Abstract
パイプが細い場合でも先端の開口位置を大きく動かすことができ、広い範囲で硝子体等の吸引を可能とする硝子体カッターを提供する。先端側面に開口(21)を備えるパイプ(20)と、そのパイプ(20)の内面に沿って摺動するカッター(30)と、パイプ(20)に接続しカッター(30)を摺動させる動力部(25)と、パイプ(20)が貫通し動力部(25)を内部に備えるアウターケース(40)と、を有し、そのアウターケース(40)が、眼球(60)に装着されるカニューレ(50)と一体的になるように接続する接続部(41)を有することを特徴とする硝子体カッター(10)とする。さらに、パイプ(20)のアウターケース(40)からの突出量を変更自在にするとよい。
Description
本発明は、眼科手術に用いられる硝子体カッターに関する。
眼科手術に用いられる硝子体カッターは、眼球内のゼリー状の硝子体や硝子体が変性して形成された網膜上の増殖膜(以下、硝子体等とする。)を切断・除去するために用いられる。特許文献1には、そのような硝子体カッター(硝子体手術用プローブ)の先端部の構造について記載されている。図5は、硝子体カッターの先端部の断面図である。
硝子体カッターは、先端部で封止したパイプ20と、そのパイプ20の内面に接しながらパイプ20の軸方向に摺動可能なカッター30とを有している。パイプ20の先端付近の側面には開口21が設けられ、そこから硝子体等65を吸引する。このとき、カッター30がパイプ20の内部を摺動し、カッター30の先端が開口21を通過するときに硝子体等65を切断し、小さく切断された硝子体等65は、パイプ20の奥側(図5の右方向)に吸引され、回収される構成である。
このような硝子体カッターは、パイプを眼球に刺通したカニューレに挿し込んで使用される。そのとき、硝子体カッターのパイプ径が小さい場合には、パイプ先端の開口位置を動かそうとしても、カニューレの外側でパイプが曲がってしまい、眼球内では開口位置を大きく動かすことが難しい。しかし、硝子体カッターは眼球内の手術に用いられるものなので、パイプ径は小さい方が好ましい。特に最近は、27ゲージ(0.4mm)のパイプを使うこともあり、そのような細い硝子体カッターは、特に剛性が低いので、手術を行うにあたっては、眼球を動かさないで手術を行うことを指示しているほどである。
しかし、パイプを大きく動かせず、また、眼球を動かさない場合には、硝子体等を吸引できる範囲は制限される。そうすると、眼球内に硝子体等が残存する可能性が高くなるので、できれば硝子体カッターのパイプが細い場合でも、硝子体カッターや眼球を自由に動かして手術をすることが望まれる。
斯かる事情に鑑み、本発明は、パイプが細い場合でも先端の開口位置を大きく動かすことができ、広い範囲で硝子体等の吸引を可能とする硝子体カッターを提供することを目的とする。
本発明の硝子体カッターは、先端側面に開口を備えるパイプと、そのパイプの内面に沿って摺動するカッターと、パイプに接続しカッターを摺動させる動力部と、パイプが貫通し動力部を内部に備えるアウターケースと、を有する硝子体カッターであって、そのアウターケースが、眼球に装着されるカニューレと一体的になるように接続する接続部を有することとする。
また、パイプ、カッター及び動力部が、アウターケースに対してパイプの軸方向に前後移動でき、パイプのアウターケースからの突出量が変更自在であることにするとよい。
本発明によれば、硝子体カッターとカニューレとを一体的にすることができるので、パイプが細い場合でも、眼球内でパイプの開口を動かすことができる範囲が広く、硝子体を吸引できる範囲を広くすることができるという効果を奏する。
また、アウターケースからのパイプの突出量を変更自在にすることで、カニューレと一体のまま眼球手前付近の硝子体等も吸引可能になるという効果を奏する。
以下、本発明の実施の形態を添付図面を参照して説明する。
図1は、本発明の硝子体カッターの外観図であって、(a)はカニューレに接続前の状況、(b)はカニューレに接続後の状況を示す。硝子体カッター10は、眼球60に刺通してあるカニューレ50に、硝子体カッター10のパイプ20を挿入して、眼球60内の硝子体等を吸引するものである。
硝子体カッター10は、アウターケース40を貫通してパイプ20が突出しており、パイプ20の先端は封止され、その先端側面に硝子体等を吸引する開口21が開いている。アウターケース40の中には、パイプ20からつながる動力部が内蔵されており、この動力部によってパイプ20の内面に沿ってカッター30を摺動させる。硝子体等の回収の方法は前述した通りで、カッター30がパイプ20内面を摺動することで、開口21から吸引した硝子体等を小さく切断し、パイプ20の奥側へ吸い込む構成になっている。
カニューレ50は、眼球60に刺通する管状の本体部51と、ストッパーとなるベース部52とからなる。図1(a)に示すように、眼球60に装着されたカニューレ50の本体部51に硝子体カッター10のパイプ20を挿し込んで、図1(b)に示す状態にする。このとき、アウターケース40に設けられた凸状の接続部41がカニューレ50のベース部52と合体することで、硝子体カッター10とカニューレ50が一体化される。なお、アウターケース40とカニューレ50の接続の方法は、特に限定するものではなく、接続した状態でアウターケース40を動かしたときに、カニューレ50とパイプ20が一体的に動くように接続していればよい。
次に、このように硝子体カッター10とカニューレ50を一体化することによる効果を説明する。図2は、パイプが細い場合の開口の移動範囲を示す図であって、(a)は従来の硝子体カッター、(b)は本発明の硝子体カッターを示す。
図2(a)では、硝子体カッター10とカニューレ50を一体化しておらず、また、パイプ20が細いため、アウターケース40を動かしたとき、パイプ20はカニューレ50の外側の位置20aで曲がることになる。そうすると、眼球内でパイプ20を大きく動かすことができないということになる。
そこで、図2(b)のように硝子体カッター10とカニューレ50を一体化すれば、アウターケース40を動かしたときに、カニューレ50やパイプ20もアウターケース40と同じ角度で動くため、パイプ20の先端に設けられている開口21を大きく動かすことができるようになる。
しかし、硝子体カッター10とカニューレ50を一体化した場合、カニューレ50が眼球60から抜けてはいけないので、アウターケース40を前後方向(眼球に近づけたり遠ざけたりする方向)に動かすことは制限される。そうすると、眼球60の一定の深さの位置でしか開口21を動かすことができなくなるということになるので、刺通深さを変えられるように、パイプ20の突出量を調整可能にすることとした。
図3は、本発明の硝子体カッターの内部構造を示す断面図であって、(a)はパイプの突出量が大きい場合、(b)はパイプの突出量を小さくした場合を示す。ここでは、カニューレ50と硝子体カッター10とが一体化された状態を図示している。そして、アウターケース40からの突出量Lについて、図3(a)は最大の突出量Lmax、図3(b)は最少の突出量Lminを例示している。
ここで、パイプ20、カッター30及び動力部25は、一体に結合されているので、突出量Lの調整は、アウターケース40に内蔵されている動力部25を、アウターケース40に対し前後方向に移動させることにより行えばよいことになる。
動力部25の動かし方については、特に限定しないが、図3に示した例では、動力部25に繋がっている突出量調整具26をアウターケース40を貫通させて設け、この突出量調整具26をパイプ20の軸方向に前後移動させることで突出量Lを変化させるものとしている。
図4は、本発明の開口の移動範囲を示す図であって、(a)はパイプの突出量が大きい場合、(b)はパイプの突出量を小さくした場合を示す。破線で示した範囲が、開口21の移動範囲である。
突出量を大きくしたときは、図4(a)のように眼球60の奥深い位置の硝子体等を吸引することができる。ここで、カニューレ50と硝子体カッター10とが一体でパイプ20の突出量を変化させられないとすると、カニューレ50を眼球60から抜かなければ、眼球60の手前側の硝子体等を吸引することが難しい。しかし、本発明では、パイプ20の突出量を変化させることができるので、突出量を小さくすることで、カニューレ50を眼球60から抜かなくても、眼球60の手前側の硝子体等を吸引することが可能となる。
以上のように、本発明は、カニューレと硝子体カッターを一体化することで、パイプが細い場合でも開口位置を大きく動かすことができるようにし、さらに、パイプの突出量を調整可能にすることで、眼球全体に亘る硝子体等の吸引を可能にすることができるものである。

- 先端側面に開口を備えるパイプと、前記パイプの内面に沿って摺動するカッターと、前記パイプに接続し前記カッターを摺動させる動力部と、前記パイプが貫通し前記動力部を内部に備えるアウターケースと、を有する硝子体カッターであって、
前記アウターケースが、眼球に装着されるカニューレと一体的になるように接続する接続部を有することを特徴とする硝子体カッター。 - 前記パイプ、前記カッター及び前記動力部が、前記アウターケースに対して前記パイプの軸方向に前後移動でき、前記パイプの前記アウターケースからの突出量が変更自在であることを特徴とする請求項1に記載の硝子体カッター。
