Abstract
Ein Stent (10) zum dauernden oder vorübergehenden Verbleib im menschlichen oder tierischen Körper weist einen aufgrund mechanischer oder geometrischer Anforderungen ausgebildeten Grundkörper aus einem Grundmaterial wie z.B. Metall, Metall-Legierung, Kunststoff oder einer Kombination dieser Materialien auf, der in Wirkverbindung mit einem Kunststoff in den Körper verbracht wird. Dadurch, daß der Stent zumindest teilweise von einem dünnen, flexiblen, bioverträglichen Kunststoff überzogen oder in diesen Kunststoff eingebettet ist, werden günstige mechanische und hämodynamische Eigenschaften gleichzeitig erreicht.

Bezug zu verwandten Anmeldungen
Die vorliegende Anmeldung beansprucht die Priorität der deutschen Gebrauchsmusteranmeldung 299 08 768.9, hinterlegt am 19.05.1999, deren Offenbarungsgehalt hiermit ausdrücklich auch zum Gegenstand der vorliegenden Anmeldung gemacht wird.
Gebiet der Erfindung
Die Erfindung betrifft einen Stent zum dauernden oder vorübergehenden Verbleib im menschlichen oder tierischen Körper nach dem Oberbegriff des Anspruches 1.
Stand der Technik
Ein derartiger Stent ist bisher z.B. als eine (Koronar)-Stent-Craft bekannt, bei der über einen Stent ein Dacron-Schlauch geschoben wird und darüber wieder ein Stent, so daß der Dacron-Schlauch zwischen den beiden Stents eingeklemmt ist. Dieses Gebilde ist nicht sehr flexibel, hat eine dicke Wandstärke und der Kunststoff Dacron hat die Tendenz Blutkörperchen anzusiedeln. Alles Effekte, die im Koronar-Gefäß uner- wünscht sind.
In der aktuellen PTCA- Anwendung und Stent-Implantationtstechnik werden bei ca. 2 % der Patienten während der Prozedur die Gefäßwände so verletzt, daß die komplette Gefäßwand aufgerissen wird und es zu Blutungen in den Pericard-Bereich kommt. Hier gibt es die Möglichkeit, diesen Defekt durch eine derartige Stent-Craft, eine Gefäßprothese, die durch einen Stent gestützt wird, zu beheben oder an dem Patienten umgehend einen herzchirurgischen Eingriff vorzunehmen, um den Defekt zu schließen.
Nach der Implantation von Stents, also nach Aufdehnung durch den Ballon-Katheter, stellt ein Stent eine Röhre dar, die durch eine metallische Maschenstruktur gebildet wird. Zwischen den Maschen kann das körpereigene Gefäßgewebe weiter in den Blutstrom hineinragen. Tritt dieser Fall ein, wird die normalerweise laminare Strömung des Blutes gestört und in eine turbulente Strömung verändert, was zur Zerstörung und Ablagerung von Blutkörperchen führen kann. Wie klinische Untersuchungen gezeigt haben, führt dies zu einem vermehrten Re-Stenose-Risiko. Daher sind die Anforderungen an eine solche Stent-Craft insbesondere in ihrer Kombination sowohl mecha- nisch biologisch wie hämodynamisch außerordentlich hoch.
Zusammenfassung der Erfindung
Ausgehend von diesem Stand der Technik und den Anforderungen an ein solches Produkt liegt der vorliegenden Erfindung die Aufgabe zugrunde, günstige mechanische und hämodynamische Eigenschaften gleichzeitig zur Verfügung zu stellen.
Diese Aufgabe wird durch einen Stent mit den Merkmalen des Anspruches 1 gelöst.
Ein hochflexibler, dünner Kunststoffüberzug überzieht vollständig (oder teilweise z.B. bei Anwendung im Bereich von Seitenästen) die Maschen des Stents nach dem Prinzip eines Regenschirms. Risse der Gefäßwand können damit ohne chirurgischen Eingriff geschlossen werden. Die Flexibilität eines normalen Stents bleibt erhalten, so dass die Anpassung an den biologischen Gefäßverlauf besser ist. Durch die geringere Wandstärke und den kantenfreien Überzug bleibt der Blutfluß laminar.
Grundsätzlich kann jeder den hämodynamischen Anforderungen entsprechende, ausreichend elastisch und flexible Kunststoff verwendet werden. Untersuchungen mit Polyurethan haben gezeigt, daß die Gefahr des Verschlusses des Gefäßes durch ver- stärkte Ablagerung von Blutkörperchen im Bereich des Stents, wie es bei Dacron bei diesen kleinen Durchmessern im Bereich der Koronargefäße (2 bis 5mm) zu erwarten ist, bei Polyurethan vermindert ist.
Weitere Vorteile ergeben sich aus den Unteransprüchen.
Kurzbeschreibung der Figur
Im folgenden wird die Erfindung anhand eines in der einzigen Figur 1 dargestellten Ausführungsbeispiels näher erläutert. Die Figur zeigt einen aufgedehneten Stent, wobei zur Verdeutlichung der Kunststoffüberzug auf der linken Seite im Vordergrund weggeschnitten ist.
Ausführliche Beschreibung bevorzugter Ausführungsbeispiele
Die Erfindung wird jetzt beispielhaft unter bezug auf die beigefügte Zeichnung näher erläutert. Allerdings handelt es sich bei den Ausführungsbeispielen nur um Beispiele, die nicht das erfinderische Konzept auf eine bestimmte Anordnung beschränken sol- len.
Die Figur zeigt einen Stent im aufgedehnten Zustand, der zum dauernden oder vorübergehenden Verbleib im menschlichen oder tierischen Körper bestimmt ist. Es versteht sich von selbst, daß der Stent auch eine beliebige andere Form haben kann. Der Stent wurde mit einem Ballonkatheter im Körper implantiert, um z.B. Gefäßverengungen wieder aufzudehnen. Ist der Stent am gewüschten Ort im Körper angelangt, wird er von dem Ballonkatheter aufgedehnt.
Der Stent 10 selbst besteht aus einem aufgrund mechanischer oder geometrischer Anforderungen ausgebildeten Grundkörper aus einem Grundmaterial wie z.B. Metall, Metall-Legierung, Kunststoff oder einer Kombination dieser Materialien. Der Stent ist im Querschnitt vorzugsweise rund, so daß er sich an die Gefäßwand anlegen kann und in seiner Mitte den Durchfluß für die Körperflüssigkeit frei läßt. Der Stent 10 besitzt mehrere Maschen, wobei die im Vordergrund liegenden Maschen in der Fig. 1 mit 10a und die im Hintergrund, also auf der Rückseite liegenden Maschen mit 10b bezeichnet sind.
Der Stent ist in Fig. 1 vollständig - zumindest aber teilweise - von einem dünnen, flexiblen, bioverträglichen Überzug 11 aus Kunststoff überzogen oder in diesen Kunststoff eingebettet. Dieser Überzug 11 ist zwischen den Maschen 10a, 10b aufgespannt. Vorzugsweise überspannt er voll (oder teilweise z.B. bei Anwendung im Bereich von Seitenästen) die Maschen des Stents nach dem Prinzip eines Regenschirms. Risse der Gefäßwand können damit ohne chirurgischen Eingriff geschlossen werden. Die Flexibilität eines normalen Stents bleibt erhalten und damit ist die Anpassung an den biologischen Gefäßverlauf besser. Durch die geringe Wandstärke und den kantenfreien Überzug bleibt der Blutfluß laminar. Der Kunststoff muß allerdings in der Lage sein, die beim Aufdehnen des Stents erfolgende Dehnung mitzumachen als auch den von der Gefäßwand ausgehenden Kräften standzuhalten. Der Überzug 11 aus Kunststoff kleidet den Stent z.B. innen in der sich nach dem Aufweiten bildenden Röhre aus. Wird der Stent als Stent-Gefäß-Prothese (Craft) eingesetzt, werden die günstigen mechanischen und hämodynamischen Eigenschaften eines geeigneten einzelnen Stents auf eine Stent-Gefäß-Prothese (Craft) übertragen.
Grundsätzlich kann jeder den hämodynamischen Anforderungen entsprechende, ausreichend elastisch und flexible Kunststoff verwendet werden. Als besonders geeignet hat sich Polyurethan erwiesen. Es zeigt sich, daß die Gefahr des Verschlusses des Gefäßes durch verstärkte Ablagerung von Blutkörperchen im Bereich des Stents, wie es bei Dacron bei diesen kleinen Durchmessern im Bereich der Koronargefäße (2 bis 5mm) zu erwarten ist, bei Polyurethan vermindert ist.
Die Erfindung kann auch bei größeren Gefäßen z.B. bei Aneurysmen eingesetzt wer- den.
Es versteht sich von selbst, dass diese Beschreibung verschiedensten Modifikationen, Änderungen und Anpassungen unterworfen werden kann, die sich im Bereich von Äquivalenten zu den anhängenden Ansprüchen bewegen.
Claims
1. Stent zum dauernden oder vorübergehenden Verbleib im menschlichen oder tieri- sehen Körper mit einem aufgrund mechanischer oder geometrischer Anforderungen ausgebildeten Grundkörper aus einem Grundmaterial wie z.B. Metall, Metall- Legierung, Kunststoff oder einer Kombination dieser Materialien, der in Wirkverbindung mit einem Kunststoff in den Körper verbracht wird, dadurch gekennzeichnet, daß der Stent zumindest teilweise von einem dünnen, flexiblen, bioverträgli- chen Kunststoff überzogen oder in diesen Kunststoff eingebettet ist.
2. Stent nach Anspruch 1 , dadurch gekennzeichnet, daß der Kunststoff die Maschen des aufgedehnten Stents wie ein „Regenschirm" überspannt, wobei die Maschen die Streben des Regenschirms bilden.
3. Stent nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß der Kunststoff den Stent innen in der sich nach dem Aufweiten bildenden Röhre auskleidet.
4. Stent nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, daß der Kunst- Stoff Polyurethan ist.
5. Stent nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, daß der Stent eine Stent-Gefäß-Prothese (Craft) ist.
