以下、本発明の実施の形態について、図面を参照しながら説明する。
本発明の実施の形態による施設情報提供方法、施設情報提供サーバ、施設情報提供プログラム及び施設情報提供システムは、第1の施設(現在入退院等している施設)から、第2の施設(退院先・転院先の介護施設等)へ対象者(高齢者患者等)を移転させる調整業務を支援するためのものである。
図1に示されるように、本実施の形態による発明は、システムを提供する管理サーバ(施設情報提供サーバ)1と、病院端末(ユーザ端末)2と、介護施設端末(施設端末)3とを含んでおり、これらはネットワーク4を介して互いに接続される。ネットワーク4は、有線または無線を問わず、インターネットやLAN(Local Area Network)などの任意の種類の通信網であってかまわない。
図2に示されるように、本実施の形態による管理サーバ1、病院端末2、介護施設端末3は、以下のような一般的なハードウェア構成を有する。なお、構成は一例であり、これ以外の構成を有していてもよいし、複数の端末が協働することによって一のハードウェアが論理的に構成されていてもよい。
図3を参照して、管理サーバ1の機能を説明する。管理サーバ1は、病院端末2及び介護施設端末3のそれぞれに対して、利用者のアカウントを登録、管理、更新することにより、本システムを利用可能に提供する。図示されるように、管理サーバ1は、記憶部、条件入力部、施設情報抽出部、施設情報提供部、施設情報選択部、リクエスト関連付け部、応答受付部、応答施設情報提供部、トークルーム生成部、ステータス出力部を備えている。

- ユーザ端末から条件情報の入力を受け付ける条件入力ステップと、
入力された前記条件情報に基づいて、一以上の施設情報を格納する記憶手段から一以上の施設情報を抽出する施設情報抽出ステップと、
抽出された前記施設情報を前記ユーザ端末に提供する施設情報提供ステップと、
提供された前記施設情報の選択操作を前記ユーザ端末から受け付ける施設情報選択ステップと、
選択操作を受けた前記施設情報に対して、対象者の受け入れに関するリクエストを関連付けるリクエスト関連付けステップと、
関連付けられた前記リクエストに対して前記施設端末から応答を受け付ける応答受付ステップと、
応答を受け付けた前記施設端末に関する前記施設情報を前記ユーザ端末に提供する応答施設情報提供ステップと、を含む、
施設情報提供方法。 - 請求項1に記載の施設情報提供方法であって、
前記施設情報抽出ステップは、前記応答施設情報提供ステップにおいて、前記ユーザ端末から追加のリクエスト操作があった場合に、前記条件情報又は当該条件情報に追加条件を受け付けて、再度、前記記憶手段から一以上の施設情報を抽出する、
施設情報提供方法。 - 請求項1又は請求項2に記載の施設情報提供方法であって、
前記施設情報抽出ステップは、前記施設端末から提供された前記対象者の受け入れ可否に関する情報を参照して、前記施設情報を抽出する、
施設情報提供方法。 - 請求項1乃至請求項3のいずれかに記載の施設情報提供方法であって、
前記施設情報抽出ステップは、前記対象者に関連付けられた所定の住所情報に基づいて前記施設情報を抽出する、

前記施設情報抽出ステップは、前記対象者に関連づけられた関係者が指定した地理情報に基づいて前記施設情報を抽出する、

前記施設情報提供ステップは、前記ユーザ端末に、地図と前記条件情報を満たす前記施設情報とを表示し、

施設情報提供方法。 - 請求項1乃至請求項7のいずれかに記載の施設情報提供方法であって、
前記施設情報は、第1属性の施設に関する第1施設情報と、当該第1属性とは異なる第2属性の第2施設に関する情報とを少なくとも含んでおり、
前記施設情報提供ステップは、抽出した前記施設情報を提供する際に、前記第1施設情報を第1表示領域に表示するとともに、前記第2施設情報を第2表示領域に表示し、
