Description
본 명세서는 사이토카인 유도 살해세포를 생산하는 방법에 관한 것으로, 구체적으로는 말초혈액유래 단핵세포를 항체 및 혈장이 포함된 배지에 배양하는 1차 증식 및 같은 배지에서 세포의 수평 및 수직 배양을 진행하는 2차 증식 과정을 포함하여 제조된 사이토카인 유도 살해세포 생산 방법에 관한 것이다. 또한 본 명세서의 사이토카인 유도 살해세포 생산방법으로 제조된 사이토카인 유도 살해세포는 종양의 감소 효과를 가져와 항암치료를 위하여 사용되는 사이토카인 유도 살해세포 생산방법에 관한 것이다.
최근 들어 면역세포치료가 기존의 표준치료법에 더하여 암을 치료하는 중요한 수단중의 하나로 떠오르고 있으며 면역세포치료는 암조직에 대항하는 인체 스스로의 반응에 기반 한 것이므로 다른 치료수단에 의한 심각한 부작용을 피할 수 있다는 장점이 있다. 항암치료에 사용되는 면역세포에는 림포카인 활성화 살해세포(Lymphokine-activated killer cell, LAK), 종양 침윤 림프구 (Tumor-infiltrating lymphocyte), 항원특이 독성 T 림프구(Antigen specific CTL), 그리고 사이토카인 유도 살해세포 (Cytokine-induced killer cell, CIK) 등이 있으며 암의 종류 및 상태에 따라 효능과 부작용의 정도에 차이가 있다.
사이토카인 유도 살해세포(CIK, Cytokine Induced Killer cell)는 자연살해세포(Natural Killer cell)의 특징과 기능을 가지고 있는 T세포이며 효과적인 암세포 파괴능력을 가지고 있고 MHC와 무관하게 다양한 종류의 암세포를 제거할 수 있다고 알려져 있으며 국내외에서 이미 림프종, 췌장암, 간세포암, 골수종, 신세포암, 비소세포폐암, 위암, 그리고 진행성 췌장암 등에 임상시험 중이거나 시험완료한 것으로 알려져 있다.
그러나 기존에 보고된 사이토카인 유도 살해세포의 증식 방법은 고가의 장비를 사용하여야 하며, 고가의 다양한 사이토카인을 고농도로 이용하여야 하는바 경제적으로 안정된 일부 환자에게만 해당 치료가 가능한 실정이다.
이러한 배경하에서 본 발명자들은 말초혈액유래 단핵세포를 1차적으로 항체 및 혈장이 포함된 배양배지에 넣어 배양한 다음 2차 배양단계에서 수평배양 후 수직배양을 실시하여 수득된 사이토카인 유도 살해세포가 우수한 항암효과가 있음을 발견하고 본 발명을 완성하게 되었다.
본 발명의 일 측면에 따른 목적은 고가의 장비나 고농도의 사이토카인을 사용하지 않고, 활성화된 말초혈액 유래 사이토카인 유도 살해세포를 효율적으로 수득하기 위한 말초혈액 유래 사이토카인 유도 살해세포의 새로운 유도 및 증식 방법을 제공하는데 있다.
본 발명은 일 측면에서, (a) 말초혈액을 항응고제가 포함된 진공 채혈관을 이용하여 채혈하고, 인산염완충용액으로 희석한 뒤 밀도구배 원심 분리법을 통해 말초혈액유래 단핵세포를 분리한 뒤 세척하는 단계, (b) 말초혈액유래 단핵세포를 항체 및 혈장이 포함된 배양배지에 넣어 배양하는단계, (c) 배양된 말초혈액유래 단핵세포를 부유하여 옮긴 후에 원심분리한 뒤 세포를 제외한 상등액을 제거하고 다시 배양용기에 접종하는 단계, (d) 접종된 말초혈액유래 단핵세포를 수평으로 눕혀 배양하는 단계, (e) 수평배양된 말초혈액유래 단핵세포를 수직으로 세워 배양하는 단계를 포함하는, 사이토카인 유도 살해세포의 생산 방법을 제공한다.
본 발명의 일 측면에 따른 생산 방법에 있어서, 상기 혈장은 자가유래 (autologous) 혈장 또는 인간혈청AB (human serum AB)인 것을 특징으로 할 수 있다.
본 발명의 일 측면에 따른 생산 방법에 있어서, 상기 항체는 항CD3 (Anti-CD3) 인 것을 특징으로 할 수 있다.

아울러, 본 발명의 일 측면에 따르면, 채혈자로부터 채집된 혈액을 인산염완충용액으로 희석한 뒤, 희석된 시료에서 밀도구배 원심분리방법을 통해 말초혈액 유래 단핵세포를 분리하고, 인산염완충용액으로 세척한 뒤 1차적으로 항 CD3항체 및 채혈자의 혈장 또는 인간 혈청 AB (human serum AB)가 포함된 배양배지에서 배양하고, 1차 증식이 완료된 세포를 부유하여 튜브로 옮긴 뒤 원심분리 하여 상등액을 제거한 뒤, 2차로 세포를 배양할 때 1차와 마찬가지로 항 CD3항체 및 채혈자의 혈장 또는 인간 혈청 AB가 포함된 배양배지에서 배양하되, 처음에는 수평으로 눕혀 (lay-down) 배양하고, 나중에는 수직으로 세워 (up-right) 배양하고, 이를 원심분리하여 세포를 수거하는 단계를 포함하는, 사이토카인 유도 살해세포 생산 방법을 제공한다. 배양할 때 사용 하는 혈장은 다양한 동물로부터 수득하거나 기존에 알려진 혈장을 구입하여 사용할 수 있으나, 바람직하게는, 배양할 때 혈장은 채혈자의 자가유래(autologous) 혈장을 사용하는 것을 특징으로 한다.
일 구현 예에 따르면, 본 발명은 본 발명의 일 측면에 의해 생산한 사이토카인 유도 살해세포를 포함하는 항암 세포 치료 조성물일 수 있다.
상기 사이토카인 유도 살해세포의 투여량은 세포수 기준으로 1 X 108 내지 1 X 1010 일 수 있다. 일 측면에서 상기 세포수는 1 X 108 이상, 5 X 108 이상, 1 X 109 이상, 3 X 109 이상, 6 X 109 이상, 9 X 109 이상, 또는 12 X 109 이상일 수 있다. 다른 측면에서 상기 세포수는 15 X 109 이하, 12 X 109 이하, 9 X 109 이하, 6 X 109 이하, 3 X 109 이하, 1 X 109 이하, 5 X 108 이하, 또는 1 X 108 이하일 수 있다. 바람직하게는 상기 세포수는 1 X 109 내지 15 X 109 일 수 있다.
본 발명 일 측면에 따른 조성물은 약학 조성물일 수 있다. 또한, 본 발명의 일 측면에 따른 조성물은 인간, 개, 닭, 돼지, 소, 양, 기니아피그 또는 원숭이를 포함하는 모든 동물에 적용될 수 있다.
본 발명의 일측면에 따른 조성물은 경구, 직장, 경피, 피내, 혈관 내, 근육 내, 복강 내, 정맥 내, 골수 내, 경막 내 또는 피하 등으로 투여될 수 있다. 바람직하게는 혈관 내로 투여되는 것일 수 있다.
경구 투여를 위한 제형은 정제, 환제, 연질 또는 경질 캅셀제, 과립제, 산제, 액제 또는 유탁제일 수 있으나, 이에 제한되는 것은 아니다. 비경구 투여를 위한 제형은 주사제, 점적제, 로션, 연고, 겔, 크림, 현탁제, 유제, 좌제, 패취 또는 분무제일 수 있으나, 이에 제한되는 것은 아니다.
본 발명의 일측면에 따른 약학 조성물은 필요에 따라 희석제, 부형제, 활택제, 결합제, 붕해제, 완충제, 분산제, 계면 활성제, 착색제, 향료 또는 감미제 등의 첨가제를 포함할 수 있다. 본 발명의 일측면에 따른 약학 조성물은 당업계의 통상적인 방법에 의해 제조될 수 있다.
본 발명의 일측면에 따른 약학 조성물의 유효 성분은 투여 받을 대상의 연령, 성별, 체중, 병리 상태 및 그 심각도, 투여 경로 또는 처방자의 판단에 따라 달라질 것이다. 이러한 인자에 기초한 적용량 결정은 당업자의 수준 내에 있으며, 이의 1일 투여 용량은 예를 들어 0.01 ㎍/kg/일 내지 10g/kg/일, 구체적으로는 0.1 ㎍/kg/일 내지 1g/kg/일, 더 구체적으로는 0.5 ㎍/kg/일 내지 100 mg/kg/일이 될 수 있으나, 용량에 따른 효과의 차이를 보이는 경우 이를 적절히 조절할 수 있다. 본 발명의 일측면에 따른 약학 조성물은 1일 1회 내지 3회 투여될 수 있으나, 이에 제한되는 것은 아니다.
본 발명의 일측면에 따른 조성물의 제형은 특별히 한정되지 않으나, 예를 들어, 정제, 과립제, 분말제, 액제, 고형 제제, 주사제 등으로 제형화될 수 있다. 각 제형은 유효 성분 이외에 해당 분야에서 통상적으로 사용되는 성분들을 제형 또는 사용 목적에 따라 당업자가 어려움 없이 적의 선정하여 배합할 수 있으며, 다른 원료와 동시에 적용할 경우 상승 효과가 일어날 수 있다.
본 발명에 따른 조성물은 본 발명의 일 측면에 의해 생산한 사이토카인 유도 살해세포를 적당량 함유할 수 있다. 적당한 함유량의 선택은 예를 들어 바람직한 투여량, 빈도 및 활성 성분의 전달의 방법과 같은 인자들에 의존적이다.
본 발명은 다양한 변환을 가할 수 있고 여러 가지 실시예를 가질 수 있는 바, 이하, 본 발명의 일 측면을 보다 구체적으로 설명한다. 그러나, 이는 본 발명을 특정한 실시 형태에 대해 한정하려는 것이 아니며, 본 발명의 사상 및 기술 범위에 포함되는 모든 변환, 균등물 내지 대체물을 포함하는 것으로 이해되어야 한다. 본 발명의 일 측면을 설명함에 있어서 관련된 공지 기술에 대한 구체적인 설명이 본 발명의 요지를 흐릴 수 있다고 판단되는 경우 그 상세한 설명을 생략한다.

[실시예 1]
말초혈액 채취와 인간
말초혈액유래
단핵세포
분리, 혈장의 제조
건강한 정상인으로부터 말초혈액 5cc ~ 100cc을 heparin과 같은 항응고제가 포함된 진공 채혈관을 이용하여 채혈하였다. 채취된 혈액과 인산염완충용액 (phosphate buffered saline, PBS)을 일정한 비율로 희석하였다. 희석된 시료를 Lymphoprep 혹은 Ficoll-Paque를 이용한 밀도구배 원심분리방법을 통해 인간 말초혈액유래 단핵세포를 분리하고, 인산염완충용액(phosphate buffered saline, PBS)으로 2회 세척한 후에 실험에 사용하였다.
혈장의 제조를 위하여, 말초혈액 10 cc ~ 100 cc을 150 g ~ 200 g의 속도로 5분 ~ 10분간 원심분리하여 혈장 성분인 상등액을 회수하였다.
[실시예 2]
사이토카인 유도 살해세포의 제조
1차 증식
인간 말초혈액유래 단핵세포(human peripheral blood mononuclear cells, hPBMC)를 1 x 105/㎖ ~ 1 x 106/㎖의 농도가 되도록 세포배양용기에 접종하였다. 5 ~ 20 ng/㎖ OKT-3(Anti-CD3 항체), 150 U/㎖ ~ 500 U/㎖ IL-2, 5 ~ 10% 채혈자의 혈장 (Plasma) 또는 5 ~ 10% human serum AB를 함유한 CellGro SCGM serum-free medium(CellGenix) 배양배지에 넣어 37℃, 5% CO2 조건 하에서 5일간 배양하였다.
2차 증식
1차 증식이 완료된 세포를 조심스럽게 부유하여 코니칼튜브로 옮긴 후에 1000 ~ 1800 rpm의 속도로 5분 ~ 10분간 원심분리하였다. 세포(pellet)를 제외한 상등액을 제거하고 배양배지 CellGro SCGM serum-free medium(CellGenix) 1 ㎖에 세포를 부유한 후에 Tryphan-blue로 염색하여 세포수를 측정하였다. 1 x 105/㎖ ~ 1 x 106/㎖의 농도가 되도록 세포를 배양용기에 접종하여 4 ~ 7일간 수평으로 눕혀 배양하고 나머지 8 ~ 11일은 수직으로 세워 37℃, 5% CO2 조건 하에서 배양하였다. 배양배지는 2 ~ 3일 간격으로 교체하였고, 150 U/㎖ ~ 500 U/㎖ IL-2, 5 ~ 10% 채혈자의 혈장(plasma) 또는 5 ~ 10% human serum AB을 함유한 CellGro SCGM serum-free medium(CellGenix)를 사용하였다.
[실시예 3]
제조된 사이토카인 유도 살해세포의 세포 표현형 분석
배양 전후의 세포를 1000 ~ 1800 rpm의 속도로 5분 ~ 10분간 원심분리하여 수거하였다. 수거된 세포를 1 ㎖의 인산염완충용액으로 부유하고 Tryphan-blue로 염색하여 세포수를 측정하였다. FACS 튜브 (flacon)에 1 x 106/tube가 되도록 분주하고 3 ㎖의 인산염완충용액을 추가로 넣었다. 400 g로 5분간 원심분리한 후에 세포를 제외한 상등액을 진공흡입하여 제거하였다. 남겨진 세포를 인산염완충용액을 100 ㎕로 부유한 후에 아래와 같이 항체를 넣고 혼합하였다.

상기 튜브들을 4℃ 에서 30분간 염색한 후에 염색이 끝난 세포에 1차 세포 세척을 위해 3 ㎖의 인산염완충용액을 넣고 혼합하여 400 g에서 5분간 원심분리하였다. 상등액을 제거하고 2차 세포 세척을 위해 3 ㎖의 인산염완충용액을 넣고 혼합하여 400 g에서 5분간 원심분리하였다. 마지막으로 상등액을 제거한 후에 2% paraformaldehyde가 포함된 용액 1 ㎖을 넣고 세포를 고정화하였다. FACSCalibur(Becton Dickinson)을 이용하여 세포 표현형을 분석하였다.
[실시예 4]
제조된 사이토카인 유도
살해세포의
항암효능 평가항목, 평가방법 및 폐암 동물모델의 준비
동종 이식 모델 제작 및 시험물질 투여
7주령 Balb/c nude mice 모델의 측복부 피하 부위에 NCI-H460 폐암 세포주를 마리당 1 x 106/100 ㎕씩 투여하여 동종 이식 모델을 제작한 후에 시험물질인 사이토카인 유도 살해세포(CIK cell)를 3 x 106/200 ㎕ 1주일 간격으로 총 3회(0일, 7일, 14일) 꼬리 정맥에 투여하였다. 대조군으로 인산염완충용액 200㎕을 투여하였고, 양성대조군으로는 Adriamycin 2㎎/㎏을 투여하였다.
종양 성장에 미치는 영향을 관찰하기 위해서 기간별로 일반 증상 관찰 및 체중 측정, 식이량 측정, 부검시 종양무게 측정과 육안적 검사, 종양 이미지 촬영을 실시하여 항암 효능을 평가하였다. 흉선이 결여된 누드 마우스들은 (nude mice) 자가면역으로 인한 효능 간섭요인을 피할 수 있기 때문에 동종이식 모델(xenograft model)로 적합하기 때문에 선택하였다.
동종 이식 모델 경과 관찰 및 평가항목
체중 측정 및 전체 체중 증가량
시험동물의 체중은 시험물질 투여 전 및 투여 개시일부터 부검당일까지 일 11회(0일, 1일, 3일, 5일, 7일, 9일, 12일, 14일, 16일, 19일, 21일) 측정하였다.
종양 크기 및 적출 중량 측정
폐암 동종 이식 모델을 제작 후에 7일째 되는 날 종양 크기를 1차로 측정하였으며 이후 2~3일 간격으로 총 7회 종양 크기를 측정하였다. 측정도구는 버니어캘리퍼스를 사용하였으며 21일 동안 지속적으로 관찰한 후, 부검하여 종양조직을 적출하고 무게를 측정하였다. 종양무게는 ㎎으로 표기하였고, 종양부피(㎣)는 4/3 x 3.14(π) x (W/2) x (L/2) x (H/2) (W: 가로, 장경, L: 세로, 단경, H: 높이)로 측정하였다.
조직학적 병리분석(Immunohistochemistry stain, CD56)
시험종료일에 맞춰 부검을 진행하였고 적출된 종양을 즉시 10% formalin에 보관하였다. 이후 고정된 조직을 paraffin block으로 만들고 슬라이드에 4 ㎛ 두께로 잘라 슬라이드조직을 제작하였다. 조직절편은 deparaffinize, rehydrate, washing 과정을 2회씩 수행하였다. 비특이적 결합을 없애기 위해 hydrogen peroxide Block에서 10분간 정치하고, 인산염완충용액으로 4회 세척하였다. 1차 항체 CD56(1:50, Thermo, Fremont, CA)를 제조사가 명시한 프로토콜에 따라 처리한 후, 인산염완충용액으로 4회 세척하였다. 1차 항체 Enhancer을 넣고 20분간 상온에서 반응시킨 후에 다시 인산염완충용액으로 4회 세척하였다. 그 후에 HRP polymer를 처리하고 30분간 상온에서 반응시키고 인산염완충용액으로 4회 세척하였다. 마지막으로 Hematoxylin을 처리하고 바로 증류수로 4회 세척한 다음 대비 염색(counterstain)을 진행하였다.
자료의 통계처리
모든 시험결과는 평균치와 표준편차를 사용하여 나타내었으며, 각 군간의 비교는 ANOVA(software StatView: version 4.51, Abacus Concepts, Berkeley, CA)를 사용하였고 사후검정은 Fisher's PLSD로 수행하였다. 음성대조군과 양성대조군 등 군간을 비교하여 5%의 유의수준에서 유의성을 검증하였다.
[실시예 5]
제조된 사이토카인 유도 세포의 증식 결과 및 항암효능의 평가 결과
사이토카인 유도 살해세포의 증식 결과
상기 실시예에 따라 제조된 사이토카인 유도 살해세포의 증식 결과를 알아보기 위하여, 1차 증식에서 5일간 OKT-3(anti-CD3)을 이용하여 CD3 양성세포를 선별을 한 후에 2차 증식에서는 수평 배양법(lay-down)과 수직 배양법(up-right)을 각각 이용하였다. 각 배양법에 따른 세포 계수는 배양일로부터 5일째, 7일째, 10일째, 13일째, 15일째, 17일째, 20일째 되는 날 각 세포를 수거하여 측정하였다.
측정 결과, 사이토카인 유도 살해세포는 평균 491.8배(±78.21)의 전체 세포 증식배수를 보여주었다 (도 1 참조).
사이토카인 유도 살해세포의 세포 표현형 분석 결과
상기 실시예에 따라 제조된 사이토카인 유도 살해세포가 배양배지에서 첨가한 항CD3 항체에 의해 CD3양성을 띄는 지 여부를 알아보기 위하여, 2차 증식의 기간별 배양방법에 따라 사이토카인 유도 살해세포는 자연살해세포(Natural Killer cell, NK cell) 및 자연살해T세포(Natural Killer T cell, NKT cell)를 동시에 증식시킴을 확인하였다.
확인 결과, CD3 음성 CD56 양성(CD3-CD56+)의 특징을 가진 자연살해세포의 발현율은 초기 15%에서 53%로 증가하여 평균 1731배 증가하였고, CD3양성 CD56 양성(CD3+CD56+)의 특징을 가지는 자연살해T세포 발현율은 3%에서 25%로 증가하여 평균 4213배 증가하였다 (도 2 참조).
동종 이식 모델에서의 항암효능 평가 결과
상기 실시예에 따라 나뉜 투여군 3군 모두에서 동종 이식 모델 제작 이후와 시험물질 투여 전후에 별다른 이상증상은 보이지 않았으며, 대조군 (인산염완충용액 투여)과 비교하여 사이토카인 유도 살해세포 투여군과 양성대조군 (Adriamycin 투여) 모두에서 체중 변화 및 독성은 관찰되지 않았다.
1차 시료투여 이후 2차 투여일(7일째)에 대조군, CIK cell(사이토카인 유도 살해세포) 투여군 및 Adriamycin 투여군(양성대조군)의 평균 종양 크기는 각각 33.63 ㎣, 19.76㎣, 8.61 ㎣로 측정되었다. 9일째에는 각각 139.55 ㎣, 41.89 ㎣, 65.47 ㎣로 나타났으며, 3차 투여(14일째)일에는 각각 619.7 1㎣, 211.34 ㎣, 217.95 ㎣로 측정되었다. 21일째에 마지막 부검이 진행되었으며, 부검 전 버니어캘리퍼스로 측정한 최종 종양 크기는 각각 1122.46 ㎣, 693.05 ㎣, 413.7 ㎣로 나타났다(도 4 참조).
종양 무게는 부검일정에 맞춰 종양을 적출하여 측량하였으며, 적출된 종양을 사진 촬영하였다(도 5 참조). 대조군, 사이토카인 유도 살해세포 투여군 및 양성대조군의 평균 종양 무게는 각각 1328.2±242.87 ㎎, 495.2±427.86 ㎎, 750.7±83.47 ㎎로 측정되었다 (도 6 참조). 즉, 종양 무게는 대조군과 비교하여 사이토카인 유도 살해세포 투여군은 평균 43.48% 감소되었고, 양성대조군은 62.72% 감소되었다.
부검 결과 각각의 투여군은 대조군과 비교하여 육안적 이상소견은 관찰되지 않았다. 이에 시험물질에 의한 독성여부는 개체에 눈에 보이는 정도로 영향을 미치지 않는다고 판단되며, 종양의 전이여부를 판단하기 위해 내부 횡경막 위쪽 폐장기와 장간막 쪽 췌장부위를 관찰하였고 부검소견상 내부 장기로의 전이는 없다고 판단되었다.
대조군에 비해 사이토카인 유도 살해세포 투여군의 폐 종양 세포의 괴사조직 전반에 CD56 항체 발현(화살표 표시)이 나타났으며, 이는 강한 면역반응을 나타내는 것이다 (도 7 참조).
상기 실험 결과를 통하여 본 발명의 일 측면에 따른 사이토카인 유도 살해세포 생산방법으로 생산된 사이토카인 유도 살해 세포는 증식이 기존 방식에 비하여 양적으로 떨어지지 않으며, 배양시에 첨가한 항체에 대하여 양성을 보이는 세포가 월등히 많이 증식되며, 폐암 동물모델의 결과에서 알 수 있듯이 종양의 크기를 감소시키는 항암효과를 보이는 것을 알 수 있다.
결과적으로, 본 발명의 일 측면에 따른 사이토카인 유도 살해세포 생산방법은 기존의 생산방식으로 생산되는 사이토카인 유도 살해세포보다 우수한 항암효과를 지니면서 개선된 생산방법을 제공함을 알 수 있으며, 이를 이용하여 효과적인 항암 치료에 사용 가능한 사이토카인 유도 살해세포를 개선된 방법으로 보다 효율적으로 생산할 수 있을 것으로 판단된다.
Claims (9)
- (a) 말초혈액을 항응고제가 포함된 진공 채혈관을 이용하여 채혈하고, 인산염완충용액으로 희석한 뒤 밀도구배 원심 분리법을 통해 말초혈액유래 단핵세포를 분리한 뒤 세척하는 단계;(b) 말초혈액유래 단핵세포를 항체 및 혈장이 포함된 배양배지에 넣어 배양하는단계;(c) 배양된 말초혈액유래 단핵세포를 부유하여 옮긴 후에 원심분리한 뒤 세포를 제외한 상등액을 제거하고 다시 배양용기에 접종하는 단계;(d) 접종된 말초혈액유래 단핵세포를 수평으로 눕혀 배양하는 단계;(e) 수평배양된 말초혈액유래 단핵세포를 수직으로 세워 배양하는 단계;를 포함하는, 사이토카인 유도 살해세포의 생산 방법.
