Abstract
Description
本発明が解決しようとする別の課題は、長期保存における粘度の低下が抑制され、患者の使用感に変動がなく、かつ、医薬品として安定な製剤特性を有するソフピロニウム臭化物の外用塗布用の非水製剤または低含水製剤を提供することである。
本発明が解決しようとするさらに別の課題は、アセチルコリンが関与する疾患(例えば、原発性局所多汗症等)に治療効果を示すソフピロニウム臭化物の外用製剤を提供することである。
先に述べたように、水溶性高分子を含有する非水製剤または低含水製剤において、経時的な粘度低下の現象は報告例がなく、ソフピロニウム臭化物の非水製剤において、そのような経時的な粘度低下の現象が生じることは全くの想定外であった。経時的な製剤粘度の低下は、患者の使用感等に影響を与えるため、この問題を回避することが望まれる。そこで、本発明者らは、当業界において従来知られていない上記の課題を解決するために鋭意研究を行った。

本発明の製剤は、ソフピロニウム臭化物を有効成分とする局所投与製剤である。
本発明の製剤中に含まれるソフピロニウム臭化物の含有量としては、特に限定されないが、1w/w%から30w/w%が好ましく、より好ましくは1w/w%から20w/w%であり、より一層好ましくは5w/w%から15w/w%である。
本発明の一実施態様において、特に好ましいソフピロニウム臭化物の含有量は、全製剤量に対して、5w/w%である。
本発明の別の実施態様において、特に好ましいソフピロニウム臭化物の含有量は、全製剤量に対して、10w/w%である。
本発明の別の実施態様において、特に好ましいソフピロニウム臭化物の含有量は、全製剤量に対して、15w/w%である。
なお、本明細書中、「AからB」、「A-B」あるいは「A~B」などと、範囲を記載する場合は、特に注記がなければ、その末端の数値も含む。
本発明の製剤は、好ましくは液剤又はゲル剤であり、より好ましくは液剤である。

本発明の別の実施態様において、本発明の製剤は、体部に薬剤を塗布するための外用液剤である。
一般に原発性の局所性多汗症では、頭部、顔面、手掌、足底、腋窩などに左右対称に過剰な発汗が生じる。そのため、本発明の製剤は、好ましくは両腋窩、両手掌に塗布するための外用製剤であるが、片側腋窩又は片側手掌などに過剰な発汗が生じる場合には、本発明の製剤を当該片側腋窩又は片側手掌などに適用することもできる。
本発明の製剤は、製剤の組成が均一に分散された製剤であり、通常の保存条件下で、油滴等が発生せず、安定的に保存される。
本発明の製剤は、好ましくは均一に溶解された製剤であり、より好ましくは均一に溶解され、かつ、澄明な製剤である。
さらに、本発明の製剤は、医薬品製剤規格を逸脱するような、経時的な変色、変質、有効成分の含量の大きな増減、類縁物質の大きな増加も生じず、微生物学的品質にも問題がなく、医薬品の製剤として好ましい。
本明細書中、「非水製剤」とは、製剤中の含水率が、0w/w%であるか、実質的に水を含まない製剤を意味する。
本明細書中、「実質的に水を含まない製剤」とは、例えば、製剤中の含水率が、1w/w%以下の製剤を意味する。
本明細書中、「低含水製剤」とは、製剤中の含水率が、20w/w%以下の製剤を意味する。
本発明の一実施態様において、本発明の製剤の含水率は、好ましくは0.001w/w%から10w/w%であり、より好ましくは0.001w/w%から5w/w%であり、さらに好ましくは0.001w/w%から3w/w%である。
本発明の別の実施態様において、本発明の製剤は、非水製剤又は含水率が5w/w%以下の低含水製剤が好ましく、非水製剤又は含水率が3w/w%以下の低含水製剤がより好ましく、非水製剤又は含水率が2w/w%以下の低含水製剤がさらに好ましく、非水製剤又は含水率が1w/w%以下の低含水製剤がより一層好ましく、非水製剤が特に好ましい。
本発明の別の実施態様において、本発明の製剤は、含水率が0.001w/w%から5w/w%の低含水製剤が好ましく、含水率が0.002w/w%から3w/w%の低含水製剤がより好ましく、含水率が0.005w/w%から2w/w%の低含水製剤がさらに好ましく、含水率が0.01w/w%から1w/w%の低含水製剤がより一層好ましい。
好ましい水溶性高分子は、水溶性高分子を全製剤量に対して1.25w/w%含有する非水製剤の場合において、25℃における粘度が、2.0mPa・sから2000mPa・sの範囲となる水溶性高分子である。水溶性高分子は、同様の条件下で、粘度が5.0mPa・sから1500mPa・sの範囲内の水溶性高分子が好ましく、粘度が10mPa・sから1000mPa・sの範囲内の水溶性高分子がさらに好ましく、粘度が100mPa・sから800mPa・sの範囲内となる水溶性高分子がより一層好ましい。
セルロース系高分子の具体例としては、ヒドロキシアルキルセルロース(例えばヒドロキシメチルセルロース(HMC)、ヒドロキシエチルセルロース(HEC)、ヒドロキシプロピルセルロース(HPC)、ヒドロキシブチルセルロース)、ヒドロキシアルキルアルキルセルロース(例えばヒドロキシエチルメチルセルロース(HEMC)、ヒドロキシプロピルメチルセルロース(HPMC))、アルキルセルロース(例えばメチルセルロース)、カルボキシメチルセルロース、セルロースエステル(酢酸セルロース)などが挙げられる。
アクリレートポリマー系高分子の具体例としては、アミノアルキルメタクリレートコポリマー(例えばアミノアルキルメタクリレートコポリマーRS)、アクリル酸エチル・メタクリル酸メチル共重合体などが挙げられる。
本発明の一実施態様において、セルロース系高分子は、好ましくはHMC、HEC、HPC、HEMC、HPMC、メチルセルロース、エチルセルロース、カルボキシメチルセルロースであり、さらに好ましくは、HEC、HPC、又はHPMCであり、より一層好ましくはHPCである。
本発明の別の実施態様において、セルロース系高分子は、好ましくはヒドロキシアルキルセルロース又はヒドロキシアルキルアルキルセルロースであり、より好ましくは、ヒドロキシC2-C4アルキルセルロース、又はヒドロキシC2-C4アルキルC1-C4アルキルセルロースであり、さらに好ましくはヒドロキシC2-C4アルキルセルロースである。
「ヒドロキシC2-C4アルキルセルロース」は、ヒドロキシアルキル基の炭素数が2個から4個であるヒドロキシアルキルセルロースを指す。具体例としては、ヒドロキシエチルセルロース(HEC)やヒドロキシプロピルセルロース(HPC)などが挙げられる。
「ヒドロキシアルキルアルキルセルロース」とは、セルロースの水酸基に多数のアルキル基及びヒドロキシアルキル基が置換されたセルロースを指す。「ヒドロキシC2-C4アルキルC1-C4アルキルセルロース」は、ヒドロキシアルキル基の炭素数が2個から4個であり、アルキル基の炭素数が1個から4個であるヒドロキシアルキルアルキルセルロースを指す。これらの具体例としては、ヒドロキシプロピルメチルセルロース(HPMC)などが挙げられる。
本明細書中、「エタノール」とは、種々のグレードのエタノールを包含する用語であり、例えば、無水エタノール、及び95%エタノールを包含する。
本発明の一実施態様において、好ましいエタノールは、95%エタノールである。
本発明の別の実施態様において、好ましいエタノールは、無水エタノールである。
本発明の一実施態様において、エタノールの好ましい含有量は、全製剤量に対して30w/w%から95w/w%であり、より好ましくは50w/w%から90w/w%であり、さらに好ましくは60w/w%から85w/w%であり、より一層好ましくは70w/w%から85w/w%である。
本発明の別の実施態様において、エタノールの好ましい含有量は、全製剤量に対して60w/w%から95w/w%であり、より好ましくは60w/w%から90w/w%であり、さらに好ましくは60w/w%から85w/w%である。
非水溶媒用pH電極には、例えば、低電気伝導率水・非水溶媒用pH電極(低電気伝導率水と非水溶媒の両方に用いることができるpH電極)が含まれる。
本明細書中、例えば、pHは、pH標準液を用いて校正された低電気伝導率水・非水溶媒用pH電極を被験製剤10.0gに5分間浸した後の値を指す。
低電気伝導率水・非水溶媒用pH電極を用いたpH測定については、第十七改正日本薬局方解説書(廣川書店)に解説があり、本試験では日本薬局方に準じて測定した。低電気伝導率水・非水溶媒用pH電極は、例えば株式会社堀場アドバンスドテクノなどから市販品を容易に入手することができる。pHの測定は、上記電極を用いて、例えば、0~60℃程度の範囲で行うことができ、好ましくは1~30℃、更に好ましくは20~30℃で行うことができる。
ここで、「pH電極を浸す」とは、pHが正確に測定できるように、pH電極の液絡部が被験製剤に完全に浸かる状態を指す。
本発明の一実施態様において、本発明の製剤は、調液時のpHが5.2以下であり、調液して1か月後のpHが5.2以下であり、調液して2か月後のpHが5.2以下であり、調液して3か月後のpHが5.2以下であり、調液して6か月後のpHが5.2以下であり、調液して12か月後のpHが5.2以下であり、調液して18か月後のpHが5.2以下であり、調液して24か月後のpHが5.2以下であり、又は調液して36か月後のpHが5.2以下である。
本発明の一実施態様において、本発明の製剤は、調液時のpHが2.5から5.2の範囲であり、調液して1か月後のpHが2.5から5.2の範囲であり、調液して2か月後のpHが2.5から5.2の範囲であり、調液して3か月後のpHが2.5から5.2の範囲であり、調液して6か月後のpHが2.5から5.2の範囲であり、調液して12か月目のpHが2.5から5.2の範囲であり、調液して18か月目のpHが2.5から5.2の範囲であり、調液して24か月後のpHが2.5から5.2の範囲であり、又は調液して36か月後のpHが2.5から5.2の範囲である。
本発明の一実施態様において、本発明の製剤は、室温で製剤が保存される場合の、調液時のpHが2.5から5.2の範囲であり、調液して1か月後のpHが2.5から5.2の範囲であり、調液して2か月後のpHが2.5から5.2の範囲であり、調液して3か月後のpHが2.5から5.2の範囲であり、調液して6か月後のpHが2.5から5.2の範囲であり、調液して12か月後のpHが2.5から5.2の範囲であり、調液して18か月後のpHが2.5から5.2の範囲であり、調液して24か月後のpHが2.5から5.2の範囲であり、又は調液して36か月後のpHが2.5から5.2の範囲である。
上記の各態様において、いずれかの時点におけるpHが上記条件を満足すればよいが、2以上の時点におけるpHが条件を満たすことが好ましく、特に好ましいのは全ての時点におけるpHが上記条件を満たす場合である。
本発明の一実施態様において、本発明の製剤のpHは、調液してから6か月間で選択される1又は複数のいずれかの時点で測定され、かつ、調液から測定までの間、室温で保存された製剤のpHを測定することによって決定されるpHである。
本発明の一実施態様において、本発明の製剤のpHは、調液してから1か月間で選択される1又は複数のいずれかの時点で測定され、かつ、調液から測定までの間、室温で保存された製剤のpHを測定することによって決定されるpHである。
本発明の製剤のpHは、5.2以下であり、好ましくは2.5から5.2であり、より好ましくは、3.0から5.2であり、さらに好ましくは3.0から5.0である。
本発明の一実施態様において、調液してから6か月間で選択される1又は複数のいずれかの時点で測定され、かつ、調液から測定までの間、室温で保存された製剤のpHを測定することによって決定されるpHは、5.2以下であり、好ましくは2.5から5.2であり、より好ましくは、3.0から5.2であり、さらに好ましくは3.0から5.0である。
なお、本明細書において、室温とは、1℃から30℃を指す。
本発明の一実施態様において、より好ましい本発明の製剤は、2.5から5.0のpHに維持され、さらに好ましい本発明の製剤は、2.5から4.5のpHに維持される。
本発明の別の実施態様において、好ましい本発明の製剤は3.0から5.2のpHに維持され、より好ましい本発明の製剤は3.0から5.0のpHに維持され、さらに好ましい本発明の製剤は3.0から4.5のpHに維持される。
本発明の一実施態様において、本発明の製剤は、調液してから40℃で3か月保存した場合に、調液してから3か月間のpHが2.5から5.2に維持され、好ましくは2.5から5.0に維持され、より好ましくは2.5から4.5に維持される。
本発明の別の実施態様において、本発明の製剤は、調液してから室温で36か月保存した場合に、調液後36か月間pHが5.2以下に維持され、好ましくは5.0以下に維持され、より好ましくはpHが4.8以下に維持される。
本発明の別の実施態様において、本発明の製剤は、調液してから室温で36か月保存した場合に、調液してから36か月間pHが2.5から5.2に維持され、好ましくは3.0から5.2に維持され、より好ましくは3.0から5.0に維持される。
本発明の別の実施態様において、本発明の製剤は、調液してから室温で24か月保存した場合に、調液してから24か月間pHが5.2以下に維持され、好ましくは5.0以下に維持され、より好ましくは4.8以下に維持される。
本発明の別の実施態様において、本発明の製剤は、調液してから室温で24か月保存した場合に、調液してから24か月間pHが2.5から5.2に維持され、好ましくは3.0から5.2に維持され、より好ましくは3.0から5.0に維持される。
本発明の別の実施態様において、本発明の製剤は、調液してから室温で12か月保存した場合に、調液してから12か月間pHが5.2以下に維持され、好ましくは5.0以下に維持され、より好ましくは4.8以下に維持される。
本発明の別の実施態様において、本発明の製剤は、調液してから室温で12か月保存した場合に、調液してから12か月間pHが2.5から5.2に維持され、好ましくは3.0から5.2に維持され、より好ましくは3.0から5.0に維持される。
本発明の別の実施態様において、本発明の製剤は、調液してから室温で6か月保存した場合に、調液してから6か月間pHが5.2以下に維持され、好ましくは5.0以下に維持され、より好ましくは4.8以下に維持される。
本発明の別の実施態様において、本発明の製剤は、調液してから室温で6か月保存した場合に、調液してから6か月間pHが2.5から5.2に維持され、好ましくは3.0から5.2に維持され、より好ましくは3.0から5.0に維持される。

無機酸塩とは、例えば、塩酸アンモニウム、炭酸カリウム、リン酸一水素ナトリウム、リン酸二水素ナトリウム等を指す。
具体的には、有機酸とは、酢酸、プロピオン酸、トリフルオロ酢酸、安息香酸、マレイン酸、フマル酸、コハク酸、タンニン酸、酪酸、吉草酸、ヒベンズ酸、パモ酸、エナント酸、タルトロン酸、デカン酸、テオクル酸、サリチル酸、α-ヒドロキシ酸、アミノ酸、シュウ酸等の有機カルボン酸、メタンスルホン酸、ベンゼンスルホン酸、p-トルエンスルホン酸等の有機スルホン酸等を挙げることができる。
アミノ酸の具体例としては、グリシン、アラニン、グルタミン酸、アスパラギン酸、フェニルアラニン、β-アラニン、イソロイシン、ロイシン、プロリン、グルタミン、セリン、トレオニン、バリン、トリプトファン、及びチロシン等を挙げることができる。
なお、本明細書中、「クエン酸」と記載した場合は、無水クエン酸及びクエン酸水和物等を含む。
本発明の一実施態様において、好ましいpH調整剤は有機酸又はその塩であり、より好ましいpH調整剤はα-ヒドロキシ酸又はその塩であり、さらに好ましいpH調整剤はクエン酸若しくは酒石酸、又はその塩であり、より一層好ましいpH調整剤は無水クエン酸若しくはD-酒石酸、又はその塩であり、特に好ましいpH調整剤は無水クエン酸である。
本発明の一実施態様において、好ましいpH調整剤は、全製剤量に対して0.015w/w%から5w/w%の無水クエン酸が好ましく、より好ましくは0.015w/w%から1.0w/w%の無水クエン酸であり、より一層好ましくは0.015w/w%から0.2w/w%の無水クエン酸であり、特に好ましくは0.015w/w%から0.075w/w%であり、さらに一層好ましくは0.05w/w%から0.075w/w%の無水クエン酸である。
本明細書中、「不揮発性オイル」とは、薬学的に許容される不揮発性の液状又はゲル状の基剤であり、具体的には、不揮発性エステル、不揮発性シリコーン、不揮発性アルコール、不揮発性脂肪酸、及び不揮発性エーテル等が包含される。
不揮発性オイルは、医薬品の添加物として使用可能で、エタノールとともに、均一に分散又は溶解されたソフピロニウム臭化物製剤として組成でき、かつ、塗布後に不快な使用感を与えないものであれば特に限定されない。
本発明の好ましい不揮発性オイルは、不揮発性エステル、不揮発性シリコーン、又は不揮発性エーテルであり、より好ましくは不揮発性エステル又は不揮発性シリコーンである。
不揮発性エステルとは、一分子中にエステル基(-COO-)を1つ又は複数有し、揮発性を有さないエステルオイルを指す。
本発明の好ましい不揮発性エステルとしては、炭素数4以上の直鎖状又は分岐鎖状のアルキル基を有するエステルが挙げられる。
本発明における不揮発性エステルは、モノエステル、ジエステル、及びトリエステルの全てを包含する。
なお、「モノエステル」とは、一分子中にエステル基を1つ有するエステルを指し、「ジエステル」とは、一分子中にエステル基を2つ有するエステルを指し、「トリエステル」とは、一分子中にエステル基を3つ有するエステルを指す。
本明細書中、単に「モノエステル」、「ジエステル」、及び「トリエステル」と記載した場合、不揮発性モノエステル、不揮発性ジエステル、及び不揮発性トリエステルを指す。
本発明の別の実施態様において、好ましい不揮発性オイルは、R1COOR2で表され、R1が、水酸基若しくはC1-C4アルキルオキシカルボニル基で置換されていてもよい、C4-C40直鎖アルキル基又はC4-C40分岐鎖アルキル基であり、かつ、R2が、水酸基若しくはC1-C40アルキルオキシカルボニル基で置換されていてもよいC1-C4アルキル基である、モノエステル、ジエステル、又はトリエステルからなる群より選択される不揮発性エステルである。
「C4-C40分岐鎖アルキル基」とは、炭素原子を4から40個有する分岐鎖のアルキル基を指す。
「C1-C4アルキル基」とは、炭素原子を1から4個有するアルキル基を指し、直鎖アルキル基及び分岐鎖アルキル基を含む。具体的には、メチル基、エチル基、ノルマルプロピル基、イソプロピル基、ノルマルブチル基、イソブチル基、sec-ブチル基、tert-ブチル基等を指す。
「C1-C40アルキル基」とは、炭素原子を1から40個有するアルキル基を指し、C4-C40直鎖アルキル基、C4-C40分岐鎖アルキル基、C1-C4アルキル基を含む。
「C1-C40アルキルカルボニルオキシ基」とは、アルキル部分が、前記C1-C40アルキル基であるアルキルカルボニルオキシ基を意味し、ノルマルブチルカルボニルオキシ基、ノルマルヘキシルカルボニルオキシ基、ノルマルへプチルカルボニルオキシ基、ノルマルオクチルカルボニルオキシ基等を意味する。
本発明のより好ましいモノエステルは、ミリスチン酸イソプロピルである。
本発明のより好ましいジエステルは、アジピン酸ジイソプロピルである。
本発明のより好ましいトリエステルは、中鎖脂肪酸トリグリセリドである。
本発明の好ましい中鎖脂肪酸トリグリセリドは、トリ(カプリル酸/カプリン酸)グリセリドである。
好ましい不揮発性シリコーンの具体例としては、ジメチコン、シラスティック(登録商標)、医療等級のシリコーン油、メチルフェニルシリコーン、メチルハイドロジェンシリコーン、デカメチルペンタシクロシロキサン、オクタメチルテトラシクロシロキサン、ジメチルポリシロキサン、メチルフェニルポリシロキサン、シクロメチコン5-NF、PEG-12ジメチコン、ジメチコン20cSt、ジメチコン100cSt、ジメチコン350cSt、ジメチコン500cSt、ジメチコン1000cSt、及びジメチコン12500cSt等が挙げられる。
本発明のより好ましい不揮発性シリコーンとしては、シクロメチコン5-NF、PEG-12ジメチコン、ジメチコン20cSt、又はジメチコン350cStが挙げられる。
本発明の製剤が、2種以上の不揮発性オイルを含有する場合、上記の好ましい不揮発性エステルから選択された2種以上の不揮発性エステルを含有する製剤は好ましい。
例えば、本発明の製剤は、2種のモノエステル、モノエステルとジエステル、モノエステルとトリエステル、2種のジエステル、ジエステルとトリエステル、2種のトリエステル、モノエステルと不揮発性シリコーン、ジエステルと不揮発性シリコーン、トリエステルと不揮発性シリコーン、又は2種の不揮発性シリコーンを含有していてもよい。
本発明の一実施態様において、より好ましい不揮発性オイルは、全製剤量に対して0.5w/w%から10w/w%の、ミリスチン酸イソプロピル、アジピン酸ジイソプロピル、又は中鎖脂肪酸トリグリセリドである。
本発明の一実施態様において、さらに好ましい不揮発性オイルは、全製剤量に対して1.0w/w%から5.0w/w%の、ミリスチン酸イソプロピル、アジピン酸ジイソプロピル、又は中鎖脂肪酸トリグリセリドである。
本明細書中、「多価アルコール」とは、炭化水素若しくはエーテルの任意の位置において、複数の水素原子が水酸基に置換された化合物を指す。
多価アルコールは、医薬品の添加物として使用可能であれば特に限定されないが、例えば、へキシレングリコール(HG)、プロピレングリコール(PG)、エチレングリコール、グリセロール、ブチレングリコール(BG)、グリセリン等が挙げられる。
本発明の一実施態様において、好ましい多価アルコールは、C2-C10の炭化水素の任意の位置において2又は3の水素原子が水酸基に置換された化合物である。
本発明の一実施態様において、好ましい多価アルコールは、C2-C6の炭化水素の任意の位置において2又は3の水素原子が水酸基に置換された化合物である。
本発明の一実施態様において、好ましい多価アルコールは、へキシレングリコール、ブチレングリコール、又はグリセリンである。
本発明の一実施態様において、より好ましい多価アルコールは、全製剤量に対して1.0w/w%から30w/w%の、へキシレングリコール、ブチレングリコール、又はグリセリンである。
本発明の一実施態様において、さらに好ましい多価アルコールは、全製剤量に対して2.0w/w%から20/w%の、へキシレングリコール、ブチレングリコール、又はグリセリンである。
本明細書における「製剤の粘度」とは、特に注記が無ければ、25℃における粘度を意味する。粘度は、一般的には日本薬局方の一般試験法において説明された方法のほか、当業者において通常使用されている試験法により容易に測定することができる。例えば、以下の実施例で示す粘度測定方法により計測することができる。
本発明の一実施態様において、本発明の製剤の粘度は、25℃における粘度が10 mPa・sから2000 mPa・sが好ましく、より好ましくは粘度が100 mPa・sから1500 mPa・sである。
本発明の別の実施態様において、本発明の製剤の粘度は、25℃における粘度が10 mPa・sから1000 mPa・sが好ましく、より好ましくは粘度が100 mPa・sから800 mPa・sである。
本発明の別の実施態様において、本発明の製剤の粘度は、25℃における粘度が10 mPa・sから800 mPa・sが好ましく、より好ましくは粘度が50 mPa・sから800 mPa・sである。
本発明の一実施態様において、本明細書において経時的な粘度の低下は、調液してから室温で6か月間保存した後の粘度、室温で12か月保存した後の粘度、室温で24か月保存した後の粘度、又は室温で36か月保存した後の粘度が、調液時の粘度に対し、低値を示す現象を指す。特に記載がなければ、粘度は25℃で測定した値を指す。

本発明の一実施態様において、本発明の製剤は、原発性腋窩多汗症の処置、治療、又は予防に用いることができる。

ソフピロニウム臭化物の様々な非水製剤における加速試験(1)
<調液方法>
表中の構成成分と濃度となるように、配合成分を無水エタノールに攪拌・溶解し製剤を得た。本法で製造した各製剤の内容を下表に示す。
** q.s. to 100: 全量が100%となるように、残分は無水エタノールで構成される。
粘度計を25℃、毎分5 rpm、プレヒート時間を30秒に設定し、本品約1 mLをコーンロータ:R-H1°34’×R24で200秒回転させた後の値を測定した(日本薬局方粘度測定法第2法)。
pHの値のばらつきを抑制するには、被験製剤の量がpH電極からの内部液に対し一定範囲である必要がある。本試験例のpH測定において、被験製剤の量は10.0gである。
pH電極の校正には、シュウ酸塩pH標準液、フタル酸塩pH標準液、又はリン酸塩pH標準液を用いた。校正に用いたpH標準液と被験製剤との温度差は±2℃であり、pHの測定時の製剤の温度は、20~30℃の範囲であった。
マルエム(登録商標)No.4瓶に被験製剤10.0 gを量り、pH標準液を用いて校正された低電気伝導率水・非水溶媒用pH電極を当該製剤に5分間浸した後の値を測定した。
調液時の粘度に対して、調液してから40℃で3か月保存後における粘度の増減率(%)を計算し、±30%以内の場合を「A」、±30%を超える場合を「B」と判定した。結果を下表に示す。
なお、下表のpHは、保存期間(即ち、調液してから3か月目まで)における最高値を示した。
一方、実施例1から実施例7で示されたように、ソフピロニウム臭化物製剤のpHが5.2以下に維持される場合、粘度の低下が抑制されることが明らかとなった。

試験例1と同様の方法で、参考例1~5(ソフピロニウム臭化物を含まない製剤)、比較例4、実施例8~12の組成物を調液した。これらの製剤を粘度安定性試験に用いた。
なお、参考例1~5、比較例4、実施例8~12は、ソフピロニウム臭化物、無水クエン酸、及び、無水エタノール、IPM(2.5w/w%)、HG(10w/w%)、HPC(1.25w/w%)を含有する製剤である。ソフピロニウム臭化物、および、無水クエン酸の含有量は表6に示した。全量が100%となるように、残分は無水エタノールで構成される。
pH測定方法は、試験例1と同様である。
BBI-4000含有量が0%及び5%の製剤については、粘度計を25℃、毎分10 rpm、プレヒート時間を30秒に設定し、約1 mLをコーンロータ:R-H1°34’×R24で200秒回転させた後の値を測定した(粘度測定法第2法)。BBI-4000含有量が15%の製剤については、粘度計を25℃、毎分7 rpm、プレヒート時間を30秒に設定し、約1 mLをコーンロータ:R-H1°34’×R24で200秒回転させた後の値を測定した(日本薬局方粘度測定法第2法)。
実施例10の製剤(BBI-4000ゲル剤5%(クエン酸濃度:0.050%))の40℃±2℃/75%RH±5%RH、遮光、3か月間の安定性試験における純度試験(類縁物質)の結果を下表に示す。

<調液方法>
試験例1と同様の方法で、実施例13~実施例15の組成物を調液した。表中の構成成分と濃度となるように、配合成分を無水エタノールに攪拌・溶解し製剤を得た。本法で製造した各製剤の内容を下表に示す。
pH測定方法は試験例1と同様であり、粘度測定方法は試験例2と同様である。
下表中、pHの定義と判定基準は、試験例1と同義である。
実施例13~実施例15の製剤(BBI-4000 5%ゲル剤(クエン酸濃度:0.050%))の40 ℃±2 ℃/75%RH±5%RH、遮光、3か月間の安定性試験における純度試験(類縁物質)の結果を下表に示す。
以上の通り、本発明にかかるソフピロニウム臭化物の低含水製剤は、少なくとも含水率が5w/w%以下である場合、類縁物質の生成は極めて微量であり、安定な製剤であることが示された。
以上の結果から、含水率が5w/w%以下であるソフピロニウム臭化物の低含水製剤は、非水製剤と同様に、経時的な粘度の低下が殆どなく、かつ、類縁物質の生成も限定的であり、医薬品の製剤として優良なプロファイルを示すことが明らかとなった。

試験例1と同様の方法で、実施例16~実施例19を調液した。表中の構成成分と濃度となるように、配合成分を無水エタノールに攪拌・溶解し製剤を得た。本法で製造した各製剤の内容を下表に示す。
pH測定方法、及び粘度測定方法は、試験例1と同様である。
下表中、pHの定義と判定基準は、試験例1と同義である。

試験例1と同様の方法で、実施例20~実施例23を調液した。表中の構成成分と濃度となるように、配合成分を無水エタノールに攪拌・溶解し製剤を得た。本法で製造した各製剤の内容を下表に示す。
** q.s. to 100: 全量が100%となるように、残分は無水エタノールで構成される。
pH測定方法、及び粘度測定方法は、試験例1と同様である。
下表中、pHの定義と判定基準は、試験例1と同義である。
ソフピロニウム臭化物製剤の長期保存試験
試験例1と同様の方法で、下表の実施例24、実施例25、比較例5、及び比較例6を調液し、各種試験に用いた。表中の構成成分と濃度となるように、配合成分を無水エタノールに攪拌・溶解し製剤を得た。本法で製造した各製剤の内容を下表に示す。
pH測定方法、及び粘度測定方法は、試験例1と同様である。
実施例24と実施例25の製剤は、安定性試験1に用いた。下表のpHは、保存期間(即ち、調液してから24か月目まで)における最高値を示した。表中の判定基準は、試験例1と同義である。比較例5は、同様に6か月間保存した。
比較例6の製剤は、安定性試験2に用いた。下表のpHは、保存期間(即ち、調液してから12か月目まで)における最高値を示した。表中の判定基準は、試験例1と同義である。
無水クエン酸濃度が0.001w/w%の比較例6の製剤は、室温下で12か月保存すると顕著に粘度が低下した(-76%)。一方、無水クエン酸濃度が0.05w/w%の実施例24、25の製剤は、調液後のpHが5.2以下に維持されており、粘度の経時的な変動は僅かであった。
したがって、調液してからpHが5.2以下に維持されるソフピロニウム臭化物製剤は、経時的な粘度の低下を抑制することができる。
特に、本試験により、室温で製剤が保存された場合において、調液してから6か月目までのいずれかの時点のpHが2.5から5.2の製剤が好ましいことが明らかとなった。例えば、室温で製剤が保存された場合の、調液してから1か月目、3か月目、又は6か月目のpHが2.5から5.2の範囲内である製剤が好ましい。
実施例24と実施例25の製剤の長期保存試験(25℃±2℃/60%RH±5%RH、遮光)において、調液してから24か月目まで純度試験(類縁物質)を実施した。実施例24において、24か月目にシクロペンチルマンデル酸エチルのHPLCピークがごく僅かに観察された。実施例25においては、調液してから保存期間中に類縁物質の増加及び新たな類縁物質の出現は認められなかった。これらの結果から、実施例24と実施例25の製剤が長期保存試験において安定であることが示された。
原発性腋窩多汗症患者を対象としたBBI-4000 の検証的試験
本試験中、「治療終了時」とは、治療終了の基準となる来院時期を指す。治療終了時は、所定の投与期間後の3つの来院日からなり、治療終了時のHDSSスコア及び発汗重量は、特に注記がなければ、その中央値を指す。
本試験中、「治療期間中」とは、治療開始時と治療終了時の間の期間を指す。
本試験例におけるベースラインのHDSSスコア及び発汗重量は、ベースライン1、ベースライン2、及びベースライン3と定義される9日以内での3回の来院日における各測定値の中央値を指す。
ソフピロニウム臭化物の医薬製剤の投与日数は、ベースライン3を1日目とした日数であり、「投与期間」や「投与週数」等の表現もこの基準に準ずる。ベースライン3が、ソフピロニウム臭化物の医薬製剤の投与を開始する日である。
本試験のタイムコースを図1に示す。
(1) 有効性の主要解析
治療終了時のHDSSスコアが1又は2であり、治療終了時の両腋窩合計発汗重量のベースラインとの比が0.5 以下の被験者の割合の解析はカイ2乗検定によって行った。
1) 発汗重量
ベースライン1~3の両腋窩合計の発汗重量の中央値をベースラインの発汗重量とし、投与6週目1~3の両腋窩合計の発汗重量の中央値を治療終了時の両腋窩合計の発汗重量とした。
両腋窩合計の発汗重量について投与群別に実施時期ごとの基本統計量を算出し、投与群間で比較した。また、以下を算出し、投与群間の差に関して信頼区間を示し、統計学的検定を行った。
・ 治療終了時の両腋窩の合計発汗重量のベースラインとの比が0.5以下の被験者の割合
・ 治療終了時の両腋窩の合計発汗重量のベースラインからの変化量
2) HDSS
ベースライン1~3のHDSSスコアの中央値をベースラインのHDSSスコアとし、投与6週目1~3のHDSSスコアの中央値を治療終了時のHDSSスコアとした。投与群別に実施時期ごとに集計した。また、治療終了時のHDSSスコアが1又は2 の被験者の割合を算出し、投与群間の差に関して信頼区間を示し、統計学的検定を行った。
以下の項目を調査し、その結果を記録した。
(1) 発汗重量測定
1) 測定条件
・ 温度:20℃~28℃、湿度:20%RH~80%RH
2) 測定方法
あらかじめ重量を測定したろ紙を被験者の両腋窩に5分間装着させた。
その後、汗を含んだろ紙の重量を測定して発汗重量を算出した。
また、被験者ごとに午前8時から午後7時の範囲で、実施時期間の差が4 時間を超えない時刻に実施した。
同意取得時の年齢が12 歳以上で、以下の診断基準及び条件を満たす原発性腋窩多汗症の患者
1. スクリーニングでの問診にて、以下の6項目のうち2項目以上に該当する、原発性腋窩多汗症と診断された患者
(1) 最初に症状がでたのが25歳以下である
(2) 左右対称性に発汗がみられる
(3) 睡眠中は発汗が止まっている
(4) 1週間に1回以上多汗のエピソードがある
(5) 家族歴がみられる
(6) 過剰な発汗により日常生活に支障をきたす
2. 以下の条件をすべて満たす患者
(1) ベースライン1~3の各時点でのHDSSスコアが3又は4
(2) ベースライン1~3の3時点のうちいずれかの2時点で、各腋窩の発汗重量がともに50 mg 以上
1. 続発性多汗症患者
2. 閉経により多汗症状が開始又は悪化した患者
3. 胸部交感神経遮断術の適応となる患者
原発性腋窩多汗症患者281 名に治験薬を無作為に割り付け(0%群140名、5%群141名)、これら患者群を対象としてデータ解析を行った。
治療終了時のHDSSスコアが1又は2であり、治療終了時の両腋窩合計発汗重量のベースラインとの比が0.5以下の被験者の割合を下表に示した。
(1) HDSS
治療終了時のHDSSスコアが1又は2の被験者の割合は、0%群で47.9%(67/140名)、5%群で60.3%(85/141名)であった。0%群より5%群が12.4%(95%信頼区間:0.86~23.99)高く、群間に統計学的な有意差がみられた(カイ2乗検定:p = 0.036)。
治療終了時の両腋窩合計発汗重量のベースラインとの比が0.5以下の被験者の割合は、0%群で66.4%(93/140名)、5%群で77.3%(109/141名)であった。0%群より5%群が10.9%(95%信頼区間:0.44~21.32)高く、群間に統計学的な有意差がみられた(カイ2乗検定:p = 0.042)。
ベースラインの両腋窩合計発汗重量発汗の程度が重症の患者での有効性を検討するため、ベースラインの両腋窩合計発汗重量が400 mg以上の患者を対象とした部分集団での有効性を検討した。
解析結果を下表に示した。
以上、ベースラインの両腋窩合計発汗重量が400 mg以上験者で、いずれの評価項目でも0%群より5%群で改善がみられた。
原発性腋窩多汗症患者を対象とした無作為化二重盲検並行群間比較により、5%のBBI-4000を1日1回、6週間、腋窩に塗布した際、各被検者の治療前のHDSSスコアから治療終了時のHDSSスコアを減算し、その差(ΔHDSS)を算出した。治療前と治療終了時の両HDSSスコアを得られた140例を解析対象とし、ΔHDSSの平均変化量と標準偏差を算出した。
その結果、5% BBI-4000群の平均ΔHDSSは1.14±0.87であった。
さらに本発明の製剤は、原発性腋窩多汗症の処置、治療、又は予防に用いることができる。
Claims (27)
- ヒトの身体表面に外用塗布するための医薬製剤であって、
(a)ソフピロニウム臭化物と、
(b)1又は複数の水溶性高分子と、
(c)エタノールと、
を含有し、pHが5.2以下であり、均一に分散された、非水製剤又は含水率が5 w/w%以下の低含水製剤であり、
前記pHは、調液してから6か月間で選択される1又は複数のいずれかの時点で測定され、前記pHは、室温で保存された前記製剤のpHを測定することによって決定され、かつ、前記pHは、非水溶媒用pH電極を前記製剤に5分間浸した後の値である、製剤。 - ソフピロニウム臭化物の含有量が、全製剤量に対して、1w/w%から20w/w%である、請求項1に記載の製剤。
- pHが2.5から5.2の範囲内である、請求項1又は請求項2に記載の製剤。
- 均一に溶解された製剤である、請求項1又は請求項2に記載の製剤。
- 水溶性高分子が、水溶性ビニルポリマー系高分子、又は水溶性セルロース系高分子である、請求項1から請求項4のいずれか一つに記載の製剤。
- 水溶性高分子が、ヒドロキシメチルセルロース、ヒドロキシエチルセルロース、ヒドロキシプロピルセルロース、ヒドロキシエチルメチルセルロース、ヒドロキシプロピルメチルセルロース、メチルセルロース、エチルセルロース、カルボキシメチルセルロース、カルボキシビニルポリマー、ポリビニルアルコール、ポリビニル系共重合体、ポリビニルピロリドン、及びコポビドンからなる群より選択される、請求項1から請求項4のいずれか一つの製剤。
- 水溶性高分子が、ヒドロキシプロピルセルロース、又はカルボキシビニルポリマーである、請求項1から請求項4のいずれか一つに記載の製剤。
- 水溶性高分子の含有量が、全製剤量に対して、0.01w/w%から5.0w/w%である、請求項1から請求項7のいずれか一つに記載の製剤。
- エタノールの含有量が、全製剤量に対して、50w/w%以上から99w/w%未満である請求項1から請求項8のいずれか一つに記載の製剤。
- さらに、pH調整剤を含有する、請求項1から請求項9のいずれか一つに記載の製剤。
- pH調整剤が、酒石酸、酢酸、及びクエン酸からなる群より選択される酸、又はその塩である、請求項10に記載の製剤。
- pH調整剤の含有量が、全製剤量に対して、0.015w/w%から5w/w%である、請求項10又は請求項11に記載の製剤。
- さらに、不揮発性オイルを含有する、請求項1から請求項12のいずれか一つに記載の製剤(但し、ジメチコノールブレンド20を含む製剤を除く)。
- 不揮発性オイルが、不揮発性エステル、不揮発性エーテル、不揮発性シリコーン、及び不揮発性アルコールからなる群から選択される、請求項13に記載の製剤。
- 不揮発性オイルが、
R1COOR2で表され、R1及びR2の一方が、置換されていてもよいC4-C40直鎖アルキル基、又は置換されていてもよいC4-C40分岐鎖アルキル基であり、かつ、
R1及びR2の他方が、置換されていてもよいC1-C40アルキル基である、モノエステル、ジエステル、及びトリエステルからなる群より選択される不揮発性エステルである、請求項13に記載の製剤。 - 不揮発性オイルが、ミリスチン酸エチル、ミリスチン酸2-オクチルドデシル、ステアリン酸ブチル、ステアリン酸イソセチル、ステアリン酸2-オクチルドデシル、ラウリン酸ヘキシル、ラウリン酸2-ヘキシルデシル、パルミチン酸2-エチルヘキシル、パルミチン酸2-オクチルデシル、オクタン酸セテアリル、イソノナン酸イソノニル、ネオペンタン酸オクチルドデシル、エルカ酸2-オクチルドデシル、安息香酸2-オクチルドデシル、デカン酸エステル、リシノール酸エステル、ミリスチン酸イソプロピル、アジピン酸ジイソプロピル、中鎖脂肪酸トリグリセリド、パルミチン酸イソプロピル、エチルヘキサン酸アルキル(C14-C18)、ミリスチン酸ミリスチル、オレイン酸エチル、オレイン酸オレイル、パルミチン酸エチルヘキシル、パルミチン酸セチル、ミリスチン酸2-ヘキシルデシル、パルミチン酸2-ヘキシルデシル、ミリスチン酸PPG-3ベンジルエーテル、イソノナン酸イソトリデシル、トリメリト酸トリエチルヘキシル、安息香酸アルキル(C12-C15)、コハク酸ジエトキシエチル、ジカプリン酸プロピレングリコール、ジカプリル酸プロピレングリコール、トリ(カプリル・カプリン酸)グリセリル、トリエチルヘキサノイン、トリイソステアリン、イソステアリン酸イソプロピル、イソステアリン酸イソステアリル、トリイソステアリン酸ポリグリセリル-2、コハク酸ジエチルヘキシル、プロピオン酸PPG-2ミリスチル、テトライソステアリン酸ペンタエリスリチル、セバシン酸ジエチル、エチルヘキサン酸PPG-3ベンジルエーテル、トリベヘン酸グリセリル、2-エチルヘキサン酸セチル、リンゴ酸ジイソステアリル、ステアリン酸2-エチルヘキシル、クエン酸トリエチルヘキシル、及び乳酸エチル等の乳酸アルキルからなる群より選択される不揮発性脂肪酸エステルである、請求項13に記載の製剤。
- 不揮発性オイルが、ミリスチン酸イソプロピル、アジピン酸ジイソプロピル、及び中鎖脂肪酸トリグリセリドからなる群より選択される不揮発性脂肪酸エステルである、請求項13に記載の製剤。
- 不揮発性オイルが、医療等級のシリコーン油、メチルフェニルシリコーン、メチルハイドロジェンシリコーン、デカメチルペンタシクロシロキサン、オクタメチルテトラシクロシロキサン、シクロメチコン5-NF、PEG-12ジメチコン、ジメチコン20cSt、ジメチコン100cSt、ジメチコン350cSt、ジメチコン500cSt、ジメチコン1000cSt、及びジメチコン12500cStからなる群より選択される不揮発性シリコーンである、請求項13に記載の製剤。
- 不揮発性オイルが、シクロメチコン5-NF、PEG-12ジメチコン、ジメチコン20cSt、及びジメチコン350cStからなる群より選択される不揮発性シリコーンである、請求項13に記載の製剤。
- 不揮発性オイルの含有量が、全製剤量に対して、0.5w/w%から10w/w%である、請求項13から請求項19のいずれか一つに記載の製剤。
- さらに、多価アルコールを含有する、請求項1から請求項20のいずれか一つに記載の製剤。
- 多価アルコールが、へキシレングリコール、プロピレングリコール、エチレングリコール、グリセロール、及びブチレングリコールからなる群より選択される、請求項21に記載の製剤。
- 多価アルコールの含有量が、全製剤量に対して、1.0w/w%から30w/w%である、請求項21又は請求項22に記載の製剤。
- 25℃における粘度が10 mPa・sから2000 mPa・sである、請求項1から請求項23のいずれか一つに記載の製剤。
- 調液してから室温で36か月保存した後、又は調液してから40℃で3か月保存した後の、25℃における粘度が10 mPa・sから1000 mPa・sである、請求項1から請求項24のいずれか一つに記載の製剤。
- 多汗症、過活動膀胱、慢性閉塞性肺疾患、心臓疾患、流涎症、眼疾患、及び気管支喘息からなる群より選択される疾患を、処置、治療、又は予防するための、請求項1から請求項25のいずれか一つに記載の製剤。
