Abstract
本発明は、C4orf48(Chromosome 4 Open Reading Frame 48)、破骨細胞による骨吸収を抑制する能力及び骨芽細胞による骨形成を促進する能力のうちの少なくとも一方を有するC4orf48変異体、並びに破骨細胞による骨吸収を抑制する能力及び骨芽細胞による骨形成を促進する能力のうちの少なくとも一方を有するC4orf48誘導体よりなる群から選択される少なくとも1種のポリペプチド又は前記ポリペプチドをコードする核酸を含む骨代謝細胞調節剤、並びに骨増加が望まれる疾患又は状態、例えば骨粗鬆症等を治療又は予防するための医薬組成物を提供する。
Description
本発明は、C4orf48(Chromosome 4 Open Reading Frame 48)、C4orf48変異体及びC4orf48誘導体よりなる群から選択される少なくとも1種のポリペプチドを含む、骨代謝細胞調節剤、及び骨増加が望まれる疾患又は状態を治療又は予防するための医薬組成物に関する。
骨の恒常性と構造的形状は、骨を吸収する破骨細胞と骨を形成する骨芽細胞の協調作用によって保持される。破骨細胞と骨芽細胞の協調作用のアンバランスは、骨密度の低下又は上昇を特徴とする様々な代謝性骨疾患をもたらし得る。代謝性骨疾患の代表例は、骨粗鬆症である。骨粗鬆症は、軽微な外傷による骨折を患者に引き起こしてQOLを著しく損なうだけでなく、骨折が契機となって当該患者が寝たきりとなり終身介護が必要となる等、医療費の増大を招く一因となっている。
骨粗鬆症等の骨減少性疾患の治療には骨吸収の抑制が重要であることから、単球・マクロファージ系の破骨細胞前駆細胞から破骨細胞への分化及び破骨細胞の活性化の阻害又は抑制を標的とした治療薬が開発されている。例えば、破骨細胞の活動阻害薬であるビスホスホネートが、臨床で広く使用されている。しかしながら、ビスホスホネートによる治療は、胃腸障害や顎骨壊死等の副作用を伴うため長期投与が推奨されない、治療前や治療中にカルシウムやビタミンDの補充が必要である、等の問題を有する(例えば非特許文献1を参照されたい)。その骨量増加効果も決して満足がいくものではない。
また、新たな治療薬として、破骨細胞分化のシグナル経路である、Receptor activator of nuclear factor κB ligand (RANKL) とその受容体であるosteoprotegerin (OPG) を中心としたRANKL/RANK/OPG経路の阻害薬、例えば抗RANKL抗体が知られている。破骨細胞の分化は、破骨細胞前駆細胞上に発現したRANK及びc-fmsが、骨芽細胞から分泌されるRANKL及びM-CSF (Macrophage-Colony Stimulating Factor) を認識することにより誘導される。RANKL/RANK/OPG経路の阻害薬は、経路の遮断により破骨細胞の分化抑制を引き起こし、骨吸収亢進を抑制するものと考えられている。一方、RANKL/RANK/OPG経路は器官形成、免疫寛容、乳腺の発達等にも関与していることから、それらに対する障害の可能性が指摘されている(例えば非特許文献2及び3を参照されたい)。
骨芽細胞による骨形成(同化作用)を促進することによる骨再生も、骨粗鬆症の治療に有効である。例えば、副甲状腺ホルモン(PTH)は、骨芽細胞の分化を促進及びアポトーシスを抑制する機能を有しており(非特許文献4)、骨粗鬆症治療薬として認可されている。

本発明は、破骨細胞及び骨芽細胞といった骨代謝細胞の制御を通じて、骨増加が望まれる疾患又は状態の新たな治療手段を提供する。
本発明者らは、機能未知のタンパク質であるC4orf48が、破骨細胞の分化誘導を抑制し、骨吸収を抑制する能力、及び骨芽細胞の同化因子の発現を亢進させ、骨形成を促進する能力を有していることを見出し、下記の各発明を完成させた。

本発明によると、破骨細胞による骨吸収を抑制し、骨芽細胞の骨形成を促進することができ、骨増加が望まれる疾患又は状態を治療又は予防することが可能になる。
以下に示す本発明の説明は、代表的な実施形態又は具体例に基づくことがあるが、本発明はそのような実施形態又は具体例に限定されるものではない。また、本発明に関連して言及するタンパク質及び遺伝子に関する説明は、特に断らない限り、ヒト由来のタンパク質及び遺伝子を例として行うが、本発明において利用されるタンパク質及び遺伝子はヒト由来のものに限定されず、また本発明の適用対象はヒトに限定されない。
本発明は、一態様において、C4orf48、C4orf48変異体及びC4orf48誘導体よりなる群から選択される少なくとも1種のポリペプチドを含む骨代謝細胞を調節するための剤を提供する。
C4orf48
C4orf48は、その遺伝子がヒト4番染色体2,035,610-2,043,970位(GRCh38/hg38)にコードされているニューロペプチド様のタンパク質であり、Chromosome 4 Open Reading Frame 48、Neuropeptide-Like Protein C4orf48、CHR4_55とも称される。C4orf48は、重度精神遅滞、成長障害、難治性てんかん、多発奇形を主徴とするウォルフ・ヒルシュホーン症候群(Wolf-Hirschhorn syndrome)に関連するタンパク質として報告されているが(S. Endele, et al., neurogenetics, 2011, Vol. 12, 155-163)、その生理学的機能、特に骨代謝に関係する機能は知られていない。

ヒトC4orf48のアミノ酸配列は、NCBI Reference SequenceにAccession番号:NP_001135408.3として登録されている。ヒトC4orf48全長のアミノ酸配列を配列番号1に、成熟型と推測されるシグナル配列を含まないヒトC4orf48部分ペプチドのアミノ酸配列を配列番号2に示す。配列番号2に示されるアミノ酸配列は、配列番号1に示されるアミノ酸配列の44-92位に位置するニューロペプチド様ドメインに相当する。
また、マウスC4orf48(Gm1673とも呼ばれる)のアミノ酸配列(配列番号9)は、NCBI Reference SequenceにAccession番号:NP_001297475.1として登録されている。配列番号9に示されるアミノ酸配列のうち、1-28位はシグナル配列、29-90位はニューロペプチド様ドメインに相当する。
C4orf48は、骨代謝細胞の分化又は機能を調節する能力(骨代謝細胞の調節能)を有する。ここで、骨代謝細胞の調節能とは、破骨細胞による骨吸収を抑制する能力、及び骨芽細胞による骨形成を促進する能力のうちの少なくとも一方、好ましくは両方を意味する。
破骨細胞による骨吸収を抑制する能力は、例えば、C4orf48の存在下での破骨細胞による骨吸収の程度が、C4orf48の非存在下での破骨細胞による骨吸収の程度を下回ることによって確認することができる。
破骨細胞による骨吸収を抑制する能力はまた、例えば、C4orf48の存在下でRANKL及びM-CSF刺激により破骨細胞前駆細胞から分化誘導された破骨細胞の数が、C4orf48の非存在下でRANKL及びM-CSF刺激により分化誘導された破骨細胞の数を下回ることによって確認することもできる。
骨芽細胞による骨形成を促進する能力は、例えば、C4orf48の存在下での骨芽細胞の石灰化の程度又はアルカリホスファターゼ活性が、C4orf48の非存在下での骨芽細胞の石灰化の程度又はアルカリホスファターゼ活性を上回ることによって、確認することができる。
また、骨芽細胞は骨形成の際、I型コラーゲン、オステオカルシン、オステオポンチン,骨シアロタンパク質、プロテオグリカンといった骨基質タンパク質を産生する。さらに、骨形成には、RUNX2(RUNX Family Transcription Factor 2)、VEGFA(Vascular Endothelial Growth Factor A)、COL1A1 (Collagen Type 1 Alpha 1 Chain)、ATF4(Activating Transcription Factor 4)、OSX又はAPIファミリーに属する転写因子といった同化因子の関与が知られている。したがって、骨芽細胞による骨形成を促進する能力は、例えば、C4orf48の存在下での骨芽細胞における上記骨基質タンパク質又は上記同化因子の遺伝子レベル又はタンパク質レベルでの発現量が、C4orf48の非存在下での骨芽細胞における上記骨基質タンパク質又は上記同化因子の遺伝子レベル又はタンパク質レベルでの発現量を上回ることによって確認することもできる。
C4orf48の変異体及び誘導体
本発明においては、C4orf48の他に、C4orf48が有する骨代謝細胞を調節する能力、すなわち破骨細胞による骨吸収を抑制する能力又は骨芽細胞による骨形成を促進する能力を保持したC4orf48の変異体及び誘導体を用いることもできる。

本発明において用いることができるC4orf48変異体の一つの例は、C4orf48の部分ペプチドである。C4orf48の部分ペプチドとしては、C4orf48のニューロペプチド様ドメインに相当する部分ペプチド(ヒトC4orf48の場合、配列番号2に示されるアミノ酸配列からなるポリペプチド)、及びニューロペプチド様ドメインを保持したC4orf48の部分ペプチド(ヒトC4orf48の場合、配列番号2に示されるアミノ酸配列を保持した、配列番号1に示されるアミノ酸配列の部分アミノ配列からなるポリペプチド)を挙げることができる。
ニューロペプチド様ドメインを保持したヒトC4orf48の部分ペプチドは、例えば、配列番号1に示されるアミノ酸配列の35-92位、35-95位、34-92位、又は34-95位のアミノ酸配列からなるポリペプチドである。
C4orf48の部分ペプチドの他の例としては、配列番号1に示されるアミノ酸配列の33-50位のアミノ酸配列からなるポリペプチド(配列番号10;P1と表す)、配列番号1に示されるアミノ酸配列の51-68位のアミノ酸配列からなるポリペプチド(配列番号11;P2と表す)、配列番号1に示されるアミノ酸配列の33-68位のアミノ酸配列からなるポリペプチド(配列番号12;P3と表す)、配列番号9に示されるアミノ酸配列の64-90位のアミノ酸配列からなるポリペプチド(配列番号13;P4と表す)、配列番号1に示されるアミノ酸配列の69-95位のアミノ酸配列からなるポリペプチド(配列番号14)、配列番号1に示されるアミノ酸配列の64-77位のアミノ酸配列からなるポリペプチド(配列番号15)を挙げることができる。
本発明において用いることができるC4orf48変異体の別の例は、C4orf48の全長又は部分ペプチドのアミノ酸配列と70%以上、好ましくは80%以上、より好ましくは90%以上、さらに好ましくは95%以上、特に好ましくは97%、98%又は99%以上の配列同一性を有するアミノ酸配列からなり、かつ破骨細胞による骨吸収を抑制する能力及び骨芽細胞による骨形成を促進する能力のうちの少なくとも一方を有するポリペプチドである。また別の例は、配列番号1に示されるアミノ酸配列において最大28個の、例えば1~28個、1~25個、1~20個、1~15個、1~10個、1~5個、1~3個、1~2個、又は1個といった数のアミノ酸残基が欠失、置換又は付加されたアミノ酸配列からなり、かつ破骨細胞による骨吸収を抑制する能力及び骨芽細胞による骨形成を促進する能力のうちの少なくとも一方を有するポリペプチドである。さらなる別の例は、配列番号2に示されるアミノ酸配列において最大14個の、例えば1~14個、1~12個、1~10個、1~5個、1~4個、1~3個、1~2個、又は1個といった数のアミノ酸残基が欠失、置換又は付加されたアミノ酸配列からなり、かつ破骨細胞による骨吸収を抑制する能力及び骨芽細胞による骨形成を促進する能力のうちの少なくとも一方を有するポリペプチドである。またさらなる別の例は、配列番号10~15のいずれかに示されるアミノ酸配列において最大4個の、例えば1~4個、1~3個、1~2個、又は1個といった数のアミノ酸残基が欠失、置換又は付加されたアミノ酸配列からなり、かつ破骨細胞による骨吸収を抑制する能力及び骨芽細胞による骨形成を促進する能力のうちの少なくとも一方を有するポリペプチドである。
置換はいわゆる保存的置換が好ましく、そのような例としては、グリシン(Gly)とプロリン(Pro)、グリシンとアラニン(Ala)又はバリン(Val)、ロイシン(Leu)とイソロイシン(Ile)、グルタミン酸(Glu)とグルタミン(Gln)、アスパラギン酸(Asp)とアスパラギン(Asn)、システイン(Cys)とスレオニン(Thr)、スレオニンとセリン(Ser)又はアラニン、リジン(Lys)とアルギニン(Arg)等のアミノ酸の間での置換を挙げることができる。
アミノ酸配列の同一性は、アラインメント長に対する同一アミノ酸残基数の割合で表され、比較される2つのアミノ酸配列のアラインメントは、同一となるアミノ酸残基の数が最も多くなるように常法に従って行われる。配列同一性は、当業者に公知の任意の方法により、例えばBLAST等の配列比較プログラムを用いて決定することができる。
本発明において用いることができるC4orf48誘導体の例は、C4orf48又はC4orf48変異体と他のペプチドとが融合した、破骨細胞による骨吸収を抑制する能力及び骨芽細胞による骨形成を促進する能力のうちの少なくとも一方を有するポリペプチドである。また、C4orf48誘導体の別の例は、C4orf48又はC4orf48変異体が化学修飾された、破骨細胞による骨吸収を抑制する能力及び骨芽細胞による骨形成を促進する能力のうちの少なくとも一方を有する化学修飾ポリペプチドである。化学修飾は、C4orf48又はC4orf48変異体の1又は複数のアミノ酸残基に対して行うことができる。
融合タンパク質における他のペプチドとしては、例えば、ヒトIgGのFc領域(Fcタンパク質)、Hisタグ、GSTタグ、HAタグ、FLAGタグ(DYKDDDDKタグ、配列番号16)等を挙げることができる。また、化学修飾としては、例えば、アミノ酸残基のアミノ基の修飾(ビオチン化、ミリストイル化、パルミトイル化、アセチル化、マレイミド化、メチル化、マロニル化等)、カルボキシル基の修飾(アミド化、エステル化等)、チオール基の修飾(ファルネシル化、ゲラニル化、メチル化、パルミトイル化等)、水酸基の修飾(リン酸化、硫酸化等)、PEG化、グリコシル化等を挙げることができる。
C4orf48変異体及び誘導体が有する、破骨細胞による骨吸収を抑制する能力又は骨芽細胞による骨形成を促進する能力は、C4orf48について上述したものと同様の方法で確認することができる。
C4orf48、C4orf48変異体又はC4orf48誘導体をコードする核酸
本発明においては、C4orf48、C4orf48変異体及びC4orf48誘導体に加えて、C4orf48、C4orf48変異体又はC4orf48誘導体をコードする核酸も骨代謝細胞調節剤として用いることができる。

核酸は、DNA、RNA又はそれらの安定性が高められた核酸修飾体のいずれでもよい。また直鎖状のものでも、環状のものでもよい。核酸がDNAである場合は発現ベクターに組み込まれた形態であるものが好ましく、RNAである場合はmRNAが好ましい。発現ベクターの例としては、pBApo-CMV(Takara)その他の市販の発現ベクターにC4orf48、C4orf48変異体又はC4orf48誘導体をコードするDNAを組み込んだものを挙げることができる。
C4orf48、C4orf48変異体及びC4orf48誘導体の調製
C4orf48、C4orf48変異体及びC4orf48誘導体は、これらをコードする核酸を含む発現ベクターを適当な宿主細胞、例えば大腸菌その他の微生物、昆虫細胞又は動物細胞等に導入し発現させることを含む遺伝子工学的生産方法によって作製することができる。核酸の作製、宿主細胞の種類と核酸の導入方法、タンパク質の発現及び精製等を含む、遺伝子工学的生産方法における各操作は、種々の遺伝子工学的操作を詳細に解説した実験操作マニュアル書の指示に基づいて、当業者に公知又は周知の方法により行うことができる。本発明は、一態様において、C4orf48、C4orf48変異体及びC4orf48誘導体のそれぞれをコードする核酸、及びこれらで形質転換された宿主細胞も提供する。ここでC4orf48、C4orf48変異体及びC4orf48誘導体のそれぞれをコードする核酸は、これらの各々をコードするmRNAであってもDNAであってもよい。またこれらの各々をコードするDNAは発現ベクターに組み込まれた形態であってもよい。

また、C4orf48、C4orf48変異体及びC4orf48誘導体をコードする核酸を利用した無細胞系の合成方法も、C4orf48、C4orf48変異体及びC4orf48誘導体の遺伝子工学的な生産方法の1つである。無細胞タンパク質合成系としては、大腸菌、コムギ胚芽、酵母、ウサギ網状赤血球、昆虫細胞及び哺乳類培養細胞といった細胞の抽出液を利用する系や、タンパク質合成に必要な因子を組み合わせて構成した再構成型の系が挙げられる。
C4orf48、C4orf48変異体及びC4orf48誘導体は、例えばFmoc法(フルオレニルメチルオキシカルボニル法)やtBoc法(t-ブチルオキシカルボニル法)等の有機化学的合成方法により、市販の適当なペプチド合成機を用いて生産することもできるが、遺伝子組換え技術によって、前記の核酸、特に発現ベクターに組み込まれたDNAを原核生物又は真核生物から選択される適当な宿主細胞を用いた好適な発現系に導入することによって生産することが好ましい。
また、遺伝子工学的生産方法又は有機化学的合成方法により調製されたC4orf48、C4orf48変異体及びC4orf48誘導体を当業者に知られた種々の方法で化学修飾することで、C4orf48、C4orf48変異体及びC4orf48誘導体の1又は複数のアミノ酸残基が化学修飾されたタンパク質を調製することができる。
骨代謝細胞調節剤
C4orf48、C4orf48変異体及びC4orf48誘導体、並びにこれらをコードする核酸は、細胞(インビトロ)又は生体(インビボ)において、骨代謝細胞調節剤の有効成分として、破骨細胞による骨吸収を抑制するために、又は骨芽細胞による骨形成を促進するために用いることができる。骨代謝細胞調節剤は、2以上の物質を含む組成物であり得る。
C4orf48、C4orf48変異体及びC4orf48誘導体、並びにこれらをコードする核酸は、細胞(インビトロ)又は生体(インビボ)において、骨代謝細胞調節剤の有効成分として、破骨細胞による骨吸収を抑制するために、又は骨芽細胞による骨形成を促進するために用いることができる。骨代謝細胞調節剤は、2以上の物質を含む組成物であり得る。
骨代謝細胞調節剤は、インビトロで破骨細胞や骨吸収に関する研究のための、又は骨芽細胞や骨形成に関する研究のためのリサーチツールとして好適に用いることができる。また骨代謝細胞調節剤は、インビボで骨増加が望まれる疾患又は状態の治療又は予防に好適に用いることができる。
さらに、本発明は、一態様において、C4orf48、C4orf48変異体及びC4orf48誘導体並びにこれらをコードする核酸よりなる群から選択される少なくとも1種の物質を、好ましくはC4orf48、C4orf48変異体及びC4orf48誘導体よりなる群から選択される少なくとも1種のポリペプチドを含む骨代謝調節剤を提供する。
医薬組成物
本発明はさらに、一態様において、C4orf48、C4orf48変異体及びC4orf48誘導体並びにこれらをコードする核酸よりなる群から選択される少なくとも1種の物質を、好ましくはC4orf48、C4orf48変異体及びC4orf48誘導体よりなる群から選択される少なくとも1種のポリペプチドを含む、骨増加が望まれる疾患又は状態の治療又は予防のための医薬組成物を提供する。
本発明はさらに、一態様において、C4orf48、C4orf48変異体及びC4orf48誘導体並びにこれらをコードする核酸よりなる群から選択される少なくとも1種の物質を、好ましくはC4orf48、C4orf48変異体及びC4orf48誘導体よりなる群から選択される少なくとも1種のポリペプチドを含む、骨増加が望まれる疾患又は状態の治療又は予防のための医薬組成物を提供する。
本発明において、骨増加が望まれる疾患又は状態とは、骨量又は骨密度の増加により治療又は予防が期待される疾患又は状態をいい、その例は骨減少性疾患、あるいは複雑性骨折、脆弱性骨折等の骨折である。
本発明において、骨減少とは、骨代謝バランスが崩れて骨吸収が骨形成を上回ったことによって骨量又は骨密度、特に骨密度が低下すること、又は低下した状態をいう。骨減少性疾患とは、症状として骨減少を伴う疾患をいい、その例としては、骨粗鬆症、骨ページェット病、骨減少症、副甲状腺機能亢進症、関節リウマチ、乾癬性関節炎、歯周病、家族性膨張性骨溶解症、骨巨細胞腫、多発性骨髄腫、癌関連骨転移、炎症性骨吸収、感染に伴う骨吸収、人工物による無菌性骨吸収、抜歯後の歯槽骨吸収、歯槽骨増生術後の骨吸収等を挙げることができる。本発明の好ましい実施形態において、医薬組成物は、骨粗鬆症の治療又は予防のために用いることができる。
本明細書において用いられる用語「治療」は、疾患又は状態の治癒、一時的寛解等を目的とする医学的に許容される全てのタイプの治療的介入を包含する。また用語「予防」は、疾患又は状態の罹患又は発症の防止若しくは抑制などを目的とする医学的に許容される全てのタイプの予防的介入を包含する。すなわち、骨増加が望まれる疾患又は状態の治療及び/又は予防とは、骨増加が望まれる疾患又は状態の進行の遅延又は停止、病変の退縮又は消失、発症の予防又は再発の防止などを含む、種々の目的の医学的に許容される介入を包含する。
医薬組成物は、骨増加が望まれる対象、例えば、骨減少を呈する、又は呈するおそれのある対象、あるいは骨折した、又は骨折のおそれのある対象に投与される。対象は、例えばマウス、ラット、ハムスター、モルモットを含むげっ歯類、ヒト、チンパンジー、アカゲザルを含む霊長類、ブタ、ウシ、ヤギ、ウマ、ヒツジを含む家畜、イヌ、ネコを含む愛玩動物といった哺乳動物である。好ましい対象は、ヒトである。
医薬組成物は、骨増加が望まれる疾患又は状態の治療又は予防のために有効な量のC4orf48、C4orf48変異体及びC4orf48誘導体、並びにこれらをコードする核酸よりなる群から選択される少なくとも1種の物質を含み、ここで有効量は、用法、対象の年齢、性別、体重、疾患又は状態の種類及び程度、剤形、投与経路その他の要因に応じて適宜決定される。
医薬組成物は、骨増加が望まれる疾患又は状態の治療又は予防のための他の薬剤をさらに含むことができる。このような他の薬剤の例としては、限定されるものではないが、カルシウム製剤、活性型ビタミンD3、ビタミンK2、副甲状腺ホルモン、ビスホスホネート、エストラジオール等の女性ホルモン、選択的エストロゲン受容体モジュレーター(SERM)、カルシトニン、抗RANKL抗体等を挙げることができる。
医薬組成物は、薬学的に許容される添加剤、例えば緩衝剤、安定剤、保存剤、賦形剤等をさらに含むことができる。薬学的に許容される添加剤は当業者に周知であり、当業者が通常の実施能力の範囲内で適宜選択して使用することができる。
医薬組成物の剤形は任意であり、標的部位や具体的な疾患又は状態等に応じて適宜選択することができる。剤形は、一般に非経口製剤であることが好ましく、例えば注射剤、点滴剤等であることができる。また医薬組成物の投与経路は、特に制限されないが、非経口製剤である場合は、例えば血管内投与(好ましくは静脈内投与)、腹腔内投与、腸管内投与、皮下投与、標的部位への局所投与等を挙げることができる。好ましい実施形態の一つにおいて、医薬組成物は、静脈内投与又は局所投与によって対象に投与される。
本発明は、一態様において、その必要がある対象に有効量のC4orf48、C4orf48変異体及びC4orf48誘導体並びにこれらをコードする核酸よりなる群から選択される少なくとも1種の物質を、好ましくは有効量のC4orf48、C4orf48変異体及びC4orf48誘導体よりなる群から選択される少なくとも1種のポリペプチドを投与することを含む、骨増加が望まれる疾患又は状態を治療又は予防する方法を提供する。
本発明はまた、一態様において、C4orf48、C4orf48変異体及びC4orf48誘導体並びにこれらをコードする核酸よりなる群から選択される少なくとも1種の物質の、好ましくはC4orf48、C4orf48変異体及びC4orf48誘導体よりなる群から選択される少なくとも1種の物質の、骨増加が望まれる疾患又は状態を治療又は予防するための医薬組成物の製造のための使用を提供する。本発明はさらに、一態様において、C4orf48、C4orf48変異体及びC4orf48誘導体並びにこれらをコードする核酸よりなる群から選択される少なくとも1種の物質の、好ましくはC4orf48、C4orf48変異体及びC4orf48誘導体よりなる群から選択される少なくとも1種の物質の、骨増加が望まれる疾患又は状態を治療又は予防するための使用を提供する。
以下の実施例によって本発明をさらに詳細に説明するが、本発明はこれらに限定されるものではない。
実験操作は、既報(Terkawi et al., 2019; Acta Biomaterialia, 15;89:242-251. doi: 10.1016/j.actbio.2019.03.028.及びMatsumae & Terkawi et al., 2021, Bioengineering & Translational Medicine, doi: 10.1002/btm2.10232)の記載に従って行い、市販のキットを用いた操作は製造者が提供するプロトコールに従って行った。また、統計解析はスチューデントのt検定又は一元配置分散分析を用いて行い、結果は平均±標準誤差(SEM)として示し、p<0.05(GraphPad)の場合に統計的に有意であるとみなした。
実施例1 C4orf48による破骨細胞分化及び骨吸収の抑制(インビトロ)
1)C4orf48
本研究では、2種類のC4orf48を購入し使用した。一つが、C4orf48(大腸菌由来:部分配列(配列番号2)、N-teminal His-tag付加)であり、もう一つがC4orf48(哺乳動物細胞:全長配列(配列番号1)、DYKDDDDK tag付加)である。前者はインビトロ実験に使用し、後者はインビボ実験に使用した。
2)単球の調製
健常ドナー(3名のアジア人、年齢幅は30-45歳)のヒト末梢血から、Ficoll-Paque TM PLUS(GE Healthcare)を使用した密度勾配遠心分離によって単核球画分を回収し、次いでMACS Pan monocyte isolation kit(Miltenyi Biotec)磁気ビーズを用いて単球を抽出した。75 cm2フラスコに10% FBS、5% ペニシリン/ストレプトマイシン及び5% L-グルタミンを含むα-MEMに懸濁した単球を加え、5% CO2、37℃で3時間培養した後、培地を25 ng/ml組換えマクロファージコロニー刺激因子(M-CSF, Peprotech)を含むα-MEMに交換して、3日間培養した。培養後、接着細胞をPBSで3回洗浄し、1%トリプシン-EDTA溶液(GE Healthcare)で5分間処理して剥がし、PBSで洗浄して単球を回収し、以下の実験に使用した。
1)C4orf48
本研究では、2種類のC4orf48を購入し使用した。一つが、C4orf48(大腸菌由来:部分配列(配列番号2)、N-teminal His-tag付加)であり、もう一つがC4orf48(哺乳動物細胞:全長配列(配列番号1)、DYKDDDDK tag付加)である。前者はインビトロ実験に使用し、後者はインビボ実験に使用した。
2)単球の調製

3)破骨細胞分化アッセイ
上記2)の単球1×104個を96ウェルプレートの各ウェルに播種し、5% CO2、37℃で3日間培養してマクロファージに分化させた後、25 ng/mlのM-CSF、0又は50 ng/mlのRANKL(PeproTech)、及び0、25、50又は100 ng/mlのC4orf48を含むα-MEM中で8日間培養した。培養中、3日ごとにサイトカインを含む新鮮な培地を交換した。培養後の細胞をLeukocyte acid phosphate TRAP kit(Sigma Aldrich)で染色し、3核以上のTRAP陽性細胞を破骨細胞とみなし、各ウェルにおいて10個のランダムな顕微鏡視野の破骨細胞数をカウントし、1 cm2あたりの破骨細胞数を算出した。
上記2)の単球1×104個を96ウェルプレートの各ウェルに播種し、5% CO2、37℃で3日間培養してマクロファージに分化させた後、25 ng/mlのM-CSF、0又は50 ng/mlのRANKL(PeproTech)、及び0、25、50又は100 ng/mlのC4orf48を含むα-MEM中で8日間培養した。培養中、3日ごとにサイトカインを含む新鮮な培地を交換した。培養後の細胞をLeukocyte acid phosphate TRAP kit(Sigma Aldrich)で染色し、3核以上のTRAP陽性細胞を破骨細胞とみなし、各ウェルにおいて10個のランダムな顕微鏡視野の破骨細胞数をカウントし、1 cm2あたりの破骨細胞数を算出した。
RANKLによって分化誘導された破骨細胞の数はC4orf48濃度の上昇と共に減少しており(図1)、C4orf48による破骨細胞の分化誘導の抑制が認められた。
4)骨吸収アッセイ
上記2)の単球3×104個を象牙質スライス(直径6 mm、厚さ300 μm、Wako)上に播種し、25 ng/mlのM-CSF、0又は50 ng/mlのRANKL及び0又は 100 ng/mlのC4orf48を含むα-MEM中で21日間培養した。培養中、3日ごとにサイトカインを含む新鮮な培地を交換した。培養後、20 mg/mlのperoxidase-conjugated wheat germ agglutininで象牙質スライスを染色し、次いで3,3'-ジアミノベンジジン(0.1% H2O2を含む0.52 mg/mL PBS)でインキュベーションした後、象牙質の吸収窩を共焦点顕微鏡によって観察・撮影した。
上記2)の単球3×104個を象牙質スライス(直径6 mm、厚さ300 μm、Wako)上に播種し、25 ng/mlのM-CSF、0又は50 ng/mlのRANKL及び0又は 100 ng/mlのC4orf48を含むα-MEM中で21日間培養した。培養中、3日ごとにサイトカインを含む新鮮な培地を交換した。培養後、20 mg/mlのperoxidase-conjugated wheat germ agglutininで象牙質スライスを染色し、次いで3,3'-ジアミノベンジジン(0.1% H2O2を含む0.52 mg/mL PBS)でインキュベーションした後、象牙質の吸収窩を共焦点顕微鏡によって観察・撮影した。
図2に単球をRANKLあり又はなしと、C4orf48(大腸菌由来:部分配列(配列番号2)、N-teminal His-tag付加)あり又はなしとでそれぞれ8日間培養した上でTRAP染色したもの(左列)と、象牙質スライス上で同じく21日間培養したもの(右列)の画像を示す。右列の画像において、色の暗い箇所が吸収窩に相当する。RANKL存在下で観察された破骨細胞(左列中段)と吸収窩(右列中段)は、C4orf48の添加によって破骨細胞数(左列下段)と吸収窩の面積(右列下段)が減少しており、C4orf48による破骨細胞数と骨吸収の抑制が認められた。
実施例2 C4orf48による破骨細胞分化及び骨吸収の抑制(インビボ)
1) RANKL誘導型骨溶解モデル
北海道大学内にある倫理委員会の承認を得て、既報(Tian & Terkawi et al., Front Immunol. 2020 Aug 4;11:1720.doi: 10.3389/fimmu.2020.01720.)の記載に従って、RANKL誘導型骨溶解マウスモデルを用いて以下の実験を行った。
1) RANKL誘導型骨溶解モデル
北海道大学内にある倫理委員会の承認を得て、既報(Tian & Terkawi et al., Front Immunol. 2020 Aug 4;11:1720.doi: 10.3389/fimmu.2020.01720.)の記載に従って、RANKL誘導型骨溶解マウスモデルを用いて以下の実験を行った。
8週齢の雄性C57BL/6マウス(日本クレア)を2群に分け(n=3)、0.1 mg/kgのRANKL(Biolegend)を100 μlのPBSに溶解した溶液、又は0.1 mg/kgのRANKL及び0.1 mg/kgのC4orf48(哺乳動物細胞:全長配列(配列番号1)にDYKDDDDK tag付加したもの)を100 μlのPBSに溶解した溶液を、それぞれ4日間連続して頭蓋冠の骨膜に注射した。5日目にマウスを安楽死させて頭蓋骨を摘出し、10%ホルマリンで固定した。固定後のマウス頭蓋骨を、マイクロコンピュータ断層撮影装置(micro-CT scanning R_mCT2 scan, Rigaku)を用いて10 μmの等方性分解能で走査し、3Dイメージ画像を作成した。さらに、頭頂骨を横断する四角形の関心領域を選択し、ソフトウェア(NIH Image J)を用いてマウス頭蓋骨表面上の骨吸収領域を定量した。
図3に代表的な頭蓋冠のμCT走査画像を、図4に骨吸収領域の面積割合のグラフを示す。RANKL及びC4orf48を注射したマウスでは、RANKLのみを注射したマウスと比べて、頭蓋冠における骨吸収領域(μCT画像の黒色部分、矢印の箇所)の面積割合が低く、骨吸収の抑制が確認された。
CT撮影後のマウス頭蓋骨を、10% EDTA(Wako)で3日間脱灰した後、頭蓋骨を冠状断で3つに分割した上で、パラフィン包埋した。5 μm厚の組織切片をヘマトキシリン&エオジンとTRAP(Sigma Aldrich)でそれぞれ染色した。3つに分割した頭蓋骨冠状断の内、遠位2部位でのTRAP染色領域を、Image Jを用いて定量的に評価した。
図5に遠位2部位の頭蓋骨冠状断のTRAP染色画像を、図6にTRAP染色領域の面積割合のグラフを示す。TRAP染色の結果もμCTの画像と同様の傾向を示した。
2) 卵巣摘出モデル
12週齢のSFF-BALB/cマウス(クレア、東京、日本)のメスに100 mg/kgのケタミンと10 mg/kgのキシラジンを腹腔内投与し、卵巣摘出術(Ovariectomy: OVX)を行った。また、対照として、開腹のみを行って卵巣を摘出していない偽手術マウス(Sham群)を準備した。OVXを行ったマウスをランダムに2群(PBS投与群、C4orf48 投与群)に分けた。Sham群及びPBS投与群には200μlのPBSを腹腔内投与し、C4orf48投与群には 0.1mg/kgのC4orf48(哺乳動物細胞:全長配列(配列番号1)にDYKDDDDK tag付加したもの)を200 μlのPBSに溶解した溶液を腹腔内投与した。投与は、OVX術後3日から開始し、週に3回を4週間行った。4週後にマウスに麻酔をかけ、頚椎脱臼により安楽死させた。大腿骨を採取し、10%ホルマリン(ワコー、日本)で24時間固定し、μCT(R_mCT2; リガク、東京、日本)を撮影した。μCTは10μmの等方性分解能でスキャンし、X線エネルギーは80kB, 80mAであった。撮影したμCT像は、TRI/3D-BON(ラトックエンジニアリングシステム、日本)を用いて、単位体積当たりの骨体積(BV/TV)を測定した。
12週齢のSFF-BALB/cマウス(クレア、東京、日本)のメスに100 mg/kgのケタミンと10 mg/kgのキシラジンを腹腔内投与し、卵巣摘出術(Ovariectomy: OVX)を行った。また、対照として、開腹のみを行って卵巣を摘出していない偽手術マウス(Sham群)を準備した。OVXを行ったマウスをランダムに2群(PBS投与群、C4orf48 投与群)に分けた。Sham群及びPBS投与群には200μlのPBSを腹腔内投与し、C4orf48投与群には 0.1mg/kgのC4orf48(哺乳動物細胞:全長配列(配列番号1)にDYKDDDDK tag付加したもの)を200 μlのPBSに溶解した溶液を腹腔内投与した。投与は、OVX術後3日から開始し、週に3回を4週間行った。4週後にマウスに麻酔をかけ、頚椎脱臼により安楽死させた。大腿骨を採取し、10%ホルマリン(ワコー、日本)で24時間固定し、μCT(R_mCT2; リガク、東京、日本)を撮影した。μCTは10μmの等方性分解能でスキャンし、X線エネルギーは80kB, 80mAであった。撮影したμCT像は、TRI/3D-BON(ラトックエンジニアリングシステム、日本)を用いて、単位体積当たりの骨体積(BV/TV)を測定した。
図7に代表的な大腿骨μCT走査画像を、図8にBV/TVの測定結果を示す。PBSを投与したOVXマウスでは、Shamマウスと比べて骨量が減少していたが、C4orf48を投与したOVXマウスでは骨量の減少は抑制された。
ホルマリン固定した大腿骨は、組織形態学的評価のためにEDTA溶液(pH 7.0)で3週間脱灰したのち、パラフィンで包埋し、5μm厚の縦切片を作成した。破骨細胞を観察するために、メチルグリーンの対比染色でTRAP(シグマ、セントルイス、ミズーリ州、米国)染色を行った。Image Jソフトウェア(アメリカ国立衛生研究所, 米国)を用いて、二次海綿骨領域における骨周囲長当たりの破骨細胞数(N.Oc/B-Pm)および破骨細胞線(Oc.Pm/B-Pm)を測定した。
図9に代表的なTRAP染色画像を、図10にN.Oc/B-Pm及びOc.Pm/B-Pmの測定結果を示す。PBSを投与したOVXマウスでは、Shamマウスと比べて破骨細胞の数及び面積は増加していたが、C4orf48を投与したOVXマウスではこれらの増加は抑制された。
以上から、C4orf48はIn vivoでも破骨細胞の分化誘導を抑制し、骨吸収を抑制することが確認された。

実施例3 C4orf48による骨芽細胞の同化促進
1)石灰化及びアルカリホスファターゼ活性のアッセイ
ヒト初期胎児骨芽細胞(Cell Applications)は、骨芽細胞分化メディウム(Cell Applications)を用いて第4世代まで継代培養した。96ウェルプレートに1×104個/ウェルの密度で細胞を播種し、50 ng/mlのTNF-α(Peprotech)を単独で、又は50 ng/mlのTNF-αと100 ng/mlのC4orf48(大腸菌由来:部分配列(配列番号2)、N-teminal His-tag付加)を組み合わせて添加して21日間培養した。メディウムは3日ごとに交換した。TNF-αは、骨芽細胞の分化誘導を抑制することが知られていることから、ネガティブコントロールとして使用した。
1)石灰化及びアルカリホスファターゼ活性のアッセイ
ヒト初期胎児骨芽細胞(Cell Applications)は、骨芽細胞分化メディウム(Cell Applications)を用いて第4世代まで継代培養した。96ウェルプレートに1×104個/ウェルの密度で細胞を播種し、50 ng/mlのTNF-α(Peprotech)を単独で、又は50 ng/mlのTNF-αと100 ng/mlのC4orf48(大腸菌由来:部分配列(配列番号2)、N-teminal His-tag付加)を組み合わせて添加して21日間培養した。メディウムは3日ごとに交換した。TNF-αは、骨芽細胞の分化誘導を抑制することが知られていることから、ネガティブコントロールとして使用した。
培養した骨芽細胞をMgCl2・CaCl2含有PBS(Sigma Aldrich)で洗浄した後、中性緩衝ホルマリン液10%(Wako)で固定し、Alizarin red-S溶液(Wako)又はBCIP/NBT溶液(Wako)で染色した。
骨芽細胞をAlizarin red-S又はBCIP/NBT溶液で染色したものを図7に示す。左側がAlizarin red-S溶液(赤色)、右側がBCIP/NBT溶液(青色)である。どちらの染色でも、Mock群以外にはTNF-αを添加することで骨芽細胞の同化作用を抑制させている。TNF-α単独添加により染色性がMock群よりも低下した。しかし、TNF-α添加にさらにC4orf48を加えることで、Mock群と同程度の染色性が得られた。これはTNF-α添加により抑制されるはずの骨芽細胞の骨形成作用が、C4orf48の添加によって回復した、つまりC4orf48は骨形成を促進することを意味する。
2)骨芽細胞同化因子の遺伝子発現量測定
第4世代まで継代培養したヒト初期胎児骨芽細胞を96ウェルプレートに1×105個/ウェルの密度で播種し、骨芽細胞分化メディウムを用いて2週間培養した後、50 ng/mlのTNF-α(Peprotech)又は100 ng/mlのC4orf48(大腸菌由来:部分配列(配列番号2)、N-terminal His-tag付加)を添加して48時間刺激した。細胞からRNeasy Plus Mini kit columns (Qiagen)を用いて全RNAを抽出し、続くcDNAの作成には GoScriptTM reverse transcriptase kit(Promega)を用いて作成した。SYBRR Premix Ex TaqTM II(Takara)と、以下に示すプライマーセットを用いたqRT-PCRによって、骨芽細胞同化因子であるRUNX2、VEGFA及びCOL1A1の発現量を測定した。

第4世代まで継代培養したヒト初期胎児骨芽細胞を96ウェルプレートに1×105個/ウェルの密度で播種し、骨芽細胞分化メディウムを用いて2週間培養した後、50 ng/mlのTNF-α(Peprotech)又は100 ng/mlのC4orf48(大腸菌由来:部分配列(配列番号2)、N-terminal His-tag付加)を添加して48時間刺激した。細胞からRNeasy Plus Mini kit columns (Qiagen)を用いて全RNAを抽出し、続くcDNAの作成には GoScriptTM reverse transcriptase kit(Promega)を用いて作成した。SYBRR Premix Ex TaqTM II(Takara)と、以下に示すプライマーセットを用いたqRT-PCRによって、骨芽細胞同化因子であるRUNX2、VEGFA及びCOL1A1の発現量を測定した。

βアクチン発現量で補正したRUNX2、VEGFA及びCOL1A1の相対的発現量を図8に示す。これらの遺伝子発現量は、TNF-αの存在下で低下する一方、C4orf48の存在下では有意に亢進しており、C4orf48が骨芽細胞による同化を促進することが示唆された。
実施例4 C4orf48部分ペプチドによる破骨細胞分化抑制(インビトロ)
C4orf48の部分ペプチドP1(配列番号10)、P2(配列番号11)、P3(配列番号12)、P4(配列番号13)、P5(配列番号15)について、実施例1の3)と同様の手法で破骨細胞分化アッセイを行った。部分ペプチドP1~P5は濃度1 μg/mlで、比較対象のC4orf48(配列番号2)は濃度100 ng/mlで使用した。
C4orf48の部分ペプチドP1(配列番号10)、P2(配列番号11)、P3(配列番号12)、P4(配列番号13)、P5(配列番号15)について、実施例1の3)と同様の手法で破骨細胞分化アッセイを行った。部分ペプチドP1~P5は濃度1 μg/mlで、比較対象のC4orf48(配列番号2)は濃度100 ng/mlで使用した。
RANKLによって分化誘導された破骨細胞の数は、C4orf48部分ペプチド、特にP1及びP2の添加により減少する傾向が認められた(図13)。
実施例5 C4orf48部分ペプチドによる破骨細胞分化及び骨吸収の抑制(インビボ)
C4orf48の部分ペプチドP1(配列番号10)、及びP2(配列番号11)について、実施例2の2)と同様の手法でOVXマウスでの評価を行った。比較対象として抗RANKL抗体(InVivoMAb anti-mouse RANKL (Cd254), Clone IK22/5, BioCell)、副甲状腺ホルモン(PTH (1-34) (HUMAN) ACETATE, BACHEM)を用いた。マウスへの投与量は、部分ペプチドP1及びP2 が1 mg/kg 及び 2 mg/kg、抗RANKL抗体及びPTHが 1 mg/kgである。骨のμCT撮影、組織染色等の分析を行う。
C4orf48の部分ペプチドP1(配列番号10)、及びP2(配列番号11)について、実施例2の2)と同様の手法でOVXマウスでの評価を行った。比較対象として抗RANKL抗体(InVivoMAb anti-mouse RANKL (Cd254), Clone IK22/5, BioCell)、副甲状腺ホルモン(PTH (1-34) (HUMAN) ACETATE, BACHEM)を用いた。マウスへの投与量は、部分ペプチドP1及びP2 が1 mg/kg 及び 2 mg/kg、抗RANKL抗体及びPTHが 1 mg/kgである。骨のμCT撮影、組織染色等の分析を行う。

e) a)~d)のいずれかのポリペプチドのアミノ酸配列と90%以上の同一性を有するアミノ酸配列からなり、かつ破骨細胞による骨吸収を抑制する能力及び骨芽細胞による骨形成を促進する能力のうちの少なくとも一方を有するポリペプチド
f) a)~e)のいずれかのポリペプチドと他のペプチドとが融合されてなり、かつ破骨細胞による骨吸収を抑制する能力及び骨芽細胞による骨形成を促進する能力のうちの少なくとも一方を有するポリペプチド
g) a)~f)のいずれかのポリペプチドが化学修飾されてなり、かつ破骨細胞による骨吸収を抑制する能力及び骨芽細胞による骨形成を促進する能力のうちの少なくとも一方を有する化学修飾ポリペプチド - 下記のa)~g)よりなる群から選択される少なくとも1種のポリペプチドを含む、骨増加が望まれる疾患又は状態を治療又は予防するための医薬組成物。
a) 配列番号1に示されるアミノ酸配列からなるポリペプチド
b) 配列番号2に示されるアミノ酸配列からなるポリペプチド
c) 配列番号10又は11に示されるアミノ酸配列からなるポリペプチド
d) 配列番号2、10又は11に示されるアミノ酸配列を保持した、配列番号1に示されるアミノ酸配列の部分アミノ配列からなるポリペプチド
e) a)~d)のいずれかのポリペプチドのアミノ酸配列と90%以上の同一性を有するアミノ酸配列からなり、かつ破骨細胞による骨吸収を抑制する能力及び骨芽細胞による骨形成を促進する能力のうちの少なくとも一方を有するポリペプチド
f) a)~e)のいずれかのポリペプチドと他のペプチドとが融合されてなり、かつ破骨細胞による骨吸収を抑制する能力及び骨芽細胞による骨形成を促進する能力のうちの少なくとも一方を有するポリペプチド
g) a)~f)のいずれかのポリペプチドが化学修飾されてなり、かつ破骨細胞による骨吸収を抑制する能力及び骨芽細胞による骨形成を促進する能力のうちの少なくとも一方を有する化学修飾ポリペプチド - 骨増加が望まれる疾患又は状態が、骨粗鬆症、骨ページェット病、骨減少症、副甲状腺機能亢進症、関節リウマチ、乾癬性関節炎、歯周病、家族性膨張性骨溶解症、骨巨細胞腫、多発性骨髄腫、癌関連骨転移、炎症性骨吸収、感染に伴う骨吸収、人工物による無菌性骨吸収、抜歯後の歯槽骨吸収、歯槽骨増生術後の骨吸収、又は骨折である、請求項2に記載の医薬組成物。
