Abstract

Description

종양 제거를 위한 스네어
본 발명은 종양 제거를 위한 스네어(투명캡) 에 관한 것이다. 구체적으로 본 발명은 용종절제 시술 시 특수금속으로 된 올가미를 내시경에 있는 가느다란 구멍 (working channel)을 통해 집어넣은 후, 올가미로 잡고 전기를 통과시켜서 용종을 잘라낼 때 사용하는 의료용 2 챔버 스네어(투명캡) 에 대한 것이다.
일반적으로 대장내시경 용종절제술은 집게(Punch forcep)나 특수금속으로 된 올가미(Snare)를 내시경에 있는 가느다란 구멍을 통해 집어 넣은 후, 용종을 잘라내는 시술이다.
이러한 대장내시경 용종절제 시술 시 작은 용종은 집게로 잘라내지만 큰 용종은 올가미로 잡고 전기를 통과시키거나 물리적으로 절단하여 제거 한다.
그리고, 용종이 크고 넓적하게 대장점막에 붙어 있는 경우에는 대장 벽의 전기 손상을 줄이기 위해서 주사침으로 점막 밑에 식염수를 주입한 후 올가미로 절제(EMR, ESD endoscopic mucosal resection, endoscopic submucosal dissection) 하기도 한다.
보통 용종절제 시술 시 사용하는 스네어는 인장 로프로 이루어지고, 이때의 인장 로프는 전방 단부에서 반원과 유사한 형태의 올가미 속으로 들어가서 끝나며, 올가미는 피크(Peak)를 갖는다.
상기 인장 로프는 부싱 그리고 부싱 단부에 배치된 파이프 모양의 대응 스토퍼 부재 안에서 이동이 용이하게 가이드되고, 두 개의 올가미 레그의 한 단부가 인장 로프에 고정 연결됨으로써, 결과적으로 올가미는 인장 로프에의하여 신장된 상태로 부싱 내부에 놓이게 되는 저장 위치로부터 올가미가 부싱의 단부 앞에 놓이게 되는 사용위치로 옮겨질 수 있게 된다.
그리고, 부싱 내부에 있는 다른 올가미 레그의 단부는 올가미를 부싱으로부터 밖으로 빼내는 과정 중에 대응 스토퍼를 향하여 이동하는 스토퍼에 연결되어 있으며, 그 결과 다른 올가미 레그가 계속해서 이동하는 경우에는 올가미가 반원 모양으로 확장된다.
다른 예로서, 전술한 유형의 올가미에 전류를 공급할 수 있는 고주파 절제 장치가 적용될 수도 있다.
이 경우 올가미는 전극으로서 작용하게 되며, 복강 내부, 특히 장 내부에 있는 하나 또는 다수의 아데노이드(adenoids)와 같은 용종을 고주파 전류를 이용하여 절제하기 위해서 이용된다.
따라서, 절제 시에 내시경 시술 의사는 통상적으로 내시경을 이용하여 복강 내부에 삽입되는 올가미로 용종을 잡은 다음에 잘라내게 된다.
Stalk 을 가진 유경성 용종 (Ip polyp)을 제거시에는 stalk 의 굵기에 따라 5mm 이하는 통전하여 응고시켜 제거하지만 굵기가 8mm 이상 용종의 경우에는 단순한 통전을 통한 절제시에 feeding vessel (stalk 내의 동맥)이 충분히 응고되지 않아 급성, 지연 출혈을 야기 할 수 있다.
또한, 출혈이 생기는 경우에 절단 후 남아 있는 stalk이 계속 움직이기 때문에 지혈이 어려운 경우가 많다.
이러한 합병증을 예방하기 위해서 올림프스(일본국) 사의 endo-loop 가 개발되었으나 초심자에게는 시술이 어렵고 stalk 이 가는 경우에는 기계적으로 절단되어 출혈이 생기는 경우도 있다. 따라서 stalk 의 굵기에 관계없이 하여 유경성 용종을 제거하는 방법이 필요하다.
또한, 용종 폴립의 자루(stalk)는 꼬인 형태로 존재하므로, 종래의 용종절제 시술용 스네어는 폴립의 헤드와 가까운 폴립의 자루(stalk)를 흡입하여 잘라내기가 어렵다는 문제점이 존재한다.
또한, 기존에 이용되는 올가미(링)은 스네어의 외경에 구비되었다가 배출됨으로써, 폴립을 흡입한 시간과 올가미(링)로 고정시키는 시간 사이의 간격이 있어 사용자가 원하는 폴립의 부분을 올라미(링)를 통해 고정하기 어렵다는 문제점도 존재한다.
따라서 전술한 문제점을 해소하기 위한 방안이 요구되고 있는 실정이다.
본 발명의 목적은 용종절제 시술 시 특수금속으로 된 올가미를 내시경에 있는 가느다란 구멍을 통해 집어넣은 후, 올가미로 잡고 전기를 통과시켜서 용종을 잘라낼 때 사용하는 의료용 2 챔버 스네어를 제안하는 것이다.
또한, 본 발명의 목적은 2 챔버 스네어 중 제 1 챔버를 통해 느슨하게 구부러져 있는 stalk을 펼치고, 제 2 챔버를 통해 올가미(링)로 용종을 고정 시켜 용종의 헤드를 확실하게 제거하여 수술을 용이하게 하는 것이다.
또한, 본 발명의 목적은 기존의 올가미(링)와 달리 제 2 챔버 스네어 중 제 2 챔버에만 올가미(링)이 구비되도록 하고, 고정기구를 통해 용종을 제 2 챔버 내부로 끌어들여 바로 올가미(링)를 통해 고정시킴으로써, 올가미의 초기 진입 방향을 안정적으로 유지시킬 수 있고, 구조적인 측면에서 유리할 뿐만 아니라 수술시간을 절약할 수 있으며, 시술을 간편하게 하는 것이다.
한편, 본 발명에서 이루고자 하는 기술적 과제들은 이상에서 언급한 기술적 과제들로 제한되지 않으며, 언급하지 않은 또 다른 기술적 과제들은 아래의 기재로부터 본 발명이 속하는 기술분야에서 통상의 지식을 가진 자에게 명확하게 이해될 수 있을 것이다.
상기의 기술적 과제를 달성하기 위한 본 발명의 일 양상인 자루(stalk)와 상기 자루에 연결된 헤드(head)를 포함하는 용종을 제거하기 위한 투명캡 (스네어)에 있어서, 중공의 관 형태로 이루어지고, 내부가 복수의 챔버(chamber)로 구획된 몸체; 탄력을 이용하여 상기 몸체의 적어도 일부에 구비된 링; 상기 복수의 챔버 중 제 1 챔버를 통해 외부로 배출되어 상기 자루에 걸쳐지는 제 1 스네어 및 갈고리; 및 상기 복수의 챔버 중 제 2 챔버를 통해 상기 외부로 배출되는 제 2 스네어;를 포함하되, 상기 자루에 걸쳐진 제 1 스네어가 당겨짐에 따라 상기 자루가 펼쳐지는 경우, 상기 제 2 스네어가 상기 펼쳐진 자루의 전체 영역 중 상기 헤드 하단의 제 1 영역을 당기고, 상기 몸체로부터 배출된 링은 상기 탄력을 통해 축소되어 상기 당겨진 제 1 영역을 괄약 (strangulate)할 수 있다.

또한, 본 발명은 2 챔버 스네어 중 제 1 챔버를 통해 구부러져 있는 용종을 펼치고, 제 2 챔버를 통해 올가미(링)로 용종의 stalk 부위만을 포획한후 고정 시켜 용종의 헤드를 확실하게 제거하여 용종 절제술을 용이하게 할 수 있다.
또한, 본 발명은 기존의 올가미(링)와 달리 제 2 챔버 스네어 중 제 2 챔버에만 올가미(링)이 구비되도록 하고, 고정기구를 통해 용종을 제 2 챔버 내부로 끌어들여 바로 올가미(링)를 통해 고정시킴으로써, 올가미의 초기 진입 방향을 안정적으로 유지시킬 수 있고, 구조적인 측면에서 유리할 뿐만 아니라 수술시간을 절약할 수 있으며, 시술을 간편하게 할 수 있다.
한편, 본 발명에서 얻을 수 있는 효과는 이상에서 언급한 효과들로 제한되지 않으며, 언급하지 않은 또 다른 효과들은 아래의 기재로부터 본 발명이 속하는 기술분야에서 통상의 지식을 가진 자에게 명확하게 이해될 수 있을 것이다.

그리고, 부싱 내부에 있는 다른 올가미 레그의 단부는 올가미를 부싱으로부터 밖으로 빼내는 과정 중에 대응 스토퍼를 향하여 이동하는 스토퍼에 연결되어 있으며, 그 결과 다른 올가미 레그가 계속해서 이동하는 경우에는 올가미가 반원 모양으로 확장된다.
그러나 종래의 용종절제 시술용 스네어는 제거를 할 용종이 발생된 부위로 내시경과 함께 올가미를 삽입한 다음, 용종이 발생된 방향으로 올가미를 회전시키기 위해서는 장치 전체를 회전시켜 올가미 내부로 용종의 원활히 삽입되도록 하고 하는데, 이 경우 장치 전체를 회전시킴으로써 시술자가 원활하게 원하는 방향으로 올가미를 회전시킬 수 없을 뿐만 아니라 손목에 부담이 증가하는 문제점이 있다.
이러한 문제점을 해결하기 위해, 도 3a 및 도 3b에서 설명하는 의료용 스네어가 적용될 수도 있다.
즉, 도 3a 및 도 3b는 본 발명과 관련된 의료용 스네어의 일례를 도시한 것이고, 도 4는 본 발명과 관련된 의료용 스네어의 사용상태의 일례를 설명하기 위한 도면이다.
도 3a 및 도 3b를 참조하면, 의료용 스네어는 스네어 전체를 회전시키지 않고 간단한 노브 조작을 통해 와이어만 회전시켜서 올가미의 전개 방향을 조절할 수 있고, 특히 와이어측과 도전 블럭측 간의 회전가능한 구조를 이용하여 노브 조작 시 와이어를 포함하는 와이어 지지관 전체가 도전 블럭측 결합부위를 지지점으로 하여 회전될 수 있도록 하는 구조로 이루어진다.
이를 위하여, 상기 의료용 스네어는 스네어 본체에 해당하는 몸체(11)를 포함하며, 상기 몸체(11)는 중공의 관 형태, 예를 들면 중심축선을 따라 나란하게 형성되는 홀 구조를 가짐과 더불어 직경방향으로 관통 절개된 형상의 슬라이드 홀(27)을 가지는 원형관 형태로 이루어지게 된다.
이때의 슬라이드 홀(27)은 몸체(11)의 길이방향을 따라가면서 일정구간에 걸쳐 형성되며, 이곳에 후술하는 슬라이더(13)에 있는 도전 블럭(18)이 위치되어 슬라이드 안내를 받을 수 있게 된다.
이러한 몸체(11)의 상단부에는 링 모양으로 되어 있는 제1손잡이(10)가 나란하게 장착되며, 이때의 제1손잡이(10)는 스네어 파지 시 엄지 손가락 등을 걸 수 있는 용도로 사용될 수 있게 된다.
또한, 상기 의료용 스네어는 와이어를 밀어주거나 당겨주는 수단으로 슬라이더(13)를 포함한다.
상기 슬라이더(13)는 몸체(11)의 외주면에 동심원으로 끼워지면서 몸체(11)의 길이방향을 따라 이동가능하게 결합된다.
이러한 슬라이더(13)의 양쪽 측면에는 링 모양으로 되어 있는 한쌍의 제2손잡이(12)가 형성되며, 이때의 제2손잡이(12)는 스네어 파지 시 검지 손가락과 중지 손가락 등을 동시에 걸 수 있는 용도로 사용될 수 있게 된다.
또한, 상기 의료용 스네어는 용종절제 시술 시 실질적으로 용종을 잘라내는 수단으로 와이어(15) 및 와이어 지지관(16)을 포함한다.
상기 와이어(15)와 와이어 지지관(16)은 직선 상으로 연결되는 형태로서, 와이어 지지관(16)의 하단부에 와이어(15)의 상단부가 삽입된 상태에서 함께 압착되는 구조에 의해 서로 일체식으로 연결된다.
이러한 와이어(15)의 하단부에는 올가미(14)가 형성되어 있어서 이것으로 용종을 걸어서 잘라낼 수 있게 된다.
그리고, 상기 와이어 지지관(16)은 몸체(11)의 하단부 홀 구조를 통해 삽입된 후, 그 상단부를 통해 슬라이더(13)에 있는 도전 블럭(18)측에 연결된다.
이에 따라, 상기 슬라이더(13)를 이동시키게 되면 와이어 지지관(16) 및 와이어(15)도 함께 따라서 움직일 수 있게 되고, 결국 이러한 움직임을 통해 와이어(15)의 올가미(14)가 용종을 잘라낼 수 있게 된다.
이때의 와이어 지지관(16)의 일부 길이는 원형관을 납작하게 만든 형태의 평판부(28)로 이루어져 있으며, 이러한 평판부(28)를 통해 후술하는 노브(17)측과 결속될 수 있게 된다.
이에 따라, 상기 노브 조작 시 와이어 지지관(16) 및 와이어(15) 전체가 노브 조작방향으로 함께 회전될 수 있게 된다.
또한, 상기 의료용 스네어는 와이어 회전을 위한 수단으로 노브(17)를 포함한다.
상기 노브(17)는 원형의 다이얼 형태로서, 노브 하우징(29) 내에 회전가능한 구조로 결합됨과 더불어 하우징 밖으로 일부 노출될 수 있게 되며, 이때의 노브 하우징(29)은 몸체(11)의 하단부에 삽입 구조로 장착된다.
그리고, 상기 노브(17)의 축선으로는 와이어 지지관(16)의 평판부(28)가 끼워지면서 결속될 수 있게 되고, 이에 따라 노브(17)를 돌리게 되면 와이어 지지관(16) 및 이에 일체식으로 결합되어 있는 와이어(15) 전체가 회전될 수 있게 된다.
이러한 노브(17)와 와이어 지지관(16) 간의 견고한 결속구조를 확보하기 위하여 노브(17)의 내부는 탄성체(30)로 이루어지게 되며, 이때의 탄성체(30) 내에 와이어 지지관(16)의 평판부(28)가 압착구조로 결속되므로서, 노브(17)와 와이어 지지관(16)이 긴밀하게 결속될 수 있게 된다.
여기서, 상기 노브 하우징(29)의 하단부에는 내측으로 와이어(15)를 수용함과 더불어 와이어(15)의 진입을 유도하는 튜브(32)가 장착될 수 있게 된다.
또한, 상기 의료용 스네어는 와이어측에 전류를 공급하는 수단으로 도전 블럭(18)과 전극 핀(19)을 포함한다.
상기 도전 블럭(18)은 사각의 블럭 형태로서, 슬라이더(13)의 하단부 내측에 삽입되는 구조로 장착된다.
이러한 도전 블럭(18)의 아래쪽으로는 대략 "T"자 형상으로 된 홈부(20)가 형성되어 있으며, 이때의 홈부(20)에 와이어 지지관(16)의 상단측이 연결될 수 있게 된다.
그리고, 상기 전극 핀(19)은 슬라이더(13)의 측면에 형성되어 있는 핀 삽입홀(31) 내에 위치되면서 선단부분의 나사부를 통해 도전 블럭(18)의 측면부에 체결되는 구조로 장착된다.
이러한 전극 핀(19)은 외부의 전원측과 도선(미도시)으로 연결되어 전류를 제공받게 되고, 이렇게 제공받은 전류를 도전 블럭(18)측에 인가하는 역할을 하게 되며, 결국 도전 블럭(18)측에 연결되는 와이어 지지관(16) 및 계속해서 이어지는 와이어(15)까지 통전 가능한 상태가 될 수 있게 된다.
특히, 상기 와이어 지지관(16)은 도전 블럭(18)측에 회전가능한 구조로 결합된다.
이를 위하여, 상기 와이어 지지관(16)의 상단부에는 대략 "T"자 형상의 단면을 가지는 원형의 부시(21)가 일체 결합되고, 이렇게 결합되는 부시(21)가 도전 블럭(18)에 있는 홈부(20)에 형합되는 형태로 끼워져 결합됨으로써, 와이어 지지관(16)은 부시(21)를 매개로 한 도전 블럭(18)측의 결합부위를 지지점으로 하여 자체 축선을 중심으로 회전될 수 있게 되고, 결국 와이어 전체가 회전될 수 있게 된다.
그리고, 상기 와이어 지지관(16)의 부시(21)는 대략 "T"자 형상의 걸림구조를 통해 도전 블럭(18)의 홈부(20) 내에서 하 방향으로 이탈되지 않게 된다.
또한, 상기 의료용 스네어는 와이어(15) 및 와이어 지지관(16) 전체의 이동을 규제하는 수단으로 스톱퍼(26)를 포함한다.
상기 스톱퍼(26)는 한쪽이 터져 있는 링 형태로서, 몸체(11)의 외주면에 동축으로 장착되어 몸체(11)의 길이방향을 따라 직선이동이 가능하게 된다.
여기서, 상기 스톱퍼(26)는 적어도 반원 형상보다는 좀더 큰 호 길이를 갖도록 하여 몸체 반경방향으로 빠지지 않도록 하는 것이 바람직하다.
그리고, 상기 스톱퍼(26)의 내주면에는 접촉돌부(25)가 형성되며, 이때의 접촉돌부(25)는 몸체(11)의 슬라이드홀(27) 내에 위치되면서 그 선단면을 통해 와이어 지지관(16)과 접촉할 수 있게 된다.
이에 따라, 상기 와이어 지지관(16)의 이동 시 또는 회전 시 스톱퍼(26)의 접촉돌기(25)에 의한 접촉규제를 받으면서 회전되지 않고 자세(방향)를 유지할 수 있게 되고, 결국 올가미의 초기 진입 방향 또는 용종 제거 후 복귀 방향이 안정적으로 유지될 수 있게 된다. 특히, 상기 접촉돌부(25)와 접촉되는 와이어 지지관(16)은 그 외경이 직사각 형태의 평판부(28)로 되어 있어 회전시 90도 각도에 따라 회전지지력을 제공받을 수 있게 된다.도 4를 참조하면, 스네어를 수술에 이용하기 위해서는 슬라이드(13)를 후방으로 최대한 당겨 올가미(14)가 튜브(32)의 내부로 가압되며 삽입되도록 한다.
이러한 상태에서 상기 튜브(32)를 항문을 통해 내시경(미도시)과 함께 대장으로 삽입한다.
이후, 상기 내시경을 통해 용종(100)의 위치가 확인되면 몸체(11)의 제1손잡이(10)에 엄지손가락을 삽입하고, 슬라이더(13)의 제2손잡이(12)에 검지손락락과 중지손가락을 삽입하여 스네어를 한 손으로 파지한다.
다음, 상기 몸체(11)와 슬라이더(13)가 이격되도록 스네어를 파지한 손을 넓게 펼쳐 튜브(32)로부터 올가미(14)가 확장되면서 노출되도록 한다.
이렇게, 상기 올가미(14)가 튜브(32)에서 노출되면 올가미(14)의 전개 공간 내부로 용종(100)이 위치되도록 한 상태에서 몸체(11)와 슬라이더(13)가 밀착되도록 스네어를 파지한 펼쳐진 손을 접는다.
이에 따라, 상기 올가미(14)가 튜브(32)의 내부로 가압되면서 삽입되어 용종(100)을 가압 절단하여 제거하게 된다.
물론, 제거할 용종이 큰 경우에는 전류를 인가한 상태에서 용종을 잘라낼 수 있다.
한편, 상기 용종(100)이 발생된 위치가 올가미(14)의 전개 공간과 일치하지 않을 경우에는 스네어를 한 손으로 파지한 상태에서 다른 한 손을 이용하여 노브(17)를 좌,우 방향으로 회전시키면, 와이어 지지관(16) 및 와이어(15), 그리고 올가미(14)가 함께 회전된다.
이로써, 다양한 방향으로 발생된 용종(100)을 노브(17)의 간단한 회전으로 와이어(15)의 올가미 전개 공간 방향을 쉽게 용종(100)이 삽입되기 좋은 방향으로 회전시킬 수 있고, 결국 용종(100)을 쉽게 올가미(14)의 전개 공간 내로 삽입시킬 수 있는 것이다.
그러나 전술한 기구 및 방법에 의하더라도, 종래의 용종절제 시술용 스네어는 용종 폴립의 자루(stalk)는 느슨하게 꼬인 형태로 존재하므로, 폴립의 헤드와 자루(stalk) 사이를 어느 정도 흡입하여 잘라야 하는가에 대한 조절이 어렵고 이로 인해 시술이 어렵다는 문제점이 존재한다.
또한, 기존에 이용되는 올가미(링)은 스네어의 외경에 구비되었다가 배출됨으로써, 폴립을 흡입한 시간과 올가미(링)로 고정시키는 시간 사이의 간격이 있어 사용자가 원하는 폴립의 부분을 올라미(링)를 통해 고정하기 어렵다는 문제점도 존재한다.
따라서 본 발명에서는 용종절제 시술 시 특수금속으로 된 올가미를 내시경에 있는 가느다란 구멍 (working channel)을 통해 집어넣은 후, 올가미로 잡고 전기를 통과시켜서 용종을 잘라낼 때 사용하는 의료용 2 챔버 스네어를 제안하고자 한다.
또한, 본 발명은 2 챔버 스네어 중 제 1 챔버를 통해 구부러져 있는 용종을 펼치고, 제 2 챔버를 통해 링으로 용종의 stalk 전체를 포획한후 ring을 배출하여 feeding artery 가 포함된 stalk을 cramping 한후 용종의 헤드를 확실하게 제거하여 수술을 용이하게 하고자 한다.
또한, 본 발명의 목적은 기존의 올가미(링)와 달리 제 2 챔버 스네어 중 제 2 챔버에만 링이 구비되도록 하고, 고정기구를 통해 용종을 제 2 챔버 내부로 끌어들여 바로 링를 통해 고정시킴으로써, 올가미의 초기 진입 방향을 안정적으로 유지시킬 수 있고, 구조적인 측면에서 유리할 뿐만 아니라 수술시간을 절약할 수 있으며, 시술을 간편하게 하는 것이다.
도 5a 내지 도 5c는 본 발명이 제안하는 종양 제거를 위한 스네어의 구조를 설명하기 위한 도면이다.
종양(tumor)은 자율적 과잉증식을 보이는 세포의 집합체로서, 기생체와는 달리 생체를 구성하는 세포 자체에서 생긴다.
도 5a를 참조하면, 다양한 형태의 종양이 도시되어 있다.
도 5a에 도시된 종양 (가) 내지 (라) 중 (나)와 같이, 헤드(head, 210)와 자루(stalk, 220)를 포함하는 종양, 즉 폴립이 있을 수 있다.
대장에 생기는 용종인 대장 용종이 대표적이지만 신체의 다른 기관에서도 발생될 수 있고, 이러한 용종(폴립, polyp, 200)은 헤드 이외에 자루(stalk)를 갖는다.
용종(200)에서 신체에 위험한 부분은 헤드(210)이고, 헤드(210)를 포함하는 부분을 완전하게 절제하여야 추후에 발생하는 질병으로부터 예방이 가능하다.
전술한 스네어 구성을 기초로 자루(stalk)를 갖는 용종을 제거하는 방법으로는 삼발 구조(310)를 갖는 도구를 이용하는 방법이 이용될 수 있다.
도 5b를 참조하면, 삼발 구조(310)를 통해 헤드(210)를 포함하여 자루(stalk, 220)를 흡입하고, 도 5c와 같이 흡입된 용종(200)을 링(320)을 통해 고정시킴으로써, 절제하는 동작을 수행할 수 있다.
그러나 실제의 용종 폴립의 자루(stalk)는 일자형이 아닌 느슨하게 꼬인 형태로 존재하므로, 상기 삼발 구조(310) 등을 이용하는 방법에 의해서도 헤드(210)를 포함하지 않는 자루(stalk, 220)를 흡입하여 고정하거나 헤드(210)의 일부만을 고정하여 제거하는 경우가 발생된다.
즉, 완벽하게 헤드(210)를 제거하지 못함으로써, 완벽한 조직 검사가 이루어지기 어렵고, 추후에 있을 질병을 유발할 수도 있는 문제점이 있다.
따라서 본 발명에서는 용종절제 시술 시 특수금속으로 된 올가미를 내시경에 있는 가느다란 구멍을 통해 집어넣은 후, 올가미로 잡고 전기를 통과시켜서 용종을 잘라낼 때 사용하는 의료용 2 챔버 스네어를 제안하고자 한다.
구체적으로 본 발명은 2 챔버 스네어 중 제 1 챔버를 통해 구부러져 있는 용종을 펼치고, 제 2 챔버를 통해 올가미(링)로 용종을 고정 시켜 용종의 헤드를 확실하게 제거하여 수술을 용이하게 하고자 한다.
도 6a 및 도 6b를 이용하여 본 발명이 제안하는 종양 제거를 위한 투명캡(400)에 대해 구체적으로 설명한다.
도 6a 및 도 6b는 본 발명이 제안하는 종양 제거를 위한 2 챔버 스네어의 구조를 설명하기 위한 도면이다.
도 6a를 참조하면, 본 발명이 제안하는 종양 제거를 위한 스네어(투명캡, 400)는 제 1 챔버(410)와 제 2 챔버(420)으로 구분된 구조를 기본적으로 갖는다.
또한, 스네어(400)의 외경에는 용종을 고정하기 위한 링(320)이 구비된다.
상기 제 1 챔버(410)와 제 2 챔버(420) 각각에는 제 1 튜브(430)과 제 2 튜브(440)가 구비된다.
또한, 제 1 고무 밴드 링 (430)의 말단에는 도 6b에 도시된 (가)의 갈고리 형태의 스네어(450) 또는 삼발형태의 스네어(450)이 구비될 수 있다.
또한, 제 2 튜브(440)의 말단에는 도 6b에 도시된 (가)의 갈고리 형태의 스네어(460) 또는 삼발형태의 스네어(460)이 구비될 수 있다.
단, 제 1 튜브(430)의 스네어(450)과 제 2 튜브(440)의 스네어(460)의 도 6b의 형태는 예시적인 것으로서, 본 발명의 내용이 이에 한정되는 것이 아니고 객체를 고정하기 위한 보다 다양한 형태를 통해 구현될 수 있는 것은 자명하다.
결과적으로 제 2 튜브(440)의 스네어(460)는 대상 용종(200)의 자루(220)에 걸림으로써, 꼬여있는 자루(220)를 펼치는 기능을 제공한다.
또한, 제 1 튜브(430)의 스네어(450)는 펼쳐진 자루(220)를 기초로 헤드(210)부분을 완전히 포함하는 영역의 자루(220)를 링(320)이 고정하게 된다.
이후, 링(320)을 통해 고정된 헤드(210)부분을 완전히 포함하는 영역의 자루(220)를 제거함으로써, 기존의 문제점을 완전히 해소할 수 있게 된다.
본 발명의 구체적인 동작에 대해 전술한 도 6a 및 도 6b의 내용을 기초로 도 7을 이용하여 설명한다.
도 7은 도 6a 및 도 6b에서 설명한 2 챔버 스네어를 이용하여 용종을 절제하는 방법을 설명하기 위한 순서도이다.
도 7을 참조하면, 제 2 캡(420)에 있는 제 2 스네어(460)를 폴립(200)에 거는 단계(S100)가 진행된다.
이후, 폴립(200)에 걸린 스네어(460)를 통해 폴립의 자루(stalk, 220)를 펼치는 단계(S200)가 수행된다.
또한, 제 1 캡(410)에 있는 제 1 스네어(450)를 이용하여 자루(stalk, 200)가 펼쳐진 폴립(200)을 고정하는 단계(S300)가 수행된다.
즉, 삼발 또는 고리 등의 형태로 구현된 제 1 스네어(450)가 링(410)을 출력하기 전에 펼쳐진 폴립(200)에서 헤드(210)를 완전히 포함하는 자루(200) 윗단을 고정하게 된다.
이후, 제 1 스네어(410)를 통해 고정된 폴립(200)을 제 1 캡(410)으로 흡입하는 단계(S400)가 수행된다.
또한, 제 1 캡(410)으로 흡입된 폴립의 자루(stalk, 220) 부분, 즉, 헤드(210)를 완전히 포함하는 하단의 자루(220) 부분을 링으로 고정하는 단계(S500)가 수행된다.
이후, 폴립(200)의 헤드(210) 및 자루(stalk, 220)의 일부를 통전시켜 커팅하는 단계(S600) 및 링이 자루(stalk)로부터 탈락하여 배변으로 배출되는 단계(S700)가 수행된다.
도 8a 내지 도 8d는 도 7에서 도시한 과정의 구체적인 단계를 설명하기 위한 도면이다.
도 8a는 도 7에서 S100 단계를 도시한 것이다.
도 8a를 참조하면, 제 2 캡(420)에 있는 제 2 스네어(460)를 폴립(200)에 걸리게 된다.
또한, 도 8b는 도 7에서 S200 단계를 도시한 것이다.
도 8b를 참조하면, 폴립(200)에 걸린 스네어(460)를 통해 폴립의 자루(stalk, 220)가 펼쳐지게 된다.
또한, 도 8c는 도 7에서 S300 단계를 도시한 것이다.
도 8c를 참조하면, 제 1 캡(410)에 있는 제 1 스네어(450)를 이용하여 자루(stalk, 200)가 펼쳐진 폴립(200)을 고정하게 된다.
또한, 도 8d는 도 7에서 S400 및 S500 단계를 도시한 것이다.
도 8d를 참조하면, 제 1 스네어(410)를 통해 고정된 폴립(200)을 제 1 캡(410)으로 흡입하고, 제 1 캡(410)으로 흡입된 폴립의 자루(stalk, 220) 부분, 즉, 헤드(210)를 완전히 포함하는 하단의 자루(220) 부분을 링으로 고정하게 된다.
따라서 본 발명의 구성에 따르면, 2 챔버 스네어 중 제 1 챔버를 통해 구부러져 있는 용종을 펼치고, 제 2 챔버를 통해 올가미(링)로 용종을 고정 시켜 용종의 헤드를 확실하게 제거하여 수술을 용이하게 할 수 있다.
또한, 본 발명은 기존의 올가미(링)와 달리 제 2 챔버 스네어 중 제 2 챔버에만 올가미(링)이 구비되도록 하고, 고정기구를 통해 용종을 제 2 챔버 내부로 끌어들여 바로 올가미(링)를 통해 고정시킴으로써, 올가미의 초기 진입 방향을 안정적으로 유지시킬 수 있고, 구조적인 측면에서 유리할 뿐만 아니라 수술시간을 절약할 수 있으며, 시술을 간편하게 할 수 있다.
한편, 본 발명에서는 설명의 편의를 위해, 스네어(400)의 챔버를 2개인 것으로 가정하였으나 본 발명의 내용이 이에 한정되는 것은 아니다.
즉, 2개 이상의 챔버로 스네어(400)를 구성함으로써, 보다 다양한 기능을 제공하는 스네어(400)를 구현할 수도 있다.
한편, 전술한 올가미(링, 320)는 스네어(400)의 외경에 구비되었다가 배출됨으로써, 폴립(200)을 흡입한 시간과 올가미(링, 320)로 고정시키는 시간 사이의 간격이 있어 사용자가 원하는 폴립(200)의 부분을 올라미(링)를 통해 고정하기 어렵다는 문제점도 존재한다.
따라서 본 발명에서는 기존의 올가미(링)와 달리 제 2 챔버 스네어 중 제 2 챔버에만 올가미(링)이 구비되도록 하고, 고정기구를 통해 용종을 제 2 챔버 내부로 끌어들여 바로 올가미(링)를 통해 고정시킴으로써, 올가미의 초기 진입 방향을 안정적으로 유지시킬 수 있고, 구조적인 측면에서 유리할 뿐만 아니라 수술시간을 절약할 수 있으며, 시술을 간편하게 하고자 한다.
도 9a 및 도 9b는 본 발명이 제안하는 종양 제거를 위한 2 챔버 스네어 구조의 다른 일례를 설명하기 위한 도면이다.
도 9a를 참조하면, 새로운 실시예에서 제안하는 스네어(400)의 구조는 기본적으로 제 6a에서 설명한 구조와 동일하다.
단, 도 9a에서는 링(320)이 제 1 챔버(410)의 외경에 존재한다.
즉, 도 6a에서는 스네어(400) 전체 외경에 링(320)이 존재하는 것에 반해, 도 9a에서는 링(320)이 제 1 챔버(410)의 외경에 존재한다.
따라서 도 8에서 설명한 제 1 스네어(410)를 통해 고정된 폴립(200)을 제 1 캡(410)으로 흡입하는 단계(S400)와 제 1 캡(410)으로 흡입된 폴립의 자루(stalk, 220) 부분, 즉, 헤드(210)를 완전히 포함하는 하단의 자루(220) 부분을 링으로 고정하는 단계(S500)가 수행될 때, 본 실시예에서 제안하는 구조는 큰 효과를 볼 수 있다.
즉, 폴립(200)을 제 1 캡(410)으로 흡입된 경우, 스네어(400) 전체 외경에 링(320)이 당해 폴립(200)을 고정하는 것이 아니라 제 1 챔버(410)의 외경에서 바로 출력되어 폴립(200)을 고정하게 되므로, 사용자는 원하는 정확한 부분을 고정할 수 있게 된다.
따라서 전술한 본 발명의 구성에 따르면, 용종절제 시술 시 특수금속으로 된 올가미를 내시경에 있는 가느다란 구멍을 통해 집어넣은 후, 올가미로 잡고 전기를 통과시켜서 용종을 잘라낼 때 사용하는 의료용 2 챔버 스네어가 제공될 수 있다.
또한, 본 발명은 2 챔버 스네어 중 제 1 챔버를 통해 구부러져 있는 용종을 펼치고, 제 2 챔버를 통해 올가미(링)로 용종을 고정 시켜 용종의 헤드를 확실하게 제거하여 수술을 용이하게 할 수 있다.
또한, 본 발명은 기존의 올가미(링)와 달리 제 2 챔버 스네어 중 제 2 챔버에만 올가미(링)이 구비되도록 하고, 고정기구를 통해 용종을 제 2 챔버 내부로 끌어들여 바로 올가미(링)를 통해 고정시킴으로써, 올가미의 초기 진입 방향을 안정적으로 유지시킬 수 있고, 구조적인 측면에서 유리할 뿐만 아니라 수술시간을 절약할 수 있으며, 시술을 간편하게 할 수 있다.
한편, 상기에서는 설명의 편의를 위해 본 발명이 제안하는 링(320)은 한 개 인 것으로 가정하였으나 본 발명의 내용이 이에 한정되는 것은 아니다.
즉, 복수의 링(320)을 동시에 이용하여, 하나씩 외부로 배출함으로써 복수의 용종의 헤드를 제거하는 것에도 이용될 수 있다.
상술한 바와 같이 개시된 본 발명의 바람직한 실시예들에 대한 상세한 설명은 당업자가 본 발명을 구현하고 실시할 수 있도록 제공되었다. 상기에서는 본 발명의 바람직한 실시예들을 참조하여 설명하였지만, 해당 기술 분야의 숙련된 당업자는 본 발명의 영역으로부터 벗어나지 않는 범위 내에서 본 발명을 다양하게 수정 및 변경시킬 수 있음을 이해할 수 있을 것이다. 예를 들어, 당업자는 상술한 실시예들에 기재된 각 구성을 서로 조합하는 방식으로 이용할 수 있다. 따라서, 본 발명은 여기에 나타난 실시형태들에 제한되려는 것이 아니라, 여기서 개시된 원리들 및 신규한 특징들과 일치하는 최광의 범위를 부여하려는 것이다.
본 발명은 본 발명의 정신 및 필수적 특징을 벗어나지 않는 범위에서 다른 특정한 형태로 구체화될 수 있다. 따라서, 상기의 상세한 설명은 모든 면에서 제한적으로 해석되어서는 아니 되고 예시적인 것으로 고려되어야 한다. 본 발명의 범위는 첨부된 청구항의 합리적 해석에 의해 결정되어야 하고, 본 발명의 등가적 범위 내에서의 모든 변경은 본 발명의 범위에 포함된다. 본 발명은 여기에 나타난 실시형태들에 제한되려는 것이 아니라, 여기서 개시된 원리들 및 신규한 특징들과 일치하는 최광의 범위를 부여하려는 것이다. 또한, 특허청구범위에서 명시적인 인용 관계가 있지 않은 청구항들을 결합하여 실시예를 구성하거나 출원 후의 보정에 의해 새로운 청구항으로 포함할 수 있다.

Claims (4)

  1. 자루(stalk)와 상기 자루에 연결된 헤드(head)를 포함하는 용종을 제거하기 위한 스네어에 있어서,
    중공의 관 형태로 이루어지고, 내부가 복수의 챔버(chamber)로 구획된 몸체;
    탄력을 이용하여 상기 몸체의 적어도 일부에 구비된 링;
    상기 복수의 챔버 중 제 1 챔버를 통해 외부로 배출되어 상기 자루에 걸쳐지는 제 1 스네어; 및
    상기 복수의 챔버 중 제 2 챔버를 통해 상기 외부로 배출되는 제 2 스네어;를 포함하되,
    상기 자루에 걸쳐진 제 1 스네어가 당겨짐에 따라 상기 자루가 펼쳐지는 경우,
    상기 제 2 스네어가 상기 펼쳐진 자루의 전체 영역 중 상기 헤드 하단의 제 1 영역을 당기고,
    상기 몸체로부터 배출된 링은 상기 탄력을 통해 축소되어 상기 당겨진 제 1 영역을 고정하는 것을 특징으로 하는 용종 제거용 스네어.
  2. 제 1항에 있어서,
    상기 링은 상기 제 2 챔버의 외경에 구비되고,
    상기 제 2 스네어가 상기 제 1 영역을 상기 제 2 챔버의 내부로 당기는 경우, 상기 링은 상기 당겨진 제 1 영역으로 바로 배출되는 것을 특징으로 하는 용종 제거용 스네어.
  3. 제 1항에 있어서,
    전류를 이용하여 상기 고정된 제 1 영역의 상단을 절제함으로써, 상기 헤드를 제거하는 것을 특징으로 하는 용종 제거용 스네어.
  4. 제 1항에 있어서,
    상기 제 1 스네어 및 제 2 스네어는 갈고리 형태 및 복수의 발 형태 중 적어도 하나로 구비되는 것을 특징으로 하는 용종 제거용 스네어.
