ペグインターフェロンおよびリバビリンの併用療法の、慢性C型肝炎治療における治療効果を予測する方法であって、 生体試料における薬剤感受性マーカーを検出する薬剤感受性マーカー検出工程と、前記薬剤感受性マーカーの発現レベルに基づいて、前記生体試料の薬剤感受性を評価する薬剤感受性評価工程とを含み、 前記生体試料が、肝臓由来の細胞または組織であり、 前記薬剤感受性マーカーが、配列番号:1から37に記載のマイクロRNAからなる群より選択される少なくとも1つ一つのマイクロRNAを含むことを特徴とする方法。
Description
本発明は、慢性C型肝炎の治療効果予測方法に関する。
C型肝炎ウイルス感染者は全世界に約3%存在している。C型肝炎ウイルスに感染すると高率に慢性肝疾患(慢性肝炎や肝硬変)に移行し最終的に肝癌に至る(非特許文献1)。現在の標準的な慢性C型肝炎の治療方法はペグインターフェロンおよびリバビリンの併用療法であるが、日本人に多いC型肝炎ウイルスの遺伝子型1b感染者では、その有効率が約50%にとどまっている。その理由として、この併用療法の副作用があげられる(非特許文献2)。そのため慢性C型肝炎において、新しい治療方法の開発がのぞまれている。
マイクロRNAはタンパク質に翻訳されない小分子RNAであり、塩基配列特異的にメッセンジャーRNAと結合し、遺伝子発現を調節している。マイクロRNAは細胞の発育などに深く関与している(非特許文献3、4)。特にマイクロRNA-122(以下miR-122)は肝臓の中のマイクロRNAの中で70%を占めている(非特許文献5)。インターフェロンベータを投与すると細胞の中のマイクロRNAの発現量が変化することが知られている。これらマイクロRNAのうち8種類のマイクロRNAはC型肝炎ウイルスの複製を制御する可能性を持っている(非特許文献6)。今までにいくつかのマイクロRNAがC型肝炎ウイルスの複製を抑制することが報告されている(非特許文献6~8)。miR-122はインターフェロン治療をした際の早期のインターフェロン応答と関係しているとされている(非特許文献9)。
インターフェロン治療の効果予測は今までにも試みられている。C型肝炎ウイルスの遺伝子型1bに感染している症例では、インターフェロン感受性領域(ISDR)の塩基配列の変異の有無がインターフェロン治療に影響している(非特許文献10)。またC型肝炎ウイルスのコアと呼ばれる領域の70番目のアミノ酸と91番目のアミノ酸の変異の有無が、インターフェロン治療に影響するとの報告がある(非特許文献11)。
病気の診断や薬物の効果予測において、マイクロアレイによる包括的な遺伝子発現解析は有用である。特に慢性関節リュウマチ(非特許文献12)、全身性ループスエリテマトーデス(非特許文献13)などで、このような解析についての報告がある。また悪性疾患では、乳がんの診断、遠隔転移の有無(非特許文献14、15)、治療前にある種の薬剤が対象疾患に効くか否かの評価などにおいて、マイクロアレイによる解析の報告が見られる(非特許文献16、17)。

本発明は、このような状況に鑑みてなされたものであり、その目的は、慢性C型肝炎に対する、ペグインターフェロンおよびリバビリンの併用療法の治療効果を予測する新規な方法を提供することにある。
本発明者らは、上記課題を解決すべく鋭意検討を行った結果、特定のマイクロRNAの発現が、慢性C型肝炎患者におけるペグインターフェロンおよびリバビリンの併用療法の治療効果と強い相関を示すことを見出し、本発明を完成するに至った。

また、本発明は、好ましい態様において、前記薬剤感受性マーカーを検出する工程における薬剤感受性マーカーの検出の方法が、マイクロアレイ解析であることを特徴とする方法を提供する。
さらに、本発明は、他の好ましい態様において、前記薬剤感受性を評価する工程における薬剤感受性マーカーの評価の方法が、モンテカルロ交差評価法(Monte Carlo Cross Validation)であることを特徴とする方法を提供する。
本発明によれば、前記薬剤感受性マーカーの発現量を指標として、簡便に、ペグインターフェロンおよびリバビリンの併用療法の、慢性C型肝炎治療における治療効果を予測することができる。特に、本発明者らにより見出されたマイクロRNAの発現パターン解析によれば、ペグインターフェロンおよびリバビリンの併用療法の有効性を、高い信頼性で治療前に判定することができる。また、マイクロRNA発現パターンを解析することにより、肝炎の進行などの病態を理解する道筋の一つを提供することができる。
以前よりウイルスの遺伝子型、ウイルス量、肝線維化の程度、年齢、体重、人種、肝組織中の脂肪の程度などを指標とした慢性C型肝炎治療の効果予測が試みられていた。また慢性C型肝炎の治療期間は48週にわたるために、治療中に本治療が有効であるか否かを判定する試みもなされている。治療開始後12週間で血中のウイルスが消失するものは60%の確率で著効例(SVR)になるとの予測がある。前述のようにインターフェロン感受性領域の変異、コア領域の70番目および91番目のアミノ酸変異も慢性C型肝炎の治療効果予測の指標として用いられている。マイクロRNAの発現を指標とする本発明の方法は、モンテカルロ交差評価法を用いて効果予測を行なったところ70.5%の正確性を持つことが判明し、これら従来の方法よりも正確性が高い。ペグインターフェロンおよびリバビリンの併用療法は治療期間が48週間と長期にわたること、費用がかかること(一般的な健康保険で30%負担の場合月額6万円以上)、副作用の発現で50%程度の患者に無効例がでてしまうことを考えると、より正確な診断を可能とする本発明の方法の利点は大きい。
また、本発明の方法における薬剤感受性の評価においてモンテカルロ交差評価法を利用した場合、解析数が増えるにつれて正確性が上昇するため(図2参照のこと)、大規模な検体の解析を行う場合に、さらなる精度の高さが期待できる。
一般的にマイクロアレイ解析はヒト遺伝子約20000個の発現を同時に解析できる。これまでに様々な臨床情報とマイクロアレイ解析とを組み合わせた研究がなされてきた。しかしながら、対象とする遺伝子数が多くなった場合、解析に必要な試料数を増やさないと十分な解析ができないなどの欠点があった。現在知られているヒトマイクロRNAの数は706個であり、解析対象となる遺伝子数は、従来の網羅的なヒト遺伝子解析の場合と比較して少ない。このため、マイクロRNAをマイクロアレイにて解析すれば、試料数が少なくても十分に解析ができる点で有利である。マイクロRNAの発現量は個体によりその差が少ないことも、マイクロアレイにてマイクロRNAを解析する利点である。本発明の方法におけるマイクロRNAの発現の検出に、マイクロアレイ解析を用いると、このような利点がもたらされる。

以下、本発明を実施例により説明するが、本発明は実施例に制限されるものではない。
(実施例1)
(解析対象患者)
99例の慢性C型肝炎(遺伝子型1bに感染)している患者を解析対象とした(表1)。これらの患者は自己免疫性肝炎、アルコール性肝障害、B型肝炎、HIV感染社ではないことを確認した。また、これらの患者は今までにインターフェロン治療や免疫抑制剤を用いた治療は受けていないことを確認している。血清のHCVRNAはAmplicor-HCV monitor(Roche社製)を用いて測定した。肝組織の炎症および線維化をMetavirの分類(J Hepatol 1996;25:649-654、Am J Clin Pathol 2000;114:712-718)を用いて評価した。インターフェロン治療前に血中AST、ALT、総ビリルビン、ALP、γ-GTP、ヘモグロビンの値、白血球数、および血小板数を測定した。この臨床研究は京都大学の医の倫理委員会にて倫理的に問題がないことの承認を受けており、この臨床研究の内容はヘルシンキ宣言に準拠している。
(解析対象患者)

ペグインターフェロン(ペグ化したインターフェロンアルファ-2b;シェリングプラウ社製)とリバビリン(シェリンクプラウ社製)を48週間投与した。ペグインターフェロンは体重1kgあたり1マイクログラムを週に1度投与した。リバビリンはシェリングプラウ社の推奨量を投与した。治療効果の判定は以下の3群に分けて行った。

SVR(著効例);ペグインターフェロンとリバビリン投与終了後6ヶ月にわたり、血液中からHCVRNAが検出できなかったもの。
TR(再発例);ペグインターフェロンとリバビリン投与終了直後は血液中からHCVRNAが検出できなかったが、その後の6ヶ月以内に、血液中からHCVRNAが検出できたもの。
NR(無効例);経過中持続して血中HCVRNAが検出できたもの。
(全RNAの抽出方法)
肝生検にて患者から肝臓の組織を採取し、採取した組織をRNA later液(Ambion社製)に浸し、-80℃にて保存した。全RNAの採取はmirVana miRNA Isolation Kit(Ambion社製)を用いて行なった。
(マイクロRNA用のマイクロアレイ)
マイクロRNA用マイクロアレイ(Agilent社製)を用いて解析を行なった。前述の方法で抽出した全RNA100ngをHuman microRNA Microarray kit(Agilent社製)を用いて、version1.5の説明書に従って標識し、アレイとハイブリダイズ(塩基配列に従って結合)を行なった。ハイブリダイズした標識したRNAを検出するためにG2505Bスキャナー(Agilent社製)を用いた。ここで得られた情報は、Agilent feature extraction software(v9.5.3.1)(Agilent社製)を用いて数量化し解析を行なった。
(治療効果予測計算方法)
モンテカルロ交差評価(Monte Carlo Cross Validation)を用いて行なった(非特許文献18、19)。
(慢性C型肝炎ウイルスのコア領域のアミノ酸変異とインターフェロン感受性領域の塩基配列変異の検出方法)
インターフェロン治療の効果予測は今までに試みられている。慢性C型肝炎ウイルスの遺伝子型1bに感染している症例では、インターフェロン感受性領域(ISDR)の塩基配列の変異の有無がインターフェロン治療に影響している(非特許文献10)。また、慢性C型肝炎ウイルスのコアと呼ばれる領域の70番目のアミノ酸と91番目のアミノ酸の変異の有無がインターフェロン治療に影響するとの報告もある(非特許文献11)。
(統計学的処理)
解析は、Studentのt検定を用いて任意の二群間の群別解析を行なった。またKruskal-Wallis検定とFisherの正確確率検定を用いて三群間の群別解析を行なった。P<0.01以下のものを有意差があると判定した。
(全RNAの抽出方法)

[実施例1]
マイクロRNAマイクロアレイを用いて99例の慢性C型肝炎患者の肝組織のマイクロRNA発現解析を行なった。ペグインターフェロンおよびリバビリンの併用療法における治療効果により分けたそれぞれの群(SVR、TR、NR)毎にマイクロRNAの発現パターンを解析した。SVR群とNR群で発現に差があるマイクロRNAを同定した。
マイクロRNAマイクロアレイを用いて99例の慢性C型肝炎患者の肝組織のマイクロRNA発現解析を行なった。ペグインターフェロンおよびリバビリンの併用療法における治療効果により分けたそれぞれの群(SVR、TR、NR)毎にマイクロRNAの発現パターンを解析した。SVR群とNR群で発現に差があるマイクロRNAを同定した。
図1に治療効果により分けた各群間で発現量に差があったマイクロRNAを示した。この図で縦軸は相対的なマイクロRNAの発現量を示し、それぞれのマイクロRNAの発現量の変動を±1標準偏差で示した。また、それぞれのマイクロRNAの発現量はStudentのt検定で解析し、p<0.01以下のものを採用した。本解析の結果、インターフェロン治療の有効性が高い群であるほど発現量が上昇したマイクロRNAは4種、逆に発現量が低下したマイクロRNAは33種認められた。なお、インターフェロン治療の有効性の高さは、NR<TR<SVRである。以上より、これらのマイクロRNAはペグインターフェロンおよびリバビリンの併用療法の有効性に応じて変化するマイクロRNAであることが判明した。
[実施例2]
マイクロRNAの発現パターンを利用して、ペグインターフェロンおよびリバビリンの併用療法を行なう前に治療対象患者がこの治療に有効であるか否かの予測を試みた。

図2にその結果を示す。マイクロRNA35種の発現パターンを、モンテカルロ交差評価法を用いて解析した。左の図はSVRと非SVR(TR+NR)を示す。トレーニングセットを80に設定し、残りの19個の生体試料を用いて予測を行なった結果、正確性70.5%、特異性63.3%、感度76.8%をもってSVRと非SVRを区別することができた。右の図はTRとNRの予想を試みたもので、トレーニングセットを42に設定して予測を行ったところ、正確性70.0%、特異性73.7%、感度67.5%を示した。以上の結果より、本発明の治療効果予測方法によれば、ペグインターフェロンおよびリバビリンの併用療法を行なう前に、ペグインターフェロンおよびリバビリンの併用療法の有効例および無効例を高い信頼性でおよび診断することができることが判明した。
[比較例1]
本発明の治療効果予測方法の有用性を示すために、従来用いられている治療効果予測方法との比較検討を行った。血液中に存在しているC型肝炎ウイルスのインターフェロン感受性領域の変異の有無がペグインターフェロンおよびリバビリンの併用療法の効果予測と関係しており、インターフェロン感受性領域に塩基配列上変異があるものがインターフェロン治療に有効であり、変異がないものは有効ではない(非特許文献10)。
本発明の治療効果予測方法の有用性を示すために、従来用いられている治療効果予測方法との比較検討を行った。血液中に存在しているC型肝炎ウイルスのインターフェロン感受性領域の変異の有無がペグインターフェロンおよびリバビリンの併用療法の効果予測と関係しており、インターフェロン感受性領域に塩基配列上変異があるものがインターフェロン治療に有効であり、変異がないものは有効ではない(非特許文献10)。
表2~5に80例のインターフェロン感受性領域の変異の有無を示した。変異がないものは「W」、変異が1または2個あるものは「IM」、変異が3個以上あるものは「M」と記載している(表3~5)。この指標に関して、本発明の予測方法を用いた時の正確性は56.6%、特異性は50.0%、感度は60.6%であった(表2)。
血液中に存在しているC型肝炎ウイルスのコア領域のアミノ酸の変異は、ペグインターフェロンおよびリバビリンの併用療法の効果予測と関係している。コア領域のと70番目のアミノ酸に変異がある場合(M)は治療効果は望めない、コア領域の91番目のアミノ酸の変異がある場合(M)も治療効果は望めない、コア領域の70番目のアミノ酸に変異がない場合(W)は治療に反応する、また91番目のアミノ酸の変異がない場合(W)も治療に反応する(非特許文献11)。この指標に関して、本発明の予測方法を用いた結果、70番目のアミノ酸については、正確性が62.5%、特異性86.5%、感度41.9%であり、91番目のアミノ酸については、正確性が47.5%、特異性が78.4%、感度が20.9%であった(表2)。表3~5では70番目のアミノ酸は変異がないものを「R」、91番目のアミノ酸の変異がないものを「L」と示した。それ以外の標記のものはすべて変異がある。
本発明の治療効果予測方法によれば、生体試料中の薬剤感受性マーカーとしてのマイクロRNAび発現レベルを検出することによって、例えば、ペグインターフェロンおよびリバビリンの併用療法の有効性を高い信頼性で判断し、適切な治療法を選択することが可能になる。同一の疾患であっても個人個人の疾患の状態によって、治療内容を変えていくことは治療効果の点からも重要であり、本発明は、テーラーメード医療への応用が期待できる。
また、本発明において、マイクロRNAの発現パターンが治療効果別に異なっていることが判明したため、これを利用して新たな遺伝子治療の可能性が考えられる。例えば、無効例(NR)において、発現が低下しているマイクロRNAの発現を亢進させることにより、ペグインターフェロンおよびリバビリンの併用療法が有効になる可能性がある。
また、慢性C型肝炎のマイクロRNAの発現パターンは、異常なマイクロRNAの発現がどのように肝炎の進行に関与しているかを理解するのに有用な情報ともなるため、本発明は、新たな肝炎の治療方法の開発にも貢献しうる。

- ペグインターフェロンおよびリバビリンの併用療法の、慢性C型肝炎治療における治療効果を予測する方法であって、
生体試料における薬剤感受性マーカーを検出する工程と、前記薬剤感受性マーカーの発現レベルに基づいて、前記生体試料の薬剤感受性を評価する工程とを含み、
前記生体試料が、肝臓由来の細胞または組織であり、
