Die vorliegende Erfindung betrifft eine erfindungsgemäße pharmazeutische Zusammensetzung mit 14 bis 17,5 mg Leflunomid hergerichtet als Einzeldosis und deren Verwendungen.
Description
PHARMAZEUTISCHE ZUSAMMENSETZUNG UMFASSEND LEFLUNOMID
Technisches Feld:
Die vorliegende Erfindung betrifft eine erfindungsgemäße pharmazeutische Zusammensetzung mit 14 bis 17,5 mg Leflunomid hergerichtet als Einzeldosis und deren Verwendungen.
Stand der Technik:
Der pharmazeutische Wirkstoff Leflunomid (Formel I) mit der chemischen Bezeichnung 5-Methyl- N-[4-(trifluormethyl)-phenyl]-4-isoxazolcarboxamid wird seit geraumer Zeit als ein krankheitsmodifi- zierendes Antirheumatikum (auch abgekürzt: DMARDs = disease modifying antirheumatic drugs) aus der Gruppe der Isoxazole verwendet. Neben der Anwendung als Antirheumatikum vorrangig zur Behandlung der rheumatoiden Arthritis und/oder Psoriasis Arthritis wird die Wirkung von Leflunomid derzeit auch bei der Behandlung von weiteren Autoimmunerkrankungen, insbesondere von Lupus Nephritis, von systemischem Lupus Erythematodes, von Uveitis, von Myasthenia Gravis und/oder von Granulomatose mit Polyangitis (Morbus Wegener); im Rahmen von festen Organtransplantationen, insbesondere bei Transplantationen von Niere und Leber; von onkologischen Erkrankungen, insbesondere von Melanomen, Sarkomen, Gliomen, Prostata-Karzinomen, Hirn- und/oder ZNS-Tumoren; und/oder von Erkrankungen mit dem Humanen Immundefizienz-Virus (HIV) erforscht.
Leflunomid selbst stellt ein Prodrug dar, dass nach oraler Verabreichung in der Darmwand, aber auch in der Leber durch Öffnung des Isoxazolrings zu dem aktiven Malonnitrilamidmetaboliten und

Wirkstoff Teriflunomid (Formel II, auch genannt A77 1726) metabolisiert wird, welches immunmodulierende Eigenschaften besitzt. Lefiunomidkonzentrationen sind nach oraler Verabreichung in vivo nur in sehr geringen Konzentrationen nachweisbar. Die Wirkweise von Leflunomid beruht deshalb maßgeblich auf der in vivo Wirkung des aktiven Metaboliten Teriflunomid, das nach derzeitigem Kenntnisstand einerseits die Dihydroorotat-Dehydrogenase inhibiert und andererseits antiproliferative und/oder antiinflammatorische Wirkungen aufweist.
Die maximale Konzentration von Teriflunomid im Blut kann ca. 6 bis 12 Stunden nach oraler Verabreichung gemessen werden. Aufgrund der verhältnismäßig langen Halbwertszeit von Teriflunomid (ca. 2 Wochen) können steady-state Blutkonzentrationen bei einer einmal täglichen Verabreichung von 10 mg oder 20 mg Leflunomid nach ca. 2 Monaten gemessen werden. Da eine verlässliche Aussage über die individuelle Wirksamkeit von Leflunomid/Teriflunomid und einer möglichen Dosiserhöhung bzw. -Senkung erst nach Erreichen der steady-state Konzentration getroffen werden kann, gab es Bestrebungen den Zeitraum bis zum Eintritt der steady-state Konzentration zu verkürzen.
Durch orale Verabreichung von 100 mg Leflunomid einmal täglich für 3 Tage (Aufsättigungsdosis) und anschließender Verabreichung von 20 mg bzw. 10 mg Leflunomid einmal täglich (Erhaltungsdosis) konnte der Zeitraum bis zum Erreichen der steady-state Konzentration von ca. 2 Monaten auf ca. 2 Wochen reduziert werden, wobei eine therapeutische Wirkung normalerweise nach ca. 4 bis 6 Wochen zu erwarten ist und sich während der nächsten 4 bis 6 Monate noch steigern kann.
Gemäß Fachinformation von Arava®, einem kommerziell erhältlichen pharmazeutischen Präparat von Leflunomidtabletten mit 10 mg, 20 mg und 100 mg Leflunomid, wird deshalb empfohlen, die Behandlung von aktiver rheumatoider Arthritis bzw. aktiver Psoriasis Arthritis jeweils mit einer Aufsättigungsdosis von 100 mg Leflunomid einmal täglich über 3 Tage zu beginnen. Im Falle der Behandlung einer aktiven rheumatoiden Arthritis soll anschließend eine Erhaltungsdosis von 10 bis 20 mg Leflunomid je nach Schwere (Aktivität) der Erkrankung einmal täglich verabreicht werden; bei der Behandlung von aktiver Psoriasis Arthritis soll anschließend eine Erhaltungsdosis von 20 mg Leflunomid einmal täglich verabreicht werden. Bei einem entsprechendem Dosierungsregime können allerdings Nebenwirkungen beobachtet, wobei folgende Nebenwirkungen häufig beobachtet werden: Blutdruckerhöhung, Leukopenie, Parästhesie, Kopfschmerzen, Schwindel, gastrointes- tinale Beschwerden, wie beispielsweise Durchfall, Übelkeit, Erbrechen, Erkrankungen der Mundschleimhaut, und/oder Bauchschmerzen, verstärkter Haarausfall, Ekzem, Hautausschlag, Pruritis, trockene Haut, Sehnenscheidenentzündung, Creatininkinase (CK) Erhöhung, Appetitlosigkeit, Gewichtsverlust, Asthenie, leichte allergische Reaktionen und erhöhte Leberwerte (Transaminasen, Gamma-GT, alkalische Phosphatasen, Bilirubin). Aufgrund ein oder mehrerer dieser Nebenwirkungen insbesondere mit hämatotoxischer und/oder hepatotoxischer und/oder gastrointestinaler
Auswirkung kann es insbesondere innerhalb von 12 Monaten nach Therapiestart zu einem Therapieabbruch bzw. zu einem Wechsel des Therapeutikums kommen.
Darüber hinaus diskutieren einige Publikationen den Einfluss von Entzündungsreaktion, wie sie beispielsweise bei Rheutmatoider Arthritis (RA) vorliegen, auf die Pharmakokinetik von Arzneistoffen. Beispielsweise werden metabolisierende Enzyme, Transporterproteine und/oder Plasma- Bindeproteine genannt, die durch Entzündungsreaktionen beeinflusst werden können. Bei chronischen Entzündungsreaktionen kann das metabolische System der Leber von Produkten der Entzündungsreaktionen überladen und/oder gesättigt sein, so dass dies den Metabolismus eines Arzneistoffes verändern kann und beispielsweise in einer Reduktion der Arzneistoffmetabolisierung und in einer Erhöhung des Arzneistoff-Plasmaspiegels resultieren kann.
Es ist nun die Aufgabe der vorliegenden Erfindung eine pharmazeutische Zusammensetzung umfassend Leflunomid bereitzustellen, welche gegenüber einer Verabreichung von 20 mg bzw. 10 mg Leflunomid einmal täglich
• eine vergleichbare oder verbesserte Wirksamkeit bei der Prophylaxe und/oder Behandlung von Autoimmunerkrankungen, bevorzugt von rheumatoider Arthritis, von Psoriasis Arthritis, von Arthritis als Komplikation zystischer Fibrose, von Lupus Nephritis, von systemischem Lupus Erythematodes, von Uveitis, von Myasthenia Gravis und/oder von Granulomatose mit Polyangitis (Morbus Wegener); und/oder bei der Prophylaxe und/oder Behandlung im Rahmen einer Organtransplantation, bevorzugt von Nierentransplantation und/oder von Lebertransplantation; und/oder bei der Prophylaxe und/oder Behandlung von onkologischen Erkrankungen, bevorzugt Melanomen, Sarkomen, Gliomen, Prostata-Karzinomen, Hirn- und/oder ZNS-Tumoren; und/oder bei der Prophylaxe und/oder Behandlung von Erkrankungen mit dem Humanen Immundefizienz-Virus (HIV) zeigt und/oder
• eine Reduzierung ein oder mehrerer der vorgenannten Nebenwirkungen, bevorzugt ein oder mehrere hämatotoxische und/oder hepatotoxische und/oder gastrointestinale Nebenwirkungen ermöglicht und/oder
• eine Reduzierung an Leflunomid-Therapieabbrüchen bzw. Wechsel zu anderen Therapeutika ermöglicht.
Kurze Beschreibung der Erfindung:
Ein oder mehrere der vorgenannten Vorteile der erfindungsgemäßen Aufgabe werden anhand der erfindungsgemäß beanspruchten Gegenstände gelöst. Vorteilhafte Ausgestaltungen werden in den abhängigen Ansprüchen sowie in der nachfolgenden Beschreibung dargestellt.
Dementsprechend betrifft ein erster Erfindungsgegenstand eine pharmazeutische Zusammensetzung umfassend Leflunomid, dadurch gekennzeichnet, dass die Zusammensetzung als Einzeldosis mit 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid hergerichtet ist.
Ein zweiter Erfindungsgegenstand betrifft die Verwendung von Leflunomid zur Prophylaxe und/oder Behandlung einer Autoimmunerkrankung, bevorzugt von rheumatoider Arthritis, von Psoriasis Arthritis, von Arthritis als Komplikation zystischer Fibrose, von Lupus Nephritis, von systemischem Lupus Erythematodes, von Uveitis, von Myasthenia Gravis und/oder von Granulomatose mit Polyangitis (Morbus Wegener); und/oder bei der Prophylaxe und/oder Behandlung im Rahmen einer Organtransplantation, bevorzugt Nierentransplantation und/oder Lebertransplantation; und/oder bei der Prophylaxe und/oder Behandlung von onkologischen Erkrankungen, bevorzugt Melanomen, Sarkomen, Gliomen, Prostata-Karzinomen, Hirn- und/oder ZNS-Tumoren; und/oder bei der Prophylaxe und/oder Behandlung von Erkrankungen mit dem Humanen Immun- defizienz-Virus (HIV) dadurch gekennzeichnet, dass Leflunomid als Einzeldosis einer pharmazeutische Zusammensetzung mit 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid hergerichtet oder herrichtbar ist und einmal, zweimal, dreimal oder viermal täglich verabreicht wird.
Die vorstehend beschriebenen Erfindungsgegenstände können, sofern aus Sicht eines Fachmannes sinnvoll, jede mögliche Kombination der bevorzugten erfindungsgemäßen Ausgestaltungen, die nachfolgend und insbesondere auch in den abhängigen Ansprüchen offenbart werden, aufweisen.
Detaillierte Beschreibung der Erfindung:
Die Erfinder der vorliegenden Erfindung haben überraschend herausgefunden, dass eine erfindungsgemäße pharmazeutische Zusammensetzung aufgrund der Einzeldosierung von 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid pro pharmazeutischer Formulierung und einer einmal, zweimal, dreimal oder viermal täglichen (simultanen oder sequentiellen) Verabreichung bei der Prophylaxe und/oder Behandlung von Autoimmunerkrankungen, bevorzugt von rheumatoider Arthritis, von Psoriasis Arthritis, von Arthritis als Komplikation zystischer Fibrose, von Lupus Nephritis, von systemischem Lupus Erythematodes, von Uveitis, von
Myasthenia Gravis und/oder von Granulomatose mit Polyangitis (Morbus Wegener); und/oder bei der Prophylaxe und/oder Behandlung im Rahmen einer Organtransplantation, bevorzugt Nierentransplantation und/oder Lebertransplantation; und/oder bei der Prophylaxe und/oder Behandlung von onkologischen Erkrankungen, bevorzugt Melanomen, Sarkomen, Gliomen, Prostata- Karzinomen, Hirn- und/oder ZNS-Tumoren; und/oder bei der Prophylaxe und/oder Behandlung von Erkrankungen mit dem Humanen Immundefizienz-Virus (HIV)
• eine vergleichbare oder verbesserte Wirksamkeit gegenüber der Standardtherapie mit 20 mg oder 10 mg Leflunomid hergerichtet als Einzeldosierung erzielt und/oder
• eine Reduzierung ein oder mehrerer der vorgenannten Nebenwirkungen, insbesondere Blutdruckerhöhung; hämatotoxischer Nebenwirkungen, wie beispielsweise Leukopenie; Paräs- thesie; Kopfschmerzen; Schwindel; gastrointestinaler Nebenwirkungen, wie beispielsweise Durchfall, Übelkeit, Erbrechen, Erkrankungen der Mundschleimhaut, und/oder Bauchschmerzen; verstärkter Haarausfall; dermatologischer Nebenwirkungen, wie beispielsweise Ekzem, Hautausschlag, Pruritis und/oder trockene Haut; Sehnenscheidenentzündung; Creatininkinase (CK) Erhöhung; Appetitlosigkeit; Gewichtsverlust; Asthenie; leichte allergische Reaktionen und/oder hepatotoxische Nebenwirkungen bevorzugt gekennzeichnet durch erhöhte Leberwerte (Transaminasen, Gamma-GT, alkalische Phosphatasen, und/oder Bilirubin), weiter bevorzugt eine Reduzierung ein oder mehrerer hämatotoxischer und/oder hepatotoxischer und/oder gastrointestinaler Nebenwirkungen erzielt und/oder
• eine Reduzierung an Leflunomid-Therapieabbrüchen bzw. Wechsel zu anderen Therapeutika ermöglicht.
Die Verabreichung der erfindungsgemäßen pharmazeutischen Zusammensetzung kann somit auch zu einer verbesserten Patientencompliance führen.
Obwohl die zugrundeliegenden Mechanismen hierzu derzeit noch nicht vollständig geklärt sind, sind die vorgenannten vorteilhaften Eigenschaften der erfindungsgemäßen Gegenstände insofern überraschend für einen Fachmann, da dieser bisher von einer linearen Dosis-Wirkungsbeziehung insbesondere bei den Autoimmunerkrankungen, wie bei rheumatoider Arthritis, bei Psoriasis Arthritis, bei Arthritis als Komplikation zystischer Fibrose, bei Lupus Nephritis, bei systemischem Lupus Erythematodes, bei Uveitis, bei Myasthenia Gravis und/oder bei Granulomatose mit Polyangitis (Morbus Wegener), ausgegangen ist, d.h. das eine höhere Leflunomiddosis mit einer höheren Wirksamkeit korreliert und somit bei einer Dosisreduktion von 20 mg auf 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid mit einer Reduktion der Wirksamkeit zu rechnen gewesen wäre.
Erfindungsgemäße pharmazeutische Zusammensetzungen, die 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid als Einzeldosis enthalten und einmal täglich verabreicht werden, zeigen überraschenderweise allerdings eine gleiche bzw. verbesserte Wirksamkeit insbesondere bei der Prophylaxe und/oder Behandlung von Autoimmunerkrankungen, bevorzugt von rheumatoider Arthritis, von Psoriasis Arthritis, von Arthritis als Komplikation zystischer Fibrose, von Lupus Nephritis, von systemischem Lupus Erythematodes, von Uveitis, von Myasthenia Gravis und/oder von Granulomatose mit Polyangitis (Morbus Wegener) gegenüber pharmazeutischen Zusammensetzungen, die 20 mg Leflunomid als Einzeldosis enthalten und einmal täglich verabreicht werden.
Erfindungsgemäße pharmazeutische Zusammensetzungen, die 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid als Einzeldosis enthalten und je nach Schwere der Krankheit einmal, zweimal, dreimal oder viermal täglich verabreicht werden, zeigen darüber hinaus überraschenderweise eine gleiche bzw. verbesserte Wirksamkeit gegenüber den derzeit bekannten Therapievorschlägen insbesondere bei der Prophylaxe und/oder Behandlung im Rahmen einer Organtransplantation, bevorzugt Nierentransplantation oder Lebertransplantation; und/oder von onkologischen Erkrankungen, bevorzugt Melanomen, Sarkomen, Gliomen, Prostata- Karzinomen, Hirn- und/oder ZNS-Tumoren; und/oder von Erkrankungen mit dem Humanen Im- mundefizienz-Virus (HIV).
Kumulativ oder alternativ zu den vorgenannten Vorteilen kann beispielsweise bei der Prophylaxe und/oder Behandlung von Autoimmunerkrankungen, bevorzugt von rheumatoider Arthritis, von Psoriasis Arthritis, von Arthritis als Komplikation zystischer Fibrose, von Lupus Nephritis, von systemischem Lupus Erythematodes, von Uveitis, von Myasthenia Gravis und/oder von Granulomatose mit Polyangitis (Morbus Wegener) die Anzahl an vorgenannten Nebenwirkungen, insbesondere hämatotoxischer und/oder hepatotoxischer und/oder gastrointestinaler Nebenwirkungen und/oder die Schwere der Nebenwirkungen durch die einmal tägliche Verabreichung einer erfindungsgemäßen pharmazeutischen Einzeldosierung von 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid gegenüber einer Einzeldosierung von 20 mg bzw. 10 mg Leflunomid gesenkt werden, wodurch auch die Anzahl an Therapieabbrüchen bzw. Therapiewechsel reduziert werden kann. Die Reduktion der Nebenwirkungen und/oder der Schwere der Nebenwirkungen ist umso erstaunlicher, als im Stand der Technik gezeigt wurde, dass die einmal täglich verabreichte 10 mg Leflunomid-Einzeldosierung gegenüber der 20 mg Leflunomid-Einzeldosierung zu einem signifikanten Anstieg an (schweren) Nebenwirkungen und damit auch zu einem Anstieg an Therapieabbrüchen führte (Poor, G„Efficacy and safety of leflunomide 10 mg versus 20 mg once daily in patients with active rheumatoid arthritis: multinational double-blind, randomized trial" Rheumatology 2004; 43: 744-749).
Überraschenderweise kann zudem beobachtet werden, dass eine erfindungsgemäße pharmazeutische Zusammensetzung enthaltend 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid-Einzeldosierung bei der Prophylaxe und/oder Behandlung von Autoimmunerkrankungen, bevorzugt von rheumatoider Arthritis, von Psoriasis Arthritis, von Arthritis als Komplikation zystischer Fibrose, von Lupus Nephritis, von systemischem Lupus Erythematodes, von Uveitis, von Myasthenia Gravis und/oder von Granulomatose mit Polyangitis (Morbus Wegener); und/oder im Rahmen einer Organtransplantation, bevorzugt Nierentransplantation und/oder Lebertransplantation; und/oder von onkologischen Erkrankungen, bevorzugt Melanomen, Sarkomen, Gliomen, Prostata-Karzinomen, Hirn- und/oder ZNS-Tumoren; und/oder von Erkrankungen mit dem Humanen Immundefizienz-Virus (HIV) gegenüber der Einzeldosierung mit 20 mg und/oder 10 mg Leflunomid zu einer schnelleren Einstellung der steady-state Konzentration führen kann, so dass eine Aussage über eine therapeutische Wirksamkeit früher getroffen werden kann und somit - sofern notwendig - Dosierungsanpassungen, d.h. Dosisreduktionen bzw. -erhöhungen früher vorgenommen und damit Unterdosierungen und/oder Nebenwirkungen vermieden werden können.
Die Bereitstellung einer erfindungsgemäßen pharmazeutischen Zusammensetzung enthalten 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid-Einzeldosierung kann somit eine (verbesserte) personalisierte Therapie bei der Prophylaxe und/oder Behandlung von Autoimmunerkrankungen, bevorzugt von rheumatoider Arthritis, von Psoriasis Arthritis, von Arthritis als Komplikation zystischer Fibrose, von Lupus Nephritis, von systemischem Lupus Erythematodes, von Uveitis, von Myasthenia Gravis und/oder von Granulomatose mit Polyangitis (Morbus Wegener); und/oder im Rahmen einer Organtransplantation, bevorzugt Nierentransplantation oder Lebertransplantation; und/oder von onkologischen Erkrankungen, bevorzugt Melanomen, Sarkomen, Gliomen, Prostata-Karzinomen, Hirn- und/oder ZNS-Tumoren; und/oder von Erkrankungen mit dem Humanen Immundefizienz-Virus (HIV) ermöglichen.
Die Verabreichung einer erfindungsgemäßen pharmazeutischen Zusammensetzung enthaltend 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid-Einzeldosierung gemäß erstem Erfindungsgegenstand kann zudem bei bestimmten Patientenpopulationen gegenüber der handelsüblichen Einzeldosierungen von 20 mg Leflunomid dann vorteilhaft sein, wenn ein individuell unvorteilhaftes Risiko-Nutzen-Profil aufgrund einer Nebenwirkung bzw. Toxizität von Leflunomid besteht. Ein entsprechend unvorteilhaftes Risiko-Nutzen-Profil kann vorzugsweise bei der Behandlung von rheumatoider Arthritis und insbesondere in einem Therapieregime mit ein oder mehreren weiteren Autoimmun-Wirkstoffen beispielsweise auf einem Polymorphismus von Genen beruhen, welche Proteine kodieren, die insbesondere in die Verstoffwechslung (Metaboli- sierung), vorzugsweise in den Transport bzw. die Aufnahme aus dem Magen-Darm-Trakt, die Verteilung im Körper (plasma level) bzw. die Ausscheidung (beispielweise über die Galle) von
Leflunomid und/oder seiner Metabolite, vorzugsweise seinem aktiven Metaboliten Teriflunomid eingreifen.
In einer bevorzugten erfindungsgemäßen Ausgestaltung wird die erfindungsgemäße Zusammensetzung des ersten erfindungsgemäßen Gegenstandes an Patienten, vorzugsweise Menschen verabreicht, die einen Polymorphismus an Cytochrom P 450 2C19 (CYP2C19) Enzym aufweisen und insbesondere schlechte bis intermediäre Metabolisierer darstellen (siehe Wiese ei al.: Polymorphisms in cytochrome P450 2C19 enzyme and cessation of leflunomide in patients with rheumatoid arthritis. Arthritis Research & Therapy 2012 14:R163). Schlechte CYP2C19- Metabolisierer kennzeichnen sich gemäß Wiese et al. üblicherweise dadurch, dass sie 2 Allele mit Funktionsverlust aufweisen, während intermediäre Metabolisierer 1 Allel mit Funktionsverlust und ein Wildtyp-Allel aufweisen. Patienten, die schlechte bzw. intermediäre CYP2C19-Metabolisierer sind, zeigen ein schlechtes Risiko-Nutzen-Profil hinsichtlich der Toxizität von Leflunomid und brechen mit einer höheren Wahrscheinlichkeit die Leflunomid-Therapie insbesondere bei rheumatoider Arthritis und insbesondere in einem Therapieregime mit weiteren Autoimmun-Medikamenten ab. Eine Verabreichung einer erfindungsgemäßen Zusammensetzung enthaltend 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid-Einzeldosierung gemäß erstem Erfindungsgegenstand, insbesondere bei der Behandlung von rheumatoider Arthritis kann aufgrund des reduzierten Leflunomidgehaltes somit zu einer Reduzierung der Toxizität und damit zu einer Reduzierung des Therapieabbruchs von Leflunomid insbesondere in Verabreichung mit weiteren Autoimmun-Medikamenten führen.
In einer weiteren kumulativen oder alternativen bevorzugten erfindungsgemäßen Ausgestaltung wird die erfindungsgemäße Zusammensetzung des ersten erfindungsgemäßen Gegenstandes an Patienten, vorzugsweise Menschen verabreicht, die einen Polymorphismus an der ATP- Bindungskassette Unterfamilie G Mitglied 2 (ATP binding cassette sub-family G member 2, ABCG2), auch bekannt als Brustkrebsresistprotein (breast Cancer resist protein BCRP), aufweisen (siehe Wiese et al.: Polymorphisms in cytochrome P450 2C19 enzyme and cessation of leflunomide in patients with rheumatoid arthritis. Arthritis Research & Therapy 2012 14:R163). Patienten mit einem entsprechenden ABCG2-Genotyp bzw. BCRP-Genotyp weisen ein unvorteilhaftes Risiko- Nutzen-Profil hinsichtlich der Nebenwirkung Durchfall auf und brechen mit einer höheren Wahr- scheinlichkeit die Leflunomid-Therapie insbesondere bei der Behandlung von rheumatoider Arthritis und insbesondere bei einer Therapie mit weiteren Medikamenten ab. Eine Verabreichung einer erfindungsgemäßen Zusammensetzung enthaltend 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid-Einzeldosierung gemäß erstem Erfindungsgegenstand, insbesondere bei der Behandlung von rheumatoider Arthritis kann aufgrund des reduzierten Leflu- nomidgehaltes somit zu einer Reduzierung der Durchfallhäufigkeit und/oder Durchfallstärke und
damit zu einer Reduzierung des Therapieabbruchs von Leflunomid insbesondere in Verabreichung von Leflunomid mit weiteren Autoimmun-Medikamenten führen.
Die Verabreichung einer erfindungsgemäßen pharmazeutischen Zusammensetzung enthaltend 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid-Einzeldosierung gemäß erstem Erfindungsgegenstand kann zudem gegenüber der handelsüblichen Einzeldosierungen von 20 mg Leflunomid dann vorteilhaft sein, wenn steady State level von Leflunomid und/oder seiner Metabolite, vorzugsweise Teriflunomid, schneller erreicht werden. Ein entsprechend vorteilhaftes Risiko-Nutzen-Profil kann vorzugsweise bei der Behandlung von rheumatoider Arthritis und insbesondere in einem Therapieregime mit ein oder mehreren weiteren Autoimmun- Wirkstoffen beispielsweise auf einer Sättigung von Transporter-Proteinen beruhen, die insbesondere in die Aufnahme aus dem Magen-Darm-Trakt, die Verteilung im Körper (Plasmaspiegel) und/oder die Ausscheidung (beispielweise über die Galle) von Leflunomid und/oder seiner Metabolite, vorzugsweise Teriflunomid eingreifen.
In einer weiteren bevorzugten erfindungsgemäßen Ausgestaltung wird die erfindungsgemäße Zusammensetzung des ersten erfindungsgemäßen Gegenstandes vorzugsweise bei der Behandlung von rheumatoider Arthritis und insbesondere in einem Therapieregime mit ein oder mehreren weiteren Autoimmun-Wirkstoffen an Patienten, vorzugsweise Menschen verabreicht, die einen . Polymorphismus des ABCG2- / BCRP-Transporters aufweisen. Zur Vereinfachung wird im Folgenden im Wesentlichen Bezug genommen auf BCRP, wobei dies im Rahmen der vorliegenden Erfindung synonym für ABCG2 verwendet wird. Solche Transporter-Proteine spielen eine wichtige Rolle bei der Absorption, Distribution, Metabolisierung und/oder Ausscheidung von Leflunomid und/oder seiner Metabolite, vorzugsweise Teriflunomid.
BCRP ist einer der wichtigsten ausschleusenden (efflux) Transporter der Zellen des Endo- und Epithels. In vitro Studien zeigen, dass insbesondere Leflunomid und sein Metabolit Teriflunomid als Substrate eine hohe Bindungsaffinität an den BCRP Transporter aufweisen [Bartlett, R.R., et al., Effects of leflunomide on immune responses and models of inflammation. Springer Semin Immunopathol, 1993. 14(4): p. 381 -94]. Die Sättigung des BCRP Transporters spielt somit eine wichtige Rolle i) für die Erreichung von therapeutisch wirksamen Plasmaspiegeln von Leflunomid und/oder seiner Metabolite, vorzugsweise Teriflunomid, gerade mit Blick auf die Induktionsphase sowie ii) für die notwendige Zeit zur Erreichung des steady State levels von Leflunomid und/oder seiner Metabolite, vorzugsweise Teriflunomid, da Leflunomid und/oder seine Metabolite, vorzugsweise Teriflunomid Substrate für diesen BCRP Transporter darstellen.
Eine Verabreichung einer erfindungsgemäßen Zusammensetzung enthaltend 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid-Einzeldosierung gemäß erstem Erfindungsgegenstand, kann vorzugsweise bei der Behandlung von rheumatoider Arthritis und
insbesondere in einem Therapieregime mit ein oder mehreren weiteren Autoimmun-Wirkstoffen zu einem
• veränderten Verhältnis des Plasmagehaltes an Leflunomid gegenüber dem Plasmagehalt seiner Metabolite, vorzugsweise Teriflunomid und/oder
• veränderten Zeitfenster über welches die steady State level von Leflunomid und/oder seiner Metabolite, vorzugsweise Teriflunomid, erreicht werden
und dadurch zu einem schnelleren Eintritt des therapeutisch wirksamen Effektes und/oder zu einer Reduzierung der Toxizität von Leflunomid und/oder seiner Metabolite, vorzugsweise Teriflunomid führen.
Patienten mit dem C.421CA Genotyp und einem Polymorphismus des BCRP Transporter-Proteins weisen üblicherweise höhere Plasmaspiegel an Leflunomidmetaboliten, vorzugsweise Teriflunomid, auf als Patienten ohne diesen Polymorphismus, weshalb dieser Polymorphismus zu einer inter-individuellen Variabilität des Plasmaspiegel an Leflunomidmetaboliten bei der Behandlung von rheumatoider Arthritis und insbesondere in einem Therapieregime mit ein oder mehreren weiteren Autoimmun-Medikamenten führt [Williams, J.W., et al., Immunosuppressive effects of leflunomide in a cardiac allograft model. Transplant Proc, 1993. 25(1 Pt 1 ): p. 745-6]. Dieser Umstand ist insbesondere dann besonders relevant, wenn ein oder mehrere der weiteren Autoimmun- Wirkstoffe in dem Kombinationstherapieregime selbst und/oder deren Metabolite ebenfalls Substrate für dieses BCRP Transporter-Protein darstellen.
Somit spielen Sättigung und Polymorphismus der BCRP Transporter-Proteine eine wichtige Rolle vorzugsweise bei der Behandlung von rheumatoider Arthritis und insbesondere in einem Therapieregime mit ein oder mehreren weiteren Autoimmun-Wirkstoffen, da aufgrund der Sättigung bzw. des Polymorphismus der BCRP Transporter-Proteine die Plasmaspiegel und/oder die Zeit, die notwendig ist, um ein steady State level von Leflunomid und/oder seiner Metabolite, vorzugsweise Teriflunomid, zu erreichen und/oder die Zeit bis zum Erreichen der therapeutischen Wirksamkeit und/oder des Toxizitätslevels von Leflunomid und/oder seiner Metabolite, vorzugsweise Teriflunomid verändert werden können. Dieser Umstand ist insbesondere dann besonders relevant, wenn ein oder mehrere der weiteren Autoimmun-Wirkstoffe in dem Kombinationstherapieregime selbst und/oder deren Metabolite ebenfalls Substrate für dieses BCRP Transporter-Protein darstellen.
Eine Verabreichung einer erfindungsgemäßen Zusammensetzung enthaltend 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid-Einzeldosierung gemäß erstem Erfindungsgegenstand, insbesondere bei der Behandlung von rheumatoider Arthritis kann folglich aufgrund eines veränderten Verhältnis des Plasmagehaltes von Leflunomid gegenüber seiner Metaboliten, vorzugsweise Teriflunomid, zu einer Reduzierung der Toxizität und damit zu einer Reduzie-
rung der Therapieabbrüche unter Leflunomid insbesondere in Kombination mit ein oder mehreren weiteren Autoimmun-Medikamenten führen.
Gemäß dem ersten erfindungsgemäßen Gegenstand ist die pharmazeutische Zusammensetzung umfassend Leflunomid, dadurch gekennzeichnet, dass die Zusammensetzung als Einzeldosis mit 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid hergerichtet oder herrichtbar ist.
Gemäß einer bevorzugten Ausgestaltung sind die erfindungsgemäßen pharmazeutischen Zusammensetzungen des ersten erfindungsgemäßen Gegenstandes bevorzugt dadurch gekennzeichnet, dass die Zusammensetzungen fest oder flüssig und zur oralen Verabreichung geeignet sind. Bevorzugte feste erfindungsgemäße pharmazeutische Zusammensetzungen zur oralen Verabreichung werden ausgewählt aus der Gruppe bestehend aus Pulvern, Pellets, Streukügelchen, Granulaten, Tabletten und/oder Kapseln, besonders bevorzugt Tabletten, insbesondere bevorzugt Filmtabletten. Bevorzugte flüssige erfindungsgemäße pharmazeutische Zusammensetzungen zur oralen Verabreichung werden ausgewählt aus der Gruppe bestehend aus Elixieren, Lösungen, Suspensionen, Emulsionen, Mixturen, Sirupe, Säfte und/oder Tropfen. Die erfindungsgemäßen pharmazeutischen Zusammensetzungen des ersten Erfindungsgegenstandes können als retardierte oder nicht-retardierte Formulierung erfindungsgemäß angewendet werden.
Gemäß vorliegender Erfindung können alle für die jeweilige pharmazeutische Formulierung üblichen pharmazeutisch akzeptablen Hilfsstoffe für die Herstellung der erfindungsgemäßen pharmazeutischen Zusammensetzungen gemäß erstem erfindungsgemäßen Gegenstand verwendet werden.
Bei der Herstellung von Tablettenformulierungen als erfindungsgemäße pharmazeutische Zusammensetzungen gemäß erstem Erfindungsgegenstand können als pharmazeutisch akzeptable Hilfsstoffe je nach Art der Tablettierung (Direkttablettierung, Trocken- oder Feuchtgranulierungs- verfahren) die jeweils geeigneten Hilfsstoffe ausgewählt werden aus der Gruppe bestehend aus i) Füllstoffe, bevorzugt Stärke und Stärkederivate, insbesondere bevorzugt Kartoffel-, Weizen- und/oder Maisstärke(derivate), Lactose, Lactose Monohydrat, Mannitol, Sorbitol, Calciumcarbonat, Lävulose, Glucose, Cellulose, mikrokristalline Cellulose, Hydroxypropylcellulose, und/oder Cal- ciumphosphate, insbesondere bevorzugt Di-Calciumphosphat, besonders bevorzugte Füllstoffe werden ausgewählt aus der Gruppe bestehend aus Lactose, Lactose Monohydrat, Di- Calciumphosphat und/oder Hydroxypropylcellulose, vorzugsweise Lactose Monohydrat; ii) Sprengmittel, bevorzugt Stärke oder Stärkederivate, insbesondere bevorzugt Kartoffel-, Weizen- und/oder Maisstärke(derivate), Pektin, Alginate, Alginsäure, mikrokristalline Cellulose, vernetzte Natriumcarboxymethylcellulose, vernetztes Polyvinylpyrrolidon, wie Crospovidon (Polyplasdone) und/oder Croscarmellose; iii) Bindemittel oder Klebstoffe (insbesondere bei der Feuchtgranulie- rung), bevorzugt Zucker, Stärke, Stärkekleister und/oder Stärkederivate, insbesondere bevorzugt
Kartoffel-, Weizen und/oder Maisstärke(derivate), Gelatine, Cellulosederivate, insbesondere bevorzugt (niedrigsubstituierte) Hydroxypropylcellulose, Hydroxyethylcellulose, und/oder Hydroxyp- ropylmethylcellulose, mikrokristalline Cellulose, arabisches Gummi, Tragant, Polyethylenglykol und/oder Polyvinylpyrrolidon, und/oder Kollidon, ein besonders bevorzugtes Bindemittel oder Klebstoff ist (niedrigsubstituierte) Hydroxypropylcellulose; iv) Trockenbindemittel (insbesondere bei der Trockengranulierung), bevorzugt mikrokristalline Cellulose, Lactose, Lactose Monohydrat, Saccharose, Mannitol, Sorbitol, Calciumcarbonat, Polyvinylpyrrolidon, Kollidon und/oder Polyethy- lenglycol; v) Fließregulierungsmittel oder Trockenmittel, bevorzugt hochdisperse Kieselsäure; und/oder vi) Gleitmittel, bevorzugt Talkum, silikonisiertes Talkum, Stearinsäure, Palmitinsäure, Calcium-, Aluminium- und/oder Magnesiumstearat, Calciumbehenat (Gemisch von Calciumsalzen höherer Fettsäuren, vorzugsweise der Behensäure), Stärke, Aerosil, (kolloidales) Siliciumdioxid, Polyethylenglycol, Hydroxypropylcellulose, Stearyl-, Cetyl- und/oder Myristylalkohol, Paraffin, hydrierte Fette und/oder Magnesiumtrisilicat, bevorzugt Gleitmittel werden insbesondere ausgewählt aus der Gruppe bestehend aus Magnesiumstearat und/oder Hydroxypropylcellulose, weiter bevorzugt Magnesiumstearat; und/oder weitere geeignete Hilfsmitteln, wie beispielsweise Benetzungsmittel wie Natriumlaurylsulfat oder Polysorbat, besonders bevorzugt Natriumlaurylsulfat, und Hilfsmittel zur Herstellung des Filmüberzugs, beispielsweise Filmbildner, Pigmente, Füllstoffe, Weichmacher und/oder Stabilisierer, bevorzugt ausgewählt aus der Gruppe bestehend aus Poly- dextrose (E1200), Hypromellose, Triacetin, Macrogol, Eisenoxide (beispielsweise rot und/oder gelb), Polyvinylalkohol, Titandioxid, Talkum, Lecithin (vorzugsweise aus Sojabohnen), Xanthan und/oder geeignete Polyacrylsäurederivate und/oder (lösliche) Kollidone.
Gemäß einer kumulativ oder alternativ bevorzugten Ausgestaltung des ersten erfindungsgemäßen Gegenstandes umfassen die erfindungsgemäßen pharmazeutischen Zusammensetzungen, vorzugsweise als feste oder flüssige orale Zubereitung zusätzlich ein pH-Regulierungsmittel, insbesondere bevorzugt ein- oder mehrbasige organische und/oder anorganische Säuren als pharmazeutisch akzeptable Hilfsstoffe, wobei die anorganischen Säuren bevorzugt aus der Gruppe bestehend aus Phosphorsäure und Schwefelsäure, deren sauer reagierende Salze wie Dihydrogen- phosphate und Hydrogensulfate sowie deren Kondensate, wie etwa Polyphosphorsäure, und/oder anorganische Puffersysteme, die in wässriger Lösung einen pH-Wert von kleiner 7 ergeben, ausgewählt werden; und wobei die organischen Säuren bevorzugt aus der Gruppe bestehend aus Carbonsäuren, vorzugsweise Weinsäure, Äpfelsäure, Fumarsäure, Citronensäure, Malonsäure, Maleinsäure sowie Methan-, Ethan- und/oder p-Toluolsulfonsäure, weiter bevorzugt Weinsäure, Äpfelsäure, Fumarsäure, Citronensäure, Malonsäure, Maleinsäure, und/oder organische Puffersysteme, die in wässriger Lösung einen pH-Wert von kleiner 7 ergeben (beispielsweise Citronensäu- re/Citratpuffer und/oder Essigsäure/Acetat-Puffer), ausgewählt werden. Entsprechende erfindungsgemäße pharmazeutische Zusammensetzungen sind deshalb bevorzugt, weil sie zusätzlich die Lagerstabilität von Leflunomid in der pharmazeutischen Zusammensetzung gegenüber phar-
mazeutischen Zusammensetzungen enthaltend Leflunomid ohne Säurezusatz erhöhen können, d.h. die Umwandlung von Leflunomid zu Teriflunomid während der Lagerung verringern können.
Gemäß einer besonders bevorzugten Ausgestaltung des ersten Erfindungsgegenstandes umfasst eine erfindungsgemäße Zusammensetzung als Tablette neben dem Anteil an 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, weiter bevorzugt 15 mg Leflunomid die folgenden Hilfsstoffe in geeigneter Menge, nämlich a) Lactose Monohydrat, niedrigsubstituiertes Hydroxypropylcellulose, ein pH- Regulierungsmittel, vorzugsweise ausgewählt aus organische Säure, besonders bevorzugt Weinsäure oder Citronensäure, Natriumlaurylsulfat und Magnesiumstearat oder b) Maisstärke, Povidon (E 1201 ), Crospovidon (E1202), hochdisperses Siliciumdioxid, Magnesiumstearat (E470b) und Lactose-Monohydrat. Bei Filmtabletten als erfindungsgemäße pharmazeutische Zusammensetzungen werden bevorzugt geeignete Mengen folgender Hilfsstoffe für den Filmüberzug verwendet, nämlich a) Polyvinylalkohol, Titandioxid, Talkum, Lecithin und Xanthan oder b) Talkum (E553b), Hypromellose (E 464), Titandioxid (E171 ), Macrogol 8000 und ggf. Eisen(lll)-hydroxid-oxid (E 172).
Gemäß einer weiteren kumulativ oder alternativ bevorzugten Ausgestaltung des ersten erfindungsgemäßen Gegenstandes werden erfindungsgemäße pharmazeutische Zusammensetzungen zur Prophylaxe und/oder Behandlung von Autoimmunerkrankungen, bevorzugt ausgewählt aus einer Gruppe bestehend aus rheumatoider Arthritis, Psoriasis Arthritis, Arthritis als Komplikation zystischer Fibrose, Lupus Nephritis, systemischem Lupus Erythematodes, Uveitis, Myasthenia Gravis, Granulomatose mit Polyangitis (Morbus Wegener); im Rahmen von Organtransplantationen, bevorzugt ausgewählt aus der Gruppe bestehend aus Nierentransplantation und/oder Lebertransplantation; von onkologische Erkrankungen, bevorzugt ausgewählt aus der Gruppe bestehend aus Melanomen, Sarkomen, Gliomen, Prostata-Karzinomen, Hirn- und/oder ZNS-Tumoren; und/oder von Erkrankungen mit dem Humanen Immundefizienz-Virus (HIV), besonders bevorzugt zur Prophylaxe und/oder Behandlung von rheumatoider Arthritis, von Psoriasis Arthritis und/oder Arthritis als Komplikation zystischer Fibrose verabreicht. Üblicherweise wird die erfindungsgemäße pharmazeutische Zusammensetzung enthaltend 14 bis 17,5 mg, bevorzugt 5 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid einmal täglich bzw. je nach Art und/oder Schwere der Erkrankung zwei-, drei oder viermal täglich (simultan oder sequentiell) zur Prophylaxe und/oder Behandlung der vorgenannten Krankheiten verabreicht.
Im Rahmen der vorliegenden Erfindung betrifft die„simultane" Verabreichung gemäß erstem Erfindungsgegenstand die zweimal, dreimal oder viermal tägliche Verabreichung einer erfindungsgemäßen pharmazeutischen Zusammensetzung enthaltend Leflunomid hergerichtet als Einzeldosis von 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid und bedeutet, dass die erfindungsgemäße pharmazeutische Zusammensetzung simultan, d.h. in enger zeitlicher Abfolge, üblicherweise über einen Zeitraum von bis zu 10 Minuten, vorzugweise bis zu 5 Minuten, weiter bevorzugt bis zu 1 Minute vom Patienten eingenommen wird.
Im Rahmen der vorliegenden Erfindung betrifft die„sequentielle" Verabreichung gemäß erstem Erfindungsgegenstand die zweimal, dreimal oder viermal tägliche Verabreichung einer erfindungsgemäßen pharmazeutischen Zusammensetzung enthaltend Leflunomid hergerichtet als Einzeldosis von 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid und bedeutet, dass die erfindungsgemäße pharmazeutische Zusammensetzung sequentiell, d.h. zeitlich versetzt, üblicherweise im Abstand von ca. 12 Stunden insbesondere bei einer zweimal täglichen Verabreichung, in einem Abstand von ca. 8 Stunden insbesondere bei einer dreimal täglichen Verabreichung und/oder im Abstand von ca. 6 Stunden insbesondere bei einer viermal täglichen Verabreichung vom Patienten eingenommen wird.
Die Einnahme der erfindungsgemäßen pharmazeutischen Zusammensetzungen gemäß erstem Erfindungsgegenstand kann üblicherweise zu allen Tages- und Nachtzeiten sowie vor, während und nach dem Essen, vorzugsweise in zeitlichem Abstand vor dem Essen durchgeführt werden.
Im Rahmen der Prophylaxe und/oder Behandlung von Autoimmunerkrankungen, bevorzugt ausgewählt aus einer Gruppe bestehend aus rheumatoider Arthritis, Psoriasis Arthritis, Arthritis als Komplikation zystischer Fibrose, Lupus Nephritis, systemischem Lupus Erythematodes, Uveitis und/oder Myasthenia Gravis, und/oder der Prophylaxe und/oder Behandlung von Erkrankungen mit dem Humanen Immundefizienz-Virus (HIV) wird die erfindungsgemäße pharmazeutische Zusammensetzung enthaltend 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid hergerichtet als Einzeldosis üblicherweise einmal oder zweimal täglich (simultan oder sequentiell), weiter bevorzugt einmal täglich verabreicht, um die erfindungsgemäßen Vorteile, insbesondere ein optimiertes Wirkungs-Nebenwirkungsprofil zu erreichen.
Im Rahmen der Prophylaxe und/oder Behandlung von Granulomatose mit Polyangitis (Morbus Wegener) wird die erfindungsgemäße pharmazeutische Zusammensetzung enthaltend 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid hergerichtet als Einzeldosis üblicherweise einmal, zweimal oder dreimal (simultan oder sequentiell) täglich, weiter bevorzugt zweimal oder dreimal täglich (simultan oder sequentiell) verabreicht, um die erfindungsgemäßen Vorteile, insbesondere ein optimiertes Wirkungs-Nebenwirkungsprofil zu erreichen.
Im Rahmen der Prophylaxe und/oder Behandlung von Organtransplantationen, bevorzugt ausgewählt aus der Gruppe bestehend aus Nierentransplantation und/oder Lebertransplantation; und/oder von onkologische Erkrankungen, bevorzugt ausgewählt aus der Gruppe bestehend aus Melanomen, Sarkomen, Gliomen, Prostata-Karzinomen, und/oder Hirn- und/oder ZNS-Tumoren wird die erfindungsgemäße pharmazeutische Zusammensetzung enthaltend 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid hergerichtet als Einzeldosis üblicherweise einmal, zweimal, dreimal oder viermal täglich (simultan oder sequentiell), weiter bevorzugt zweimal, dreimal oder viermal täglich (simultan oder sequentiell), besonders bevorzugt drei-
mal oder viermal täglich (simultan oder sequentiell) verabreicht, um die erfindungsgemäßen Vorteile, insbesondere ein optimiertes Wirkungs-Nebenwirkungsprofil zu erreichen.
Gemäß einer weiteren vorteilhaften Ausgestaltung der vorliegenden Erfindung wird die erfindungsgemäße pharmazeutische Zusammensetzung gemäß erstem Erfindungsgegenstand vorzugsweise an Patienten mit einem Alter von gleich oder mehr als 45 Jahren, weiter bevorzugt gleich oder mehr als 55 Jahre, noch weiter bevorzugt gleich oder mehr als 65 Jahre, noch weiter bevorzugt gleich oder mehr als 70 Jahre verabreicht, um optimale Ergebnisse der vorteilhaften erfindungsgemäßen Wirkungen insbesondere bezüglich der gleichen oder verbesserten Wirksamkeit, der reduzierten Nebenwirkungen und der reduzierten Therapieabbrüche bzw. Wechsel zu anderen Medikamenten zu erreichen.
Der zweite Erfindungsgegenstand betrifft die Verwendung von Leflunomid zur Prophylaxe und/oder Behandlung einer Autoimmunerkrankung, bevorzugt von rheumatoider Arthritis, von Psoriasis Arthritis, von Arthritis als Komplikation zystischer Fibrose, von Lupus Nephritis, von systemischem Lupus Erythematodes, von Uveitis, von Myasthenia Gravis und/oder von Granulomatose mit Po- lyangitis (Morbus Wegener); und/oder bei der Prophylaxe und/oder Behandlung im Rahmen einer Organtransplantation, bevorzugt Nierentransplantation und/oder Lebertransplantation; und/oder bei der Prophylaxe und/oder Behandlung von onkologischen Erkrankungen, bevorzugt Melanomen, Sarkomen, Gliomen, Prostata-Karzinomen, Hirn- und/oder ZNS-Tumoren; und/oder bei der Prophylaxe und/oder Behandlung von Erkrankungen mit dem Humanen Immundefizienz-Virus (HIV) dadurch gekennzeichnet, dass Leflunomid als Einzeldosis einer pharmazeutische Zusammensetzung mit 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid hergerichtet oder herrichtbar ist und einmal, zweimal, dreimal oder viermal täglich verabreicht wird. Die erfindungsgemäß bevorzugten Ausgestaltungen, die in der vorliegenden Anmeldung in Bezug auf den ersten Erfindungsgegenstand beschrieben werden, sind alternativ oder kumulativ auch auf den zweiten Erfindungsgegenstand erfindungsgemäß anzuwenden.
Gemäß der vorliegenden Erfindung bedeutet der Ausdruck,„dass Leflunomid als Einzeldosis einer pharmazeutische Zusammensetzung mit 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid hergerichtet ist", dass die kommerziell erhältliche erfindungsgemäße pharmazeutische Zusammensetzung bereits in einer solchen Form abgeteilt vorliegt, dass eine oder mehrere Einzeldosisformulierungen mit 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid enthalten sind.
Gemäß der vorliegenden Erfindung bedeutet der Ausdruck,„dass Leflunomid als Einzeldosis einer pharmazeutische Zusammensetzung mit 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid herrichtbar ist", dass die kommerziell erhältliche erfindungsgemäße pharmazeutische Zusammensetzung noch nicht als Einzeldosisformulierung mit 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid abgeteilt vorliegt, jedoch insbe-
sondere durch den Patienten so abgeteilt werden kann, dass eine erfindungsgemäße pharmazeutische Einzeldosisformulierung mit 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid hergerichtet wird. Bei den erfindungsgemäßen flüssigen pharmazeutischen Zusammensetzungen zur oralen Verabreichung kann eine solche Einzeldosierungsformulierung beispielsweise durch Abmessen der Flüssigkeit mittels Messbecher oder Messlöffel oder Abzahlung von Tropfen oder jede andere geeignete Methode hergerichtet werden. Bei den erfindungsgemäßen festen pharmazeutischen Zusammensetzungen zur oralen Verabreichung kann eine solche Einzeldosierungsformulierung beispielsweise durch Brechen einer (Film-) Tablette, bevorzugt einer (Film-) Tablette mit Bruchkerbe, Abzählen von Pellets, Streukügelchen, oder Granulaten oder jede andere geeignete Methode hergerichtet werden.
Die vorliegende Erfindung wird im Weiteren anhand von beispielhaften Ausführungsarten beschrieben, die lediglich als Beispiele verstanden werden sollen und die nicht den Schutzumfang des vorliegenden Schutzrechtes beschränken sollen.

Die einzelnen Merkmale der vorstehenden Ausführungsbeispiele zu erfindungsgemäßen pharmazeutischen Zusammensetzungen können separat jeweils auch in Kombination mit Merkmalen der weiteren Ausführungsbeispiele oder der allgemeinen Erfindungsbeschreibung erfindungsgemäß kombiniert werden.
Die Vorteile der Verabreichung einer erfindungsgemäßen pharmazeutischen Zusammensetzung enthaltend 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid- Einzeldosierung gemäß erstem Erfindungsgegenstand sowie deren erfindungsgemäße Verwendung gemäß zweitem Erfindungsgegenstand, vorzugsweise bei der Behandlung von rheumatoider Arthritis und insbesondere in einem Therapieregime mit ein oder mehreren weiteren Autoimmun- Wirkstoffen gegenüber den handelsüblichen Einzeldosierungen von 10 mg und 20 mg Leflunomid können in Tiermodellen nachgewiesen werden. Solche Tiermodelle lassen eine Untersuchung von Autoimmun-Erkrankungen insbesondere von chronischen Erkrankungen wie der rheumatoider Arthritis (RA) zu. Entsprechende Tiermodelle umfassen insbesondere, sind aber nicht limitiert auf, das Collagen-Induced-Arthritis (CIA) Tiermodell, das Adjuvant-Induced-Arthritis (AIA) Tiermodell und das Experimental-Allergic-Encephalomyelitis (EAE) Tiermodell sowie Tiermodelle für Graft- versus-Host-Disease oder für Transplantatabstoßungsreaktionen. Außerdem können auch Tiermodelle für die Untersuchung anderer Autoimmunerkrankungen, wie beispielsweise Psoriasis Arthritis, Arthritis als Komplikation zystischer Fibrose, Lupus Nephritis, systemischer Lupus Erythematodes, Uveitis, Myasthenia Gravis und/oder Granulomatose mit Polyangitis (Morbus Wege- ner) verwendet werden. Alternativ verwendbare Tiermodelle lassen die Untersuchung von Organtransplantationen, bevorzugt Nierentransplantationen und/oder Lebertransplantationen, und/oder die Untersuchung onkologischer Erkrankungen, bevorzugt Melanomen, Sarkomen, Gliomen, Prostata-Karzinomen, Hirn- und/oder ZNS-Tumoren; und/oder die Untersuchung von Erkrankungen mit dem Humanen Immundefizienz-Virus (HIV) zu.
Gezeigt wurde, dass in Sicherheits- und Toxizitätsstudien mit Leflunomid in naiven, unbehandelten Tieren (Hund, Ratte) ebenfalls Nebenwirkungen auftreten, die auch bei Patienten unter Leflunomid Behandlung vorkommen. Dies sind u.a. hämatotoxische Nebenwirkungen, gastrointestinale Nebenwirkungen, Creatininkinase (CK)-Erhöhung, Gewichtsverlust und/oder hepatotoxische Nebenwirkungen.
Bei Tiermodellen für chronische Autoimmunerkrankungen insbesondere Tiermodelle der rheumatoiden Arthritis kann es ähnlich wie bei der Behandlung von Patienten mit Autoimmunerkrankungen wie rheumatoider Arthritis, Psoriasis Arthritis, Arthritis als Komplikation zystischer Fibrose, Lupus Nephritis, systemischem Lupus Erythematodes, Uveitis, Myasthenia Gravis und/oder Granulomatose mit Polyangitis (Morbus Wegener) zu einer veränderten Verstoffwechslung (Metabolisierung) und/oder Pharmakokinetik und/oder Pharmakodynamik der Wirkstoffe.
Aus diesem Grund eignen sich die vorgenannten Tiermodelle, um das Risiko-Nutzen Verhältnis entsprechender Wirkstoffe, vorzugsweise von Leflunomid und/oder seinen Metaboliten, wie vorzugsweise Teriflunomid zu studieren. Bisher sind Sicherheitsstudien, in denen naive, nicht vorbehandelte Tiere mit Leflunomid behandelt wurden, publiziert. Vorgenannte Tiermodelle können die Patientensituation darstellen und lassen Rückschlüsse auf das Risiko-Nutzen-Verhältnis zu ohne die Sicherheit von Menschen zu beeinträchtigen.
Die Erfinder der vorliegenden Erfindung haben überraschend herausgefunden, dass eine erfindungsgemäße pharmazeutische Zusammensetzung aufgrund der Einzeldosierung von 14 bis 17,5 mg, bevorzugt 15 bis 17,5 mg, besonders bevorzugt 15 mg Leflunomid pro pharmazeutischer Formulierung gemäß erstem Erfindungsgegenstand sowie deren Verwendung gemäß zweitem Erfindungsgegenstand in ein oder mehreren Tiermodellen dieser Erkrankungen, wie beispielsweise, aber nicht limitiert auf, das Collagen-Induced Arthritis (CIA) RA Tiermodell
• eine vergleichbare oder verbesserte Wirksamkeit bei der Prophylaxe und/oder Behandlung von Autoimmunerkrankungen, bevorzugt von rheumatoider Arthritis, von Psoriasis Arthritis, von Arthritis als Komplikation zystischer Fibrose, von Lupus Nephritis, von systemischem Lupus Erythematodes, von Uveitis, von Myasthenia Gravis und/oder von Granulomatose mit Polyangitis (Morbus Wegener); und/oder bei der Prophylaxe und/oder Behandlung im Rahmen einer Organtransplantation, bevorzugt von Nierentransplantation und/oder von Lebertransplantation; und/oder bei der Prophylaxe und/oder Behandlung von onkologischen Erkrankungen, bevorzugt Melanomen, Sarkomen, Gliomen, Prostata-Karzinomen, Hirn- und/oder ZNS-Tumoren; und/oder bei der Prophylaxe und/oder Behandlung von Erkrankungen mit dem Humanen Immundefizienz-Virus (HIV) gegenüber der Standardtherapie mit 20 mg und/oder 10 mg Leflunomid hergerichtet als Einzeldosierung zeigt und/oder
• eine Reduzierung ein oder mehrerer der Nebenwirkungen, bevorzugt ein oder mehrere hä- matotoxische und/oder hepatotoxische und/oder gastrointestinale Nebenwirkungen ermöglicht und damit eine verbesserte Sicherheit gegenüber der Standardtherapie mit 20 mg und/oder 10 mg Leflunomid hergerichtet als Einzeldosierung zeigt und/oder
• eine Reduzierung an Leflunomid-Therapieabbrüchen bzw. Wechsel zu anderen Therapeutika gegenüber der Standardtherapie mit 20 mg und/oder 10 mg Leflunomid hergerichtet als Einzeldosierung ermöglicht.
Die erfindungsgemäßen Filmtabletten können gemäß gängiger Herstellverfahren (Direkttablettierung, Trocken- oder Feuchtgranulierung) hergestellt werden. Gemäß einer bevorzugten Ausgestaltung können alle oder ein Teil der Inhaltsstoffe des Tablettenkerns gemäß Feuchtgranulierungsver- fahren zu einem Tablettenkern verpresst werden. In einer geeigneten Apparatur können die Tab-
lettenkerne anschließend mit den Inhaltsstoffen des Filmüberzugs beschichtet und optional anschließend getrocknet werden.
Nachfolgend wird eine Auswahl an Nachweismöglichkeiten beschrieben, um die Wirksamkeit und Sicherheit von Leflunomid und/oder seines aktiven Metaboliten (A77 1726) Teriflunomid gemäß der erfindungsgemäßen pharmazeutischen Zusammensetzungen insbesondere bei rheumatoider Arthritis (RA) nachzuweisen.
B: Nicht-klinische Studien zur Untersuchung der Wirksamkeit von Leflunomid
Rheumatoide Arthritis (RA) ist eine chronisch entzündliche Erkrankung, die durch eine symmet- risch-polyartikuläre Synovitis mit Knorpel- und Knochenschäden gekennzeichnet ist. Grundsätzlich können viele Rattenmodelle zur Nachahmung der RA eingesetzt werden, einschließlich der Kollagen Typ ll-induzierten Arthritis (CIA), der Pristan (2, 6, 10, 14-Tetramethylpentadecan)- induzierten Arthritis (PIA), der Adjuvans-induzierten Arthritis, der Öl-induzierten Arthritis, der Avri- din-induzierten Arthritis, der Kollagen Typ Xl-induzierten Arthritis und der oligomeren Matrixprotein- induzierten Arthritis des Knorpels.
CIA ist eine der am häufigsten verwendeten Modelle der RA, und kann leicht durch intradermale Injektion von autologem oder heterologem Kollagen-Typ II (Cll) mit unvollständigem Freund- Adjuvans bei empfindlichen Ratten und Mäusen induziert werden. Ein anderes häufig verwendetes Modell der RA ist PIA, welches von einer einzigen subkutanen Pristan-Injektion induziert wird. Im PIA Modell wird der Krankheitsverlauf über 28 Tage verfolgt, wobei die Symptome regelmäßig 14 Tage nach Pristan-Injektion einsetzen. Der Höhepunkt der Krankheitssymptome liegt üblicherweise etwa 21 Tage nach Krankheits-Induktion. Im Gegensatz hierzu setzen die Krankheitssymptome im CIA Modell üblicherweise 18 Tage nach der Cll Verabreichung ein, wobei der Höhepunkt der Krankheitssymptome etwa 28 Tage nach Krankheits-Induktion liegt.
Materialien und Methoden
Alle menschlichen und tierischen Materialien bei den vorgenannten Modellen unterliegen den ethischen Bestimmungen bzw. werden nach Erlaubnis durch die Ethikkommission für den jeweiligen Gebrauch gewonnen.
Tiere
Beispielsweise können Dark Argouti (DA) Ratten von Taconic Europa (DK) als Tiere für die vorgenannten Modelle verwendet werden, wobei die Tiere vorzugsweise bei spezifischem Pathogen- freien Zustand und bei einem 12h Hell-Dunkel-Zyklus gehalten werden.
Zubereitung von Cll
CM wird üblicherweise aus dem Schwertfortsatz-Knorpel der DA-Ratte durch die Pepsin- verdauungs-Methode hergestellt (s. LU et al: Die Immunisierung von Ratten mit homologen Typ XI Kollagen führt zu chronischer und rezidivierender Arthritis, mit unterschiedlicher Genetik und Gelenkpathologie, als mit homologen Typ-Il-Kollagen induzierte Arthritis. J Autoimmun 2002; 18: 199- 211 ).
Nach Spaltung durch Pepsin wird der Ansatz üblicherweise mit Puffer (1 ,0 M NaCI/50 mM Tris / pH 7,5) extrahiert. Anschließend kann 0,9 M NaCI verwendet werden, um C II auszufällen. Das Kollagen wird üblicherweise lyophilisiert, gewogen, und dann aufgelöst und bis zur Verwendung in 0,1 M Essigsäure gelagert. Die Reinheit des Cll kann mittels der Coomasssie Blaufärbung nach SDS- PAGE-Analyse nachgewiesen.
Induktion von Arthritis
Im CIA-Modell werden Ratten üblicherweise durch eine einzige intradermale Injektion von 150μΙ Emulsion, welche 150pg C II gelöst in 75μΙ_ 0,1 mol / I Essigsäure und 75μΙ_ unvollständigem Freund-Adjuvans (Sigma-Aldrich, USA) enthält, immunisiert, d.h. die Krankheit induziert.
Im PIA-Modell wird den Ratten üblicherweise 150μΙ_ Pristan (Acros Organics, Belgien) subkutan an der Basis des Schwanzes injiziert, um das Krankheitsbild zu induzieren.
Kontroll-Ratten wird üblicherweise 150μΙ_ phosphatgepufferte Kochsalzlösung subkutan injiziert.
Klinische Bewertung der Arthritis
Üblicherweise wird ein makroskopisches Bewertungssystem verwendet, um die Entwicklung der Arthritis in allen 4 Extremitäten zu überwachen. Innerhalb dieses Bewertungssystems wird üblicherweise
(i) ein Punkt für jedes geschwollene oder rote metacarpophalangeal/metatarsophalangeal (MCP/MTP) Gelenk vergeben,
(ii) ein Punkt für geschwollene oder rote interphalangeal (IP) Gelenke jeden einzelnen Zehs, und
(iii) fünf Punkte höchstens für jedes Handgelenk oder Sprunggelenk (1 , leichte Rötung; 2, innen, außen oder Zentral der Gelenkschwellungen; 3, mäßige Schwellung / Rötung; 4, starke, aber nicht vollständige Gelenkschwellungen; 5, vollständige Gelenkschwellung / Rötung).
Die maximale Punktzahl für jede Pfote beträgt 15. Die Ratten werden üblicherweise 3-mal pro Woche nach Krankheits-Induktion betrachtet.
Pathologische Bewertung der Arthritis
Die linken Hinterpfoten der Ratten werden üblicherweise entfernt und fixiert, dann in 12,5% EDTA (Ethylendiamintetraessigsäure)-Lösung für 4 Wochen entkalkt. In dieser Zeit wird die Lösung üblicherweise alle 2 Tage gewechselt.
Die entkalkten Proben werden anschließend in Paraffin eingebettet und in 6 pm große Gewebeabschnitte geschnitten, die dann mit Hämatoxylin und Eosin (H & E) gefärbt werden.
Ein pathologisches Bewertungssystem wird üblicherweise eingesetzt, um die Schwere der Arthritis zu bewerten. Synovitis wird hierbei anhand der Anzahl von synovialen Zellschichtbereichen, Bereichen der Pannus-Beschichtungsgelenkfläche, Infiltration von synovialen Entzündungszellen und Bildung neuer Gefäße bewertet. Jedem Index werden Punkte von 0 bis 3 gegeben. Gelenkzerstörungen durch Knorpelabbau, Knochenerosion, Synarthrophysis und Gelenkstruktur werden mit 0 bis 3 Punkte bewertet. Eine Reparatur von Schäden durch die Bildung von neuem Knorpel und Knochen wird mit 0 bis 3 Punkte bewertet.
Pathologische Bewertung anderer Organe
Es können Magen-Darm-Trakt, Niere, Leber, inguinale Lymphknoten und popliterale Lymphknoten der Ratte entnommen und nach Entnahme gewogen werden. Nach Paraformaldehyd-Fixierung und H & E-Färbung können die pathologischen Veränderungen in den geernteten Geweben von beiden, den Kontroll- und PIA-Tieren, identifiziert und ausgewertet werden.
Messung der A77 1726 Level
Die Messung der A77 1726 Level können, wie in Rakhila et al. (2011 ) J. Pharm. Biomed. Anal. 5; 55 (2), S. 325-31 beschrieben, durchgeführt werden.
Wechselwirkungen von Leflunomid und/oder seiner Metaboliten mit Cytochrom-P450- Enzymen
Der Spiegel an Cytochrom-P450 kann unter Verwendung von Ratten spezifischen, kommerziell erhältlichen ELISA-Kits gemessen werden.
Messung des Arzneistoff-Transporterprotein-Levels
Die Menge an mRNA des ATP-binding cassette sub-family G member 2 (ABCG2), auch bekannt als Brustkrebsresistenzprotein (BCRP), kann, wie in Takara et al. (2003) Drug Metab Dispos 31 :1235-1239 und Takara et al. (2007) EXCLI Journal 6:138-144 beschrieben, gemessen werden.
Messung des DHODH Levels / Aktivität
Der DHODH-Spiegel kann unter Verwendung eines Ratten-spezifischen handelsüblich erhältlichen ELISA-Kits gemessen werden.
Messung des Kinase-Levels / Aktivität
Identifizierung von Kinasen, welche durch Leflunomid und / oder A77 1726 aktiviert wurden
Ein Kinoscan kann, wie in Fabian, M.A. et al. (A small molecule-kinase interaction map for clinical kinase inhibitors. Nat. Biotechnol. 23, 329-336; 2005) und Wodicka, L.M. et al. (Activation state- dependent binding of small molecule kinase inhibitors: structural insights from biochemistry. Chem. Biol. 17, 1241-1249; 2010) beschrieben, durchgeführt werden. Kinasen als Fusionen mit T7- Phagen werden in einem E. coli-Wirt produziert, welcher üblicherweise aus dem Stamm BL21 hergestellt wird. E. coli wird hierbei üblicherweise bis zur log-Phase kulitiviert und mit T7-Phagen infiziert und unter Schütteln bei 32 ° C bis zur Lyse inkubiert. Die Lysate werden zentrifugiert und filtriert, um Zelltrümmer zu entfernen. Die verbleibenden Kinasen werden als Fusionen mit NF-KB in HEK-293 Zellen exprimiert. Die Konstrukte werden anschließend mit DNA für einen PCR- Nachweis markiert.
Streptavidin-beschichtete magnetische Perlen können mit biotinylierten kleinen Molekül-Liganden 30 Minuten bei Raumtemperatur behandelt werden, um Affinitätsharze für Kinase-Assays zu erzeugen. Die ligandierten Perlen werden üblicherweise hierbei mit überschüssigem Biotin blockiert und mit Blockierungspuffer (Seablock (Pierce), 1 % BSA, 0,05% Tween 20, 1 mM DTT) gewaschen, um ungebundene Liganden zu entfernen und nicht-spezifischer Bindungen zu vermeiden.
Phosphorylierungsstatus /-aktivität
Durch Verwendung von Standardtechniken wird üblicherweise frisch isoliertes Gewebe mit eiskaltem Homogenisierungspuffer gewaschen und homogenisiert.
Lysiertes Gewebe wird üblicherweise mit kommerziell erhältlichen primären Antikörper inkubiert, die spezifisch für die entsprechenden Kinasen sind. Diese werden durch Leflunomid und / oder Teriflunomid aktiviert. Die Anwesenheit von phosphorylierten und Gesamt-Kinasen wird unter Verwendung von zellbasiertem ELISA nach den Anweisungen des Herstellers gemessen.
Lokalisierung von Leflunomid und / oder A77 1726 aktivierten Kinasen in situ
Durch Anwendung von Standard Immunhistochemie (IHC)-Techniken und Verwendung kommerziell erhältlicher Phospho-Antikörper (BD Biosciences) kann die Kinase-Phosphorylierung in situ untersucht werden. Monoklonale Antikörper für Kinasen, welche durch Leflunomid und / oder Teriflunomid (kinoscan) aktiviert sind, werden hierbei üblicherweise verwendet, um die Kinase- Aktivität auf zellulärer Quelle zu lokalisieren.
C: In-vitro-Studien mit Leflunomid und seinen Metaboliten
Beispiel 1 : Kinome Scan von Leflunomid und A77 1726
Bindungsreaktionen können durch die Kombination von Kinasen und Liganden-Affinitäts-Perlen für Leflunomid und seinen aktiven Metaboliten A77 1726 (Teriflunomid) durchgeführt werden. Die Kinase-Konzentration in den Eluaten wird üblicherweise mittels quantitativer PCR gemessen.
Beispiel 2: Proliferationsassay unter Verwendung Mitogen induzierter peripherer Blutzellen (Mensch und Ratte)
Heparinisiertes Vollblut, Buffy Coat oder Leukozytenfilter Fraktion wird von gesunden Freiwilligen (Mensch) für die Isolierung von humanen peripheren mononukleären Blutzellen verwendet. Heparinisiertes Vollblut oder Milz wird von weiblichen oder männlichen DA-Ratten (Taconic Europa, DK) entnommen.
Humane periphere mononukleäre Blutzellen (PBMCs) können aus heparinisiertem venösem Blut von gesunden Freiwilligen durch Zentrifugation auf einem Ficoll-Paque-Gradienten (Amersham Biosciences AB) isoliert werden. Die weißen Zellen werden hierbei aus der Ficoll-Plasma- Zwischenphase gesammelt und zweimal mit frischer phosphatgepufferter Kochsalzlösung (PBS) gewaschen.
Erythrozyten im Zellpellet werden mit destilliertem H20 lysiert und die Reaktion durch Zugabe des gleichen Volumens an 1 ,8% NaCI gestoppt, um die Isotonizität wiederherzustellen. Isolierte Zellen werden in sterilem Komplettmedium R10 (RPMI 1640 mit HEPES Puffer 25 mM, Glutamax I, 10% fötales Rinderserum und Penicillin-Streptomycin (5 ml Penicillin 5000IU/mL + Streptomycin 5000pg/mL) resuspendiert. Die Zellen werden üblicherweise auf 96w-Platten mit einer Dichte von 1 x 106 Zellen / ml ausgesät und für eine Stunde mit A771726 inkubiert (Konzentrationen von 1 x 10-9 - 1 x 10-4 M).
Anschließend werden die Zellen üblicherweise entweder mit anti-CD3 (10 ng / ml), PHA (1 pg/ml) oder mit ConA (10 g/ml) stimuliert und bei 37 ° C, 5% C02, 90% relative Feuchte (relative humidity, RH) für 24, 48 oder 72 Stunden kultiviert. Die Zellen werden üblicherweise eine Stunde vor der Ernte mit BrdU (30 μΜ) gepulst (zur Messung der proliferativen Kapazität). Zum Zeitpunkt der Ernte werden üblicherweise die Überstände gesammelt und für weitere Analysen bei -80° C gelagert.
Die Zellen werden üblicherweise mit Oberflächenmarkern zur Identifizierung der relevanten Popu- lationen von Leukozyten und T-Zellen mit anti-CD3, anti-CD4, anti-CD8, B-Zellen mit anti-CD19 und Monozyten sowie Neutrophile mit anti-CD14/antiCD16 gefärbt. Die Zellen werden anschließend üblicherweise fixiert, permeabilisiert und auf Anwesenheit auf nukleäres BrdU gefärbt. Anschließend werden die Zellen üblicherweise unter Verwendung eines FACS-Calibur.und% BrdU- positive Zellen analysiert, und innerhalb der verschiedenen Leukozyten-Populationen aufgezeich- net als Hinweis auf proliferative Kapazität.
Die Zellen, die mit Teriflunomid inkubiert und dann mit Mitogen stimuliert wurden, werden üblicherweise auch auf DHODH und ausgewählte Tyrosin-Kinase-Aktivität analysiert, um die Konzent- rations-Wirkungs-Beziehung zu studieren.
Weiße Blutzellen (WBC) der Ratte werden aus der Milz der Kontrollratten und Pristan-behandelten Ratten isoliert und sanft homogenisiert, um eine Einzelzellsuspension zu erzeugen. In PIA Ratten werden die Milzzellen üblicherweise auf dem Höhepunkt der Entzündung gewonnen und auf Ficoll- Paque gesetzt. Anschließend können die Milzzellen, wie für menschliche PBMC beschrieben, aufbereitet werden, allerdings unter Verwendung von Ratten spezifischen Antikörper für Leukozytensubpopulation. Die Zellen, die mit Teriflunomid inkubiert und dann mit Mitogen, wie erwähnt, stimuliert worden sind, können ebenfalls auf DHODH und ausgewählte Tyrosin-Kinase- Aktivität analysiert werden, um die Konzentrations-Wirkungs-Beziehung zu studieren.
D: In-vivo-Studien mit Leflunomid
Beispiel 3: Einfluss der Entzündung auf die Dosis-Exposure Beziehung Leflunomid
Männliche DA-Ratten im Alter von 8 bis 12 Wochen werden nach dem Zufallsprinzip in 2 Gruppen aufgeteilt: Eine PIA-Gruppe und eine Kontrollgruppe. Jede Gruppe wird weiter unterteilt in Dosierungsgruppen (n = 3-4) und erhalten eine Einzeldosierung von Leflunomid (beispielsweise 5 verschiedene Dosierungen). Um pharmakokinetische Profile zu ermitteln, werden üblicherweise Blutproben an 7 bis 10 Zeitpunkten nach der Verabreichung zur Messung der Plasmakonzentrationen an aktivem Leflunomid-Metabolit, (A77 1726) Teriflunomid, entnommen. Bei den Tieren mit Entzündung, erfolgt die Dosierung und Blutentnahme auf dem Höhepunkt der Entzündung.
Jede Gruppe enthält vorzugsweise eine Untergruppe, die nicht behandelt wird.
Diese Tiere werden üblicherweise verwendet, um den Einfluss von Entzündungen auf Systeme unter Beteiligung der Leber Arzneimittel-Clearance, z. B. Ausdruck / Aktivität des Cytochrom P450 (CYP) und von Zufluss / Abfluss Transportern zu bestätigen, und werden auf dem Höhepunkt der Entzündung geopfert. Die Lebern werden den Tieren entnommen. Ein Teil der Leber wird für immunhistochemische Analysen vorbereitet und verwendet, um die Expressionsniveaus von Transporterproteinen zu studieren. Das verbleibende Lebergewebe wird homogenisiert und Mikrosomen werden durch differentielle Zentrifugation isoliert.
Die gesamte CYP Expression in Mikrosomen kann gemessen werden (Kohlenmonoxid- Differenzspektrum) und Aktivitäten von CYP-Isoformen durch die zu quantifizierende Metabolisie- rung von Testosteron bestimmt werden.
Beispiel 4: Wirkung von Entzündungen auf die Wirksamkeit und Toxizität Dosis- Wirkungsbeziehung von Leflunomid
Männliche DA-Ratten im Alter von 8 bis 12 Wochen werden nach dem Zufallsprinzip in 2 Gruppen aufgeteilt, eine PIA-Gruppe und eine Kontrollgruppe.
Jede Gruppe wird weiter unterteilt in Behandlungsgruppen (vorzugsweise n = 10), die einmal täglich Leflunomid (beispielsweise 4 verschiedene Dosierungen oder Vehikel) über 3-4 Wochen erhält.
Um die Toxikokinetik zu etablieren, werden üblicherweise Blutproben an 7 bis 10 Zeitpunkten am Tag 1 und am Tag des Höhepunktes der Krankheit bei Tieren mit Entzündungen zur Messung der Plasmakonzentrationen an aktivem Leflunomid-Metabolit, (A77 1726) Teriflunomid, entnommen. Blutproben die zum Höhepunkt der Erkrankung entnommen werden, können weiter für die klinische Chemie, Hämatologie und Plasma-Marker, z. B. Zytokine analysiert werden. Während der Studie werden Wirksamkeitsparameter (disease-score) und allgemeine Gesundheitsparameter
(Gewicht, Verhalten) gemessen. Nach der Entnahme der zweiten Blutprobe werden die Tiere üblicherweise getötet und die Wirksamkeits- und Toxizitäts-Parameter wie Histopathologie der relevanten Gewebe (Gelenke, Leber-, Gl-Trakt, lymphatischen Organen) werden erhoben. Die pathologische Auswertung von Leflunomid behandelten PIA- und gesunden Kontrollratten ermöglicht die Ermittlung der Dosis-Toxizität und Dosis-Wirkung-Beziehungen für Leflunomid.
E: Ex-vivo-Studien mit Leflunomid
Beispiel 5: Ex-vivo-Messung von DHODH-Aktivität
Gewebeproben, die in Beispiel 4 zur ex-vivo-Messung entnommen werden, können zur Etablierung der DHODH-Aktivität ex vivo verwendet werden.
Beispiel 6: Ex-vivo-Messung der Kinase-Aktivität
Gewebeproben, die in Beispiel 4 zur ex-vivo-Messung entnommen werden, können zur Etablierung der Kinase-Aktivität ex vivo verwendet werden.
Der Phosphorylierungsstatus /-aktivität ausgewählter Kinasen welche im Kinome Scan (Beispiel 1 ) identifiziert werden, können unter Verwendung eines ELISA-basierten Assays in homogenisiertem Gewebe quantifiziert und ihre zelluläre Quelle in den Geweben unter Verwendung von Immunhistochemie lokalisiert werden.
Claims
1. Pharmazeutische Zusammensetzung umfassend Leflunomid, dadurch gekennzeichnet, dass die Zusammensetzung als Einzeldosis mit 14 bis 17,5 mg Leflunomid hergerichtet ist.
2. Pharmazeutische Zusammensetzung gemäß Anspruch 1 , dadurch gekennzeichnet, dass die Zusammensetzung als Einzeldosis mit 15 mg Leflunomid hergerichtet ist.
3. Pharmazeutische Zusammensetzung gemäß Anspruch 1 oder 2, dadurch gekennzeichnet, dass die Zusammensetzung fest oder flüssig ist und zur oralen Verabreichung geeignet ist.
' 4. Pharmazeutische Zusammensetzung gemäß Anspruch 3, dadurch gekennzeichnet, dass die feste Zusammensetzung ausgewählt wird aus der Gruppe bestehend aus Pulvern, Pellets, Streukügelchen, Granulaten, Tabletten und/oder Kapseln, oder dass die flüssige Zusammensetzung ausgewählt wird aus der Gruppe bestehend aus Elixieren, Lösungen, Suspensionen, Emulsionen, Mixturen, Sirupe, Säfte und/oder Tropfen.
5. Pharmazeutische Zusammensetzung gemäß einem der Ansprüche 1 bis 4 zur Prophylaxe und/der Behandlung einer Autoimmunerkrankung; im Rahmen einer Organtransplantation; von onkologischen Erkrankungen; und/oder von Erkrankungen mit dem Humanen Immunde- fizienz-Virus (HIV).
6. Pharmazeutische Zusammensetzung gemäß Anspruch 5, dadurch das die Einzeldosis mit 14 bis 17,5 mg Leflunomid einmal, zweimal, dreimal oder viermal täglich simultan oder sequentiell verabreicht wird.
7. Verwendung von Leflunomid zur Prophylaxe oder Behandlung einer Autoimmunerkrankung; im Rahmen einer Organtransplantation; von onkologischen Erkrankungen; und/oder von Erkrankungen mit dem Humanen Immundefizienz-Virus (HIV), dadurch gekennzeichnet, dass Leflunomid als Einzeldosis einer pharmazeutische Zusammensetzung mit 14 bis 17,5 mg Leflunomid hergerichtet oder herrichtbar ist und einmal, zweimal, dreimal oder viermal täglich verabreicht wird.
8. Pharmazeutische Zusammensetzung gemäß Anspruch 5 oder Verwendung gemäß Anspruch 7, dadurch gekennzeichnet, dass die
a. Autoimmunerkrankung ausgewählt wird aus einer Gruppe bestehend aus rheumatoider Arthritis, Psoriasis Arthritis, Arthritis als Komplikation zystischer Fibrose, Lupus Nephritis, systemischem Lupus Erythematodes, Uveitis, Myasthenia Gravis, und/oder Granulomatose mit Polyangitis (Morbus Wegener); und/oder
b. Organtransplantation ausgewählt wird aus Nierentransplantation und/oder Lebertransplantation und/oder
c. Onkologische Erkrankungen ausgewählt werden aus der Gruppe bestehend aus Melanomen, Sarkomen, Gliomen, Prostata-Karzinomen, Hirn- und/oder ZNS-Tumoren.
