Description

Gattung des Anwendungsgegenstandes
Die Erfindung betrifft eine Hüftpfannenschale, wie sie in der Endoprothetik zum Ersatz der geschädigten knöchernen Hüftpfanne verwendet wird. Die hier beschrieben Pfanne ist insbesondere geeignet, Knochendefekte bei Prothesenwechseloperationen, aber auch bei anlagebedingten oder erworbenen Schädigungen der knöchernen Pfanne auszugleichen. Die Hüftpfannenschale ist aber auch allein für Standardfälle einsetzbar.
Stand der Technik
Bei knöchernen Defekten der Pfanne ist dieser häufig durch eine Standard-Hüftpfannenschale nicht in adäquater Weise zu ersetzen. Verbleibende Defekte müssen durch Knochen, Knochenersatzstoffe, Knochenzement oder ähnliches aufgefüllt werden, oder es muß unnötig viel Knochen weggefräst werden, um die nächst größere Pfanne zu implantieren. Speziell konfigurierte asymmetrische Pfannen gleichen dieses Problem nur zum Teil aus, da sie nicht alle Situationen berücksichtigen und darüberhinaus eine große Lagerhaltung verschiedener Formen notwendig ist.
Aufgabe der Erfindung
Das erfindungsgemäße modulare Hüftpfannensystem soll vorzugsweise bei Prothesenwechseloperationen zum Ausgleich von vorhandenen knöchernen Pfannendefekten eingesetzt werden. Durch den modularen Aufbau können vorhandene Defekte durch Anpassen von Modulen an die Hüftpfannenschale ausgeglichen werden. Das System kann stabil mit dem Knochen verankert werden durch das bekannte Press-fit System und/oder durch Schrauben, die sowohl die Hüftpfannenschale und/oder die Module fest mit dem verbleibenden Knochen verbinden. Damit können auch stabilitätsgefährdende knöcherne Defekte, z. B. ventrale oder dorsale Pfeilerdefekte, mit dem hier dargestellten System stabil versorgt werden.
Operatives Vorgehen
Die Vorbereitung des Pfannenlagers erfolgt in der bisher übliche Weise. Vorzugsweise mit einem Probeset oder z. B. mit einer entsprechenden Schablone wird die ideale Kombination von Hüftpfannenschale und Modulen zusammengesetzt. Das Zusammensetzen kann entweder vor der Implantation erfolgen oder einzelne Teile werden zunächst implantiert und ggf. auch schon mit Schrauben am Knochen stabilisiert und erst anschließend wird die Kombination aus Hüftpfannenschale und Modulen fest miteinander verbunden. Abschließend wird in die Hüftpfannenschale ein Inlay als Gleitpartner für den Prothesenkopf eingesetzt.
Vorteile
Mit dem modularen Hüftpfannensystem können alle denkbaren Defekte der knöchernen Pfanne des Beckenknochens versorgt werden.
Die Lagerhaltung ist geringer, da die notwendigen Implantatformen intraoperativ individuell aus den Modulen zusammengesetzt werden.
Die Notwendigkeit bzw. Menge für Knochentransplantate, Knochenersatzstoffe, Knochenzement, etc. wird eingeschränkt. Dadurch werden logistische Probleme, Patientenbelastungen und Kosten reduziert.
Gewerbliche Nutzbarkeit
Das modulare Hüftpfannensystem ist bei jeder Versorgung von Pfannendefekten in der Hüftendoprothetik einsetzbar. Die Hüftpfannenschale ist auch als Standardimplantat einsetzbar.
Ausführungsbeispiel
Abb. 1 zeigt einen linken Beckenknochen (1) im Schnitt entsprechend einem anterior­ posterioren Röntgenbild mit einem typischen Pfannendefekt (2) z. B. nach Migration einer gelockerten Pfanne nach cranial. In den Pfannendefekt (2) sind eine Hüftpfannenschale (3) und 2 Module (4) den individuellen Erfordernissen entsprechend eingepaßt. Zur Demonstration sind Module (4) und Hüftpfannenschale (3) nicht formschlüssig miteinander verbunden und auch nicht formschlüssig dem Pfannendefekt (2) angepaßt. Die Module (4) können durch eine geeignete Vorrichtung, z. B. Metallschrauben (5) stabil mit der Hüftpfannenschale (3) verbunden werden. Die Hüftpfannenschale (3) und/oder die Module (4) können z. B. durch eine oder mehrere Knochenschrauben (6) mit dem Beckenknochen (1) stabil verbunden werden.
Ungenutzte Löcher (7) in der Hüftpfannenschale oder den Modulen können ggf. durch Stöpsel (nicht gezeichnet) verschlossen werden. In der Hüftpfannenschale (3) wird abschließend ein Inlay (nicht gezeichnet) als Gleitpartner für den Prothesenkopf(nicht gezeichnet) verankert.

Claims (11)

1. Hüftpfannenschale zur Implantation in den Beckenknochen, dadurch gekennzeichnet, daß an der Hüftpfannenschale Module befestigt werden können, die eine individuelle Anpassung des Verbundes aus Hüftpfannenschale und einem oder mehreren Modulen an die intraoperativ vorgefundene Pfannensituation ermöglichen.
2. Hüftpfannenschale zur Implantation in den Beckenknochen nach Anspruch 1, dadurch gekennzeichnet, daß die Module sowohl vor der Implantation des Gesamtverbundes, nach der Implantation der Hüftpfannenschale oder auch bei primärer Implantation zunächst der Module stabil mit der Hüftpfannenschale verbunden werden können.
3. Hüftpfannenschale zur Implantation in den Beckenknochen nach einem der vorgenannten Ansprüche, dadurch gekennzeichnet, daß die Module sowohl im Randbereich als auch an beliebiger Stelle der gewölbten Fläche der Hüftpfannenschale fixiert werden können.
4. Hüftpfannenschale zur Implantation in den Beckenknochen nach einem der vorgenannten Ansprüche, dadurch gekennzeichnet, daß die Oberfläche von Hüftpfannenschale und Modulen so strukturiert ist, daß eine zementfreie Einheilung möglich ist.
5. Hüftpfannenschale zur Implantation in den Beckenknochen nach einem der vorgenannten Ansprüche, dadurch gekennzeichnet, daß die Oberfläche von Hüftpfannenschale und Modulen so strukturiert ist, daß eine Zementfixation möglich ist.
6. Hüftpfannenschale zur Implantation in den Beckenknochen nach einem der vorgenannten Ansprüche, dadurch gekennzeichnet, daß durch Löcher in der Hüftpfannenschale und den Modulen eine zusätzliche Fixation der Pfanne und/oder der Module im Knochen z. b. mit Schrauben möglich ist.
7. Hüftpfannenschale zur Implantation in den Beckenknochen nach einem der vorgenannten Ansprüche, dadurch gekennzeichnet, daß auch die Module miteinander verbunden werden können.
8. Hüftpfannenschale zur Implantation in den Beckenknochen nach einem der vorgenannten Ansprüche, dadurch gekennzeichnet, daß die Hüftpfannenschale auch ohne Module implantiert werden kann.
9. Hüftpfannenschale zur Implantation in den Beckenknochen nach einem der vorgenannten Ansprüche, dadurch gekennzeichnet, daß die Hüftpfannenschale und/oder die Module auch allein nach dem Press-fit System verankert werden können.
10. Hüftpfannenschale zur Implantation in den Beckenknochen nach einem der vorgenannten Ansprüche, dadurch gekennzeichnet, daß die Hüftpfannenschale auch ein Standardinlay als Gleitpartner für den Prothesenkopf aufnehmen kann.
11. Hüftpfannenschale zur Implantation in den Beckenknochen nach einem der vorgenannten Ansprüche, dadurch gekennzeichnet, daß nicht genutzte Öffnungen der Hüftpfannenschale oder der Module ggf. durch geeignete Stöpsel verschlossen werden können.
