Abstract
本発明は、血液がん患者を治療又は寛解するための、高い効果を奏すると共に、副作用の少ない新たな手段を提供することを目的とするものであり、血液がん患者の治療又は寛解のための方法において使用するための、4-アミノ-1-(2-シアノ-2-デオキシ-β-D-アラビノフラノシル)-2(1H)-ピリミジノン又はその塩とベネトクラクス又はその塩とを含む組み合わせ医薬に関する。
Description
本発明は、血液がん患者の治療又は寛解に使用するための、4-アミノ-1-(2-シアノ-2-デオキシ-β-D-アラビノフラノシル)-2(1H)-ピリミジノンとベネトクラクスとの組み合わせ医薬に関する。
血液がんは、血液中の細胞が癌化した病気の総称であり、例えば、白血病や悪性リンパ腫、多発性骨髄腫等が挙げられる。今日、種々の抗腫瘍剤が開発され、血液がんは最早、不治の病でなくなったものの、病巣の骨髄からがん細胞を完全に除去するための治療は、例え、体力の有る若者であっても、耐え難い重篤な副作用を伴い易い。さらに、引き続いて骨髄移植が必要であるが、骨髄バンクに於ける骨髄保存量は十分でなく、適合する骨髄が中々見つからないのが現状である。
4-アミノ-1-(2-シアノ-2-デオキシ-β-D-アラビノフラノシル)-2(1H)-ピリミジノン(以下、「CNDAC」と記載する)は、デオキシシチジンのリボースの2’β位をシアノ基に置換した構造を有し代謝拮抗剤として利用される。CNDACは、細胞内デオキシシチジンキナーゼによりリン酸化され、トリリン酸化体を形成し、これがDNA鎖に取り込まれた後、細胞周期のG2/M期に於いて、CNDACの核酸塩基の糖の部位がβ-開裂し、DNA鎖を切断することが知られている(特許文献1、非特許文献1、2)。このため、CNDACは、臨床上有効な抗腫瘍剤であると考えられている(非特許文献3、特許文献2)。
ベネトクラクス(Venetoclax)は、Bcl-2に対し選択的に結合及び阻害する作用を有する。多くのがんにおいてBcl-2の過剰発現が認められ、これによりがん細胞は自然死や自己破壊(アポトーシス)の過程を阻止している。ベネトクラクスはこのBcl-2を阻害して、がん細胞におけるアポトーシスの過程を回復させる作用を有する。ベネトクラクスは、標準療法が無効の慢性リンパ性白血病(CLL)の治療剤としてFDAから承認され、臨床使用されている。また、ベネトクラクスは、急性骨髄性白血病(AML)の適用を持つ、アザシチジン、デシタビン、シタラビン等の他剤との併用療法が、未治療で高齢のAMLの患者の治療剤として2018年末に承認されている。

上述のとおり、依然として、血液がんの治療は耐え難い重篤な副作用を伴い易く、また、移植することが可能な適合骨髄を調達することは容易ではない。
そこで、本発明は、血液がん患者を治療又は寛解するための、高い効果を奏すると共に、副作用の少ない新たな手段を提供することを目的とする。
本発明者らは、上記課題を解決すべく鋭意検討した結果、CNDAC又はその塩とベネトクラクス又はその塩とを組み合わせて投与することで、その相乗効果により、血液がん患者を治療又は寛解できることを見出した。
すなわち、本発明は、以下の発明を包含する。
[1] 血液がん患者の治療又は寛解のための方法において使用するための、CNDAC又はその塩とベネトクラクス又はその塩とを含む組み合わせ医薬。
[2] 血液がんが急性骨髄性白血病(AML)、慢性骨髄性白血病(CML)、急性リンパ球性白血病(ALL)、慢性リンパ球性白血病(CLL)、骨髄増殖性腫瘍(MPN)、及びリンパ腫からなる群から選択される、[1]の医薬。

CNDAC又はその塩とベネトクラクス又はその塩との併用によりもたらされる相乗効果により、血液がんに対し高い治療又は寛解効果を得ることができる。
本発明は、血液がん患者にCNDAC又はその塩とベネトクラクス又はその塩との組み合わせを投与することを含む、血液がんの治療又は寛解のための方法、ならびに当該方法にて用いられる医薬又は組み合わせ医薬に関する。
本発明において「4-アミノ-1-(2-シアノ-2-デオキシ-β-D-アラビノフラノシル)-2(1H)-ピリミジノン」又は「CNDAC」とは、下記構造式:
本発明において「ベネトクラクス」とは、下記構造式:
CNDAC又はその塩、ならびに、ベネトクラクス又はその塩はそれぞれ、従来公知の手法に従って工業的に合成されたものであってもよいし、あるいは医薬品用として市販されるものを利用してもよい。
本発明において「血液がん」とは、造血器から発生するがんを意味する。このような血液がんとしては、急性骨髄性白血病(AML)、慢性骨髄性白血病(CML)、急性リンパ球性白血病(ALL)、慢性リンパ球性白血病(CLL)、骨髄増殖性腫瘍(MPN)、及びリンパ腫などが挙げられるが、これらに限定はされない。本発明において好ましくは、血液がんは急性骨髄性白血病(AML)である。
本発明において「患者」は血液がんの患者であればよく、特に限定はされない。患者は初発患者であっても、再発患者であってもよいが、好ましくは初発の患者である。ならびに/あるいは、患者の年齢は特に限定されず、乳幼児(5歳以下)、児童(6~14歳)、青年(15~34歳)、壮年(35~64歳)又は高齢(65歳以上)の患者であってもよいが、好ましくは高齢の患者である。
本発明において、CNDAC又はその塩、ならびに、ベネトクラクス又はその塩は、血液がん患者に対して併用投与される。
本発明において「併用投与」とは、各成分を同時に投与する場合だけではなく、治療期間にわたって各成分を、それぞれ所定の間隔で、順次投与する場合、各成分を同時に投与する前及び/又は後に、いずれかの成分のみを投与する場合も含まれる。併用投与される各成分の投与経路や投与手段は同一であってもよいし、異なっていてもよい。
CNDAC又はその塩の投与量及び投与経路は、血液がんの種類や重篤度、患者の年齢、体重、状態等などの要因に応じて変化し得るが、ベネトクラクス又はその塩との併用投与において血液がんを治療又は寛解するのに十分な量を任意の投与経路(経口投与、又は非経口投与)を用いて投与することができる。
例えば、CNDAC又はその塩を、CNDACの量にして1日当たり、患者の体表面積1m2に対し、1~10mg/m2/day、好ましくは4~6mg/m2/dayから選択される量にて、非経口投与することができる。「非経口投与」としては、静脈内注射、皮下注射、皮内注射、筋肉内注射、点滴等を挙げることができる。特に好ましくは、持続静脈内注射である。持続静脈内注射は上記の量を168~336時間かけて持続点滴投与することを意味する。投与間隔は特に限定されない。
また、ベネトクラクス又はその塩の投与量及び投与経路は、血液がんの種類や重篤度、患者の年齢、体重、状態等などの要因に応じて変化し得るが、CNDAC又はその塩との併用投与において血液がんを治療又は寛解するのに十分な量を任意の投与経路(経口投与、又は非経口投与)を用いて投与することができる。
例えば、ベネトクラクス又はその塩を、20~400mg/日、好ましくは200~400mg/日から選択される量にて、1日に1回~5回(例えば、2回もしくは3回)に分けて、毎日、隔日、数日おき、毎週1日、1月に2~3日、隔週1日、もしくは1月に1日、経口投与することができる。あるいは、静脈内におけるベネトクラクス又はその塩の量が上記経口投与された場合に匹敵する用量にて、ベネトクラクス又はその塩を非経口投与してもよい。「非経口投与」は上述の手法により行うことができる。
併用投与されるCNDAC又はその塩ならびにベネトクラクス又はその塩は、それぞれ別々の医薬(又は医薬組成物)の形態で提供されてもよいし、あるいは組み合わせ医薬の形態で提供されてもよい。
別々の医薬(又は医薬組成物)として提供される場合には、当該医薬(又は医薬組成物)は上記併用投与にて用いられることが意図されるものであり、その添付文書の「効能・効果」を示す欄、又は「用法・用量」を示す欄において、上記併用投与にて用いられることを記載することができる。例えば、CNDAC又はその塩を含む医薬(又は医薬組成物)であれば、その添付文書に血液がんの治療又は寛解のために「ベネトクラクス又はその塩と併用する」旨を記載することができ、ベネトクラクス又はその塩を含む医薬(又は医薬組成物)であれば、その添付文書に血液がんの治療又は寛解のために「4-アミノ-1-(2-シアノ-2-デオキシ-β-D-アラビノフラノシル)-2(1H)-ピリミジノン又はその塩と併用する」旨を記載することができる。
組み合わせ医薬は、各成分を同一の組成物中に含む配合剤の形態であってもよいし、あるいは各成分がそれぞれ別々に準備され、併用投与に適した単一のパッケージとして製造・包装・流通される形態(すなわち、キット製剤)としてもよい。
上記医薬(又は医薬組成物)及び組み合わせ医薬においては、上記成分に加えてさらに、医薬の製造において通常用いられている、賦形剤、結合剤、崩壊剤、滑沢剤等を含めることができ、企図される投与経路に適した剤型として製造することができる。
賦形剤としては、例えば、糖(単糖、二糖類、シクロデキストリン及びアルギン酸等の多糖類)、金属塩、カオリン、ケイ酸、ポリエチレングリコール及びこれらの混合物等が挙げられる。
結合剤としては、例えば、単シロップ、ブドウ糖液、デンプン液、ゼラチン溶液、ポリビニルアルコール、ポリビニルエーテル、ポリビニルピロリドン、カルボキシメチルセルロース、セラック、メチルセルロース、エチルセルロース、及びこれらの混合物等が挙げられる。
崩壊剤としては、例えば、乾燥デンプン、アルギン酸ナトリウム、カンテン末、ラミナラン末、炭酸水素ナトリウム、炭酸カルシウム、ポリオキシエチレンソルビタン脂肪酸エステル類、ラウリル硫酸ナトリウム、ステアリン酸モノグリセリド、デンプン、乳糖及びこれらの混合物等が挙げられる。
滑沢剤としては、例えば、精製タルク、ステアリン酸塩、ホウ砂、ポリエチレングリコール及びこれらの混合物等が挙げられる。
必要に応じて、さらに医薬の製造において通常用いられている希釈剤、安定化剤、等張化剤、pH調整剤、緩衝剤、溶解補助剤、懸濁化剤、着色剤、矯味剤、矯臭剤、コーティング剤、保存剤、防腐剤、抗酸化剤等も適宜含めることができる。
例えば、経口投与に適した剤形としては、錠剤、丸剤、カプセル剤、顆粒剤、粉剤、シロップ剤、懸濁剤等が挙げられる。固形の剤形を有するものは、必要に応じてコーティングを施すことができる(例えば、糖衣錠、ゼラチン被包錠、腸溶錠等)。
また、非経口投与に適した剤形として、注射剤や点滴剤等が挙げられる。これらの剤形は、凍結乾燥化し保存し得る状態で提供され、用時、水や生埋的食塩水等を含む緩衝液等で溶解して適当な濃度に調製した後に使用されるものであってもよい。
本発明は、血液がんを治療又は寛解するために用いられる抗がん剤や放射線療法と併用することができる。このような抗がん剤としては、例えば、アザシチヂン、エノシタビン、デシタビン、シタラビン、シタラビン・オクフォスフェート、フルダラビン、ペントスタチン、ネララビン、アクラルビシン、イダルビシン、ダウノルビシン、ドキソルビシン、サイクロフォスファミド、6-メルカプトプリン、メソトレキサート、ブサルファン、ナイムスチン、ラニムスチン、ビンクリスチン、ビンデシン、エトポシド、プレドニソロン、デキサメサゾン、レチノイン酸、タミバロテン、イマチニブ、ニロチニブ、ダサチニブ、ミトザントロン、ハイドロキシウレア、アスパラギナーゼ、亜ヒ酸、インターフェロン、レナリドマイド等が挙げられるが、これらに限定はされない。ここで「併用投与」とは、上記定義のとおりである。
抗がん剤は、血液がんの種類や重篤度、患者の年齢、体重、状態等などの要因に応じて、一又は複数種を適宜選択することができる。
本発明は、血液がんの治療又は寛解を目的として使用することができる。すなわち、本発明は、本発明の医薬(又は医薬組成物)及び組み合わせ医薬を血液がん患者に投与することを含む、血液がんの治療又は寛解の方法に関する。
本発明において「血液がんの治療又は寛解」とは、血液がんが完全に消失した状態を意味するだけでなく、一時的あるいは永続的に、血液がんが減少又は消失している状態や血液がんが進行(増悪)せず安定している状態も意味するものである。例えば、本発明における「がんの治療又は寛解」には、本発明の組成物の投与又は摂取前と比べて、患者における血液がん細胞数の低下、血液がんマーカーレベルの低下、血液がんに伴う症状の改善、全生存期間、無増悪生存期間、生存期間中央値等の尺度の延長等の一以上が含まれる。
本発明によれば、本発明の上記医薬(又は医薬組成物)及び組み合わせ医薬を投与することによって、血液がん患者を治療又は寛解することができる。特に、患者が初発及び/又は高齢の患者である場合においてその効果は顕著に認められ、その生存率(生存期間中央値等)を大きく高めることができる。
以下、本発明を実施例により具体的に説明するが、本発明はこれらに限定されるものではない。
実施例1:in vitroにおける4-アミノ-1-(2-シアノ-2-デオキシ-β-D-アラビノフラノシル)-2(1H)-ピリミジノン一塩酸塩(DFP-10917)とベネトクラクスの併用効果
ヒト由来の急性骨髄性白血病(AML)細胞株であるKG-1、SKM-1、OCI-AML-2、MV4-11、SKNO-1に対して、DFP-10917単独、ベネトクラクス単独、ならびにDFP-10917とベネトクラクス併用をそれぞれ各種濃度で作用させ、72時間後にCellTiter-Glo assayにより細胞数を計数し、細胞数を50%減少させる濃度であるIC50値を算出した。
結果、4種類のAML細胞(KG-1、OCI-AML-2、MV4-11、SKNO-1)で、DFP-10917とベネトクラクスの併用効果が認められた。
実施例2:タンパク質含量の低い条件下におけるDFP-10917とベネトクラクスの併用効果
ベネトクラクスのタンパク質結合率が高い(>99.9%)点を鑑み、タンパク質含量の低い条件にてDFP-10917とベネトクラクスの併用効果を調べた。

AML細胞株であるMV4-11に対して、DFP-10917単独、ベネトクラクス単独、ならびにDFP-10917とベネトクラクス併用をそれぞれ各種濃度で作用させ、48時間後にThiazolyl Blue Tetrazolium Bromideを用いたMTTアッセイにより細胞数を計数し、細胞数を50%減少させる濃度であるIC50値を算出した。
結果を図1に示す。DFP-10917添加群において(図1(A))、DFP-10917単独と比較してDFP-10917とベネトクラクス(96.8pM)との併用で若干の細胞増殖抑制効果の増強が見られた。IC50値はDFP-10917単独で1.53μMを示し、DFP-10917とベネトクラクスとの併用で0.51μMであった。
一方、ベネトクラクス添加群に於いて(図1(B))、ベネトクラクス単独と比較してベネトクラクスとDFP-10917(1.53μM)との併用で顕著な細胞増殖抑制効果の増強が認められた。IC50値はベネトクラクス単独で96.8pMを示し、ベネトクラクスとDFP-10917との併用で9.14pMであった。この結果は、ベネトクラクスとDFP-10917との併用により、ベネトクラクス単独と比べて、細胞増殖抑制効果がおよそ10.6倍増強したことを示す。また、各々のIC50値からより以下の式に基づいて、CI(Combination Index)値を算出した(F.Bruzzese,et al.Clin.Cancer Res.2006,12,617-625)。
CI値=併用時のDFP-10917のIC50値(0.51μM)/DFP-10917単独のIC50値(1.53μM)+併用時のベネトクラクスのIC50値(9.14pM)/ベネトクラクス単独のIC50値(96.8pM)+[DFP-10917単独のIC50値(1.53μM)×ベネトクラクス単独のIC50値(96.8pM)]/[併用時のDFP-10917のIC50値(0.51μM)+併用時のベネトクラクスのIC50値(9.14pM)]
CI値は1を基準として、CI値=1の場合は相加効果を示し、CI値<1の場合は相乗効果を示す。

上記式よりCI値はおよそ0.43となることから、DFP-10917とベネトクラクスの併用で得られる効果は相乗効果であることが確認された。
実施例3:動物モデルにおけるDFP-10917とベネトクラクスの併用効果
AML細胞株であるOCI-AML-2をNOD SCIDマウスに皮下移植し(5×106cells/mouse)、担がんモデルマウスを作製した。

DFP-10917は、Alzet(登録商標)osmotic mini-pumpをマウス体内に埋め込み、3mg/kg/日の用量で7日間持続投与した。
ベネトクラクスは100mg/kg/日の用量で14日間経口投与した。
対照にはvehicleのみを投与した。
投与は、移植後7日目から開始した。

結果を図2に示す。
DFP-10917とベネトクラクスとを併用投与した場合に、それぞれを単独で投与した場合と比べて、腫瘍の増殖がより抑えられていることが確認された。一方、試験期間にわたって、各マウスに有意な体重変化は認められず、DFP-10917とベネトクラクスとの併用による副作用(毒性)は確認されなかった(図3)。
DFP-10917とベネトクラクスとを併用投与した場合に、それぞれを単独で投与した場合と比べて、腫瘍の増殖がより抑えられていることが確認された。一方、試験期間にわたって、各マウスに有意な体重変化は認められず、DFP-10917とベネトクラクスとの併用による副作用(毒性)は確認されなかった(図3)。
Claims (11)
- 血液がん患者の治療又は寛解のための方法において使用するための、4-アミノ-1-(2-シアノ-2-デオキシ-β-D-アラビノフラノシル)-2(1H)-ピリミジノン又はその塩とベネトクラクス又はその塩とを含む組み合わせ医薬。
- 血液がんが急性骨髄性白血病(AML)、慢性骨髄性白血病(CML)、急性リンパ球性白血病(ALL)、慢性リンパ球性白血病(CLL)、骨髄増殖性腫瘍(MPN)、及びリンパ腫からなる群から選択される、請求項1に記載の医薬。
- 4-アミノ-1-(2-シアノ-2-デオキシ-β-D-アラビノフラノシル)-2(1H)-ピリミジノン又はその塩が非経口投与される、請求項1又は2に記載の医薬。
- ベネトクラクス又はその塩が経口投与される、請求項1~3のいずれか一項に記載の医薬。
- 患者が初発及び/又は高齢の患者である、請求項1~4のいずれか一項に記載の医薬。
- 血液がん患者の治療又は寛解のための方法において使用するための、ベネトクラクス又はその塩と併用投与される、4-アミノ-1-(2-シアノ-2-デオキシ-β-D-アラビノフラノシル)-2(1H)-ピリミジノン又はその塩を含む医薬組成物。
- 血液がん患者の治療又は寛解のための方法において使用するための、4-アミノ-1-(2-シアノ-2-デオキシ-β-D-アラビノフラノシル)-2(1H)-ピリミジノン又はその塩と併用投与されるベネトクラクス又はその塩を含む医薬組成物。
- 4-アミノ-1-(2-シアノ-2-デオキシ-β-D-アラビノフラノシル)-2(1H)-ピリミジノン又はその塩が非経口投与される、請求項6又は7に記載の医薬組成物。
- ベネトクラクス又はその塩が経口投与される、請求項6~8のいずれか一項に記載の医薬組成物。
- 血液がんが急性骨髄性白血病(AML)、慢性骨髄性白血病(CML)、急性リンパ球性白血病(ALL)、慢性リンパ球性白血病(CLL)、骨髄増殖性腫瘍(MPN)、及びリンパ腫からなる群から選択される、請求項6~9のいずれか一項に記載の医薬組成物。
- 患者が初発及び/又は高齢の患者である、請求項6~10のいずれか一項に記載の医薬組成物。
