Gerät zur Durchführung von Impfungen, insbesondere solchen gegen Pocken
Das Impfen gegen Pocken wird mit einem lebenden, aber geschwächten Pockenvirus, insbesondere Kuhpokkenvirus, durchgeführt, der auch gegen den gefährlichen Kuhpockenvirus, den sogenannten Variola-Virus immun macht.
Das Prinzip beim Impfen besteht darin, dass der Impfstoff in das Körpergewebe eingeführt wird. Die Methode hierfür ist in den einzelnen Ländern verschieden. Es sind mehr äusserliche Methoden bekannt, bei deren Anwendung der Impfstoff in oder unter die Haut eingespritzt wird. In Schweden z. B. wird er in die Haut eingeführt, was auf zweierlei Weise durchgeführt werden kann, nämlich nach einer älteren Methode oder der sogenannten Mehrfach-Druckmethode, mit der sich die vorliegende Erfindung befasst. Diese letztere Methode wird bisher in der Weise durchgeführt, dass ein oder mehrere Tropfen des Impfstoffes auf die Haut aufgebracht und dann mit einem spitzen Instrument, z.
B. einer Nadel, Lanzette oder dergleichen, das im wesentlichen parallel zur Hautoberfläche gehalten wird, wiederholte Eindrücke (20 bis 30) der Spitze des Instrumentes gegen die Hautoberfläche mit einer Bewegungsrichtung durchgeführt werden, die senkrecht zur Haut gerichtet ist. Dieses Einschneiden findet bei gespannter Haut in einem Bereich statt, der einen Durchmesser von etwa 3 bis 5 Millimeter hat. Durch diese Druckeinwirkungen auf die Haut entstehen in der hornigen Schicht kleine Risse und Brüche, durch welche die Virus-Partikelchen mittels der Spitze des Instrumentes in das stratum germinativum niedergebracht werden. Möglicherweise werden sie durch Kapillarkräfte in diese Schicht heruntergesaugt. Hierbei tritt keine oder nur eine unbedeutende Blutung auf. Es entsteht auch kein merklicher Schmerz, wenn der Impfvorgang vorschriftsmässig durchgeführt wird.
Beim Impfen ist für jeden Patienten ein neues steriles Instrument, z. B. eine neue Nadel, zu benutzen, oder das Instrument bzw. die Nadel muss erhitzt werden, wenn sie das Erhitzen aushält.
Durch die vorliegende Erfindung soll die Mechanisierung dieses bekannten Impfvorganges erreicht und hierdurch die für jede Impfung erforderliche Zeit derart herabgesetzt werden, dass mit der Mehrfach-Druckimpfung statt wie bisher mit Hilfe eines Assistenten 250 Personen nunmehr etwa 700 Personen in der Stunde geimpft werden können.
Das erfindungsgemässe Gerät ist gekennzeichnet durch einen von einem Handgriff getragenen elektromagenetischen Vibrator zum Erzeugen von Vibrationen eines Impfinstrumentes sowie eine zu dessen elektrischer Erhitzung dienende Vorrichtung.
Nachstehend ist die Erfindung anhand der in der Zeichnung als Beispiele dargestellten Ausführungsformen des Impfgerätes näher beschrieben.

Das Gerät nach Fig. 1 weist ein Gehäuse 1 mit einem Handgriff 2 auf, durch den eine elektrische Stromschnur 3 hindurchgeführt ist.
Das Gehäuse 1 enthält einen elektromagnetischen Vibrator bekannter Art, der in Fig. 3 mit V bezeichnet ist und dessen Vibrationsarm 4 mit einer Öffnung zum Einsetzen eines Halters 5 für eine Nadel oder Lanzette P versehen ist. Die Nadel bzw. Lanzette ist mittels einer Schraube 6 in dem Vibrationsarm befestigt. Ihr Ende bildet zusammen mit dem Ende einer sie in dem Halter umgebenden Heizspule 7 Elektroden 8, die an durch den Vibrationsarm 4 hindurchgeführten Drähten 9 anliegen. Die Befestigung des Halters 5 in dem Arm 4 ist in grösserer Darstellung aus dem horizontalen Schnitt nach Fig. 2 zu ersehen. Die Heizspule 7 ist mit ihrem vorderen Ende an der Nadel P angeschweisst, der andere Teile der Nadel ist aber von diesem Teil der Nadel isoliert.
Anstelle der Nadel mit der Heizspule kann auch eine geschlitzte Lanzette verwendet werden, die als Glühwiderstand dient und deren durch das Schlitzen gebildete Enden als Elektroden 8 dienen. Die Vibrationsamplitude des Vibrators ist mittels einer Schraube 10 in bekannter Weise einstellbar. An dem Gehäuse ist eine Lampe L1 angebracht, die zur Beleuchtung des Hauptbereiches dient, in dem die Impfung durchgeführt wird.
Die Lampe kann auch in das Gehäuse eingebaut sein.
Ferner ist an beiden Seiten des Gehäuses 1 je ein doppelt gebogenes Stahldrahtglied 11 mit seinen Enden dreh- und verschiebbar angeordnet, so dass es in eine Stellung gebracht werden kann, in der es zur Abstützung des Gerätes auf der Haut dienen kann. An dem Handgriff 2 ist in nicht dargestellter Weise eine Ausgabevorrichtung für den Impfstoff angebracht, die durch eine Spritze aus einem Zylinder 12 und einem in diesem verschiebbaren Kolben 13 gebildet ist. Der Kolben 13 steht in Gewindeeingriff mit einer Schraube 14, die mittels eines an ihr befestigten Rändelknopfes 15 drehbar ist.
Am vorderen Ende des Zylinders 12 ist das eine Ende eines Kunststoffschlauches 16 abnehmbar befestigt, dessen anderes Ende 17 durch ein an dem Gehäuse 1 angebrachtes Rohr 18 hindurchgeführt ist, das mit einer Halteschraube 19 für den Schlauch versehen ist. Gegebenenfalls kann der Schlauch 16 entsprechend der strichpunktierten Linie 20 als Wicklung in eine Nut eines nicht dargestellten kreisförmigen Innenflansches eingelegt sein. Die Lampe L,, der Heizstromkreis für die Nadel bzw. Lanzette P und die Vibratorwicklung V sind, wie aus Fig. 3 ersichtlich, durch den Schalter S3, den Schalter S2 und den Umschalter 0 verbindbar.
In Fig. 3 stellt das durch strichpunktierte Linien begrenzte Rechteck H den Handgriff des Gerätes nach Fig. 1 dar, während das strichpunktierte Rechteck L ein Gerätegehäuse darstellt, das eine Stromquelle enthält und z. B. auf dem Fussboden aufstellbar ist. Der Stecker 22, der in eine Wanddose einsteckbar ist, ist über einen Hauptschalter S, und Sicherungen 23 mit der Primärwindung T1 eines Transformators T und mit einer Glühlampe L2 verbunden, die parallel zu der Primärwindung geschaltet ist und erregt wird, wenn der Transformator eingeschaltet ist. Die Sekundärwicklung des Transformators T besteht aus zwei hintereinandergeschalteten Wicklungen T2, T3. Die Wicklung T2 erregt über den Schalter S3 die Beleuchtungslampe L1 (vgl.
Fig. 1) und parallel hierzu den Heizstromkreis über den Umschalter 0 und den Schalter S2. Die Wicklung T5 dient dazu, der Vibratorwicklung V über den Umschalter 0 Strom zuzuführen. Der Lampenschater S3 ist ein Kippschalter mit zwei Stellungen und in seiner geöffneten Ruhestellung gezeigt. Der Schalter S2 und der Umschalter 0 sind Mikroschalter und insbesondere für hohe Ströme ausgebildet. Sie werden automatisch in ihre dargestellten Ausgangs stellungen zurückgeführt. Wie aus dem Schaltschema ersichtlich ist, kann der Strom zu dem Heizschalter S3 mittels des Schalters S2 nur geschlossen werden, wenn der Umschalter 0 seine Ausgangsstellung einnimmt, d. h., wenn der Vibrator ohne Strom ist.
Die Nadel oder die Lanzette kann daher beim Impfen nicht zum Glühen gebracht werden, so dass die Gefahr, einem Patienten Brandwunden zuzufügen auf diese Weise beseitigt ist.
Nach Fig. 4 ist das Gerät mit einem einstellbaren Zeitschalter bekannter Art versehen, durch den die Zahl der Vibrationen des Vibrators nach dem Umstellen des Umschalters 0 in die Stellung b begrenzt werden kann.
Entsprechend der erläuterten Mehrfach-Druckmethode sind 20 bis 30 Eindrückungen der Haut als Norm festgestellt worden.
Wenn die Eindrückungen in die Haut in grösseren zeitlichen Abständen erfolgen sollen, d. h. die Vibrationen langsamer aufeinander folgen sollen, kann die Vibratorspule durch einen Generator bekannter Art und mit entsprechender Frequenz gesondert erregt werden.
Hierzu kann z. B. der in Fig. 5 dargestellte Röhrenoszillator OSC dienen. Die Anordnung nach Fig. 5 unterscheidet sich von derjenigen nach Fig. 3 nur in bezug auf den das Gehäuse bildenden Teil L, während der andere Teil identisch der Fig. 3 entspricht und daher in Fig. 5 weggelassen ist.
Nach Fig. 6 ist das Gerät mit einer automatischen Ausgabevorrichtung für den Impfstoff versehen, die vorzugsweise in dem Gehäuse L angeordnet ist und einen Elektromagnet VM bekannter Art, vorzugsweise einen Drehmagnet, aufweist. Wenn dieser Magnet eingeschaltet ist, dreht er ein Zahnrad 24 um einen einstellbaren Drehwinkel. Das Zahnrad 24 wird durch eine Feder 25 gegen ein zweites Zahnrad 26 gepresst und dreht dieses um denselben Drehwinkel zusammen mit der Schraube 14a, die in ein Innengewinde des Kolbens 13a eingreift und hierdurch den Kolben 13a in dem Zylinder 12a um ein bestimmtes Mass verschiebt. Wie bei der Ausführungsform nach Fig. 1 ist der dem Zylinder 12 entsprechende Zylinder 12a mit dem einen Ende des Schlauches 16 verbunden, dessen anderes Ende in das an dem Handgriff 18 angeordnete Rohr 17 eingeführt ist.
Der Drehmagnet VM kann eingestellt werden, um einen Tropfen oder eine bestimmte Anzahl von Tropfen des Impfstoffes durch den Schlauch 16 abzugeben.
Device for carrying out vaccinations, especially against smallpox
The vaccination against smallpox is carried out with a live but weakened smallpox virus, in particular cowpox virus, which also makes you immune to the dangerous cowpox virus, the so-called variola virus.
The principle behind vaccination is that the vaccine is introduced into the tissues of the body. The method for doing this is different in the individual countries. More external methods are known where the vaccine is injected into or under the skin when used. In Sweden z. B. it is inserted into the skin, which can be done in two ways, namely by an older method or the so-called multiple pressure method, with which the present invention is concerned. This latter method has hitherto been carried out in such a way that one or more drops of the vaccine are applied to the skin and then with a pointed instrument, e.g.
B. a needle, lancet or the like, which is held essentially parallel to the skin surface, repeated impressions (20 to 30) of the tip of the instrument against the skin surface with a direction of movement which is perpendicular to the skin. When the skin is taut, this incision takes place in an area that has a diameter of about 3 to 5 millimeters. These pressure effects on the skin cause small cracks and breaks in the horny layer, through which the virus particles are brought into the stratum germinativum using the tip of the instrument. They may be sucked down into this layer by capillary forces. There is little or no bleeding. There is also no noticeable pain if the vaccination process is carried out according to the instructions.
When vaccinating, a new sterile instrument, e.g. B. a new needle, or the instrument or needle must be heated if it can withstand the heating.
The present invention is intended to achieve the mechanization of this known vaccination process and thereby reduce the time required for each vaccination so that with multiple pressure vaccination, instead of 250 people as previously with the help of an assistant, about 700 people can be vaccinated per hour.
The device according to the invention is characterized by an electromagnetic vibrator carried by a handle for generating vibrations of a vaccination instrument, as well as a device serving for its electrical heating.
The invention is described in more detail below with reference to the embodiments of the vaccination device shown as examples in the drawing.

The device according to FIG. 1 has a housing 1 with a handle 2 through which an electric power cord 3 is passed.
The housing 1 contains an electromagnetic vibrator of a known type, which is designated by V in FIG. 3 and the vibration arm 4 of which is provided with an opening for the insertion of a holder 5 for a needle or lancet P. The needle or lancet is fastened in the vibration arm by means of a screw 6. Its end, together with the end of a heating coil 7 surrounding it in the holder, forms electrodes 8 which bear against wires 9 passed through the vibration arm 4. The fastening of the holder 5 in the arm 4 can be seen in a larger representation from the horizontal section according to FIG. The front end of the heating coil 7 is welded to the needle P, but the other part of the needle is isolated from this part of the needle.
Instead of the needle with the heating coil, a slotted lancet can also be used, which serves as a glow resistor and whose ends formed by the slits serve as electrodes 8. The vibration amplitude of the vibrator can be adjusted in a known manner by means of a screw 10. A lamp L1 is attached to the housing and is used to illuminate the main area in which the vaccination is carried out.
The lamp can also be built into the housing.
Furthermore, a double bent steel wire member 11 is arranged with its ends rotatable and displaceable on both sides of the housing 1 so that it can be brought into a position in which it can serve to support the device on the skin. A dispensing device for the vaccine is attached to the handle 2 in a manner not shown, which is formed by a syringe made up of a cylinder 12 and a piston 13 which can be displaced in this. The piston 13 is in threaded engagement with a screw 14, which is rotatable by means of a knurled knob 15 attached to it.
At the front end of the cylinder 12, one end of a plastic tube 16 is detachably attached, the other end 17 of which is passed through a tube 18 attached to the housing 1 which is provided with a retaining screw 19 for the tube. If necessary, the hose 16 can be inserted as a winding in a groove of a circular inner flange (not shown) according to the dash-dotted line 20. The lamp L 1, the heating circuit for the needle or lancet P and the vibrator winding V can be connected through the switch S3, the switch S2 and the changeover switch 0, as can be seen from FIG.
In Fig. 3, the bounded by dash-dotted lines rectangle H represents the handle of the device according to FIG. 1, while the dash-dotted rectangle L represents a device housing which contains a power source and z. B. can be set up on the floor. The plug 22, which can be plugged into a wall socket, is connected via a main switch S and fuses 23 to the primary winding T1 of a transformer T and to an incandescent lamp L2 which is connected in parallel to the primary winding and is energized when the transformer is switched on . The secondary winding of the transformer T consists of two windings T2, T3 connected in series. The winding T2 excites the lighting lamp L1 via the switch S3 (cf.
Fig. 1) and parallel to this the heating circuit via the changeover switch 0 and the switch S2. The winding T5 is used to supply current to the vibrator winding V via the changeover switch 0. The lamp switch S3 is a toggle switch with two positions and shown in its open rest position. The switch S2 and the changeover switch 0 are microswitches and are designed in particular for high currents. They are automatically returned to their shown starting positions. As can be seen from the circuit diagram, the current to the heating switch S3 can only be closed by means of the switch S2 when the changeover switch 0 is in its initial position, i. i.e. when the vibrator is without power.
The needle or the lancet cannot therefore be made to glow during vaccination, so that the risk of inflicting burns on a patient is eliminated in this way.
According to FIG. 4, the device is provided with an adjustable time switch of a known type, by means of which the number of vibrations of the vibrator can be limited after switching the switch 0 to position b.
According to the multiple printing method explained, 20 to 30 impressions of the skin have been established as the norm.
If the impressions in the skin are to take place at longer intervals, i. H. the vibrations are to follow one another more slowly, the vibrator coil can be excited separately by a generator of a known type and with a corresponding frequency.
For this purpose, z. B. the tube oscillator OSC shown in Fig. 5 are used. The arrangement according to FIG. 5 differs from that according to FIG. 3 only with regard to the part L forming the housing, while the other part corresponds identically to FIG. 3 and is therefore omitted in FIG.
According to FIG. 6, the device is provided with an automatic dispensing device for the vaccine, which is preferably arranged in the housing L and has an electromagnet VM of a known type, preferably a rotary magnet. When this magnet is switched on, it rotates a gear 24 through an adjustable angle of rotation. The gear 24 is pressed by a spring 25 against a second gear 26 and rotates this by the same angle of rotation together with the screw 14a, which engages in an internal thread of the piston 13a and thereby moves the piston 13a in the cylinder 12a by a certain amount. As in the embodiment according to FIG. 1, the cylinder 12 a corresponding to the cylinder 12 is connected to one end of the hose 16, the other end of which is inserted into the tube 17 arranged on the handle 18.
The rotary magnet VM can be adjusted to deliver a drop or a specific number of drops of the vaccine through the hose 16.
