Abstract
血液浄化デバイスの透析液等の液量を独立に設定し、複数のポンプの動作を管理し易くし、除水量を上げた条件下で、血液中の老廃物の除去性能や膜の寿命性能を評価することが可能な閉鎖系循環試験装置提供する。本発明に係る閉鎖系循環試験装置は、血液浄化デバイスと血液バッグと補液容器とがセットされ、大気非接触の閉鎖回路下で該デバイスの性能評価試験を行うための閉鎖系循環試験装置であって、該血液バッグから血液を血液ポンプを介して該血液浄化デバイスへと送る送血ライン;該血液浄化デバイスから出た血液を抵抗付与手段を介して該血液バッグへと送る返血ライン;該血液浄化デバイスの透析液出口から出た濾過液を濾過ポンプを介して該補液容器へと送る濾過液ライン;及び該補液容器から透析液又は補液を透析液ポンプを介して該血液浄化デバイスの透析液入口及び/又は該返血ラインへと送る透析液ライン;を具備することを特徴とする。
Description
本発明は、血液浄化デバイスの閉鎖系循環試験装置に関する。
急速に腎機能が低下する急性腎不全等の重症患者は、血液中に水分や尿素、クレアチニン、β2-ミクログロブリンなど(以下、血液老廃物)が貯留するため、持続的腎機能代替療法(CRRT)が施行され、血液から水分を除去すること(以下、「除水」と称する。)と、血液老廃物を除去することが行われる。このCRRT療法には、持続緩除式血液濾過(CHF)、持続緩除式血液透析(CHD)、持続緩除式血液濾過透析(CHDF)があり、CHDFではさらに補液を入れる場所の違いにより前希釈と後希釈に分類される。一般に、持続緩除式血液濾過器(以下、単に「血液濾過デバイス」と称する。)では、濾過量や除水量を上げると血液老廃物の除去性能は向上するものの、膜への負担が大きくなり、膜の寿命が短くなると言われている。従って、血液濾過デバイスには、濾過量や除水量を上げた条件下でも、膜の寿命が長いという製品の開発が求められる。しかしながら、これまでのin vitro試験では、CHF、CHD、CHDF(前希釈、後希釈)の各モードで、透析液、濾過液、補液(以下、透析液等という)の液量を自由に設定し、除水量を上げた条件で、「血液老廃物の除去性能評価」と「膜の寿命性能評価」を行うことができるin vitro試験装置やその試験方法は存在しない。
例えば、以下の非特許文献1には、血液濾過デバイスによる血液老廃物の除去性能評価を行う試験方法が記載されているが、血液が外空気に接触する開放系での評価方法であるため、実際の患者で用いられる閉鎖系の凝固条件より凝固が早く進むことが推察され、膜に血栓等が形成されやすく、膜の寿命性能を正確には評価できない。
また、以下の特許文献1には、閉鎖系での凝固条件でのCHFを行う試験方法が記載されているが、濾過液量と補液量が一定のため除水量を上げた検討ができない。さらに、実際臨床で多く使われているCHD、CHDF(前希釈、後希釈)のモードにも応用ができない。

前記した従来技術に鑑み、本発明が解決しようとする課題は、CHF、CHD、CHDF(前希釈、後希釈)の各モードで、透析液等の液量を独立に(自由に)設定でき、複数のポンプの動作を管理し易くし、「血液老廃物の除去性能評価」と「膜の寿命性能評価」を可能とした血液浄化デバイスの閉鎖系循環試験装置を提供することである。
本発明者らは、血液浄化デバイスの閉鎖系循環試験装置に補液容器(7)を設けることにより、CHF、CHD、CHDF(前希釈、後希釈)の各モードで、透析液等の液量を自由に設定でき、複数のポンプの動作を管理し易く、「血液老廃物の除去性能評価」と「膜の寿命性能評価」を可能とする試験装置が得られることを予想外に見出し、本発明を完成するに至ったものである。
すなわち、本発明は、以下のとおりのものである。
[1]血液浄化デバイス(1)と血液バッグ(2)と補液容器(7)とがセットされ、大気非接触の閉鎖回路下で該血液浄化デバイス(1)の性能評価試験を行うための閉鎖系循環試験装置(14)であって、
該血液バッグ(2)から血液を、血液ポンプ(5)を介して該血液浄化デバイス(1)へと送る送血ライン(3);
該血液浄化デバイス(1)から出た血液を、抵抗付与手段(6)を介して該血液バッグ(2)へと送る返血ライン(4);
該血液浄化デバイス(1)の透析液出口(1b)から出た濾過液を、濾過ポンプ(9)を介して該補液容器(7)へと送る濾過液ライン(8);及び
該補液容器(7)から透析液又は補液を、透析液ポンプ(11)を介して該血液浄化デバイス(1)の透析液入口(1a)及び/又は該返血ライン(4)へと送る透析液ライン(10);
を具備する閉鎖系循環試験装置(14)。

本発明に係る血液浄化デバイスの閉鎖系循環試験装置は、CHF、CHD、CHDF(前希釈、後希釈)の各モードで、透析液量等を独立に(自由に)設定し、複数のポンプの動作を管理し易くし、血液浄化デバイスによる「血液老廃物の除去性能評価」と「膜の寿命性能評価」を同時に可能にすることができる。
以下、本発明の実施形態について詳細に説明する。
本発明の1の実施形態は、血液浄化デバイス(1)と血液バッグ(2)と補液容器(7)とがセットされ、大気非接触の閉鎖回路下で該血液浄化デバイス(1)の性能評価試験を行うための閉鎖系循環試験装置(14)であって、
該血液バッグ(2)から血液を、血液ポンプ(5)を介して該血液浄化デバイス(1)へと送る送血ライン(3);
該血液浄化デバイス(1)から出た血液を、抵抗付与手段(6)を介して該血液バッグ(2)へと送る返血ライン(4);

図1に、本実施形態の血液浄化デバイスの閉鎖系循環試験装置の一の例を示す。図1中、閉鎖系循環試験装置(14)は、血液の濾過や透析等の際に血液中から老廃物を分離する血液浄化デバイス(1)に対し、患者への使用を伴わない非臨床環境により、実際の使用時における血流や血圧、濾液量や除水量の状況とほぼ同一の状況下で、膜寿命性能(ライフタイムとファウリング、抗血栓性等)の比較評価を行うことのできる試験用回路として構成されている。
本実施形態では、閉鎖系循環試験装置(14)で試験される血液浄化デバイス(1)として、持続緩徐式血液濾過法(CHF)による治療時に血液を濾過する血液濾過デバイスを対象とし、各種の血液濾過デバイスについて、膜寿命性能の比較試験を行うために閉鎖系循環試験装置(14)が用いられる。尚、閉鎖系循環試験装置(14)は、血液濾過デバイスに対する試験のための使用に限定されるものではなく、透析に用いられる血液透析デバイス等の各種の血液浄化デバイスの試験にも用いることができる(CHD)。
本実施形態では、閉鎖系循環試験装置(14)で試験される血液浄化デバイス(1)として、持続緩徐式血液濾過法(CHF)による治療時に血液を濾過する血液濾過デバイスを対象とし、各種の血液濾過デバイスについて、膜寿命性能の比較試験を行うために閉鎖系循環試験装置(14)が用いられる。尚、閉鎖系循環試験装置(14)は、血液濾過デバイスに対する試験のための使用に限定されるものではなく、透析に用いられる血液透析デバイス等の各種の血液浄化デバイスの試験にも用いることができる(CHD)。
血液浄化デバイス(1)としては、患者から脱血された血液の流入口となる入口側ポート(1c)と、入口側ポート(1c)から流入した血液が通過する中空糸膜を多数束ねてなる中空膜体(1e)と、中空膜体(1e)を通過した血液の器外への流出口となる出口側ポート(1d)と、中空膜体の外側に透析液の流入口となる入口側ポート(透析液入口ともいう。)(1a)と、中空膜体の外側に透過した老廃物を含む液体の排出口及び/又は透析液の流出口となる浄化ポート(透析液出口ともいう。)(1b)とを主として備えた公知の構造のものが、本実施形態で用いられる。
閉鎖系循環試験装置(14)は、大気非接触下において、血液浄化デバイス(1)を介して試験用液体が循環して流れる閉鎖回路として構成されており、試験用液体(血液)が流れる送血ライン(3)と返血ライン(4)、及び送血ライン(3)又は返血ライン(4)の途中に設けられ、並びに送血ライン(3)と返血ライン(4)内の試験用液体(血液)の流量や圧力を実使用環境に合わせるための抵抗付与手段(6)とを備えている。
本実施形態では、試験用液体として、ヒト血液(全血)が用いられるが、これに限定されるものではなく、ヒト血液に類する血液成分を有する動物の血液や人工血液等の他の液体を試験用液体として用いることもできる。さらに、血液に生理食塩水等で希釈した液体も使用可能である。
血液浄化デバイスの閉鎖系循環試験装置(14)に用いられる各ライン(血液回路ともいう。)には、特に限定されるものではないが、ポリ塩化ビニル製のチューブが用いられる。送血ライン(3)と返血ライン(4)の内部に収容される試験用液体の合計容量を約150mLとなる長さ及び径のものを用いることができる。
血液浄化デバイスの閉鎖系循環試験装置(14)に用いられる各ライン(血液回路ともいう。)には、特に限定されるものではないが、ポリ塩化ビニル製のチューブが用いられる。送血ライン(3)と返血ライン(4)の内部に収容される試験用液体の合計容量を約150mLとなる長さ及び径のものを用いることができる。
閉鎖系循環試験装置(14)は、血液浄化デバイス(1)の中空膜体(1e)の外側に透過した老廃物を含む液体の排出口及び/又は透析液の流出口となる透析液出口(1b)に接続され、濾過ポンプ(9)を介して中空膜体の外側に透過した老廃物を含む液体及び/又は中空膜体の外側を通過した濾過液を補液容器(7)へと送る濾過液ライン(8)と、該補液容器(7)に接続され、透析液ポンプ(11)を介して透析液又は補液を透析液入口(1a)(CHD)又は返血ライン(4)(CHF)へと送る透析液ライン(10)を含む。尚、透析液ポンプ(11)を介した透析液又は補液を透析液入口(1a)(CHD)又は返血ライン(4)(CHF)への移送は、例えば、三方弁(図示せず)により可能である。
本発明の他の実施形態は、補液容器(7)から透析液又は補液を、補液ポンプ(13)を介して前記送血ライン(3)及び/又は前記返血ライン(4)へと送る補液ライン(12)をさらに具備する(CHDF)、前記閉鎖系循環試験装置(14)である(図2参照)。補液ポンプ(13)から送血ライン(3)へと送るラインはCHDFの前希釈を模擬し、補液ポンプ(13)から返血ライン(4)へと送るラインはCHDFの後希釈を模擬している。尚、補液ポンプ(13)を介した送血ライン(3)及び/又は返血ライン(4)への移送は、例えば、三方弁(図示せず)により可能である。
血液浄化デバイスの閉鎖系循環試験装置に補液容器(7)を設けることにより、血液浄化デバイス(1)を用いて試験用液体(血液)から濾液及び/又は除水が可能となる。
図1に示す1の実施形態においては、血液浄化デバイスの閉鎖系循環試験装置(14)に補液容器(7)を設けることにより、濾過ポンプ(9)と透析液ポンプ(11)を同調して稼働することが可能である。除水は、濾過液ライン(8)を流れる流量が、透析液ライン(10)を流れる流量より多くすることで設定できる。
図2に示す他の実施形態においては、血液浄化デバイスの閉鎖系循環試験装置(14)に補液容器(7)を設けることにより、濾過ポンプ(9)、透析液ポンプ(11)、及び補液ポンプ(13)の3台のポンプを同調して稼働することが可能である。除水は、濾過液ライン(8)を流れる流量が、透析液ライン(10)と補液ライン(12)を流れる流量の合算より多くすることで設定できる。
図1に示す1の実施形態においては、血液浄化デバイスの閉鎖系循環試験装置(14)に補液容器(7)を設けることにより、濾過ポンプ(9)と透析液ポンプ(11)を同調して稼働することが可能である。除水は、濾過液ライン(8)を流れる流量が、透析液ライン(10)を流れる流量より多くすることで設定できる。
図2に示す他の実施形態においては、血液浄化デバイスの閉鎖系循環試験装置(14)に補液容器(7)を設けることにより、濾過ポンプ(9)、透析液ポンプ(11)、及び補液ポンプ(13)の3台のポンプを同調して稼働することが可能である。除水は、濾過液ライン(8)を流れる流量が、透析液ライン(10)と補液ライン(12)を流れる流量の合算より多くすることで設定できる。
血液バック(2)内の液量は特に限定はしないが、200mL以上であることが好ましい。血液バッグ(2)内の液量が200mL未満であると、本発明の他の実施形態において、除水時に血液バッグ(2)内の液量が不足し循環し難い傾向にある。さらに、血液バッグ(2)内の血液は、血球成分が下方に沈降しないように、スターラーやシェーカー等によって適度に攪拌することが望ましい。
補液容器(7)内の液量は100mL以上であることが好ましい。補液容器(7)内の液量が100mL未満であると、本発明の他の実施形態において、血液浄化デバイスの閉鎖系循環試験装置(14)の稼働時に補液容器(7)内の液量が不足し循環し難い傾向にある。
抵抗付与手段(6)は、送血ライン(3)又は返血ライン(4)に設けられる。尚、図1、2中、抵抗付与手段(6)は、返血ライン(4)に設けられている。
送血ライン(3)及び返血ライン(4)には、各々圧力計(21~23)が設けられ、血液浄化デバイス(1)の入圧と出圧を常時測定することが可能である。濾過液ライン(8)、透析液ライン(10)、及び補液ライン(12)にも、各々圧力計(25~27)が設けられ、各ライン内の圧力を常時測定することが可能である。
送血ライン(3)、返血ライン(4)、濾過液ライン(8)、透析液ライン(10)、及び補液ライン(12)には、各々流量計(28~32)が設けられ各ライン内の液がポンプ等の設定通りの流量であるかを確認可能である。
血液浄化デバイスの閉鎖系循環試験装置(14)は、該装置内の各種液体の温度を一定にするために、血液浄化デバイス(1)、血液バッグ(2)、補液容器(7)、及び装置全体を温調する恒温手段も備えて構成されていることが好ましい。例えば、内部に水が貯められた水槽と、水槽内の水温を所定温度に維持するヒータから成る恒温手段を例示できる。血液バッグ(2)と送血ライン(3)の一部を該水槽内に入れることにより、送血ライン(3)及び返血ライン(4)を流れる試験用液体を人間の体温(36~37℃程度)と同程度となる一定温度に維持することが可能である。同様に、補液容器(7)と、濾過液ライン(8)、透析液ライン(10)、及び補液ライン(12)の一部を該水槽内に入れることにより、濾過液ライン(8)、透析液ライン(10)、及び補液ライン(12)内を流れる透析液又は補液を人間の体温(36~37℃程度)と同程度となる一定温度に維持することが可能である。
血液浄化デバイスの閉鎖系循環試験装置(14)全体を密閉空間内に入れて、密閉空間を所定温度に維持するヒータから成る恒温手段も例示できる。
血液浄化デバイスの閉鎖系循環試験装置(14)の各構成要素は、実際の血液濾過時に用いられる血液浄化装置内で、血液等の回路内に設けられた各機器に高低差が設けられていることによる差圧の影響をも模擬するために、高低差を生じさせる位置関係となっており、当該高低差による重力の影響を加味できるようになっている。特に限定されものではないが、血液バッグ(2)は、血液浄化デバイス(1)のほぼ中間の高さに配置され、補液容器(7)の高さ位置も血液バッグ(2)の高さから、上700mmと下700mmとの間に配置されるのが好ましい。
血液浄化デバイスの閉鎖系循環試験装置(14)の各構成要素は、実際の血液濾過時に用いられる血液浄化装置内で、血液等の回路内に設けられた各機器に高低差が設けられていることによる差圧の影響をも模擬するために、高低差を生じさせる位置関係となっており、当該高低差による重力の影響を加味できるようになっている。特に限定されものではないが、血液バッグ(2)は、血液浄化デバイス(1)のほぼ中間の高さに配置され、補液容器(7)の高さ位置も血液バッグ(2)の高さから、上700mmと下700mmとの間に配置されるのが好ましい。
図1、2に示す実施形態では、送血ライン(3)を通る血液流量や、濾過液ライン(8)を通る濾過量と、透析液ライン(10)を通る透析量は特には限定されないが、臨床のCRRT療法を模擬するためには、送血ライン(3)を通る血液流量は、50mL/min~150mL/minの間が好ましく、濾過液ライン(8)を通る濾過流量と、透析液ライン(10)を通る透析流量は、その両者の流量の合計が500mL/hr~1500mL/hrの間が好ましい。
図1、2に示す実施形態では、血液ポンプ(5)及び濾過ポンプ(9)、透析液ポンプ(11)、補液ポンプ(13)として、ローラポンプが用いられているが、これに限らず、駆動が可能な限り種々のポンプに代替することも可能である。抵抗付与手段(6)は、人体の末梢抵抗を模擬して返血ライン(4)に絞り抵抗を付与するクランプ等によって構成されているが、これに限定されるものではなく、同様の作用を奏する限りにおいて、弁等の種々の機器を代替的に採用することができる。抵抗付与手段(6)は、人体の静脈を模擬して試験用液体の流れを調整する静脈モデルとして機能する。
採血ポート(24,24’)は、試験中における試験用液体のサンプル採取に用いられる。図1、2では、採血ポート(24)は送血ライン(3)に設置しているが、この部位に限定されず、返血ライン(4)にも設置することができる(採血ポート(24’)。

以下、前記血液浄化デバイスの閉鎖系循環試験装置(14)を用いた血液浄化デバイス(1)の血液老廃物の除去性能および膜劣化の評価試験方法について説明する。
本発明のさらに他の実施形態は、前記血液浄化デバイスの閉鎖系循環試験装置(14)において、透析液等の液量を調整することにより、血液浄化デバイスの血液老廃物の除去性能および膜劣化を評価する方法である。

試験対象となる血液浄化デバイス(1)を、前記血液浄化デバイスの閉鎖系循環試験装置(14)に設置(セット)した上で、試験用液体として血液を、送血ライン(3)及び返血ライン(4)に充填する。透析液又は補液を濾過液ライン(8)、透析液ライン(10)に充填する。補液ライン(12)を使用する場合には、透析液又は補液を補液ライン(12)に充填する。
血液ポンプ(5)及び濾過ポンプ(9)、透析液ポンプ(11)を駆動する。補液ライン(12)を使用する場合には、補液ポンプ(13)を駆動する。
実際の使用環境と同一となる前述の条件で血液ポンプ(5)により脈動流が付与された血液は、血液バッグ(2)から血液ポンプ(5)を通過し、血液浄化デバイス(1)の入口側ポート(1c)からその内部の中空膜体(1e)を通過する。中空膜体(1e)内を通過した血液は、出口側ポート(1d)から返血ライン(4)に流れ、抵抗付与手段(6)を通過し、血液バッグ(2)に戻される。
実際の使用環境と同一となる前述の条件で血液ポンプ(5)により脈動流が付与された血液は、血液バッグ(2)から血液ポンプ(5)を通過し、血液浄化デバイス(1)の入口側ポート(1c)からその内部の中空膜体(1e)を通過する。中空膜体(1e)内を通過した血液は、出口側ポート(1d)から返血ライン(4)に流れ、抵抗付与手段(6)を通過し、血液バッグ(2)に戻される。
試験中、採血ポート(24、24’)から血液がサンプルとして採取され、各種成分等が都度測定され、当該各種成分等の経時的変化に関するデータも取得される。このデータを用いることで、血液浄化デバイス(1)による血液老廃物の除去性能は、例えば、前記した非特許文献1に記載される方法に準じてクリアランスを算出することにより評価することができる。
試験中、血液浄化デバイス(1)の膜寿命性能はライフタイムで評価できる。血液ポンプ(5)は、実際の使用環境に相当する時間(例えば、3時間から3日間程度)継続して駆動される。この間、各圧力計21~23、25~27や各流量計28~32が定期的にモニタリングされ、血液浄化デバイス(1)の入口圧力、出口圧力やそれらの差圧等の経時的変化に関するデータが取得される。取得されたデータから、液浄化デバイス(1)のライフタイムを求めることが可能である。血液浄化デバイス(1)の入口圧力が初期値(例えば、70mmHg)から、所定値(例えば、150mmHg)まで上昇した際は、前述の駆動時間が経過していなくても、血液ポンプ(5)、濾過ポンプ(9)、透析液ポンプ(11)、補液ポンプ(13)が停止され、試験終了となり、試験開始時からの経過時間もデータとして取得される。
試験中、血液浄化デバイス(1)の膜寿命性能は、透析膜の目詰まり(ファウリング)でも評価できる。例えば、「(除水量)/(限外濾過圧(TMP))/(透析時間)」で算出される限外濾過率を用いて評価することができる。ここでのTMPは,血液側と透析液側の圧力差を表している。
試験終了後、血液浄化デバイス(1)の寿命性能は抗血栓性でも評価できる。抗血栓性は、例えば、前記した特許文献1に記載される方法に準じて評価することができる。この方法により、血液の流路となる中空糸膜内の経時的な血栓の形成や、当該血栓の形成による血液流路の目詰まりが評価できる。
このように、本実施形態によれば、大気非接触で、所望の流量及び圧力と試験用液体の成分とを維持しながら血液浄化デバイスの評価試験を行うことができ、患者を介さない非臨床環境下において、患者を介した実際の使用環境で行うのとほぼ同一の試験を行えるという格別顕著、有利な効果が奏される。
尚、本発明に係る装置の各部(各要素)の構成は図示構成例に限定されるものではなく、実質的に同様の作用を奏する限りにおいて、種々の変更が可能である。

本発明に係る血液浄化デバイスの閉鎖系循環試験装置は、透析液等の液量を独立に(自由に)設定し、複数のポンプの動作を管理し易いため、「血液老廃物の除去性能評価」と「膜の寿命性能評価」に好適に利用可能である。

Claims (4)
- 血液浄化デバイス(1)と血液バッグ(2)と補液容器(7)とがセットされ、大気非接触の閉鎖回路下で該血液浄化デバイス(1)の性能評価試験を行うための閉鎖系循環試験装置(14)であって、
該血液バッグ(2)から血液を、血液ポンプ(5)を介して該血液浄化デバイス(1)へと送る送血ライン(3);
該血液浄化デバイス(1)から出た血液を、抵抗付与手段(6)を介して該血液バッグ(2)へと送る返血ライン(4);
該血液浄化デバイス(1)の透析液出口(1b)から出た濾過液を、濾過ポンプ(9)を介して該補液容器(7)へと送る濾過液ライン(8);及び
該補液容器(7)から透析液又は補液を、透析液ポンプ(11)を介して該血液浄化デバイス(1)の透析液入口(1a)及び/又は該返血ライン(4)へと送る透析液ライン(10);
を具備する閉鎖系循環試験装置(14)。 - 前記補液容器(7)から透析液又は補液を、補液ポンプ(13)を介して前記送血ライン(3)及び/又は前記返血ライン(4)へと送る補液ライン(12);
をさらに具備する、請求項1に記載の閉鎖系循環試験装置(14)。 - 請求項1又は2に記載の閉鎖系循環試験装置において、透析液、濾過液、補液の各液流量を独立に設定し調整することにより、血液浄化デバイスによる血液老廃物の除去性能評価と膜の寿命性能評価を同時に可能とする方法。
- 濾過液流量を透析液と補液の合算流量より多くする除水条件下で実施する、請求項3に記載の方法。
