Abstract

Description
本発明は、糖鎖分析を用いた関節リウマチの診断方法に関する。
関節リウマチ(RA)は関節の滑膜組織を病変の主座とする慢性炎症性疾患であり、有病率が人口の約1%を占める疾患である。RAは、初期には滑膜炎を来し、次第に軟骨や骨が侵され、進行すると関節が破壊され変形する。軟骨組織のプロテオグリカンとコラーゲンを分解する多数のタンパク質分解酵素(例えばマトリックスメタロプロテイナーゼ(MMP)、カテプシン、ペプチダーゼ等)の濃度上昇がそのような組織において見られることが知られている(非特許文献1、2参照)。さらに、インターロイキン-1β(IL-1β)、腫瘍壊死因子-α(TNF-α)、種々のサトカイン及び酵素阻害因子もまたRAの病因に重要な役割を果たすことが知られている(非特許文献3,4参照)。
これらのタンパク質は膝、関節部と複雑に関与しながら、慢性的炎症から関節の破壊に至るまで病状を増悪化する。RA患者の薬物治療には、非ステロイド系の抗炎症剤(NSAID)、抗リウマチ薬剤(DMARD)及び生物学的薬剤(抗TNFレセプター及びIL-1拮抗剤)等が用いられている(非特許文献5-7参照)。しかしながら、懸命の治療にもかかわらず、多くのRA患者の病状は回復せず痛みを感じ続け、骨破壊を受け続ける。期発見と早期治療がRA患者の生活の質を改良する上からも重要である。
これらのタンパク質は膝、関節部と複雑に関与しながら、慢性的炎症から関節の破壊に至るまで病状を増悪化する。RA患者の薬物治療には、非ステロイド系の抗炎症剤(NSAID)、抗リウマチ薬剤(DMARD)及び生物学的薬剤(抗TNFレセプター及びIL-1拮抗剤)等が用いられている(非特許文献5-7参照)。しかしながら、懸命の治療にもかかわらず、多くのRA患者の病状は回復せず痛みを感じ続け、骨破壊を受け続ける。期発見と早期治療がRA患者の生活の質を改良する上からも重要である。
RAは上記したように重篤にいたる疾患でありながら、現在のRAの診断方法は、アメリカリウマチ学会(ACR)の「ACR改訂診断基準」に基づくものである。該診断基準は、
1)1時間以上続く朝のこわばり(主に手指);
2)3箇所以上の関節腫れ;
3)手の関節(手関節、中手指節関節、近位指節間接)の腫れ;
4)対称性の関節(左右同じ関節)の腫れ;
5)手のエックス線写真の異常所見;
6)皮下結節、及び
7)血液検査でリウマチ反応が陽性、
であって、このうち、4項目以上満たせばRAと診断される。

ここで前記7)の血液検査は、血清中のリウマチ因子(RF)が主として測定される。RFは、RA患者の血清中に高頻度で出現するIgGのFc部分に対する自己抗体である。健常人のRFはIgG以外の種々のタンパクにも反応する、反応性の多様な抗体(polyactive antibody)であるが、RA患者由来のRFはこの反応性の多様な抗体以外にIgGFcのみに反応する、単一反応性の抗体(monoactive antibody)が存在する。この単一反応性のRFは、反応性の多様な抗体に比べ、IgGFcに対する反応が約100倍高い。このため、一般には該RFがRA患者の判定の指標として測定される。
しかし、RA患者でも常にRFが検出されない血清反応陰性RAも存在し、また健常人でも陽性反応(2%程度)を呈しうることが知られている。リウマチ因子以外の自己抗体の検出の早期診断における有用性も報告されており、そのような自己抗体としては、例えば、抗Sa蛋白抗体、抗核周辺因子(perinuclear factor)抗体、抗ケラチン抗体、抗フィラグリン抗体、抗環状シトルリン化ペプチド(抗CCP)抗体などが知られている(非特許文献8参照)。抗CCP抗体はRA患者血清の約76%から検出されるが、リウマチ様症状を呈するがRAとは判定されない患者血清でも約4%検出される (非特許文献9および10参照)。RA患者の滑膜にはシトルリン化されたタンパクが検出されることから、シトルリンを含むエピトープのRAの病原性への関与が指摘されている(非特許文献11および12参照)が、シトルリンを含むエピトープは十分に明らかにされていない。
他にもRA患者の尿を、液体クロマトグラフィーを用いてRA特有成分のクロマトピークの存在を以ってRAであると判定する方法(特許文献1参照)、糖タンパク質から糖鎖を切り出し、該糖鎖を標識した後精製して糖鎖分析用試料を調製し、その調製した試料をODS-シリカカラムを用いた高速液体クロマトグラフィーで分析し、試料の糖鎖組成を求めることにより、糖鎖組成変化に基づきRAを診断する方法(特許文献2参照)等が提案されているが、これらの方法は分析用試料の調製が煩雑であり、かつ感度・特異度の点で満足すべきものではなかった。

上述の診断法の中で、抗CCP抗体は特異度、感度の点から最も有望視されているが、なお特異度は約76%にとどまる。単独で、または併用により鋭敏にかつ特異性および感度の高い早期診断の可能な、あるいは、予後予測可能な新たなバイオマーカーを開発することが強く求められている。
本発明者は既に開発された高速網羅的糖鎖エンリッチ技術〔西村等(Nishimura, S, K Niikura, M Kurogochi, T Matsushita, M Fumoto, H Hinou, R Kamitani, H Nakagawa, K Deguchi, N Miura, K Monde, and H Kondo)「ハイスループット・プロテイングライコミクス:化学選択的グリコブロッティングとマルディトフ/トフ質量分析との併用」“High-Throughput Protein Glycomics: Combined Use of Chemoselective Glycoblotting and Maldi-Tof/Tof Mass Spectrometry.” Angew Chem Int Ed Engl 44, no.1 (2004): 91-96〕に基づき調製された血清中糖鎖をMALDI-TOF MS等の質量分析技術により各罹患状態の患者血清を大規模解析した。そして各患者の血清中糖鎖の定量的発現プロファイルをもとに各罹患状態を結びつけ、変動する糖鎖を見出し、新たなバイオマーカーとなる血清糖鎖あるいは、変動する糖鎖を有する糖タンパク質あるいは変動する糖鎖の生合成経路に関わる分子群を同定し、感度、特異度の高い診断法を確立するに至った。
本発明は、

本発明によれば、感度ならびに特異度において従来法を上回る、あるいは相補的な優位性を有した診断法が提供され、関節リウマチ患者の早期診断が可能となる。更には、早期診断の導入により、適確な医療処置を施すことが可能となり、関節リウマチの治療効果および予後の改善に貢献することが期待できる。
本発明によれば、被験者血清からグリコブロッティング(Glycoblotting)法により血中の全N-型糖鎖を回収し、MALDI-TOF MSにより定量的プロファイルを取得する。このとき、内部標準として既知量のオリゴ糖を添加しておけば、そのスペクトル上の面積より、他の血清由来糖鎖量を容易に算出できる。検出されるオリゴ糖は例えば表1に示すとおりである。
その後、一つまたは二つ以上の糖鎖発現量または糖鎖発現量の増減から関節リウマチの罹患を推定し、同疾患の診断および予後の管理・予測に有用な方法を得ることができる。
例えば、バイセクティングGlcNAcを持つ4種類の糖鎖(No.7、25、29および38)については、関節リウマチ患者において増加する傾向が見出される。トリシアリル構造を有するNo.43およびフコースの付加したNo.45についても関節リウマチ患者において顕著な増加がみられる。また血清中のN-結合型糖鎖総量および主要な糖鎖であるNo.31に関してもリウマチ患者において有意な発現量の増加を認める。一方、No.5の糖鎖が関節リウマチによって増加する傾向のあることは既に報告されているが、No.5にガラクトースが結合したNo.18は健常人よりもむしろ減少する傾向が見られる。
特にNo.45は最も有意に健常人とリウマチ患者を識別可能(P<0.0005)で、ROCカーブは既存の関節リウマチのバイオマーカーであるRFよりも明らかに優れており、現在最も有用性が認められている抗CCP抗体と比較しても同程度またはそれ以上に有用である。
例えば、バイセクティングGlcNAcを持つ4種類の糖鎖(No.7、25、29および38)については、関節リウマチ患者において増加する傾向が見出される。トリシアリル構造を有するNo.43およびフコースの付加したNo.45についても関節リウマチ患者において顕著な増加がみられる。また血清中のN-結合型糖鎖総量および主要な糖鎖であるNo.31に関してもリウマチ患者において有意な発現量の増加を認める。一方、No.5の糖鎖が関節リウマチによって増加する傾向のあることは既に報告されているが、No.5にガラクトースが結合したNo.18は健常人よりもむしろ減少する傾向が見られる。
特にNo.45は最も有意に健常人とリウマチ患者を識別可能(P<0.0005)で、ROCカーブは既存の関節リウマチのバイオマーカーであるRFよりも明らかに優れており、現在最も有用性が認められている抗CCP抗体と比較しても同程度またはそれ以上に有用である。
関節リウマチ患者14名および健常人11名の血清の糖鎖について解析をおこなった。今回用いた血清はすべて性別が女性で、ともに平均年齢が56歳であった。西村らのグリコブロッティング(GlycoBlotting)法により血中の全N-結合型糖鎖を回収し、MALDI-TOF MSにより定量的プロファイルを取得した。
関節リウマチ患者および健常人のN-結合型糖鎖のMALDIスペクトルを図1に示す。また、健常人および関節リウマチ患者における糖鎖発現量を図2に示す。各糖鎖の発現量は、内部標準として加えた既知量オリゴ糖とのスペクトル上の面積比より算出した糖鎖の定量値を用いた。関節リウマチ患者ではNo.7、25、29、31、38、43および45の各糖鎖において発現量の有意な増加が、No.18の糖鎖においては発現量の有意な減少がそれぞれ認められた。また、血清のN-結合型糖鎖の総量についても、関節リウマチ患者における有意な増加が認められた。
関節リウマチ患者および健常人のN-結合型糖鎖のMALDIスペクトルを図1に示す。また、健常人および関節リウマチ患者における糖鎖発現量を図2に示す。各糖鎖の発現量は、内部標準として加えた既知量オリゴ糖とのスペクトル上の面積比より算出した糖鎖の定量値を用いた。関節リウマチ患者ではNo.7、25、29、31、38、43および45の各糖鎖において発現量の有意な増加が、No.18の糖鎖においては発現量の有意な減少がそれぞれ認められた。また、血清のN-結合型糖鎖の総量についても、関節リウマチ患者における有意な増加が認められた。
図3、4は健常人および関節リウマチ患者において、各検体における糖鎖発現分布を示す箱ひげ図であって、統計解析ソフト・スポットファイアー(SpotFire Decision Site for Functional Genomics, Spotfire, Inc.)により作成した。
図5には、既存のRAバイオマーカー(RF、抗CCP抗体およびMMP3)およびNo.45のROC曲線を示す。
ここで、ROCとは、受信者動作特性(Receiver Operation Characteristic)の略であって、本実施例では患者とそうでない者の2群で、対象となるマーカーを測定し、得られた測定値ごとに、感度、特異度を算出した。「感度」とは「疾患を持っている者について陽性の結果が得られる割合」を、「特異度」とは「疾患を持っていない者について陰性の結果が得られる割合」を、意味する。横軸に「1から特異度を減じた値(偽陽性:false positive,FP)」を、縦軸に「感度(真陽性true positive, TP)」をプロットし、点を繋いで得られる曲線がROC曲線である。左上の隅に曲線が近いほど、誤診が少なくかつ真性度の高い、即ち診断効率の高い優れたマーカーと判断される。これを定量化したものがROC曲線のAUC(曲線下面積)で、最大値(1)に近いほど優れたマーカーといえる。試験した既存のRAバイオマーカー三種および糖鎖No.45について得られたROC曲線のAUCを下表2に示す。
図5には、既存のRAバイオマーカー(RF、抗CCP抗体およびMMP3)およびNo.45のROC曲線を示す。

血清中に高濃度に含まれる6種類のタンパク質(アルブミン、IgG、IgA、トランスフェリン、アンチトリプシン、ハプトグロビン)に対する抗体カラムを用いて、血清の高濃度タンパク質を含む画分(HAP)、およびHAPを除去した中~低濃度タンパク質を含む画分(LAP)を調製し、グリコブロッティング(GlycoBlotting)法によりそれぞれのN-結合型糖鎖を回収し、MALDI-TOF MSにより定量的グライコームプロファイルを健常人と関節リウマチ患者で比較した。
各糖鎖の発現量は、スペクトル上に認められた個々の糖鎖のシグナルの面積を検出された全ての糖鎖のシグナルの面積の和で除した面積比より算出した相対値を用いて比較した。両側t検定で、健常と関節リウマチ患者の個々の糖鎖の発現量を比較した結果、関節リウマチ患者HAPではNo.25、41、および45の各糖鎖において発現量の有意(p<0.01)な増加が、10、18、27の糖鎖においては発現量の有意な(p<0.01)減少がそれぞれ認められた。LAPではNo.45、46、48の各糖鎖において発現量の有意な(p<0.01)増加が、No.1、8、10、11、17、18、21、24の糖鎖においては発現量の有意な(p<0.01)減少がそれぞれ認められた。有意な発現変動が認められた糖鎖について、健常人および関節リウマチ患者間の糖鎖発現分布を示す箱ひげ図を図6、7に、LAPで最も有意に罹患に伴い発現量が変動した糖鎖(No.45)のROC曲線を図8に、有意な発現変動が認められた糖鎖について得られたROC曲線のAUCを下表3に示す。また、既存のRAバイオマーカー3種および糖鎖No.45について得られたROC曲線のAUCを下表4に示す。
各糖鎖の発現量は、スペクトル上に認められた個々の糖鎖のシグナルの面積を検出された全ての糖鎖のシグナルの面積の和で除した面積比より算出した相対値を用いて比較した。両側t検定で、健常と関節リウマチ患者の個々の糖鎖の発現量を比較した結果、関節リウマチ患者HAPではNo.25、41、および45の各糖鎖において発現量の有意(p<0.01)な増加が、10、18、27の糖鎖においては発現量の有意な(p<0.01)減少がそれぞれ認められた。LAPではNo.45、46、48の各糖鎖において発現量の有意な(p<0.01)増加が、No.1、8、10、11、17、18、21、24の糖鎖においては発現量の有意な(p<0.01)減少がそれぞれ認められた。有意な発現変動が認められた糖鎖について、健常人および関節リウマチ患者間の糖鎖発現分布を示す箱ひげ図を図6、7に、LAPで最も有意に罹患に伴い発現量が変動した糖鎖(No.45)のROC曲線を図8に、有意な発現変動が認められた糖鎖について得られたROC曲線のAUCを下表3に示す。また、既存のRAバイオマーカー3種および糖鎖No.45について得られたROC曲線のAUCを下表4に示す。
健常人とリウマチ患者を最も有意に識別可能な糖鎖No.45によって修飾されている血清タンパク質を健常人血清から同定した。
血清タンパク質を還元アルキル化後、トリプシン消化し、コンカナバリンAカラムにアプライした。コンカナバリンAカラム非吸着画分をニホンニワトコレクチンカラムにアプライし、ラクトースで溶出した。ニホンニワトコレクチンカラム吸着画分を逆相HPLCカラムで分画し、N-グリコシダーゼF処理で糖鎖を遊離させた。N-グリコシダーゼF処理前後のサンプルをMALDI-TOF MSを用い解析し、糖鎖No.45の質量分のシフトが認められたペプチドのMALDI-TOF/TOFスペクトルを得、MASCOTで検索した。
糖鎖No.45を有するタンパク質の同定例を図9に示した。同定した糖鎖No.45を有するタンパク質および同タンパク質のトリプシン消化物ペプチド配列を下表5に示す。
血清タンパク質を還元アルキル化後、トリプシン消化し、コンカナバリンAカラムにアプライした。コンカナバリンAカラム非吸着画分をニホンニワトコレクチンカラムにアプライし、ラクトースで溶出した。ニホンニワトコレクチンカラム吸着画分を逆相HPLCカラムで分画し、N-グリコシダーゼF処理で糖鎖を遊離させた。N-グリコシダーゼF処理前後のサンプルをMALDI-TOF MSを用い解析し、糖鎖No.45の質量分のシフトが認められたペプチドのMALDI-TOF/TOFスペクトルを得、MASCOTで検索した。
糖鎖No.45を有するタンパク質の同定例を図9に示した。同定した糖鎖No.45を有するタンパク質および同タンパク質のトリプシン消化物ペプチド配列を下表5に示す。
関節リウマチ患者14名および健常人11名の血清の糖鎖の中で最も有意に関節リウマチ患者と健常人を識別可能であった糖鎖No.45は、(Hex)3 (HexNAc)3 (Deoxyhexose)1 (NeuAc)3 + (Man)3(GlcNAc)2の糖組成を有する(表1)が、本糖組成を有する構造には、フコース(Deoxyhexose)の結合位置の多様性等のために複数の構造異性体が存在する。関節リウマチのバイオマーカーとして有用なNo.45の構造を特定するために、より詳細な構造解析を行った。
実施例1で調製した関節リウマチ患者14名および健常人11名のMALDI-TOF解析用試料を用い、糖鎖No.45に相当するイオンを親イオンとしてMALDI-TOF/TOF(MS/MS)解析を行った。健常人およびRA患者で認められたMALDI-TOF/TOF解析の結果得られたフラグメントパターンの例を図10に示した。還元末端の構造に特徴的なYイオンが良好に観察されており、図中(2)で示したフラングメントピークは、フコースが糖鎖の還元末端に結合した場合にのみ検出される特徴的なフラグメントであり、(3)で示したフラグメントはフコースが糖鎖の分岐側鎖に結合した場合により高頻度に出現するフラグメントである。図10に示す通り、フラグメントピーク(2)と(3)の相対的強度は、健常人とRA患者間で異なる傾向が認められたことから、No.45で表される糖鎖の中でフコース残基の結合位置の分布に健常人とRA患者間に違いがあることが示唆された。フコース残基の結合位置の分布を健常、RA患者の2群間で定量的に評価するため、MALDI-TOF/TOFスペクトルよりフラグメントピーク(2)と(3)を含む3つのYイオンを抽出し、フコース残基の結合位置によらず一定のフラグメントを与えるフラグメントピーク(1)の値に対する相対値として規格化して表5に示した。値として規格化したピーク強度(normalized intensity、表6-1)と規格化したピーク面積(normalized area、表6-2)を用いた。
実施例1で調製した関節リウマチ患者14名および健常人11名のMALDI-TOF解析用試料を用い、糖鎖No.45に相当するイオンを親イオンとしてMALDI-TOF/TOF(MS/MS)解析を行った。健常人およびRA患者で認められたMALDI-TOF/TOF解析の結果得られたフラグメントパターンの例を図10に示した。還元末端の構造に特徴的なYイオンが良好に観察されており、図中(2)で示したフラングメントピークは、フコースが糖鎖の還元末端に結合した場合にのみ検出される特徴的なフラグメントであり、(3)で示したフラグメントはフコースが糖鎖の分岐側鎖に結合した場合により高頻度に出現するフラグメントである。図10に示す通り、フラグメントピーク(2)と(3)の相対的強度は、健常人とRA患者間で異なる傾向が認められたことから、No.45で表される糖鎖の中でフコース残基の結合位置の分布に健常人とRA患者間に違いがあることが示唆された。フコース残基の結合位置の分布を健常、RA患者の2群間で定量的に評価するため、MALDI-TOF/TOFスペクトルよりフラグメントピーク(2)と(3)を含む3つのYイオンを抽出し、フコース残基の結合位置によらず一定のフラグメントを与えるフラグメントピーク(1)の値に対する相対値として規格化して表5に示した。値として規格化したピーク強度(normalized intensity、表6-1)と規格化したピーク面積(normalized area、表6-2)を用いた。
(MS/MSスペクトルより4つのYイオンについて抽出し、詳細な解析を行った結果であり、データは全て分子量433のフラグメントピーク(図10の(1))の値を1とするように規格化した。値として規格化したピーク強度(normalized intensity、表6-1)と規格化したピーク面積(normalized area、表6-2)を用いた。)
それぞれの値を使って、関節リウマチ患者と健常人のデータの間に分散の差が無いかF検定を用いて検定したところ、2群の間に分散の差はないと言えた。そこで、等分散を仮定した両側t検定で、前述の4つのYイオンに関して差があるか検定したところ、フラグメントピーク(2)とフラグメントピーク(3)の両者において有意差が認められた。この結果から、糖鎖No.45はフコースの結合部位の異なった構造異性体の混合物であり、その構造異性体の存在比は、関節リウマチ患者と健常人で有意に異なり、RA患者ではフコース残基が分岐側鎖に結合したものが顕著に増加することが判明した。これは既存の糖鎖の全質量のみを利用して行う測定では検出されないデータであり、MS/MSを用いることによってはじめて明らかになった構造異性体のバイオマーカー候補である。

関節リウマチおよび健常人の血清についてα2,3-シアリダーゼ消化後の糖鎖プロファイルを取得した。すでに血清より糖鎖を遊離したサンプル20μLに200mMの酢酸緩衝液(pH4および5)を10μL加えpHを5に調製し、カルビオケム社製のノイラミニダーゼ(α2,3-Neuraminidase, Macrobdella decora,大腸菌組み替え)を2μL加え37℃で16時間反応を行った。反応後、グリコブロッティング(GlycoBlotting)法により糖鎖プロファイル(図11)を取得した。その結果、糖鎖No.45よりシアル酸が一つ切断された化合物が主生成物として観測され、少なくとも一つのシアル酸がα2,3結合していることが明らかとなった。
本発明に従って糖鎖分析を行い、特定の糖鎖の増減を検討することで、感度ならびに特異度において従来法を上回る、あるいは相補的な優位性を有した関節リウマチの診断が可能となる。
Claims (10)
- No.1、7、8、10、11、17、18、21、24、25、27、29、31、38、41、43、45、46および48よりなる群から選択される一つまたは二つ以上の糖鎖の発現量を指標とする、関節リウマチの診断方法。
- No.45の糖鎖の発現量を指標とする、関節リウマチの診断方法。
- No.45の糖鎖組成中のフコースが側鎖に付加している、請求項1または2記載の診断方法。
- No.45の糖鎖組成を有し、かつシアリルルイスx(NeuNAcα2→3Galβ1→4(Fucα1→3)GlcNAc)またはシアリルルイスa(NeuNAcα2→3Galβ1→3(Fucα1→4)GlcNAc)をエピトープとして有する糖鎖の発現量を指標とする、請求項1から3のいずれかに記載の診断方法。
- 糖鎖の血清中発現量を指標とする、請求項1から4のいずれかに記載の方法。
- No.1、7、8、10、11、17、18、21、24、25、27、29、31、38、41、43、45、46および48よりなる群から選択される一つまたは二つ以上の糖鎖の、関節リウマチマーカーとしての使用。
- No.45の糖鎖の、関節リウマチマーカーとしての使用。
- No.45の糖鎖組成中のフコースが側鎖に付加している、請求項6または7に記載の使用。
- No.45の糖鎖組成を有し、かつシアリルルイスx(NeuNAcα2→3Galβ1→4(Fucα1→3)GlcNAc)またはシアリルルイスa(NeuNAcα2→3Galβ1→3(Fucα1→4)GlcNAc)をエピトープとして有する糖鎖の、請求項6から8のいずれかに記載の使用。
- (i)被験サンプルの糖鎖を解析する工程、
(ii)健常人および被験サンプルにおける、No.1、7、8、10、11、17、18、21、24、25、27、29、31、38、41、43、45、46および48よりなる群から選択される、一つまたは二つ以上の糖鎖の発現量を比較する工程、
を包含する、関節リウマチサンプルを選別する方法。
