Abstract
プローブ本体が十分な曲げ剛性を備え、かつ十分な吸引効率を確保しながら、従来の硝子体手術用プローブでは届きにくい網膜付近の硝子体等の回収を容易にする硝子体手術用プローブを提供する。パイプ形状のプローブ本体(11)と、そのプローブ本体の先端側に接続し、プローブ本体より外径が小さく、先端面(14)が封止され、側部に開口(13)を有する略円筒形状の先端部(12)と、プローブ本体と先端部のそれぞれの内径に適合する外径で、先端に刃部(21)を備え、プローブ本体と先端部の内面を摺動するカッター(20)と、を有する硝子体手術用プローブ(10)とする。
Description
本発明は、眼科手術に用いられる硝子体手術用プローブに関するものである。
眼科手術に用いられる硝子体手術用プローブは、眼球内のゼリー状の硝子体や硝子体が変性して形成された網膜上の増殖膜(以下、「硝子体等」とする)を切断・除去するために用いられる(例えば、特許文献1、特許文献2参照)。
図5は、従来の硝子体手術用プローブの断面図である。このような従来の硝子体手術用プローブ100は、管の先端面14を封止した形状のプローブ本体11と、そのプローブ本体11の内面に接しながら軸方向に沿って摺動可能で先端に刃部21を備えるカッター20とを有している。プローブ本体11の先端付近の側部には開口13が設けられ、そこから硝子体等50を吸引する。このとき、カッター20が摺動して刃部21が開口13を通過するときに硝子体等50を切断し、小さく切断された硝子体等50は、プローブの奥側(図5の左方向)に吸引され、回収される構成である。
硝子体手術用プローブ100においては、先端面14から開口13までの距離は、短い方が望ましい。それは、硝子体等50が、網膜近くまであったり、若しくは網膜付近を浮遊しているためであり、できるだけ開口13を網膜に近づけることで、硝子体等50の取り残しを減らすことを可能とする。また、プローブ本体11が網膜に当たって傷つけることがないように、先端面14は突起等のない平面であることが望ましい。
また、よりプローブ本体11を網膜に近づけるために、先端面14を傾斜面にした硝子体手術用プローブ101を用いることがある(例えば、特許文献2参照)。図6は、先端面を傾斜面にした従来の硝子体手術用プローブの断面図である。
このように先端面14を傾斜面にすることで、よりプローブ本体11を網膜に近づけることができる場合があり、それによって網膜付近の硝子体等50を吸引し易くしている。しかし、このときのカッター20は、プローブ本体11の内部を摺動するときに回転することがあるため、傾斜面14の先端側までは届かず、先端側に開口13を設けることはできない。つまり、開口13は、カッター20の刃部21が届く傾斜面14の手前側に設けなければならないことになる。
斯かる事情に鑑み、本発明は、先端部をプローブ本体の径よりも細い径の略円筒形状にすることにより、プローブ本体が十分な曲げ剛性を備え、かつ十分な吸引力を確保しながら、従来の硝子体手術用プローブでは届きにくい網膜付近の硝子体等の回収を容易にする硝子体手術用プローブを提供することを目的とする。
本発明の硝子体手術用プローブは、パイプ形状のプローブ本体と、そのプローブ本体の先端側に接続し、プローブ本体より外径が小さく、先端面が封止され、側部に開口を有する略円筒形状の先端部と、プローブ本体と先端部のそれぞれの内径に適合する外径で、先端に刃部を備え、プローブ本体と先端部の内面を摺動するカッターと、を有するものである。
ここで、プローブ本体と先端部の内径が同一であることにしてもよい。
本発明の硝子体手術用プローブによれば、十分な曲げ剛性と吸引力を確保しつつ、先端部の径が細いことで網膜付近の硝子体等の回収を容易に行うことができるという効果を奏することができる。
以下、本発明の実施の形態を添付図面を参照して説明する。
図1は、本発明の硝子体手術用プローブであって、(a)は側面図、(b)は正面図である。硝子体手術用プローブ10は、パイプ形状のプローブ本体10と、開口13を有し径の細い先端部12とを接続した外観をしている。
先端部12は、先端を平坦な先端面14で封止した略円筒形状であって、先端部12の外径は、プローブ本体11の外径よりも細くなっている。このような硝子体手術用プローブ10を形成する材料としては、プローブ本体11には従来から用いられている27ゲージのパイプを用い、先端部12には29ゲージのパイプを用いることができるが、この組み合わせに限定するものではない。
プローブ本体11と先端部12の内面をカッター20が摺動し、カッター20の先端に備えられている刃部21が開口13を通過するときに、開口付近にある硝子体等を切断することで、硝子体等を回収し易くしている。
図2は、硝子体手術用プローブの使用状況を説明する図である。本発明の硝子体手術用プローブ10は、先端部12がプローブ本体11よりも径が細くなっているため、先端部12に設けられた開口13を、より網膜30に近付けることが可能になっている。これによって、網膜30の近くの硝子体や硝子体が変性して形成された網膜上の増殖膜(硝子体等50)を吸引し易い構造になっている。なお、プローブ本体11を先端部12と同じ細い材料にすれば、確かに開口13を網膜30に近付けることは可能となるのだが、その場合は、プローブ本体11が細すぎることで、曲げ剛性が弱くなり、かつ、吸引力が従来よりも弱くなる。また、硝子体等50を回収するときに、内部で詰まりを生じて吸引力が低下する可能性もある。従って、プローブ本体11は、従来使用されている太さの材料よりも細い材料を用いることは、性能の面から難しいと考えられる。
図3は、硝子体手術用プローブの実施例を示す断面図であって、(a)はカッターを引いた状態、(b)は硝子体等を切断した状態である。硝子体手術用プローブ10は眼球を貫通して使用するものなので、プローブ本体11の径は、例えば27ゲージのようにかなり細いものである。従って、前述した通り、硝子体等50を開口13から吸引するときには、内部を摺動するカッター20によって硝子体等50を小さく切断する必要がある。
一方、先端部12に設けられた開口13は、硝子体等50の吸入口となるので、回収したい硝子体等50にできるだけ開口13を近付けるのが望ましい。そこで、本発明のように、先端部12をプローブ本体11の外径よりも細くしているのだが、硝子体手術用プローブ10は、プローブ本体11に径の細い先端部12を接続したものなので、プローブ本体11と先端部12とでは、基本的に内径が異なることになる。そこで、カッター20もプローブ本体11と先端部12のそれぞれの内径に適合させて外径を絞ることで、プローブ本体11と先端部12の内面に摺動することを可能にしている。なお、カッター20はプローブの内部で回転することがあるので、プローブ本体11と先端部12の中心軸は一致させる必要がある。
小さく切断された硝子体等50は、硝子体手術用プローブ10の奥側(図の左側)に向かって吸引されて回収される。このとき、プローブ本体11は従来製品と同様の太さのパイプとすることができるので、従来通りの吸引力を有し、吸引の際、硝子体等50が詰まりにくく、従来通りの回収能力を確保できる。なお、プローブ本体11の曲げ剛性も従来製品と同様である。
図4は、別の硝子体手術用プローブの実施例を示す断面図であって、(a)はカッターを引いた状態、(b)は硝子体等を切断した状態である。ここで図示した硝子体手術用プローブ10aは、外径は先端部12がプローブ本体11よりも細くしているが、内径はプローブ本体11と先端部12を同じにしたものである。
図4に示すような硝子体手術用プローブ10aの構造は、プローブ本体11と先端部12とにそれぞれ異なる材料のパイプを接合して製造することは難しい。しかし、壁厚の厚い材料を用いて、先端部12を切削加工等により形成することにすれば、製造は可能である。
このような硝子体手術用プローブ10aであれば、図3に示す硝子体手術用プローブと比較すると、カッター20を一定の径にすることができるため、カッター20の製造が容易であるという利点がある。

- パイプ形状のプローブ本体と、
前記プローブ本体の先端側に接続し、前記プローブ本体より外径が小さく、先端面が封止され、側部に開口を有する略円筒形状の先端部と、
前記プローブ本体と前記先端部のそれぞれの内径に適合する外径で、先端に刃部を備え、前記プローブ本体と前記先端部の内面を摺動するカッターと、
を有することを特徴とする硝子体手術用プローブ。 - 前記プローブ本体と前記先端部の内径が同一であることを特徴とする請求項1に記載の硝子体手術用プローブ。
