Abstract
検体中のPSAを修飾している糖鎖を解析し、マルチシアリル化LacdiNAc構造、特にGlycanID:7512、GlycanID:7603、GlycanID:7612、及び/又はGlycanID:7613の存在量を検出することによる前立腺がんを識別する検査方法を提供する。さらに、GlycanID:7512、及び/又はGlycanID:7603の相対存在量から、PSA G-インデックスを算出することによって、PSA値グレーゾーンの患者であっても前立腺がんを特異度良く検出することが可能となった。
Description
本発明は、前立腺がん診断のための指標、及びこれを用いた検査方法に関する。
前立腺がんは男性に特有のがんであるが、高齢者に多発するがんであり、患者の約90%以上が60歳以上で占められている。高齢者の増加とともに、患者が増加しており、国立がん研究センターがん情報サービスの「がん登録・統計」の2017年の男性がん部位別罹患数予測によれば、新たに前立腺がんに罹患する患者数は、86,100人であり、胃がん、肺がんに続き罹患数では第3位となっており、今後高齢化に伴いさらに増加すると予測されている。前立腺がんは多くの場合比較的ゆっくり進行することや、手術に加えて放射線治療、内分泌(ホルモン)療法など有効な治療法をとることができることから、早期に発見すれば治療が可能である。
前立腺がんは、血液中のPSA(Prostate specific antigen、前立腺がん抗原)を測定することによって診断されている。PSA検査は、血液検査であることから低侵襲であり、感度が高いことから、前立腺がんの検査として、検診、人間ドックにも取り入れられ検査が行われている。
しかし、PSAは前立腺上皮で産生され、前立腺がんだけではなく、前立腺肥大、前立腺炎のような他の前立腺疾患においても過剰に産生される。そのため、PSAによる患者スクリーニングは、偽陽性が非常に多いことが問題とされている。PSAは4ng/mlをカットオフ値とし、血中のPSA値がそれ以上の場合には、前立腺がんの疑いがあるとされ、経直腸前立腺触診、経直腸エコー、あるいは経会陰前立腺針生検によって診断を行う必要がある。PSA値4~10ng/mlの「グレーゾーン」と言われる領域では70%程度が、10~20ng/mlでも半数はがんではないという結果も得られている(非特許文献1)。
経会陰前立腺針生検によって診断を確定する場合には、入院、麻酔のうえ初回の生検で10~12ヶ所の組織採取を行う必要があり激しい痛みを伴う。PSA値がグレーゾーンの領域の患者では偽陽性率が高いこともあり、検診によって二次検査を勧められても前立腺生検を行わない者も多く、早期発見の機会を見逃す場合も少なくない。実際に検診センターではPSA陽性と判定された者のうち、30~50%程度しか前立腺生検を受けていないという統計もある。
従来からPSA検査の特異度を高め、前立腺がんと前立腺肥大を区別し、前立腺がんを特異的に検査する方法が提案されている(非特許文献2~4、特許文献1~8)。すでに、FDA(アメリカ食品医薬品局)は、PHI(prostate health index、非特許文献2)、及びPCA3(prostate cancer antigen 3)検査(非特許文献3、4)をPSA検査として承認している。特許文献1~5には、PSAを特定の結合型シアル酸を認識するレクチンと接触させ、レクチンとの結合性を利用して検査する方法が記載されている。また、特許文献6~8には、質量分析によって糖鎖構造を解析し、前立腺がんを検出する方法が開示されている。

しかしながら、上記文献に開示されている検査方法はいずれも特異度、あるいは感度に問題があり、前立腺がんを特異的に識別できる方法ではなかった。PHIは、感度は85%と高いものの、特異度は45%と低く、前立腺がんを特異的に識別することはできない(非特許文献5)。また、PCA3は前立腺肥大や前立腺炎といったがん以外の疾患に影響を受けないという点は優れた検査方法ではあるものの、4つの臨床試験の結果から、感度が53~84%と低くPSA値がグレーゾーンを示す患者で前立腺がんを検出するのには不十分であった(非特許文献6)。
特許文献1~5に記載の方法は、レクチンとの結合性によってPSAを修飾している糖を解析して前立腺がんを診断している。しかし、レクチンは抗体と比較した場合に親和性が低く、感度が低いという問題がある。また、レクチンは標的とするPSAを修飾している糖のみを検出するわけではなく、他のタンパク質を修飾している糖にも結合するという問題がある。そのため、レクチンを用いた方法では偽陽性を完全に排除することができない。
特許文献6~8に記載の方法は、質量分析を用いて前立腺がんと前立腺肥大を識別する方法が開示されている。特許文献6は、フコース結合糖鎖、フコース非結合糖鎖の比によって、特許文献7はLacdiNAcを有する糖鎖の量によって、特許文献8には、LacdiNAcを有する3本鎖以上の糖鎖の有無によって、前立腺がんを検出する方法が記載されている。しかしながら、いずれも特異度が低く偽陽性を排除するにはいたらなかった。
PSAは糖鎖による修飾を受けており、修飾される糖の種類と前立腺疾患とが相関するということが報告されている。しかしながら、前立腺がんと前立腺肥大を精度良く区別するにはいたっていない。本発明は、PSAの増加がわずかに認められるグレーゾーンの領域であっても、精度良く前立腺がんを検出できる方法、及び前立腺がんを識別するための指標を提供することを課題とする。
本発明は、以下の前立腺がんを検査する方法、及び前立腺がんを検出するためのインデックスに関する。さらに、グレード(悪性度)を判断するためのマーカーに関する。
(1)前立腺がんの検査方法であって、検体中のPSAを修飾している糖鎖を解析し、マルチシアリル化LacdiNAc構造を解析することを特徴とする前立腺がんの検査方法。
(2)前立腺がんの検査方法であって、検体中のPSAを修飾している糖鎖を解析し、GlycanID:7512、GlycanID:7603、GlycanID:7612、及び/又はGlycanID:7613の相対存在量を解析することを特徴とする(1)記載の前立腺がんの検査方法。

本発明者らの解析によって、前立腺がんの患者ではLacdiNAc(GalNAcβ1-4GlcNAc、N-アセチルガラクトサミン-N-アセチルグルコサミン)構造を有する特定の糖鎖が有意に増加していることが明らかとなった。ここでは、質量分析を用いて解析を行っているが、マルチシアリル化LacdiNAc構造、あるいは以下にGlycanIDで示す特定の糖鎖を認識、又は解析することができれば、どのような方法を用いてもよい。
質量分析による解析方法は、以下に示すように非常に感度良く検査できる方法であるが、質量分析によらずマルチシアリル化LacdiNAc構造を認識する抗体、レクチン、アプタマーなど特定の糖鎖を認識すればどのようなものを用いて解析してもよい。抗体を用いて検出する場合には、ELISA、SPRなど、本分野で用いられているどのような方法によって検出を行ってもよい。また、レクチンによって検出する場合には、レクチンブロット、キャピラリー電気泳動など、どのような方法を用いて検出を行ってもよい。以下の実施例で示すように、WFA、MAMなどのレクチンはマルチシアリル化LacdiNAc構造を特異的に認識するものではない。しかし、いくつかのレクチンを組み合わせて用いることによって、マルチシアリル化LacdiNAc構造を特異的に検出することができる系を作成すればよい。
また、GlycanID:7512、及びGlycanID:7603の相対存在量による解析だけではなく、マルチシアリル化LacdiNAc構造を含む糖のプロファイリングデータのプロファイル認識からも本発明を実施することが可能である。具体的には、前立腺がんが確定した患者から複数収集した糖鎖のプロファイルを教師用の学習データとしてコンピューターに機械学習させ、診断支援用システムとする。その後、検査時には被験者から得られたプロファイルをそのまま入力することによって、パターン認識から前立腺がん候補データを検出してもよい。
以下の実施例で示すように、PSAを修飾している糖鎖構造を解析した結果、以下に定義するPSA G-インデックスを用いることにより、前立腺がんを精度良く検出することができることが明らかとなった。従来のPSA試験でグレーゾーンの患者に、二次スクリーニングとして、PSA G-インデックスを用いて検査することによって、非侵襲的に前立腺がんの患者を検出することができる。以下にデータを示しながら詳細に説明する。
最初にPSA標準品を用いて、PSAを修飾している糖鎖を感度良く検出、定量できるか確認を行った。ヒト精液から得られたPSAを8M尿素、50mM HEPES-NaOH、pH8.0に溶解し、10mM DTT(GE Healthcare Life Science株式会社)で還元し、25mM ヨードアセトアミド(Sigma-Aldrich株式会社)でアルキル化した後に、Trypsin/Lys-C mix(プロメガ株式会社)によって消化した。糖タンパク質は、親水性精製方法(非特許文献7)によって濃縮し、真空乾燥した。
得られた糖タンパク質は、糖鎖を同定するために、Prominenceナノフロー液体クロマトグラムを連結させたトリプル四重極型質量分析計LCMS-8060(株式会社島津製作所)を用いて、オキソニウムイオンモニタリング法(Erexim法、非特許文献8、特許文献9)によって解析した。
図1Aに、多重反応モニタリング質量分析(multiple reaction monitoring mass spectrometry、MRM-MS)を応用したオキソニウムイオンモニタリング法の概要を模式的に示している。PSAはトリプシンによって、ペプチドと糖ペプチドの混合物に分解され、ナノフロー液体クロマトグラムによって分離される。質量分析計においては、第1の四重極(Q1)において、特定の質量を有する糖ペプチドイオンを単離する。単離された糖ペプチドイオンは第2の四重極(Q2)に導入されCID(collision induced dissociation、衝突誘起解離)を引き起こす。第3の四重極(Q3)では、フィルターによって糖由来のオキソニウムイオンを選択的に検出する。最適衝突エネルギー条件下で、m/z138.1のオキソニウムイオンは、種々の糖構成の糖ペプチドの定量的なレポーターとして機能する。
ヒト精液から得られたPSA標準品100ngを用いて、糖鎖構造解析を行った結果を図1Bに示す。図中横軸に記載している数字は、GlycanIDとして各グリカンの種類を規定するものであり、左からHexNAc、Hexose、Fucose、Neu5Acの数に相当する。PSAの45位のアスパラギンを修飾している糖の種類、及び量的な分布を解析した。その結果、オキソニウムイオンモニタリング法により、67種の糖構造が存在することが明らかとなった。
オキソニウムイオンモニタリング法は、酵素によるグリカンの分離や、化学修飾を行う必要がなく、LS/MSの作動時間が25分という短時間で解析が終了するだけではなく、非常に鋭敏な検出方法であった。最も含有量の多い糖は、4[HexNAc]5[Hex]1[Fuc]2[Neu5Ac](GlycanID:4512、44.5±0.9%)であり、最も含有量の少ない糖は、6[Hex]7[NAcHex]0[Fuc]0[Neu5Ac](GlycanID:6700、0.01±0.001%)であった。従来、PSAを修飾しているグリカンとして67種という非常に多くの種類の糖を検出したことや、0.01%という非常に微量の糖の検出を行ったという報告はなく、非常に感度の高い方法であることが示された。
PSA糖鎖解析のダイナミックレンジを図1Cに示す。トリプシン消化したPSAを段階希釈し、検出限界の解析を行った。その結果、0.03ngのPSAから得られた糖ペプチド1fmolまで検出することが可能であり、定量的なダイナミックレンジは5桁以上であった(R2>0.99)。この結果は、オキソニウムイオンモニタリング法による糖鎖解析が、4~10ng/mlというグレーゾーンのPSAであっても、十分に検査可能であることを示している。
次に、PSA値が4~10ng/mlのグレーゾーンを示す前立腺がん、又は前立腺肥大患者各15名から得られたサンプルを用いてPSAの糖鎖解析を行い、トレーニングセットとして前立腺がんを識別する指標を求めた。表1のトレーニングセットに患者特性を示す。
臨床サンプルを用いた解析は、前立腺がん患者、及び前立腺肥大患者の血清サンプルを用いている。なお、最終診断は前立腺生検による組織病理学的診断によっている。PSAの精製は以下のようにして行った。血清に、4倍量の洗浄液(0.1% Tween-20を添加したPBS)を加え、抗PSAモノクローナル抗体(Fitzgerald社)を固定化したProtein G Sepharoseと混合し、4°Cで一晩反応させ、洗浄後、8M尿素、50mM HEPES-NaOH、pH8.0によってPSAを溶出させ、精製して用いた。溶出されたタンパク質は、10mM DTTで還元し、25mMヨードアセトアミドでアルキル化した後に、Trypsin/Lys-C mixによって消化した。糖タンパク質は、親水性精製方法(非特許文献7)によって濃縮し、真空乾燥した。なお、ここでは血清サンプルを用いているが、血液由来のサンプルだけではなく、尿もサンプルとして使用することができる。
上記と同様の方法により、PSA上の52の糖鎖構造を定量することができた(図2A)。GlycanID:7512、7603、7612、7613の糖鎖構造は、前立腺肥大患者群、前立腺がん患者群間で有意に量的な差が見られた。また、ここではデータを示さないが、PSAを修飾している糖鎖は、前立腺がん患者群と健常者群間でも異なることを見出している。したがって、糖鎖構造を解析することによって、前立腺がん患者と、それ以外の者、すなわち前立腺がん以外の前立腺疾患及び健常者を区別することができる。
52種類の糖鎖のうち、マルチシアリル化構造、具体的にはジ-/トリ-シアリル化LacdiNAc(GalNAcβ1-4GlcNAc)を含有している糖鎖は、前立腺肥大患者群に比べて前立腺がん患者群で有意に増加していた(図2(D)、p=0.0023)。一方、シアリル化LacdiNAcの総量(図2B)や単一のシアリル化LacdiNAcが付加された糖鎖構造(図2C)は、前立腺肥大患者群、前立腺がん患者群間では有意な変化を示さなかった。また、末端LacdiNAc構造およびシアル酸(Neu5Ac)残基の存在は、Erexim法によって確認した(図2E、F)。
これらの知見に基づいて、PSA検査の特異性を補完し、偽陽性率を確実に低減できる新規診断アルゴリズムの確立を目指した。トレーニングセットで前立腺がん患者群と前立腺肥大患者群で有意に差が認められた糖鎖であるGlycanID:7512(p=9.91×10-8)、GlycanID:7603(p=1.66×10-5)の2つの前立腺がんに特異的な糖鎖構造を選択し、相対量をプロットした。図3AにGlycanID:7512の、図3BにGlycanID:7603の前立腺肥大患者(BPH)、前立腺がん患者における相対存在量を示す。GlycanID:7512、7603ともに、両者で有意に存在量が異なっている。
これらの結果をもとにロジスティック回帰に基づく診断モデル(PSA G-インデックス)を構築した。式1中pは、PSA G-インデックスの値を示し、x1は、GlycanID:7512の相対存在量、x2は、GlycanID:7603の相対存在量を示す。
PSA G-インデックスのカットオフ値を0.5に設定した場合、トレーニングセットの感度、特異度は、夫々93.3%、100%であった(図3C)。なお、PSA G-インデックスはロジスティック解析に基づくものであるから、今後解析する検体数の増加によって、式が多少異なってくる可能性があるが、GlycanID:7512、GlycanID:7603を検出し、その量によって、感度、特異度良く前立腺がんと他の前立腺疾患とを識別することができる。
また、ここではPSAを修飾している糖をすべて解析しその相対的な量を求めているが、マルチシアリル化LacdiNAc構造を有する糖など、特定の糖のみの解析を行ってもよい。また、PSAのペプチドのうち、糖修飾されない領域のペプチドを基準とすることにより、特定の糖によって修飾されているPSAの全PSAに対する割合を求めて解析に用いてもよい。上述した糖の相対値は、基準として用いる糖、あるいはPSA量によって異なるものとなるが、前立腺がんとそれ以外の前立腺疾患では有意にその量が異なっていることから、ロジスティック解析によって、前立腺がんとそれ以外の前立腺疾患とを感度良く区別することが可能である。
次に、表1の検証サンプルセットを用いてPSA G-インデックスを評価した(図3D)。15例の前立腺肥大患者群、15例の前立腺がん患者群いずれの群でも、すべての臨床サンプルが正しい疾患として診断された。ROC曲線解析では、PSA G-インデックスの曲線下面積(AUC)は1.00(100%感度、及び100%特異度)であったが、PSA値(Total PSA)、あるいは遊離型PSAと総PSAの比(PSA f/T)のAUCは、夫々0.50(感度80.0%、特異度33.3%)、0.60(感度73.3%、特異度60.0%)であった(図3E)。これらの結果は、PSA G-インデックスが、従来の前立腺がんの指標とされているPSA値、あるいはPSA f/T値と比較して、前立腺がん診断の特異度を大幅に改善し、偽陽性を回避できることを示している。
次に、血清中のPSA上の糖鎖構造が、前立腺がんのグレード(悪性度)を反映しているかどうかを解析した。表2に示すグレードの異なる77人の患者の血清を用いて、オキソニウムイオンモニタリング法による糖鎖解析を実施した。
その結果、複数のシアリル化LacdiNAcが付加された糖鎖構造であるGlycanID:7512(図4A)またはGlycanID:7603(図4B)の頻度は、前立腺生検による悪性度の指標であるグリーソンスコア(GS)と正の相関があった(p=3.34×10-8、又はp=2.56×10-9)。なお、グリーソンスコアとは、前立腺がんは同じ前立腺の中に悪性度の異なるがんが発生することから、生検により優勢病変、随伴病変を判定し、その数値の合計でGS2からGS10の9段階に分類するものであり、数値が大きいほど悪性度が高いことを示している。
さらに、GlycanID:3401(図4C)、又はGlycanID:5602(図4D)はグリーソンスコアに対して負の相関を示した(p=1.69×10-6、又はp=9.66×10-7)。これらの結果は、前立腺がんの病理学的悪性度と、特定の糖鎖の量が相関し、リキッドバイオプシーによって、前立腺がんの悪性度診断を行える可能性を示している。
血清PSA中のマルチシアリル化LacdiNAc構造の増加の原因を確認するために、正常、前立腺肥大患者、前立腺がん患者の前立腺組織を夫々9、31、111含む組織マイクロアレイ(US Biomax社)を用いてレクチン組織化学染色を行った。LacdiNAc構造を検出するレクチンであるWFA、又はSiaα2-3Gal構造を検出するレクチンであるMAMを用いて組織化学染色を行った。WFA、MAMともにビオチン化されたレクチンを用いVectastain Elite ABCキット(Vector Laboratories社)を用いて解析を行った。その結果、がん細胞の細胞質および原形質膜が有意に染色された(図5A)。さらに、染色強度を「強い」、「中程度」、「弱い」の3段階に分類し、前立腺がん患者の組織と、正常及び前立腺肥大患者の組織染色像を比較した。前立腺がん組織では、WFAを用いて染色した場合には、9.00倍、MAMを用いた場合には2.24倍、染色強度の「強い」像が高い頻度で観察された(図5B、5C)。
WFAやMAMレクチンは、それぞれLacdiNAcまたはNeu5Acα2-3Gal構造を含むGalNAc構造を特異的に認識する。上記結果は、前立腺がん組織では、LacdiNAcやNeu5Ac構造を含む糖タンパク質を強く発現している傾向を示している。これらの組織学的知見は、質量分析によるPSA糖鎖構造解析の結果と一致していた。したがって、組織生検を行うことなく、血液中のPSAの糖鎖を解析することによって、前立腺がん、さらに前立腺がんの病期を判定することが可能である。
PSA値から前立腺がんの疑いがあると診断された患者の二次スクリーニングとして、実施例で示した診断マーカー、PSA G-インデックスを用い、検査を行うことによって、特異度良く前立腺がんを識別することができる。早期の前立腺がんであっても、特異度良く識別することができ、早期治療に結びつけることが可能となる。
