- the present invention relates to the development of dental/dental implants, more specifically, to the optimization of the implant's macrogeometry to improve bone integration.

Abstract
A presente invenção se refere ao campo dos implantes dentários, mais especificamente a otimização da macrogeometria do implante para melhora na osseointegração. Para alcançar essa melhora, o implante dentário (1) contém, rebaixos cilíndricos (2) começando do ápice da rosca (6) na direção do corpo, permitindo uma melhor irrigação sanguínea; menor torque de do implante (4)inserção, sem perder a estabilidade primária; e criando câmaras de ossoeintegração.
Description
IMPLANTE DENTÁRIO
CAMPO DA INVENÇÃO
[001] A presente invenção refere-se ao desenvolvimento de implantes dentários/odontológicos, mais especificamente, à otimização da macrogeometria do implante para melhora na osseo integração.
ANTECEDENTES DA INVENÇÃO
[002] Os implantes dentários são elementos, geralmente produzidos em titânio, para serem integrados ao tecido ósseo e, sobre eles, colocada uma prótese para substituir os dentes que foram perdidos. Essa integração é o crescimento do tecido ósseo sobre o implante, também chamada de osseo integração.
[003] A osseo integração pode ser observada em estudos em laboratório, onde os implantes são inseridos em ossos de animais e após o período de inflamação (geralmente 15 dias), são medidos os torques (em Nem.) necessários para remoção dos mesmos em diferentes períodos. Quanto maior for o torque de remoção, melhor foi a integração do implante dentário.
[004] Diversos fatores devem ser considerados para o sucesso do implante dentário. Em primeiro lugar, as condições do paciente, como por exemplo: a densidade óssea, se é portador de diabete e se o paciente é ou já foi fumante. Outros fatores a serem considerados seriam em relação à construção do implante, tais como a qualidade do produto, o tratamento superficial e a macrogeometria do implante. Então, em busca do sucesso na fixação do implante, diversas soluções foram
sendo criadas.
[005] A relação entre o diâmetro da broca e o diâmetro do implante deve ser tal que, o torque para colocação não seja muito elevado (hoje é preconizado um torque médio de colocação de por volta de 45N). Torques elevados podem causar a necrose do tecido ósseo, devido à falta de espaço para a irrigação sanguínea, levando-se assim mais tempo para recuperação após a cirurgia. Torques de colocação muito baixos diminuem a estabilidade primária do implante, dificultando assim a carga precoce ou até mesmo, a carga imediata.
[006] Os implantes do estado da técnica, em sua maioria, são corpos cilíndricos ou cónicos envoltos por uma rosca. Porém, com o intuito de solucionar os problemas supracitados, o estado da técnica vem buscando novas soluções geométricas.
[007] Na tentativa de resolver os problemas do estado da técnica, a carta patente PI 0317837-4 descreve um implante com ranhuras periféricas ao longo de toda sua extensão, a fim de aumentar a superfície dos flancos de rosca e ajudar no processo de fusão (termo usado no documento) ou retenção do implante no osso. Em uma configuração adicional, o documento ainda propõe a adição de estimuladores de crescimento ósseo nas ranhuras apresentadas. Porém, a solução apresentada nesta patente não é a solução ótima, sendo o torque de remoção alcançado após 6 semanas de implante (coletado em estudos com coelhos), menor que o encontrado após 4 semanas com a da presente invenção (Figura 4).
[008] A carta patente BR
102014031426-1 apresenta cavidades esféricas de corte nos topos dos filetes da rosca região central com maior para diminuir tanto a quantidade de osso
retirado quanto os danos aos vasos sanguíneos na região próxima do implante. Apesar de serem detalhes na rosca do implante, eles são bem diferentes e alcançam resultados distintos se comparados com a presente invenção.
[009] Outra solução apresentada no estado da técnica é a presente no documento CN 208371937, que mostra um implante dentário oco, apresentando furos, indo até a parte oca do implante, entre os filetes da rosca. Essa solução seria para permitir que o osso alveolar penetrasse o implante e diminuísse o risco de infecções bacterianas. Assim como em BR 102014031426-1, os resultados alcançados não são os mesmos e a macrogeometria é totalmente diferente, sendo que no documento chinês, os furos são localizados no corpo do implante e não rebaixos cilíndricos (2) nos flancos da rosca.
[0010] A presente invenção vem para diminuir consideravelmente o tempo de recuperação do paciente implantado. Para isso, a invenção foi projetada para melhorar os fatores fundamentais para o sucesso do implante.
[0011] Os rebaixos cilíndricos (2) reduzem o torque de inserção sem alterar a relação entre os diâmetros da broca (7) e do implante (1), não prejudicando a estabilidade primária.
[0012] Além disso, os espaços livres deixados entre o diâmetro da broca e as áreas onde se encontram os rebaixos cilíndricos (2) melhoram a circulação sanguínea e criam câmaras de osseo integração onde são observadas maiores formações de novos tecidos ósseos (Figura 5).
[0013] O gráfico da figura 4 e as imagens das figuras 5 e 6, mostram resultados de estudos com o implante em ossos de
coelhos para observar as melhorias alcançadas pela invenção (Group TEST) quando comparada a um implante sem a otimização da macrogeometria (GROUP CON). Observando-se o gráfico e as figuras, fica claro o aumento do torque de remoção e a formação de novo tecido ósseo na superfície do implante.

DESCRICÃO DETALHADA DA INVENÇÃO
[0022] Implante dental (1), preferencialmente de titânio, podendo ele ser cilíndrico ou cónico, compreendendo uma rosca e caracterizado por apresentar rebaixos cilíndricos (2) começando do ápice da rosca (6), passando pelo flanco superior e/ou inferior (5) no sentido do corpo do implante (4).
[0023] O número de rebaixos cilíndricos (2) pode variar de acordo com a distância (L) entre os rebaixos (2) consecutivos e a largura (a) dos rebaixos. Para o implante apresentar melhorias com relação ao estado da técnica, a distância (L) deve ser menor que 4 (quatro) vezes a largura do rebaixo (a). Para distâncias (L) maiores que 4 vezes a largura do rebaixo (a), as melhorias alcançadas na integração do implante com o osso são reduzidas.
[0024] A configuração do implante submetido a estudo, é feito de titânio, tendo 4 mm de diâmetro em sua parte superior, 3 mm em sua parte inferior, passo de rosca de 1 mm, raio dos rebaixos (2) de 0,35 mm, comprimento de 10 mm e tendo 6 rebaixos a cada volta da rosca.
[0025] As dimensões do implante podem variar, podendo ter de 3,5 mm a 6 mm de diâmetro em sua parte superior, de 2,5 mm a 4 mm de diâmetro em sua parte inferior, passo de rosca de 0,75 mm a 1,25 mm, raio dos rebaixos (2) não superiores a 1 mm, comprimento de 5mm a 20 mm e quantidade de rebaixos a cada volta da rosca de acordo com o critério anteriormente citado.
[0026] Em sua região superior, o implante
(1) compreende os mais diversos tipos de encaixe dos elementos de prótese
presentes no estado da técnica, dentre eles: hexágono interno, hexágono externo e cone morse.
Claims
REIVINDICAÇÕES
1) Implante dentário (1) de corpo cilíndrico ou cónico, compreendendo uma rosca e caracterizado por apresentar rebaixos cilíndricos (2) começando do ápice da rosca (6), passando por seu flanco (5) no sentido do corpo do implante (4).
2) Implante dentário (1), de acordo com a reivindicação 1, caracterizado pelos rebaixos serem localizados nos flancos superiores e/ou inferiores da rosca (5).
3) Implante dentário (1), de acordo com as reivindicações 1 e 2, caracterizado pela distância (L) entre os rebaixos consecutivos ser inferior a 4 vezes a largura do rebaixo (a).
