第1実施形態に係る経頭蓋磁気刺激システムは、対象者の疼痛緩和を目的とした磁気刺激治療に適した構成を有しており、コイル部の位置決めを精密に行い、それを再現できる機構部を備えている。
具体的には、操作者は、まず、対象者90を椅子31に座らせる。このとき、ヘッドギア33が体格調整ガイド324に接続されている場合には、着脱機構336を操作して、ヘッドギア33を上側に上げる。次に、操作者は、高さ調整機構325により、体格調整ガイド324の高さを対象者90の体格に合わせて調整する。また、操作者は、前後方向調整機構322及び上下方向調整機構323により、後頭部レスト321の位置を対象者90の後頭部に合うよう前後上下方向に調整する。
具体的には、操作者は、着脱機構336を操作して、ヘッドギアガイド335を下側に降ろし、体格調整ガイド321に接続する。
具体的には、操作者は、ヘッドギア33の各調整機構を調整してコイル部11の位置を仮設定する。仮設定する位置は、対象者90の頭部の医用画像を参照し疼痛部位に応じて指定された刺激部位に対応した位置である。
なお、このコイル位置の仮設定において、上述の通り、各調整機構の調整がうまくいかず、更には効率の良い調整手順も分かりづらいという状況が発生し得る。この場合の対応については、後述する。
具体的には、操作者は、受付部501(ハードウエア上での操作部51)での入力操作を行い、磁気刺激強度を比較的強く設定する。そして、ヘッドギア33の各調整機構を調整してコイル部11の位置を仮設定した位置の周辺で少しずつ変えながら、磁気刺激発生指令を受付部501においてその都度入力する。コイル駆動制御部503は、その磁気刺激発生指令に逐次応答して磁気刺激を発生させる。操作者は、磁気刺激を発生させるごとに対象者90のトゥイッチ反応を見る。そして、最も強いトゥイッチ反応が現れた位置を、最適刺激位置に決定する。
具体的には、操作者は、受付部501(操作部51)での入力操作を行い、磁気刺激強度を治療に最適な強度に戻す。そして、受付部501において治療開始指令を入力する。コイル駆動制御部503は、コイル駆動部12を制御し、設定された磁気刺激条件に従って治療用の磁気刺激を開始する。
図11に示すフロー図は、「頭頂部側」の片当り状態を解消するための調整手順を示したものである。なお、図11のフロー図において、「相当量」とは、操作者が適当と考える量を意味する。「相当量」に代えて調整機構における可動部材の具体的な移動量を、目盛りの数値を用いて示してもよい。
この調整手順は以下のとおりである。
第2実施形態に係る経頭蓋磁気刺激システムは、位置決め機構部の各調整機構における可動部材の位置情報を信号に変換して制御装置に送り、当該位置情報を用いて位置決め機構部の調整手順を案内するよう構成されている。
具体的には、操作者が、複数の選択肢の中から現在の片当り状態を選択する。
具体的には、受付部501が、選択された片当り状態の入力を受け付ける。
具体的には、案内部502が、選択された片当り状態を解消するための調整手順を表す調整手順情報を、調整手順記憶部502Aから読み出す。
具体的には、案内部502が、読み出した調整手順情報に基づいて、調整手順を示すフロー図を表示する。例えば、どの調整機構をどの順番でどの向きに調整するのかを、調整機構ごとに各調整ステップとして分けて表示する。また、案内部502は、位置決め機構部30の各調整機構をモデル化したイメージ図を表示する。
具体的には、案内部502が、図18に示すように、調整手順を示すフロー図の中で、現時点の調整対象である調整機構の調整ステップを強調して表示する。なお、図18のフロー図において、目標位置及び現在位置の数値は、調整機構における可動部材の位置に対応した目盛りを示している。「相当量」とは、操作者が適当と考える量という意味である。
具体的には、操作者91は、現時点の調整対象である調整機構において、インジケータを見ながら現在位置が目標位置に一致するよう調整を開始する。案内部502は、インジケータ上の現在位置を示すマークの表示をほぼリアルタイムで更新する。
具体的には、案内部502は、調整機構の現在位置が目標位置に一致するか否かを判定する。この一致は、完全一致だけでなく、所定の許容範囲内で一致する場合も含む。一致した場合には、現時点の調整対象である調整機構での調整は完了したと認識し、次のステップに進む。一致判定できない場合は、次の手順でこのステップT207を終了させることができる。すなわち、調整機構の調整において、目標位置が明確に導出できないもの、例えば、対象者の体格、頭部の位置や形状に合わせて違和感のないように調整するものなどの場合には、操作者が、調整完了後、操作部51において「調整完了」の入力を行う。案内部502は、この「調整完了」の入力によって、調整完了を認識する。調整完了が認識されたら、次のステップに進む。
具体的には、案内部502が、次の順番の調整すべき調整機構があるか否かを判定する。あれば、次の順番の調整機構を現時点での調整対象となる調整機構に設定してステップT205に戻り、なければ案内を終了する。

第3実施形態に係る経頭蓋磁気刺激システムは、第2実施形態と同様の構成であるが、制御装置が有する機能ブロックの機能が異なる。
具体的には、操作者が、コイル部の目標の位置及び姿勢を入力する。

前記受付手段は、前記検出された接触具合の入力を受け付ける、請求項2に記載の経頭蓋磁気刺激システム。 - 前記検出手段は、前記接触具合を定量的に検出し、
前記案内手段は、該定量的に検出された接触具合に基づいて、前記調整機構の調整量または目標位置を求める、請求項21に記載の経頭蓋磁気刺激システム。 - 前記接触具合は、前記コイル部の外縁部のうち一部のみが前記頭部に接触する片当り状態と、前記コイル部の前記頭部に対する密着状態との少なくとも一つを含む、請求項21または請求項22に記載の経頭蓋磁気刺激システム。
- 前記センサ部は、撮像センサ、超音波センサ、赤外線センサ、圧力センサ、機械スイッチセンサ、静電センサ、及び加速度センサのうち少なくとも一つを含む、請求項21から請求項24のいずれか一項に記載の経頭蓋磁気刺激システム。
