技术领域

背景技术
在手术中使用内窥镜操作已被广泛接受。在此使用的术语内窥镜定义为包括所有类型的微创手术操作,这些操作包括腹腔镜以及关节内窥镜操作。因此,发展了许多内窥镜仪器,这些仪器允许外科医生利用特定体腔或解剖区域周围皮肤和组织内极微小的切口就能执行复杂的外科操作。为了将内窥镜仪器引入体腔,常常需要使用套管针刺穿体腔并且将导管插入体腔。在现有技术中广泛已知套管针,其一般包括闭塞器和套管针插管。在于2000年1月25日颁发给Frederick等人的美国专利6,017,356中,能够查到一个套管针实例,其通过参考在此引入。

发明内容
具体而言,本发明提供一种对病人执行操作的套管针,所述套管针包括:
有远端和近端的中空插管;
具有接附到所述插管的所述近端的远端和具有与其接附的壁的近端的壳体,所述壁具有贯穿其间的孔;
位于所述壳体内的仪器密封组件,其包括第一充分刚性的环、第二充分刚性的环,以及多个分开的压在一起的密封件部分,各密封件部分具有180度到270度的圆周且彼此一起叠成层,以形成具有远端孔眼的圆锥形密封件,并且适于密封穿过所述密封组件的仪器。

具体实施方式
现在参考附图,在所有视图中相同的数字指示相同的元件,图1显示了根据本发明的套管针100。套管针100可以是类似颁发给Frederick等人的美国专利6,017,356中描述的套管针,其通过参考在此引入。套管针100包含插管部分110和闭塞器部分。闭塞器部分对本发明所属技术领域的普通技术人员来说是公知的,闭塞器可以是有叶片的也可以是没有叶片的,例如由俄亥俄州辛辛那提市的EthiconEndo-Surgery销售的ENDOPATH II套管针上使用的那些闭塞器。闭塞器部分具有穿孔尖122,其用于刺穿病人组织,直到到达腹腔。在穿入腹腔后,闭塞器部分可以从插管部分110卸下。卸下闭塞器部分后,任何数量的例如象组织结扎仪器那样的手术仪器,可以通过套管针插管插入,从而执行手术操作。

通过将保护器嵌入冠顶环30的远端表面31和垫圈环20的近端表面22之间,将保护器40接附在密封组件2上。冠顶环30的远端表面31上的冠顶销32穿过保护器40的保护器凸缘44内的保护器孔眼42。这些冠顶销32也固定在垫圈环20的近端表面22上的冠顶孔眼28内,从而保持在两部分表面之间的合适位置处固定的保护器40。
当组装为密封组件2时,保护器如上所述那样一起叠成层,而密封件部分如下所述那样一起叠成层。参考图3可以更好地看出,垫圈销10穿过密封件部分4的孔眼6,嵌入垫圈销孔眼26内,而垫圈销29嵌入保持器环8的孔眼14内,从而将垫圈环20与垫圈保持器环8固定在一起,而密封件部分位于其间。冠顶销32穿过保护器40的孔眼42,嵌入冠顶孔眼28内,从而有助于将垫圈环20固定到冠顶30上,而保护器40位于其间。当所有组装完后,就形成了密封组件2(在图7中显示)。
在本技术领域中公知密封件保护器40,在于1994年5月3日颁发给Hart等人的美国专利5,308,336中,对保护器进行了描述,其通过参考在此引入。如在图2-6中所示,保持器环组件3包括四个密封件部分4a-4d、垫圈保持器环8以及垫圈环20。可以使用任何数量的密封件部分,在此的本发明不限于4个密封件部分。密封件部分4a-4d中的每一个可以模制成平的或非平面状,密封件部分可以由弹性体制成,例如硅树脂,但不仅限于弹性体。
在另一实施例中,在垫圈保持器环8和垫圈环20之间,销和孔眼的配合被分离了。四个销和十二个孔眼可以和垫圈环20成一整体,而十二个销和四个孔眼可以和垫圈保持器环8成一整体。随后密封件“交织”在垫圈环20上,每一个密封件部分只需要放置在3个销上。这与先前描述的实施例中需要的10个销形成了对比。这些销将密封件保持在适当的结构中,提供垫圈孔眼6与其余的销孔眼对(10和26)的对准。当垫圈保持器环8装配到垫圈环20上时,垫圈保持器环的十二个销穿过对准的垫圈孔眼,从而与先前实施例中一样地定住密封件。可使用垫圈构件8和20上的任何数量的销和孔或销和孔的组合。
作为另一种选择,可仅利用四个销穿过垫圈孔眼(6)来实现确实固定。四个销可位于垫圈环20上,而四个相应的孔眼位于垫圈保持器环上。密封件部分(4a-4d)具有只相应于这些位置的孔眼。为了补充与这些销孔眼组合相关的固定,在垫圈环20和垫圈保持器环8上可设置额外的“夹紧销”。这些夹紧销为圆锥形突起,其从垫圈环20的远端表面24和垫圈保持器环8的近端表面11上伸出。每个部分上的这些圆锥形突起彼此径向对齐,并且当垫圈环20和垫圈保持器环8组装在一起时,压缩密封件部分的周边凸缘。由于圆锥形突起上尖端的锋利,其“插入”到密封件部分的柔性材质上,利于密封件部分周边的固定。

以上描述的实施例的密封件的交织设计允许一个密封件部分的单个厚度在仪器使用的大多数时间里接触仪器(有时,重叠使得两个密封件部分在局部区域与仪器产生接触)。这不像其他分段密封件那样,在指定位置需要多个密封件层与仪器接触来提供充足的密封。因此,交织设计提供了比其他分段密封件设计更好的性能。在与仪器使用有关的力的所有区域,这是十分明显的。
此外,因为描述的实施例降低了应变,所以在密封件中心制造比唇形密封件通常所需的孔眼更小的孔眼是可能的。在唇形密封件设计中,与仪器使用有关的力大受孔眼尺寸减小的影响。虽然对于本申请的分段密封件设计也有一些冲击,但影响大大减小了。因此,通过减小中心的孔眼尺寸,当小仪器轴向偏移到套管针通道一侧时,分段密封件能够获得改良的维持吹入的性能,使得对仪器使用作用力有非常小的影响。作为另一种选择,密封件可以合起来,使得不存在孔眼,以提供零闭合设计,当没有仪器时以及存在仪器时都可密封。
密封组件最好通过例如波纹管的浮选装置或系统来安装在套管针上。在于1995年1月31日颁发给Hart等人的美国专利5,385,553中,对这种密封浮选系统进行了描述,其通过参考在此引入。通过参考图7,可以看到:密封组件2连接到浮选系统80上。波纹管80允许密封组件径向移动。径向偏移波纹管所需的径向力比仪器施加的接触压力要小得多。这使得浮选系统发生偏移,而密封件部分仍保持着与仪器的密封状态。保护器凸缘44最先放置在环表面22的顶端。浮选装置在其内径处可具有带孔眼的凸缘,其可位于保护器凸缘上,使得波纹管凸缘内的孔眼与保护器42内的孔眼对齐。这导致了浮选装置夹在环31和保护器44之间。浮选系统有助于防止保护器无意间地上下移动。
图8显示了垫圈环120,作为垫圈环20的替代实施例。垫圈环120与垫圈环20十分类似。然而,垫圈环120具有从其底部或远端表面124伸出的若干突出200。突出200最好为圆锥形。类似的,图9显示了垫圈保持器环108,作为垫圈保持器环8的替代实施例。垫圈保持器环108与垫圈保持器环8十分类似。然而,垫圈保持器环108具有从其顶端或近端表面111伸出的若干突出302。突出302最好为圆锥形。这些圆锥形突起或“夹紧销”可位于垫圈环120和垫圈保持器环108两者上,或仅位于两者之一上。当在108和120两者上时,这些圆锥形突起彼此径向校准,并且当垫圈环20和垫圈保持器环8组装在一起时,压缩密封件部分的周边凸缘。由于圆锥形突起上尖端的锋利,其“插入”到密封件部分的柔性材质上,利于密封件部分周边的固定。
此外,垫圈环120和垫圈保持器环108可以用超声波焊接在一起。本领域技术人员已知的这个过程,利用超声波振动将两个部件压在一起。此过程迫使对齐的突起302从近端211和远端124表面穿入密封件部分的柔性材质。一旦突起302穿透密封件部分,就利用超声波振动将其焊接到一起。可以利用垫圈构件108和120上的任何数量的销和孔或者销和孔的组合以及/或突起202。

Claims (8)
1、一种对病人执行操作的套管针,所述套管针包括:
a.有远端和近端的中空插管(202);
b.具有接附到所述插管的所述近端的远端和具有与其接附的壁的近端的壳体(210),所述壁具有贯穿其间的孔(218);
c.位于所述壳体内的仪器密封组件(2),其包括第一充分刚性的环(8)、第二充分刚性的环(20),以及多个分开的压在其间的密封件部分(4a-4d),各密封件部分具有180度到270度的圆周且彼此一起叠成层,以形成圆锥形密封件(5),并且适于密封穿过所述密封组件的仪器(90)。
2、根据权利要求1所述的套管针,其特征在于,所述多个密封件部分为层状弹性体构件。
3、根据权利要求1或2所述的套管针,其特征在于,所述密封件部分包括近端凸缘部分(70),以及从近端凸缘向远端延伸的向内延伸部分(72),其中所述近端凸缘部分位于所述环之间并且靠着所述环。
4、根据权利要求1或2所述的套管针,其特征在于,所述密封组件具有接附在浮选装置上的外周边。
5、根据权利要求1或2所述的套管针,其特征在于,所述密封组件包括若干位于所述密封件部分近端的保护器(40)。
6、根据权利要求1或2所述的套管针,其特征在于,所述多个密封件部分设置为使得在所述密封组件内具有大致在中心定位的眼孔(74)。
7、根据权利要求1或2所述的套管针,其特征在于,在组装在一起之前,所述多个密封件部分具有非平面形状。
8、根据权利要求1或2所述的套管针,其特征在于,所述套管针包括一个零关闭密封。
