Appareil pour la suture de tissus, principalement de tissus vivants. La présente invention a pour objet un ap pareil pour la suture de tissus en général et en particulier de tissus vivants, il est princi palement destiné aux opérations chirurgicales, telles que la. résection du duodénum, cet ap pareil permettant la plus grande rapidité et précision du fait qu'il réunit en lui-même les organes nécessaires pour l'exécution d'une su ture, c'est-à-dire les organes de prise, de ten sion et de serrage du tissu et l'organe de su ture, ces organes pouvant être mis en action successivement en l'espace de quelques se condes.
Deux formes d'exécution de l'appareil suivant l'invention sont représentées, à titre d'exemple, au dessin annexé, -dans lequel: Les fig. 1 et 2 sont deux vues de l'avant et de l'arrière de la première forme d'exécu tion, dans laquelle le dispositif de suture est combiné avec un dispositif de résection;
La fig. 3 montre une vue d'avant du même appareil, en particulier le dispositif de prise, de tension et de serrage (pince) du tissu, le -dispositif de résection étant repré- senté en position fermée (traits pleins) et en position ouverte (pointillé).

Les fig. 10 et 11 sont respectivement des vues en -perspective et en plan de la pièce munie de dents dans lesquelles s'engagent les crochets -des leviers; La fig. 12 est une vue de l'aiguille pour la suture du tissu; Les fi-. 13 et 14 sont des vues -de l'avant et de l'arrière respectivement de la deuxième forme d'exécution de l'appareil, qui ne com porte pas de dispositif @de résection.
L'appareil suivant les fig. 1 à 12 comporte une pince composée de deux mâchoires, su- périeure 1 et inférieure 2, munies respective ment de dents 3 sur lesquelles sont pratiquées des encoches transversales 3' -qui -se trouvent en alignement rectiligne les unes avec les au tres et avec une rainure 3" -du flasque de la mâchoire 1 lorsque la pince est fermée.
La mâchoire -supérieure 1 est fixe, tandis que la mâchoire inférieure 2 est mobile dans le sens vertical 1-e long de deux saillies a et b (fig. 3) et d'une fente médiane c (fig. 2), servant de guide, ménagées sur le flasque 1 -de la mâ choire supérieure, ladite mâchoire inférieure 2 coulisse le long du flasque 1 fait d'une seule pièce avec la mâchoire 1.
La mâchoire inférieure 2 est munie d'un appendice 4 pour pouvoir être manoeuvrée par deux biellettes 5 -et 6 articulées à l'extrémité inférieure de cet -appendice .et à ,deux leviers 7 et 8 servant de poignées et articulés sur le flasque 1 de la pince. Le ressort. 9 appuie par ses- branches 9' et 9" contre les leviers 7 et 8, afin de ra mener la pince à sa position ouverte, indiquée en pointillé aux fig. 1 et 3, dès que Q'on cesse de presser avec la main sur lesdits leviers 7 et 8.
Le dispositif de résection, fixé par des vis à la mâchoire inférieure 2, comporte la pièce 9a. D'ans cette pièce est ménagée une rainuae 10 (fi-. 4) dans laquelle .coulisse la partie saillante 12 de la tige 11. A l'une -des extré mités de cette tige est fixé de bistouri 13, tan dis que l'autre extrémité est munie d'une ma nette 14 qui sert à la manaeuvre (introduction et extraction) de la tige 11.
Le flasque 1 pré sente une saillie 21 en forme -de dent qui sert à empêcher l'extraction complète,du bistouri dans la position fermée de la pince. A chaque extrémité de la pièce 9a sont respectivement ménagées des fentes 15 et 15' dans lesquelles sont logés et articulés deux leviers 16 et 16' (fig. 8 et 9) munis à leurs extrémités supé rieures de crochets 17. et 17' ces leviers ayant leur partie médiane amincie pour pouvoir être introduite dans les fentes 15 et 1,5'.
Des ressorts 18, 18' sont montés dans les fentes 15 et 15' et servent à ramener à leur posi tion de -départ les leviers 16 et 1,6' lorsqu'on cesse d'exercer sur eux une pression au moyen des doigts. Pour rendre le dispositif de résection et, par suite, la mâchoire infé rieure 2. solidaires de la mâchoire supérieure 1, on a prévu la pièce 19 elle-même fixée à ladite mâchoire supérieure 1: cette pièce 19 est munie à chacune de ses extrémités élar gies, de dents d'arrêt \?0, 20' avec lesquelles entrent en prise les crochets 17 et 17'.
Le fonctionnement de l'appareil décrit est le :suivant: Pour serrer et tendre un tissu à opérer, on serre la pince en pressant avec la main sur les leviers. 7 et 8. Grâce à. l'action clé cette pression, la mâchoire inférieure 2 et le -dispositif de résection qui en est solidaire, se soulèvent jusqu'à ce que les. crochets 17, 17' viennent s'engager avec les dents 20 (fig. 1). De cette fac,Ion, la pince se trouve ,dans la po sition fermée et le tissu à opérer se trouve saisi par les dents des mâchoires supérieure 1 et inférieure 2., de manière à. être coincé entre elles.
On enfile l'aiguille dans la rainure for mée par les encoches 3' alignées par suite de la fermeture de la pince, on la munit de catgut et on la retire ensuite. On effectue ainsi la suture du tissu. Après. quoi il ne reste plus qu'à manoeuvrer le bistouri 11 en le sai sissant par la poignée 14, pour effectuer la résection de la partie du tissu à éliminer. On ouvre ensuite la. pince en pressant avec les :doigts sur les leviers 16 et 16', l'opération -de suture et de résection s'effectue ainsi en quelques secondes. Dans la. position ouverte de la pince, le bistouri 11, n'étant plus retenu par l'ergot 21, peut être retiré complètement.
La forme d'exécution représentée aux fig. 13 et 14 diffère -de la forme décrite ci dessus en ce qu'elle ne comporte pas de dispo- sitif de résection en vue du fait qu'il est pré férable quelquefois que la résection ait lieu séparément et indépendamment -de la suture.
Cette deuxième forme d'exécution comporte comme organe d'arrêt et de fermeture de la pince un ressort en forme de tige 4a, qui est logé dans une rainure ménagée dans l'ap pendice 4 et est fixé à la partie inférieure dudit appendice, tandis qu'à.
l'autre extré mité se termine par une tête 4a ayant la forme -d'un crochet, qui s'engage dans la fente e de l'appendice t- lorsque ce dernier est soulevé par l'effet de la pression sur les leviers î, 8 et précisément lorsque le pivot 4' dudit appendice 4 est parvenu à la fin de sa course, c'est-à-dire à l'extrémité supérieure de la fente c, de façon à, fixer ou arrêter la pince dans sa position de fermeture.
Pour libérer la tête 4a' du ressort 4a, c'est-à-dire pour ouvrir la. pince, on n'a qu'à presser avec un doigt la tête dudit ressort, de façon à la faire sortir de la. fente c.
L'appareil décrit ci-dessus peut servir non seulement à des buts chirurgicaux, mais encore pour la suture et la résection d'un tissu quelconque, par exemple: étoffe, cuir, ete., et peut avoir les dimensions propres à ces buts industriels.

The apparatus according to fig. 1 to 12 comprises a clamp composed of two jaws, upper 1 and lower 2, respectively provided with teeth 3 on which are made transverse notches 3 '- which are in rectilinear alignment with each other and with each other. a groove 3 "of the flange of the jaw 1 when the clamp is closed.
