Abstract
Description
クローン病(Crohn's disease、CD)は、消化管全域に非連続性の炎症及び潰瘍を生じる非特異性炎症性疾患である。北米で40~60万人、欧州で60万人、日本では約3万人がCDに罹患している希少疾病であり、患者数は生活習慣の変化に伴い増加し続けている(非特許文献1)。CDは、消化管全般に炎症や潰瘍が生じ、下痢、腹痛、発熱、下血に伴う貧血、体重減少、体力低下や倦怠感などの多彩な臨床症状が見られ、QOLが著しく低下する。また、CDの炎症病変は粘膜内から発症し、粘膜下層、固有筋層へと波及するため、全層性の肉芽腫性炎症、浮腫、線維化による腸壁の肥厚が特徴である(非特許文献2~4)。
CDに対する治療は、抗炎症並びに抗免疫機序に基づく薬剤、すなわち、5-ASA製剤、経口ステロイド、免疫抑制剤から開始され、近年抗TNF製剤も使用されるようになった(非特許文献5)。CDの臨床的活動性を評価するCDAI(Crohn's Disease Activity Index)の減少においては症状のコントロールが可能になってきた。しかしながら、CDAIで寛解あるいは軽症と判定された患者でも、内視鏡検査では局所の活動性病変が残存している症例は多く、繰り返す炎症の再燃や線維化による狭窄の起点病巣となると考えられている(非特許文献2、3、6、7)。CDは進行性の腸管組織障害(Progressive digestive damage)であるとする概念が確立され(非特許文献4、6、7)、治療戦略も症状コントロールから、内視鏡病変の治癒を介したi)長期的な腸管保護とii)腸狭窄による外科手術の回避へと、その重点がシフトしている(非特許文献6~11)。とりわけ、内視鏡的な腸管治癒(Intestinal Healing: IH)あるいは粘膜治癒(Mucosal Healing: MH)がCD患者の予後に影響を及ぼす因子であるとする報告がなされ(非特許文献6~11)、CDの内視鏡的活動性を評価するSES-CD(Simple Endoscopic Score for CD)の改善が、CD治療における新たな効果判定基準となってきた。既存薬によるCD患者のMH効果は、5-ASA製剤や経口ステロイド剤ではほとんど無く、免疫抑制剤では16.5%、抗TNF製剤でも30.1%と報告されている(非特許文献7、11)。このような全身投与薬で十分に制御しきれない内視鏡病変を如何に治癒させるか(IH/MH)が重要であるという医学的知識を付加した事が、レミケードの貢献であり、CDに対する新薬に求められる喫緊の課題が明確化されたとも言える。
内視鏡病変が残存する病因は十分には解明されていないが、潰瘍と線維化が同居する点に、CDにおける内視鏡病変治癒の難しさがある。組織損傷に対する修復過程において、適切なMHの代わりに、不適切な「線維化形成性」修復(Fibrotic healing)が誘導される事により、潰瘍と線維化を伴う炎症病巣が持続する事が一つの要因と考えられ(非特許文献2、3、12)、そのような局所病変に対しては炎症・免疫機序に基づく全身性既存薬だけでは十分な効果が獲得できないものと推測される。病理学的には損傷・修復を繰り返しながら残存していると考えられ(非特許文献3)、CDにおいてはアフタ(浮腫状の小隆起と紅斑を伴うびらん)や潰瘍、それも、浮腫性に内腔の狭小化を伴っている病変として、内視鏡検査で診断される。更には近年、拡大内視鏡や共焦点内視鏡など、リアルタイムでより詳細に観察できる技術も普及してきており、早期からの治療を可能にする環境が整ってきている。
炎症時早期から病変部に集積する線維芽細胞の過剰な活性化がFibrotic healingへバランスを傾けると証明されてきた事から、「線維芽細胞活性化の抑制」が炎症性腸疾患における炎症性線維化(Inflammation-driven intestinal fibrosis)に対する新しい治療ターゲットとして注目されている(非特許文献2~3)。炎症・免疫機序に基づいた既存薬だけでは十分な効果を得られない内視鏡病変は、潰瘍と線維化が同居する病態である事から、線維化/線維芽細胞を標的とした治療は、CDにおける新しい治療戦略として台頭してきたと言える。
糖硫酸転移酵素遺伝子であるCarbohydrate sulfotransferase 15 (CHST15)はコンドロイチン硫酸-A(CS-A)のGalNAc(4SO4)残基の6位に硫酸塩を転移し、高度に硫酸化されたコンドロイチン硫酸-E(CS-E)を合成するII型の膜貫通型ゴルジタンパク質である(非特許文献13~14)。CD患者の活動性大腸病変で高度に硫酸化されたコンドロイチン硫酸(CS)の合成が有意に増加している事(非特許文献15)、高度に硫酸化されたCS-Eは、CD患者の肥厚した粘膜下層で増加するV型コラーゲンと結合する事(非特許文献16~17)、更には高度に硫酸化されたCS-Eは、コラーゲン線維(フィブリル)形成を促進する事(非特許文献18)、が報告されており、CS-Eは局所線維化病変の維持と増強に関与していると考えられている。更に、高度に硫酸化されたCS-Eは、線維芽細胞の接着に関わる分子CD44、線維芽細胞の遊走に関わるケモカインMCP-1、SDF-1、線維芽細胞の増殖に関わるPDGF、TGF-βと結合する事が報告されており(非特許文献19)、粘膜下局所におけるこれらの分子の濃縮を介しても線維芽細胞の定着と活性化に関わっている事が示唆されている。
本発明のsiRNA(抗CHST15 siRNA)はCHST15産生に関与するhuman CHST15遺伝子の発現を抑制するように特異的にデザインされた27mer siRNAである。27mer siRNA二重鎖はnMあるいはpMのオーダーで遺伝子発現を非常に強く抑制する。本発明の抗CHST15 siRNAはCHST15 mRNA、すなわち、human CHST15のシークエンス領域に相補的であるシークエンスを含む合成siRNAであり、そのアンチセンス鎖はRNAi機能へのガイド・シークエンスとして働き、human CHST15を特異的に認識し分解させることにより、糖硫酸転移酵素(CHST15タンパク質)産生に関する遺伝子の発現をブロックする。その結果、CSの硫酸基の転移が起こらず、線維芽細胞も活性化されずに線維化が抑制されると推定される。
本発明者は、大腸炎モデルマウスを用いた実験により、CHST15遺伝子の発現を抑制するsiRNAが、クローン病もしくは潰瘍性大腸炎に対して治療効果を奏することを見出した。即ち、CHST15遺伝子の発現を抑制するsiRNAは、クローン病もしくは潰瘍性大腸炎の治療剤となることを見出し、本発明を完成させた。
〔1〕CHST15遺伝子の発現を抑制するsiRNAであって、配列番号:1に記載の塩基配列からなるRNAと、該RNAと相補的な配列からなるRNAとがハイブリダイズした構造を含むsiRNA。

本発明者によって、CHST15(Carbohydrate sulfotransferase 15)遺伝子の発現を抑制することにより、クローン病もしくは潰瘍性大腸炎の治療効果、または、創傷治癒、粘膜治癒、もしくは潰瘍治癒の効果が発揮されることが見出された。より詳しくは、本発明者は、CHST15遺伝子の発現をRNAi効果によって抑制することにより、クローン病もしくは潰瘍性大腸炎の治療効果、または、創傷治癒、粘膜治癒、もしくは潰瘍治癒の効果を有するRNA分子(siRNA)を見出した。
なお、本発明のCHST15は、別称として、GalNAc4S-6ST (N-acetylgalactosamine 4-sulfate 6-0 sulfotransferase)とも呼ばれる。

本発明の慢性炎症性疾患治療剤は、例えば、皮膚をはじめとする外皮及び上皮組織における弾性線維症、強皮症、慢性腹膜炎、後腹膜腔線維化症など、結合組織などの支持組織、筋肉における多発性筋炎、皮膚筋炎、結節性多発動脈炎、軟組織線維症、慢性関節リウマチ、手掌線維腫、腱炎、腱鞘炎、アキレス腱炎、足菌腫など、骨髄、心臓などの血液組織、脈管系における骨髄線維症、脾機能亢進症、脈管炎、徐脈性不整脈、動脈硬化、閉塞性血栓性血管炎、結節性線維症、狭心症、拡張型うっ血性心筋症、心不全、拘束型心筋症、びまん性非閉塞性心筋症、閉塞性心筋症、肺性心、僧帽弁狭窄、大動脈弁狭窄、慢性心膜炎、心内膜線維症、心内膜心筋線維症など、肝臓などの消化器系では慢性膵炎、クローン病、潰瘍性大腸炎、アルコール性肝炎、慢性B型肝炎、慢性C型肝炎、ウィルソン病、肝硬変、ウイルス性肝炎、ゴーシェ病、糖原病、α抗トリプシン欠損、ヘモクロマトーシス、チロシン血症、果糖血症、ガラクトース血症、ゼルウェガー症候群、先天性肝線維症、門脈圧亢進症、肝肉芽腫症、バッド-キアリ症候群、原発性硬化性胆管炎、脂肪肝、非アルコール性肝炎、肝線維症、先天性肝線維症、アルコール性肝硬変、ウイルス性肝硬変、寄生虫性肝硬変、中毒性肝硬変、栄養障害性肝硬変、うっ血性肝硬変、肝硬化症、シャルコー肝硬変、トッド肝硬変、続発性胆汁性肝硬変、単葉性肝硬変、慢性非化膿性破壊性胆炎から移行した肝硬変、閉塞性肝硬変、胆細管性肝硬変、胆汁性肝硬変、萎縮性肝硬変、壊死後性肝硬変、肝炎後肝硬変、結節性肝硬変、混合型肝硬変、小結節性肝硬変、代償性肝硬変、非代償性肝硬変、大結節性肝硬変、中隔性肝硬変、突発性肝硬変、門脈周囲性肝硬変、門脈性肝硬変、原発性胆汁性肝硬変など、肺などの呼吸系におけるコクシジオイデス症、ブラストミセス症、アレルギー性気管支肺アスペルギルス症、グッドパスチャー症候群、成人呼吸促迫症候群における肺線維症、慢性閉塞性肺疾患、無気肺、肺炎、珪肺症、石綿肺症、過敏性肺炎、特発性肺線維症、リンパ球性間質性肺炎、ランゲルハンス細胞肉芽腫症、嚢胞性線維症、膿胞性線維症、肺線維症、特発性肺線維症、線維化性肺胞隔炎、間質性線維症、びまん性肺線維症、慢性間質性肺炎、気管支拡張症、細気管支炎線維症、気管支周囲線維症、胸膜線維症など、腎臓などの泌尿器、生殖器系における男性性腺機能低下症、筋強直性ジストロフィー、ペイロニー病などによる線維症、慢性尿細管間質性腎炎、常染色体劣性嚢胞腎、骨髄腫腎、水腎症、急速進行性糸球体腎炎、腎毒性疾患、黄色肉芽腫性腎盂腎炎、鎌状赤血球腎症、腎性尿崩症、常染色体優性多発性嚢胞腎疾患、慢性糸球体腎炎、IgA腎症、腎硬化症、巣状糸球体硬化症、膜性腎炎、膜増殖性糸球体腎炎、慢性腎盂腎炎、腎アミロイドーシス、多発性嚢胞腎、後腹膜線維症、ループス腎炎など膠原病に伴う腎病変、糖尿病性腎症、慢性前立腺炎、住血吸虫症による膀胱炎、乳腺線維症、乳腺線維腺腫など、脊椎などの神経系における先天性斜頸、強直性脊椎炎、神経線維腫や脊髄損傷後の神経機能欠損等の脊髄疾患、パーキンソン病やアルツハイマー病等の脳神経疾患、眼球における後部水晶体線維化症、増殖性網膜症など、また、全身に病変の生じるサルコイドーシス、全身性エリテマトーデスによる線維症や全身性強皮症、多発性筋炎、皮膚筋炎など、術後の合併症、並びに糖尿病、癌等の種々の慢性炎症性疾患の治療剤として有用である。
また、Wynn TA. Cellular and molecular mechanisms of fibrosis. J Pathol 2008; 214: 199-210のTable 1に記載の疾患も本発明の治療剤の治療対象となり得る。
なお、本実施例において使用した「抗CHST15 siRNA」は、配列番号:3および4に記載されたRNAがハイブリダイズした構造のsiRNAである。
Dextran sulfate sodium (DSS)で誘発した慢性大腸炎モデルマウスに対する抗CHST15 siRNAの治療効果を検討した。本モデルは粘膜層ならびに粘膜下層に炎症性線維化病変を示すモデルであり、クローン病患者で認められる粘膜下層の肥厚を伴う炎症性線維化病変と合致する事から、同病変に対する治療効果の評価が可能である。
2.5% DSSの摂取開始から6日目(Day 5)より通常飲水に変更し、7日目(Day 6)にマウス大腸体積当たり25 nM、250 nM、2500 nMの抗CHST15 siRNAを、大腸粘膜下に投与した(図1)。12日目(Day 11)と20日目(Day 19)に剖検を行い、CHST15遺伝子発現及び大腸ハイドロキシプロリン量の測定を行った。試験条件の詳細を、以下の表1~6に示す。
体重スコア、下痢便スコア、血便スコアをDay0~Day6の間、毎日観察した。体重スコア、下痢便スコア及び血便スコアは、それぞれ表7、8、9に従い算出し、その合計値をDisease Activity Score(DAI)とした。一般状態はDay0~Day19の間、毎日観察した。
1. 採血方法
以下の表11の条件にて採血を行った。
(1)RNA抽出
遺伝子発現解析用の直腸サンプルをRNAlater(Lot No. 108K0483、SIGMA Alderich)に浸漬し、4℃にて保存した。このサンプルをRNAiso Plus(Takara)を用いて、ホモジナイズし、フェノール/クロロホルム処理後、採取したlysateをRNA fast pure kit(Lot No. 1001、Takara)のカートリッジに注入し、室温で8000 x g、1分間の遠心によりカートリッジに核酸を吸着させた。カートリッジに10% EtOH WB溶液 750 μLを注入し、室温で8000 x g、 1分間の遠心によりカートリッジを洗浄。EB溶液30 μLを注入し、室温で4分間インキュベートした後、室温で8000 x g、1分間の遠心により、カートリッジよりtotal RNAを溶出。得られたtotal RNAの吸光度測定値よりtotal RNA濃度を算出した(1 OD=40 μg/mL total RNA)。
Total RNA溶液を1000 ng/μLに希釈して定量用サンプルとし、random 6 merプライマーを用いて逆転写反応を下表に示す反応条件で行った。得られたcDNAを鋳型に標的遺伝子(マウスCHST15)に対するリアルタイムPCR用プライマーを用いて、相対定量リアルタイムRT-PCRを行った。検量線用に陰性対照群のcDNAを使用して、10 ng/μLから4倍の希釈系列を作製した。また、マウスβアクチンに対するプライマーを用いて、同一サンプルを同様に測定した。マウスCHST15遺伝子の発現については、βアクチン発現量で補正し、無処置群を1とした相対発現量で示した。リアルタイムRT-PCR反応は、SYBR Premix ExTaq II (Perfect Real Time)のプロトコールに従い、リアルタイムRT-PCR装置Thermal Cycler Dice Real Time Systemを用いて表12~13に示す反応条件で行った。
ハイドロキシプロリン測定用の直腸サンプルを計量後、2N NaOHを添加し、Block incubator (ASTEC)にて65℃で10分間加熱処理を行った。得られた腸溶解物より、10 μLをBCA assay kit (Lot No. LD144450、Thermoscientific)を用いてタンパク量を測定し、残りを121℃、20分間オートクレーブ処理して、熱アルカリ分解を行った。熱アルカリ分解サンプルに6N HClを添加し、121℃、20分間オートクレーブ処理して加水分解後、活性炭-4N NaOH溶液、酢酸-クエン酸緩衝液を添加して中和した。得られたサンプルを遠心し、上清を回収した。得られた上清にクロラミンT試薬を加えて混和し、室温で25分間反応させ、続いてエールリッヒ試薬を加えて混和し、65℃、20分間加熱、その後5分間氷冷した。4℃にて遠心後、得られた上清150 μLを96wellプレートに分注し、Power wave XS (Biotek)を用いて560 nmの吸光度を測定した。サンプル中のハイドロキシプロリン量は、ハイドロキシプロリンの標準溶液を用い作製した検量線より4-parametric法を用いて、算出した。得られたハイドロキシプロリン量をタンパク質濃度で補正した。
試験結果は平均値 ± SDで表示した。全ての検定には、Bonferroni multiple comparison test (PRISM software)を使用した。P <0.05の場合に統計学的に有意とした。
1. 一般症状
無処置群以外の全ての群で、Day 1~2以降に立毛が見られた。全個体に試験期間中、病態以外の一般症状の異常は見られなかった。
無処置群(N)と比較して、対照群及び陰性対照群(C)においては、Day 11およびDay 19ともにCHST15遺伝子の発現上昇が認められた。抗CHST15 siRNA投与群においては、陰性対照群の発現量と比較して、CHST15遺伝子発現の低下が認められた。Day 19においては、濃度依存性の傾向も認められた。
無処置群(N)におけるCHST15遺伝子発現量をベースライン(0%)として、陰性対照群(C)の平均値を100%と設定した時の抗CHST15 siRNAの平均ノックダウン効率(%)は表14の通りである。
無処置群(N)と比較して、対照群及び陰性対照群(C)においては、Day 11ならびにDay 19ともに大腸ハイドロキシプロリン量の増加傾向が認められた。Day 19において、抗CHST15 siRNA投与群(250 nM及び2500 nM)は大腸ハイドロキシプロリン量の低下が認められた。濃度依存性の傾向も認めていた。
陰性対照群(C)における大腸ハイドロキシプロリン量の平均値を100%と設定した時の抗CHST15 siRNAによる平均低下率(%)は表15の通りである。
陰性対照群においては、Day 6と比してDay 19でSegmental SES-CDスコアの減少傾向が認められた。Day 19においては、全濃度の抗CHST15 siRNA投薬群は陰性対照群よりもSegmental SES-CDスコアの有意な低下が認められた。濃度依存性は明確ではなかった。
2.5% DSSで誘発した慢性大腸炎モデルマウスにおいて、大腸粘膜下に投与した抗CHST15 siRNAの25、250、2500 nMの投与用量では、明確な濃度依存性を示さなかったものの、CHST15の遺伝子発現量の低下を認めた。抗CHST15 siRNAの250、2500 nMではDay 19における大腸ハイドロキシプロリン含量の低下が認められ、標的遺伝子であるCHST15遺伝子発現抑制による、抗線維化効果を示した。
Claims (6)
- CHST15遺伝子の発現を抑制するsiRNAであって、配列番号:1に記載の塩基配列からなるRNAと、該RNAと相補的な配列からなるRNAとがハイブリダイズした構造を含むsiRNA。
- 末端において1もしくは複数の核酸がオーバーハングした構造を有する請求項1に記載のsiRNA。
- 請求項1もしくは2に記載のsiRNAを発現し得るDNAベクター。
- 請求項1または2に記載のsiRNA、または請求項3に記載のDNAを有効成分として含む、粘膜治癒促進剤。
- 請求項1または2に記載のsiRNA、または請求項3に記載のDNAを有効成分として含む、クローン病もしくは潰瘍性大腸炎の治療剤。
- 請求項1または2に記載のsiRNA、または請求項3に記載のDNAを有効成分として含む、創傷治癒、粘膜治癒、もしくは潰瘍治癒のための医薬組成物。
