中的细胞角蛋白,所述抗体标记癌细胞。将载玻片在显微镜下检查并用ImagePro分析软件分析。
单核细胞浸润的表征
通过免疫组织化学(从石蜡切片)确定肿瘤细胞巢附近中存在的单核细胞。将切片脱蜡并用微波处理以恢复抗原性。仅给药商业抗体,以前证明这些抗体始终如一地对脱蜡的石蜡切片染色。用抗髓过氧化物酶抗体(DAKO的小鼠单克隆抗体)标记嗜中性粒细胞,用小鼠单克隆抗-CD34抗体(DAKO)标记造血干细胞,通过CD68抗原的表达(小鼠单克隆抗-CD68)鉴定巨噬细胞群体,通过CD1a标记的表达(小鼠单克隆抗-CD1a,Immunotech,Paris,France)鉴定树突细胞。通过LCA抗原表达(小鼠单克隆抗-CD45,DAKO)鉴定淋巴样细胞。通过小鼠单克隆抗-CD3抗体(DAKO)鉴定T细胞。通过抗-CD8细胞毒性T细胞抗体(DAKO)测定CD8的表达来鉴定细胞毒性T细胞。给药抗-CD4+T细胞抗体(Novocastra,Newcastle Upon Tyne,UK)鉴定效应CD4 T细胞。使用来自Novocastra的CD56/CD57抗原小鼠单克隆抗-CD56和抗-CD57抗体鉴定NK细胞。在所有情况中,制备并评估合适的抗体(相同同种型抗体)阳性和阴性对照载玻片。使用针对IL-2R的小鼠单克隆抗体CD25(Novocastra)鉴定肿瘤上皮和基质内表达IL-2R的细胞。在所有情况中,使用对目的靶标非特异的同种型匹配的单克隆抗体来制备阴性对照载玻片。
所有免疫化学(IHC)标记都用DAKO LSAB-2试剂盒进行,使用生物素化的抗-小鼠/抗兔IgG连接体和链霉抗生物素蛋白-HRP来显现特异结合的抗体。所用的发色团是AEC(红色)标记。用苏木精对切片复染以显示细胞核。
热点技术
基于“热点”技术测定单核细胞的密度。在最高的肿瘤浸润单核细胞密度区测量每个研究的肿瘤区密度。该方法使得组织中细胞浸润的极端不均匀性变得最小。
病理学评估
采用HE使用肿瘤的显微外观评估来自LI治疗组和对照的肿瘤活组织检查。还进行CD3、CD4、CD8、CD1a和CD25标记,条件是肿瘤的大小允许进行所有5种标记步骤。对对照组和LI治疗组使用完整的分析程序。用30倍放大倍数来选择载玻片用于观察。在组织学分析中使用100倍放大倍数。通过对该研究不知情的3个独立病理学家(JT、CSF和BD)进行组织切片的病理学和组织学评估和形态测量。如本领域已知的,可以使用其他病理学和组织学评估。
统计学分析
通过单因子ANOVA分析病理学数据,认为“p”值<0.05(α=0.05)是统计学显著的。
组织学评估
在表3和4中列出了所有LI-处理的(19)和对照(20)患者的组织学诊断、Brodes得分和TNM(肿瘤结节转移)。如表3和4中显示,LI处理的和对照在组织学上并且在角化(Brodes得分)或者TNM阶段方面都没有不同(Odell等人,″The Progostic Value of IndividualHistologic Grading Parameters in Small Lingual Squamous CellCarcinomas;The Importance of the Pattern of Invasion″,Cancer 74:789(1994))。这得到了细胞角蛋白免疫染色模式的证实,表明了两个组中CK-19和pan-CK的高度异质表达。

19名LI治疗的患者中的两名在切除的肿瘤中没有癌组织。认为这两名患者对LI是完全临床应答者,每名显示出如表4中所示的100%肿瘤减少。两个其他病例具有大于50%的组织学证实的肿瘤体积减少并且被认为具有部分(主要)应答。四名额外的患者具有大于30%但是小于50%的体积减小并且被认为具有次要应答。该试验中LI治疗组中的客观应答率为21%,在19名患者中的8名中测量的总体应答为42%,如表4和5中所示的病理学测定。

肿瘤浸润性淋巴细胞
OSCC中的淋巴样细胞浸润液由T细胞占优势,特别是肿瘤基质中。在对照(非LI治疗的)组中,浸润肿瘤和基质的CD8+T细胞数超过CD4+T细胞数(2∶1),如图1所示。结果,在研究的所有肿瘤区域中,对照(非LI治疗的)组中CD4/CD8比率都大大低于1(约0.5),如图2所示。另一方面,LI治疗导致基质和肿瘤上皮巢中CD4+T细胞的密度和数目的显著(p<0.05)增加和CD8+T细胞密度的显著(p<0.05)减小(图2)。当将相同分析仅用于治疗应答组时,该趋势得以保持(图1和3)。结果,LI治疗导致OSCC肿瘤的细胞浸润液组成中从低(<1)CD4/CD8比率到高(>2.5-3.5)CD4/CD8比率的显著偏移(图2和3)。
不论肿瘤区域或者所研究的患者群体如何,在基质或者肿瘤上皮巢中没有发现CD34+单核细胞和CD56+NK细胞。在LI治疗的和对照OSCC患者中报导了类似的发现。
抗原呈递树突细胞(CD1a+)几乎唯一地定位于肿瘤上皮巢中并且在LI治疗组的肿瘤-基质界面中最丰富。观察了LI治疗对所研究的3个区域(表面(R1);肿瘤中心(R2);和肿瘤基质界面(R3)中树突(CD1a+)细胞的分布的差别影响。具体地,在肿瘤中心没有观察到改变;在肿瘤表面检测到CD1a+树突细胞减少和在肿瘤-基质界面处存在的CD1a+树突细胞增加,如图4所示。在临床应答者小组分析中,从肿瘤表面向CD1a+树突细胞的肿瘤-基质界面的偏移最显著,如图4所示。两名完全应答者没有可观察到的肿瘤组织,故没有进行免疫组织化学分析。
炎症细胞
通过分别对髓过氧化物酶(MPX)和CD68标记染色,确定切除的肿瘤样品中嗜中性粒细胞和巨噬细胞浸润液的存在。在肿瘤的三个区(表面(R1);中心(R2);和肿瘤基质界面(R3)中进行形态测定。
巨噬细胞密度的分析表明LI治疗不改变CD68+巨噬细胞的基质存在,但是在上皮内显著减少(p<0.002),如图5所示。
LI治疗显著(p<0.005)增加了嗜中性粒细胞的上皮内密度,而与所分析的区域(R1、R2或者R3)无关,如图6所示。
肿瘤坏死
坏死的组织学外观显示了LI治疗组和对照组之间的显著差异,如图7所示。LI治疗组中多病灶微观坏死(10/17名治疗的患者)比对照组中(4/20名对照患者)显著更频繁。

肿瘤基质/癌巢比率
癌细胞巢相对于肿瘤基质的百分数表明LI治疗对癌巢的选择性作用。用Mallory三色技术染色的所切除的组织样品的癌巢标记为粉红色,并通过形态测定法,使用ImagePro软件测定肿瘤组织中上皮巢的比例,如图8所示。LI治疗导致与对照(非LI治疗的)相比,肿瘤组织中癌细胞巢的比例显著降低(p<0.005,ANOVA单因子分析)。
在对照和LI治疗的患者中上皮周围胶原性疾病是相似的,而LI治疗组中间质上皮内纤维质生成(10/19,或53%治疗患者)比对照组(2/20,或者10%的对照患者)显著(p<0.01)更频繁,如表7所示。

在常规疗法之前作为第一线治疗给药于晚期原发性OSCC患者的白细胞白介素(LI)注射液诱导了浸润性免疫细胞而不是炎症细胞中的肿瘤内改变。T细胞群体迁移到癌巢。当将LI以约20IU到1600IU的范围或者特别地以400IU或者800IU沿肿瘤外周给药,一周三次,为期两周,或者以约20IU到1600IU或者以400IU或者800IU每周给药5次时,浸润性CD8+T细胞增加了CD25+表达。因为在2周治疗方案结束时,该治疗方案不导致肿瘤组织的大块坏死,所以结缔组织与肿瘤上皮组分的比率没有显著改变。
这与本文描述的方法形成对比,其中在手术然后放射疗法之前,用LI进行三周预先治疗后,与对照组相比,该方法导致LI治疗的OSCC患者的肿瘤块显著减小。该方法预期相当于800IU/天IL-2的LI预治疗剂量,每周5天,总共三周,从而诱导浸润性免疫细胞的显著调节。CD8+T细胞数目(p<0.05)减小,肿瘤浸润性CD4+T细胞中相应地显著(p<0.05)增加。这些改变导致肿瘤中CD4/CD8比率的根本改变。
由于LI治疗,在OSCC患者的对照非治疗组中看到的低CD4/CD8比率(<1)在基质和上皮内显著增加(CD4/CD8比>2到>3.5)。从而,晚期原发性OSCC患者的高剂量(定义为:800IU/天IL-2,5天/周,为期3周)非佐剂LI治疗是显著的CD4+T细胞浸润液的典范转移。抗原呈递细胞(树突细胞)和炎症细胞,特别是嗜中性粒细胞的密度和定位的清除且特定的改变由图9的LI的宿主免疫活化的示意图显示。
还发生了抗肿瘤免疫应答如癌巢的多病灶坏死的肿瘤浸润性细胞组分的复杂改变和LI治疗的OSCC患者中检测的结缔组织比例的增加。这些改变反映了通过给药新佐剂LI诱导的针对OSCC的成功的抗肿瘤免疫应答。
对OSCC的成功的免疫应答需要逆转低的(<1)肿瘤内CD4/CD8比率。首次用于实验的未处理的OSCC患者中CD8+T细胞相对于肿瘤和周围基质内CD4+T细胞的优势可以在许多其他实体瘤中看到并且不是OSCC特异的。类似地,已经在癌、宫颈癌和乳腺癌中检测到低的CD4/CD8比率,提示这些肿瘤诱导的患者(宿主)的获得性免疫抑制状态。
LI治疗诱导的OSCC中宿主免疫细胞的细胞浸润液的病理组织学改变导致LI预先治疗组中42%的总体应答率。客观应答率为21%,2个病理证实了19名可评估的LI治疗的患者中的完全应答。

根据本文描述的剂量,对晚期原发性OSCC患者进行LI新佐剂的预治疗导致浸润肿瘤的免疫细胞类型的典范转移(主要是CD4+T细胞而不是CD8+T细胞)。该典范转移导致肿瘤细胞坏死和与主要具有CD8+T细胞浸润并且很少有浸润性CD4+T细胞的对照组中微小的抗肿瘤应答相比,肿瘤块显著减小。与对照(未治疗的)OSCC患者相比,肿瘤免疫细胞浸润液组成典范转移(肿瘤内CD4/CD8比率的逆转)以及以前报导的临床发现,即LI治疗的患者显示出对残余肿瘤细胞的放射疗法的敏感性增加,和如通过延长复发的时间所测量的治疗的生物学效应和LI治疗的OSCC患者的复发率的减小一起得到了治疗OSCC和可能地其他实体瘤的新方法。
从而描述了本发明,显然可以在许多方面对本发明进行变通。此类变通方案不应认为是背离了本发明的精神范围并且所有此类修改都意在包括在下面的权利要求的范围内。

1.改变肿瘤中CD4/CD8比率的方法,其包括
向肿瘤给予治疗活性量的无血清且无促分裂原的细胞因子混合物的步骤。

14.权利要求13的无血清且无促分裂原的细胞因子混合物,其中所述细胞因子的特定比率如下:

GM-CSF与IL-2的比率为3.5-4.5。
15.用于治疗癌症的药物组合物,其包含选自由IL-1β、TNF-α、IFN-γ和GM-CSF构成的组的细胞因子与白介素-2(IL-2),所述细胞因子与白介素-2(IL-2)的特定比率如下:
IL-1β与IL-2的比率为0.4-1.5;
TNF-α与IL-2的比率为3.2-10.9;

并且任选地与药学上可接受的赋形剂、载体或添加剂组合。
