Abstract
Description
このような輸液ポンプ等の医療機器等で使用される薬剤情報等については、その情報を管理するため「薬剤ライブラリ」等のデータベースが構築され、輸液ポンプ等を用いて薬剤等を適切に処方等することが可能となるシステムが採用されている。
このため、かかる薬剤については、使用履歴情報を収集し、定期的にその使用情報を解析する必要がある。
具体的には、薬剤の使用場所である手術室やICU(Intensive Care Unit、集中治療室)等毎に、薬剤等の使用情報を収集し解析等していた。
したがって、一定の効能程度情報を有する薬剤毎のより詳細な使用情報の解析等が可能となる。
このため、一定の効能程度情報を有する薬剤毎の医療機器における警告情報及び使用実績情報を得ることができる。
また、これら警告情報及び使用実勢情報は、色彩情報と共に表示部に表示されるので、利用者が視認し易く、理解し易い構成となっている。
例えば、当該利用者識別情報(例えば、ID番号)を有する病院職員が通常、医療事務を担当しているが、その業務上、通常関連のない、「薬剤ライブラリ」等の「第1の医療情報」の「薬剤ライブラリ解析」等の「第2の医療情報」にアクセスする必要が生じる場合がある。
このとき、前記構成では、例えば、当該利用者関連情報の病院職員は、通常「薬剤ライブラリ」という「第1の医療情報」と業務上関連がないため、この「薬剤ライブラリ」には、アクセスが不可となっている。
これにより、当該病院職員が自己の業務と関連のない「薬剤ライブラリ」等のデータベースを誤って変更等する可能性を排除し、システムが扱う医療情報の管理上の安全性を確保している。
そこで、前記構成では、第3の医療情報に基づき、当該第2の医療情報の程度識別情報を特定し、この特定された程度識別情報と、利用者アクセス関連情報の程度識別情報を比較して、当該アクセスの可否を判断する構成となっている。
ここで、第3の医療情報は、第2の医療情報と、当該第2の医療情報の程度を示す程度識別情報とを関連付ける情報となっている。
すなわち、「薬剤ライブラリ解析」等の「第2の医療情報」と、程度識別情報である例えば、「権限レベル6」とが関連付けられている。
また、利用者アクセス関連情報記憶部には、利用者識別情報に対応する程度識別情報(例えば、権限レベル6等)が関連付けて登録されている。
したがって、この特定された「権限レベル6」の程度識別情報と、利用者アクセス関連情報の程度識別情報(例えば、権限レベル6)を比較して、当該アクセスの可否を判断する構成となっている。
例えば、利用者アクセス関連情報の程度識別情報(例えば、権限レベル6)が、特定された「権限レベル6」の程度識別情報以上場合には、自動的に当該アクセスを可能にする構成となっている。
また、このアクセスは、無条件ではなく、アクセスしようとする「第2の医療情報」の「程度識別情報」と、当該病院職員等の利用者識別情報の「程度識別情報」とを比較して判断する。
したがって、医療情報の管理の安全性を確保しつつ、利用者の利便性を向上させることができる。
また、特定された程度識別情報の段階程度情報、例えば、権限レベル6が、利用者アクセス関連情報の程度識別情報の段階程度情報(例えば、権限レベル6)を超えないときは、当該利用者識別情報に基づくアクセス要求を、時期的限定(例えば、その1回限り)を条件に認める構成となっている。
したがって、病院職員等の利用者の本来の業務(予め定められている業務)と異なる「薬剤ライブラリ編集」等の「第2の医療情報」のアクセスを許可する場合でも、時間的に限定されるので、医療情報の管理上の安全性を確保することができる。
尚、以下に述べる実施の形態は、本発明の好適な具体例であるから、技術的に好ましい種々の限定が付されているが、本発明の範囲は、以下の説明において特に本発明を限定する旨の記載がない限り、これらの態様に限られるものではない。
図1に示すように、薬剤ライブラリ管理システム1は、医療情報管理装置である例えば、薬剤ライブラリサーバ10を有している。また、端末装置である例えば、病院職員用端末50A、50B及び50Cが薬剤ライブラリサーバ10と通信可能に接続されていると共に、病院内に配置されている薬液注入用医療機器である例えば、輸液ポンプ70a、70b及び70cも薬剤ライブラリサーバ10と通信可能に接続されている。
図2(a)は、図1の「薬剤ライブラリサーバ10」に格納されている「薬剤ライブラリ」のデータベースの内容の一部を示す概略図である。
「薬剤ライブラリ」は、薬剤の処方等に関する詳細なデータ(例えば、薬剤種類(抗ガン剤、麻酔剤)、薬剤名、流量(mL/h)の上限・下限値、注入量(mL)の上限・下限値、近畿情報など)を含むものであり、このデータは、主に図1の輸液ポンプ70a等に送信され、輸液ポンプ70a等を操作する看護師等の医療従事者が薬剤を患者に投与する際に活用されるデータとなる。
図2(a)に示す内容は、特に、薬剤ライブラリのデータベースに対する後述する処理内容やその権限に関する部分を示すものである。
なお、図1に示す輸液ポンプ70a等は、薬剤を患者に正確に投与等する場合に用いられる医療機器であって、薬剤の投与に高精度な制御が求められる場合等に点滴装置等に配置される。
「病院事務管理データ」は、病院の医療事務や経営用のデータ等を管理するデータである。
したがって、看護師等の医療従事者や病院の医療事務の職員、さらには病院の経営者等の病院用職員は、図1の「病院職員用端末50A」等を操作し、この「病院事務管理データ」にアクセスすることで、自己の業務に関するデータに円滑に利用することができる構成となっている。
したがって、図1の薬剤ライブラリサーバ10に格納されている「薬剤ライブラリ」のデータベースには、アクセス権限が定められ、当該データベースと関連のない病院職員等の当該データベースへのアクセスが制限されている。
例えば、医師、看護師及び薬剤師等の医療従事者等にのみ「薬剤ライブラリサーバ10」へのアクセス権限が付与され、他の医療事務の職員はアクセス権限が認められていない。
なお、図2(a)では、権限レベル2~6までの5つのレベルが示されているがこれに限られるものではない。
一方、図2(b)で示す「病院事務管理データ」のデータベース(図1では図示せず)は、医療事務の職員や経営担当職員にのみ、そのアクセス権限が与えられ、医師等の医療従事者等にはアクセス権限が与えられていない。
そして、これらの処理内容は、その程度によって実行可能な「程度識別情報」である例えば、「権限レベル」が定められている。そして、このような「権限レベル」を有する病院職員のみが、かかる処理を実行できる構成となっている。具体的には、後述するように、権限レベルは、病院職員の「利用者識別情報」である例えば、ID番号と紐付けられている。
なお、図2(b)では、権限レベル1~7までの7つのレベルが示されているがこれに限られるものではない。
図3に示すように、病院職員用端末50A等は、端末制御部51を有している。この端末制御部51は、データ等の入力を行う入力装置である例えば、「端末側入力装置52」、データ等の表示を行う表示部である例えば、「端末側ディスプレイ53」及び薬剤ライブラリサーバ10等との通信を行う「端末側通信装置54」を制御する他、「端末側各種情報記憶部55」も制御する構成となっている。
図4に示すように、輸液ポンプ70a等は、輸液ポンプ制御部71を有している。この輸液ポンプ制御部71は、データ等の入力を行うダイヤルや入力ボタン等の入力装置である例えば、「輸液ポンプ側入力装置72」、データ等の表示を行う表示部である例えば、「輸液ポンプ側ディスプレイ73」及び薬剤ライブラリサーバ10等との通信を行う「輸液ポンプ側通信装置74」及び輸液ポンプ70aのポンプ動作等を実行する「輸液ポンプ本体部75」を制御する他、「輸液ポンプ側各種情報記憶部76」も制御する構成となっている。
図5に示すように、薬剤ライブラリサーバ10は、「サーバ制御部11」を有している。このサーバ制御部11は、データ等の入力を行う「サーバ側入力装置12」、データ等の表示を行う「サーバ側ディスプレイ13」及び輸液ポンプ70a等や病院職員用端末50A等との通信を行う「サーバ側通信装置14」も制御する構成となっている。
また、図4に示すように、サーバ制御部11は、「第1記憶部20」、「第2記憶部30」及び「第3記憶部40」等を制御する構成となっている。
このアラートが発生する原因情報としては、例えば、薬剤の投与制限情報である「ソフトリミット超過」、「ソフトリミット後再設定」、ソフトリミットより厳しい、絶対的な投与制限情報である「ハードリミット抵触」、「薬剤キャンセル」、薬剤の投与の「流量異常」及び薬液の「残量」等の情報が挙げられる。
ST1で、輸液ポンプ70aが薬剤の送液を開始したと判断したときは、ST2へ進む。ST2では、輸液ポンプ70aは、送液を開始した「薬剤」の「薬剤名」情報、例えば、「薬剤A」を「薬剤ライブラリサーバ10」へ送信する。具体的には、図4の「輸液ポンプ側通信装置74」を介して送信する。
この「薬剤カラー」が、「効能程度範囲情報」及び「色彩情報」の一例である。また、「当該薬剤Aの薬剤ライブラリ」が、「個別薬剤情報」の一例である。
図13は、図2の薬剤ライブラリデータベース内に記憶されている個々の薬剤の薬剤ライブラリを示す概略図である。
図13に示すように、個々の薬剤(例えば、薬剤名「A」)の薬剤ライブラリには、薬剤名以外に「投与量」や「閉塞圧」等の情報が記録されるが、本実施の形態では、さらに「薬剤カラー」の情報も記録される。
この薬剤カラーの情報は、当該薬剤の投与時の危険度(効能程度である例えば、効能や効用が強いため、その投与に注意を要する程度等)に従って、色彩を違えて示している。
すなわち、薬害の効能効用等の危険度が高い順に薬剤をグループに分類し、グループ毎に色彩情報と関連付けている。
例えば、危険度が高いグループから順に、「赤」、「紫紅」、「黄緑」、「黄色」、「青緑」、「紫」、「緑」、「青」、「えび茶」、「オリーブ」及び「濃紺」としている。
したがって、利用者がディスプレイ等に表示されたこれらの色彩を視認することで、当該薬剤の危険度が高いか否かを迅速に認識することができる構成となっている。
また、これらの工程は,図6の「薬剤使用回数情報処理部(プログラム)22」が動作して実行される。
また、「個別薬剤使用履歴情報21a」が、「使用履歴情報」の一例となっている。
ST4で、輸液ポンプ70aが「アラート」信号を発したときは、輸液ポンプ70aは、アラート信号を発したときの送液中の「薬剤」の「薬剤名」情報、例えば、薬剤A及び「アラート原因(ソフトリミット超過、ソフトリミット後再設定、ハードリミット抵触、薬剤キャンセル、流量異常、残量等の情報のいずれか等)を「薬剤ライブラリサーバ10」へ送信する。
これらの工程は、図6の「アラート回数情報処理部(プログラム)23」が実行する。
以上で、図6の「個別薬剤使用履歴情報21a」には、薬剤名A等と関連付けて、「累積アラート回数/月」とその「アラート原因」、及び「累積使用回数/月」が記憶されると共に、当該薬剤の危険度を示す「色彩情報」も関連付けて記憶されることになる。
したがって、図1の薬剤ライブラリサーバ10にアクセスしようとする病院職員は、図1に示す病院職員用端末50Aを用いて、ST11に示すように、自己の病院職員用端末50Aに、自己のID番号及びパスワード等を入力する。
したがって、この「病院職員情報記憶部24」に記憶されているか否かを判断することで、入力者が登録済みか否かを判断でき、未登録者の「薬剤ライブラリサーバ10」へのアクセスを未然に防止することができる。
この動作は、具体的には、図6の「入力情報判断処理部(プログラム)25」が実行する。
ST14では、入力されたID番号を図7の「当該入力職員ID情報記憶部31」に記憶させる。
例えば、処理内容として「薬剤ライブラリ解析(例えば、「色別薬剤アラート回数等グラフ」及び「色別薬剤使用率グラフ」等を作成)」(図2(a)参照)と入力する
具体的には、図7の「アクセス権限判断処理部(プログラム)32」が動作し、図7の「当該入力職員ID情報記憶部31」と図6の「病院職員情報記憶部24」を参照し、当該ID番号の「アクセス可能なデータベース」に「薬剤ライブラリデータベース」が含まれているか否かを判断する。
一方、ST16で一致しない場合は、後述する。
具体的には、「色別薬剤アラート回数等グラフ作成」及び「色別薬剤使用率グラフ作成」のいずれか、又は双方を選択するための選択画面が「端末側ディスプレイ53」に表示される。
すなわち、「色別薬剤アラート回数等グラフ」は、上述した「薬剤カラー」情報が同じ薬剤の使用で発生したアラート回数情報等を、当該薬剤カラー毎に表示するグラフである。したがって、薬剤カラー毎のアラートの発生回数等を容易に解析することができる。
一方、「色別薬剤使用率グラフ」は、上述した「薬剤カラー」情報が同じ薬剤の使用率等を、当該薬剤カラー毎に表示するグラフである。したがって、薬剤カラー毎の使用率等を容易に解析することができる。
一方、かかる医療事務管理の担当者は,図2(b)のデータベースである「病院事務管理データ」にアクセス可能とし、その利便性を確保している。
しかし、医療事務管理の担当者であっても、経営に関する担当者で且つ医師でもある担当者等も存在し、かかる担当者の場合は、その業務上、本来の業務とは直接関係のないデータベースである例えば、「薬剤ライブラリデータベース」へのアクセスが必要となる場合がある。
この場合でも、一律、データベースへのアクセスが拒否され、その都度、アクセス権限の登録を強いられると、業務の遂行に支障を来すため、本実施の形態では、以下のような処理を行う。
具体的には、「アクセス権限判断処理部(プログラム)32」が判断する。
この場合、当該ID番号の病院職員が,「薬剤ライブラリデータベース」へアクセスし、「薬剤ライブラリ解析」の処理を実行するには、管理者等に申請し、図6の「病院職員情報記憶部24」の記憶を変更させることが必要となる。これにより、必要性の低い病院職員の例外的なアクセス等を制限することができ、医療情報の管理上の安全性を確保することができる。
ST18では、上述のように、当該ID番号を入力した病院職員は、当該処理内容である「薬剤ライブラリ解析」を実行する権限があるとし、当該処理内容である「薬剤ライブラリ解析」が実行される。
具体的には、「色別薬剤アラート回数等グラフ作成」及び「色別薬剤使用率グラフ作成」のいずれか、又は双方を選択するための選択画面が端末側ディスプレイ53に表示される。
ただし、この実行許可は、その都度、許可の判断工程を経るため、1回毎に許可が必要な構成となっている。この「1回毎の許可」が、「時期的限定」の一例となっている。
また、このように例外的にアクセスを認めるときは、処理内容の権限レベルの段階程度情報である数値が同等若しくはそれ以上の場合に限り認める構成となっているため、医療情報の管理上の安全性を確保しつつ、利用者の利便性を向上させる構成となっている。
ST20で、「色別薬剤アラート回数等グラフ作成」が選択されたときは、ST21へ進む。ST21では、図7の「色別薬剤アラート回数等グラフ作成処理部(プログラム)35」が動作し、図6の「個別薬剤使用履歴情報21a」の例えば、薬剤Aと同一色彩が付された薬剤の「累積アラート回数/月」及び「アラート原因」の情報を抽出する。
そして、図14に示すような「色別薬剤アラート回数等グラフ41a」を作成し、図8の「色別薬剤アラート回数等グラフ記憶部41」に記憶させる。
図14に示すように「色別薬剤アラート回数グラフ41a」は、赤等の同じ色彩情報が関連付けられた薬剤のグループ毎に、例えば、月単位の輸液ポンプ70a等におけるアラートの発生回数が示されている。
また、このグラフは、アラートの発生原因(ソフトリミット超過等)毎の発生回数も把握することができるように作成される。
したがって、このようなグラフを視認した利用者は、その薬剤の危険度が色彩情報で迅速に把握できると共に、そのような同じ色彩の危険度のグループ内の薬剤がどのような原因でどの程度アラートを発生させているかを容易に把握でき、この結果、解析し易い構成となっている。
ST22で、「色別薬剤使用率グラフ作成」が選択されたときは、ST23へ進む。ST23では、図8の「色別薬剤使用率グラフ作成処理部(プログラム)42」が動作し、図6の「個別薬剤使用履歴情報21a」の同一色彩が付された薬剤の「累積使用回数/月」の情報を抽出する。
そして、図15に示すような、色毎の「色別薬剤使用率グラフ43a」を作成し、図8の「色別薬剤使用率グラフ記憶部43」に記憶させる。
図15に示すように「色別薬剤使用率グラフ43a」は、赤等の同じ色彩情報が関連付けられた薬剤のグループ毎に、例えば、月単位の輸液ポンプ70a等における使用率(%)が示されている。
したがって、このようなグラフを視認した利用者は、その薬剤の危険度が色彩情報で迅速に把握できると共に、そのような同じ色彩の危険度のグループ内の薬剤がどのような使用率(%)であるかを容易に把握でき、この結果、解析し易い構成となっている。
したがって、上述のように、これらのグラフを視認した利用者は、その薬剤の危険度に応じたアラート回数や使用率等を容易に解析することができると共に、薬剤の使用情報の詳細な解析等が可能となる。
Claims (7)
- 各薬剤の効能程度を複数の効能程度範囲情報で区分した効能程度範囲情報を各薬剤の個別薬剤情報と関連付けて記憶し、
医療機器で使用される前記薬剤の使用履歴情報を、前記効能程度情報毎に生成することを特徴とする医療情報管理装置。 - 前記効能程度範囲情報が、色彩情報であり、
前記使用履歴情報は、前記薬剤が前記医療機器で使用されたときに発生した警告情報と、前記薬剤の使用実績情報を含み、これら警告情報及び使用実勢情報は、前記色彩情報と共に表示部に表示されることを特徴とする請求項1に記載の医療情報管理装置。 - 前記警告情報が、その警告情報の原因情報と関連付けて生成されることを特徴とする請求項2に記載の医療情報管理装置。
- 医療情報を利用する利用者を識別する利用者識別情報と、
前記医療情報を、複数の種類に区分した複数の第1の医療情報と、
前記第1の医療情報を、さらに複数の種類に区分した複数の第2の医療情報と、
前記第2の医療情報と、当該前記第2の医療情報の程度を示す程度識別情報とを関連付ける第3の医療情報を記憶する第3の医療情報記憶部と、
前記利用者識別情報と、少なくとも当該前記利用者識別情報が、アクセスが可能な前記第1の医療情報及び当該前記利用者識別情報に対応する前記程度識別情報を関連付けて登録する利用者アクセス関連情報を記憶する利用者アクセス関連情報記憶部と、を有し、
前記利用者識別情報に基づいて、アクセス要求された前記第2の医療情報の属する前記第1の医療情報が、前記利用者アクセス関連情報に基づいて、アクセス不可と判断されたとき、前記第3の医療情報に基づき、当該前記第2の医療情報の前記程度識別情報を特定し、この特定された前記程度識別情報と、前記利用者アクセス関連情報の前記程度識別情報を比較して、当該アクセスの可否を判断する構成となっていることを特徴とする請求項1乃至請求項3のいずれか1項に記載の医療情報管理装置。 - 前記第2の医療情報の前記程度識別情報は、各前記第1の医療情報に共通の基準である段階程度情報を有し、
前記特定された前記程度識別情報の前記段階程度情報が、前記利用者アクセス関連情報の前記程度識別情報の前記段階程度情報を超えないときは、当該利用者識別情報に基づくアクセス要求を、時期的限定を条件に認める構成となっていることを特徴とする請求項4に記載の医療情報管理装置。 - 前記医療機器と
利用者が利用する端末装置を有し、
前記医療情報管理装置は、前記医療機器及び前記端末装置に通信可能に接続されていることを特徴とする請求項1乃至請求項5のいずれか1項に記載の医療情報管理装置を備える医療情報管理システム。 - 各薬剤の効能程度を複数の効能程度範囲情報で区分した効能程度範囲情報を各薬剤の個別薬剤情報と関連付けて記憶し、
医療機器で使用される前記薬剤の使用履歴情報を、前記効能程度情報毎に生成することを特徴とする医療情報管理方法。
