Abstract

Cotyle de correction, pour le traitement de pathologies de la hanche, dont la particularité consiste dans le fait qu'il comprend une coque (10) ayant une surface externe essentiellement hémisphérique et aplatie en son pôle, dont une portion du bord dépasse par rapport au plan diamétral de la surface hémisphérique, au moins pour un segment partiel de ce plan diamétral, un insert (13) pouvant être logé dans ladite coque.

Description

La présente invention concerne un cotyle de correction pour le traitement de pathologies de la hanche. Plus particulièrement, l'invention concerne un cotyle de correction de forme perfectionnée et dont la fonctionnalité est optimalisée.
Comme l'on sait, dans les diverses pathologies de la hanche on est souvent contraint d'employer des prothèses articulaires. Elles sont généralement constituées d'une tige qui est placée dans le canal fémoral, d'un cotyle qui est placé dans la cavité acétabulaire du bassin et d'une tte sphérique qui est placée sur le sommet de la tige et qui vient s'articuler sur la surface interne du cotyle prothétique permettant ainsi la restauration fonctionnelle de l'articulation de la hanche.
Les cotyles sont généralement de forme hémisphérique et sont réalisés dans des matériaux divers.
On peut distinguer deux types de cotyles : cimentés et non cimentés.
Les premiers sont réalisés en polyéthylène. Leur surface externe est munie de cannelures pour la fixation du ciment. Les seconds sont par contre réalisés avec deux composants : une coque extérieure en matière métallique qui est en contact avec l'os, et un composant interne ou insert, qui peut tre en polyéthylène ou en céramique lequel est couplé en permanence à la coque métallique d'un côté et de l'autre et muni d'une cavité hémisphérique à l'intérieur de laquelle s'articule la tte de la prothèse susmentionnée.
Avec le temps l'implantation prothétique dans son ensemble ou en partie peut devenir mobile ce qui rend nécessaire une intervention chirurgicale de correction. La situation osseuse aussi bien du fémur que de l'acétabulum peut tre notablement modifiée par rapport à leur état originel, souvent avec de graves détériorations, rendant nécessaire l'utilisation de tiges et/ou de cotyles spéciaux appelés, pour cette raison, cotyles de correction.
En ce qui concerne la correction cotytoïdienne, les produits actuellement dans le commerce, tout en se basant sur divers concepts, sont difficiles à implanter et généralement se trouvent en position excessivement verticale.
On commence à parler de verticalisation du cotyle quand, une fois implanté, son plan équatorial forme avec le plan horizontal un angle supérieur à 45 degrés.
Le cotyle de première implantation, aussi bien cimenté que non cimenté, quand il se met à bouger, sous l'effet de la charge directement verticale, subit une migration de direction verticale médiale. En pratique, comme cela est illustré dans la figure 1, la direction de migration du cotyle de première implantation, globalement indiqué par le chiffre de référence 1, représentée par la tangente au profil externe dans les diverses positions adoptées par le cotyle lui-mme durant sa migration est inclinée selon un angle général a mesuré par rapport à la direction de 45 degrés, différent de zéro.
Par conséquent l'empreinte laissée par le cotyle dans le bassin a une surface formée par un hémisphère dont le centre se déplacerait d'une valeur indiquée dans la figure 1 par le chiffre de référence 2, dans la direction formant un angle n avec le plan à 45 . Un cotyle de correction obtenu par interpolation des surfaces créées par un hémisphère dont le centre se déplacerait d'une valeur indiquée sur la figure par 2 sur le plan équatorial et monté dans la cavité acétabulaire de façon que sa surface externe soit en appui complet avec la surface osseuse, serait inévitablement en position verticale sur le plan frontal et aurait donc pivoté d'un angle a par rapport à l'inclinaison idéale de 45 degrés.
Dans la figure 1 la portion marquée par des tirets indique le déplacement que subit le cotyle toge dans la cavité acétabulaire du bassin 3. Donc, le cotyle de correction qui est par la suite réimplanté dans la cavité acétabulaire doit avoir la forme qui découle de la portion en ligne épaisse unie à la portion en tirets illustrée dans la figure 1.
Ce qui a été décrit ci-dessus entraîne en substance une réduction de la couverture de la tte de la prothèse de la valeur A indiquée dans la figure 2, à la valeur B avec les deux inconvénients suivants :
Plus grandes probabilités de luxation et pressions élevées de contact qui peuvent provoquer des déformations des matériaux en contact et/ou une augmentation de l'usure de ces matériaux dans le mouvement correspondant durant la déambulation.
Le rôle principal de la présente invention est donc de réaliser un cotyle de correction pour le traitement de pathologies de la hanche qui ait une forme particulière prenant en compte le changement de forme de la cavité acétabulaire et donc qui conserve une surface de contact avec l'os qui soit toujours disposée selon l'inclinaison idéale.
Dans cette perspective, I'un des objectifs de la présente invention est de réaliser un cotyle de correction pour le traitement de pathologies de la hanche dans lequel sera créé un emplacement approprié pour loger un insert dans lequel s'articulera la tte de la prothèse.
Un autre objectif de la présente invention est de réaliser un cotyle de correction pour le traitement de pathologies de la hanche qui soit doté de moyens de fixation pour son ancrage stable à l'os.
Un autre objectif de la présente invention est de réaliser un cotyle de correction pour le traitement de pathologies de la hanche qui soit adapté pour recevoir aussi bien des inserts en céramique qu'en polyéthylène.
Un autre objectif, entre autres, de la présente invention est de réaliser un cotyle de correction pour le traitement de pathologies de la hanche qui soit d'une haute fiabilité, de réalisation relativement facile et à des coûts compétitifs.
Ce rote, de mme que les autres objectifs qui apparaîtront mieux par la suite, est rempli par un cotyle de correction pour le traitement de pathologies de la hanche, caractérisé par le fait qu'il comprend une coque ayant une surface externe essentiellement hémisphérique et aplatie en son pôle, dont une portion du bord dépasse par rapport au plan diamétral de la surface hémisphérique, au moins pour un segment partiel de ce plan diamétral, un insert pouvant tre logé dans cette coque.
Les autres caractéristiques et avantages de l'invention apparaîtront mieux lors de la description des modes de réalisation jugés les meilleurs, illustrés à titre indicatif et non exhaustif par les dessins ci-joints, dont :

La figure 9 qui est une vue du cotyle selon la présente invention vu de la partie opposée par rapport à l'emplacement d'insertion de la tte de la prothèse.
En se référant aux figures susmentionnées, le cotyle de correction selon la présente invention, globalement indiqué par le chiffre de référence 10 et illustré dans les figures 3 à 9, est constitué d'une calotte hémisphérique (ou coque) essentiellement aplatie en son pôle, obtenue par le raccordement de surfaces créées par un hémisphère de diamètre D et de centre O qui durant le mouvement de déplacement qu'il subit une fois inséré dans l'acétabulum (la cavité du bassin dans laquelle normalement se place le cotyle), se déplace d'une valeur indiquée par le chiffre de référence 2 le long de son plan diamétral indiqué par le chiffre de référence 4.
Le cotyle essentiellement hémisphérique ainsi réalisé est en outre muni d'une portion essentiellement triangulaire 5 dont les côtés forment un angle au sommet a par rapport au plan diamétral 4 de façon à éviter la verticalisation du cotyle de correction 10.
La portion essentiellement triangulaire 5 constitue un rebord ou collier qui est défini en pratique par l'intersection du plan indiqué par le chiffre de référence 6 avec le plan diamétral 4.
Le plan 6 représente l'inclinaison idéale d'environ 45 degrés par rapport à une droite coupant horizontalement le bassin dont le cotyle est soumis à un intervention de correction.
A l'intérieur, le cotyle de correction 10 présente un emplacement 12 propre à recevoir un insert 13 dans lequel est articulée la tte de la prothèse, non représentée.
Cet insert 13 qui est soit en polyéthylène ou en céramique, peut tre doté, en fonction d'une seconde forme de réalisation du cotyle selon la présente invention, d'un bord anti-luxation 14 qui forme un angle P avec le plan 6, afin de compenser des migrations du cotyle formant un angle a + P avec la direction à 45 degrés, comme cela est illustré dans la figure 4.
Sur la surface externe de la coque hémisphérique constituant le cotyle de révision 10 sont en outre prévus des trous taraudés 16 pour le passage de vis de fixation 17 propres à fixer solidement le cotyle 10 à l'os du bassin.
Sont également prévus, à la base du cotyle 10, des trous taraudés pour l'implantation à l'aide de vis de systèmes pour la fixation extérieure sur l'aile iliaque, tels que la plaque 18, et pour la fixation dans le trou obturateur, tel que le crochet 19.
La surface externe du cotyle 10 présente une certaine rugosité, qui favorise un ancrage secondaire à l'os 3,1'ancrage primaire étant fourni par la plaque 18 et par le crochet 19.
La figure 6 illustre une troisième forme de réalisation du cotyle de correction selon la présente invention, dans laquelle l'insertion dans l'emplacement 12 défini par l'insert d'éléments tronconiques 20 ayant différents angles d'inclinaison, permet de varier l'angle y formé entre le plan 6 du cotyle 10 et le plan 21 de l'insert.
Comme cela est illustré dans la figure 8, l'insert 12 présente plusieurs saillies 25 propres à s'engager dans les emplacements correspondants 26 définis le long du périmètre de la coque extérieure métallique qui constitue le cotyle 10.
Le diamètre D du cotyle 10 peut varier d'un minimum de 40 mm à un maximum de 70 mm ; la dimension indiquée par le chiffre de référence 2 peut varier d'un minimum de 4 mm à un maximum de 20 mm et l'angle a varie d'un minimum de 10 degrés à un maximum de 45 degrés.
Le cotyle de correction selon l'invention est avantageusement réalisé dans un matériau métallique tandis que l'insert peut tre réalisé aussi bien en polyéthylène qu'en céramique.
On a constaté dans la pratique que le cotyle de correction selon la présente invention remplit pleinement la fonction fixée car il permet d'éviter la verticalisation par rapport au plan horizontal, c'est-à-dire d'éviter que le plan diamétral du cotyle ne forme un angle supérieur à 45 degrés par rapport au plan horizontal susmentionné.
Le cotyle ainsi conçu est susceptible de nombreuses modifications et variantes, qui entrent toutes dans le concept de l'invention ; en outre tous les éléments détaillés pourront tre remplacés par d'autres éléments techniquement équivalents.
Dans la pratique, les matériaux employés, à condition qu'ils soient compatibles avec l'usage spécifique, ainsi que les dimensions, pourront tre quelconques selon les besoins et l'état de la technique.

Claims (14)

REVENDICATIONS
1. Cotyle de correction, pour le traitement de pathologies de la hanche, caractérisé par le fait qu'il comprend une coque (10) ayant une surface externe essentiellement hémisphérique et aplatie en son pote, dont une portion du bord dépasse par rapport au plan diamétral (4) de la surface hémisphérique, au moins pour un segment partiel de ce plan diamétral, un insert pouvant tre toge dans cette coque.
2. Cotyle de correction selon la revendication 1, caractérisé par le fait que ladite surface externe est formée par le raccordement d'un premier hémisphère et d'un second hémisphère dont le centre est déplacé par rapport à ce premier hémisphère, ce centre étant disposé sur 1'axe diamétral du premier hémisphère.
3. Cotyle de correction selon la revendication 1, caractérisé par le fait que ladite portion du bord est définie par une autre portion du premier hémisphère.
4. Cotyle de correction selon la revendication 1, caractérisé par le fait que le bord inférieur de ladite portion du bord forme un angle avec le plan diamétral.
5. Cotyle de correction selon la revendication 4, caractérisé par le fait que cet angle est compris entre 10 et 45 degrés.
6. Cotyle de correction selon l'une quelconque des revendications précédentes, caractérisé par le fait que ledit insert (13) est placé dans ladite coque (10) tourné de sorte qu'une portion dudit insert ressort par rapport à la portion du bord en formant un angle avec ladite portion.
7. Cotyle de correction selon l'une quelconque des revendications précédentes, caractérisé par le fait que ledit insert ménage un emplacement où loger la tte du fémur.
8. Cotyle de correction selon l'une quelconque des revendications précédentes, caractérisé par le fait que sont prévus des éléments tronconiques (20) insérés dans 1'emplacement (12) défini par l'insert (13) et selon des angles différents de façon à varier l'angle formé entre le bord inférieur de ladite portion de bord et le plan défini par le bord inférieur dudit insert.
9. Cotyle de correction selon l'une quelconque des revendications précédentes, caractérisé par le fait que la surface externe de ladite coque est rugueuse.
10. Cotyle de correction selon l'une quelconque des revendications précédentes, caractérisé par le fait que la surface externe de ladite coque est dotée de plusieurs trous taraudés (16) pour l'insertion de vis de fixation (17) de la coque à l'os du bassin.
11. Cotyle de correction selon l'une quelconque des revendications précédentes caractérisé par le fait que sont prévus des systèmes d'ancrage fixés sur le bord de la coque pour l'ancrage de ladite coque à l'os du bassin.
12. Cotyle de correction selon l'une quelconque des revendications précédentes, caractérisé par le fait que ledit insert présente plusieurs saillies (25) propres à s'appliquer en des emplacements correspondants (26) définis par rapport au bord de la coque.
13. Cotyle de correction selon l'une quelconque des précédentes revendications, caractérisé par le fait que la coque (10) est réalisée dans un matériau métallique.
14. Cotyle de correction selon l'une quelconque des revendications précédentes, caractérisé par le fait que l'insert (13) est réalisé en céramique ou en polyéthylène.
