Abstract

Les endoprothèses sur de l'os spongieux ont soit une face inférieure relativement lisse et présentent une stabilité primaire sûre, soit une structure rugueuse qui favorise la croissance de l'os mais n'offre pas de stabilité primaire satisfaisante. L'invention prévoit une structuration de la surface de contact avec l'os des endoprothèses dans au moins deux éléments de surface différents, à savoir dans des surfaces de contact primaires lisses qui entraînent une stabilité primaire sûre et génèrent un minimum de résistance à la friction lors de l'enfilement de la prothèse sur l'os, ainsi que dans des surfaces d'ostéointégration, situées dans des dépressions (par exemple, des rainures) et ayant une profondeur d'aspérité importante, qui favorisent l'ostéointégration par leur structure et/ou leur revêtement. Une cupule de prothèse de resurfaçage de hanche (1b) est subdivisée en surface de contact primaire (2a) et en surfaces d'ostéointégration évidées (4) par des rainures circulaires dans cet exemple, dans sa zone de bord (1g) réalisée coniquement sur la surface interne ainsi qu'au niveau de la transition du cône à la zone sphérique (1e). La transition de la surface de contact primaire au bord de la surface d'ostéointégration est arrondie (2d) pour faciliter le glissement de la surface de contact primaire sur l'os spongieux. Ce type de réalisation de la surface de contact avec l'os est particulièrement avantageux pour les endoprothèses qui sont implantées sur ou dans de l'os spongieux.
