この発明の更に他の一態様に係る基端側プローブユニットは、第2中心軸を規定し、超音波振動を前記第2中心軸に沿ってその基端部から先端部の処置部に向かって伝達可能な先端側プローブを保持する先端側保持部に着脱可能な基端側カバーと、前記基端側カバーの内側に配置され、第1中心軸を規定し超音波振動子からの超音波振動が前記第1中心軸に沿ってその基端部から先端部に向かって伝達され、前記基端側カバーが前記先端側保持部に装着されたときに、前記第1及び第2中心軸が一致し、その先端部が前記先端側プローブの基端部に突き当てられるとともに、前記先端側プローブの前記基端部がその先端部に突き当てられた状態で前記超音波振動子からの振動をその基端部から前記先端側プローブの先端部の処置部に伝達可能であるとともに、前記先端側プローブに対して相対的に前記第1及び第2中心軸の軸回りに回転又は回動可能で前記先端側プローブの先端部の処置部を所望の方向を向かせる、基端側プローブとを有する。
図1Aに示すように、この実施形態に係る処置具10は、基端側プローブユニット(超音波振動子ユニット)12と、先端側プローブユニット14とを備えている。基端側プローブユニット12及び先端側プローブユニット14は、互いに対して、取り付け及び取り外し可能である。
第1接続部28は、外装ケース26の本体26aの先端部の環状凹部26dに固定されたカムリング32と、カムリング32に形成されたカム溝34とを有する。カムリング32は、外装ケース26の本体26aの先端部に対して例えばネジ止め等により適宜に固定されている。カムリング32は、中心軸C1に沿って先端の先端縁部32aと、基端の基端縁部32bとを有する。カムリング32の基端縁部32bは、外装ケース26の環状凹部26dに当接され、中心軸C1に沿って基端側への移動が規制されている。
カム溝34は、1つであっても良いが、2つや3つなど、複数であることも好ましい。この実施形態では、図1Aに示すように、1対のカム溝34が、中心軸C1に対して対向している。また、カム溝34は、中心軸C1に対して例えば90度離れた位置など、適宜の位置に形成されていることも好適である。
先端側振動伝達体54は、基端部と先端部とにより中心軸C2を規定する。この実施形態では、中心軸C2は真っ直ぐであることが好ましい。先端側振動伝達体54は、その基端部と先端部との間にフランジ部54aが形成されている。フランジ部54aは、円形以外の形状に形成されている。
先端側振動伝達体54の基端側突き当て部55の外径は、基端側振動伝達体24の先端側突き当て部25の外径と同じか、それよりも僅かに大きく形成されていることが好ましい。また、先端側振動伝達体54の基端側突き当て部55の外径は、基端側振動伝達体24の先端側突き当て部25の外径よりも僅かに小さく形成されていても良い。
基体56の内径は、先端側プローブユニット14の先端側振動伝達体54の第1振動伝達部54b、及び、基端側プローブユニット12の基端側振動伝達体24の外径よりも僅かに小さく形成されている。このため、基体56に対して第1振動伝達部54bがその中心軸C2の軸周りに回転可能である。
ノブ本体72は、先端側振動伝達体54が挿通され、処置部54dが回転ノブ58の先端から中心軸C2に沿って先端側に突出する。ノブ本体72の外周面は、ユーザが中心軸C2の軸周りにノブ58を回転させる操作部(回転操作部)72aとして形成されている。ノブ本体72の先端の内周面は、先端側振動伝達体54の第2振動伝達部54cを中心軸C2の軸周りに回動又は回転可能に支持する支持部72bとして形成されている。
ボール78は、この実施形態では基体連結部74に支持されているものとして説明したが、ノブ連結部64に支持されていても良いことはもちろんである。すなわち、ノブ連結部64と回転ノブ58との間には、複数のボール78が支持され、特に、ノブ連結部64及び回転ノブ58の一方に、複数のボール78が支持されていることが好ましい。このため、ノブ連結部64と回転ノブ58との間の回転の摩擦力を低減させている。
ボール78がノブ連結部64又は基体連結部74に支持されている他、スラスト玉軸受等、中心軸C2の軸周りの回動又は回転を許容する軸受がノブ連結部64と回転ノブ58との間に配置されていることも好適である。その他、ノブ連結部64と回転ノブ58との間には、ボール78又はスラスト玉軸受の代わりに、例えば、潤滑性シート部材、無給油ブッシュ等が配置されていることが好ましい。
ここで、基体56のノブ連結部64と回転ノブ58の基体連結部74との間には、互いの連結により、例えば0から500N程度の押し付け力が発生していることが好ましい。このため、先端側プローブユニット14単体では、基体56に対して回転ノブ58を中心軸C2の軸周りに回動又は回転させる際に、容易に操作することができる。
また、後述するように、基端側プローブユニット12と先端側プローブユニット14とが接続され、基端側振動伝達体24の先端側突き当て部25と先端側振動伝達体54の基端側突き当て部55との間の摩擦力が発生する。このとき、後述する付勢体86により先端側突き当て部25と基端側突き当て部55との間の摩擦力を調整することで、回転ノブ58を回動又は回転させるのに必要な力(回転ノブ58の回動又は回転し難さ)を適宜に設定することができる。
なお、付勢体86は、圧縮コイルバネの他、皿バネ、多重皿バネ等を適宜に用いることができる。
一般には生体組織を処置する処置具に用いられる超音波振動子31が駆動される駆動周波数は例えば10kHzから100kHz程度である。
例えば、超音波振動子31の駆動周波数をFとし、半波長の長さをλ/2とする。このため、基端側振動伝達体24の先端と先端側振動伝達体54の基端とを振動の腹で突き当てられる場合、超音波振動子31の先端から基端側振動伝達体24の先端までの長さは、λ/2×n(n:1以上の整数)である。同様に、先端側振動伝達体54の基端から先端の処置部54dまでの長さは、λ/2×n(n:1以上の整数)である。すなわち、超音波振動子31から先端側振動伝達体54の先端の処置部54dまでの長さは、半波長の2倍以上の整数倍(n倍)である。
したがって、超音波振動子31の選択によっては、基端側振動伝達体24、先端側振動伝達体54だけでなく、外装ケース26の中心軸C1に沿った長さ、基体56の中心軸C2に沿った長さ等が変化する。
まず、基端側プローブユニット12と先端側プローブユニット14とを取り付ける場合について説明する。
すなわち、基端側保持部(基端側カバー)22及び先端側保持部52は、基端側振動伝達体24の先端部に先端側振動伝達体54の基端部を突き当てて超音波振動子31からの振動を基端側振動伝達体24の基端部から先端側振動伝達体54の先端部に伝達可能な状態を保持するとともに、第1及び第2中心軸C1,C2を一致させた状態を保持する。
なお、基端側プローブユニット12の基端側振動伝達体24の先端側突き当て部25と、先端側プローブユニット14の先端側振動伝達体54の基端側突き当て部55とは、基端側振動伝達体24の先端側突き当て部25から先端側振動伝達体54の基端側突き当て部55に超音波振動が伝達されたときに、振動の節位置以外の位置であり、特に、応力がなくなる振動の腹位置であることが好適である。
ここで、基端側プローブユニット12及び基体56に対して回転ノブ58を中心軸C2の軸周りに回動させることが可能な向きは2方向あるが、いずれの方向にも回動させることができる。
回転ノブ58の回動に伴って、回転ノブ58に回り止めされた先端側振動伝達体54が、回転ノブ58とともに、第2中心軸C2の軸周りに回動する。回転ノブ58は、ボール78等により基体56との摩擦が軽減されている。このため、基体56は、回転ノブ58とは一緒に回転しない。また、外装ケース26に回り止めされた基端側振動伝達体24は超音波振動子ユニット30とともにそのままの状態を維持する。
そして、基端側プローブユニット12の基端側振動伝達体24の先端側突き当て部25と、先端側プローブユニット14の先端側振動伝達体54の基端側突き当て部55とは中心軸C1,C2の軸周りに互いに対して摺動しながら回動する。このようにして、先端側振動伝達体54がその中心軸C2の軸周りに回動して、基端側振動伝達体24及び超音波振動子ユニット30に対して、処置部54dの向きが変えられる。このため、この実施形態に係る処置具10は、基端側振動伝達体24、外装ケース26及び超音波振動子ユニット30を動かさずに、処置部54dの向きを調整することができる。したがって、ユーザに対して、基体56の基部62の外周面63に配置されたハンドスイッチの位置を変更せずに、処置部54dの向きを所望の向きに変更することができる。
そして、基端側プローブユニット12を中心軸C1に沿って基端側に移動させ、先端側プローブユニット14を中心軸C2に沿って先端側に移動させて、両者を離す。このため、先端側プローブユニット14の基体56の管部66が、基端側プローブユニット12の収容部26cから取り出される。このようにして、基端側プローブユニット12の第1接続部28と先端側プローブユニット14の第2接続部60との接続が解除される。
基端側プローブユニット12の外装ケース26と、先端側プローブユニット14の基体56とを把持して、両者を中心軸C1,C2の軸周りに相対的に反対方向に回動させることにより、基端側プローブユニット12の第1接続部28と先端側プローブユニット14の第2接続部60とを接続し、又は接続を解除することができる。すなわち、第1及び第2接続部28,60は、協働して、基端側プローブユニット12と先端側プローブユニット14との間の着脱機構を構成する。このような着脱機構28,60は、第1及び第2保持部22,52を第1及び第2中心軸C1,C2の軸回りに相対的に回転又は回動することにより互いに対して取り付け可能であるとともに、取り外し可能である。
このように、基端側プローブユニット12と先端側プローブユニット14とを接続し即ち取り付け、又は接続を解除する即ち取り外す場合、専用の工具が必要ではない。したがって、基端側プローブユニット12に対して、処置部54dを有する先端側プローブユニット14を交換する作業を容易に行うことができる。
このとき、付勢体86の作用により、基端側振動伝達体24の先端側突き当て部25と、先端側振動伝達体54の基端側突き当て部55とは、超音波振動を伝達可能な適宜の圧力で互いに対して密着している。このため、超音波振動子31に振動を発生させたとき、基端側振動伝達体24の先端側突き当て部25と、先端側振動伝達体54の基端側突き当て部55との間をネジ止めするのと同様な振動伝達性を維持することができる。
超音波振動を基端側振動伝達体24から先端側振動伝達体54に伝達させるため、基端側振動伝達体24の先端部と先端側振動伝達体54の先端部とを突き当てる際に、付勢体86により大きな力が付与される。この実施形態に係るカムピン84は、カム溝34の開口34aから係合部34cに向かう際に回転しながら移動する。このため、付勢体86により大きな力が付与されるが、基端側プローブユニット12と先端側プローブユニット14との接続を、比較的小さな力の付与で行うことができる。
処置具10の先端側プローブユニット14は、先端側保持部52と、第2振動伝達体(先端側プローブ)54とを有する。先端側保持部52は、第1中心軸C1を規定し超音波振動子31からの超音波振動が第1中心軸C1に沿ってその基端から先端に向かって伝達される第1振動伝達体(基端側プローブ)24を保持する外装ケース(基端側保持部)26に着脱可能である。先端側振動伝達体54は、先端側保持部52に保持され、第2中心軸C2を規定し、超音波振動を第2中心軸C2に沿ってその基端から先端の処置部54dに向かって伝達可能である。先端側振動伝達体54は、先端側保持部52と基端側保持部22とが装着されたときに、第1及び第2中心軸C1,C2が一致した状態で基端側振動伝達体24の先端の先端側突き当て部25がその基端の基端側突き当て部55に突き当てられるとともに、基端側振動伝達体24の先端側突き当て部25が基端側突き当て部55に突き当てられた状態で超音波振動子31からの振動を基端側振動伝達体24の基端からその先端の処置部54dに伝達可能である。そして、先端側振動伝達体54は、基端側振動伝達体24に対して相対的に第1及び第2中心軸C1,C2の軸回りに回転又は回動可能でその先端の処置部54dを所望の方向に向かせることができる。
そして、先端側保持部52は、第2中心軸C2の軸回りに回転させる操作により、第1振動伝達体(基端側プローブ)24に対して相対的に第2振動伝達体(先端側プローブ)54を先端側保持部52の第2中心軸C2の軸回りに回転又は回動させる回転ノブ(操作部)58を有する。
処置具10の基端側プローブユニット12は、基端側保持部(基端側カバー)22と、第1振動伝達体(基端側プローブ)24とを有する。基端側保持部22は、第2中心軸C2を規定し、超音波振動を第2中心軸C2に沿ってその基端から先端の処置部54dに向かって伝達可能な第2振動伝達体(先端側プローブ)54を保持する先端側保持部52に着脱可能である。基端側振動伝達体24は、基端側保持部22に保持され、第1中心軸C1を規定し超音波振動子31からの超音波振動が第1中心軸C1に沿ってその基端から先端に向かって伝達される。基端側振動伝達体24は、基端側保持部22が先端側保持部52に装着されたときに、第1及び第2中心軸C1,C2が一致し、その先端の先端側突き当て部25が先端側振動伝達体54の基端の基端側突き当て部55に突き当てられるとともに、先端側振動伝達体54の基端側突き当て部55がその先端の先端側突き当て部25に突き当てられた状態で超音波振動子31からの振動をその基端から先端側振動伝達体54の先端の処置部54dに伝達可能である。そして、基端側振動伝達体24は、先端側振動伝達体54に対して相対的に第1及び第2中心軸C1,C2の軸回りに回転又は回動可能で先端側振動伝達体54の先端の処置部54dを所望の方向を向かせることができる。
処置具10は、第1振動伝達体(基端側プローブ)24と、第2振動伝達体(先端側プローブ)54と、保持部(基端側保持部(基端側カバー)22及び先端側保持部52)と、回転ノブ(操作部)58とを有する。第1振動伝達体(基端側プローブ)24は、その基端とその先端とにより規定される第1中心軸C1に沿って延出され、超音波振動子31からの超音波振動が第1中心軸C1に沿ってその基端から先端に向かって伝達される。第2振動伝達体(先端側プローブ)54は、その基端とその先端とにより規定される第2中心軸C2に沿って延出され、超音波振動を第2中心軸C2に沿ってその基端から先端に有する処置部54dに向かって伝達可能である。保持部は、基端側振動伝達体24の先端側突き当て部25に先端側振動伝達体54の基端側突き当て部55を突き当てて超音波振動子31からの振動を基端側振動伝達体24の基端から先端側振動伝達体54の先端に伝達可能な状態を保持するとともに、第1及び第2中心軸C1,C2を一致させた状態を保持する。回転ノブ58は、保持部に設けられ、第1及び第2中心軸C1,C2の軸回りに回転させる操作により、基端側振動伝達体24に対して相対的に先端側振動伝達体54を第1及び第2中心軸C1,C2の軸回りに回転又は回動させ先端側振動伝達体54の先端の処置部54dを所望の方向を向かせることができる。
保持部(先端側保持部)52は、基端側振動伝達体(基端側プローブ)24の先端部と第2振動伝達体(先端側プローブ)54の基端部とを一定の力で押し付ける付勢体86を有する。
この実施形態では、図1Aに示すように基端側振動伝達体24として超音波振動子ユニット30のホーン30b及びフランジ部30aと一体のチタン合金材製ロッドを用いる例について説明した。この変形例では、基端側振動伝達体24の基端の超音波振動子ユニット30のホーン30b及びフランジ部30aが基端側振動伝達体24に一体的に形成されているのではなく、別体として形成されている例について説明する。
図3に示すように、超音波振動子ユニット30は、ホーン30bにより振幅が拡大された振動を伝達する振動伝達部30cを有する。振動伝達部30cの先端には雌ネジ30dが形成され、基端側振動伝達体24の基端には雄ネジ24aが形成され、両者が連結されている。
したがって、基端側振動伝達体24の基端に、超音波振動子ユニット30が一体的に形成されていても、別体として着脱可能に形成されていても良い。
図1A中、基端側振動伝達体24の先端側突き当て部25と、先端側振動伝達体54の基端側突き当て部55とを中心軸C1,C2に直交する平面として示した。図4に示すように、例えば基端側振動伝達体24の先端側突き当て部25に凹部25a及び円環面(先端面)25bが形成され、先端側振動伝達体54の基端側突き当て部55に凸部55a及び円環面(基端面)55bが形成され、円環面25b,55b同士が突き当てられた状態で凹部25aと凸部55aとが嵌合可能であることも好適である。円環面25b,55b同士が突き当てられている場合、超音波振動子31に発生させた振動を基端側振動伝達体24から円環面25b,55bを通して先端側振動伝達体54に伝達可能である。
そして、円柱状の凹部25aの内周面と、3つの円弧面92aとが互いに対して面接触して、回動又は回転可能に形成されている。このとき、凹部25aと凸部55aとの関係により、中心軸C1,C2の軸周りに摺動して回動又は回転する際、中心軸C1,C2が一致した状態を維持することができる。
また、基端側振動伝達体24の先端側突き当て部25と、先端側振動伝達体54の基端側突き当て部55とが当接される際にも、中心軸C1,C2を一致させることができる。このため、中心軸C1,C2間の軸ズレを防止することができる。
なお、凸部55aのうち、中心軸C2に沿って基端側を先細の傾斜面とし、円環面55bに近接する位置で図5Aに示す状態に形成されていれば、凹部25aと凸部55aとが嵌合する際に、中心軸C1,C2をより容易に一致させ易くすることができる。
第1実施形態では、基端側振動伝達体24と先端側振動伝達体54とが同一の素材を用いるものとした。その他、両者の素材を適宜に変更して、基端側振動伝達体24の先端側突き当て部25が先端側振動伝達体54の基端側突き当て部55に対して摩耗し難く形成されていることも好適である。基端側振動伝達体24の先端側突き当て部25が先端側振動伝達体54の基端側突き当て部55に対して摩耗し難く形成されているのは、基端側プローブユニット12が再利用される確率が、先端側プローブユニット14が再利用される確率に比べて高いからである。
なお、基端側振動伝達部154aの先端と先端側振動伝達部154bの基端との連結部は、振動の腹位置に相当する。
ここで、第1実施形態では、図2Aに示すように、チタン合金材製の基端側振動伝達体24の先端側突き当て部25に直接、同じチタン合金材製の先端側振動伝達体54の基端側突き当て部55を突き当てて、基端側振動伝達体24に対して先端側振動伝達体54の処置部54dの向きを変えるなどの操作をする例について説明した。ここでは、図6Aに示すように、チタン合金材製の基端側振動伝達体24の先端側突き当て部25に、アルミニウム合金材製の先端側振動伝達体54の基端側振動伝達部154aの基端側突き当て部55を突き当てて、基端側振動伝達体24に対して先端側振動伝達体54の処置部54dの向きを変えるなどの操作をすることができる。この場合、超音波振動子30から処置部54dまでの距離を、適宜に長くすることができる。そして、チタン合金材製の基端側振動伝達体24の先端側突き当て部25よりも、アルミニウム合金材製の先端側振動伝達体54の基端側振動伝達部154aの基端側突き当て部55は軟らかく、ヤング率が低いので、基端側振動伝達体24の先端側突き当て部25の摩耗等の劣化を防止することができる。したがって、基端側プローブユニット12を長寿命化することができる。
また、基端側突き当て部55は、先端側突き当て部25よりも軟らかいので、先端側突き当て部25を基端側突き当て部55に突き当てたときに、両者が同じ素材で形成されている場合よりも先端側突き当て部25を基端側突き当て部55に密着し易い。このため、先端側突き当て部25を基端側突き当て部55に突き当てたときに、両者の密着性を向上させることができる。このため、先端側突き当て部25と基端側突き当て部55とを突き当てておく力量を、両者が同じ素材を用いる場合よりも小さくすることができる。このため、付勢体86の調整により、基端側プローブユニット12と先端側プローブユニット14とを連結する際の力量、分離させる際の力量を、第1実施形態で説明した場合よりも、小さくすることができる。
先端側振動伝達体54は、基端側振動伝達部154aにステンレス鋼材を用い、先端側振動伝達部154bにチタン合金材を用いても良い。これらの素材を適宜に用いても、図6Aに示す状態と同様に用いることができる。
さらに、先端側振動伝達体54は、基端側振動伝達部154aにアルミニウム合金材を用い、先端側振動伝達部154bにステンレス鋼材を用いても良い。これらの素材を適宜に用いても、図6Aに示す状態と同様に用いることができる。
図6Bに示す構造は、超音波振動子31から処置部54dまでの長さを短くして振幅を大きくする場合や、図6Aで示す構造に比べて大きな振幅で振動させたい場合に採用することが好適である。
中間部材157は、先端側突き当て部25が嵌合されて突き当てられる基端側凹部157aと、基端側突き当て部55が嵌合されて突き当てられる先端側凹部157bとを有する。
中間部材157は、基端側振動伝達体24よりも軟らかく、音響特性が良好な素材(振動を伝達し易い素材)が用いられる。基端側振動伝達体24がチタン合金材で形成されている場合、中間部材157は例えばアルミニウム合金材が用いられる。
中間部材157は、基端側振動伝達体24の先端及び先端側振動伝達体54の基端に嵌合される。中間部材157の中心軸C1,C2に沿った長さ(振動伝達に寄与する厚さ)は、例えば数ミリメートル程度であれば良い。
中間部材157は、基端側プローブユニット12と先端側プローブユニット14とが装着される際に、例えば付勢体86の付勢力により、基端側振動伝達体24の先端と先端側振動伝達体54の基端との間に装着される。処置部54dの向きを中心軸C2の軸周りに変更する場合、中間部材157は先端側振動伝達体54とともに回転しても良く、基端側振動伝達体24とともにその位置を維持しても良い。
基端側プローブユニット12の基端側振動伝達体24は、チタン合金材を用いるほか、音響特性が良好な、例えばアルミニウム合金材を用いることができる。基端側振動伝達体24にアルミニウム合金材を用いる場合、先端側突き当て部25には、硬化アルマイト処理、セラミック溶射等の表面処理を行うことが好ましい。
図7及び図8Aに示すように、この変形例では、基体56は、その中心軸C2に対して基端側に、基部62に連続して、第2接続部60を覆う筒状体68を有する。
基体56の筒状体68の外周面68aには、可動レバー142を収容する、例えば中心軸C2に平行に長手軸を有することが好ましい収容部168aと、可動レバー142の係合突起142aが係合される係合凹部168bとが形成されている。可動レバー142には、枢支軸(可動部)144を支点としてトーションバネ(可動部)146aが配置されている。トーションバネ146aが可動レバー142を収容部168aに向かって付勢することで、可動レバー142が収容部168aに収容されている。このため、超音波振動を用いた処置の最中に、可動レバー142が急激に外側に開いたり、先端側プローブユニット14の可動レバー142がユーザの視界を遮るのを極力防止している。
例えば基端側プローブユニット12と先端側プローブユニット14とを装着する場合、可動レバー142をトーションバネ146aの付勢力に抗して、枢支軸144の軸周りに回動させる。すなわち、図8A中に実線で示すように、可動レバー142を回動させて立ち上げる。このとき、可動レバー142の係合突起142aと筒状体68の係合凹部168bとを係合させる。このため、可動レバー142を立ち上げた状態に維持することができる。
また、可動レバー142を最も立ち上げた状態で、可動レバー142の遠位端142bを係合凹部168bに引っ掛けて係合させることにより、可動レバー142の位置を保持することができる。このため、ユーザは、立ち上げた可動レバー142の遠位端142bを中心軸C2の軸周りに容易に回動させることができる。
ユーザは、基体56の外周面の収容部168aと可動レバー142の遠位端142bとの間に指を入れる。このため、トーションバネ146bの付勢力に抗して可動レバー142を例えば収容部168aに収容された状態を0度としたときに、略90度の範囲まで適宜に立ち上げることができる。ユーザは、基体56の外周面68aを保持して中心軸C2の軸周りに回動させて基端側プローブユニット12に対して装着し、又は、離脱させるよりも、立ち上げた可動レバー142の遠位端142bを中心軸C2の軸周りに回動させる方が、軽い力で基端側プローブユニット12に対して装着し、又は、離脱させることができる。
この変形例では、基端側プローブユニット12に対して先端側プローブユニット14を装着し、離脱させる際に使用する治具(着脱補助具)210について主に説明する。
基端側プローブユニット12と先端側プローブユニット14とを離脱させる場合も、治具210を使用することにより、付勢体86の付勢力に抗して、基端側プローブユニット12と先端側プローブユニット14とをより軽い力で容易に離脱させることができる。
基端側プローブユニット12に対して先端側プローブユニット14を取り付ける場合、先端側プローブユニット14の回動ノブ58の押圧ピン370の遠位端370aを中心軸C2に向かって押し込んで近接させる。このため、中心側端部370bが延出筒368の嵌合溝368aの1つに入り込む。このため、基体56に対する回転ノブ58の回転が防止される。この状態で、第1実施形態で説明したのと同様に、例えば右手で基端側プローブユニット12の外装ケース26を把持し、左手で押圧ピン370を押圧しながら回転ノブ58を把持して、基端側プローブユニット12に対して先端側プローブユニット14を装着する。
この変形例の基端側プローブユニット12が第1実施形態で説明した基端側プローブユニット12と異なるのは、基端側プローブユニット12の外装ケース26の先端に対する、基端側振動伝達体24の先端面25の位置である。図14Aに示すように、この変形例では、基端側プローブユニット12の外装ケース26の先端よりも、基端側振動伝達体24の先端面25が中心軸C1に沿って先端側にある。
なお、基体56の先端側には、筒状のノブ連結部464が形成されている。
基端側プローブユニット12と先端側プローブユニット14とを取り付ける場合、例えば右手で基端側プローブユニット12を把持し、左手で先端側プローブユニット14を把持する。このとき、左手では、先端側プローブユニット14の筒状体468を把持するか、操作体472aが図14A中の実線位置にある状態で操作体472aとともに筒状体468を把持する。そして、図14Aに示すように、基端側プローブユニット12の先端と、先端側プローブユニット14の基端とを、中心軸C1,C2の軸周りに相対的に回動させて嵌合させる。このとき、操作体472aを実線位置に保持しておく。
図16Aから図16Fに示すように、この実施形態に係る処置具10は、基端側プローブユニット12と、先端側プローブユニット14と、基端側プローブユニット12及び先端側プローブユニット14を着脱する筒状のアダプタ616とを有する。
基体56の内周面は、先端側振動伝達体54のフランジ部54aを回り止めした状態に支持する支持部76を有する。基体56の外周面は、基端側から先端側に向かって、大外径部656aと、小外径部656bを有する。基体56の外周面には、アダプタ616の後述する嵌合部618dが嵌合される嵌合部662と、爪部618eを受け入れる爪部受部663が形成されている。嵌合部662は、大外径部656aと小外径部656bの境界位置にある。爪部受部663は小外径部656bに形成されている。なお、嵌合部662は環状に形成されている。一方、爪部受部663は、図16Fに示すように、中心軸C2に対して例えば120度おきに3つ形成されているなど、複数形成されている。
摺動面618aは、基端側プローブユニット12及び先端側プローブユニット14が図16Aに示す位置に配置された状態で、先端側プローブユニット14の先端側から、基体56及び基端側プローブユニット12の外装ケース26の大外径部626aの外周面に配置される。凹部618bは、摺動面618aがチャック部684の外側を通った後、チャック部684が径方向外方に延出された状態に復帰する。なお、凹部618bは、アダプタ616を基端側プローブユニット12及び先端側プローブユニット14に取り付ける際に用いられる先端側傾斜面619を有する。
アダプタ616の先端には、基体56の爪部受部663に嵌合される複数の爪部618eを有する。爪部618eは、中心軸C2のうち、先端側に向かって法線Nが規定される傾斜面618fを有する。
基端側プローブユニット12、先端側プローブユニット14及びアダプタ616を適宜の状態に取り付ける場合について説明する。
基端側プローブユニット12の外装ケース26の内側に配置された基端側振動伝達体24の中心軸C1と、先端側プローブユニット14の基体56及びシースユニット658の内側に配置された先端側振動伝達体54の中心軸C2とを一致させる。この状態で、基端側プローブユニット12の基端側振動伝達体24の突き当て部25と、先端側プローブユニット14の先端側振動伝達体54の突き当て部55とを突き当てる。そして、アダプタ616を図16Aに示す位置まで先端側プローブユニット14の基体56の外周面を通して、基端側プローブユニット12の外装ケース26の外周面まで移動させる。このとき、図16Aに示すように、チャック部684がチャック収納部634の外側にある。
例えば、基端側プローブユニット12又は先端側プローブユニット14とアダプタ616とを保持した状態で、中心軸C1,C2に沿って、互いに反対方向に力を加える。アダプタ616の爪部618eを先端側プローブユニット14の爪部受部663を乗り越えさせて、中心軸C2の先端側に移動させる。爪部618eの傾斜面618fは中心軸C2の先端側に向かって法線Nを規定しているので、一旦爪部受部663に対する係合が解除されると、先端側プローブユニット14に対して、アダプタ616を図16Aに示す位置に容易に移動させることができる。
図17A及び図18に示すように、この実施形態では、先端側プローブユニット14に回転ノブ58(図1A参照)が配置されているのではなく、基端側プローブユニット12に回転ノブ758が配置されている。
回転ノブ758の内周面には、カム溝34が形成されたカムリング32が配置されている。図17Bに示すように、カム溝34は、第1実施形態で説明したカム溝と同様に形成されている。
まず、基端側プローブユニット12と先端側プローブユニット14とを取り付ける場合について説明する。
図17Aに示すように、基端側プローブユニット12の基端側振動伝達体24の先端を、先端側プローブユニット14の先端側振動伝達体54の基端に突き当てる。このとき、カムピン84がカム溝34の開口34aに配置される。この状態で、例えば左手で基体56を把持し、右手で回転ノブ758を把持して、両者を付勢体86の付勢力に抗して中心軸C1,C2の軸周りに相対的に回動させる。このため、カムピン84が係合部34cに係合される。
処置部54dの向きを変える場合、右手で外装ケース26を把持して、左手で回転ノブ758を中心軸C1の軸周りに回動させる。回転ノブ758の回動により、先端側プローブユニット14の基体56及び先端側振動伝達体54が一緒に回動する。このため、外装ケース26超音波振動子ユニット30及び基端側振動伝達体24に対して処置部54dの向きが変えられる。
Claims (21)
- その基端部とその先端部とにより規定される第1中心軸に沿って延出され、超音波振動子からの超音波振動が前記第1中心軸に沿ってその基端部から先端部に向かって伝達される基端側プローブと、
その基端部とその先端部とにより規定される第2中心軸に沿って延出され、超音波振動を前記第2中心軸に沿ってその基端部から先端部に有する処置部に向かって伝達可能な先端側プローブと、
前記基端側プローブの前記先端部に前記先端側プローブの前記基端部を突き当てて前記超音波振動子からの振動を前記基端側プローブの前記基端部から前記先端側プローブの前記先端部に伝達可能な状態を保持するとともに、前記第1及び第2中心軸を一致させた状態を保持する保持部と、
前記保持部に設けられ、前記第1及び第2中心軸の軸回りに回転させる操作により、前記基端側プローブに対して相対的に前記先端側プローブを前記第1及び第2中心軸の軸回りに回転又は回動させ前記先端側プローブの先端部の前記処置部を所望の方向を向かせる操作部と
を具備する、処置具。 - 前記保持部は、前記基端側プローブの先端部と前記先端側プローブの基端部とを一定の力で押し付ける付勢体を有する、請求項1に記載の処置具。
- 前記保持部は、
前記基端側プローブをその内側に配置するカバーと、
前記カバーに対して着脱可能で、前記先端側プローブをその内側に保持する先端側保持部と
を有する、請求項1に記載の処置具。 - 前記操作部は、前記先端側保持部に対して、前記第2中心軸の軸周りに回転可能に連結されている、請求項3に記載の処置具。
- 前記操作部は、前記カバーに対して、前記第1中心軸の軸周りに回転可能に連結されている、請求項3に記載の処置具。
- 前記基端側プローブの前記先端部と前記先端側プローブの前記基端部とを突き当てながら前記カバー及び前記先端側保持部を互いに対して装着し、前記基端側プローブの前記先端部と前記先端側プローブの前記基端部とを突き当てを解除しながら前記カバー及び前記先端側保持部を互いに対して離脱させる着脱機構を有する、請求項3に記載の処置具。
- 前記着脱機構は、前記カバー及び前記先端側保持部を前記第1及び第2中心軸の軸回りに相対的に回転又は回動することにより互いに対して取り付け及び取り外し可能である、請求項6に記載の処置具。
- 前記着脱機構は、前記カバー及び前記先端側保持部の少なくとも一方の外周に配置された回転トルク付与部を有する、請求項7に記載の処置具。
- 前記回転トルク付与部は、外径が拡大及び縮小可能な可動部を有する、請求項8に記載の処置具。
- 前記回転トルク付与部は、前記先端側保持部に対して着脱可能である、請求項8に記載の処置具。
- 前記着脱機構は、軸方向への移動により前記カバー及び前記先端側保持部を着脱可能な機構を有する、請求項6に記載の処置具。
- 前記操作部及び前記保持部は、互いに対して前記第1及び第2中心軸の軸回りの回転を規制する規制部を有する、請求項1に記載の処置具。
- 前記規制部は、前記保持部に対して、選択的に互いに対して前記第1及び第2中心軸の軸回りの回転を規制し、規制を解除する、請求項12に記載の処置具。
- 前記基端側プローブの前記先端部と前記先端側プローブの前記基端部とが接触した状態にあるとき、その接触部は、前記超音波振動子から前記基端側プローブを通して前記先端側プローブの先端部まで振動が伝達されるときに、振動の腹位置又はその近傍である、請求項1に記載の処置具。
- 前記先端側プローブは、前記超音波振動子の超音波振動の半波長の整数倍の長さを有する、請求項1に記載の処置具。
- 前記基端側プローブの基端部には、超音波振動子が固定されている、請求項1に記載の処置具。
- 前記基端側プローブの基端部は、超音波プローブの先端部に連結されている、請求項1に記載の処置具。
- 第1中心軸を規定し超音波振動子からの超音波振動が前記第1中心軸に沿ってその基端部から先端部に向かって伝達される基端側プローブを内側に配置する基端側カバーに着脱可能な先端側保持部と、
前記先端側保持部に保持され、第2中心軸を規定し、超音波振動を前記第2中心軸に沿ってその基端部から先端部の処置部に向かって伝達可能であり、前記先端側保持部と前記基端側カバーとが装着されたときに、前記第1及び第2中心軸が一致した状態で前記基端側プローブの前記先端部がその基端部に突き当てられるとともに、前記基端側プローブの前記先端部がその基端部に突き当てられた状態で前記超音波振動子からの振動を前記基端側プローブの前記基端部からその先端部の処置部に伝達可能であるとともに、前記基端側プローブに対して相対的に前記第1及び第2中心軸の軸回りに回転又は回動可能でその先端部の処置部を所望の方向に向かせる先端側プローブと
を具備する、先端側プローブユニット。 - 前記先端側保持部は、前記第2中心軸の軸回りに回転させる操作により、前記基端側プローブに対して相対的に前記先端側プローブを前記先端側保持部の前記第2中心軸の軸回りに回転又は回動させる操作部を有する、請求項18に記載の先端側プローブユニット。
- 第2中心軸を規定し、超音波振動を前記第2中心軸に沿ってその基端部から先端部の処置部に向かって伝達可能な先端側プローブを保持する先端側保持部に着脱可能な基端側カバーと、
前記基端側カバーの内側に配置され、第1中心軸を規定し超音波振動子からの超音波振動が前記第1中心軸に沿ってその基端部から先端部に向かって伝達され、前記基端側カバーが前記先端側保持部に装着されたときに、前記第1及び第2中心軸が一致し、その先端部が前記先端側プローブの基端部に突き当てられるとともに、前記先端側プローブの前記基端部がその先端部に突き当てられた状態で前記超音波振動子からの振動をその基端部から前記先端側プローブの先端部の処置部に伝達可能であるとともに、前記先端側プローブに対して相対的に前記第1及び第2中心軸の軸回りに回転又は回動可能で前記先端側プローブの先端部の処置部を所望の方向を向かせる、基端側プローブと
を具備する、基端側プローブユニット。 - 前記基端側カバーに設けられ、前記第2中心軸の軸回りに回転させる操作により、前記先端側プローブに対して相対的に前記基端側プローブを前記基端側カバーの前記第1中心軸の軸回りに回転又は回動させる操作部をさらに具備する、請求項20に記載の基端側プローブユニット。
