Abstract
本発明は、動脈硬化関連疾患を検出するための、又は動脈硬化の進行度を評価するための新規バイオマーカーを提供することを目的とし、具体的には、NPC2(Niemann-Pick disease type C2)タンパク質及び/又はIGFBP7(Insulin-like growth factor-binding protein 7)タンパク質を含む、動脈硬化関連疾患を検出するための、又は動脈硬化の進行度を評価するためのマーカーに関する。
Description
本発明は、動脈硬化関連疾患及び動脈硬化の診断を補助する臨床検査薬の技術分野に関する。詳細には、動脈硬化関連疾患のバイオマーカー、動脈硬化の状態(以下、「進行度」ともいう。)を反映するバイオマーカー、それらを用いる測定試薬、試薬キット、動脈硬化関連疾患の検出方法、及び動脈硬化の進行度の評価方法に関するものである。
動脈硬化は、動脈の内部にさまざまな物質が沈着して、内壁が肥厚したり、硬化して弾力性を失ったり、内腔が狭くなったりした状態の総称である。粥状動脈硬化(以下、アテローム動脈硬化という。)、中膜石灰化、細動脈硬化の3つに分けられるが、一般に動脈硬化といえばアテローム動脈硬化を指す場合が多い。アテローム動脈硬化は、大動脈や脳動脈、冠動脈などの比較的太い動脈に起こる動脈硬化であり、動脈の内膜にコレステロールなどの脂肪からなる粥状物質がたまってアテローム(粥状硬化巣)ができ、次第に肥厚することで動脈の内腔が狭くなるといわれている。また、形成される不安定な粥腫が剥がれて血栓となり各種梗塞の原因となることも多く、このように、動脈硬化は心筋梗塞、末梢動脈疾患、大動脈瘤、大動脈解離、脳卒中(アテローム血栓性脳梗塞、一過性脳虚血発作など)、腎動脈狭窄症、内頚動脈狭窄症、狭心症などの疾患を引き起こすことが知られており、人口の高齢化、生活習慣の変化により近年増加している。動脈硬化は無症状で進行し、心筋梗塞、脳梗塞、大動脈瘤などの発症に至ると生命の危機につながる重篤な疾患である。しかし、適切な治療を行うことにより発症を回避することが可能であるため、動脈硬化や動脈硬化性疾患を早期に検出することや、動脈硬化の進行度を管理することは非常に重要である。
例えば、動脈硬化などの原因によって発症する胸部大動脈瘤は、健康診断などで胸部レントゲン写真を撮った時に偶然発見されることがほとんどであり、腹部大動脈瘤の場合は、胃潰瘍や胆石症などの消化器疾患を診断するために腹部を触診した際に、拍動するしこりとして発見されたり、消化管や腎臓、前立腺などの腹部エコー(超音波)の検査中あるいは胸部腹部のMRI検査中に偶然に発見されたりすることが多い。
動脈硬化や動脈硬化関連疾患のマーカーとしては、リスク評価として用いられるLDLコレステロールや、酸化LDLなどの脂質マーカーや、CRP、Pentraxin3などの炎症性マーカーが知られている(非特許文献1)。しかし、いずれも、感度、特異度や疾患特異性の面から十分なものとは言えない。
また、非侵襲的に動脈硬化の状態を把握する手段として、頸動脈エコー検査による血管内膜中膜複合体厚(Intima-Media Thickness)(以下、IMTという。)測定や、手と足の動脈波の速度の差(上腕-足首間脈波伝導速度:brachial-ankle Pulse Wave Velocity)(以下、baPWVという。)の測定等が知られている(非特許文献2)。これらの検査は、十数分程度の所要時間で、非侵襲、簡便に動脈硬化の進行度を調べられることから多くの病院で実施されているが、設備の揃った病院に行かなければ検査を受けられない点や、大規模な集団から動脈硬化リスクの高い群を選別するためにスクリーニング検査に使用する場合のスループットの点において、血液検査等と比較すると明らかに不向きである。

これまでに健康診断等でスクリーニング的に動脈硬化の危険因子として用いられてきた血圧や、LDLコレステロール、HDLコレステロール等の血液検査は検査時点での数値であり、様々な危険因子やストレスに異なる強度、異なる期間さらされた血管壁の組織や細胞の状態を反映するものではない。以上のような現状から、頚動脈エコーやbaPWV検査といった動脈硬化の評価法と強い関連性を示す、スクリーニング的に利用可能なバイオマーカーが開発されれば、動脈硬化の予防、動脈硬化関連疾患の発症診断、進行度評価、治療薬の開発などに新たな局面をもたらすことが予想される。
本発明の課題は、動脈硬化の状態を反映し、簡便、高感度に動脈硬化関連疾患の検出や動脈硬化の進行度の評価が可能な新規バイオマーカーを提供することにある。
本発明者らは、動脈硬化の状態を反映した優れた動脈硬化性疾患マーカーを提供するという目的を達成すべく、動脈硬化を原因として発症する疾患として動脈硬化性胸部大動脈瘤組織を用いたプロテオーム解析を実施した。動脈硬化性胸部大動脈瘤は、動脈硬化を原因として発症し、さらに疾患部位が明確に区別可能なため、組織を用いた動脈硬化性疾患の解析に非常に適している。得られたタンパク質プロファイルから、組織を疾患の進行度に従って区分することが可能であり、それらの区分は既知の構造タンパク質の変化やプロテアーゼ等の変化をよく説明できるものであった。さらに同様の手法で、健常者大動脈組織のプロテオーム解析も実施した。得られたデータから、疾患の発症に関連する因子を同定するために、健常者大動脈組織と動脈硬化性胸部大動脈瘤患者組織のプロテオームデータの比較を行った。さらに疾患の進行に関連する因子を同定するために、動脈硬化性胸部大動脈瘤患者組織プロテームデータの各区分間を比較した。両比較で共通して変動する因子は、疾患の発症・進行に関わるものであると推測される。共通して変動する因子のうち、血液中においても変動する因子を探索し、動脈硬化の発症と進行、すなわち動脈硬化関連疾患の発症と、動脈硬化の進行度を高精度に反映する新たなマーカーを見出し、本発明を完成させるに至った。
すなわち、本発明は以下のような構成から成るものである。
(1)対象から得られた試料において、以下の1)及び/又は2)を測定する工程を含む、動脈硬化関連疾患を検出するための方法。

本発明により見出したNPC2又はIGFBP7を動脈硬化関連疾患のマーカー(すなわち動脈硬化関連疾患の検出のためのマーカー)又は動脈硬化マーカー(すなわち動脈硬化の進行度を評価するためのマーカー)として用いることにより、動脈硬化関連疾患の発症や動脈硬化を高感度に検出することができる。さらに、NPC2及びIGFBP7は実施例で示したように、それぞれ異なる動脈硬化の側面を捉えたマーカーであると考えられ、これらを組み合わせて用いることでより詳細な動脈硬化の進行度の情報を得ることが可能になる。
また、本発明による動脈硬化関連疾患の検出のためのマーカー又は動脈硬化の進行度を評価するためのマーカーを用いる測定試薬及び測定キットは動脈硬化関連疾患発症や動脈硬化の早期診断、動脈硬化進行状態の把握(モニタリング)に使用することが可能であり、これらの疾患の早期検出により疾患の重症化の回避に貢献可能である。
本発明による動脈硬化関連疾患の検出のためのマーカー又は動脈硬化の進行度を評価するためのマーカーはこれらの疾患の予防薬や治療薬の開発、評価にも使用することが可能である。さらに、動脈硬化関連疾患の再発を検出又はスクリーニングすることや、将来、動脈硬化関連疾患を発症するリスクを有する対象を検出又はスクリーニングすることも可能である。
本発明者等は、動脈硬化の状態を反映したマーカーの同定を目的として、健常者大動脈組織14例、動脈硬化性胸部大動脈瘤患者組織29例から採取した大動脈中膜平滑筋層組織を用いてプロテオーム解析を以下のように行った。
・組織の破砕とトリプシン消化
採取後、凍結した組織はビーズ破砕機を用いて破砕し、Lys-C (Lysyl Endopeptidase)、Trypsinを添加して37℃で一晩インキュベーションにより消化した。得られた消化ペプチドはC18(オクタデシルシリル)カラムを用いて脱塩し、解析サンプルとした。
・解析
解析サンプルはナノLC(日立Nano Frontier、日立ハイテクノロジーズ社)で分離した後、連結されたTripleTOF 5600(SCIEX社)でMS/MS解析を行った。測定データからMascot database search(Matrix science社)でタンパク質を同定し、LC-MSデータを集積して用いる比較定量プロテオーム解析ソフトである2DICAL (2 Dimensional Image Converted Analysis of Liquid chromatography mass spectrometry、三井情報株式会社)を用いて定量を行い、動脈硬化性胸部大動脈瘤群の区分化と区分内での比較、正常大動脈群と動脈硬化性胸部大動脈瘤群間で比較し、差のあるタンパク質の探索を行った。その結果、動脈硬化性胸部大動脈瘤群の区分間及び正常大動脈群と動脈硬化性胸部大動脈瘤群間で顕著な増加を共通して示すタンパク質としてNPC2、IGFBP7を同定し、本発明を完成するに至った。

なお、本発明においては、健常者と動脈硬化性胸部大動脈瘤との比較だけではなく、非動脈硬化性胸部大動脈瘤と動脈硬化性胸部大動脈瘤の比較データも参照しつつ候補因子を選択し、本発明のマーカーを見出した。
以上のとおり、NPC2及びIGFBP7はいずれも、動脈硬化を原因として発症する動脈硬化性胸部大動脈瘤患者由来の大動脈中膜組織において、健常者の大動脈中膜組織と比較して顕著に発現が亢進していることが確認されている。NPC2、IGFBP7をそれぞれ単独でマーカーとして使用することにより動脈硬化関連疾患及び動脈硬化を検出可能であるが、後に実施例で示す様に、両マーカーは異なる動脈硬化の指標とより強く関係する(後に実施例1~2に示す様に、NPC2は頸動脈プラーク有無及びbaPWV値とより強く関係するマーカーであり、実施例3に示す様に、IGFBP7は頸動脈球部のIMTの最大値(以下、maxIMT値という。)とより強く関係するマーカーである)ため、NPC2及びIGFBP7を組み合わせることで動脈硬化の状態(進行度)をより正確に反映した優れた検出効果を発揮する。
本発明は、NPC2及び/又はIGFBP7を動脈硬化関連疾患の検出のためのマーカー又は動脈硬化の進行度を評価するためのマーカーとすることを特徴とする。
本発明において、「マーカー」又は「バイオマーカー」は、対象から得られた試料の測定用の標的として使用される分子のことをいう。
本発明に係るマーカーは、NPC2及び/又はIGFBP7を含む。ヒトNPC2(すなわち、Niemann-Pick disease type C2 protein)は、UniProt のアクセション番号がP61916で表される分泌性のタンパク質である(http://www.uniprot.org/uniprot/P61916)。ヒトNPC2のアミノ酸配列を配列番号1に示す。配列番号1に示すアミノ酸配列において、第1番目~第19番目のアミノ酸配列はシグナルペプチドであり、第20番目~第151番目のアミノ酸配列は成熟型タンパク質である。NPC2は、ヒトでは132アミノ酸から成る糖タンパク質であり、精巣上体に豊富に存在するが、その他の多くの組織にも存在する。NPC2はコレステロール結合能を有しており、NPC1とともにリソソームからのコレステロール排出に関与すると考えられている。NPC2の機能欠損は、リソソームへのコレステロール異常蓄積を呈するニーマンピック病C型の原因となる(Marie T. Vanier, Gilles Millat. Structure and function of the NPC2 protein. Biochimica et Biophysica Acta 2004; 1685: 14-21)。
本発明において、NPC2タンパク質には、配列番号1に示すアミノ酸配列における第20番目~第151番目のアミノ酸配列に対して70%以上、75%以上、80%以上、85%以上、90%以上、より好ましくは95%以上、96%以上、97%以上、98%以上、99%以上、最も好ましくは100%の配列同一性を有するアミノ酸配列から成り、且つコレステロール結合活性及び/又はリソソームからのコレステロール排出活性を有するか、あるいは機能欠損変異体等の当該活性を有しないタンパク質が含まれ、さらにそのようなタンパク質の多量体(二量体以上)も含まれる。
一方、ヒトIGFBP7(すなわち、Insulin-like growth factor-binding protein 7)は、UniProtのアクセション番号がQ16270で表される分泌性のタンパク質である(http://www.uniprot.org/uniprot/Q16270)。ヒトIGFBP7のアミノ酸配列を配列番号2に示す。配列番号2に示すアミノ酸配列において、第1番目~第26番目のアミノ酸配列はシグナルペプチドであり、第27番目~第282番目のアミノ酸配列は成熟型タンパク質である。IGFBP7は、心臓、脳、胎盤、肺、肝臓、骨格筋、腎臓、膵臓、脾臓、胸腺、前立腺、精巣、卵巣、小腸、結腸など多くの組織で発現している。他のIGFBPタンパク質と異なり、インスリン様成長因子(IGF)との結合性が弱く、インスリンと強く結合することが知られている。IGFBP7は、細胞増殖の制御や、アポトーシス、細胞老化や血管新生に関わることが報告されている(Shuzhen Zhu, Fangying Xu, Jing Zhang , Wenjing Ruan, Maode Lai. Insulin-like growth factor binding protein-related protein 1 and cancer. Clinica Chimica Acta 2014; 431: 23-32)。
本発明において、IGFBP7タンパク質には、配列番号2に示すアミノ酸配列における第27番目~第282番目のアミノ酸配列に対して70%以上、75%以上、80%以上、85%以上、90%以上、より好ましくは95%以上、96%以上、97%以上、98%以上、99%以上、最も好ましくは100%の配列同一性を有するアミノ酸配列から成り、且つインスリン結合活性を有するか、あるいは当該活性を有しないタンパク質が含まれ、さらにそのようなタンパク質の多量体(二量体以上)も含まれる。
なお、本発明に係るマーカーを、LDLコレステロール、HDLコレステロール、酸化LDL等の変性LDLやsdLDL(small dense LDL)、C反応性蛋白(CRP)、Pentraxin3、IL-6などの既知の動脈硬化性疾患との関連が報告されているマーカーと組み合わせて使用することも可能である。
本発明において、「動脈硬化関連疾患」は、動脈硬化を原因として発症する疾患のことをいう。本発明に係る動脈硬化関連疾患マーカーにより検出可能な疾患は、動脈硬化に起因して発症する疾患であれば特に限定されない。具体例としては、心筋梗塞、末梢動脈疾患、大動脈瘤、大動脈解離、脳卒中(アテローム血栓性脳梗塞、一過性脳虚血発作など)、腎動脈狭窄症、内頚動脈狭窄症、狭心症などが挙げられる。本明細書においては、動脈硬化及びその関連疾患を包括して「動脈硬化性疾患」という場合がある。また、前記の種々の動脈硬化関連疾患を、本発明に係るマーカーで診断、検査、評価又は判定可能な、動脈硬化関連疾患ということができる。
本発明において動脈硬化関連疾患マーカーを検出する方法、及び動脈硬化マーカーを測定する方法(すなわち動脈硬化の進行度を評価するためのマーカーの測定方法)は、NPC2タンパク質及び/又はIGFBP7タンパク質を測定する工程を含む。試料中のNPC2タンパク質及び/又はIGFBP7タンパク質を測定するに際しては、糖タンパク質としてのNPC2及び/又はIGFBP7を測定しても良く、また、前処理により糖鎖、リン酸等の翻訳後修飾を除去した後にNPC2及び/又はIGFBP7のアミノ酸配列部分を測定しても良い。
動脈硬化関連疾患を検出する方法は、対象から得られた試料において本発明に係るマーカーを測定することで、当該対象の動脈硬化関連疾患を検出することができる。従って、動脈硬化関連疾患を検出する方法は、動脈硬化関連疾患の診断、検査、評価又は判定方法、あるいは動脈硬化関連疾患を検出するためのin vitroにおけるデータ収集方法等ということができる。
後に実施例1~3に示す様に、本発明に係るマーカーは動脈硬化の指標である頚動脈エコーやbaPWV検査等との強い関係性が認められており、動脈硬化の進行度を評価する方法、すなわち動脈硬化の進行の程度を評価する方法は、対象から得られた試料において本発明に係るマーカーを測定するのみで、頚動脈エコーやbaPWV検査等によって得られる血管の動脈硬化進行度と同程度の情報を把握することができる。従って、動脈硬化の進行度を評価する方法は、動脈硬化の診断、検査、評価又は判定方法、あるいは動脈硬化を検出するためのin vitroにおけるデータ収集方法等ということができる。
動脈硬化関連疾患を検出する方法及び動脈硬化の進行度を評価する方法の被験対象となり得る動物としては、例えばヒト、サル、ウシ、ブタ、ウマ、イヌ、ネコ、ヒツジ、ヤギ、ウサギ、ハムスター、モルモット、マウス、ラット等の哺乳類が挙げられるが、好ましくはヒトである。
また、動脈硬化関連疾患を検出する方法及び動脈硬化の進行度を評価する方法において使用する試料としては、例えば血清、血漿、血液、尿、唾液等が挙げられる。
本発明においては、例えば、動脈硬化が疑われる患者から採取された試料において、in vitroでNPC2タンパク質レベル及び/又はIGFBP7タンパク質レベルを測定することにより、現在動脈硬化関連疾患もしくは動脈硬化が進行している可能性、将来動脈硬化関連疾患を発症するリスク(可能性)、又は動脈硬化関連疾患の再発を判断、検出又はスクリーニングすることができる。患者検体中のNPC2タンパク質レベル及び/又はIGFBP7タンパク質レベルを健常者のレベルと比較し、患者検体中のNPC2タンパク質レベル及び/又はIGFBP7タンパク質レベルが健常者のレベルに比べて高い場合、現在動脈硬化関連疾患もしくは動脈硬化が進行している可能性、将来、動脈硬化関連疾患を発症するリスク(可能性)、又は動脈硬化関連疾患が再発している可能性が高いと判断できる。
具体的には、動脈硬化関連疾患を検出する方法及び動脈硬化の進行度を評価する方法に準じて、本発明は、以下の方法をさらに含む:
動脈硬化関連疾患の再発のリスクを有する対象から得られた試料において、本発明に係るマーカーを測定することで、動脈硬化関連疾患の再発を検出又はスクリーニングする方法;
対象から得られた試料において、本発明に係るマーカーを測定することで、動脈硬化関連疾患を将来発症するリスクを有する対象を検出又はスクリーニングする方法。
動脈硬化関連疾患の再発のリスクを有する対象から得られた試料において、本発明に係るマーカーを測定することで、動脈硬化関連疾患の再発を検出又はスクリーニングする方法;
対象から得られた試料において、本発明に係るマーカーを測定することで、動脈硬化関連疾患を将来発症するリスクを有する対象を検出又はスクリーニングする方法。
また、動脈硬化関連疾患を発症する可能性あるいは現在動脈硬化が進行している可能性が高いと判断するための各マーカーのカットオフ値としては、限定されるものではないが、以下のものが挙げられる。下記のカットオフ値は、実施例に示すデータから作成したROC曲線に基づき算出したものである。
(i)実施例1(頸動脈プラーク有無との比較解析)に基づくNPC2のカットオフ値:2.8 ng/mL(感度68.0%、特異度22.2%);
実施例2(baPWVのカットオフ値1400cm/s)に基づくNPC2のカットオフ値:2.3 ng/mL(感度75.0%、特異度29.0%);
実施例2(baPWVのカットオフ値1700cm/s)に基づくNPC2のカットオフ値:3.4 ng/mL(感度65.0%、特異度35.0%)。
(ii) 実施例3(頸動脈プラーク有無との比較解析)に基づくIGFBP7のカットオフ値:178.0 ng/mL(感度53.3%、特異度22.2%)。
実施例2(baPWVのカットオフ値1400cm/s)に基づくNPC2のカットオフ値:2.3 ng/mL(感度75.0%、特異度29.0%);
実施例2(baPWVのカットオフ値1700cm/s)に基づくNPC2のカットオフ値:3.4 ng/mL(感度65.0%、特異度35.0%)。
(ii) 実施例3(頸動脈プラーク有無との比較解析)に基づくIGFBP7のカットオフ値:178.0 ng/mL(感度53.3%、特異度22.2%)。
これらカットオフ値に基づいて、患者検体中のNPC2タンパク質レベル及び/又はIGFBP7タンパク質レベルがカットオフ値に比べて高い場合、動脈硬化関連疾患を発症する可能性あるいは動脈硬化が進行している可能性が高いと判断できる。
本発明に係るマーカーは、動脈硬化関連疾患又は動脈硬化の予防薬又は治療薬のスクリーニングや、動脈硬化関連疾患又は動脈硬化の予防薬又は治療薬の効果の評価や判定のために使用することも可能である。例えば、動脈硬化関連疾患又は動脈硬化の予防薬又は治療薬の効果の評価や判定を行う場合は、動脈硬化関連疾患又は動脈硬化の予防又は治療を必要とする被験対象動物に該当する予防薬又は治療薬を投与する前と投与した後、被験対象動物から試料を採取するか、又は当該予防薬又は治療薬を投与した後、被験対象動物から時系列で2以上の時点で試料を採取し、試料におけるNPC2タンパク質レベル及び/又はIGFBP7タンパク質レベルの経時変化を調べることにより行うことができる。該当する予防薬又は治療薬の投与により、動脈硬化病変の形成、進展が抑制された場合には、被験対象動物から採取された試料におけるNPC2タンパク質レベル及び/又はIGFBP7タンパク質レベルの経時的な増加が抑制又は横ばい傾向を示す。一方、該当する予防薬又は治療薬の投与により、動脈硬化病変の形成、進展が改善された場合には、被験対象動物から採取された試料におけるNPC2タンパク質レベル及び/又はIGFBP7タンパク質レベルは経時的な減少傾向を示す。
本発明に係るマーカータンパク質を測定する方法としては、例えば、免疫測定法、質量分析法など、NPC2タンパク質、IGFBP7タンパク質を特異的に測定できる方法であれば周知のいかなる方法を用いても良い。本発明に係るマーカータンパク質を測定する際に用いる試薬において、検出剤として抗体やアプタマーなどを用いることができる。
免疫測定法としては、特に限定されないが、各種のエンザイムイムノアッセイ、ラジオイムノアッセイ(RIA)、酵素結合免疫測定法(ELISA)、二重モノクローナル抗体サンドイッチイムノアッセイ法、モノクローナルポリクローナル抗体サンドイッチアッセイ法、免疫染色法、免疫蛍光法、ウェスタンブロッティング法、ビオチン-アビジン法、免疫沈降法、金コロイド凝集法、イムノクロマト法、ラテックス凝集法(LA)、免疫比濁法(TIA)などを挙げることができる。
免疫測定法において使用する試薬として、既に市販されている抗NPC2抗体、抗IGFBP7抗体を検出剤として使用しても良いが、公知のNPC2のアミノ酸配列(配列番号1)やIGFBP7のアミノ酸配列(配列番号2)に基づいて定法により抗体を調製しても良い。NPC2、IGFBP7それぞれのアミノ酸配列の構造を特異的に認識する抗体で良いが、それぞれの糖鎖やジスルフィド結合、リン酸化等の翻訳後修飾を含む全体構造を特異的に認識するものでも良い。
NPC2タンパク質、IGFBP7タンパク質を検出可能な抗体であれば由来する動物種やクローンは特に限定されない。ウサギ、ヤギ、マウス、ラット、モルモット、ウマ、ヒツジ、ラクダ、ニワトリ等に由来する抗体が挙げられ、モノクローナル抗体でも、ポリクローナル抗体でも構わない。また、NPC2タンパク質、IGFBP7タンパク質への特異的な結合に適する全てのサブクラスの抗体を用いることができる。組換え抗体、Fab、Fab'又はF(ab')2断片のような断片を用いることももちろん可能である。
本発明に係る動脈硬化関連疾患の検出剤又は動脈硬化の評価剤として、NPC2タンパク質、IGFBP7タンパク質と結合性を有するアプタマーを利用することもできる。アプタマーの製造には、公知のNPC2のアミノ酸配列(配列番号1)やIGFBP7のアミノ酸配列(配列番号2)から、結合し得るDNA若しくはRNA又はそれらの誘導体である核酸アプタマーやアミノ酸で構成されるペプチドアプタマーを周知の方法により合成して用いれば良い。測定の際には、アプタマーの結合を発光法や蛍光法、表面プラズモン共鳴法により検出することができる。
質量分析法としては、特に限定されないが、エレクトロスプレーイオン化法(ESI)、マトリックス支援レーザー脱離イオン化法(MALDI)、表面増強レーザー脱離イオン化法(SELDI)などを用いたイオン源と、飛行時間型分析計(TOF)、イオントラップ型分析計(IT)、フーリエ変換型分析計(FT)などを組み合わせた質量分析計が利用できる。質量分析計を高速液体クロマトグラフィー(HPLC)やキャピラリー電気泳動(CE)などの分離装置と連結したLC-MSやCE-MSなどを用いることが出来る。また、質量分析データの取得方法として、データ非依存性解析(DIA)、データ依存性解析(DDA)、多重反応モニタリング法(MRM)などが挙げられる。質量分析では、試料をiTRAQ試薬(SCIEX社)などによる安定同位体標識を行う場合も含まれる。
本発明の動脈硬化関連疾患検出又は動脈硬化の進行度の評価を行う際に必要な各種の試薬類は、予めパッケージングしてキット化することができる。例えば、(i)捕捉抗体として本発明に係るマーカータンパク質に特異的なモノクローナル抗体又はポリクローナル抗体、(ii)検出抗体として本発明に係るマーカータンパク質に特異的な酵素標識化モノクローナル抗体又はポリクローナル抗体、(iii)基質溶液等の必要な試薬がキットとして提供される。
以下に本発明の実施例を示すが、本発明はこれら実施例により限定されるものではない。
〔実施例1〕
血清NPC2値とmaxIMT値、プラーク有無との関係
大動脈瘤、大動脈解離、下肢閉塞性動脈硬化症及び心筋梗塞患者を対象として、ELISA法によりNPC2の血清濃度を測定し、エコー検査による頸動脈球部のmaxIMT値、及び頚動脈エコー検査によるプラーク所見(プラーク有無)との比較を行った。NPC2は、NPC2 ELISA Kit (Aviva Systems Biology社)を用い、キット付属のプロトコールに従って測定を実施した。maxIMT値は、キヤノンメディカルシステムズ株式会社製Aplioシリーズにより頸動脈球部のプラークを含む最大壁厚を測定した。エコー検査中にプラークの存在が認められた群をPlaqueあり群、明らかなプラークを認めなかった群をPlaqueなし群とし、プラーク有無の判定とした。
血清NPC2値とmaxIMT値、プラーク有無との関係
大動脈瘤、大動脈解離、下肢閉塞性動脈硬化症及び心筋梗塞患者を対象として、ELISA法によりNPC2の血清濃度を測定し、エコー検査による頸動脈球部のmaxIMT値、及び頚動脈エコー検査によるプラーク所見(プラーク有無)との比較を行った。NPC2は、NPC2 ELISA Kit (Aviva Systems Biology社)を用い、キット付属のプロトコールに従って測定を実施した。maxIMT値は、キヤノンメディカルシステムズ株式会社製Aplioシリーズにより頸動脈球部のプラークを含む最大壁厚を測定した。エコー検査中にプラークの存在が認められた群をPlaqueあり群、明らかなプラークを認めなかった群をPlaqueなし群とし、プラーク有無の判定とした。
詳細には、ELISA法によるNPC2濃度測定は、抗NPC2抗体の固相化されたプレートに、希釈した血清検体を添加してインキュベーションした。その後、検体液を取り除き、ビオチン標識された抗NPC2抗体を添加してインキュベーションした。インキュベーション後に洗浄操作を行い、HRP (Horseradish peroxidase)標識されたアビジンを添加してインキュベーションした。続いて洗浄操作を行い、TMB (3,3',5,5'-tetramethylbenzidine)溶液を添加して発色反応を行った後、停止液を添加し、マイクロプレートリーダー(SpectraMax M2e、Molecular Devices社)を用いて主波長450nm、副波長540nmで測定を行った。そのデータを、同時に測定した各標準液の測定データから作成した検量線から濃度を算出しNPC2濃度を測定した。
血清NPC2値とmaxIMT値の相関解析は、動脈硬化関連疾患23例(下肢閉塞性動脈硬化症14例、心筋梗塞9例)を対象として実施した。その結果、血清NPC2値とmaxIMT値の相関係数は0.151(p=0.491)であった。(図1-1:横軸のB_maxIMTは、Bifurcation(頸動脈球部)maxIMTを意味する。)
血清NPC2値とmaxIMT値の傾向解析は、大動脈瘤(胸部大動脈瘤、腹部大動脈瘤及び胸部、腹部複合発症の大動脈瘤を含む。以下のすべての実施例においても同様。)48例について、maxIMT低値側から順にlow group12例、middle1 group12例、middle2 group12例、high group12例の4群に分け、各群の血清NPC2値を調べることにより行った。Jonckheere-Terpstra検定を用いて傾向性の有意差検定を行った。図1-2に示したとおり、血清NPC2値はmaxIMT値が高いほど高値になる傾向を認めた。
血清NPC2値と頸動脈プラーク有無の比較解析は、合計84例(大動脈瘤52例、大動脈解離10例、下肢閉塞性動脈硬化症14例、心筋梗塞8例)について、Plaqueなし、Plaqueありの2群に分け、各群の血清NPC2値を調べることにより行った。統計処理にはマン・ホイットニーのU検定を用いた。図1-3に示したとおり、血清NPC2値は頸動脈Plaqueありの群で、有意に高値であった(p=0.013)。
〔実施例2〕
血清NPC2値とbaPWV値との関係
大動脈瘤、大動脈解離、下肢閉塞性動脈硬化症及び心筋梗塞患者を対象として、ELISA法によりNPC2の血清濃度を測定し、baPWV値との比較を行った。NPC2の血清濃度は、実施例1に記載の方法に準じて測定した。baPWV値は、日本コーリン株式会社製BP-203RPEII(Exceed)により測定した。
血清NPC2値とbaPWV値との関係
大動脈瘤、大動脈解離、下肢閉塞性動脈硬化症及び心筋梗塞患者を対象として、ELISA法によりNPC2の血清濃度を測定し、baPWV値との比較を行った。NPC2の血清濃度は、実施例1に記載の方法に準じて測定した。baPWV値は、日本コーリン株式会社製BP-203RPEII(Exceed)により測定した。
血清NPC2値とbaPWV値の比較解析は、合計85例(大動脈瘤49例、大動脈解離11例、下肢閉塞性動脈硬化症8例、心筋梗塞17例)について、baPWVのカットオフ値を1400(cm/s)又は1700(cm/s)として2群に分け、各群の血清NPC2値を調べることにより行った。統計処理にはマン・ホイットニーのU検定を用いた。図2-1及び2-2に示したとおり、血清NPC2値は、baPWV高値の群で有意に高値であった(カットオフ値1400(cm/s)の場合;p=0.014、カットオフ値1700(cm/s)の場合;p=0.031)。
血清NPC2値は、頸動脈プラークの存在、baPWV高値群で有意に高値化する(実施例1及び2)ことから、動脈硬化の進展及び動脈硬化関連疾患のマーカーとなる可能性が示唆された。
〔実施例3〕
血清IGFBP7値とmaxIMT値、プラーク有無との関係
実施例1と同様の患者を対象として、ELISA法によりIGFBP7の血清濃度を測定し、エコー検査による頸動脈球部のmaxIMT値、及び頚動脈エコー検査によるプラーク所見(プラーク有無)との比較を行った。IGFBP7は、ELISA Kit for Insulin Like Growth Factor Binding Protein 7 (cloud-clone社)を用い、キット付属のプロトコールに従って測定を実施した。maxIMT値測定、プラーク有無の判定は、実施例1に記載の方法に準じて行った。
血清IGFBP7値とmaxIMT値、プラーク有無との関係
実施例1と同様の患者を対象として、ELISA法によりIGFBP7の血清濃度を測定し、エコー検査による頸動脈球部のmaxIMT値、及び頚動脈エコー検査によるプラーク所見(プラーク有無)との比較を行った。IGFBP7は、ELISA Kit for Insulin Like Growth Factor Binding Protein 7 (cloud-clone社)を用い、キット付属のプロトコールに従って測定を実施した。maxIMT値測定、プラーク有無の判定は、実施例1に記載の方法に準じて行った。
詳細には、ELISA法によるIGFBP7濃度測定は、抗IGFBP7抗体の固相化されたプレートに、希釈した血清検体を添加してインキュベーションした。その後、検体液を取り除き、検出試薬A液を添加してインキュベーションした。インキュベーション後に洗浄操作を行い、検出試薬B液を添加してインキュベーションした。続いて洗浄操作を行い、TMB溶液を添加して発色反応を行った後、停止液を添加し、マイクロプレートリーダー(SpectraMax M2e、Molecular Devices社)を用いて主波長450nm、副波長540nmで測定を行った。そのデータを、同時に測定した各標準液の測定データから作成した検量線から濃度を算出しIGFBP7濃度を測定した。
血清IGFBP7値とmaxIMT値の相関解析の結果、血清IGFBP7値とmaxIMT値は相関係数R=0.503と有意に(p=0.015)相関した(図3-1:横軸のB_maxIMTは、Bifurcation(頸動脈球部)maxIMTを意味する。)。また、血清IGFBP7値とmaxIMT値の傾向解析の結果は、図3-2に示したとおりであり、血清IGFBP7値はmaxIMT値が高いほど有意に高値になる傾向を示した(p<0.05)。さらに、血清IGFBP7値と頸動脈プラーク有無の比較解析の結果は、図3-3に示したとおりであり、頸動脈Plaqueありの群で血清IGFBP7が高値になる傾向を認めた。
以上のとおり、血清IGFBP7値は頸動脈球部maxIMT値と有意に相関し、頸動脈球部maxIMT値が高いほど有意に高値になることから、動脈硬化の進展及び動脈硬化関連疾患のマーカーとなる可能性が示唆された。また、実施例1~3のデータから、NPC2、IGFBP7はそれぞれ異なる動脈硬化進行の指標とより強い関連性を持つことが示された。すなわち、これらNPC2、IGFBP7を組み合わせて検査することで、より詳細な動脈硬化の進行度の情報を得ることが可能であると予想される。
〔比較例1〕
血清CRP値とmaxIMT値、プラーク有無との関係
実施例1と同様の患者を対象として、ELISA法によりCRPの血清濃度を測定し、エコー検査による頸動脈球部のmaxIMT値、及び頚動脈エコー検査によるプラーク所見(プラーク有無)との比較を行った。CRPの測定試薬にはC反応性蛋白キットCRP-ラテックスX2「生研」NX(デンカ生研社)を用い、測定装置には、LABOSPECT 008日立自動分析装置及び7180形日立自動分析装置(日立ハイテクノロジーズ社)を用いた。maxIMT値測定及びプラーク有無の判定は、実施例1に記載の方法に準じて行った。
血清CRP値とmaxIMT値、プラーク有無との関係
実施例1と同様の患者を対象として、ELISA法によりCRPの血清濃度を測定し、エコー検査による頸動脈球部のmaxIMT値、及び頚動脈エコー検査によるプラーク所見(プラーク有無)との比較を行った。CRPの測定試薬にはC反応性蛋白キットCRP-ラテックスX2「生研」NX(デンカ生研社)を用い、測定装置には、LABOSPECT 008日立自動分析装置及び7180形日立自動分析装置(日立ハイテクノロジーズ社)を用いた。maxIMT値測定及びプラーク有無の判定は、実施例1に記載の方法に準じて行った。
血清CRP値とmaxIMT値の相関解析の結果、血清CRP値とmaxIMT値の相関係数は-0.274(p=0.207)であった(図4-1:横軸のB_maxIMTは、Bifurcation(頸動脈球部)maxIMTを意味する。)。また、血清CRP値とmaxIMT値の傾向解析の結果は、図4-2に示したとおりであり、血清CRP値とmaxIMT値の間に傾向性は認められなかった。さらに、血清CRP値と頸動脈プラーク有無の比較解析の結果は、図4-3に示したとおりであり、血清CRP値と頸動脈プラーク有無に関係性は認められなかった。
〔比較例2〕
血清CRP値とbaPWV値との関係
実施例2と同様の患者を対象として、ELISA法によりCRPの血清濃度を測定し、baPWV値との比較を行った。CRPの血清濃度測定は比較例1に記載の方法、baPWV値測定は実施例2に記載の方法に準じて行った。

図5-1及び5-2に示したとおり、カットオフ値1400(cm/s)の場合には血清CRP値がbaPWV高値群で有意に高値であった(p=0.024)が、カットオフ値1700(cm/s)の場合には関係性が認められなかった。
以上の実施例1~3、比較例1、2により、NPC2、IGFBP7はいずれも動脈硬化の進展及び動脈硬化関連疾患マーカーとしてCRPより有用なマーカーであることが示された。
〔実施例4〕
非動脈硬化性胸部大動脈瘤患者と動脈硬化性胸部大動脈瘤患者における血清検体中のNPC2、IGFBP7の比較(ELISA法)
NPC2、IGFBP7の動脈硬化関連疾患マーカーとしての有用性を確認するため、非動脈硬化性胸部大動脈瘤(遺伝性結合組織疾患)患者と動脈硬化性胸部大動脈瘤患者の血清中のNPC2値、IGFBP7値の測定を行った。NPC2は実施例1に記載の方法、IGFBP7は実施例3に記載の方法に準じて測定した。統計処理には、StatFlex ver6.0(アーテック社)を使用し、有意差検定にはMann-Whitney U検定を用いた。
非動脈硬化性胸部大動脈瘤患者と動脈硬化性胸部大動脈瘤患者における血清検体中のNPC2、IGFBP7の比較(ELISA法)
NPC2、IGFBP7の動脈硬化関連疾患マーカーとしての有用性を確認するため、非動脈硬化性胸部大動脈瘤(遺伝性結合組織疾患)患者と動脈硬化性胸部大動脈瘤患者の血清中のNPC2値、IGFBP7値の測定を行った。NPC2は実施例1に記載の方法、IGFBP7は実施例3に記載の方法に準じて測定した。統計処理には、StatFlex ver6.0(アーテック社)を使用し、有意差検定にはMann-Whitney U検定を用いた。
図6-1に示すとおり、NPC2は健常者(44例)、非動脈硬化性胸部大動脈瘤患者(20例)及び動脈硬化性胸部大動脈瘤患者(29例)でそれぞれ、2.91±2.01ng/mL、3.02±3.21ng/mL、9.03±4.37ng/mLと、非動脈硬化性胸部大動脈瘤患者よりも動脈硬化性胸部大動脈瘤患者で高値であった。従って、NPC2が動脈硬化関連疾患の有用なマーカーであることが示された。
図6-2に示すとおり、IGFBP7は健常者(44例)、非動脈硬化性胸部大動脈瘤患者(20例)及び動脈硬化性胸部大動脈瘤患者(29例)でそれぞれ、139.20±28.47ng/mL、187.25±58.97ng/mL、291.96±316.83ng/mLと、非動脈硬化性胸部大動脈瘤患者よりも動脈硬化性胸部大動脈瘤患者で高値であった。従って、IGFBP7も動脈硬化関連疾患の有用なマーカーであることが示された。
本発明による動脈硬化関連疾患及び動脈硬化進行の新規マーカーであるNPC2及び/又はIGFBP7を用いることにより、動脈硬化の進行及び動脈硬化関連疾患の発症を高感度に検出することができる。そのため、本発明による動脈硬化関連疾患マーカー又は動脈硬化進行マーカーを用いる測定試薬及び試薬キットは動脈硬化関連疾患又は動脈硬化の発症のスクリーニングや早期診断、動脈硬化進行度のモニタリング、動脈硬化関連疾患の再発リスク評価、将来発症するリスクの評価、予防薬や治療薬の開発、評価等に使用することができ、また、本発明は、当該測定試薬及び試薬キットを製造する産業において利用することができる。
本明細書で引用した全ての刊行物、特許及び特許出願はそのまま引用により本明細書に組み入れられるものとする。
Claims (20)
- 対象から得られた試料において、以下の1)及び/又は2)を測定する工程を含む、動脈硬化関連疾患を検出するための方法。
1)NPC2(Niemann-Pick disease type C2)タンパク質、
2)IGFBP7(Insulin-like growth factor-binding protein 7)タンパク質。 - 動脈硬化関連疾患の再発のリスクを有する対象から得られた試料において、以下の1)及び/又は2)を測定する工程を含む、動脈硬化関連疾患の再発を検出又はスクリーニングするための方法。
1)NPC2(Niemann-Pick disease type C2)タンパク質、
2)IGFBP7(Insulin-like growth factor-binding protein 7)タンパク質。 - 対象から得られた試料において、以下の1)及び/又は2)を測定する工程を含む、動脈硬化関連疾患を将来発症するリスクを有する対象を検出又はスクリーニングするための方法。
1)NPC2(Niemann-Pick disease type C2)タンパク質、
2)IGFBP7(Insulin-like growth factor-binding protein 7)タンパク質。 - 動脈硬化関連疾患又は動脈硬化の予防薬又は治療薬の投与前に、動脈硬化関連疾患又は動脈硬化の予防又は治療を必要とする対象から得られた試料において、以下の1)及び/又は2)を測定する工程と、
動脈硬化関連疾患又は動脈硬化の予防薬又は治療薬の投与後に、前記対象から得られた試料において、以下の1)及び/又は2)を測定する工程と、
を含むか、又は、
動脈硬化関連疾患又は動脈硬化の予防薬又は治療薬の投与後に、動脈硬化関連疾患又は動脈硬化の予防又は治療を必要とする対象から時系列で2以上の時点で得られた試料において、以下の1)及び/又は2)を測定する工程を含む、
動脈硬化関連疾患又は動脈硬化の予防薬又は治療薬の効果を評価する方法。
1)NPC2(Niemann-Pick disease type C2)タンパク質、
2)IGFBP7(Insulin-like growth factor-binding protein 7)タンパク質。 - 前記動脈硬化関連疾患が心筋梗塞、末梢動脈疾患、大動脈瘤、大動脈解離、アテローム血栓性脳梗塞、一過性脳虚血発作、腎動脈狭窄症、内頚動脈狭窄症、狭心症からなる群より選択される、請求項1~4のいずれか1項に記載の方法。
- 対象から得られた試料において、以下の1)及び/又は2)を測定する工程を含む、動脈硬化の進行度を評価するための方法。
1)NPC2(Niemann-Pick disease type C2)タンパク質、
2)IGFBP7(Insulin-like growth factor-binding protein 7)タンパク質。 - 試料が、血清、血漿、血液、尿及び唾液から成る群より選択される、請求項1~6のいずれか1項に記載の方法。
- 前記方法は、前記対象が動脈硬化関連疾患又は動脈硬化の予防又は治療を必要とする動物であり、前記NPC2タンパク質及び/又はIGFBP7タンパク質を指標として、動脈硬化関連疾患又は動脈硬化の予防薬又は治療薬の効果を評価又は判定する、請求項1~7のいずれか1項に記載の方法。
- 前記対象がヒトである、請求項1~8のいずれか1項に記載の方法。
- 測定方法が免疫測定法又は質量分析法である、請求項1~9のいずれか1項に記載の方法。
- 以下の1)及び/又は2)を含む動脈硬化関連疾患を検出するためのマーカー。
1)NPC2タンパク質、
2)IGFBP7タンパク質。 - 前記動脈硬化関連疾患が心筋梗塞、末梢動脈疾患、大動脈瘤、大動脈解離、アテローム血栓性脳梗塞、一過性脳虚血発作、腎動脈狭窄症、内頚動脈狭窄症、狭心症からなる群より選択される、請求項11に記載のマーカー。
- 以下の1)及び/又は2)を含む動脈硬化の進行度を評価するためのマーカー。
1)NPC2タンパク質、
2)IGFBP7タンパク質。 - 請求項11~13のいずれか1項に記載のマーカーを測定するための抗体又はアプタマーを含む、動脈硬化関連疾患を検出するための、又は動脈硬化の進行度を評価するための測定試薬。
- 前記抗体がモノクローナル抗体及び/又はポリクローナル抗体である請求項14に記載の測定試薬。
- 請求項14又は15に記載の測定試薬を含む、動脈硬化関連疾患を検出するための、又は動脈硬化の進行度を評価するための試薬キット。
- 動脈硬化関連疾患を検出するために、又は動脈硬化の進行度を評価するために使用される試料が、血清、血漿、血液、尿及び唾液から成る群より選択される、請求項11~16のいずれか1項に記載のマーカー、測定試薬又は試薬キット。
- 動脈硬化関連疾患又は動脈硬化の予防薬又は治療薬のスクリーニングのための、請求項11~13のいずれか1項に記載のマーカーの使用。
- 動脈硬化関連疾患の再発をin vitroで検出又はスクリーニングするための、請求項11~13のいずれか1項に記載のマーカーの使用。
- 動脈硬化関連疾患を将来発症するリスクを有する対象をin vitroで検出又はスクリーニングするための、請求項11~13のいずれか1項に記載のマーカーの使用。
