Abstract
体内の湾曲部に挿入し易い延長カテーテルを提供する。カテーテル内に挿入され前記カテーテルの遠位側の開口から突出可能な延長カテーテルであって、筒状部材と、遠位端部が前記筒状部材に固定されている線状部材とを有しており、前記線状部材は、側面に前記筒状部材の径方向における外側を向いている第1切削面を有している延長カテーテル。
Description
本発明は延長カテーテルに関する。
これまでに、狭心症や心筋梗塞等の虚血性心疾患に対して、ステントやバルーン等の血管内治療用器具を用いて心臓の冠動脈の狭窄部を拡張し、血流を増加させる経皮的冠動脈形成術(PCI)が行われている。この際、ガイディングカテーテルの先端を冠動脈の入口に挿入して留置した後、ガイディングカテーテルの近位側の開口から延長カテーテルを挿入して遠位側の開口から延長カテーテルの一部を突出させて冠動脈に挿入する場合がある。このような延長カテーテルを用いれば血管内治療用器具を冠動脈内の患部にまで送達し易くなる。このような延長カテーテルは種々知られており、例えば特許文献1には、筒状部と、筒状部よりも近位側に位置する第1テーパ部と、第1テーパ部よりも近位側に位置する第2テーパ部を備え、第1テーパ部は外側面と第1テーパ面とを有し、第2テーパ部は外側面と第2テーパ面とを有しており、第1テーパ面と筒状部の軸方向とのなす角度θ1が90~145°、第2テーパ面と軸方向とのなす角度θ2が120~175°である延長カテーテルが開示されている。
特許文献1のように従来の延長カテーテルでは、線状部材の遠位端部に固定された筒状部材を体内の動脈等の湾曲部に挿入するときに挿入し難い場合があった。本発明は上記の様な事情に着目してなされたものであって、その目的は、体内の湾曲部に挿入し易い延長カテーテルを提供することにある。
上記課題を解決することのできた実施の形態に係る延長カテーテルは、以下の通りである。
[1]カテーテル内に挿入され前記カテーテルの遠位側の開口から突出可能な延長カテーテルであって、
筒状部材と、
遠位端部が前記筒状部材に固定されている線状部材とを有しており、
前記線状部材は、側面に前記筒状部材の径方向における外側を向いている第1切削面を有している延長カテーテル。
[1]カテーテル内に挿入され前記カテーテルの遠位側の開口から突出可能な延長カテーテルであって、
筒状部材と、
遠位端部が前記筒状部材に固定されている線状部材とを有しており、
前記線状部材は、側面に前記筒状部材の径方向における外側を向いている第1切削面を有している延長カテーテル。
本発明者の検討により、延長カテーテルを製造するに当たって、線状部材の遠位端部を切削して外径を小さくした上で切削面を筒状部材に向けて固定すると、切削の残留応力により線状部材の切削面とその近傍が筒状部材の中心軸から離れる方向に曲がり易くなることが分かった。一方、上記[1]のように、線状部材の切削面が筒状部材とは反対側、即ち筒状部材の径方向における外側に向いている構成では、切削の残留応力により切削面とその近傍が筒状部材の中心軸に向かう方向に曲がり易くなることが分かった。このような構成によれば、体内の湾曲部に線状部材を介して筒状部材を押し込んだときに線状部材と筒状部材は体内の湾曲部の形状に沿って湾曲し易くなるため、延長カテーテルを体内の湾曲部に挿入し易くなる。実施の形態に係る延長カテーテルは、以下の[2]~[18]のいずれか一項であることが好ましい。
[2]前記第1切削面の少なくとも一部は、前記遠位端部に位置する[1]に記載の延長カテーテル。
[3]前記線状部材は、前記側面に複数の切削面を有しており、

本発明によれば、体内の湾曲部に挿入し易い延長カテーテルを提供することができる。
以下では、下記実施の形態に基づき本発明をより具体的に説明するが、本発明はもとより下記実施の形態によって制限を受けるものではなく、前・後記の趣旨に適合し得る範囲で適当に変更を加えて実施することも勿論可能であり、それらはいずれも本発明の技術的範囲に包含される。なお、各図面において、便宜上、部材符号等を省略する場合もあるが、かかる場合、明細書や他の図面を参照するものとする。また、図面における種々部材の寸法は、本発明の特徴の理解に資することを優先しているため、実際の寸法とは異なる場合がある。
実施の形態に係る延長カテーテルは、カテーテル内に挿入されカテーテルの遠位側の開口から突出可能な延長カテーテルであって、筒状部材と、遠位端部が筒状部材に固定されている線状部材とを有しており、線状部材は、側面に筒状部材の径方向における外側を向いている第1切削面を有している。本発明者の検討により、延長カテーテルを製造するに当たって、線状部材の遠位端部を切削して外径を小さくした上で切削面を筒状部材に向けて固定すると、切削の残留応力により線状部材の切削面とその近傍が筒状部材の中心軸から離れる方向に曲がり易くなることが分かった。一方、上記のように、線状部材の切削面が筒状部材とは反対側、即ち筒状部材の径方向における外側に向いている構成では、切削の残留応力により切削面とその近傍が筒状部材の中心軸に向かう方向に曲がり易くなることが分かった。このような構成によれば、体内の湾曲部に線状部材を介して筒状部材を押し込んだときに線状部材と筒状部材は体内の湾曲部の形状に沿って湾曲し易くなるため、延長カテーテルを体内の湾曲部に挿入し易くなる。
以下では、図1~図11を参照しながら、実施の形態に係る延長カテーテルについて説明する。図1は、実施の形態に係る延長カテーテルの側面図である。図2は、図1の延長カテーテルをガイディングカテーテル内に挿入して、遠位側の開口から一部を突出させたときの側面図である。図3は、図1の延長カテーテルのテーパ部とその近傍における軸方向の断面図である。図4、図5、図9は、順に図1の延長カテーテルのIV-IV断面図、V-V断面図、IX-IX断面図である。図6は、湾曲状態のガイディングカテーテル内に挿入された図1の延長カテーテルの側面図である。図7、図8は、それぞれ図1の延長カテーテルの線状部材の変形例の側面図である。図10は、図1の延長カテーテルのテーパ部とその近傍の側面図である。図11は、X線不透過性リングを有する図1の延長カテーテルの側面図である。
図1に示す通り、実施の形態に係る延長カテーテル91は、筒状部材1と、遠位端部2Bが筒状部材1に固定されている線状部材2とを有している。これにより操作者は、線状部材2を介して筒状部材1を遠位側に押し込んだり近位側に引き戻したりすることができる。
図2に示す通り、延長カテーテル91は、カテーテル99内に挿入されカテーテル99の遠位側の開口99Pbから突出可能なものである。例えば、カテーテル99の遠位端部を冠動脈の入口に挿入して留置した後、カテーテル99の近位側の開口99Paから延長カテーテル91をカテーテル99内に挿入して、カテーテル99の遠位側の開口99Pbから延長カテーテル91の一部を突出させて冠動脈に挿入することにより、カテーテル99と延長カテーテル91を介して血管内治療用器具を冠動脈内の患部にまで送達することができる。血管内治療用器具として、バルーン、ステント等が挙げられる。カテーテル99は、いわゆるガイディングカテーテルであることが好ましい。ガイディングカテーテルは内腔にバルーンカテーテルやステントデリバリーカテーテル等の治療用カテーテルが挿入されるものである。治療用カテーテルは冠動脈に挿入されるものであることが好ましいが、脳動脈等の他の動脈、静脈、膵管、胆管、尿管、気管支等の体内の管に挿入されるものであってもよい。カテーテル99は、外側面と内側面を貫通する貫通孔、溝等を有していないことが好ましい。これにより、延長カテーテル91をカテーテル99の近位側の開口99Paから挿入して、遠位側の開口99Pbから一部を突出させ易くなる。
図3、図4、図5に示す通り、線状部材2は、側面に筒状部材1の径方向1Dにおける外側1D1を向いている第1切削面2S1を有している。線状部材2に第1切削面2S1を形成するときの残留応力により、第1切削面2S1とその近傍は筒状部材1の径方向1Dにおける内側1D2に向かって曲り易くなる。その結果、図6に示すように、体内の湾曲部に線状部材2を介して筒状部10を押し込んだときに筒状部10と線状部材2は湾曲部の形状に沿って湾曲し易くなるため、延長カテーテル91を体内の湾曲部に挿入し易くなる。第1切削面2S1が筒状部材1の径方向1Dにおける外側1D1を向いているとは、第1切削面2S1が筒状部材1の径方向1Dにおける筒状部材1の中心軸1Cに向かう方向とは反対の方向を向いていることを意味する。一方、第1切削面2S1が筒状部材1の径方向1Dにおける内側1D2を向いているとは、第1切削面2S1が筒状部材1の径方向1Dにおける筒状部材1の中心軸1Cに向かう方向を向いていることを意味する。以下では、筒状部材1の径方向1Dを単に径方向1Dと呼ぶ場合がある。
図3に示す通り、第1切削面2S1の少なくとも一部は、線状部材2の遠位端部2Bに位置することが好ましい。これにより筒状部材1のうち線状部材2の遠位端部2Bが固定されている部分が筒状部材1の径方向1Dにおける内側1D2に向かって曲り易くなる。
第1切削面2S1の遠位端2S1bは、線状部材2の遠位端2bに位置することが好ましい。これにより線状部材2を遠位端2bから湾曲させ易くすることができる。
第1切削面2S1の近位端2S1aは、筒状部材1の近位端1aよりも近位側に位置することが好ましい。これにより、筒状部材1のうち線状部材2の遠位端部2Bが固定されている部分が筒状部材1の径方向1Dにおける内側1D2に向かって一層、曲り易くなる。
第1切削面2S1は、筒状部材1の軸方向1Xに対して傾斜しており、径方向1Dにおいて第1切削面2S1の遠位端2S1bは近位端2S1aよりも内側に位置することが好ましい。このように第1切削面2S1が傾斜していることにより、線状部材2の第1切削面2S1とその近傍が筒状部材1の径方向1Dにおける内側1D2に向かって一層、曲り易くなる。
第1切削面2S1は、筒状部材1の軸方向1Xと平行である部分を有していないことが好ましい。これにより、線状部材2の第1切削面2S1とその近傍が筒状部材1の径方向1Dにおける内側1D2に向かって一層、曲り易くなる。
図7に示す通り、線状部材2は、側面に複数の切削面を有していてもよい。この場合、複数の切削面は、複数の切削面のうち最も遠位側に位置する第1切削面2S1と第1切削面2S1よりも近位側に位置する第2切削面2S2を有することが好ましい。第1切削面2S1と第2切削面2S2は、筒状部材1の径方向1Dにおける外側1D1を向いていることが好ましい。第1切削面2S1と第2切削面2S2は、それぞれ線状部材2の軸方向2Xとのなす角度が、0.1°以上、45°以下であることが好ましく、0.2°以上、30°以下であることがより好ましく、0.2°以上、5°以下であることがさらに好ましい。第1切削面2S1と線状部材2の軸方向2Xとのなす角度は、第2切削面2S2と線状部材2の軸方向2Xとのなす角度よりも小さいことが好ましい。また線状部材2の軸方向2Xにおいて、第1切削面2S1は第2切削面2S2よりも長いことが好ましい。これにより第1切削面2S1の面積を大きくして線状部材2を体内の湾曲部において曲り易くすることができる。一方、第1切削面2S1と線状部材2の軸方向2Xとのなす角度は、第2切削面2S2と線状部材2の軸方向2Xとのなす角度よりも大きくてもよい。また線状部材2の軸方向2Xにおいて、第1切削面2S1は第2切削面2S2よりも短くてもよい。これにより線状部材2の遠位端2bの近傍の強度を向上できる。
図7に示す通り、複数の切削面は、それぞれ筒状部材1の径方向1Dにおける外側1D1を向いており、筒状部材1の軸方向1Xに対して傾斜しており、且つ径方向1Dにおいて各切削面の遠位端は近位端よりも内側に位置することが好ましい。これにより、線状部材2の複数の切削面とその近傍が筒状部材1の径方向1Dにおける内側1D2に向かって一層、曲り易くなる。なお複数の切削面は、筒状部材1の軸方向1Xと平行である切削面を有していてもよい。
複数の切削面のうちの最も近位に位置する切削面の近位端は、筒状部材1の近位端1aよりも近位側に位置することが好ましい。例えば図7のように複数の切削面が、第1切削面2S1と第1切削面2S1よりも近位側に位置する第2切削面2S2とを有する場合、第2切削面2S2の近位端2S2aは、筒状部材1の近位端1aよりも近位側に位置することが好ましい。これにより、筒状部材1のうち線状部材2の遠位端部2Bが固定されている部分が筒状部材1の径方向1Dにおける内側1D2に向かって一層、曲り易くなる。
第1切削面2S1は、線状部材2の遠位端2bから近位側に20cm離れた位置までの間において最も面積が大きい切削面であることが好ましい。これにより線状部材2の遠位端部2Bが筒状部材1の径方向1Dにおける内側1D2に向かって曲り易くなる。
図3、図4、図5に示す通り、線状部材2は、少なくとも筒状部材1に固定されている部分において、径方向1Dにおける内側1D2を向いている切削面を有していないことが好ましい。これにより線状部材2の遠位端部2Bは、一層、筒状部材1の径方向1Dにおける内側1D2に向かって曲り易くなる。
一方、図8に示すように、線状部材2は、径方向1Dにおける内側1D2を向いている内向きの切削面2S3を有していてもよい。この場合、図8に示すように第1切削面2S1の径方向1Dにおける長さH1は、内向きの切削面2S3の径方向1Dにおける長さH3よりも長いことが好ましい。このような構成では、筒状部材1の径方向1Dにおける外側1D1に向かう方向の残留応力よりも内側1D2に向かう方向への残留応力が大きくなるため、第1切削面2S1とその近傍は筒状部材1の径方向1Dにおける内側1D2に向かって曲り易くなる。内向きの切削面2S3の少なくとも一部は、線状部材2の遠位端部2Bに位置することが好ましい。更に、内向きの切削面2S3の遠位端2S3bは、線状部材2の遠位端2bに位置することが好ましい。これにより線状部材2を遠位端2bとその近傍の外径を小さくすることができる。
図1、図3に示す通り、線状部材2は、第1切削面2S1を含む第1部分2P1と、第1部分2P1よりも近位側に位置し切削面を側面に有していない第2部分2P2とを有していることが好ましい。更に図4、図5、図9に示すように、筒状部材1の径方向1Dにおける線状部材2の断面において、第1部分2P1は第2部分2P2よりも短径に対する長径の比の値が大きいことが好ましい。長径/短径の値が大きくなると一方向に曲がり易くなり異方性が発揮され易くなるため、第1部分2P1の長径/短径の値が大きいことにより、線状部材2の遠位側から体内の湾曲部に挿入し易くすることができる。短径とは、各断面において、線状部材2の外縁上の2点を結ぶ線分であって、線状部材2の図心を通る線分のうち最も長さが短い線分の長さである。長径とは、各断面において、線状部材2の外縁上の2点を結ぶ線分であって、線状部材2の図心を通る線分のうち最も長さが長い線分の長さである。なお第1部分2P1は、複数の切削面を有していてもよい。
図1、図3に示す通り、第1部分2P1の近位端2P1aは、筒状部材1の近位端1aよりも近位側に位置することが好ましい。これにより、筒状部材1のうち線状部材2の遠位端部2Bが固定されている部分が筒状部材1の径方向1Dにおける内側1D2に向かって一層、曲り易くなる。
第1部分2P1の長さは、線状部材2の長さの10%以下の長さであることが好ましい。これにより、線状部材2の異方性のある部分の長さを低減することができるため、例えば血管の直線状部分に対して線状部材2を押し込み易くすることができる。第1部分2P1の長さは、線状部材2の長さの9%以下の長さであることがより好ましい。一方、第1部分2P1の長さは、線状部材2の長さの1%以上であることが好ましく、2%以上であることがより好ましい。これにより、線状部材2の遠位側を体内の湾曲部に挿入し易くすることができる。
第1部分2P1は、筒状部材1の遠位端1bから、筒状部材1の遠位端1bから20cm以上、40cm以下の距離離れた位置までの間に位置することが好ましい。この間の少なくとも一部に第1部分2P1が位置することにより、筒状部材1を体内の湾曲部に挿入し易くすることができる。
図4、図5、図9に示すように、線状部材2は、内腔を有していない中実の線状部材であることが好ましい。これにより線状部材2の厚みを低減することができる。線状部材2は、筒状部材1を遠位側に押し込むことができればよく、その素材は特に限定されないが、ステンレス鋼、チタン、ニッケルチタン合金、コバルトクロム合金等、タングステン合金、またはこれらの組み合わせを含むことが好ましく、ステンレス鋼を含むことがより好ましい。
線状部材2の側面の切削面は、例えば厚さ方向の断面形状が円形である線材の側面を、研磨機により研磨することにより形成することができる。線材の厚さ方向の断面形状は円形に限定されず、例えば多角形、楕円形等であってもよい。
線状部材2は、厚さ方向の断面形状が凸多角形、凹多角形、円形、D字形、または楕円形であることが好ましい。凸多角形は、正方形、長方形、台形、または六角形であってもよい。第1部分2P1における線状部材2は、厚さ方向の断面形状は、D字形、長方形、または楕円形であることが好ましい。これにより第1部分2P1は異方性を発揮し易くなる。一方、第2部分2P2における線状部材2は、円形または正多角形であることが好ましい。これにより第2部分2P2が異方性を発揮し難くなる。
図4と図9に示すように、第1部分2P1と第2部分2P2における線状部材2の厚さ方向の断面形状は異なっていることが好ましい。これにより第1部分2P1と第2部分2P2の各機能が発揮され易くなる。
図4と図9に示すように、第1部分2P1における線状部材2の厚さ方向の断面積は、第2部分2P2における線状部材2の厚さ方向の断面積よりも小さいことが好ましい。これにより第1部分2P1は湾曲し易くなる。
図1に示す通り、延長カテーテル91は、更に線状部材2の近位端部に固定されているハンドル部材3を有していることが好ましい。操作者は、ハンドル部材3を把持して線状部材2を遠位側または近位側に移動させることができる。ハンドル部材3は、樹脂を含むことが好ましい。樹脂はポリオレフィン樹脂であることが好ましい。ポリオレフィン樹脂は、ポリエチレン、ポリプロピレン、またはこれらの組みあわせを含むことが好ましい。
図1、図10に示す通り、筒状部材1は、筒状部10と、筒状部10よりも近位側に位置するテーパ部11を有していることが好ましい。テーパ部11は、筒状部材1の径方向1Dにおける長さが筒状部10よりも短くなっており、且つ筒状部材1の内腔と連通し径方向1Dを向いている開口11Pを有している。テーパ部11は、筒状部10の軸方向10Xに対して傾斜している少なくとも1つのテーパ面を有しているため、例えば図6のように体内の湾曲部に挿入されて湾曲している筒状部材1のテーパ部11の開口11Pから筒状部材1の内腔へ血管内治療用器具を挿入し易くなる。開口11Pへ血管内治療用器具を挿入する際には、例えばバルーンカテーテルやステントデリバリーカテーテル等の治療用カテーテルを用いればよい。
図10に示す通り、筒状部材1のテーパ部11は複数のテーパ面を有していることが好ましい。複数のテーパ面は、複数のテーパ面のうち最も遠位側に位置する第1テーパ面S1を含むことが好ましい。第1テーパ面S1と筒状部10の軸方向10Xとのなす角度θ1は、20°以上であることが好ましく、30°以上であることがより好ましい。これにより、図3に示すようにテーパ部11の遠位端11bの近傍の薄肉部分を低減することができ、血管内治療用器具がテーパ部11の遠位端11bの近傍に引っかかり難くなる。一方、角度θ1は、89°以下であることが好ましく、85°以下であることがより好ましい。これにより少なくとも第1テーパ面S1の内側に位置する開口11Pの開口面積を大きくすることができる。なお筒状部材1のテーパ部11はテーパ面を1つのみ有していてもよい。
図10に示す通り、テーパ部11の複数のテーパ面は、第1テーパ面S1と、第1テーパ面S1より近位側に位置する第2テーパ面S2を含むことが好ましい。第2テーパ面S2は、筒状部10の軸方向10Xとのなす角度が、第1テーパ面S1と筒状部10の軸方向10Xとのなす角度θ1よりも小さいことが好ましい。これにより開口11Pの開口面積を大きくすることができる。第2テーパ面S2と筒状部10の軸方向10Xとのなす角度は、85°未満であることが好ましく、80°以下であることがより好ましい。これにより開口11Pの開口面積を大きくすることができる。一方、当該角度は10°以上であることが好ましく、20°以上であることがより好ましい。これにより、開口11Pへ血管内治療用器具を挿入するときに血管内治療用器具がブレ難くなる。
図10に示す通り、テーパ部11の複数のテーパ面は、第1テーパ面S1と、第1テーパ面S1より近位側に位置する第2テーパ面S2と、第2テーパ面S2より近位側に位置する第3テーパ面S3を含むことが好ましい。第3テーパ面S3は、筒状部10の軸方向10Xとのなす角度が、第2テーパ面S2と筒状部10の軸方向10Xとのなす角度よりも小さいことが好ましい。これにより開口11Pの面積を大きくすることができる。第3テーパ面S3と筒状部10の軸方向10Xとのなす角度は、好ましくは5°以下、より好ましくは2°以下、最も好ましくは0°である。また第3テーパ面S3は、テーパ部11の近位端11aを含むことが好ましい。また第1テーパ面S1と第2テーパ面S2は隣接していることが好ましい。第2テーパ面S2と第3テーパ面S3は隣接していることが好ましい。テーパ部11の複数のテーパ面は、第1テーパ面S1と第2テーパ面S2からなるものであってもよく、第1テーパ面S1と第3テーパ面S3からなるものであってもよく、第2テーパ面S2と第3テーパ面S3からなるものであってもよい。筒状部材1のテーパ部11のテーパ面は、例えばカッター等の切込器具により、筒状体の近位端部に切込みを入れて筒状体の近位端部の一部を切除することにより形成することができる。
図10に示すようにテーパ部11のテーパ面が線状となる向きに筒状部材1を向けたときの視野において、第1テーパ面S1、第2テーパ面S2、第3テーパ面S3は、それぞれ直線状または湾曲線状であることが好ましく、直線状であることがより好ましい。テーパ面が直線状であることにより製造効率が向上する。なお当該視野において第1テーパ面S1が湾曲線状であるとき、第1テーパ面S1と筒状部10の軸方向10Xとのなす角度θ1は、第1テーパ面S1の近位端S1aと遠位端S1bを結ぶ線と軸方向10Xとのなす角度である。当該視野において第2テーパ面S2、第3テーパ面S3がそれぞれ湾曲線状であるときも同様に、各面と筒状部10の軸方向10Xとのなす角度は、各面の近位端と遠位端を結ぶ線と軸方向10Xとのなす角度である。
第2テーパ面S2は、複数のテーパ面のうち筒状部10の軸方向10Xに対して5°超、89°以下の角度で傾斜しているテーパ面の中で最も面積の大きいテーパ面であることが好ましい。図10では、第1テーパ面S1と第2テーパ面S2が筒状部10の軸方向10Xに対して5°超、89°以下の角度で傾斜しており、これらのうち第2テーパ面S2が最も面積が大きくなっている。これにより上述した第2テーパ面S2の傾斜の効果が一層、得られ易くなる。
第2テーパ面S2は、複数のテーパ面のうち最も筒状部材1の径方向1Dにおける長さが長いことが好ましい。図10では、第1テーパ面S1と第2テーパ面S2と第3テーパ面S3のうち第2テーパ面S2が最も径方向1Dにおける長さが長くなっている。これにより上述した第2テーパ面S2の傾斜の効果が一層、得られ易くなる。
図10に示す通り、筒状部材1の径方向1Dにおいて、テーパ部11のテーパ面の近位端は、テーパ面の遠位端よりも線状部材2に近いことが好ましい。具体的には、第1テーパ面S1の近位端S1aは、第1テーパ面S1の遠位端S1bよりも線状部材2に近いことが好ましい。第1テーパ面S1がこのように傾斜していることにより、少なくとも第1テーパ面S1の内側に位置する開口11Pから血管内治療用器具を挿入し易くなる。更に、筒状部材1の径方向1Dにおいて、第2テーパ面S2の近位端は、第2テーパ面S2の遠位端よりも線状部材2に近いことが好ましい。第3テーパ面S3の近位端は、第3テーパ面S3の遠位端よりも線状部材2に近くてもよいが、第3テーパ面S3は線状部材2と平行であることが好ましい。
図3に示す通り、第1切削面2S1の遠位端2S1bは、線状部材2の遠位端2bに位置し、筒状部材1のテーパ部11の遠位端11bは、第1切削面2S1の遠位端2S1bよりも近位側であって、且つ第1切削面2S1の近位端2S1aよりも遠位側に位置することが好ましい。これにより、線状部材2を介して筒状部材を体内の湾曲部に押し込んだときに筒状部10、テーパ部11の順に湾曲し易くなる。
図3に示す通り、筒状部材1は、内層10Lと、内層10Lよりも径方向1Dの外側に位置する外層10Mとを有していることが好ましい。筒状部材1が内層10Lと外層10Mを有することにより、筒状部材1は内側と外側において異なる機能を発揮することができる。
内層10Lはフッ素樹脂を含むことが好ましく、フッ素樹脂からなることがより好ましい。フッ素樹脂は、耐薬品性、非粘着性、低摩擦性に優れる。フッ素樹脂は、ポリテトラフルオロエチレン、エチレンテトラフルオロエチレン、フッ素化エチレンプロピレン、またはこれらの組み合わせを含むことが好ましい。
外層10Mは、ポリアミド樹脂、ポリエステル樹脂、ポリウレタン樹脂、ポリオレフィン樹脂、塩化ビニル樹脂、シリコーン樹脂、天然ゴム、またはこれらの組み合わせを含むことが好ましく、ポリアミド樹脂、ポリウレタン樹脂、またはこれらの組み合わせを含むことがより好ましい。樹脂は、ゴム弾性を有するエラストマーを含んでいてもよい。例えばポリアミド樹脂はポリアミドエラストマーを含んでいてもよく、ポリウレタン樹脂はポリウレタンエラストマーを含んでいてもよい。
外層10Mは、径方向に積層された複数の層を有していてもよい。複数の層のうち径方向に隣接する層は互いに異なる種類の樹脂を含んでいてもよく、同じ種類の樹脂を含んでいてもよい。
外層10Mは、外側面に親水性ポリマーを有することが好ましい。これにより筒状部材1をガイディングカテーテル内や血管内への挿入し易くなる。親水性ポリマーは、ポリ2-ヒドロキシエチルメタアクリレート、ポリアクリルアミド、ポリビニルピロリドン、無水マレイン酸共重合体、またはこれらの組み合わせを含むことが好ましい。無水マレイン酸共重合体は、メチルビニルエーテル無水マレイン酸共重合体であることが好ましい。
線状部材2の遠位端部2Bは、筒状部材1の径方向1Dにおいて内層10Lと外層10Mの間、または外層10M内に配置されていることが好ましい。これにより線状部材2は筒状部材1に強固に固定され易くなる。なお線状部材2の外側面にはホットメルト接着剤等の接着剤が付着していてもよい。
内層10Lと外層10Mは、筒状部10からテーパ部11にわたって軸方向10Xに延在していることが好ましい。これにより筒状部10からテーパ部11の順に滑らかに湾曲し易くなる。
筒状部材1は、筒状部10、テーパ部11、またはこれらの両方の外側面と内側面を貫通する貫通孔、溝等を有していないことが好ましい。これにより、血管内治療用器具の送達時に、筒状部材1内において血管内治療用器具が引っ掛かり難くなる。また筒状部材1は、筒状部10、テーパ部11、またはこれらの両方の外側面において非貫通型の溝を有していないことが好ましい。これにより、ガイドワイヤ、血管内治療用器具等の挿入時の筒状部材1の破損が生じ難くなる。
筒状部材1は補強層12を有していることが好ましい。具体的には、筒状部材1は少なくとも筒状部10において補強層12を有していることが好ましい。補強層12により筒状部10を補強することができ、筒状部10の内腔に例えばガイドワイヤを挿入したときにガイドワイヤが筒状部10を突き抜け難くなる。
補強層12は、コイル、編組チューブ、またはこれらの組み合わせを含むことが好ましい。補強層12がコイルまたは編組チューブを有すること、更にはコイルと編組チューブを有することにより柔軟性を維持したまま筒状部材1は径方向1Dに潰れにくくなる。一方、補強層12がコイルまたは編組チューブを有しないことにより筒状部材1の径方向1Dの長さを低減することができる。編組チューブは、複数の線材が互いに交差するように編み込まれたメッシュ構造を有していることが好ましい。各線材は、単線であってもよいし、撚線であってもよい。
補強層12は、線材として、金属線、繊維、またはこれらの組み合わせを有することが好ましく、金属線を有することがより好ましい。金属線は、ステンレス鋼、チタン、ニッケルチタン合金、ニッケルクロム系合金、コバルトクロム合金等、タングステン合金、またはこれらの組み合わせを含むことが好ましく、ステンレス鋼を含むことがより好ましい。金属線は、後述するX線不透過性物質を含んでいてもよい。繊維は、ポリアリレート繊維、アラミド繊維、超高分子量ポリエチレン繊維、PBO繊維、炭素繊維、またはこれらの組み合わせを含むことが好ましい。繊維は、モノフィラメントであってもよいし、マルチフィラメントであってもよい。
筒状部材1は、筒状部10において補強層12を有しており、テーパ部11において補強層12を有していないことがより好ましい。これによりテーパ部11は湾曲し易くなる。また補強層12の遠位端12bは、筒状部10の遠位端よりも近位側であって筒状部10の遠位端から5mm以内の領域に位置することが好ましい。これにより筒状部10の遠位端近傍まで補強することができる。
図10に示す通り、補強層12の近位端12aは、第1テーパ面S1の遠位端S1bよりも遠位側に位置することが好ましい。これにより第1テーパ面S1とその近傍は湾曲し易くなる。補強層12の近位端12aは、テーパ部11の遠位端11bよりも遠位側に位置することがより好ましい。これによりテーパ部11は湾曲し易くなる。
補強層12は、筒状部材1の径方向1Dにおいて内層10Lと外層10Mの間、または外層10M内に配置されていることが好ましい。これにより補強層12は、筒状部10の内腔に露出し難くなる。
図3に示す通り、線状部材2の遠位端2bは、補強層12の近位端12aよりも遠位側に位置することが好ましい。これにより、線状部材2の第1切削面2S1によって筒状部材1が径方向1Dにおける内側1D2に向かって一層、曲り易くなる。
図示していないが、線状部材2の遠位端2bは、補強層12の近位端12aよりも近位側に位置していてもよい。これにより、補強層12が存在する部分における筒状部10の外径を低減することができる。この場合、線状部材2の遠位端2bは、第1テーパ面S1の遠位端S1bよりも遠位側であってもよく、第1テーパ面S1の遠位端S1bよりも近位側であってもよい。線状部材2の遠位端2bは、テーパ部11の遠位端11bよりも遠位側であってもよく、テーパ部11の遠位端11bよりも近位側であってもよい。
図11に示すように、筒状部材1は、更に補強層12の近位端12a、遠位端12b、またはこれらの両端が内側に位置するように配置されている少なくとも1つのX線不透過性リング13を有していてもよい。図11では、X線不透過性リング13は補強層12の遠位端12bに配置されており、X線透視下における筒状部10の遠位端近傍の目印として機能させることができる。図示していないが補強層12の近位端12aにX線不透過性リング13を配置すると、X線不透過性リング13はX線透視下におけるテーパ部11の遠位端11b近傍の目印として機能させることができる。X線不透過性リング13は、筒状部材1の径方向1Dにおいて補強層12と外層10Mの間に配置されていることが好ましい。これにより補強層12の端部の開きを抑制して、筒状部材1の外側面における補強層12の露出を防止し易くなる。
X線不透過性リング13は、X線不透過性物質を含むリングであり、X線不透過性物質からなるものであることが好ましい。X線不透過性物質は、鉛、バリウム、ヨウ素、タングステン、金、白金、イリジウム、白金イリジウム合金、ステンレス、チタン、コバルトクロム合金、パラジウム、タンタル、またはこれらの組み合わせを含むことが好ましい。
筒状部10の軸方向10Xにおいて、筒状部材1の近位端から線状部材2の近位端までの長さは、筒状部材1の長さの2倍以上であることが好ましい。これによりカテーテル99内に挿入するときの摩擦を低減することができる。当該倍率は10倍以下であってもよい。延長カテーテル91の長さは、1000mm以上、2000mm以下であることが好ましい。筒状部材1の長さは、150mm以上、500mm以下であることが好ましい。筒状部材1の外径は1.2mm以上、3mm以下であることが好ましい。筒状部材1の内腔の直径は1.0mm以上、2.2mm以下であることが好ましい。

遠位端部が前記筒状部材に固定されている線状部材とを有しており、
前記線状部材は、側面に前記筒状部材の径方向における外側を向いている第1切削面を有している延長カテーテル。 - 前記第1切削面の少なくとも一部は、前記遠位端部に位置する請求項1に記載の延長カテーテル。
- 前記線状部材は、前記側面に複数の切削面を有しており、
前記第1切削面は、前記線状部材の遠位端から近位側に20cm離れた位置までの間において最も面積が大きい切削面である請求項1または2に記載の延長カテーテル。 - 前記線状部材は、少なくとも前記筒状部材に固定されている部分において、前記径方向における内側を向いている切削面を有していない請求項1または2に記載の延長カテーテル。
- 前記第1切削面は、前記筒状部材の軸方向に対して傾斜しており、前記径方向において前記第1切削面の遠位端は近位端よりも内側に位置する請求項1または2に記載の延長カテーテル。
- 前記第1切削面は、前記筒状部材の軸方向と平行である部分を有していない請求項1または2に記載の延長カテーテル。
- 前記複数の切削面のうちの最も近位に位置する切削面の近位端は、前記筒状部材の近位端よりも近位側に位置する請求項3に記載の延長カテーテル。
- 前記線状部材は、前記第1切削面を含む第1部分と、前記第1部分よりも近位側に位置し切削面を前記側面に有していない第2部分とを有し、
前記筒状部材の径方向における前記線状部材の断面において、前記第1部分は前記第2部分よりも短径に対する長径の比の値が大きい請求項1または2に記載の延長カテーテル。 - 前記第1部分の近位端は、前記筒状部材の近位端よりも近位側に位置する請求項8に記載の延長カテーテル。
- 前記第1部分の長さは、前記線状部材の長さの10%以下の長さである請求項8に記載の延長カテーテル。
- 前記第1部分は、前記筒状部材の遠位端から、前記筒状部材の前記遠位端から20cm以上、40cm以下の距離離れた位置までの間に位置する請求項8に記載の延長カテーテル。
- 前記筒状部材は、補強層を有している請求項1または2に記載の延長カテーテル。
- 前記線状部材の遠位端は、前記補強層の近位端よりも遠位側に位置する請求項12に記載の延長カテーテル。
- 前記線状部材の遠位端は、前記補強層の近位端よりも近位側に位置する請求項12に記載の延長カテーテル。
- 前記筒状部材は、筒状部と、前記筒状部よりも近位側に位置するテーパ部を有している請求項1または2に記載の延長カテーテル。
- 前記第1切削面の遠位端は、前記線状部材の遠位端に位置し、
前記筒状部材の前記テーパ部の遠位端は、前記第1切削面の遠位端よりも近位側であって、且つ前記第1切削面の近位端よりも遠位側に位置する請求項15に記載の延長カテーテル。 - 前記線状部材は、内腔を有していない中実の線状部材である請求項1または2に記載の延長カテーテル。
- 前記筒状部材の径方向において、前記テーパ部のテーパ面の近位端は、前記テーパ面の遠位端よりも前記線状部材に近い請求項15に記載の延長カテーテル。
