Abstract
본 발명은 히드로플루메티아지드 또는 이의 약학적으로 허용 가능한 염을 유효성분으로 포함하는 TNF-α 관련 질환 예방 또는 치료용 조성물에 관한 것으로, TNF-α의 발현 또는 활성을 효과적으로 억제할 수 있고, 현재 시판 중인 류마티스 관절염 치료제인 메토트렉세이트(MTX) 또는 히드록시클로로퀸(HCQ)와 비교하여 우수한 치료 효과를 나타내므로, TNF-α 관련 질환의 예방 또는 치료에 유용하게 활용될 수 있다.
Description
본 발명은 히드로플루메티아지드 또는 이의 약학적으로 허용 가능한 염을 유효성분으로 포함하는 TNF-α 관련 질환 예방 또는 치료용 조성물에 관한 것이다.
생체의 방어 메커니즘 중 하나인 염증 반응은 감염이나 상처에 의해 발생하는 복잡한 생체 신호전달 반응으로 이루어지며, 이에 포함되어 있는 다양한 염증성 사이토카인에 의해 매개된다. 일반적으로, 이러한 염증 반응의 이상으로 정상적인 조직까지 파괴되는 질환을 염증성 질환이라고 하고, 그 자세한 기전에 대한 연구는 전세계적으로 활발히 진행되고 있다.
염증 관련 신호전달계는 일련의 인산화-탈인산화 연쇄 반응으로서 크게 세 단계로 나눌 수 있다. 즉, 생체 막에서 염증 신호가 생체 막 수용체와 결합하면서 일련의 신호전달 연쇄 반응을 일으키는 초기단계, 핵 내에서 전사 조절 인자를 통해서 염증 관련 단백질의 유전자 발현을 조절하는 말기 단계, 및 초기 단계와 말기 단계를 연결시키는 세포질 내의 일련의 신호전달 연쇄 반응인 중기단계로 구성된다.
초기 단계의 염증 발생 인자로는 종양 괴사 인자(TNF; tumor necrosis factor, 분비 형태인 TNF-α로도 언급됨) 및 인터루킨-1(IL-1; interleukin-1) 등이 알려져 있다. 말기 단계의 대표적 전사 인자로는 활성화 단백질-1(AP-1; activating protein-1), 핵 전사 인자 카파비(NF-kB; nuclear transcription factor kappa B) 및 활성화된 T 세포의 핵 인자(NFAT; nuclear factor of activated T cells) 등이 알려져 있다. 중기 단계의 연쇄 반응에 관한 인자는 잘 알려져 있지 않다.
염증 발생 인자 중에서도 TNF-α는 염증성 사이토카인 중 가장 강력한 것으로서 주로 활성화된 대식세포 및 T 세포에서 생성된다. TNF-α는 NK-kB 및 c-jun/AP-1과 같은 전사 인자 및 인터루킨-1(IL-1), 인터루킨-6(IL-6) 및 인터루킨-8(IL-8)과 같은 다른 염증성 사이토카인의 생성을 자극하여, 정상적인 염증 반응과 후천성 및 선천성 면역에 중요한 역할을 담당한다.
TNF-α는 유리형(free form)으로 존재하기도 하고 세포막에 결합된 형태(membrane-bound form)로 존재하기도 한다. 이 두 가지 형태의 TNF-α는 매우 강하게 세포의 염증 반응을 유발하고 조직 내의 질환 상태를 촉진시킨다. 세포막에 결합된 TNF-α는 세포 대 세포 접촉을 통해 세포 독성과 염증 효과를 나타내며, 아다말리신(adamalysin)이라고 불리는 종양 괴사 인자 전환 효소(TACE)에 의해 세포막에서 유리형으로 떨어져 세포 밖에 존재하게 된다.
염증성 질환에서 TNF-α가 비정상적으로 많이 생산될 경우에는 면역 체계의 다양한 세포를 활성화시켜 세포 독성 효과를 유발하고 염증, 조직 파괴 또는 장기 손상 등의 병적인 반응들이 일어난다.
다양한 TNF-α 관련 질환에 대한 1차 투여 약물은 스테로이드, 항암제 또는 면역 억제제 등이 있다. 그러나 이러한 1차 투여 약물들은 단기간의 효과가 뛰어나지만 부작용이 존재하고 장기간 사용이 힘들다. 2차 투여 약물로 TNF-α를 억제하는 항체 의약품이 사용되며, 예를 들어 에타너셉트(Etanercept; 엔브렐, Enbrel), 아달리무맙(adalimumab; 휴미라, Humira), 또는 인플릭시맙(infliximab; 레미케이드, Remicade) 등의 블록버스터 바이오 의약품들이 사용되고 있다. 이러한 항체 의약품은 고비용이고 반복적인 주사를 필요로 하기 때문에 환자들의 거부감이 크며, 약 1/3 정도의 환자들에게는 치료 효과가 발생하지 않아 효과에 한계가 있으며, 약에 반응하던 환자들도 면역원성 부작용으로 인해 수년 내에 내성이 발생할 수 있고, 저온 보관이 필수적이므로 보관이 어려운 단점이 있다.
한편, 히드로플루메티아지드(Hydroflumethiazide)는 1950년대 후반에 이뇨 제 활성을 갖는 물질로서 처음 합성되었으며, 고혈압 치료제 및 예방제로서 사용되어 왔다. 그러나 히드로플루메티아지드가 면역 관련 질환에서 어떠한 약리 효과를 나타내는지에 대해서는 알려진 바가 없다.
이러한 배경 하에, 본 발명자들은 TNF-α 관련 질환, 특히 류마티스 관절염의 예방 또는 치료를 위한 조성물을 개발하기 위해 예의 노력한 결과, 인공지능(Artificial Intelligence, AI) 딥러닝(deep learning) 기술을 통해 선택된 히드로플루메티아지드가 활막 증식, 골 파괴 및 연골 손상을 억제한다는 사실을 확인하고 본 발명을 완성하였다.
본 발명의 목적은 TNF-α의 발현 또는 활성을 억제하여 TNF-α 관련 질환을 효과적으로 예방 또는 치료하기 위한 화합물 또는 이의 약학적으로 허용 가능한 염을 유효성분으로 포함하는 TNF-α 관련 질환의 예방 또는 치료용 약학 조성물을 제공하는 것이다.
본 발명의 다른 목적은 상기 화합물 또는 이의 식품학적으로 허용 가능한 염을 유효성분으로 포함하는 TNF-α 관련 질환의 예방 또는 개선용 건강 기능 식품 조성물을 제공하는 것이다.
본 발명의 또 다른 목적은 상기 화합물 또는 이의 염을 사용하는 것을 포함하는 시험관 내(in vitro)에서 TNF-α의 발현 또는 활성을 억제하는 방법 또는 이를 위하여 상기 화합물을 유효성분으로 포함하는 시약 조성물을 제공하는 것이다.
상기 목적을 달성하기 위하여, 본 발명은 히드로플루메티아지드(Hydroflumethiazide) 또는 이의 약학적으로 허용 가능한 염을 유효성분으로 포함하는 TNF-α 관련 질환의 예방 또는 치료용 약학 조성물을 제공한다.
또한, 본 발명은 히드로플루메티아지드 또는 이의 식품학적으로 허용 가능한 염을 유효성분으로 포함하는 TNF-α 관련 질환의 예방 또는 개선용 건강 기능 식품 조성물을 제공한다.
또한, 본 발명은 히드로플루메티아지드 또는 이의 염을 사용하는 것을 포함하는 시험관 내(in vitro)에서 TNF-α의 발현 또는 활성을 억제하는 방법을 제공한다.
또한, 본 발명은 히드로플루메티아지드 또는 이의 염을 유효성분으로 포함하는 시험관 내 TNF-α의 발현 또는 활성 억제용 시약 조성물을 제공한다.
또한, 본 발명은 TNF-α 관련 질환을 예방 또는 치료하기 위한 히드로플루메티아지드 또는 이의 약학적으로 허용 가능한 염을 유효성분으로 포함하는 조성물의 용도를 제공한다.
또한, 본 발명은 TNF-α 관련 질환 예방용 또는 치료용 약제를 제조하기 위한 히드로플루메티아지드 또는 이의 약학적으로 허용 가능한 염을 유효성분으로 포함하는 조성물의 용도를 제공한다.
또한, 본 발명은 히드로플루메티아지드 또는 이의 약학적으로 허용 가능한 염을 유효성분으로 포함하는 조성물을 개체에 투여하는 단계를 포함하는 TNF-α 관련 질환의 예방 또는 치료방법을 제공한다.
본 발명에 따른 TNF-α 관련 질환 예방 또는 치료용 약학 조성물은 TNF-α의 발현 또는 활성을 효과적으로 억제할 수 있고, 현재 시판 중인 류마티스 관절염 치료제인 메토트렉세이트(MTX) 또는 히드록시클로로퀸(HCQ)와 비교하여 우수한 치료 효과를 나타내므로, TNF-α 관련 질환의 예방 또는 치료에 유용하게 활용될 수 있다.
도 1은 RAW 264.7에 DMSO(대조군; CTL), LPS(Lipopolysaccharide; 지질다당류), PS1145 또는 다양한 농도의 히드로플루메티아지드를 처리한 후, 세포 생존률을 측정한 결과이다.
도 2는 염증 반응을 유도한 RAW 264.7에 PS1145 또는 다양한 농도(0.1 μM, 1 μM, 10 μM, 20 μM)의 히드로플루메티아지드를 처리한 각 군에서 TNF-α의 발현량을 측정한 결과이다.
도 3은 류마티스 관절염 DBA-1J 마우스 모델에 약물을 처리한지 9일차 또는 14일차에 각 군에서 관절염 지수를 측정한 결과이다(G1: 정상 대조군, G2: 비교 대조군, G3: MTX 투여군, G4: HCQ 투여군, G5: 5 mg/kg/day 히드로플루메티아지드 투여군, G6: 20 mg/kg/day 히드로플루메티아지드 투여군).

이하, 본 발명에 대하여 상세히 설명하도록 한다.
본 발명은 일 측면에서, 히드로플루메티아지드(Hydroflumethiazide) 또는 이의 약학적으로 허용 가능한 염을 유효성분으로 포함하는 TNF-α 관련 질환 예방 또는 치료용 약학 조성물을 제공한다.
본 명세서에서 용어 "히드로플루메티아지드(Hydroflumethiazide)"는 C8H8F3N3O4S2의 분자식으로 표시되는 화합물이며, 하기와 같은 화학식 1의 구조를 갖는다.

본 발명에 있어서, 히드로플루메티아지드는 TNF-α의 발현 또는 활성을 억제할 수 있다.
본 발명의 일 구현예에서, LPS(Lipopolysaccharide; 지질다당류)로 염증 반응이 유도된 대식세포에 히드로플루메티아지드를 처리하여 TNF-α의 발현이 억제될 수 있다.
본 명세서에서 용어 "TNF-α(tumor necrosis factor-α)"는 종양 괴사 인자-α를 의미하고, 이는 염증 반응에 포함되고 급성기 반응(acute-phase protein)의 구성원인 사이토카인이다. TNF-α는 주로 활성화된 대식세포에 의해 분비되는데, 보조 T 세포, 자연살해세포, 그리고 손상된 뉴런 등의 다양한 세포에서도 분비된다. 또한, TNF-α의 가장 중요한 역할은 면역 세포의 조절로서, TNF-α는 체내 발열원으로서, 열이 나게 유도하거나, 세포 자살을 유도하거나, IL-1과 IL-6의 생산을 통해 패혈증을 유발하거나, 악액질을 유도하거나, 감염을 유발하며 종양 생성과 바이러스 복제를 억제하는 능력을 갖는다. TNF-α의 비정상적인 조절은 알츠하이머 병, 암, 우울증, 그리고 염증성 장질환(IBD) 등 다양한 인간의 질환에서 나타나는 것으로 알려져 있다.
본 발명에 있어서, 히드로플루메티아지드는 활막 증식(synovium proliferation), 골 파괴(bone destruction) 또는 연골 손상(cartilage damage)을 억제할 수 있다.
본 발명의 일 구현예에서, TNF-α에 의해 유발된 류마티스 관절염을 갖는 동물 모델에 히드로플루메티아지드를 경구 투여하여 농도 의존적으로 치료 효능을 나타내고, 시판 중인 의약품인 HCQ 보다 우수하고 MTX와 유사한 수준의 항 관절염 효과를 나타낼 수 있다.
본 발명에 있어서, 상기 TNF-α 관련 질병은 자가 면역 질환, 염증성 질환, 호흡기 질환 및 당뇨병일 수 있다. 구체적으로, 상기 TNF-α 관련 질환은 류마티스 관절염(Rheumatoid arthritis), 건선(Psoriasis), 건선성 관절염(Psoriatic arthritis), 강직성 척추염(Ankylosing Spondylitis), 소아 특발성 관절염(Juvenile idiopathic arthritis), 크론병(Crohn's disease), 궤양성 결장염(Ulcerative colitis), 다발성 경화증(Multiple sclerosis), 패혈증(Sepsis), 호흡기 질환(Respiratory diseases), 혈관염(Vasculitis), 베체트병(Behcet Disease), 제1형 당뇨병(Type 1 diabetes), 전신 홍반성 낭창(Systemic lupus erythematosus), 성인 발증형 스틸병(Adult-onset Still's Disease), 다발성근염(Polymyositis), 피부근염(Dermatomyositis) 및 베게너 육아종증(Wegener's Granulomatosis)으로 구성된 군에서 선택되는 하나 이상일 수 있으나, 이에 제한되는 것은 아니다. 보다 구체적으로, 상기 TNF-α 관련 질환은 관절염일 수 있다.
과잉 생산된 TNF-α는 류마티스 관절염 환자의 대식세포를 자극하여 염증반응을 증폭시키는 염증성 매개체(proinflammatory mediator)를 생산한다. 또한, 과잉 생산된 TNF-α는 혈관 내피 세포에서 부착 분자를 발현하게 하여 더 많은 염증 세포들이 염증이 일어난 부위로 모일 수 있도록 하고, 섬유아세포로 하여금 단백분해효소를 생산하게 하여 연골, 뼈, 또는 인대 등에 손상을 입혀 질환을 악화시킨다(송정수, 류마티스관절염 치료에서 종양괴사인자억제제 사용에 대한 고찰, 대한 류마티스 학회지, Vol.14, No.1, March, 2007). 즉, TNF-α 억제제는 류마티스 관절염의 치료 또는 예방에 유용하다고 알려져 있다.
건선은 유전적인 소인이 있는 상태에서 스트레스, 약물 또는 감염 등의 자극 이 반복되면 피부의 면역 체계가 발동하면서 T 세포에 영향을 주게 되고, 이에 따라 TNF-α 또는 IL-17A와 같은 사이토카인이 작용하여 표피 세포가 정상보다 많이 증식하여 발생한다(최병철, 건선과 건선성 관절염, 이슈&트렌드, 약학정보원). 즉, TNF-α 억제제는 건선의 치료 또는 예방에 유용하다고 알려져 있다.
건선성 관절염(Psoriatic arthritis) 환자의 관절낭에서 TNF-α와 함께 용해성의 TNF-α 수용체가 일반 관절염 환자보다 많이 증가되어 조직 손상이 나타나므로, TNF-α 억제제가 건선성 관절염 환자의 치료에 유용하다고 알려져 있다 (Mease PJ. 강직성 척추염(Ankylosing Spondylitis)은 천장 관절과 척추에서 나타나는 만성 염증 질환으로 새로운 골격 형성과 골 감소증을 특징으로 한다. 이 질환에서의 염증 반응에서도 TNF-α가 중요한 역할을 하므로, TNF-α 억제제를 강직성 척추염 환자의 치료제로 사용하고 있다(Toussirot E. 소아 특발성 관절염(Juvenile idiopathic arthritis)은 아동기에 나타나는 관절염으로 대식세포의 활성이 중요한 역할을 한다. 대식세포는 TNF-α를 방출하는 주요 세포이므로 TNF-α를 억제하는 약물이 소아 특발성 관절염 환자의 치료에 유용하다고 알려져 있다(Mellins ED, et al. 크론병(Crohn's disease)은 소화 기관에서의 만성적 염증 반응으로 일어나는데 이러한 염증 반응에서도 TNF-α의 역할이 중요하므로, 1998년부터 TNF-α 억제제가 크론병 치료제로 사용되고 있다(Adegbola S, et al. Anti-TNF Therapy in Crohn's Disease. 궤양성 결장염(Ulcerative colitis)은 염증 부위가 직장을 포함한 대장 점막부위에서 주로 나타나는 질환으로 크론병과 비슷한 기전과 증상을 가져 TNF-α 억제제가 궤양성 결장염 환자의 치료에 유용하다고 알려져 있다(Sands BE, et al. The role of TNFalpha in ulcerative colitis. 다발성 경화증(Multiple sclerosis)은 신경 세포의 미엘린초에 손상이 나타나 발생하는 자가면역질환으로 TNF-α는 다발증 경화증을 일으키는 가장 중요한 사이토카인 중 하나로 알려져 있다. 따라서, TNF-α를 억제하는 약물이 다발성 경화증 환자의 치료에 유용하다고 알려져 있다(Sastry PS. Inhibition of TNF-alpha synthesis with thalidomide for prevention of acute exacerbations and altering the natural history of multiple sclerosis. Med Hypotheses. 1999 Jul; 53(1):76-7).
패혈증(Sepsis)은 미생물에 감염되어 나타나는 전신에 걸친 염증 반응이며, 감염에 의해 나타나는 선천적 면역반응의 주요 세포들인 단핵구 또는 대식세포 등에서 TNF-α가 방출된다. 그 결과 발열, 호흡수 증가, 백구 증가 등의 증상이 나타나게 되고, 정도가 심해지면 패혈성 쇼크(Septic shock)에 의해서 생명이 위험할 수 있다. 따라서, TNF-α를 억제하는 약물이 패혈증 환자의 치료 또는 예방에 유용하다고 알려져 있다(Lv S, et al. 호흡기 질환(Respiratory diseases)에서 TNF-α는 만성 기관지염(chronic bronchitis), 만성 폐쇄성 폐질환(chronic obstructive pulmonary disease, COPD), 천식(asthma), 급성 폐손상(acute lung injury) 또는 급성 폐손상이 심하게 나타나 발생하는 급성 호흡 곤란 증후군(acute respiratory distress syndrome, ARDS) 등을 유발 또는 악화 시키므로, TNF-α를 억제하는 약물이 호흡기 질환 환자의 치료 또는 예방에 유용하다고 알려져 있다(Mukhopadhyay S, et al. 베체트병(Behcet Disease)도 혈관에 염증이 생기는 자가 면역질환으로 주로 구강 궤양, 음부 궤양 또는 눈 안에서 염증이 나타나는 등의 증세가 나타나는데 TNF-α를 억제하는 약물이 베체트병 환자의 치료에 유용하다고 알려져 있다 (Vallet H, et al. 제1형 당뇨병(Type 1 diabetes)에서 TNF-α는 직접적으로 베타세포를 파괴하므로 TNF-α를 억제하는 약물이 제1형 당뇨 환자의 치료에 유용하다고 알려져 있다(Christen U, et al. 전신 홍반성 낭창(Systemic lupus erythematosus) 환자에서도 혈장에서 높은 수준의 TNF-α가 검출되며, 신장염 발생 시에도 TNF-α가 높게 검출되므로 TNF-α를 억제하는 약물이 전신 홍반성 낭창 환자의 치료 또는 예방에 유용하다고 알려져 있다(Aringer M, et al. 성인 발증형 스틸병(Adult-onset Still's Disease)은 16세 이하에서 나타나는 스틸병이 30-40대에 나타나는 질환으로 35세 이하의 젊은 층에서 많이 발생한다. 이 질환에서도 혈장과 장기에서 높은 수준의 TNF-α가 검출되므로 TNF-α를 억제하는 약물이 성인 발증형 스틸병 환자의 치료 또는 예방에 유용할 수 있다고 알려져 있다(Al-Homood IA. Biologic treatments for adult-onset Still's disease. Rheumatology (Oxford). 2014 Jan; 53(1):32-8).
다발성근염(Polymyositis)은 환자의 근내막에서 TNF-α 수준과 TNF-α-양성인 대식세포 및 림프구의 침범이 늘어나 있다. 또한 근내막에서의 TNF-α 수준과 근섬유위축 사이에 상관관계가 존재하므로 TNF-α를 억제하는 약물이 다발성근염 환자의 치료 또는 예방에 유용할 수 있다고 알려져 있다(Kalden JR. Emerging role of anti-tumor necrosis factor therapy in rheumatic diseases. Arthritis Res. 2002; 4 Suppl 2:S34-40; 및 Lamprecht P. TNF-alpha inhibitors in systemic vasculitides and connective tissue diseases. Autoimmun Rev. 2005 Jan; 4(1):28-34).
피부근염(Dermatomyositis) 환자에서 TNF-α를 억제하는 약물을 사용했을 때 증세가 감소하는 보고가 존재하므로, TNF-α 억제제가 피부근염 환자의 치료 또는 예방에 유용할 수 있다고 예상된다(Norman R, et al. Case reports of etanercept in inflammatory dermatoses. J Am Acad Dermatol. 2006 Mar; 54(3 Suppl 2):S139-42).
베게너 육아종증(Wegener's Granulomatosis) 환자는 육아종과 혈관염을 특징으로 하는 질환으로 호흡기와 신장에서 주로 병증이 나타난다. 베게너 육아종증 환자에서 TNF-α를 억제하는 약물을 처리했을 때 증세가 감소하는 보고가 존재하므로, TNF-α 억제제는 베게너 육아종증 환자의 치료 또는 예방에 유용할 수 있다고 예상된다(Lamprecht P, et al. Effectiveness of TNF-alpha blockade with infliximab in refractory Wegener's granulomatosis. Rheumatology (Oxford). 2002 Nov; 41(11):1303-7).
본 발명에 있어서, 상기 히드로플루메티아지드의 약학적으로 허용 가능한 염은 유기산, 무기산 또는 산성 아미노산과의 염 등을 포함할 수 있다. 구체적으로, 유기산과의 염은 초산, 아세트산, 프로피온산, 이소부틸산, 옥살릭산, 말레익산, 말로닉산, 숙신산, 글루타르산, 살리실산, 수버릭산, 푸마릭산, 만데릭산, 프탈릭, 벤젠설포닉, p-토릴설포닉, 구연산, 주석산, 메탄설포닉산 또는 그 유사체 등의 염을 포함할 수 있다. 또한, 무기산과의 염은 염산, 브롬산, 질산, 탄산, 일수소탄산, 인산, 일수소인산, 이수소인산, 황산, 일수소황산, 요오드화수소 또는 아인산 및 그 유사체 등의 염을 포함할 수 있다. 또한, 산성 아미노산과의 염은 아스파르트산, 글루탐산 등의 염을 포함할 수 있다. 보다 구체적으로, 상기 히드로플루메티아지드의 약학적으로 허용 가능한 염은 초산, 아세트산, 말레익산, 푸마릭산, 숙신산, 글루타르산 및 살리실산으로 구성된 군에서 선택되는 하나 이상일 수 있다.
본 발명에 있어서, 상기 약학 조성물은 약학적으로 허용되는 담체를 추가로 포함할 수 있다.
본 발명의 약학 조성물에 포함되는 약학적으로 허용되는 담체는 제제화 시에 통상적으로 이용되는 것으로서, 락토오스, 덱스트로오스, 수크로오스, 솔비톨, 만니톨, 전분, 아카시아 고무, 인산 칼슘, 알기네이트, 젤라틴, 규산 칼슘, 미세결정성 셀룰로스, 폴리비닐피롤리돈, 셀룰로오스, 물, 시럽, 메틸 셀룰로오스, 메틸하이드록시벤조에이트, 프로필하이드록시벤조에이트, 활석, 마그네슘 스테아레이트, 미네랄 오일, 식염수, PBS(phosphate buffered saline) 또는 배지 등을 포함하나, 이에 한정되는 것은 아니며, 상기 성분들 이외에 윤활제, 습윤제, 감미제, 향미제, 유화제, 현탁제, 보존제 등을 추가적으로 포함할 수 있다.
본 발명의 약학 조성물은 당해 발명이 속하는 기술분야에서 통상의 지식을 가진 자가 용이하게 실시할 수 있는 방법에 따라, 약학적으로 허용되는 담체 및/또는 부형제를 이용하여 제형화함으로써 단위 용량 형태로 제조되거나 또는 대용량 용기 내에 내입시켜 제조될 수 있으며, 분산제 또는 안정화제를 추가적으로 포함할 수 있다.
상기 약학 조성물의 제형은 사용 방법에 따라 달라질 수 있으나, 경고제, 과립제, 산제, 시럽제, 액제, 유동엑스제, 유제, 현탁제, 침제, 정제, 주사제, 캅셀제 및 환제 등으로 제조할 수 있다. 또한, 상기 약학적 조성물의 제형은 국소 또는 경피 투여용 제형인 연고, 페이스트, 크림, 로션, 젤, 분말, 용액, 스프레이, 흡입제 또는 패치를 포함한다.
상기 약학 조성물은 약학적으로 허용되는 담체 및 필요에 따라서 방부제, 완충제등과 무균 혼합될 수 있다. 본 발명에 따른 연고, 페이스트, 크림 및 젤 제형은 추가적으로 부형제인 동물성 및 식물성 지방, 기름, 왁스, 파라핀, 전분, 토라가칸토, 셀룰로오스 유도체, 폴리에틸렌 글리콜, 실리콘, 벤토나이트 등과 규산, 활석, 산화 아연, 또는 이들의 혼합물을 포함할 수 있다.
본 명세서에서 용어 "예방"은 본 발명에 따른 조성물의 투여로 TNF-α 관련 질환을 억제하거나 진행을 지연시키는 모든 행위를 의미하며, "치료"는 본 발명의 조성물의 투여로 TNF-α 관련 질환의 증상이 호전 또는 이롭게 변경되는 모든 행위를 의미한다.
본 발명의 히드로플루메티아지드 또는 약학 조성물은 치료학적으로 유효한 양 또는 약학적으로 유효한 양으로 환자에 투여될 수 있다.
여기서 "치료학적으로 유효한 양" 또는 "약학적으로 유효한 양"이란 대상 질환을 예방 또는 치료하는데 유효한 화합물 또는 조성물의 양으로서, 의학적 치료에 적용 가능한 합리적인 수혜/위험 비율로 질환을 치료하기에 충분하며 부작용을 일으키지 않을 정도의 양을 의미한다. 또한, 상기 "유효한 양"은 생체 외(in vitro) 또는 생체 내(in vivo) 어떤 쪽이든 TNF-α 관련 질환에서 TNF-α의 활성을 억제 또는 줄이기에 충분한 양을 말한다. 상기 유효량의 수준은 환자의 건강상태, 질환의 종류, 중증도, 약물의 활성, 약물에 대한 민감도, 투여 방법, 투여 시간, 투여 경로 및 배출 비율, 치료 기간, 배합 또는 동시 사용되는 약물을 포함한 요소 및 기타 의학 분야에 잘 알려진 요소에 따라 결정될 수 있다.
본 발명의 히드로플루메티아지드 또는 이의 약학적으로 허용 가능한 염은 임의의 적합한 경로에 의하여 이러한 경로에 적당한 약학 조성물의 형태, 그리고 의도된 치료를 위하여 효과적인 투여량으로 투여될 수 있다.
본 발명의 히드로플루메티아지드 또는 약학 조성물은 개별 치료제로 투여하거나 다른 치료제와 병용하여 투여될 수 있고, 종래의 치료제와 순차적으로 또는 동시에 투여될 수 있으며, 단일 또는 다중 투여될 수 있다. 상기한 요소들을 모두 고려하여 부작용 없이 최소한의 양으로 최대 효과를 얻을 수 있는 양을 투여하는 것이 중요하며, 이는 당업자에 의해 용이하게 결정될 수 있다.
구체적으로, 본 발명의 조성물에서 화합물의 히드로플루메티아지드 또는 이의 약학적으로 허용 가능한 염의 효과적인 투여량은 환자의 나이, 성별, 체중에 따라 달라질 수 있으며, 단일 또는 분할 투여로 일반적으로 약 12.5 mg/kg/일 내지 약 200 mg/kg/일이며, 100 mg 이상은 하루에 2회 이상 나누어 투여해야 한다. 그러나, 투여 경로, 질환의 중증도, 성별, 체중, 연령 등에 따라서 증감될 수 있으므로, 본 발명의 범위는 이에 한정되지 않는다.
TNF-α 관련 질환의 치료를 위하여, 본 명세서에서 설명된 상기 히드로플루메티아지드 또는 이의 약학적으로 허용 가능한 염들은 다음과 같이 다양한 방법으로 투여될 수 있다.
본 발명의 약학 조성물은 구강으로 투여될 수 있으며, 구강은 연하(swallowing)를 포함하는 개념이다. 구강 투여에 의하여 본 발명의 약학 조성물이 위장관(gastrointestinal tract)에 들어가거나, 예를 들어, 구강(buccal) 또는 설하(sublingual) 투여와 같이, 입으로부터 혈류로 직접적으로 흡수될 수 있다.
구강 투여를 위한 적합한 조성물은 고형상, 액상, 겔(gel), 또는 파우더 형상일 수 있으며, 정제(tablet), 로젠지(lozenge), 캡슐(capsule), 과립제, 산제 등의 제형을 가질 수 있다.
구강 투여를 위한 조성물은 선택적으로 장용 코팅(enteric coating)될 수 있으며, 장용 코팅을 통하여 지연된(delayed) 또는 지속된(sustained) 방출을 나타낼 수 있다. 즉, 본 발명에 따른 구강 투여를 위한 조성물은 즉시 또는 변형된(modified) 방출 패턴을 가진 제형일 수 있다.
액체 제형은 용액, 시럽 및 현탁액을 포함할 수 있으며, 이러한 액상 조성물은 연질 또는 경질 캡슐 내에 함유된 형태일 수 있다. 이러한 제형은 약학적으로 허용 가능한 담체, 예를 들어, 물, 에탄올, 폴리에틸렌글리콜, 셀룰로오스 또는 오일(oil)을 포함할 수 있다. 상기 제형은 또한 하나 이상의 유화제 및/또는 현탁제를 포함할 수 있다.
정제(tablet) 제형에서, 활성 성분인 약물의 양은 정제 총 중량 대비 약 0.05 중량% 내지 약 95 중량%, 더욱 일반적으로 제형의 약 2 중량% 내지 약 70 중량%로 존재할 수 있다. 또한, 정제는 약 0.5 중량% 내지 약 35 중량%, 더욱 일반적으로 제형의 약 2 중량% 내지 약 25 중량%를 포함하는 붕해제를 함유할 수 있다. 붕해제의 예로는 유당, 전분, 소디움스타치글리콜레이트, 크로스포비돈, 크로스카멜로스소디움(croscarmellose sodium), 말토덱스트린 또는 이들의 혼합물이 사용될 수 있으나 이에 한정되는 것은 아니다.
정제로 제조하기 위해 포함되는 적합한 활택제는 약 0.1 중량% 내지 약 10 중량% 양으로 존재할 수 있고, 탈크(talc), 이산화규소, 스테아린산, 칼슘, 아연 또는 마그네슘 스테아레이트, 소듐 스테아릴 푸마레이트 등이 활택제로 사용될 수 있으나, 본 발명은 이러한 첨가제들의 종류에 한정되는 것은 아니다.
정제로 제조하기 위한 결합제(binder)로는 젤라틴, 폴리에틸렌글리콜, 당(sugar), 검(gum), 녹말(starch), 폴리비닐피롤리돈, 하이드록시프로필셀룰로오스, 하이드록시프로필메틸셀룰로오스 등이 사용될 수 있으며, 정제로 제조하기 위한 적합한 희석제로는 만니톨, 자일리톨, 락토오스, 덱스트로오스, 수크로오스, 솔비톨, 녹말(starch), 미결정셀룰로오스 등이 사용될 수 있으나, 본 발명은 이러한 첨가제들의 종류에 한정되는 것은 아니다. 상기 결합제는 정제 총 중량 대비 약 0.1 중량% 내지 약 20 중량% 양이 사용될 수 있으며, 상기 희석제는 정제 총 중량 대비 약 0.1 중량% 내지 약 20 중량% 양이 사용될 수 있다.
선택적으로 정제에 포함될 수 있는 가용화제는 정제 총 중량 대비 약 0.1 중량% 내지 약 3 중량% 양이 사용될 수 있고, 예를 들어, 폴리소르베이트, 소디움 라우릴설페이트, 소디움 도데실설페이트, 프로필렌 카보네이트, 디에틸렌글리콜모노에틸에테르, 디메틸이소소르비드, 폴리옥시에틸렌글리콜화된 천연 또는 수소화 피마자유, HCORTM(Nikkol), 올레일에스테르, 젤루시어(GelucireTM), 카프릴릭/카프릴산 모노/디글리세리드, 소르비탄지방산에스테르, 솔루톨HSTM 등이 본 발명에 따른 약학 조성물에 사용될 수 있으나, 본 발명은 이러한 가용화제의 구체적 종류에 한정되는 것은 아니다.
본 발명의 약학 조성물은 혈류, 근육, 또는 내장 내로 직접 투여될 수 있다. 비경구 투여를 위한 적합한 방법은 정맥내(intravenous), 근육내(intra-muscular), 피하 동맥내(subcutaneous intraarterial), 복강내(intraperitoneal), 척추강내(intrathecal), 두개내(intracranial) 주사 등을 포함한다. 비경구 투여를 위한 적합한 장치는 (바늘 및 바늘 없는 주사기를 포함하는) 주사기(injector) 및 주입 방법(infusion method)을 포함한다.
비경구 투여를 위한 조성물은 즉시 또는 변형된 방출 패턴을 가진 제형일 수 있으며, 변형된 방출 패턴은 지연된(delayed) 또는 지속된(sustained) 방출 패턴일 수 있다.
대부분의 비경구 제형은 액상 조성물이며, 이러한 액상 조성물은 본 발명에 따른 유효성분, 염, 완충제, 등장화제 등을 포함하는 수용액이다.
비경구 제형은 또한 건조된 형태(예를 들어, 동결 건조) 또는 멸균 비-수용액으로서 제조될 수 있다. 이들 제형은 멸균수(sterile water)와 같은 적합한 비히클(vehicle)과 함께 사용될 수 있다. 용해도 증강제(solubility-enhancing agents) 또한 비경구 용액의 제조에 사용될 수 있다.
본 발명의 약학 조성물은 피부 또는 경피로 국소적으로 투여될 수 있다. 이 국소 투여를 위한 제형은 로션, 용액, 크림, 젤, 하이드로젤, 연고, 폼(foam), 임플란트(implant), 패치 등을 포함한다. 국소 투여 제형을 위한 약학적으로 허용 가능한 담체는 물, 알코올, 미네랄 오일, 글리세린, 폴리에틸렌글리콜 등을 포함할 수 있다. 국소 투여는 또한 전기천공법(electroporation), 이온도입법(iontophoresis), 음파영동(phonophoresis) 등에 의하여 수행될 수 있다.
국소 투여를 위한 조성물은 즉시 또는 변형된 방출 패턴을 가진 제형일 수 있으며, 변형된 방출 패턴은 지연된(delayed) 또는 지속된(sustained) 방출 패턴일 수 있다.
본 발명은 다른 측면에서, 히드로플루메티아지드 또는 이의 식품학적으로 허용 가능한 염을 유효성분으로 포함하는 TNF-α 관련 질환 예방 또는 개선용 건강 기능 식품 조성물을 제공한다.
본 명세서에서 용어 "식품"은 영양소를 한 가지 또는 그 이상 함유하고 있는 천연물 또는 가공품을 의미하며, 바람직하게는 어느 정도의 가공 공정을 거쳐 직접 먹을 수 있는 상태가 된 것을 의미하며, 통상적인 의미로서의 식품, 식품 첨가제, 건강 기능성 식품, 음료 및 음료 첨가제 등을 모두 포함하는 용어이다.
본 발명의 조성물이 식품 조성물로 제조되는 경우, 식품 제조 시에 통상적으로 첨가되는 성분을 포함하며, 예를 들어, 단백질, 탄수화물, 지방, 영양소, 조미제 및 향미제를 포함할 수 있다.
본 발명은 또 다른 측면에서, 히드로플루메티아지드 또는 이의 염을 사용하는 것을 포함하는 시험관 내(in vitro)에서 TNF-α의 발현 또는 활성을 억제하는 방법을 제공한다.
본 발명은 또 다른 측면에서, 히드로플루메티아지드 또는 이의 염을 유효성분으로 포함하는 시험관 내(in vitro) TNF-α의 발현 또는 활성 억제용 시약 조성물을 제공한다.
본 발명은 또 다른 측면에서, TNF-α 관련 질환을 예방 또는 치료하기 위한 히드로플루메티아지드 또는 이의 약학적으로 허용 가능한 염을 유효성분으로 포함하는 조성물의 용도를 제공한다.

이때, 상기 히드로플루메티아지드 또는 이의 약학적으로 허용 가능한 염은 약학 조성물에서 상술한 바와 동일하다.
이하, 본 발명을 하기 실시예에 의하여 더욱 상세하게 설명한다.
단, 하기 실시예는 본 발명을 예시하기 위한 것일 뿐, 본 발명의 범위가 이들만으로 한정되는 것은 아니다.
실시예 1. 유도된 염증 세포에서 히드로플루메티아지드의 TNF-α 발현 억제 효과
본 발명자들은 인공지능 딥러닝 기술을 이용한 스크리닝 방법을 통해 TNF-α 관련 질환, 특히 류마티스 관절염에 치료 효과가 있을 것으로 예상되는 물질로 히드로플루메티아지드를 선택하였다.
대식세포(macrophage; RAW 264.7; TIB-71)를 ATCC로부터 구입하고, MTT assay를 통해서 RAW 264.7에 하기의 표 1에 기재된 바와 같이 DMSO, LPS(Lipopolysaccharide; 지질다당류), PS1145 또는 다양한 농도의 히드로플루메티아지드를 처리하여 세포 독성을 나타내지 않는 것을 확인하였다(도 1).
상기 "PS1145"는 N-(6-Chloro-9H-pyrido[3,4-b]indol-8-yl)-3-pyridinecarboxamide dihydrochloride를 의미하며, IKK(IkB Kinase) 억제제로 알려져 있으며, 하기와 같은 화학식 2의 구조를 갖는다.

또한, 유도된 염증 세포에서 히드로플루메티아지드의 TNF-α 발현 억제 효과를 확인하기 위하여 다음과 같이 실험하였다. RAW 264.7에 무처리 대조군으로 DMSO를 0.01%(v/v) 농도로 처리하고, 대조군으로 LPS를 10 ng/㎖ 농도로 처리하여 염증 반응을 유도하였다. 또한, 실험군으로서 LPS를 처리하여 염증 반응이 유도된 RAW 264.7에 표 1에 기재된 바와 같이 PS1145 또는 다양한 농도(0.1 μM, 1 μM, 10 μM, 20 μM)의 히드로플루메티아지드를 처리하였다. 그리고 나서, 각 약물을 처리한 군에서 ELISA 방법을 통하여 TNF-α의 발현량을 측정하여 비교하였다.
그 결과, 염증 반응이 유도된 RAW 264.7에 0.1 μM 이상의 히드로플루메티아지드를 처리하는 경우에 TNF-α의 발현량이 적어도 20% 이상 감소하였으며, 1 μM 이상의 히드로플루메티아지드를 처리한 군에서는 PS1145를 처리한 군과 비교하여 TNF-α의 발현량이 보다 더 감소하였다(도 2). 특히, 염증 반응이 유도된 RAW 264.7에 10 μM 이상의 히드로플루메티아지드를 처리한 군들은 TNF-α의 발현량이 50% 이상 현저하게 감소하였다.
이를 통하여, 히드로플루메티아지드가 염증 세포에서 TNF-α의 발현을 억제하는 효과가 있는 것을 확인하였다.
실시예 2. 동물 모델을 이용한 히드로플루메티아지드의 류마티스 관절염 치료 효과
실시예 2.1. 실험 방법
DBA-1J 마우스를 중앙실험동물㈜로부터 구입하였다. 2 mg/㎖ Bovine Type II collagen을 4 mg/㎖ Complete Freund's adjuvant에 1:1로 유화시킨 후, DBA-1J 마우스의 꼬리 기저부에 100 ㎕/head로 피하 내 주사하여 1차로 관절염을 유도하였다. 그리고 나서 3주 후에 2 mg/㎖ Bovine Type II collagen을 Incomplete Freund's adjuvant에 1:1로 유화시켜 DBA-1J 마우스의 꼬리 기저부에 100 ㎕/head로 피하 내 주사하여 류마티스 관절염(rheumatoid arthritis) 동물 모델을 제조하였다.
정상 대조군으로 건강한 DBA-1J 마우스에 0.5% MC(Methyl cellulose)를, 비교 대조군으로 제조한 류마티스 관절염 마우스 모델에 0.5% MC를 2주 동안 매일 경구 투여하였다. 실험군으로서 제조한 류마티스 관절염 마우스 모델에 시판 중인 류마티스 관절염 치료제 메토트렉세이트(methotrexate, MTX) 2 mg/kg/2day, 히드록시클로로퀸(hydroxychloroquine, HCQ) 80 mg/kg/day 또는 히드로플루메티아지드 5 mg/kg/day 또는 20 mg/kg/day을 각각 2주 동안 매일 경구 투여하였다. 각 실험군의 구성은 하기의 표 2에 기재된 바와 같다.

실시예 2.2. 관절염 지수 및 하지 부종의 크기 측정
각 대조군 및 실험군에서 각각의 약물을 투여한 지 9일째 및 14일째에 다음과 같은 기준으로 관절염 지수를 측정하였다. 기준: 0, 부종이나 종창이 없음: 1, 관절에 국한된 경한 부종과 발적: 2, 관절에서 수근골 및 족근골에 걸친 경한 부종과 발적: 3, 관절에서 수근골 및 족근골에 걸친 중증도의 부종과 발적: 4, 전체적으로 부종과 발적이 있고 관절경직이 나타남: 5. 관절염 지수는 사지 점수를 합산하여 최고 점수를 16점으로 하였다. 또한, 각 대조군 및 실험군에서 각각의 약물을 투여한 지 14일째에 마이크로미터(micrometer)를 이용하여 하지 부종의 크기를 측정하였다.
각 군의 관절염 지수는 하기의 표 3에, 하지 부종 크기는 하기의 표 4에 기재한 바와 같다(N = 동물수(마리), **/* A significant difference at p<0.01/p<0.05 level compared to the G1, ##/# A significant difference at p<0.01/p<0.05 level compared to the G2).

그 결과, 히드로플루메티아지드 투여군에서는 투여 9일째에서부터 HCQ 투여군에 비해 관절염 지수가 현저하게 감소하였고, 14일째에서는 히드로플루메티아지드 고농도(20 mg/kg/day) 투여군에서 MTX 투여군과 유사한 수준으로 관절염 지수와 하지 부종 크기가 감소하였다(도 3 및 도 4).
이를 통하여, 류마티스 관절염 동물 모델에서 히드로플루메티아지드의 경구 투여가 치료 효과가 있음을 확인하였다.
실시예 2.3. 조직병리학적 관찰
투여 14일째에 각 대조군 및 실험군의 마우스를 희생시킨 후, 다음과 같은 과정을 통하여 관절 염색 사진을 촬영하였다. 희생시킨 마우스의 발목 부위에 대하여 탈회, 삭정, 탈수, 파라핀 포매, 박절 등 일반적인 조직 처리 과정을 거쳐 조직병리학적 검사를 위한 검체를 제작한 뒤, Hematoxylin & Eosin (H&E) 및 Safranin O 염색을 수행하고, 광학 현미경을 이용하여 조직병리학적 변화를 관찰하였다(도 5).
또한, 활막 증식(synovium proliferation), 골 파괴(bone destruction) 및 연골 손상(cartilage damage) 정도를 수치화하여, 그 결과를 하기의 표 5에 기재하였다.

이를 통하여, TNF-α에 의해 이미 발생된 류마티스 관절염에 대하여 히드로플루메티아지드는 농도 의존적으로 치료 효능을 나타냈으며, 시판 중인 의약품인 HCQ 보다 우수하고 MTX와 유사한 수준의 항 관절염 효과를 나타내는 것을 확인하였다(도 6).
Claims (12)
- 히드로플루메티아지드(Hydroflumethiazide) 또는 이의 약학적으로 허용 가능한 염을 유효성분으로 포함하는 TNF-α 관련 질환 예방 또는 치료용 약학 조성물.
- 제1항에 있어서,상기 조성물은 TNF-α의 발현 또는 활성을 억제하는 것인, TNF-α 관련 질환 예방 또는 치료용 약학 조성물.
- 제1항에 있어서,상기 조성물은 활막 증식(synovium proliferation), 골 파괴(bone destruction) 또는 연골 손상(cartilage damage)을 억제하는 것인, TNF-α 관련 질환 예방 또는 치료용 약학 조성물.
- 제1항에 있어서,상기 TNF-α 관련 질환은 자가 면역 질환, 염증성 질환, 호흡기 질환 및 당뇨병으로 구성된 군에서 선택되는 것인, TNF-α 관련 질환 예방 또는 치료용 약학 조성물.
- 제1항에 있어서,상기 TNF-α 관련 질환은 관절염인 것인, TNF-α 관련 질환 예방 또는 치료용 약학 조성물.
- 제1항에 있어서,상기 TNF-α 관련 질환은 류마티스 관절염(Rheumatoid arthritis), 건선(Psoriasis), 건선성 관절염(Psoriatic arthritis), 강직성 척추염(Ankylosing Spondylitis), 소아 특발성 관절염(Juvenile idiopathic arthritis), 크론병(Crohn's disease), 궤양성 결장염(Ulcerative colitis), 다발성 경화증(Multiple sclerosis), 패혈증(Sepsis), 호흡기 질환(Respiratory diseases), 혈관염(Vasculitis), 베체트병(Behcet Disease), 제1형 당뇨병(Type 1 diabetes), 전신 홍반성 낭창(Systemic lupus erythematosus), 성인 발증형 스틸병(Adult-onset Still's Disease), 다발성근염(Polymyositis), 피부근염(Dermatomyositis) 및 베게너 육아종증(Wegener's Granulomatosis)으로 구성된 군에서 선택되는 하나 이상인 것인, TNF-α 관련 질환 예방 또는 치료용 약학 조성물.
- 히드로플루메티아지드 또는 이의 식품학적으로 허용 가능한 염을 유효성분으로 포함하는 TNF-α 관련 질환 예방 또는 개선용 건강 기능 식품 조성물.
- 히드로플루메티아지드 또는 이의 염을 사용하는 것을 포함하는 시험관 내(in vitro)에서 TNF-α의 발현 또는 활성을 억제하는 방법.
- 히드로플루메티아지드 또는 이의 염을 유효성분으로 포함하는 시험관 내 TNF-α의 발현 또는 활성 억제용 시약 조성물.
- TNF-α 관련 질환을 예방 또는 치료하기 위한 히드로플루메티아지드 또는 이의 약학적으로 허용 가능한 염을 유효성분으로 포함하는 조성물의 용도.
- TNF-α 관련 질환 예방용 또는 치료용 약제를 제조하기 위한 히드로플루메티아지드 또는 이의 약학적으로 허용 가능한 염을 유효성분으로 포함하는 조성물의 용도.
- 히드로플루메티아지드 또는 이의 약학적으로 허용 가능한 염을 유효성분으로 포함하는 조성물을 개체에 투여하는 단계를 포함하는 TNF-α 관련 질환의 예방 또는 치료방법.
