众所周知,支气管胸膜瘘是指胸膜腔和支气管互相连接交通。它是胸外科手术后一种常见的并发症,肺切除术后支气管胸膜瘘的发生率为1.5%到28%,其中全肺切除术后的发生率为4.5%到20%。支气管胸膜瘘常常合并脓胸,病程迁延不愈,是肺切除术后主要的致死原因之一。对于良性病变,如肺结核、支气管扩张和慢性阻塞性肺病等,有时也会发生支气管胸膜瘘。目前临床上对于支气管胸膜瘘和慢性难治性脓胸的治疗缺少有效的方法,而脓胸在普胸外科领域又比较常见。治疗难度大,治愈率低,治疗周期长,患者身心痛苦,治疗费用较高。
目前治疗支气管胸膜瘘和慢性难治性脓胸的方法主要有:胸廓成形术、大网膜移植、胸壁局部肌肉填充等,但是效果欠佳。其治疗的核心问题是封闭支气管和胸膜腔之间的通道,使气管内的分泌物不再持续流入胸腔,从而通过持续换药等手段控制胸腔的感染,为进一步治疗提供条件。在现有技术中,支架可以用于胸膜瘘,但是,影响排痰,并且封堵不彻底;封堵伞也可以用于硬膜瘘,但是,是双蘑菇状,成本高,而且容易滑脱。
因此,亟需一种用于胸膜瘘的支架封堵伞来解决上述问题。
发明内容
本发明的目的是提供一种用于胸膜瘘的支架封堵伞,以解决现有技术存在的问题。
为实现上述目的,本发明提供了如下方案:本发明提供一种用于胸膜瘘的支架封堵伞,包括:收放装置、固定装置和封堵装置;所述固定装置和所述封堵装置可收纳在所述收放装置中,所述固定装置的一端与所述收放装置底部可拆卸连接,所述固定装置的另一端与所述封堵装置的一侧通过螺纹连接,所述固定装置外壁与所述收放装置内壁滑动抵触,所述封堵装置外壁与所述收放装置内壁滑动抵触;
所述固定装置包括支架本体,所述支架本体的一端设置为缩径部,所述缩径部的顶面固接有连接机构,所述连接机构与所述收放装置底部通过螺纹连接,所 述支架本体的设置为L型结构,且所述支架本体外侧弯折部开设有通孔,所述支架本体另一端设置为连接部,所述连接部内径沿远离所述支架本体方向逐渐减小,所述连接部开口处外壁固接有连接座的一端,所述连接座的另一端与所述封堵装置通过螺纹连接。
优选的,所述收放装置包括鞘管本体;所述鞘管本体内设置有内鞘管,所述内鞘管外壁与所述鞘管本体内壁滑动连接,所述内鞘管内壁开设有螺纹,所述内鞘管内壁与所述连接机构外壁通过螺纹连接,所述鞘管本体底部开口处外壁固接有辅助环。
优选的,所述辅助环顶面与所述鞘管本体底部开口处外壁固接,所述辅助环内径从上到下逐渐增加,所述支架本体外壁和所述封堵装置外壁与所述辅助环内壁滑动连接。
优选的,所述连接机构包括连接环,所述连接环外壁上开设有螺纹,所述连接环外壁与所述内鞘管内壁通过螺纹连接,所述连接环底面与所述缩径部顶面固接,所述连接环顶面周向等间距固接有若干辅助座,所述辅助座侧面设置为倾斜面,且所述倾斜面靠近所述连接环外壁。
优选的,所述封堵装置包括封堵伞本体,所述封堵伞本体两端分别固接有第一端点和第二端点,所述第一端点的一端设置有圆台体,所述圆台体与所述辅助座可拆卸连接。
优选的,所述连接座的侧面开设有锥形孔,所述锥形孔内开设有螺纹,所述圆台体的外壁开设有螺纹,所述圆台体与所述锥形孔通过螺纹连接。
优选的,所述封堵伞本体外壁与所述鞘管本体内壁滑动抵触,所述封堵伞本体外壁与所述辅助环内壁滑动抵触。
优选的,所述封堵伞本体与所述支架本体均为镍钛合金金属丝材质。
本发明公开了以下技术效果:设置的收放装置可用于将固定装置和封堵装置进行安装和收取,提高了实际操作中的容错率,避免由于在进行封堵过程中造成由于封堵装置或者固定装置尺寸不合适而造成的额外手术,可以将其进行回收,进行更换,设置的固定装置可以用于进行固定,不影响排痰,并且封堵彻底;同时封堵伞也可以用于硬膜瘘,但是由于封堵伞是双蘑菇状,成本高,而且容易滑脱,因此使用固定装置与封堵装置进行结合,这样支架不需要太密,只要达到固 定作用就可以,伞也只需要单面就可以且不需要担心移位的问题发生,设置的缩径部用于固定的刚性的连接机构,这样在进行收取时,只需将收放装置内部螺纹与连接机构的螺纹进行连接,在将内鞘管向外退出,即可是实现封堵装置与固定装置的释放,设置通孔可以用于满足患者日常排痰和呼吸,在进行安装时将收放装置进入气管中,将封堵装置穿过瘘处后在另一侧进行释放,在继续释放固定装置,实现固定装置在气管内的固定,这样的设置既达到了固定目的,又达到了密封目的,还可以扩大适应症提高了适应性,同时还避免了对病患处的机械挤压,设置的支架本体较为稀疏,这样避免了支架本体内肉芽组织的过度繁殖。

下面将结合本发明实施例中的附图,对本发明实施例中的技术方案进行清楚、完整地描述,显然,所描述的实施例仅仅是本发明一部分实施例,而不是全部的实施例。基于本发明中的实施例,本领域普通技术人员在没有做出创造性劳动前提下所获得的所有其他实施例,都属于本发明保护的范围。
为使本发明的上述目的、特征和优点能够更加明显易懂,下面结合附图和具体实施方式对本发明作进一步详细的说明。
实施例1
参照图1-5,本发明提供一种用于胸膜瘘的支架封堵伞,包括:收放装置、固定装置和封堵装置;固定装置和封堵装置可收纳在收放装置中,固定装置的一端与收放装置底部可拆卸连接,固定装置的另一端与封堵装置的一侧通过螺纹连接,固定装置外壁与收放装置内壁滑动抵触,封堵装置外壁与收放装置内壁滑动抵触;
固定装置包括支架本体1,支架本体1的一端设置为缩径部2,缩径部2的顶面固接有连接机构,连接机构与收放装置底部通过螺纹连接,支架本体1的设置为L型结构,且支架本体1外侧弯折部开设有通孔3,支架本体1另一端设置为连接部4,连接部4内径沿远离支架本体1方向逐渐减小,连接部4开口处外壁固接有连接座5的一端,连接座5的另一端与封堵装置通过螺纹连接。

进一步优化方案,收放装置包括鞘管本体6;鞘管本体6内设置有内鞘管7,内鞘管7外壁与鞘管本体6内壁滑动连接,内鞘管7内壁开设有螺纹,内鞘管7内壁与连接机构外壁通过螺纹连接,鞘管本体6底部开口处外壁固接有辅助环8。
进一步优化方案,辅助环8顶面与鞘管本体6底部开口处外壁固接,辅助环 8内径从上到下逐渐增加,支架本体1外壁和封堵装置外壁与辅助环8内壁滑动连接。
内鞘管7用于对连接机构进行螺纹连接固定,同时内鞘管7可以在鞘管本体6内进行伸缩,实现对支架本体1的释放和回收,设置的辅助环8防止在进行回收时的刚性接触,避免对支架本体1和封堵伞本体11造成破坏。
进一步优化方案,连接机构包括连接环9,连接环9外壁上开设有螺纹,连接环9外壁与内鞘管7内壁通过螺纹连接,连接环9底面与缩径部2顶面固接,连接环9顶面周向等间距固接有若干辅助座10,辅助座10侧面设置为倾斜面,且倾斜面靠近连接环9外壁。
辅助座10的设置提高在收回时便捷性,便于内鞘管7与连接环9进行螺纹连接。
进一步优化方案,封堵装置包括封堵伞本体11,封堵伞本体11两端分别固接有第一端点12和第二端点13,第一端点12的一端设置有圆台体14,圆台体14与辅助座10可拆卸连接。
进一步优化方案,连接座5的侧面开设有锥形孔15,锥形孔15内开设有螺纹,圆台体14的外壁开设有螺纹,圆台体14与锥形孔15通过螺纹连接。
设置的锥形孔15可以适应不同尺寸的圆台体14,以适应不同尺寸的封堵伞本体11可以使其更换,提高对不同直径瘘孔的适应性。
进一步优化方案,封堵伞本体11外壁与鞘管本体6内壁滑动抵触,封堵伞本体11外壁与辅助环8内壁滑动抵触。
进一步优化方案,封堵伞本体11与支架本体1均为镍钛合金金属丝材质。
实施例2
参照图6,设置的支架本体1还可以设置为Y型结构,其中一个支管为开放设置,另一支管与封堵装置进行连接,同于对瘘进行封堵。
在本发明的描述中,需要理解的是,术语“纵向”、“横向”、“上”、“下”、“前”、“后”、“左”、“右”、“竖直”、“水平”、“顶”、“底”、“内”、“外”等指示的方位或位置关系为基于附图所示的方位或位置关系,仅是为了便于描述本发明,而不是指示或暗示所指的装置或元件必须具有特定的方位、以特定的方位构造和操作,因此不能理解为对本发明的限制。

Claims (8)

  1. 一种用于胸膜瘘的支架封堵伞,其特征在于,包括:收放装置、固定装置和封堵装置;所述固定装置和所述封堵装置可收纳在所述收放装置中,所述固定装置的一端与所述收放装置底部可拆卸连接,所述固定装置的另一端与所述封堵装置的一侧通过螺纹连接,所述固定装置外壁与所述收放装置内壁滑动抵触,所述封堵装置外壁与所述收放装置内壁滑动抵触;
    所述固定装置包括支架本体(1),所述支架本体(1)的一端设置为缩径部(2),所述缩径部(2)的顶面固接有连接机构,所述连接机构与所述收放装置底部通过螺纹连接,所述支架本体(1)的设置为L型结构,且所述支架本体(1)外侧弯折部开设有通孔(3),所述支架本体(1)另一端设置为连接部(4),所述连接部(4)内径沿远离所述支架本体(1)方向逐渐减小,所述连接部(4)开口处外壁固接有连接座(5)的一端,所述连接座(5)的另一端与所述封堵装置通过螺纹连接。
  2. 根据权利要求1所述的一种用于胸膜瘘的支架封堵伞,其特征在于:所述收放装置包括鞘管本体(6);所述鞘管本体(6)内设置有内鞘管(7),所述内鞘管(7)外壁与所述鞘管本体(6)内壁滑动连接,所述内鞘管(7)内壁开设有螺纹,所述内鞘管(7)内壁与所述连接机构外壁通过螺纹连接,所述鞘管本体(6)底部开口处外壁固接有辅助环(8)。
  3. 根据权利要求2所述的一种用于胸膜瘘的支架封堵伞,其特征在于:所述辅助环(8)顶面与所述鞘管本体(6)底部开口处外壁固接,所述辅助环(8)内径从上到下逐渐增加,所述支架本体(1)外壁和所述封堵装置外壁与所述辅助环(8)内壁滑动连接。
  4. 根据权利要求3所述的一种用于胸膜瘘的支架封堵伞,其特征在于:所述连接机构包括连接环(9),所述连接环(9)外壁上开设有螺纹,所述连接环(9)外壁与所述内鞘管(7)内壁通过螺纹连接,所述连接环(9)底面与所述缩径部(2)顶面固接,所述连接环(9)顶面周向等间距固接有若干辅助座(10),所述辅助座(10)侧面设置为倾斜面,且所述倾斜面靠近所述连接环(9)外壁。
  5. 根据权利要求4所述的一种用于胸膜瘘的支架封堵伞,其特征在于:所述封堵装置包括封堵伞本体(11),所述封堵伞本体(11)两端分别固接有第一端点(12)和第二端点(13),所述第一端点(12)的一端设置有圆台体(14), 所述圆台体(14)与所述辅助座(10)可拆卸连接。
  6. 根据权利要求5所述的一种用于胸膜瘘的支架封堵伞,其特征在于:所述连接座(5)的侧面开设有锥形孔(15),所述锥形孔(15)内开设有螺纹,所述圆台体(14)的外壁开设有螺纹,所述圆台体(14)与所述锥形孔(15)通过螺纹连接。
  7. 根据权利要求6所述的一种用于胸膜瘘的支架封堵伞,其特征在于:所述封堵伞本体(11)外壁与所述鞘管本体(6)内壁滑动抵触,所述封堵伞本体(11)外壁与所述辅助环(8)内壁滑动抵触。
  8. 根据权利要求7所述的一种用于胸膜瘘的支架封堵伞,其特征在于:所述封堵伞本体(11)与所述支架本体(1)均为镍钛合金金属丝材质。
