Abstract
Die Erfindung betrifft eine modulare Revisionspfanne für die Hüftgelenkendoprothetik, welche entsprechend den intraoperativ im Acetabulum vorgefundenen Defekten während der Revisionsoperation angepasst werden kann. Die erfindungsgemäße modulare Revisionspfanne besteht aus einem Pfannengrundkörper (1), einer Befestigungslasche (3), einem oder mehreren Verankerungszapfen (2) und einem Pfanneneinsatz. Der Pfannengrundkörper (1) ist als ovaläre Pressfitschale geformt und hat zwei polyaxiale Spreizpressfitlager (6) für Verankerungszapfen (2), ein Montagelager (13) für eine Befestigungslasche (3) sowie eine Aussparung (12) zur Erweiterung der Bewegungsumfanges und Verhinderung von Impingment. Die Befestigungslasche (3) ist in Blockmontage am Pfannengrundkörper (1) befestigt. Durch ihre formschlüssige Montage werden Kippmomente im Montagelager (13) aufgenommen und sichern eine hohe Stabilität des Implantats. Die Verankerungszapfen (2) besitzen mehrere Funktionsbereiche. Ein Spitzenbereich (17) dient zur Führung während des Eindrückens, ein Führungsbereich (18) dient zur Verdrehsicherung und Kompression des spongiosen Knochens während des Einführens, ein Haltebereich (19) sichert die mechanische Stabilität im Knochen und ein Kopfbereich (20) dient zur Befestigung in der Revisionspfanne und ermöglicht die Arretierung in polyaxialer Ausrichtung.

Technisches Gebiet
[0001] Die Erfindung betrifft eine modulare Revisionspfanne für die Hüftgelenkendoprothetik und Verankerungszapfen hierzu.
Stand der Technik
[0002] Beim Ersatz eines verschlissenen Hüftgelenkimplantats stellt der Operateur regelmäßig sowohl im Bereich des Hüftstieles als auch im Bereich der Hüftpfanne partiellen Knochenverlust fest. Der Operateur muss das Knochenbett für die Fixierung des Implantates unter Verlust von weiterer Knochenmasse vorbereiten und für die Revisionsimplantation eine Gelenkskonfiguration wählen, die die veränderten räumlichen Bedingungen berücksichtigt. Während für den Revisionsstiel bereits bewährte Lösungen existieren, ist dies für die Revisionspfannenprothese nicht der Fall bzw. die existierenden Lösungen sind unbefriedigend. Als wesentliche Anforderung ist insbesondere die variable und dennoch sichere, primär- und dauerstabile Verankerung der acetabulären Implantatkcmponente, im folgenden Revisionspfanne genannt, im Beckenknochen bisher nicht zufriedenstellend gelöst.
[0003] Zusätzlich muss die Revisionspfanne insbesondere bei Wechseloperationen aufgrund von Endoprothesenlockerungen mit schweren Schäden des Knochenlagers, z.B. nach Mehrfach wechseln, dem besonderen Anspruch genügen, auch regenerative Prozesse im umgebenden Knochenlager zu ermöglichen um damit eine hohe Sekundärstabilität zu realisieren.
[0004] Durch eine entsprechende Formgebung der Revisionspfanne muss sichergestellt sein, dass die Lage des Bewegungszentrums trotz des defekten Knochenlagers rekonstruiert wird und dass der Bewegungsumfang in weitgehend natürlichem Maße möglich sein soll.
[0005] Der Operateur stellt die Forderung, neben der unkomplizierten Handhabung der Revisionspfanne, in der konkreten operativen Situation variabel auf den vorgefundenen Befund, den unterschiedlichen Knochenverlust, reagieren zu können.
[0006] Pfannen für die Hüftgelenkendoprothetik sind in verschiedener Ausführung bekannt. Durch ihre Form, ihre funktionellen Bereiche und durch ihre Zusatzelemente wird mit unterschiedlichem Erfolg versucht, die aufgezeigten Anforderungen auch für
den Revisionsfall zu erfüllen. [0007] Sie lassen sich grob in folgende Gruppen einteilen:
- Hüftpfannen mit ovalärer Form
- Hüftpfannen mit Lasche
- Hüftpfannen mit Zapfen
- Hüftpfannen mit Lasche und Zapfen
- Hüftpfannen mit anderen Verankerungsmitteln
[0008] Dabei ist die ovaläre Form besonders vorteilhaft, um den Knochenverlust, der in den typischerweise auftretendenden acetabulären Defekten offenbar wird, teilweise ausgleichen zu können.
[0009] Aus der Patentschrift DE 197 01 778 C2 ist ein Gelenkpfanneninplantat bekannt, das aus einem länglich geformten Pfannenkörper und einem Pfanneneinsatz (Inlay) besteht, der auf der Frontseite eine halbkugelförmige Öffnung zur Aufnahme einer Gelenkkugel aufweist. Das Gelenkpfanneninplantat besteht aus zwei Teilen, dem äußeren Pfannenkörper und dem darin einrastenden Pfanneneinsatz. Der Pfannenkörper ist oval gehalten, wobei die kugelige Aufnahme für den Pfanneneinsatz auf der Hauptachse einseitig verschoben ist. Der Pfanneneinsatz trägt außen Nasen, die in entsprechende Aussparungen im Pfannenkörper einrasten können, und zwar in verschiedenen Winkelstellungen.
[0010] Eine ähnliches Revisionspfanneimplantat wird in dem Gebrauchsmuster DE 201 08 114 Ul offenbart, welches aus einem Stück besteht und direkt in das Acetabulum eingesetzt wird. Das Pfannenimplantat hat eine ovale Außenkontur und eine Halbkugelöffnung zur Aufnahme einer Kugel.
[0011] Nachteil dieses Gelenkpfannenimplantates ist es, dass keine zusätzlichen Verankerungen zur Befestigung am Knochen vorgesehen sind und es somit als Revisionspfannenimplantat bei größeren Defekten, die nicht mit Pressfit- Implantaten versorgt werden können, nicht geeignet sind.
[0012] Ein weiteres Problem besteht darin, dass das Revisionspfannenimplantat im
Acetabulum mit dem patientenindividuell einzustellenden Pfanneneingangswinkel in die richtige Stellung gebracht werden muss, da ansonsten der Bewegungsumfang nach der Implantation eines künstlichen Hüftgelenkes nicht mehr physiologisch nachgebildet ist. Durch ein zu tiefes Einpressen des Implantats in das Acetabulum entsteht ein verändertes Rotationszentrum, wodurch die Beinlänge und der femurale Abstand negativ beeinflusst werden. Zur Verhinderung dieses zu tiefen Einpressens können am Revisionspfannenimplantat Befestigungslaschen vorhanden sein.
[0013] In dem Gebrauchsmuster DE 201 05 862 Ul wird ein Pfannenimplantat beschrieben, dessen Knochenlagerseite wenigstens abschnittsweise eine dreidimensionale, offenmaschige Raumnetzstruktur aufweist. Auf dieser Seite befindet sich ein zusätzliches, in den Knochen einzuführendes Fixierungselement für die Sekundärfixierung. Die Beckenpfanne hat nach einer Seite hin einen ausladenden Bereich, derart, dass sie in Draufsicht eine ovaläre und in Seitenansicht eine halb- ovaläre Kontur aufweist. Die flügelartig ausgebildete Lasche, die sich beim Einpressen der Pfanne in das Acetabulum an die Darmbeinschaufel anlegt, dient der Erhöhung der Primärstabilität des Implantats.
[0014] Die Lasche ist in einem festen Winkel mit der Pfanne verbunden. Dieser Winkel ist so gewählt, dass er für eine empirisch ermittelte mittlere Beckenform optimal ist. Auf patientenspezifisch verschiedene Formen kann damit nicht reagiert werden oder es müssen in diesem Fall implantatbedingte, in der Regel abtragende Anpassungen des Knochenlagers vorgenommen werden.
[0015] In DE 101 13 614 B4 wird ein modulares Implantat für eine stark defekte
Hüftgelenkpfanne beschrieben. Dieses besteht aus einem in das defekte Acetabulum zu setzenden, ovalären Pfannenimplantat, welches mindestens drei, die Wandung durchsetzende Bohrungen aufweist. Ein langgestrecktes Fixierungselement ist an das Implantat wahlweise im Bereich der drei Bohrungen anschraubbar und so fest arretierbar.
[0016] Aus DE 197 24- 828 Al ist ein modulares Hüftpfannenimplantat mit der Möglichkeit zur Anpassung an die vorhandenen individuellen anatomischen Voraussetzungen bei der Implantation bekannt. Der Grundkörper eines Hüftpfannenimplantats wird mit einem oder mehreren Modulen durch Metallschrauben fest miteinander verbunden. Das System kann zusätzlich durch Knochenschrauben stabil verankert werden.
[0017] In WO 95/30389 Al wird eine Prothese beschrieben, die die Form eines
Kugelsegmentes aufweist. Sie wird mit Befestigungselementen, die gesteckt und dann verschraubt werden, oder mit Schrauben am Knochen befestigt.
[0018] Aus US 5,360,452 ist eine Gelenkpfanne bekannt, die über zapfenförmige Befestigungselemente unterschiedlicher Art am Knochen fixiert wird. Diese Befestigungselemente haben gemeinsam, dass sie über Schraubgewinde verfügen, die in korrespondierende Öffnungen in der Pfanne eingeschraubt werden.
[0019] In DE 195 02 503 Cl wird ein Knocheninplantat mit Pfanne und Stiften beschrieben. Die hier beschriebene Pfanne weist eine kugelsegmentartige Form auf.
Die Befestigung erfolgt über Stifte, die durch über die Pfanne verteilte Ausnehmungen eingeschoben werden und zur Verankerung im Knochen dienen. [0020] Bei diesen letztgenannten Veröffentlichungen sind Befestigungslaschen nicht vorgesehen. Die drei zuletzt genannten Anordnungen weisen keine ovaläre Form auf.
[0021] In CA 2 454 672 Al wird ein modulares Revisionspfannenimplantat offenbart, welches neben dem Pfannengrundkörper auch einen fest zu montierenden Laschenüberkörper als Modul mit drei Laschen besitzt. Diese Anordnung ist nicht durch Verpressung zu implantieren. Der Laschenüberkörper ist durch seine Abmessungen, die den Maximalabmessungen des Implantats entsprechen und durch die daran befindlichen festen Laschen zu unflexibel, um individuelle anatomische Besonderheiten berücksichtigen zu können.
[0022] Keine der in den vorgenannten Veröffentlichungen vorgeschlagenen Lösungen ist geeignet, um intraoperativ auf verschiedene Defektqualitäten des Acetabulums im Revisionsfall hinreichend variabel zu reagieren und sowohl die Primärstabilität, eine gute Prognose für die Sekundärstabilität und einen nahezu physiologischen Bewegungsumfang sichern zu können.
[0023] Aus EP 1 034 754 Al ist der Bausatz einer Außenschale für ein künstliches
Hüftgelenk bekannt, der ein ovales, schalenförmiges Außenteil sowie mindestens eine Lasche, einen Dorn oder einen Haken zur Verankerung aufweist. Auf der Außenschale sind an deren Äquator Bohrungen angebracht, an denen die Verankerungsmittel mittels Schrauben befestigt werden können.
[0021-] Nachteilig ist, dass die Befestigung der Laschen auf der Oberseite der Außenschale erfolgt. Dies verursacht Impingement, welches im Interesse des Patienten zu vermeiden ist. Hinzu kommt, dass die Laschen mit einer hohen Zahl von Schrauben befestigt werden müssen.
Darstellung der Erfindung
[0025] Aufgabe der Erfindung ist es, ein modulares Revisionspfannenimplantat zu entwickeln, welches während der Operation entsprechend dem Defektzustand des Acetabulums angepasst werden kann, das eine hohe Primärstabilität besitzt und eine hohe Sekundärstabiltät ermöglicht, sowie einen nahezu physiologischen Bewegungsumfang gestattet..
[0025] Die Lösung der Aufgabe erfolgt entsprechend der Patentansprüche.
[0027] Die erfindungsgemäße modulare Revisionspfanne besteht aus einem Pfannengrundkörper, einer Befestigungslasche, einem oder mehreren
Verankerungszapfen und einem Pfanneneinsatz.
[0028] Der Pfannengrundkörper ist als ovaläre Pressfitschale geformt, deren Grundform durch Radien beschreibbar ist. Er weist einen Pressfitwinkel zwischen 75°-82° und eine breite Pressfitfläche auf, die im vorwiegend erhaltenen Defektbereich abgeflacht ist. Der Pfannengrundkörper hat zwei polyaxiale Spreizpressfitlager für Verankerungszapfen, ein Montagelager für eine Befestigungslasche und eine Aussparung zur Verhinderung des Anschlagens des Kopfhalses (Impingement) und konsekutiver Luxation des Kopfes.
[0029] Die Befestigungslasche ist entsprechend den operativen Anforderungen in unterschiedlichen Geometrien einsetzbar. Die Aufnahme von Kippmomenten der Befestigungslasche erfolgt durch ein Befestigungslager im Pfannengrundkörper.
[0030] Ein oder mehrere Verankerungszapfen können in verschiedenen Größen gesetzt werden, wobei die Verankerung im Grundkörper durch Spreizpressfit erfolgt. Der Verankerungszapfen wird mittels Führhülse durch eine Bohrung im Pfannengrundkörper gesetzt.
[0031] Der Pfanneneinsatz wird durch Pressfit in der Pfanne befestigt. Auch er hat eine Aussparung zur Verhinderung von Impingement.
[0032] Der Verankerungszapfen besitzt mehrere Funktionsbereiche. Der Spitzenbereich ist zylindrisch und dient zur Führung des Zapfens während des Eindrückens. Der Führungsbereich, welcher sich dem Spitzenbereich anschließt, ist kegelförmig polyedrisch ausgeführt und dient zur Verdrehsicherung und Kompression des spongiosen Knochens während des Einführens. Der darauf folgende Haltebereich besitzt die funktional gleich modifizierte Oberfläche wie der Pfannengrundkörper und sichert als bevorzugter knöcherner Einwachsungsbereich die mechanische Stabilität des Verankerungszapfens im Knochen. Der abschließende Kopfbereich ist dübelähnlich mit sphärischen Teilflächen aufgebaut. Er dient zur Befestigung des Verankerungszapfens in der Revisionspfanne und ermöglicht die polyaxiale Ausrichtung des Verankerungszapfens. Mit dem Kopfbereich sind Sicherungsnoppen verbunden, welche in Aussparungen des Zapfenlagers eingreifen und den Verankerungszapfen gegen Verdrehung sichern.
[0033] Die kegelförmig polyedrisch ausgeführten Flächen des Führungsbereichs sind als Viererflächen oder Sternflächen ausgeführt.
[0034] Der Haltebereich besitzt eine osteointegrative Fläche, die durch Beschichtung oberflächenvergrößert ist und als Oberflächenabschluss eine bioaktive Calciumphosphat-Schicht aufweist.
[0035] Der Kopfbereich wird durch eine Schraube, welche eine Konusfläche entsprechender Steigung besitzt, mit der sphärischen Fläche des Zapfenlagers verpresst.

Weg(e) zur Ausführung der Erfindung
[0037] Die modulare Revisionspfanne in Fig. 1 besteht aus dem Pfannengrundkörper 1, dem Pfanneneinsatz aus Kunststoff, dargestellt in Fig. 7, zwei Verankerungszapfen 2 und einer Befestigungslasche 3. Das Implantat wird, bis auf den Pfanneneinsatz, aus Titanmaterial in notwendiger Güte hergestellt, die Implantatoberflächen werden je nach Aufgabenbereich funktionalisiert. Der Pfanneneinsatz wird aus UHMW- Polyethylen (ultra high molecular weight polyethylen) hergestellt.
[0038] Der Pfannengrundkörper 1 besitzt eine ovaläre Form, durch die die
Revisionspfanne in zwei Dimensionen verdrehsicher ist. In der Grundebene des Implantats lässt sich diese Form durch Radien beschreiben und ist den typischerweise auftretenden Defekten in der Weise angepasst, dass der Defekt räumlich ausgefüllt wird, wodurch sich das Drehzentrum des Gelenks weitgehend wieder in der natürlichen Lage befindet.
[0039] Die Form selbst ist für einen weiten Bewegungsbereich (ROM, R_ange O_f M_otion) optimiert.
[0040] Der Pfannengrundkörper 1 besitzt eine breite Pressfitfläche und ist im erfahrungsgemäß vorwiegend erhaltenen Defektbereich abgeflacht (geringe Tiefe), um ein Implantatlager mit möglichst wenig Knochenverlust zu ermöglichen und die verbliebene Knochensubstanz zu schonen. Das Implantatlager kann durch Fräsungen
mit lediglich zwei Fräsköpfen vorbereitet werden.
[0041] Die Steigung der seitlichen Pfannenflächen in Bezug auf die Implantatgrundfläche ist mit einem Winkel zwischen 75°-82° so gewählt, dass ein belastbarer Halt nach dem Einpressen der Pfanne in ein intaktes Implantatlager (Pressfit im weiten Randbereich der Revisionspfanne) gegeben ist.
[0042] Die Kanten 10 in der Pfannengrundebene (Fig. 2 und 3) sind verrundet, um bei gleitendem Gewebekontakt die Reibung zu verringern. Zur Rekonstruktion des natürlichen Bewegungsbereiches des Beins besitzt der Pfannengrundkörper 1 eine Pfannenaussparung 12 zur Verhinderung von Impingement. Das Einpressen des Pfannengrundkörpers 1 wird mit einer Hilfsvorrichtung vorgenommen, die im zentralen Gewinde 9 zu diesem Zweck temporär eingeschraubt wird.
[0043] Im Pfannengrundkörper befinden sich Öffnungen zur Aufnahme von konventionellen Knochenschrauben .
[0044] Für Defektsituationen, bei denen das Implantatlager in Bezug auf die Primärstabilität nicht hinreichend belastbar ist, werden zur Erhöhung der Primärstabilität, abgesehen von konventionellen Knochenschrauben, die modular wählbaren Teile Befestigungslasche 3, Verankerungszapfen lang 2a und Verankerungszapfen kurz 2b zur Sicherung der Stabilität eingesetzt. Diese Teile werden formschlüssig am Pfannengrundkörper 1 befestigt. Die Befestigungslasche 3 wird in das Laschenlager 13 gesetzt und mit zwei Befestigungsschrauben 5 in das Gewinde für die Laschenschrauben 14 angeschraubt. Durch die Formschlüssigkeit nimmt das Laschenlager 13 den größten Teil der auf die Befestigungslasche 3 wirkenden Kräfte auf.
[0045] Die Verankerungszapfen 2 werden in je einem polyaxialen Zapfenlager 6
(Spreizpressfitlager) formschlüssig im Pfannengrundkörper 1 verpresst. Die zwei polyaxialen Zapfenlager 6 weisen an der Innenseite 11 des Pfannengrundkörpers neben dem sphärischen Pressbereich 7 zwei seitliche Aussparungen 8 zur Aufnahme der Zapfenverdrehsicherung auf. Das Blind-Setzen der Zapfenlager ist möglich. Die Verankerungszapfen 2 können nach Ausführung einer Richtungsbohrung in Bohrungsrichtung in den Pfannengrundkörper 1 gesetzt werden.
[0046] Zur Sicherung des Einwachsens und zur Vergrößerung der osteointegrativen
Oberfläche der Revisionspfanne wird die Pressfit-Seite des Implantats in einem ersten Schritt mit einer strukturierten Oberfläche versehen, die in einem weiteren Schritt osteoinduktiv beschichtet wird. Die Strukturierung erfolgt indirekt durch Aufrauhung mittels Abrasionsstrahl und nachfolgender Oberflächenvergrößerung durch Aufbringen
von Ti-Partikeln in einem plasmagestützten Beschichtungsprozess. Die osteoinduktive Beschichtung erfolgt durch elektrochemische Abscheidung einer biomimetischen Calciumphosphatschicht (z.B. mit BONIT®).
[0047] Für die Befestigungslasche 3 sind unterschiedliche Geometrien einsetzbar und entsprechend den operativen bzw. anatomischen Anforderungen während der Operation montierbar. Figur 4 zeigt ein typisches Beispiel für die wahlweise zu benutzende Befestigungslasche 3. Sie besteht aus einer anatomisch geformten Ti- Flachstruktur mit einer Basis 15, die passend zu dem Laschenlager 13 der Revisionspfanne ist. Dadurch kann die Befestigungslasche 3 formschlüssig gesetzt werden und sie erhält durch die Blockmontage Rotationsstabilität. Durch das Befestigungslager im Pfannengrundkörper 1 werden außerdem Kippmomente aufgenommen.
[0048] Die Montage der Befestigungslasche erfolgt durch Schraub Verbindung. Die Basis der Lasche 15 beinhaltet zwei Öffnungen 16 für den Befestigungsschraubendurchlass. Die in Figur 1 dargestellten Befestigungsschrauben 5 werden in der Befestigungslasche 3 versenkt.
[0049] Die in Figur 4 dargestellten drei Öffnungen 16 dienen zur Aufnahme von
Knochenschrauben 4. Die konkrete Ausformung der Befestigungslasche 3 in diesem Bereich kann variabel und den jeweiligen Bedingungen im Acetabulum angepasst sein.
[0050] Eine wahlweise zusätzliche Verankerung der Pfanne im Beckenknochen erfolgt durch zwei Verankerungszapfen 2, die aus anatomischen Gründen unterschiedlich lang sind. Die einfache Vorbereitung des Sitzes erfolgt durch eine Richtungsbohrung mit reduziertem Durchmesser mittels Führhülsen durch den Pfannengrundkörper 1. Die Verankerungszapfen 2 werden durch das Zapfenlager 6 im Pfannengrundkörper 1 in den Knochen gedrückt und in den Zapfenlagern 6 der Revisionspfanne fest verpresst. Ihre Richtung ist in einem vorgegebenen Bereich polyaxial einstellbar.
[0051] Die Verankerungszapfen 2 (Fig. 5) bestehen aus mehreren Funktionsbereichen. Der erste, zylindrische Bereich (Spitzenbereich 17) dient zur Führung des Verankerungszapfens 2 während des Eindrückens.
[0052] Der zweite Bereich (Führungsbereich 18) ist kegelförmig polyedrisch ausgeführt (Vierer-Sternfläche) und dient zur Verdrehsicherung und Kompression des spongiosen Knochens während des Einführens.
[0053] Der dritte Bereich (Haltebereich 19) besitzt die gleiche funktional modifizierte Oberfläche wie der Pfannengrundkörper 1 (Osteointegrative Fläche mit TPS, BONIT®) und sichert durch die bevorzugte Verwachsung mit dem Knochen die
mechanische Stabilität des Verankerungszapfens 2 im Knochen.
[0054] Der vierte Bereich ist als Kopfbereich 20 dübelähnlich mit sphärischen Teilflächen aufgebaut und dient zur Befestigung des Verankerungszapfens 2 in der Revisionspfanne und ermöglicht die polyaxiale Ausrichtung des Verankerungszapfens 2. Durch Aufspreizung (Spreizpressfit) des Verankerungszapfens 2 (Fig. Q mit einer Schraube 22, welche im Kopfbereich eine Konusfläche entsprechender Steigung besitzt, werden die sphärischen Teilflächen des vierten Bereiches (Kopfbereich 20) mit der sphärischen Fläche des Zapfenlagers 6 verpresst. Die Verdrehsicherheit im Zapfenlager 6 wird durch zwei gegenüberliegende Sicherungsnoppen 21 erreicht. Im Zapfenlager 6 sind Aussparungen 8 für diese Noppen vorgesehen. Entsprechend den operativen Gegebenheiten können Verankerungszapfen 2 in verschiedenen Größen verwendet werden.
[0055] Figur 7 zeigt den Pfanneneinsatz aus geeignetem Kunststoff (z.B. UHMW-
Polyethylen u.a.). Der Pfanneneinsatz wird so dimensioniert, das er im Pfannenkörper 1 durch Verpressung gehalten wird. Die in Figur 7 dargestellte Aussparung 23 ist formoptimiert für großen Bewegungsbereich (ROM, R_ange O_f M_otion) und entspricht der Fortsetzung der Pfannenaussparung 12.
Claims
Ansprüche
[0001] Modulare Revisionspfame für die Hüftgelenkendoprothetik bestehend aus a) einem Pfameηgrundkörper (1), welcher als ovaläre Pressfitschale geformt ist, die im vorwiegend erhaltenen Defektbereich abgeflacht ist, deren Basisfläche durch Radien beschreibbar ist, und die eine breite Pressfitfläche bei einem Pressfitwinkel zwischen 75°-82° aufweist und wobei der Pfameηgrundkörper (1) zwei polyaxiale Spreizpressfitlager für Verankerungszapfen (2) und ein Montagelager für eine Befestigungslasche (3) und eine Aussparung (12) zur Verhinderung von Impingement besitzt, b) einer Befestigungslasche (3), welche entsprechend den operativen Anforderungen in unterschiedlichen Geometrien einsetzbar ist, deren Blockmontage durch formschlüssige "Verbindung in einem auch Kippmomenten auihehmenden Befestigungslager im Pfamengrundkörper (1) erfolgt, c) einem oder mehreren Verankerungszapfen (2) in verschiedenen Größen, wobei die polyaxial mögliche Verankerung im Grundkörper durch Spreizpressfit erfolgt, d) einem Pfanneneinsatz aus UHMW-Polyethylen, welcher durch Pressfit in der Pfanne gehalten wird und eine Aussparung (23) zur Verhinderung von Impingement und zur Erhöhung des Bewegungsumfangs besitzt.
[0002] Modulare Revisionspfanne nach Anspruch 1 dadurch gekennzeichnet, dass der Pfamengrundkörper (1) zur Vergrößerung der osteointegrativen Oberfläche mit Titanpulver beschichtet ist. [0003] Modulare Revisionspfame nach Anspruch 2 dadurch gekennzeichnet, dass die Beschichtung aus einer biomimetischen, elektrolytisch abgeschiedenen
Calciumphosphatschicht besteht. [0004] Modulare Revisionspfame nach Anspruch 1 dadurch gekennzeichnet, dass die Befestigungslasche (3) durch Schraubverbindung montiert wird. [0005] Modulare Revisionspfame nach Anspruch 1 dadurch gekennzeichnet, dass eine Bohrung im Pfamengrundkörper (1) für das Setzen der Verankerungszapfen
(2) vorgesehen ist. [0006] Modulare Revisionspfame nach Anspruch 1 dadurch gekennzeichnet, dass der Pfameneinsatz aus Kunststoff, insbesondere UHMW-Polyethylen, besteht. [0007] Verankerungszapfen mit mehreren Funktionsbereichen für eine modulare
Revisionspfame, gekennzeichnet durch,
a) einen zylindrischen Spitzenbereich (17), welcher zur Führung des Verankerungszapfens (2) während des Eindrückens diert, b) einen kegelförmig polyedrisch ausgeführten Führuηgsbereich (18), welcher sich dem Spitzenbereich (17) anschließt, der zur Verdrehsicheruηg und Kompression des spoηgiosen Knochens während des Einführens diert, c) ein darauf folgenden Haltebereich (19), welcher die gleiche funktional modifizierte Oberfläche wie der Pfamengrundkörper (1) besitzt, zur Sicherung der mechanische Stabilität des Verankerungszapfens (2) im Knochen, d) einen abschließenden dübelähnlich mit sphärischen Teilflächen aufgebauten Kopfbereich (20), der zur Befestigung des Verankerungszapfens (2) in der Revisionspfanne diert und die polyaxiale Ausrichtung des Verankerungszapfens (2) ermöglicht, und e) mit dem Kopfbereich (20) verbundene Sicherungsnoppen (21), welche in Aussparungen (8) im Zapfenlager (6) eingreifen und den Verankerungszapfen (2) gegen Verdrehung sichern.
[0008] Verankerungszapfen nach Anspruch 7 dadurch gekennzeichnet, dass die kegelförmig polyedrisch ausgeführten Flächen des Führungsbereichs (18)
Vierer-Sternflächen sind. [0009] Verankerungszapfen nach Anspruch 7 dadurch gekennzeichnet, dass der Haltebereich (19) eine osteoirtegrative Fläche besitzt, welche durch
Beschichtung mit Titanpulver oberflächenvergrößbrt ist. [0010] Verankerungszapfen nach Anspruch 7 dadurch gekennzeichnet, dass der Haltebereich (19) eine osteoirtegrative Oberfläche besitzt, welche mit einer biomimetischen, elektrolytisch abgeschiedenen Calciumphophatschicht versehen ist. [0011] Verankerungszapfen nach Anspruch 7 dadurch gekennzeichnet, dass der Kopfbereich (20) durch eine Schraube (22), welche eine Konusfläche entsprechender Steigung besitzt, mit der sphärischen Fläche des Zapfenlagers (6) verpresst wird.
