본 발명은 마이크로 카테터 제거를 위한 풍선 카테터에 관한 것으로서, 루멘에 삽입되는 유도관과, 상기 유도관에 삽입되어 병변 타겟을 향하는 가이드 와이어를 이동가능하게 지지하는 마이크로 카테터와의 사이에 형성된 통로를 거쳐 상기 유도관의 내측벽과 상기 가이드 와이어 사이에 배치되어, 팽창 또는 수축하는 풍선; 상기 풍선을 팽창시키기 위한 유체가 배출되는 유체 배출공과, 상기 유체 배출공으로 유체의 유동을 안내하는 유체 유동로를 가지고, 상기 풍선의 일측을 지지하며 상기 풍선을 상기 통로를 거쳐 상기 유도관의 내측에 인출입시키는 풍선 카테터 본체; 및 상기 유체 배출공으로부터 돌출된 돌출부를 가지며 상기 풍선 카테터 본체의 유동로에 삽입되어, 상기 풍선의 타측을 상기 돌출부에 지지하는 풍선 지지 로드를 포함하는 것을 특징으로 한다.

Description

마이크로 카테터 제거를 위한 풍선 카테터
본 발명은 마이크로 카테터 제거를 위한 풍선 카테터에 관한 것이다.
CTO 병변(Chronic Total Occlusion Lesion, 만성폐쇄성 병변)은 오랜 시간에 걸쳐 단단하게 막힌 혈관을 말한다.
특히, 심장혈관에 국한해서는 인체 해부학적으로 중요한 장기이기 때문에 위험도와 시술 난이도가 높은 특징이고, 성공률은 다른 시술에 비해 낮은 편이다.
중재 시술할 때 시술시간이 오래 걸리기 때문에 다양한 시술 도구 및 시술자의 숙련도나 기술의 의존도가 높은 시술이다.
오래된 병변이면서 단단하게 진행된 CTO 병변 시술에서는, 일 예로서 0.014인치 직경의 가이드 와이어(Guide wire)가 병변(Lesion)으로 쉽게 진입하지 못하는 문제점이 있다.
따라서, CTO 병변을 성공적으로 시술하기 하기 위해서는 다양한 기구가 필요하다.
그 중, 마이크로 카테터(Micro-catheter)를 추가로 사용하여, 시술 시 지지력(backup support)을 향상시켜, 가이드 와이어가 단단한 CTO 병변의 진입을 할 수 있게 도움을 주며 시술의 성공률을 높인다.
그리고, 가이드 와이어가 CTO 병변을 통과하면, 가이드 와이어를 남겨둔 채, 후속 시술을 위해 마이크로 카테터를 유도관으로부터 안전하게 제거해야 한다.
그러나, CTO 병변을 통과한 가이드 와이어를 남겨둔 채, 마이크로 카테터만 가이드 와이어 및 유도관으로부터 안전하게 제거를 할 때, 마이크로 카테터를 최대한 유도관(guiding catheter)의 내측으로부터 인출하지만 마이크로 카테터가 유도관과의 길이의 차이로 인해 안전하게 제거할 수 없고, 시술 시간도 소요되는 문제점이 있다.
예컨대, 만성폐쇄성 병변을 시술하기 위해서 지지력(back up support)을 향상시키려고, 마이크로 카테터를 가이드 와이어와 같이 사용하는 경우가 대부분이며, 병변을 통과하게 되면 관상 동맥 원위부(distal part)에 도달한 가이드 와이어를 유지한 채, 마이크로 카테터를 체외로 안전하게 제거를 해야 한다.
하지만, 130~150cm 길이의 마이크로 카테터와 180~190cm 길이의 가이드 와이어와의 길이 차이가 최소 유도관 길이(100cm)만큼 차이가 나면 체외로 안전하게 제거할 수 있으나, 임상에서 사용하는 기구의 길이 차이는 50~60cm 밖에 나지 않기 때문에 안전하게 제거를 할 수 없다.
이 상황에서 계속 마이크로 카테터를 체외로 제거하려고 하면, 관상 동맥 원위부에 가이드 와이어가 안전하게 유지되지 않은 채, 마이크로 카테터와 같이 빠지게 되는 경우가 생긴다. 이 때, 다시 병변을 재진입 하려고 하면, 한번 가이드 와이어가 들어가면서 약간의 상처가 생겼던 관상동맥 내막에 큰 상처를 발생시켜 심각한 합병증을 유발할 수 있다.
본 발명은 상기한 문제점을 해결하기 위한 것으로서, 마이크로 카테터를 사용하는 시술에 있어서, 마이크로 카테터를 가이드 와이어와 유도관을 남긴 채 유도관으로부터 쉽게 안전하게 제거할 수 있고, 시술 시간도 단축할 수 있는 마이크로 카테터 제거를 위한 풍선 카테터를 제공하는 것을 발명의 목적으로 한다.
본 발명이 해결하고자 하는 과제들은 이상에서 언급된 과제로 제한되지 않으며, 언급되지 않은 또 다른 과제들은 아래의 기재로부터 통상의 기술자에게 명확하게 이해될 수 있을 것이다.
본 발명의 목적은, 루멘에 삽입되는 유도관과, 상기 유도관에 삽입되어 병변 타겟을 향하는 가이드 와이어를 이동가능하게 지지하는 마이크로 카테터와의 사이에 형성된 통로를 거쳐 상기 유도관의 내측벽과 상기 가이드 와이어 사이에 배치되어, 팽창 또는 수축하는 풍선; 상기 풍선을 팽창시키기 위한 유체가 배출되는 유체 배출공과, 상기 유체 배출공으로 유체의 유동을 안내하는 유체 유동로를 가지고, 상기 풍선의 일측을 지지하며 상기 풍선을 상기 통로를 거쳐 상기 유도관의 내측에 인출입시키는 풍선 카테터 본체; 및 상기 유체 배출공으로부터 돌출된 돌출부를 가지며 상기 풍선 카테터 본체의 유동로에 삽입되어, 상기 풍선의 타측을 상기 돌출부의 일측에 지지하는 풍선 지지 로드를 포함하는 마이크로 카테터 제거를 위한 풍선 카테터에 의해 달성될 수 있다.
상기 풍선이 상기 유도관의 내측벽과 상기 가이드 와이어 사이에 배치되어 팽창할 때, 상기 풍선은 상기 가이드 와이어를 상기 유도관의 내측벽에 밀착 고정시킬 수 있다.
수축된 상기 풍선의 최대 외경은 상기 통로의 간격보다 작은 크기를 가질 수 있다.
일 예로서, 상기 풍선에 수용된 돌출부에 간격을 두고 마련되어, 상기 풍선의 위치를 표시하는 한 쌍의 마커를 더 포함할 수 있다.
다른 예로서, 상기 풍선의 길이에 대응하며 상기 풍선에 수용된 돌출부에 마련되어, 상기 풍선 위치를 표시하는 마커를 더 포함할 수 있다.
상기 풍선은 팽창 및 수축가능한 고무 또는 실리콘 재질을 포함할 수 있다.
본 발명에 따르면 시술자(operator) 및 시술 지원팀(방사선사, 간호사, 임상병리사) 등 기구 조작이 간단하고 쉬워서 기구 선택 시 고민을 줄여주고, 시술 시간을 단축할 수 있으며, 시술 단축 시간이 곧 피로도 감소로 이어지고 만족할 시술 환경을 제공하게 된다.
그리고, 시술에 따른 합병증도 최소화하며, 시술 비용도 줄일 수 있다.
본 발명의 효과들은 이상에서 언급된 효과로 제한되지 않으며, 언급되지 않은 또 다른 효과들은 아래의 기재로부터 통상의 기술자에게 명확하게 이해될 수 있을 것이다.

본 발명의 이점 및 특징, 그리고 그것들을 달성하는 방법은 첨부되는 도면과 함께 상세하게 후술되어 있는 실시예들을 참조하면 명확해질 것이다. 그러나, 본 발명은 이하에서 개시되는 실시예들에 제한되는 것이 아니라 서로 다른 다양한 형태로 구현될 수 있으며, 단지 본 실시예들은 본 발명의 개시가 완전하도록 하고, 본 발명이 속하는 기술 분야의 통상의 기술자에게 본 발명의 범주를 완전하게 알려주기 위해 제공되는 것이다.

이하, 첨부 도면들을 참조하며, 본 발명에 대해 상세히 설명한다.
설명에 앞서, 여러 실시예에 있어서, 동일한 구성을 가지는 구성요소에 대해서는 동일 부호를 사용하여 대표적으로 제1실시예에서 설명하고, 그 외의 실시예에서는 제1실시예와 다른 구성에 대해 설명하기로 한다.
또한, 이하에서 풍선을 팽창시키는 유체는 X-선이 투과되지 않는 조영제 또는, 생리 식염수, 에어 등과 같은 인체에 무해한 유체 등을 포함할 수 있음을 미리 밝혀둔다.
도 1 및 도 2에는 본 발명의 제1실시예에 따른 마이크로 카테터 제거를 위한 풍선 카테터가 도시되어 있다.
도 1은 본 발명의 제1실시예에 따른 마이크로 카테터 제거를 위한 풍선 카테터의 풍선이 수축된 상태의 구성을 도시한 도면이고, 도 2는 도 1의 풍선이 팽창된 상태의 구성을 도시한 도면이다.
이들 도면에 도시된 바와 같이, 본 발명의 제1실시예에 따른 마이크로 카테터 제거를 위한 풍선 카테터(1a)는 풍선(10a)과, 풍선 카테터 본체(20)와, 풍선 지지 로드(40)를 포함한다.
풍선(10a)은 루멘(200, 도 5 참조)에 삽입되는 유도관(100, 도 5 참조)과, 병변 타겟(210, 도 5 참조)을 향하는 가이드 와이어(110, 도 5 참조)를 이동가능하게 지지하는 마이크로 카테터(120, 도 5 참조)와의 사이에 형성된 통로(130, 도 5 참조)를 거쳐, 유도관(100)의 내측벽과 가이드 와이어(110) 사이에 배치되어, 팽창 또는 수축한다.
마이크로 카테터(120)는 가이드 와이어(110)에 이동가능하게 장착되고, 시술시 가이드 와이어(110)의 지지력을 향상시켜 가이드 와이어(110)가 병변 타겟(210)에 쉽게 진입하도록 도와준다.
한편, 수축된 풍선(10a)의 최대 외경은 통로(130)의 간격보다 작은 크기를 가진다. 이로써, 풍선(10a)을 통로(130)를 따라 원활하게 삽입 이동시킬 수 있게 되어, 풍선(10a)을 마이크로 카테터(120)의 전방에 위치하는 유도관(100)의 내측벽과 가이드 와이어(110) 사이에 위치시킬 수 있게 된다.
여기서, 풍선(10a)은 팽창 및 수축가능한 고무 또는 실리콘 재질을 포함할 수 있다.
풍선 카테터 본체(20)는 일정 길이의 봉 형상을 가지며, 풍선(10a)을 유도관(100)에 인출입시킨다.
풍선 카테터 본체(20)는 유체 배출공(25)과, 유체 유동로(31)를 포함한다.
풍선 카테터 본체(20)의 일측에는 풍선(10a)의 일측이 지지된다.
유체 배출공(25)은 풍선 카테터 본체(20)의 단부에 마련되어, 풍선(10a)을 팽창시키기 위한 유체를 배출한다.
유체 유동로(31)는 풍선 카테터 본체(20)의 내부에 일정 직경을 가지며 풍선 카테터 본체(20)의 길이방향을 따라 형성되어 있다.
유체 유동로(31)는 유체 배출공(25)과 연통하며, 풍선(10a)으로 유체를 주입하기 위한 유체의 유동을 안내한다.
풍선 카테터 본체(20)의 타측에는 풍선 카테터 본체(20)를 파지하며 조작하기 위한 조작부(35)가 마련되어 있다.
조작부(35)는 외부로부터 제공되는 유체를 유체 유동로(31)를 거쳐 풍선(10a)에 공급하도록 조작하거나, 풍선(10a)에 공급된 유체를 유체 유동로(31)를 거쳐 외부로 배출하도록 조작할 수 있다.
또한, 조작부(35)는 카테터 본체(20)를 파지하여, 풍선(10a)이 통로(130)를 거쳐 마이크로 카테터(120)의 전방에 위치하는 유도관(100)의 내측벽과 가이드 와이어(110) 사이에 위치하도록, 풍선(10a)의 위치를 조정할 수도 있다.
한편, 풍선 지지 로드(40)는 일정 길이를 가지며, 풍선 카테터 본체(20)의 유체 배출공(25)으로부터 돌출하며 유동로(31)에 삽입된다.
풍선 지지 로드(40)의 돌출부(42)의 일측에는 유체 배출공(25)을 수용하도록 풍선(10a)의 타측이 지지된다
또한, 본 발명의 제1실시예에 따른 마이크로 카테터 제거를 위한 풍선 카테터(1a)는 풍선(10a)의 위치를 표시하는 한 쌍의 마커(50a)를 더 포함한다.
한 쌍의 마커(50a)는 풍선(10a)에 수용된 돌출부(45)에 간격을 두고 마련된다.
또한, 마커(50a)는 X-선 등이 투과되지 않는 재질로 이루어질 수 있다.
이에 의해, X-선을 풍선(10a)을 향해 조사시, 시술자는 마커(50a)의 위치를 쉽게 인식할 수 있게 되어, 풍선(10a)의 정확한 위치를 확인할 수 있게 된다.
여기서, 도시되어 있지 않지만, 풍선의 표면에는 다수의 돌기를 돌츨 형성하여, 풍선이 팽창하여 다수의 돌기가 유도관의 내측벽과 가이드 와이어에 밀착되어, 풍선이 유도관의 내측벽과 가이드 와이어로부터 미끄러지는 것을 방지하여, 좀 더 원활하게 마이크로 카테터를 유도관으로부터 인출할 수 있다.
도 3 및 도 4에는 본 발명의 제2실시예에 따른 마이크로 카테터 제거를 위한 풍선 카테터가 도시되어 있다.
도 3은 본 발명의 제2실시예에 따른 마이크로 카테터 제거를 위한 풍선 카테터의 풍선이 수축된 상태의 구성을 도시한 도면이고, 도 4는 도 3의 풍선이 팽창된 상태의 구성을 도시한 도면이다.
이들 도면에 도시된 바와 같이, 본 발명의 제2실시예에 따른 마이크로 카테터 제거를 위한 풍선 카테터(1b)는 전술한 제1실시예와 달리, 하나의 마커(50b)가 풍선(10b)의 길이에 대응하는 길이를 가지며, 풍선(10b)에 수용된 돌출부(45)에 마련되어 있다.
이에, 풍선(10b)으로 공급되는 유체는 유체 유동로(31)와 유체 배출공(25)을 거쳐 풍선(10b)의 내부로 공급되어, 풍선(10b)을 팽창시킬 수 있게 된다.
이러한 구성에 의하여, 본 발명의 제1실시예에 따른 마이크로 카테터 제거를 위한 풍선 카테터(1a)를 이용하여, 마이크로 카테터(120)를 적용하여 시술하는 CTO 병변(Chronic Total Occlusion Lesion, 만성폐쇄성 병변) 시술에 있어서, 마이크로 카테터(120)를 제거하는 과정에 대해 설명하면 다음과 같다.
먼저, 루멘(200)에 삽입되는 유도관(100)을 관통하며 마이크로 카테터(120)에 의해 지지되는 가이드 와이어(110)가 병변 타겟(210)을 통과하면, 가이드 와이어(110)를 원위부(distal part)에 유지한 채, 마이크로 카테터(120)를 최대한 유도관(100)의 내측으로 삽입시킨다.
이 때, 마이크로 카테터(120)는 가이드 와이어(110)와 마이크로 카테터(120)의 길이의 차이를 고려하여, 도 5에 도시된 바와 같이 마이크로 카테터(120)의 선단이 유도관(100)의 내측에 위치하도록 일 예로, 마이크로 카테터(120)의 선단이 유도관(100)의 선단으로부터 5cm 길이의 내측에 위치하도록 마이크로 카테터(120)를 위치시킨다.
이어서, 도 5에 도시된 바와 같이, 본 발명에 따른 마이크로 카테터 제거를 위한 풍선 카테터(1a)의 수축된 풍선(10a)을 유도관(100)과 마이크로 카테터(120) 사이의 통로(130)를 거쳐 유도관(100)에 삽입된 마이크로 카테터(120)의 전방에 위치시킨다.
이 때, 풍선(10a)의 이동 경로에 X-선을 조사하여 풍선(10a)에 수용된 마커(50a)를 통해 풍선(10a)의 위치를 확인하면서, 풍선(10a)을 마이크로 카테터(120)의 전방에 위치하도록 배치, 좀 더 구체적으로 설명하면 풍선(10a)을 유도관(100)의 내측벽과 가이드 와이어(110) 사이에 배치한다.
다음, 도 6에 도시된 바와 같이 풍선(10a)이 유도관(100)의 내측에 원하는 위치에 위치한 상태에서, 풍선을 팽창시키기 위한 유체를 풍선 카테터 본체(20)의 유체 유동로(31)를 거쳐 유체 배출공(25)을 통해 풍선(10a)의 내부로 공급하여, 풍선(10a)을 팽창시킨다.
이 때, 가이드 와이어(110)가 유도관(100)의 내측벽에 밀착하도록 풍선(10a)을 팽창시킨다.
그리고, 팽창된 풍선(10a)에 의해 가이드 와이어(110)가 유도관(100)의 내측벽에 밀착된 후, 가이드 와이어(110)와 유도관(100)이 움직이지 않도록 풍선 카테터(1a)를 잡은 상태에서, 시술자가 마이크로 카테터(120)를 병변 타겟(210)의 반대방향으로 잡아당기면, 도 7에 도시된 바와 같이 유도관(100)과 가이드 와이어(110)는 풍선(10a)에 의해 밀착 고정되어, 마이크로 카테터(120)만 유도관(100)의 내측벽과 가이드 와이어(110)를 따라 병변 타겟(210)으로부터 멀어지는 방향으로 이동하여, 유도관(100)으로부터 인출된다.
한편, 본 발명에 따른 마이크로 카테터 제거를 위한 풍선 카테터(1a)는, 풍선(10a)이 마이크로 카테터(120)의 전방에 위치한 유도관(100)의 내측벽과 가이드 와이어(110) 사이에 배치되어 유도관(100)과 가이드 와이어(110)를 일체로 고정하므로, 마이크로 카테터(120)를 유도관(100)으로부터 인출하는 과정에서 마이크로 카테터(120)만 유도관(100)의 다양한 굴곡 형태에 대응하며 CTO 병변을 통과한 가이드 와이어(110)와 유도관(100)을 남겨둔 채, 마이크로 카테터(120)를 유도관(100)으로부터 안전하게 제거할 수 있고, 시술 시간도 단축할 수 있게 된다.
또한, 시술을 위해 사용한 풍선(10a)을 원래의 상태로 수축 복원시켜 재활용이 가능하므로, 시술 비용도 절감할 수 있게 된다.
이상, 첨부된 도면을 참조로 하여 본 발명의 실시예를 설명하였지만, 본 발명이 속하는 기술 분야의 통상의 기술자는 본 발명이 그 기술적 사상이나 필수적인 특징을 변경하지 않고서 다른 구체적인 형태로 실시될 수 있다는 것을 이해할 수 있을 것이다. 그러므로, 이상에서 기술한 실시예들은 모든 면에서 예시적인 것이며, 제한적이 아닌 것으로 이해해야만 한다.

Claims (6)

  1. 루멘에 삽입되는 유도관과, 상기 유도관에 삽입되어 병변 타겟을 향하는 가이드 와이어를 이동가능하게 지지하는 마이크로 카테터와의 사이에 형성된 통로를 거쳐 상기 유도관의 내측벽과 상기 가이드 와이어 사이에 배치되어, 팽창 또는 수축하는 풍선;
    상기 풍선을 팽창시키기 위한 유체가 배출되는 유체 배출공과, 상기 유체 배출공으로 유체의 유동을 안내하는 유체 유동로를 가지고, 상기 풍선의 일측을 지지하며 상기 풍선을 상기 통로를 거쳐 상기 유도관의 내측에 인출입시키는 풍선 카테터 본체; 및
    상기 유체 배출공으로부터 돌출된 돌출부를 가지며 상기 풍선 카테터 본체의 유동로에 삽입되어, 상기 풍선의 타측을 상기 돌출부의 일측에 지지하는 풍선 지지 로드를 포함하는 마이크로 카테터 제거를 위한 풍선 카테터.
  2. 제1항에 있어서,
    상기 풍선이 상기 유도관의 내측벽과 상기 가이드 와이어 사이에 배치되어 팽창할 때, 상기 풍선은 상기 가이드 와이어를 상기 유도관의 내측벽에 밀착 고정시키는, 마이크로 카테터 제거를 위한 풍선 카테터.
  3. 제1항에 있어서,
    수축된 상기 풍선의 최대 외경은 상기 통로의 간격보다 작은 크기를 갖는. 마이크로 카테터 제거를 위한 풍선 카테터.
  4. 제1항에 있어서,
    상기 풍선에 수용된 돌출부에 간격을 두고 마련되어, 상기 풍선의 위치를 표시하는 한 쌍의 마커를 더 포함하는, 마이크로 카테터 제거를 위한 풍선 카테터.
  5. 제1항에 있어서,
    상기 풍선의 길이에 대응하며 상기 풍선에 수용된 돌출부에 마련되어, 상기 풍선 위치를 표시하는 마커를 더 포함하는, 마이크로 카테터 제거를 위한 풍선 카테터.
  6. 제1항에 있어서,
    상기 풍선은 팽창 및 수축가능한 고무 또는 실리콘 재질을 포함하는, 마이크로 카테터 제거를 위한 풍선 카테터.
