Abstract

本发明公开了一种心脏流出道带瓣补片,带瓣补片包括内衬片、外衬支撑片和瓣叶,内衬片上具有一个近似半圆弧形狭缝,所述瓣叶下缘沿狭缝穿设于内衬片与外衬支撑片之间,且瓣叶上沿与内衬片之间构成一个自然的瓣兜,缝线沿内衬片狭缝边缘将内衬片、瓣叶下缘和外衬支撑片三者缝合在一起,瓣叶下缘被固定在内衬片、外衬支撑片之间。本发明瓣叶与内衬片和外衬支撑片缝合在一起,且瓣叶下缘位于内衬片和外衬支撑片之间,免除了仅靠缝线固定瓣叶,使瓣叶的牢固性更好,可以显著提高患者术后远期治疗效果。

心脏流出道带瓣补片
技术领域
本发明涉及一种改建或重构异常心脏流出道的修补器械,特别涉及用于加宽心脏流出道的带瓣补片。
背景技术
先天性紫绀型心脏病又称法乐氏四联症(Tetralogy of Fallot),该病患者的病变之一是其右室流道狭窄(肺动脉发育异常)。因此,需要进行外科手术治疗以修补并加宽右室流出道。以往在这种手术中,心外科医生通常需要采用自体心包片或用牛心包补片作为修补材料以加宽或重建流出道。然而,多数患者因伴有肺动脉瓣关闭不全或无肺动脉瓣而导致右心负荷加重,极大地影响了患者术后治疗效果。为此,使某些心外科医生不得不在手术过程中自行于补片材料上缝上一个瓣叶,此举虽能改善术后效果但因牢固性有限,或材料发生钙化等导致长期结果不能令人满意。此外,也有部分有条件的医生采用同种异体主动脉瓣或同种异体肺动脉瓣,作为右室流出道修补或替代。虽然可以受到较好临床效果,但因同种材料的来源有限,其储存、运输都存在诸多问题。
发明内容
本项发明所要解决的技术问题是提供一种制成成品的心脏流出道带瓣补片,这种带瓣补片在加宽流出道的同时较好地解决肺动脉瓣关闭不全所引起的返流,更重要地是补片的瓣叶不仅可以明显改善术后患者血流动力学,同时,特有的结构所赋予人工瓣叶具有极好的耐疲劳性,显著提高术后患者远期生活质量。
为解决上述技术问题,本发明所采取的技术方案是:一种心脏流出道带瓣补片,所述带瓣补片包括内衬片、外衬支撑片和瓣叶,所述内衬片上具有一个近似半圆弧形狭缝,所述瓣叶下缘沿狭缝穿设于内衬片与外衬支撑片之间,且瓣叶上沿与内衬片之间构成一个自然的瓣兜,缝线沿内衬片狭缝边缘将内衬片、瓣叶下缘和外衬支撑片三者缝合在一起,瓣叶下缘被固定在内衬片、外衬支撑片之间。
其中,所述心脏流出道带瓣补片自瓣叶上缘以上部分为只具有内衬片的单层结构;所述内衬片沿瓣叶上缘上延3.5~8.5cm,外衬支撑片与内衬片于瓣叶上缘处缝合在一起,所述内衬片和外衬支撑片沿瓣叶下缘向下延长1.5~6.5cm;所述内衬片、瓣叶和外衬支撑片为同种组织片或经化学改性处理的异种组织片;所述经化学改性处理的异种组织片为经抗钙化改性处理的牛心包片;所述瓣叶下缘置于内衬片和外衬支撑片片之间的部分切有数个切口,以使瓣叶下缘缝合更加平整。
本发明所能达到的有益效果是:
1、瓣叶与内衬片和外衬支撑片缝合在一起,且瓣叶下缘位于内衬片和外衬支撑片之间,免除了仅靠缝线固定瓣叶,使瓣叶的牢固性更好,可以显著提高患者术后远期治疗效果。
2、瓣叶与内衬片和外衬支撑片缝合时,夹于内衬片和外衬支撑片的瓣叶下缘周边的部分有数个切口,可使缝合更平整,减少了应力集中现象。
3、瓣叶与内衬片均经过抗钙化处理,可有效地防止瓣叶和内衬片过早发生钙化,可以显著提高患者术后远期治疗效果。
4、心脏流出道带瓣补片自瓣叶上缘以上部分为单层,无外衬支撑片,更加易于流出道的缝合。
5、心脏流出道带瓣补片自瓣叶上缘以下的部分为双层,由于有外衬支撑片,从而有针对加固右心室,以消除右室收缩时易出现的反常博动。

具体实施方式
参阅图2、图3,流出道单瓣补片包括由经过化学改性处理的牛心包片裁切制成的内衬片1、外衬支撑片2和瓣叶3,内衬片1上裁切有一半圆弧形的狭缝4(参阅图1),瓣叶3的下缘沿狭缝4塞于内衬片1、外衬支撑片2之间,且瓣叶3与内衬片1之间具有一定的空间,形成一瓣兜,内衬片1、外衬支撑片2和瓣叶3通过沿狭缝4的两边缘的缝线6缝合在一起。为使缝合平整,在瓣叶3置于内衬片1、外衬支撑片2之间的下缘部分,裁切两个切口5,这样也可以有效地减少应力集中现象。通常,带瓣补片自瓣叶3上缘以上部分为单层,只具有内衬片1,且长度约为3.5~4.5cm,补片自瓣叶3上缘以下的部分均为双层,具有内衬片1和外衬支撑片2,其长度约为4.5~6.5cm。
手术过程中,只需将内衬片1裁成所需形状,置于流出道缺损部分,并与流出道缝合在一起,大大节约了手术时间。由于瓣叶3置于内衬片、外衬支撑片之间,连接更为牢固,可大大加强对血液的抗冲击能力。

Claims (6)

1.一种心脏流出道带瓣补片,其特征在于:所述带瓣补片包括内衬片、外衬支撑片和瓣叶,所述内衬片上具有一个近似半圆弧形狭缝,所述瓣叶下缘沿狭缝穿设于内衬片与外衬支撑片之间,且瓣叶上沿与内衬片之间构成一个自然的瓣兜,缝线沿内衬片狭缝边缘将内衬片、瓣叶下缘和外衬支撑片三者缝合在一起,瓣叶下缘被固定在内衬片、外衬支撑片之间。
2.根据权利要求1所述的一种心脏流出道带瓣补片,其特征在于:所述心脏流出道带瓣补片自瓣叶上缘以上部分为只具有内衬片的单层结构。
3.根据权利要求1或2所述的一种心脏流出道带瓣补片,其特征在于:所述内衬片沿瓣叶上缘延设3.5~4.5cm,外衬支撑片与内衬片于瓣叶上缘处缝合在一起,所述内衬片、外衬支撑片沿瓣叶下缘延设4.5~6.5cm。
4.根据权利要求1所述的一种心脏流出道带瓣补片,其特征在于:所述内衬片、瓣叶和外衬支撑片为同种组织片或经化学改性处理的异种组织片。
5.根据权利要求4所述的一种心脏流出道带瓣补片,其特征在于:所述经化学改性处理的异种组织片为经抗钙化改性处理的牛心包片。
6.根据权利要求1所述的一种心脏流出道带瓣补片,其特征在于:所述瓣叶下缘置于内衬片、外衬支撑片之间的部分切有数个切口,以使瓣叶下缘缝合更加平整。
