Beschreibung
Die vorliegende Erfindung bezieht sich auf einen Prothesen- Liner nach dem Oberbegriff des Anspruchs 1.
Die weichelastische Hülse derartiger Prothesen-Liner hat neben polsternden und stoßdampfenden Wirkungen vor allem die Aufgabe, den Stumpf luftdicht zu umschließen, so dass der Liner nicht vom Stumpf abgezogen werden kann und daher eine Prothese über eine mechanische Verbindung mit dem Liner und damit mit dem Stumpf sicher verbunden werden kann. Dafür nπuss die weichelastische Hülse eines solchen Lmers sich eng an die Haut des Tragers anschmiegen, nachdem sie durch Umstülpen auf den Stumpf des Tragers unter Vermeidung jeglichen Lufteinschlusses gebracht worden ist. In der Regel ist es also erwünscht, dass das Linermaterial direkten Kontakt mit der Haut des Prothesentragers hat. Die zusatzliche Verwendung eines textilen Überzuges als Lage zwischen Stumpf und Liner ist allgemein als nachteilig bekannt, da die hierdurch eingeschlossene Luft zu vermehrter Schweißbildung fuhrt und weil bei Verwendung eines solchen Strumpfes in der Regel über dessen Poren zusätzlich Luft in den Liner eindringt und dieser dann leichter vom Stumpf abrutschen kann. Es versteht sich, dass sich zwischen der Haut und dem Liner zwangsweise ein feuchtwarmes Milieu bildet, welches die Vermehrung von Bakterien und Pilzen begünstigt. Sowohl die Hülse des Prothesen-Liners als auch der Extremitatenstumpf des Patienten müssen stets sorgfaltig gereinigt werden. Dennoch besteht eine latente Gefahr dahingehend, dass sich zwischen den Kontaktflachen Bakterien und/oder Pilze bilden, die zu Irritationen der Haut des Extremitatenstumpfes fuhren können.
Aufgabe der vorliegenden Erfindung ist es deshalb, einen Prothesen-Lmer der eingangs genannten Art zu schaffen, dem zusätzlich zu der Vorsorge durch gründliches Reinigen eine verbesserte Verträglichkeit eigen ist.
Zur Losung dieser Aufgabe sind bei einem Prothesen-Lmer der genannten Art die im Anspruch 1 angegebenen Merkmale vorgesehen .
Durch die erfindungsgemaßen Maßnahmen ist außerhalb der notwendigen täglichen Reinigung eine materialspezifische Vorsorge gegenüber möglichen Irritationen der Haut durch Bakterien und/oder Pilzinfektionen gegeben.
Eine bevorzugte Ausfuhrung eines Prothesen-Lmers verwendet ein Silikon bzw. ein Silikon-Kautschuk mit antibakterieller und/oder fungizider Wirkung. Solche antibakterielle und/oder fungizide Silikonmaterialien sind an sich bekannt, wobei neben chemischen Bakteriziden/Fungiziden die unterschiedlichsten Metalle und Metaliverbindung und auch Herstellungsverfahren verwendet werden. In bevorzugter Weise wird bei der erfindungsgemaßen Anwendung als antibakterieller und/oder fungizider Wirkstoff atomares, also reines Silber in einer nicht toxischen Konzentration verwendet, da dies hier am effektivsten und Silber am wenigstens toxisch ist.
Der Prothesen-Lmer kann als becherförmige, das heißt distal geschlossene Hülse oder als schlauchförmige Hülse ausgebildet sein, wobei die Hülse entweder insgesamt aus einem weichelastischen Kunststoffmaterial mit antibakterieller und/oder fungizider Wirkung sein oder auch nur an ihrer Innenflache mit einem solchen Material beschichtet sein kann.
Bei einer weiteren Ausgestaltung können auch die Merkmale nach Anspruch 7 vorgesehen sein. Haupt- und Innenliner können jeweils becherförmig oder Strumpf- beziehungsweise
schlauchförmig ausgebildet sein. Diese Ausfuhrung hat den Vorteil, dass nur eine geringe Menge Silber für einen dünnen Schlauch- bzw. Strumpfuberzug benotigt wird und dieser in jedem bekannten Liner getragen werden kann. Wenn die Wirkung des antibakteriellen/fungiziden Zusatzes nachlasst, muss lediglich der Innenliner ausgetauscht werden, was zu Kosteneinsparungen fuhrt.
Weitere vorteilhafte Ausgestaltungen ergeben sich aus den weiteren Unteranspruchen.
Weitere Einzelheiten der Erfindung sind der folgenden Beschreibung zu entnehmen, in der die Erfindung anhand des in der Zeichnung dargestellten Ausfuhrungsbeispieles naher beschrieben und erläutert ist.
Die einzige Figur zeigt in schematischer, langsgeschnittener Darstellung einen Prothesen-Liner gemäß einem Ausfuhrungsbeispiel vorliegender Erfindung.
Der in der Zeichnung dargestellte Prothesen-Liner 10 besitzt eine Hülse 11 aus weichelastischem Kunststoff von beispielsweise etwa 6 mm Dicke, die an ihrem oberen Ende offen zum Überstülpen über einen Extremitatenstumpf eines Patienten ist und die an ihrem unteren, einer Prothese zugewandten Ende, becherförmig geschlossen und mit einem Distalanschluss 12 versehen ist.
Beim dargestellten Ausfuhrungsbeispiel ist die Hülse 11 an ihrem distalen Bereich mit einer Matrix beziehungsweise Verstärkung 13 versehen, die in diesem Bereich die Langendehnung der Hülse 11 minimieren soll und die mit der Hülse 11 flachig verbunden ist. Der Distalanschluss 12 besitzt eine Innengewindemuffe 14, die die Hülse 11 in Richtung der Außenseite des Liners durchdringt und die im distalen Bereich mit einer Tasse 15 fest verbunden ist, die
eine Verankerung der Innengewindemuffe 14 mit der Hülse 11 bewirkt. Die Innengewindemuffe 14 hat den Zweck, eine Möglichkeit zur Befestigung einer Prothese an dem Liner 10 und über diesen letztendlich an dem Extremitatenstumpf zur Verfugung zu stellen. Hierzu kann in bekannter Weise beispielsweise ein Stift in die Muffe 14 des Lmers 10 eingeschraubt werden, welcher dann wiederum nach dem Einsteigen in einen harten Prothesenschaft in diesem über einen manuell losbaren Einweg-Verriegelungsmechanismus verbunden wird.
Beim bevorzugten Ausfuhrungsbeispiel vorliegender Erfindung ist die weichelastische Hülse 11 aus einem Silikon, das mit einem antibakteπellen und/oder fungiziden Wirkstoff versehen ist. Dieser Wirkstoff ist beim Ausfuhrungsbeispiel atomares, also reines Silber. Der antibakterielle und/oder fungizide Wirkstoff kann entweder lediglich auf der Innenflache der weichelastischen Hülse 11 vorgesehen oder aber im für die weichelastische Hülse 11 verwendeten Silikon vollständig enthalten sein.
Bei einem Ausfuhrungsbeispiel wird reines Silber (Ag) in Form von Partikeln im Mikrometer-Bereich oder im Nanometer-Bereich verwendet, wobei der letztere Bereich wegen der relativ größeren Oberflache bevorzugt ist. Der Anteil der Ag-Partikel im betreffenden Kunststoffmaterial liegt bei 0,01 bis 0,1 Gew.-%, vorzugsweise bei 0,05 Gew.-%. Das reine Silber wird dem betreffenden Kunststoffmaterial bei dessen Herstellung beigemischt, beispielsweise bei Silikon, indem im bei der Herstellung verwendeten Silikonol Ag-Partikel dispergiert werden .
Gemäß einem nicht dargestellten Ausfuhrungsbeispiel vorliegender Erfindung besteht der Prothesen-Liner aus einem Hauptliner in Form einer hier mit einem Distalanschluss versehenen Hülse aus weichelastischem Kunststoffmaterial und
aus einem davon separaten Unterzieh- beziehungsweise Innenlmer, dessen hier becherförmige Hülse und nur diese in oben beschriebener Weise antibakteriell und/oder fungizid ausgebildet ist. Dieser Innenlmer kann vergleichsweise dünn, zum Beispiel bei einer Wandstarke von 2 mm, sein und als gegen Mikroben wirkende Zwischenschicht zwischen dem Stumpf und dem Hauptliner getragen werden. Es wird also für die relativ dünne weichelastische Kunststoffhulse auch nur eine relativ geringe Menge an antibakteriellem und/oder fungizidem Wirkstoff benotigt.
Es versteht sich von selbst, dass schon wegen des großflächigen intimen Hautkontaktes über vielen Stunden Prothesen-Liner keine toxischen oder andere die Haut irritierende Eigenschaften haben dürfen. Aus diesem Grund wird erfmdungsgemaß vorzugsweise Silber in einer nichttoxischen Konzentration zur Erzeugung der antibakteriellen/fungiziden Wirkung verwendet. Ebenso versteht es sich, dass für den Prothesen-Liner auch Elastomere mit antibakterieller und/oder fungizider Wirkung verwendet werden können, deren Wirkstoff aus einem anderen Metall als Silber oder aus einer hierfür verwendeten bekannten Metallverbindung sein kann; auch eine die Wirkung bietende chemische Ausrüstung ist möglich.
Des Weiteren ist es gemäß anderen Ausfuhrungsbeispielen möglich, als elastomere Kunststoffe, zum Beispiel PU (Polyurethan) oder ein TPE (thermoplastisches Elastomer) zu verwenden, das mit einem der vorgenannten antibakteπellen und/oder fungiziden Wirkstoffen, insbesondere ebenfalls atomares Silber (Ag), ausgerüstet ist.
Weiterhin versteht sich, dass sich die Erfindung nicht auf Prothesen-Liner der beschriebenen Ausfuhrung beschrankt, sondern selbstverständlich auch auf alle anderen bekannten Ausfuhrungsformen von elastomeren Stumpfuberzugen angewendet
werden kann. Solche elastomeren Stumpfϋberzύge werden becherförmig oder aber auch schlauchförmig im Prothesenbau zur Erzielung von beispielsweise Polsterungseffekten oder zum Abdichten gegen das Eindringen von Luft zwischen Bein und proximaler Prothesenschaftoffnung verwendet.
Claims
1. Prothesen-Liner (10), mit einer Hülse (11) aus einem weichelastischen Kunststoffmaterial, gekennzeichnet durch die Verwendung eines weichelastischen Kunststoffmaterials mit antibakterieller und/oder fungizider Wirkung.
2. Prothesen-Lmer nach Anspruch 1, dadurch gekennzeichnet, dass das antibakteriell und/oder fungizid wirkende Kunststoffmaterial ein Silikon ist.
3. Prothesen-Liner nach Anspruch 1, dadurch gekennzeichnet, dass das antibakteriell und/oder fungizid wirkende Kunststoffmaterial ein Polyurethan ist.
4. Prothesen-Liner nach Anspruch 1, dadurch gekennzeichnet, dass das antibakteriell und/oder fungizid wirkende Kunststoffmaterial ein thermoplastisches Elastomer ist.
5. Prothesen-Liner nach mindestens einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der antibakterielle und/oder fungizide Wirkstoff im Kunststoffmaterial enthalten ist.
6. Prothesen-Lmer nach mindestens einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, dass die Innenflache der Hülse (11) aus dem betreffenden Kunststoffmaterial mit dem antibakteriellen und/oder fungiziden Wirkstoff beschichtet ist.
7. Prothesen-Liner nach mindestens einem der Ansprüche 1 bis 4, gekennzeichnet durch einen Hauptlmer mit einer Hülse aus einem weichelastischen Kunststoffmaterial und einen Unterzieh- beziehungsweise Innenlmer, der aus einem weichelastischen Kunststoffmaterial mit antibakterieller und/oder fungizider Wirkung besteht.
8. Prothesen-Liner nach mindestens einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der antibakterielle und/oder fungizide Wirkstoff ein Metall oder eine Metallverbindung, vorzugsweise Silber oder eine Silberverbindung ist.
9. Prothesen-Liner nach den Ansprüchen 5 und 8, dadurch gekennzeichnet, dass reines Silber im Kunststoffmaterial in einem Bereich von 0,01 bis 0,1 Gew.-%, vorzugsweise von 0,05 Gew.-%, enthalten ist.
10. Prothesen-Lmer nach mindestens einem der Ansprüche 1 bis 9, dadurch gekennzeichnet, dass die Hülse (11) distalseitig geschlossen ist.
11. Prothesen-Lmer nach Anspruch 10, dadurch gekennzeichnet, dass die Hülse (11) mit einem Distalanschluss (12) versehen ist.
12. Prothesen-Liner nach mindestens einem der Ansprüche 1 bis 9, dadurch gekennzeichnet dass die Hülse (11) schlauchförmig ausgebildet ist.
