Abstract
본 발명은 치주질환 치료 효용성 평가용 바이오마커 및 이를 이용한 진단키트에 관한 것으로, 더욱 상세하게는 객관적 지표를 활용하여 치주질환 경과 및 치료효과 판정이 가능한 치주질환 치료 효용성 평가용 바이오마커 및 이를 이용한 진단키트에 관한 것이다. 본 발명은 생물학적 시료 내 혈액, 치은열구액 또는 타액에 분포한 IFITM1(Interferon Induced Transmembrane Protein 1), CRIP1(Cysteine Rich Protein 1), MIF(Macrophage Migration Inhibitory Factor) 및 RPS17(Ribosomal Protein S17) 유전자가 코딩하는 단백질 수준을 측정하는 진단용 조성물로 구성하여 치주질환 진단용 바이오마커 조성물이다. 또한, 본 발명은 생물학적 시료 내 혈액, 치은열구액 또는 타액에 분포한 IFITM1(Interferon Induced Transmembrane Protein 1), CRIP1(Cysteine Rich Protein 1), MIF(Macrophage Migration Inhibitory Factor) 및 RPS17(Ribosomal Protein S17) 유전자의 mRNA 수준을 측정하는 진단용 조성물 구성하여 치주질환 효용성 평가하는 치주질환 치료 효용성 평가용 바이오마커 조성물이다.
Description
본 발명은 치주질환 치료 효용성 평가용 바이오마커 및 이를 이용한 진단키트에 관한 것으로, 더욱 상세하게는 객관적 지표를 활용하여 치주질환 경과 및 치료효과 판정이 가능한 치주질환 치료 효용성 평가용 바이오마커 및 이를 이용한 진단키트에 관한 것이다.
치은(잇몸)과 치아 사이에는 V자 모양의 틈이 있는데 이 홈(sulcus)의 잇몸 선 아래 부분을 박테리아가 공격하여 치주인대와 인접조직을 손상시키는 것이 치주질환이다. 염증이 진행되어 더 많은 조직이 손상되면서 홈이 치주낭(periodontal pocket)으로 발전하게 되며 치주낭이 깊어지면서 치주인대에 염증이 생기게 되고 골소실이 일어나게 되는 것이 치주질환이다.
최근 대사 질환, 암 등 다양한 질환의 진행에서 만성 염증의 역할이 강조되고 있다. 대표적인 만성 염증성 질환인 치주질환은 전신 질환과 관련이 있는 것으로 알려져있다. 만성 치주염으로 인한 낮은 등급의 전신 염증 신호가 조직과 기관으로 전달되는 것으로 의심되며, 이러한 상호 작용은 질병을 연결하는 주요 메커니즘이다. 그러나 만성 치주염과 전신 질환을 연결하는 주요 원인은 확인되지 않고 있다.
현재 치주질환의 진단은 환자의 자각증상, 방사선 촬영 후 치주낭 탐침기를 이용하여 치주낭 깊이, 잇몸의 퇴측 정도를 측정하여 치주염 진행 정도를 파악하거나, 세균감염 정도를 분석하는 것에 의존하고 있다.
또한, 기존 치주질환에 대한 진단과 치료 후 치주건강 개선도에 대한 평가는 주로 치주탐침(periodontal probe)을 활용하여 이루어지므로 다소 침습적이며 부분적으로는 술자의 주관이 반영된다. 종래의 경우 병인기전 또는 인자에 기반 한 객관적 평가 기법이 없어 질환 진행 예측이 제대로 되지 않으며, 치료 효용성 또한 정확하게 판정할 수 없는 문제점이 있다.
본 발명은 상기의 문제점을 해결하기 위해서 안출된 것으로서, 본 발명의 목적은 치주질환 병인기전 인자 발현에 기반하여 평가함으로써 치주질환 진단과 평가 및 예후 판정에 대한 객관적 지표를 제시하고자 한다.
또한, 본 발명의 목적은 치주질환 치료 후 치주 건강도 개선에 대한 객관적인 판정이 가능한 치주질환 발병기전 인자 중심의 바이오마커를 제공하고자 한다.
또한, 본 발명의 목적은 개인의 치주염 진행 특성에 적합한 치료계획 수립 기준 및 근거를 제시할 수 있는 치주질환 발병기전 인자 중심의 바이오마커를 제공하고자 한다.
또한, 본 발명의 목적은 유전자가 코딩하는 단백질 수준을 측정하는 진단용 조성물로 구성된 치주질환 효용성 평가용 키트를 제공하는 것이다.
또한, 본 발명의 목적은 유전자의 mRNA 수준을 측정하는 진단용 조성물로 구성된 치주질환 효용성 평가용 키트를 제공하는 것이다.
발명이 해결하고자 하는 기술적 과제들은 이상에서 언급한 기술적 과제들로 제한되지 않으며, 언급되지 않은 또 다른 기술적 과제들은 아래의 기재로부터 본 발명이 속하는 기술분야에서 통상의 지식을 가진 자에게 명확하게 이해될 수 있을 것이다.
본 발명에 따른 치주질환 진단용 바이오마커 조성물은 생물학적 시료 내 혈액, 치은열구액 또는 타액에 분포한, IFITM1(Interferon Induced Transmembrane Protein 1), CRIP1(Cysteine Rich Protein 1), MIF(Macrophage Migration Inhibitory Factor) 및 RPS17(Ribosomal Protein S17) 유전자가 코딩하는 단백질 수준을 측정하는 진단용 조성물로 구성하여 치주질환을 진단하는 것을 특징으로 한다.
본 발명에 따른 치주질환 치료 효용성 평가용 바이오마커 조성물은 생물학적 시료 내 혈액, 치은열구액 또는 타액에 분포한, IFITM1(Interferon Induced Transmembrane Protein 1), CRIP1(Cysteine Rich Protein 1), MIF(Macrophage Migration Inhibitory Factor) 및 RPS17(Ribosomal Protein S17) 유전자가 코딩하는 단백질 수준을 측정하는 진단용 조성물로 구성하여 치주질환 효용성을 평가하는 것을 특징으로 한다.
또한, 본 발명은 치주질환 진단용 바이오마커 조성물을 이용한 치주염 진단용 키트 이다.
또한, 본 발명은 치주질환 치료 효용성 평가용 바이오마커 조성물을 이용한 치주질환 치료 효용성 평가용 키트 이다.
상기 치주염 진단용 키트 및 치주질환 치료 효용성 평가용 키트는 마이크로어레이, 앱타머 칩 키트, 엘리사(ELISA, Enzyme Linked Immunosorbent Assay) 키트, 블랏팅(Blotting) 키트, 면역침전법 키트, 면역형광검사 키트, 단백질 침 키트 또는 이들의 조합으로 이루어진 군에서 선택되는 것을 특징으로 한다.
상기 과제의 해결 수단에 의해, 본 발명은 치주질환 병인기전 인자 발현에 기반하여 평가함으로써 치주질환 진단과 평가 및 예후 판정에 대한 객관적 지표를 제시할 수 있다.
또한, 본 발명은 치주질환 치료 후 치주 건강도 개선에 대한 객관적인 판정이 가능한 치주질환 발병기전 인자 중심의 바이오마커를 제공할 수 있다.
또한, 본 발명은 개인의 치주염 진행 특성에 적합한 치료계획 수립 기준 및 근거를 제시할 수 있다.

본 발명에서 사용되는 용어는 본 발명에서의 기능을 고려하면서 가능한 현재 널리 사용되는 일반적인 용어들을 선택하였으나, 이는 당 분야에 종사하는 기술자의 의도 또는 판례, 새로운 기술의 출현 등에 따라 달라질 수 있다. 따라서 본 발명에서 사용되는 용어는 단순한 용어의 명칭이 아닌, 그 용어가 가지는 의미와 본 발명의 전반에 걸친 내용을 토대로 정의되어야 한다.

본 발명은 대사물질을 포함하는 치주 질환 진단용 바이오마커 조성물 및 이를 이용한 진단용 키트를 제공한다.
이하, 본 명세서에 대하여 더욱 상세하게 설명한다.
치주 질환은 광범위한 인구에 영향을 미치는 만연한 만성 염증성 질환이다. 수십 년 동안 많은 연구자들은 구강 건강과 전신 질환 사이의 양방향 관계를 제안했다. 그러나 구강 질환이 어떻게 감수성을 증가시키고 신체에 영향을 미치는지에 관해서는 아직 정확하게 확립되지 않았다. 본 발명에서는 말초 혈액 단핵 세포를 사용하여 단일 세포 RNA 시퀀싱을 수행하고 치료 전과 후의 치주질환 환자 및 건강한 기증자의 분자 차이를 비교하였다. 특정 유전자의 발현 수준과 이들이 속한 세포 유형에 따라 염증 반응이 변경되었다. 또한, 본 발명에서는 치료적 개입을 통해 유사한 수준의 건강한 기증자로 회복되고 다양한 세포 유형에 공통적으로 나타나는 유전자를 추출했다.
본 발명에 따르면, 단일 세포 수준에서 만성 구강염증성 질환인 치주질환에서의 면역세포 유전자 발현변화 확인을 통해 전신 질환의 연관성을 확인할 수 있다. 또한, 이러한 연관성을 끊어 질병을 줄이는 치료 표적으로 활용할 수 있다.
치주 질환 진단용 바이오마커
본 발명은 정상인(대조군)의 체액 대비 치주 질환 환자의 혈액, 치은열구액 또는 타액에서 상대적으로 발현의 차이를 나타내는 대사물질을 포함하는 치주 질환 진단용 바이오마커를 제공한다.
본 발명에 사용된 용어 “정상인(대조군)”이란, 실험 결과가 제대로 도출되었는지 여부를 판단하기 위한 집단을 의미한다. 본 발명의 목적상, 상기 “정상인(대조군)”이란 치주 질환을 앓고 있지 않은 개체를 의미한다.
본 발명에 사용된 용어 "치주질환(periodontal disease)" 이란, 풍치라고도 하며, 치은(잇몸)과 치아 사이의 틈에 박테리아가 감염되어 치주인대와 인접조직을 손상시키는 질환을 의미하는데, 병증의 정도에 따라 치은염과 치주염으로 구분된다. 상기 치주질환의 발병시 염증이 진행되어 더 많은 조직이 손상되면서 치주낭(periodontal pocket)이 형성되고, 치주염이 심할수록 치주낭의 깊이가 깊어지게 되는데, 치주낭이 깊어지면서 치주인대에 염증이 생기게 되고 최종적으로는 골소실이 유발된다고 알려져 있다.
본 발명에 사용된 용어 “진단”이란, 병리 상태의 존재 또는 특징을 확인하는 것을 의미한다. 본 발명의 목적상, 상기 진단은 치주 질환의 발병 여부, 병의 진전 정도 또는 이로 인한 위험군 여부를 확인하는 것이다.
본 발명은 혈액과 같은 생물학적 시료 내에 분포한 IFITM1(Interferon Induced Transmembrane Protein 1), CRIP1(Cysteine Rich Protein 1), MIF(Macrophage Migration Inhibitory Factor) 및 RPS17(Ribosomal Protein S17) 유전자가 코딩하는 단백질 수준을 측정하는 진단용 조성물로 구성하여 치주질환 효용성 평가하는 치주질환 치료 효용성 평가용 바이오마커 조성물이다.
또한, 본 발명은 치은열구액과 같은 생물학적 시료 내에 분포한 IFITM1(Interferon Induced Transmembrane Protein 1), CRIP1(Cysteine Rich Protein 1), MIF(Macrophage Migration Inhibitory Factor) 및 RPS17(Ribosomal Protein S17) 유전자의 mRNA 수준을 측정하는 진단용 조성물 구성하여 치주질환 효용성 평가하는 치주질환 치료 효용성 평가용 바이오마커 조성물이다.
본 발명에 있어서, 상기 치주 질환은 치은염(gingivitis), 치주염(periodontal inflammation), 치은출혈(gingival bleeding), 치은퇴축(gingival retraction), 치주낭(periodontal pocket), 치조골 파괴(alveolarbone destruction) 또는 치조골 소실(alveolar bone loss)일 수 있으나, 이에 한정되는 것은 아니다.
본 발명에 있어서, 상기 치조골 소실은 치조골에서 골형성과 골흡수의 균형이 깨어져 골흡수가 골형성을 초과하여 골량이 한계 이하로 감소하면서 나타나는 치주 질환의 일종으로, 치조골 흡수(alveolar bone resorption)가 나타날 수 있다.
실험예
1) 치주염 환자에서 PBMC의 단일 세포 전사
치주염 환자에서 얻은 말초 혈액 단핵세포(PBMC)의 단일 세포 전사 환경 전신에서 치주염의 면역학적 특징과 메커니즘을 프로파일링 하기 위해, 본 발명에서는, 도 1에 나타난 바와 같이, 4명의 건강한 기증자로부터 말초 혈액 단핵세포를 그리고 4명의 치주질환자로부터 4 쌍의 치료 전 후 말초 혈액 단핵세포(PBMC)를 수집하였다.
액적 기반 단일세포 RNA 시퀀싱(scRNA-seq)을 수행했다. 도 2에 나타난 바와 같이, 저품질 셀을 필터링하고 데이터를 통합 한 후 참가자 당 평균 9,268 개의 셀이 있는 111,213 개의 셀을 획득하였다. 유전자 별 세포 발현 매트릭스를 사용하여 균일한 다양체 근사 및 투영 (UMAP) 및 26개의 세포 집단을 캡처 한 그래프 기반 클러스터링으로 차원 감소를 수행하였다.
먼저 각 집단에 대한 마커 유전자를 찾은 다음 상기 유전자를 활용하여 참조 전사체 데이터 세트를 활용하여 세포의 기원을 확인하였다. 각 셀의 원점을 추정하여 클러스터 단위에서 확률이 가장 높은 셀 유형에 할당했다. 도 3 및 도 4에 나타난 바와 같이, 면역 반응에 중요한 13가지 주요 세포 유형을 발견하고 알려진 마커를 사용하여 기원을 확인했다 : B(MS4A1), naive B(MS4A1 and TCL1A), CD4+ T(CD4, and CD3), naive CD4+ T(SELL, CCR7, CD4, and CD3), CD8+ T(CD8T, and CD3), naive CD8+ T(SELL, CCR7, CD8T, and CD3), gamma delta T cell, gdT(TRGC1, and CD3), MAIT(SLC4A10, and CD3), NK(KLRF1), plasmacytoid Dendritic Cell, pDC(IL3RA), myeloid Dendritic Cell, mDC(CD1C), monocyte(LYZ,CD4), progenitor(ITGA2B). 따라서 PBMC의 세포 집단은 명확하게 식별되었으며, 도 5에 나타난 바와 같이, 각 참가자의 세포집단은 이러한 클러스터로 구성되었다. PBMC 전체에서 각 세포가 차지하는 비율은 T 세포(60 ~ 73%), B 세포(4 ~ 15%), NK 세포(5 ~ 23%), 단핵구(5 ~ 24%) 및 수지상 세포(1 ~ 2%)로 질병 유무와 치료에 상관없이 정상 분포를 가진 PBMC로 구성된다. 도 6에 나타난 바와 같이, 대부분의 샘플은 세 그룹 사이에 세포 조성의 유의 한 차이가 없었지만 CD8+ T 세포는 치료 전 치주질환자 그룹에서 약간의 감소를 보였다 (p- 값 0.063).
차별적으로 발현 된 유전자(이하, DEG) 분석을 수행하고 건강한 기증자와 치료 전 환자 사이 및 치료 전후 환자 사이에서 공통 유전자를 추출했다.
2) 치주 상태에 따른 B세포의 특성 변화
B 세포를 전환되지 않은 메모리 B세포(non-switched MB), 전환된 메모리 B 세포(switched MB), 비활성 B 세포(exhausted B cells) 및 형질모세포로 분리했다. 도 7 및 도 8에 나타난 바와 같이, 비 전환된 MB(non-switched MB), 전환된 메모리 B 세포(switched MB)는 모두 CD19 및 CD27을 발현하는 것으로, 전환된 메모리 B 세포(switched MB)에서는 낮은 발현을 나타내는 IGHD 및 IGHM을 사용하여 구별할 수 있으며, 형질모세포는 CD38을 사용하여 확인할 수 있다.
도 9에 나타난 바와 같이, DEG 분석에서는 치료 전 치주질환자의 전환되지 않은 메모리 B세포와 전환된 메모리 B 세포에서 뚜렷하게 상향 및 하향 조절되는 유전자가 있다. 6개의 유전자(IGLL5, CRIP1, RPS17, MIF, RCSD1 및 GABARAPL2)가 증가했고, 2개의 유전자(DUSP2, RPS26)가 치료 전 환자의 비 전환 B 세포(non-switched MB)에서 감소했다. 또한 전환 된 B 세포(switched MB)에서 3 개의 유전자(CRIP1, MIF 및 RPS17)가 상향 조절되었고 RPS10은 하향 조절되었다. CD4+T 세포에서도 IGLL5와 RPS26은 치료 전 환자에서 각각 매우 높고 낮은 발현을 나타냈다. 또한 치은 조직에서 IGLL5의 발현 수준이 높은 것으로 보고 되었다. 그 기능은 거의 알려지지 않았지만 IGLL5는 B 세포 개발에 관여하는 IGLL1과 상동하다. CD4T 하위 집합에서 감소 된 RPS26은 전환되지 않은 B 세포(non-switched MB)에서도 가장 큰 감소를 나타냈다. 도 10에 나타난 치료 전 치주질환자에서 상향 조절된 IGLL5, MIF 및 CRIP1을 포함한 유전자의 경우, 생물학적 기능에 관해서는 흥미롭게도 전환되지 않은 MB에서 B 세포 활성화와 관련이 있다.
3) 치주 상태에 따른 CD8+ T 세포의 특성 변화
도 11 및 도 12에 나타난 바와 같이, SELLhi / CCR7hi central memoryT (TCM), SELL(CD62L)lo / CCR7lo effector memory T(TEM) cells and SELL(CD62L)lo / CCR7lo / IL7Rlo terminal effector T(TTE)를 분류하여 CD8+ T 세포를 비교했다. 도 13에 나타난 바와 같이, 치료 전 치주질환자의 TCM 세포에는 3 개의 상향 조절 된 유전자(IFITM1, CRIP1 및 MIF)와 3 개의 하향 조절 된 유전자(GNLY, FOS 및 RPS10)가 있다. 또한 TTE 세포의 4 개 유전자(CCL3, CRIP1, IFITM1 및 MIF)와 2 개 유전자(S100B, ZNF683)는 치료 전 군에서만 각각 높고 낮은 발현을 보였다. 특히 GNLY와 S100B는 다른 그룹에 비해 치료 전 그룹에서 가장 큰 차이를 보였다. GNLY는 NK 세포와 활성화 된 세포 독성 T 세포에 의해 생성되는 최초의 인정 된 림프구 유래 알람이다. 항원 제시 세포의 모집을 촉진하고 염증성 질환에서 내인성 위험 신호를 전달한다. GNLY 발현의 증가는 자가 면역 질환 및 감염에서 보고되었으며, 감소는 심각한 면역 결핍에서 발생했다. 치주염 환자에서 GNLY의 감소 된 발현 수준은 다른 세포에 비해 TCM에서 가장 낮았다.
S100 단백질 군의 유전자 인 S100B는 기능적으로 뇌 또는 폐 질환과 관련이 있다. 또한, 세포아포프토시스, 증식 및 염증을 포함하는 조절을 포함하는 광범위한 세포 내 및 세포 외 기능을 가지고 있지만 CD8+ T 세포에는 명시되어 있지 않다. 생물학적 기능을 조사했을 때, 치주염 환자의 TTE 세포는 화학 주성, 세포 이동 및 사이토카인 분비와 같은 능력이 향상되어 있다. 특히, 도 14에 나타난 바와 같이, 본 발명에서는 LPS가 표적 세포의 수용체에 결합함으로써 개시되는 분자 신호 인 LPS(lipopolysaccharide) 매개 신호 전달 경로를 발견했다. 치주 질환의 주요 병원체 인 Porphyromonas gingivalis(P. gingivalis)는 LPS를 보유한 그람 음성균으로 전신 염증과 질병을 유발하는 LPS에 대한 많은 연구가 진행되고 있다.
4) 치주 상태에 따른 CD4+ T 세포의 특성 변화
도 15에 나타난 바와 같이, 우리는 CD4+ T 세포를 하위 유형인, T-helper 1 (Th1), T-helper 17(Th17), follicular helper T(Tfh) and regulatory T (Treg) 로 나누었다. 도 16에 나타난 바와 같이, STAT1 및 STAT4는 Th1 세포를 CCR6는 Tfh 세포를 MAF는 Th17 세포를, IL2RA 및 FOXP3는 Treg 세포를 식별하는 데 사용되었다. 세포 내 박테리아 및 바이러스에 대한 세포 매개 면역에 관여하는 Th1 세포는 치료 전 치주질환자에서 5 개의 상향 조절 된 유전자 (CRIP1, IFITM1, CCR10, RPS17 및 MIF)와 1 개의 하향 조절 된 유전자 (RPS26)를 가지고 있다. 도 17에 나타난 바와 같이, CRIP1 및 IFITM1은 건강한 그룹과 치료 후 그룹보다 약 2 배 높게 발현되었다. 특정 진균 및 세포 외 세균에 대한 세포 매개 염증 과정이 요구되는 Th17 세포의 경우, 3 개의 유전자(CCR7, CRIMAP4, CRIP1)가 치료 전 치주질환자에서 높게 관찰되었으며 6 개의 유전자(GZMK, TRGC2, GNLY, AC020916.1)는 건강한 사람에서 높게 발현되었으나 치주질환자의 경우 치료 전후 변화는 없었다. 이들 유전자 중 특히 GZMK, TRGC2, GNLY의 전사체는 치주질환자에서 크게 감소하였다. GZMK는 세포 독성 유전자로 널리 알려져 있으며 CD8+ T 세포 및 세포 독성 CD4+ T 세포와 관련이 있다. TRGC2는 면역 수용체 티로신 기반 활성화 모티프(ITAM)를 유발하고 궁극적으로 T 세포 확장 및 항원에 결합하여 효과기 세포로의 분화를 유발한다. 병원균 제거 및 조직 복구를 유도하는 것으로 보고되었지만 CD4+ T 세포에 대해서는 정보가 부족하다. GNLY는 세포 독성이 있는 살해 림프구에 의해 방출되는 단백질로 미세 안검하수증을 유발한다. B 세포의 성숙을 조절하는 것으로 알려진 Tfh 세포의 경우 치료 전 치주질환자에서 7 개의 상향 조절 된 유전자(IFITM1, CRIP1, RPS17, MIF, GNLY, RCSD1, MT-ND3)와 3 개의 하향 조절 된 유전자 (RPS26, RPS10, RPL36A)가 있다. IFITM1과 CRIP1은 증가했고 RPS26은 두 배 이상 감소했다. 면역 항상성 유지 및 자가 면역 질환 예방에 중요한 Treg 세포는 치료전 치주질환자에서 2 개의 고도로 발현 된 유전자 (CRIP1 및 IFITM1)를 가지고 있으며 하향 조절 된 유전자는 없다. 생물학적 기능은 도 18에 나타나 있다.
5) 치주 상태에 따른 단핵구의 특성 변화
단핵구에는 표현형 수용체 CD14 및 CD16 (FCGR3A)에 따라, 도 19에 나타난 바와 같이, 고전적 단핵구, 비 고전적 단핵구 및 중간 단핵구라고하는 세 가지 하위 유형이 있다. 도 20에 나타난 바와 같이, 고전적 단핵구는 CD14++ CD16-을 발현하지만, 비 고전적 단핵구 및 중간 단핵구는 CD14+ CD16++ 및 CD14++ CD16+를 각각 발현한다.
도 21에 나타난 바와 같이, 고전적인 단핵구에서 치료 전 환자는 다른 피험자보다 4 개의 고도로 발현 된 유전자(AC007952, CRIP1, VNN2 및 CD93)와 11 개의 낮은 발현 된 유전자(LDLR, GPR183, PF4, CD69, ATF3, PTX3, HLA-DPB1, HLA-DPA1, IL1B, CTNNB1 및 ZFP36)가 있다. 치료 전 치주질환자의 비 고전적 단핵구에서 5 개의 유전자(VMO1, IGKC, CRIP1, HSPA1B 및 SCGB3A1)가 상향 조절되었고 13 개의 유전자(LGALS2, APOBEC3A, CCL4L2, LYPD2, CDKN1A, NFKBIA, AC020916, DUSP2, HLA-DRB5, CD83, ZFP36, HSPB1 및 EPSTI1)은 하향 조절되었다. 중간 단핵구는 13 개 유전자(AC007952, ATP2B1-AS1, CRIP1, NUP214, RBMS1, RPS17, TALDO1, CAST, LYZ, RPL17, CCNI, NCL 및 CSDE1)의 치료 전 치주질환자에서 상향 조절만 있었으며 발현 감소 유전자는 없었다. 이 중 AC007952, VMO1, IGKC가 가장 많이 증가한 반면 LDLR과 LGALS2가 가장 많이 감소했다.
lncRNA 인 AC007952는 고전적 단핵구와 중간 단핵구 모두에서 발견되었지만 염증 관련 연구는 없었다. VMO1은 CD16- 단핵구보다 CD16+ 단핵구에서 특징적으로 발현되는 유전자이다. 치주염 환자에서 3 배 더 높은 것으로 관찰되지만 그 기능은 잘 알려져 있지 않다. IGKC의 단핵구 관련 기능은 확인되지 않았지만 종양 진단 및 화학 요법에서 강력하고 유명한 마커이다. LDLR은 죽상 동맥 경화증 또는 암에서 콜레스테롤 항상성과 염증 조절제를 조율하는 것과 관련이 있다. LGALS2 또는 갈렉틴 2는 단핵구 이동을 약화시키면서 전 염증성 사이토카인을 촉진하여 혈관 항상성을 유지하는 단핵구를 조절하는 것으로 알려져 있다. 관련 유전자 온톨로지 용어는 도 22에 나타나 있다.
6) 치주 상태에 따른 Naive CD4 + T, Naive CD8 + T, Naive B, gdT, NK, MAIT, mDC의 특성 변화
도 23에 나타난 바와 같이, 면역 반응에 중요한 13가지 주요 세포 중 pDC와 전구 세포를 제외한 거의 모든 세포에서 치료 전 치주질환자에서 유전자가 뚜렷하게 증가 또는 감소하는 것을 발견했다. naive B 세포는 치주질환자에서 5 개 유전자(GNLY, CRIP1, IGLL4, MIF 및 RPS17)의 발현이 증가하였으며 2 개 유전자(RPS26 및 HLA-DRB5)는 감소했다. naive B 세포에서 증가 된 유전자 중, 특히 GNLY와 CRIP1이 훨씬 더 높은 발현 수준을 나타냈다. 치료 전 치주질환자에서 naive CD4+ T 세포는 5 개의 상향 조절 된 유전자(IFITM1, MIF, CRIP1, GNLY 및 RPS17)과 1 개의 하향 조절 된 유전자(RPS26)를 보유한 반면, naive CD8+ T 세포는 5 개의 유전자(IFITM1, CRIP1, MIF, RPS17 및 TLN1) 발현이 증가되었고 4 개의 유전자(RPS10, TNFAIP3, CD69 및 CCL5) 발현이 감소하였다. 치료 전 치주질환자에서 gdT는 8 개의 상승 된 유전자(IFITM1, CRIP1, ALOX5AP, MT2A, TLN1, FGFBP2, GIMAP4 및 CD247)와 7 개의 감소 된 유전자(TNFAIP3, AC020916.1, HLA-DRA, FOS, HOPX, NFKBIA 및 CD69)를 보였다. 치료 전 치주질환자의 NK 세포에서는 3 개의 유전자(CCL3L1, CRIP1, IFITM1)가 높게 발현되었고 2 개의 유전자(KLRC2, NUSAP1)가 낮은 발현을 보였다. MAIT 세포에는 4 개의 상향 조절 된 유전자(IFITM1, CRIP1, RPS17 및 MIF)와 1 개의 하향 조절 된 유전자(HOPX)가있는 반면, mDC에는 높은 8 개의 유전자(CRIP1, S100A12, CD14, VACN, MIF, TNFRSF1B, RPS17 및 FTL) 및 낮은 5 개 유전자(CCL5, PMAIP1, IL18, RPS10 및 HLA-DRB5)를 포함한다. 각 세포와 연계된 생물학적 기능은 도 24에 설명되어 있다.
여러 가지 연구에서 치주질환은 전신염증 반응을 통해 다양한 질환의 발생과 진행에 영향을 미치는 것으로 보고되고 있다. 그러나 이와 같은 상호관계를 연결짓는 구체적인 인자는 아직 밝혀지지 않았으며 연구할 필요가 있다. 본 발명에서 PBMC에 scRNA-seq를 적용하여 치주질환 치료 후 및 건강한 상태를 비교하여 세포 수준에서 치주질환의 면역인자 변화 특징 및 관련 생물학적 기능을 설명한다. 본 발명은 분자 이질성과 특징적인 변화를 밝혀냈을 뿐만 아니라 큰 변화를 보이는 유전자가 관련 세포 유형에 해당하는 염증 반응에 중요한 역할을 하는 것을 명확히 밝혀냈다. 또한, 앞서 기재한 바와 같이 다른 질병과 관련된 기능을 입증하였다.
한편, 본 발명에서는 다양한 세포 유형에 걸쳐 중첩되게 관찰되는 4개의 유전자에 집중했다. 해당 유전자가 세포 유형에 관계없이 동일한 양상을 보인다는 것이 흥미롭다. 각 세포 유형에는 고유의 세포가 아닌 것을 구분하기 위한 고유한 특성과 기계적 시스템이 있다. 그럼에도 불구하고 치주염 환자에서의 세포 유형 비특이적 변화는 치주염이 수많은 세포에 대한 면역 반응을 일으키는 동시에 상당한 영향을 미칠 것을 암시한다. CRIP1, IFITM1, MIF 및 RPS17이 여러 세포 유형에 걸쳐 증가했음을 고려할 때, 해당 유전자는 전신 염증의 효과적인 표적으로 잠재적 후보로서의 가치가 있다.
CRIP1의 상승은 DEG가 존재하는 모든 세포 유형에서 관찰되었다. CRIP1은 LIM 계열 도메인으로 구성된 물질로 단백질-단백질 상호 작용을 통해 유전자 조절, 세포 운명 결정 및 종양 형성에 직접적으로 영향을 미친다. 또한 결장 직장암, 유방암, 골육종, 위암 등 다양한 암 유형에서 변화가 관찰되고 있다. 갑상선 암종의 경우 CRIP1이 과발현되었고, CRIP1을 침묵 시키면 세포의 증식, 이동 및 침입이 억제되었다. CRIP1에 대한 연구는 원래 장(intestine)에 초점을 맞추었지만 PBMC에서도 동일하게 관찰되었다. 특히, 쥐의 복막 대식세포, PBMC, 비장 및 장에서 LPS 자극 후 CRIP1 mRNA 수준이 증가 하였다. CRIP1 과발현 된 트랜스 제닉 마우스는 LPS 처리 후 높은 농도의 IL-6 및 IL-10을 생성했지만, 낮은 농도의 TNF- 및 IFN-을 생성하여 CRIP1이 사이토카인 생산의 불균형에 영향을 미친다는 것을 알 수 있다. CRIP1 과발현 트랜스 제닉 마우스는 체중 감소와 설사가 심했다.
MIF는 Th1, Tfh, TCM, TTE, 전환되지 않은 MB, 전환 된 MB, naive CD4 + T, naive CD8+ T, naive B, MAIT 세포 및 mDC에서 상승했다. MIF는 선천 면역과 적응 면역을 조절하는 인자로서 숙주 염증 반응의 기본 구성 요소이다. MIF는 치주염의 경우 P. gingivalis를 보유한 LPS와 같은 독소 함유 박테리아에 의해 유도된다. 조직이나 전신 순환으로 방출 된 후 대식세포와 T세포를 활성화시켜 염증을 일으키는 사이토카인으로 숙주가 즉시 반응하여 빠르게 방어 할 수 있도록 한다. 천식, 관절염, 염증성 장 질환, 아토피 성 피부염 및 죽상 경화증을 포함한 여러 급성 및 만성 염증성 질환 상태에서 LPS가 있을 경우 고용량의 순환 MIF 는 내독소 혈증을 치명적인 수준으로 악화시키는 데 기여하는 것으로 보고되었다.
반면, IFITM1은 Th1, Tfh, Treg, TCM, TTE, naive CD4+ T, naive CD8+ T, gdT, NK 및 MAIT 세포에서 관찰되었다. IFITM1은 인터페론 자극 유전자이며 적응 면역에 직접 관여하고 세포 사멸 및 세포주기 제어에 역할을 하는 것으로 설명된다. 특히, IFITM1의 증가는 대장 암, 두경부, 위장관, 염증성 유방암, 궤양성 대장염 및 크론 병에서 전반적인 예후가 악화되는 종양 진행 및 침습성과 관련이 있는 것으로보고된다. 염증 및 전신 질환의 유도와 관련이 높지만 치주 염증 반응에서 IFITM1에 대한 연구에 대해서는 알려진 바가 없다.
RPS17의 경우 Th1, Tfh, 전환되지 않은 MB, 전환 된 MB, 중간 단핵구, naive CD4+ T, naive CD8+ T, naive B, MAIT 세포 및 mDC에서 높게 발현되었다. 리보솜 단백질로서 그 돌연변이는 다이아몬드-블랙판 빈혈의 바이오 마커로 잘 알려져 있지만, 기능에 대한 정보는 매우 적다. 리보솜 단백질은 종양 형성, 발달 및 면역 신호 전달에서 미세하게 조정된다. 예를 들어, RPS3와 같은 일부 리보솜 단백질은 표적 유전자의 전사 발현을 선택적으로 조절한다. 리보솜 단백질은 면역 신호를 강화하는 숙주 면역 반응에서 다양한 역할을 하지만 인간 종양에서 mRNA 또는 단백질 수준에서 상향 조절되어 독립적인 발암 능력을 가지고 있음이 많은 연구에서 입증되었다. 이러한 양면성 때문에 리보솜 단백질을 조사하는 것이 필수적이다.
결론적으로, 본 발명은 치주질환에서 관찰되는 유전자 발현의 특징적인 변화를 세포 수준에서 설명하고 치주염이 전신 질환 진행에 영향을 미친다는 증거를 제시한다. 최근까지 많은 질병과의 인과 관계가 반복적으로 언급 되었지만 이 관계에 대한 연구는 없다. 참가자가 다른 질병을 앓지 않았음을 감안할 때 치주 변화는 PBMC에 영향을 미치고 변화된 PBMC 순환은 전신에 영향을 미치며 전신 질환 진행에 기여할 수 있다. 이를 위해 조사가 필요한 4 가지 주요 조절 유전자를 제안한다.
추가 실험방법
1) 방법 세부 사항
10X 게놈을 사용한 샘플 준비 및 scRNA-seq [대상 수집, 윤리 성명서, 임상 정보, PBMC(Peripehral blood mononuclear cell) 수집, 단일 세포 현탁액 준비]를 준비한다.
2) 정량화 및 통계 분석
- scRNA-seq 데이터 전처리 (정렬, 바코드 할당 및 UMI 계수, 품질 관리)
FASTQ 파일은 정렬, 필터링, 바코드 계수 및 UMI 계수를 수행하는 Cell Ranger (v3.1.0)를 사용하여 세포 특성 바코드 매트릭스로 변환한다. 파이프 라인은 기본 인수 (10X Genomics)로 실행한다. 저품질 세포를 제거하기 위해 UMI가 500 개 미만이거나 UMI가 20,000 개 이상이고 미토콘드리아 유전자가 20 % 이상인 세포를 걸러 낸다. 또한 유전자가 250 개 미만이거나 유전자가 5,000 개 이상인 세포와 복잡성이 80 % 미만인 덜 복잡한 세포는 제거한다. 또한 세포의 0.1 % 이상에서 발현되는 유전자는 남긴다. 그럼에도 불구하고 일부 샘플에는 다량의 세포 (최대 12,177 개 세포)가 있었기 때문에 Scrublet을 사용하여 가능한 이중선을 추정하고 3.5 %의 세포 (최대 11,852 개 세포)는 제거한다.
- scRNA-seq 데이터 분석 (클러스터링, DEG, UMAP, 주석)
데이터 통합, 확장, 클러스터링 및 시각화를 위해 Seurat (v3.2.2) R 패키지를 실행한다. 나머지 카운트 데이터는 셀룰러 총 읽기 카운트에 대한 정규화 된 음이항 회귀를 기반으로 SCTransform을 사용하여 정규한다. 각 샘플에 대한 정규화 후 FindIntegrationAnchors 및 IntegrateData 함수를 사용하여 12 개 샘플 중 가장 큰 크기 데이터인 참조 데이터와 통합한다.
그런 다음 ScaleData 및 RunPCA 기능을 통해 스케일링 및 주성분 분석을 수행하고 처음 30 대의 PC를 RunUMAP 및 FindNeighbors 기능을 사용하여 UMAP 차원 축소 및 공유 SNN(가장 가까운 이웃 그래프) 구성에 활용한다. 이후, 0.6의 설정 해상도를 가진 커뮤니티를 탐지하기 위한 Louvain 방법의 그래프 기반 모듈성 최적화 알고리즘을 사용하여 세포 클러스터를 구별하고, 27 개의 클러스터를 구별한다. 모든 마커 찾기 기능을 사용하여 세포 동일성 마커를 조사하고, 로그-폴드 변경 임계 값>0.5 및 거짓 발견율 (FDR)<0.01을 갖는 유전자는 유의한 차별적으로 발현 된 유전자 (DEG)로 간주한다.
각 클러스터에 면역 세포 유형에 주석을 달기 위해 이러한 세포 식별 마커를 SingleR (v3.12) R 패키지에 제공한다. SingleR은 단일 셀을 고도로 일치하는 셀 유형에 할당하고 주석을 달고 가장 가능성이 높은 셀 유형에 클러스터를 결합한다. 동일한 클러스터에 있는 대부분의 세포는 일반적으로 70 % 이상의 일관된 세포 유형으로 제안되었으며, 낮은 일관성을 가진 클러스터는 세포 특이적 시그니처를 나타내는 여러 유전자를 사용하여 주석을 달았다.

주석을 명확히 한 후 B 세포, CD4T 세포 (helper T cells), CD8T 세포 (cytotoxic T cells) 및 단핵구를 추출하고 정규화, 스케일링, 차원 축소, 클러스터링 및 주석 단계를 반복하여 각 세포를 계층적 클러스터로 하위 클러스터링한다. ‘건강한 공여자 vs 치료 전 환자’ 또는 ‘치료 전 vs 치료 후 환자’ 간의 DEG를 찾은 경우, edgeR 및 유연한 제로 팽창 음이항 기반 원한 변동 추출 (ZINB-WaVE)을 수행하여 제로 인플레이션 및 과대 분산을 설명할 수 있다.

본 발명에 있어서, 상기 특이적으로 결합하는 분자는 항체, 기질, 리간드 단백질, 펩타이드, 핵산, 화학물질, 고분자 또는 보조인자(cofactor) 등 일 수 있으며, 구체적으로는 각 마커를 인식하는 모노클로날 또는 폴리클로날 항체, 각 마커에 상호작용하는 특정 단백질, 펩타이드, 핵산, 고분자 또는 화학물질, 또는 각 마커와 화학반응을 일으켜 특정 화학 물질로 변환되도록 가능한 화학물질 또는 화합물, 또한 추가적으로 항체, 단백질, 펩타이드, 핵산, 고분자 또는 화학물질에 수식(modification) 가능한 나노입자, 형광 염료(fluorescent dyes) 등의 측정 시약을 포함할 수 있다.
본 발명에 있어서, 상기 진단용 키트는 검체로서 혈액, 치은열구액 또는 타액을 사용하는 것이 바람직하다.
본 발명은 혈액, 치은열구액 또는 타액과 같은 생물학적 시료 내에 분포한 IFITM1(Interferon Induced Transmembrane Protein 1), CRIP1(Cysteine Rich Protein 1), MIF(Macrophage Migration Inhibitory Factor) 및 RPS17(Ribosomal Protein S17) 유전자가 코딩하는 단백질 수준을 측정하는 진단용 조성물로 구성하여 치주질환 효용성 평가하는 치주질환 치료 효용성 평가용 바이오마커 조성물이다.
또한, 본 발명은 혈액, 치은열구액 또는 타액과 같은 생물학적 시료 내에 분포한 IFITM1(Interferon Induced Transmembrane Protein 1), CRIP1(Cysteine Rich Protein 1), MIF(Macrophage Migration Inhibitory Factor) 및 RPS17(Ribosomal Protein S17) 유전자의 mRNA 수준을 측정하는 진단용 조성물 구성하여 치주질환 진단용 바이오마커 조성물이다.
본 발명의 이점 및 특징, 그리고 그것들을 달성하는 방법은 상세하게 후술되어 있는 실시 예들을 참조하면 명확해 질 것이다. 그러나, 본 발명은 이하에서 개시되는 실시 예들에 한정되는 것이 아니라 서로 다른 다양한 형태로 구현될 수 있으며, 단지 실시 예들은 본 발명의 개시가 완전하도록 하고, 본 발명이 속하는 기술 분야에서 통상의 지식을 가진 자에게 발명의 범주를 완전하게 알려 주기 위해 제공되는 것이며, 본 발명은 청구항의 범주에 의해 정의될 뿐이다.
이하, 본 발명을 구체적으로 설명하기 위해 실시 예를 들어 상세하게 설명하기로 한다. 그러나 하기의 실시 예는 본 발명을 보다 쉽게 이해하기 위하여 제공되는 것일 뿐, 하기 실시 예에 의해 본 발명의 내용이 한정되는 것은 아니다.
상기 과제의 해결 수단에 의해, 본 발명은 치주질환 병인기전 인자 발현에 기반하여 평가함으로써 치주질환 진단과 평가 및 예후 판정에 대한 객관적 지표를 제시할 수 있다.
또한, 본 발명은 치주질환 치료 후 치주 건강도 개선에 대한 객관적인 판정이 가능한 치주질환 발병기전 인자 중심의 바이오마커를 제공할 수 있다.
또한, 본 발명은 개인의 치주염 진행 특성에 적합한 치료계획 수립 기준 및 근거를 제시할 수 있다.
또한, 본 발명의 목적은 유전자가 코딩하는 단백질 수준을 측정하는 진단용 조성물로 구성된 치주질환 효용성 평가용 키트를 제공할 수 있다.
또한, 본 발명의 목적은 유전자의 mRNA 수준을 측정하는 진단용 조성물로 구성된 치주질환 효용성 평가용 키트를 제공할 수 있다.
이와 같이, 상술한 본 발명의 기술적 구성은 본 발명이 속하는 기술분야의 당업자가 본 발명의 그 기술적 사상이나 필수적 특징을 변경하지 않고서 다른 구체적인 형태로 실시될 수 있다는 것을 이해할 수 있을 것이다.
그러므로 이상에서 기술한 실시예들은 모든 면에서 예시적인 것이며 한정적인 것이 아닌 것으로서 이해되어야 하고, 본 발명의 범위는 상기 상세한 설명보다는 후술하는 특허청구범위에 의하여 나타나며, 특허청구범위의 의미 및 범위 그리고 그 등가 개념으로부터 도출되는 모든 변경 또는 변형된 형태가 본 발명의 범위에 포함되는 것으로 해석되어야 한다.
Claims (10)
- 생물학적 시료 내 혈액, 치은열구액 또는 타액에 분포한,IFITM1(Interferon Induced Transmembrane Protein 1), CRIP1(Cysteine Rich Protein 1), MIF(Macrophage Migration Inhibitory Factor) 및 RPS17(Ribosomal Protein S17) 유전자 중 선택된 어느 하나 이상의 유전자를 포함하는 것을 특징으로 하는 치주질환 진단용 바이오마커 조성물.
- 제 1항의 바이오마커 조성물의 mRNA 또는 이의 단백질 발현 수준을 측정하는 제제를 포함하는 것을 특징으로 하는 치주질환 진단용 바이오마커 조성물.
- 생물학적 시료 내 혈액, 치은열구액 또는 타액에 분포한,IFITM1(Interferon Induced Transmembrane Protein 1), CRIP1(Cysteine Rich Protein 1), MIF(Macrophage Migration Inhibitory Factor) 및 RPS17(Ribosomal Protein S17) 유전자 중 선택된 어느 하나 이상의 유전자를 포함하는 것을 특징으로 하는 치주질환 치료 효용성 평가용 바이오마커 조성물.
- 제 3항의 바이오마커 조성물의 mRNA 또는 이의 단백 발현 수준을 측정하는 제제를 포함하는 것을 특징으로 하는 치주질환 치료 효용성 평가용 바이오마커 조성물.
- 제 1항 내지 제 2항 중 어느 한 항의 조성물을 포함하는 치주질환 진단용 키트.
- 제 3항 내지 제 4항 중 어느 한 항의 조성물을 포함하는 치주질환 치료 효용성 평가용 키트.
- 제 5항에 있어서,상기 치주염 진단용 키트는,마이크로어레이, 앱타머 칩 키트, 엘리사(ELISA, Enzyme Linked Immunosorbent Assay) 키트, 블랏팅(Blotting) 키트, 면역침전법 키트, 면역형광검사 키트, 단백질 침 키트 또는 이들의 조합으로 이루어진 군에서 선택되는 것을 특징으로 하는 치주질환 진단용 키트.
- 제 6항에 있어서,상기 치주염 진단용 키트는,마이크로어레이, 앱타머 칩 키트, 엘리사(ELISA, Enzyme Linked Immunosorbent Assay) 키트, 블랏팅(Blotting) 키트, 면역침전법 키트, 면역형광검사 키트, 단백질 침 키트 또는 이들의 조합으로 이루어진 군에서 선택되는 것을 특징으로 하는 치주질환 치료 효용성 평가용 키트.
- 생물학적 시료 내 혈액, 치은열구액 또는 타액에 분포한,IFITM1(Interferon Induced Transmembrane Protein 1), CRIP1(Cysteine Rich Protein 1), MIF(Macrophage Migration Inhibitory Factor) 및 RPS17(Ribosomal Protein S17) 유전자 중 선택된 어느 하나 이상의 유전자의 mRNA 또는 상기 유전자가 코딩하는 단백질의 발현 수준을 측정 하는 단계; 및상기 측정된 발현 수준을 정상 대조군의 발현 수준과 비교하는 단계를 포함하는 치주질환 진단을 위한 정보제공방법.
- 제 9항에 있어서,mRNA 또는 상기 유전자가 코딩하는 단백질의 발현 수준을 측정 하는 단계에서 생물학적 시료 내 생물학적 시료 내 혈액 또는 치은열구액에 존재하는 말초 혈액 단핵세포(PBMC)를 수집하여 B(MS4A1), naive B(MS4A1, and TCL1A), CD4+ T(CD4, and CD3), naive CD4+ T(SELL, CCR7, CD4, and CD3), CD8+ T(CD8T, and CD3), naive CD8+ T(SELL, CCR7, CD8T, and CD3), gamma delta T cell, gdT(TRGC1, and CD3), MAIT(SLC4A10, and CD3), NK(KLRF1), plasmacytoid Dendritic Cell, pDC(IL3RA), myeloid Dendritic Cell, mDC(CD1C), monocyte(LYZ,CD4), progenitor(ITGA2B) 으로 이루어진 군으로부터 선택되는 어느 하나 이상의 유전자의 mRNA 또는 상기 유전자가 코딩하는 단백질의 발현 수준을 더 측정하는, 치주질환 진단을 위한 정보제공방법.
