Definitions
- the present invention relates to a probe for inspecting the presence or absence of a lesion such as cancer and its progress by irradiating a measurement target site in a lumen with light and acquiring measurement light emitted from the measurement target site.
- Observation and diagnosis of a body lumen using an electronic endoscope is a diagnostic method that is currently widely used. Since this diagnostic method directly observes the body tissue, it is not necessary to excise the lesion, and the burden on the subject is small. Recently, in addition to a so-called video scope, diagnostic devices and ultrasonic devices utilizing various optical principles have been proposed and some of them have been put into practical use. In this way, introducing a new measurement principle or combining a plurality of measurement principles has been performed.
- a fluorescence image endoscope system has also been proposed in which a fluorescence image is acquired and displayed so as to overlap a normal visible image. Such a system is highly promising because it leads to early detection of malignant tumors.
- fluorescence is acquired without using an image sensor mounted on an electronic endoscope.
- diagnostic probes for performing such fluorescent diagnosis that is, probes that are in the body via the forceps channel of the endoscope, or that are integrated with the endoscope.
- Such a probe is generally composed of a first optical fiber group that guides light to irradiate the body tissue and a second optical fiber group that guides light generated from the body tissue, or one fiber. Configured to play both roles in a group.
- the side of the probe that is inserted into the body and faces the living tissue is configured to oppose the tissue via an optical window material such as a slide glass or an optical element such as a lens.
- an optical window material such as a slide glass or an optical element such as a lens.
- Patent Document 1 a technique has been proposed in which a holding portion for holding the optical window agent and the optical element from the probe tip side is provided (see, for example, Patent Document 1).
- the optical element 100 is probed by forming a catching part 120 on which the optical element 100 is caught on a holding part 110 that holds the optical element 100 on the probe tip side. To prevent falling off.
- a space 130 is generated between the distal end side surface 125 of the optical element 100 and the measurement target site of the biological tissue 120.
- liquids such as water, blood, and body fluid on the surface of the living tissue 120 may enter the space 130 and remain in the space 130. If the liquid remains in the space 130 as described above, there is a problem that the measurement result by the probe is adversely affected depending on the type of the remaining liquid.
- Patent Documents 2 and 3 have been proposed as techniques related to the above problem.
- the technique described in Patent Document 2 even if air is supplied and supplied from an air supply / water supply nozzle located on the distal end surface of the endoscope, and the liquid remaining in the observation window is evaporated and diffused, the liquid remains in surface tension.
- surface tension breaking means for example, a plurality of grooves for breaking the surface tension of the liquid discharged from the air / water feeding nozzle is provided in the space direction of the air / water feeding nozzle.
- Patent Document 3 when a foreign substance such as a body fluid adheres to the lens surface of the objective lens located on the distal end surface of the endoscope, a cleaning liquid such as water is applied from the water supply nozzle to the lens surface of the objective lens. It is a technology that effectively ejects foreign matter by ejecting. Specifically, vibration application means for applying minute vibrations to the distal end portion of the endoscope is provided. Thereby, the film-like foreign material adhering to the lens surface of the objective lens can be easily separated from the lens surface.
- Patent Documents 2 and 3 can be applied to a place where a large area can be ensured, such as the distal end side of an endoscope, the diameter is small as in the distal end side of the probe described above. Therefore, it has been difficult to apply as it is to a place where it is difficult to secure a large area.

Thereby, an endoscopic video based on the endoscopic video signal is displayed on the screen of the monitor 8. That is, when an observation target region in the lumen is imaged, the image is displayed on the monitor 8.
- the CPU 33 controls the operation of the illumination light source 31 and the video processing unit 32.
- FIG. 3 is a diagram showing the configuration of the distal end side of the probe 11 (also referred to as a tubular body, hereinafter the same).
- the probe 11 includes an excitation optical fiber 50, a light receiving fiber 52, and a lens 54 that functions as an optical member.
- the probe 11 may also include an optical filter that selectively transmits or blocks excitation light.
- the lens 54 is a hemispherical condensing lens having a positive refractive power.
- the lens 54 is fitted in front of the excitation optical fiber 50 and the light receiving fiber 52 toward the distal end side of the probe 11, and the optical axis coincides with the central axis of the probe 11.
- conventionally well-known lenses other than a hemisphere can be used.
- the excitation optical fiber 50 irradiates the measurement target site with the excitation light emitted from the light source 41.
- the light receiving fiber 52 receives, as measurement light, fluorescence generated from a measurement target site or a drug injected into the measurement target site in advance due to the excitation light emitted from the excitation optical fiber 50.
- the measurement light from the region to be measured may be scattered light or Raman scattered light instead of fluorescence.
- the probe 11 has a plurality of light receiving fibers 52.
- the probe 11 prevents the lens 54 from dropping from the probe 11 by forming a catching portion 58 on which the lens 54 is caught on the holding portion 56 that holds the lens 54 on the distal end side of the probe 11.
- the catch portion 58 of the holding portion 56 has a space having an inner diameter d2 (for example, 1.35 mm) smaller than the inner diameter d1 (for example, 2.6 mm) of the probe 11 at the tip of the probe 11.
- the depth of the space is, for example, 0.3 mm.
- FIG. 4A is a view of the probe 11 as viewed from the distal end side of the probe 11.
- 4B is a cross-sectional view taken along the line AA in FIG. 4A.
- one liquid discharge groove 61 that communicates between the space 60 and the outer peripheral side surface of the holding portion 56 of the probe 11 is formed.
- the liquid discharge groove 61 has a width of, for example, 0.3 mm and a depth of, for example, 0.3 mm. As a result, a path through which the liquid that has entered the space 60 is discharged from the space 60 can be secured.
- the number and width of the liquid discharge grooves communicating between the space 60 and the outer peripheral side surface of the holding portion 56 are not limited to the examples shown in FIGS.
- four liquid discharge grooves 61, 62, 63, and 64 that communicate between the space 60 and the outer peripheral side surface of the holding portion 56 may be formed at intervals of 90 degrees.
- the corners and sides of the liquid discharge grooves 61, 62, 63, 64 may be chamfered.
- the width of the liquid discharge grooves 61, 62, 63, 64 is wider. This is because if the width of the liquid discharge grooves 61, 62, 63, 64 is narrow, the liquid does not easily flow through the liquid discharge grooves 61, 62, 63, 64 due to the effect of surface tension. Therefore, if the liquid discharge grooves have the same area, it is preferable to provide a small number of wide liquid discharge grooves rather than a large number of thin liquid discharge grooves.
- the holding portion 56 having the space 60 having an inner diameter smaller than the inner diameter of the probe 11 is provided at the tip of the probe 11, and the holding portion 56 includes the space 60 and the holding portion.
- Liquid discharge grooves 61, 62, 63, 64 communicating with the outer peripheral side surface of 56 are formed.
- the distal end side of the probe 11 when the distal end side of the probe 11 is in close contact with the living tissue when the excitation light is irradiated to the measurement target site, the living tissue enters the space 60 and enters the space 60.
- the discharged liquid can be discharged from the space 60 through the liquid discharge grooves 61, 62, 63, 64. Therefore, it is possible to prevent the liquid from remaining in the space 60 generated between the lens 54 and the measurement target site of the living tissue, and to prevent the influence of the liquid from affecting the measurement result by the probe 11.
- the action of bringing the probe tip 11b into contact with the living tissue serves as a liquid diffusing function, and becomes a driving force for discharging the liquid when the living tissue enters the space 60. Therefore, it is not necessary to add a complicated mechanism such as a vibration mechanism or a nozzle in order to realize the liquid diffusion function.
- the probe tip portion 11b includes a catching portion 58 for holding the lens 54, but even if the liquid discharge grooves 61, 62, 63, 64 are provided in part, the function of preventing the catching portion 58 from falling off is sufficiently provided. Kept.
- the liquid discharge groove 65 may be formed in a tapered shape whose width decreases from the space 60 side toward the outer peripheral side surface of the holding portion 56.
- the liquid discharge groove 66 may be formed in a tapered shape whose width increases from the space 60 side toward the outer peripheral side surface of the holding portion 56. From the viewpoint of facilitating the flow of the liquid from the space 60, the liquid discharge groove 65 having a tapered shape whose width decreases from the space 60 side toward the outer peripheral side surface of the holding portion 56 is desirable.
- the face 58 a facing the measurement target portion in the holding portion 67 has a countersink-shaped tapered surface that becomes deeper inward in the radial direction of the probe 11. It may be formed. In this way, when the probe tip portion 11b comes into contact with the living tissue, the living tissue can easily enter the space 60, and the function of diffusing liquid by the living tissue can be sufficiently performed.
- a liquid discharge groove 68 whose bottom does not reach the end face of the lens 54 may be formed in the holding portion 56.
- the holding part 56 plays a role of ensuring watertightness or airtightness inside the probe 11 in order to prevent the lens 54 from dropping off and also prevent liquid from entering the probe 11. Therefore, it is not desirable that the liquid discharge groove portion occupies most of the tip of the probe 11.
- the liquid discharge groove 68 whose bottom does not reach the end face of the lens 54 is formed in the holding portion 56, so that the contact area between the holding portion 56 and the lens 54 is increased and the lens 54 is brought into close contact with the holding portion 56.
- the watertightness or airtightness inside the probe 11 can be reliably ensured.
- FIG. 10A is a view of the probe 11 as viewed from the distal end side of the probe 11.
- FIG. 10B is a GG cross-sectional view of FIG. 10A.
- FIG. 10C is a cross-sectional view taken along the line HH of FIG. 10A.
- the liquid discharge groove 70 may have a cross-sectional shape notched in a V shape whose width is narrowed toward the bottom.
- FIG. 11A is a view of the probe 11 as viewed from the distal end side of the probe 11.
- 11B is a cross-sectional view taken along the line II of FIG. 11A.
- FIG. 11C is a JJ cross-sectional view of FIG. 11A. In this way, it is possible to ensure a wide contact area between the lens 54 and the holding portion 56 while ensuring a wide width of the entire liquid discharge groove 70. Therefore, it is possible to ensure water tightness or air tightness inside the probe 11 while facilitating liquid flow.
- FIG. 12 shows the behavior of the liquid in the space 60 when the liquid discharge groove corresponding to FIG.
- the liquid that has entered the space 60 can be discharged from the space 60 through the liquid discharge grooves 61, 62, 63, 64.
- some of the liquids 71a, 71b, 71c, 71d remain in the portions where the liquid discharge grooves 61, 62, 63, 64 are not present.
- the axial end of the fiber disposed at the shifted position is the holding portion 56. It is desirable that the liquid discharge grooves 61, 62, 63, 64 on the space 60 side be disposed at positions corresponding to the openings.
- the axial ends of the pumping optical fiber 50 and the axes of the light receiving fibers 52a and 52c are used. It is preferable that the direction end portion is disposed at a position corresponding to the opening portion of the liquid discharge groove 63 on the space 60 side of the holding portion 56. As shown in FIG. 14, the axial ends of the pumping optical fiber 50 and the light receiving fiber 52 c are arranged at positions corresponding to the openings of the liquid discharge grooves 61, 62, 63, 64 on the space 60 side of the holding unit 56.
- the portions 56 are arranged side by side at positions corresponding to the openings of the liquid discharge grooves 62 and 64 on the space 60 side.
- the holding portion 56 may be formed with a liquid storage portion communicating with the liquid discharge groove 61.
- FIG. 15A is a view of the probe 11 as viewed from the distal end side of the probe 11.
- FIG. 15B is a cross-sectional view taken along line MM in FIG. 15A.
- FIG. 15C is a side view of the holding portion 56 of the probe 11.
- a liquid storage portion 72 communicating with the liquid discharge groove 61 is formed on the side surface of the holding portion 56.
- the liquid storage part 72 is wider than the liquid discharge groove 61. Therefore, it is possible to prevent the liquid from returning to the space 60 again due to the effect of the surface tension, because a part of the liquid discharged from the space 60 moves to the accumulation portion 72 and remains.
- the holding portion 56 may be formed with a liquid storage portion 73 that communicates with the liquid discharge groove 61.
- the convex portions 74a and 74b and the concave portions 75a, 75 b and 75 c are preferably in an angular shape in order to effectively obtain a liquid backflow prevention effect. Further, as shown in FIG. 17C, the same effect can be obtained by forming the projections 74a and 74b and the flat recess 75a on the bottom surface of the liquid discharge groove 61.
- FIG. 18A is a diagram illustrating a configuration of a distal end portion of a confocal endoscope.
- 18B is a cross-sectional view taken along the line PP of FIG. 18A. As shown in FIG.
- a tubular distal end protruding portion (confocal scanning portion) 90 that functions as the probe 11, forceps channel 91, air / water feeding nozzle 92, light guide 93, a CCD camera 94, and the like.
- a slide glass 90b as an optical member is fitted in the tip of the tip protrusion 90.
- a holding portion 90 a having a space 90 c having an inner diameter smaller than the inner diameter of the tip protrusion 90 is provided at the tip of the tip protrusion 90.
- a catching portion 120 is formed on the holding portion 90a, and the slide glass 90b is prevented from falling off from the tip protruding portion 90.
- a space 90 c is generated between the flat side surface of the slide glass 90 b and the observation target portion of the living tissue 95.
- the holding portion 90a is formed with any of the above-described liquid discharge grooves that communicate between the space 90c and the outer peripheral side surface of the tip protrusion 90.
- the present invention is not limited to this.
- it may be applied to the above-described confocal endoscope, or the probe 11 may be inserted into the body alone.
