Abstract
Es wird eine implantierbare Gelenkpfanne, insbesondere für Hüftgelenkendoprothesen beschrieben, die eine im Knochen zu verankernde Schale (10) und einen in dieser Schale (10) gehaltenen Einsatz (20.1, 20.2) zur Lagerung des Gelenkkopfes aufweist. Um wahlweise Einsätze (20.1, 20.2) aus verschiedenen Materialien mittels einer selbsthemmenden Konusverbindung in der Schale (10) zu befestigen, ist ein Adapter (28) vorgesehen, der in die Innenkonusfläche (14) der Schale (10) eingesetzt werden kann, um den Kegelwinkel für die selbsthemmende Konusverbindung zu verkleinern.

Implantierbare Gelenkpfanne, insbesondere für Hüftgelenken- doprothesen
Die Erfindung betrifft eine implantierbare Gelenkpfanne, insbesondere für Hüftgelenkendoprothesen, gemäß dem Oberbegriff des Anspruchs 1.
Gelenkendoprothesen insbesondere Hüftgelenkendoprothesen bestehen aus einer Gelenkpfanne, die einen Gelenkkopf aufnimmt und gleitfähig lagert. Bei der Hüftgelenkendoprothese wird die Gelenkpfanne in Beckenknochen verankert, während der Gelenkkopf mittels eines Schaftes im Femur verankert wird. Die Ge- lenkpfanne besteht häufig aus einer Schale, die in dem Knochen verankert wird und einem in der Schale gehaltenen Einsatz, der den Gelenkkopf aufnimmt und die Gleitfläche für den Gelenkkopf bildet. Dies hat den Vorteil, dass die Schale in Bezug auf Material und Gestaltung zur Verankerung im Knochen optimiert werden kann, während das Material des Einsatzes im Hinblick auf eine optimale Gleitpaarung mit dem Gelenkkopf und eine maximale Lebensdauer gewählt werden kann. Außerdem kann bei Wechseloperationen der Einsatz aus der Schale entfernt und ersetzt werden.
Es ist bekannt, den Einsatz mittels einer selbsthemmenden Konusverbindung in der Schale zu halten. Hierzu weist die Schale eine Innenkonusfläche auf, in welche der Einsatz mit einer äußeren Konusfläche eingeschlagen wird, so dass sich der selbsthemmende Halt des Einsatzes in der Schale ergibt. Soll bei einer Wechseloperation der Einsatz aus der Schale entfernt werden, wird dieser gegen die Selbsthemmung aus der Schale herausgeschlagen.
Die Schale wird in der Regel im Hinblick auf die mechanischen Eigenschaften aus Metall gefertigt. Für den Einsatz werden vorwiegend Metall, Keramik oder Kunststoff (Polyethylen) ver-

wendet. Besteht die Schale aus Metall ergibt sich ein unterschiedlicher Selbsthemmungswinkel für die KonusVerbindung, je nachdem welches dieser Materialien für den Einsatz verwendet wird. Beispielsweise ergibt sich für die Paarung Metall-Kera- mik ein größerer Selbsthemmungswinkel als für die Paarung Metall-Metall. Die Einhaltung des Selbsthemmungswinkels ist von großer Bedeutung, weil bei einem größeren Kegelwinkel der Einsatz nicht zuverlässig in der Schale hält und weil bei einem kleineren Kegelwinkel der Einsatz nicht mehr oder nur mit großen Schwierigkeiten wieder aus der Schale gelöst werden kann.
Bei den bekannten Gelenkpfannen ist es aus diesem Grund notwendig, für jedes Material des Einsatzes eine Schale mit einem Kegelwinkel der Innenkonusfläche zur Verfügung zu stellen, der dem Selbsthemmungswinkel der Paarung mit dem Einsatzmaterial entspricht. Dies bedeutet einen wesentlichen Kostenaufwand, insbesondere für die Lagerhaltung der Kliniken. Es müssen nämlich nicht nur unterschiedliche Schalentypen bevorratet werden, sondern auch noch jeder Schalentyp in unterschiedlichen Ausführungen für die verschiedenen Materialien des Einsatzes .
Aus der EP 0 694 294 AI ist eine implantierbare Gelenkpfanne, insbesondere für Hüftgelenkendoprothesen bekannt, bei welcher in den Innenkonus der Schale eine Zwischenhülse eingesetzt wird, die ihrerseits mit einer Innenkonusfläche den Einsatz aufnimmt. Die Zwischenhülse hat die Form eines Kegelstumpf- mantels, wobei die Kegelwinkel der äußeren Mantelfläche und der inneren Mantelfläche übereinstimmen und dem Selbsthemmungswinkel zwischen Innenkonusfläche der Schale und Außenko- nusflache des Einsatzes entsprechen. Die Zwischenhülse wird selbsthemmend in dem Innenkonus der Schale gehalten und der Einsatz wird selbsthemmend in der Zwischenhülse gehalten. Die Zwischenhülse dient zum Entfernen des Einsatzes aus der Schale. Hierzu weist die Zwischenhülse einen äußeren Rand mit Gewindebohrungen auf, in welche Gewindestifte eingeschraubt
werden können, die gegen die Stirnfläche der Schale drücken, um so die Zwischenhülse mit dem Einsatz gegen die Selbsthemmung aus der Schale herauszudrücken. Auch hier ist für jede Materialpaarung von Schale und Einsatz eine besondere Konus- form der Schale und der Zwischenhülse erforderlich.
Der Erfindung liegt die Aufgabe zugrunde, eine implantierbare
Gelenkpfanne, insbesondere für Hüftgelenkendoprothesen zur
Verfügung zu stellen, bei welcher eine einheitliche Schale für Einsätze aus verschiedenen Materialien verwendet werden kann.
Diese Aufgabe wird erfindungsgemäß gelöst durch eine Gelenkpfanne mit den Merkmalen des Anspruchs 1.
Vorteilhafte Ausführungen der Erfindung sind in den Unteransprüchen angegeben.
Erfindungsgemäß ist ein Adapter vorgesehen, der in den Innenkonus der Schale der Gelenkpfanne eingesetzt werden kann, um den Kegelwinkel für den Einsatz entsprechend dem Selbsthemmungswinkel des jeweils verwendeten Materials des Einsatz anzupassen. Die Schale weist eine Innenkonusfläche auf, deren Kegelwinkel dem größten bei dem üblicherweise verwendeten Materialien des Einsatzes auftretenden Selbsthemmungswinkel entspricht, z. B. dem Selbsthemmungswinkel eines Keramik-Einsatzes. Wird ein Einsatz aus diesem Material, z. B. ein Keramik-Einsatz verwendet, so kann dieser Einsatz unmittelbar in die Schale eingesetzt werden und wird selbsthemmend in dem Innenkonus der Schale gehalten. Soll ein Einsatz aus einem anderen Material verwendet werden, welches in Paarung mit dem Metall der Schale einen kleineren Selbsthemmungswinkel aufweist, so wird der Adapter in den Innenkonus der Schale eingesetzt, wobei nun der kleinere Kegelwinkel der inneren Mantelfläche des Adapters den Selbsthemmungswinkel für den Einsatz definiert.
Dadurch wird der Aufwand für die Herstellung der Gelenkpfanne
und insbesondere der Aufwand für die Lagerhaltung in der Klinik erheblich reduziert. Es ist nur noch notwendig, Schalentypen auf Lager zu halten, die sich in Bezug auf die Verankerung unterscheiden. Es ist nicht mehr notwendig, von jedem dieser Typen auch noch unterschiedliche Ausführungen für unterschiedliche Materialien des Einsatzes zu bevorraten.
Der Adapter wird in einer bevorzugten Ausführung in die Schale eingeschnappt, so dass der Adapter einfach eingesetzt werden kann. Die Schnappverbindung besteht vorzugsweise aus einem an dem Adapter ausgebildeten Außenwulst, der in eine Hinter- schneidung der Schale einschnappt . Um das Einschnappen zu ermöglichen, ist der Adapter axial geschlitzt, wobei vorzugsweise ein durchgehender axialer Schlitz vorgesehen ist, um den Durchmesser des gesamten Adapters für das Einsetzen bzw. Herausnehmen zu verkleinern.
Im Folgenden wird die Erfindung anhand von in der Zeichnung dargestellten Ausführungsbeispielen näher erläutert. Es zeigen
Figur 1 eine Gelenkpfanne in teilweise axial geschnittener Seitenansicht ,
Figur 2 eine erste Ausführung des Adapters in Axialansicht und in Seitenansicht und
Figur 3 eine zweite Ausführung des Adapters in Axialansicht und in Seitenansicht .
Figur 1 zeigt die Gelenkpfanne einer implantierbaren Hüftge- lenkendoprothese . Die Gelenkpfanne weist eine Schale 10 auf, die mit einem selbstschneidenden Außengewinde 12 ausgebildet ist, um in den Beckenknochen eingeschraubt werden zu können. Die Schale 10 besteht vorzugsweise aus einer Titan. Andere Ausführungen der Schale 10 und andere Arten der Befestigung der Schale 10 in dem Knochen sind ebenfalls bekannt und können erfindungsgemäß in gleicher Weise verwendet werden.
Die Schale 10 weist eine Innenkonusfläche 14 auf, die sich nach innen verjüngt und an ihrem inneren Ende mit einer Hin- terschneidung 16 erweiterten Durchmessers in den abgerundet flachen Schalenboden 18 übergeht.
In die Schale 10 wird ein Einsatz 20.1 bzw. 20.2 eingesetzt, der eine innere konkave halbkugelförmige Lagerfläche 22 aufweist. In die Lagerfläche 22 wird der nicht dargestellte Kugelkopf der Hüftgelenkendoprothese eingesetzt und gleitend gelagert.
Die Innenkonusfläche 14, die die Form eines geraden Kreiskegelstumpfes hat, weist einen Kegelwinkel auf, der dem Selbst- hemmungswinkel einer Metall -Keramik-Gleitpaarung entspricht. Dieser Kegelwinkel (Winkel zwischen zwei diametralen Mantellinien) beträgt ca. 19°.
In die Innenkonusfläche 14 der Schale 10 kann ein Einsatz 20.1 aus Keramik-Material eingesetzt werden. Dieser Einsatz 20.1 weist eine äußere Mantelfläche 24.1 auf, die in der Form der Innenkonusfläche 14 der Schale 10 entspricht, d. h. ebenfalls die Form eines geraden Kreiskegelstumpfes hat, dessen Kegelwinkel mit dem Kegelwinkel der Innenkonusfläche 14 der Schale 10 übereinstimmt. An die äußere Mantelfläche 24.1 schließt sich ein Einsatzboden 26 an, der in dem Schalenboden 18 der Schale 10 aufgenommen wird. Wird der Einsatz 20.1 aus Keramik-Material in die Schale 10 eingesetzt und leicht eingeschlagen, so ergibt sich eine selbsthemmende Konusverbindung zwischen der äußeren Mantelfläche 24.1 des Einsatzes 20.1 und der Innenkonusfläche 14 der Schale 10, welche den Einsatz 20.1 fest in der Schale 10 hält.
Um den Einsatz 20.1 im Bedarfsfalle wieder aus der Schale 10 entfernen zu können, sind an dem Einsatz 20.1 nicht darge- stellte, an sich bekannte Mittel vorgesehen, um ein Extraktionsinstrument ansetzen zu können, mit welchem der Einsatz mit leichten Hammerschlägen unter Überwindung der Selbsthem-
mung der Konusverbindung wieder gelöst werden kann.
Alternativ zu dem Einsatz 20.1 aus Keramik-Material kann in die Schale 10 auch ein Einsatz 20.2 aus Metall eingesetzt werden. Da eine Metall-Metall-Gleitpaarung einen kleineren Selbsthemmungswinkel aufweist als die Metall -Keramik-Gleitpaarung, wird in die Innenkonusfläche 14 der Schale 10 ein Adapter 28 eingesetzt. Der Adapter 28 besteht aus Metall, vorzugsweise aus Titan wie die Schale 10. Der Adapter 28 hat die Form eines geraden Kreiskegelstumpfraantels. Die äußere Mantelfläche 30 des Adapters 28 weist den selben Kegelwinkel auf wie die Innenkonusfläche 14 der Schale 10. Auf diese Weise kann der Adapter 28 passend in die Innenkonusfläche 14 eingesetzt werden. Die innere Mantelfläche 32 des Adapters 28 weist einen kleineren Kegelwinkel auf als die äußere Mantelfläche 30. Dieser Kegelwinkel der inneren Konusfläche 32 entspricht dem Selbsthemmungswinkel einer Metall-Metall-Gleitpaarung und beträgt beispielsweise etwa 10°.
In den in die Schale 10 eingesetzten Adapter 28 wird der Einsatz 20.2 aus Metall eingesetzt. Der Einsatz 20.2 entspricht in seiner Form im Wesentlichen dem Einsatz 20.1 aus Keramik- Material. Seine kegelstumpfförmige äußere Mantelfläche 24.2 weist jedoch einen Kegelwinkel auf, der dem Kegelwinkel der inneren Mantelfläche 32 des Adapters 28 entspricht. Wird der Einsatz 20.2 in den Adapter 28 eingeschlagen, so ergibt sich dadurch eine selbsthemmende Konusverbindung zwischen der äußeren Mantelfläche 24.2 des Einsatzes 20.2 und der inneren Mantelfläche 32 des Adapters 28. Diese selbsthemmende Konusver- bindung hält den Einsatz 20.2 aus Metall in dem Adapter 28, der seinerseits in der Schale 10 gehalten ist. Auch hier kann der Einsatz 20.2 in der vorstehend beschriebenen Weise wieder unter Überwindung der Selbsthemmung der KonusVerbindung aus dem Adapter 28 gelöst werden.
Figur 2 zeigt eine erste Ausführung des Adapters 28. Der Adapter 28 hat die Form eines Kegelstumpfmanteis, an dessen Ende
mit dem kleineren Durchmesser ein umlaufender Außenwulst 34 angeformt ist. Mit diesem Außenwulst 34 schnappt der Adapter 28 in die Hinterschneidung 16 der Schale 10 ein, die sich innen an deren Innenkonusfläche 14 anschließt. Dadurch wird der Adapter 28 formschlüssig in der Innenkonusfläche 14 der Schale 10 gehalten.
Um den aus Metall bestehenden Adapter 28 mit seinem Außenwulst 34 in die Hinterschneidung 16 einschnappen zu können, weist der Adapter 28 einen axial durchgehenden Schlitz 36 auf, dessen Breite beispielsweise etwa 3 mm beträgt. Der Adapter 28 kann durch axiales Eindrücken in den Innenkonus der Schale 10 oder mittels eines geeigneten Werkzeuges gegen seine Elastizität im Umfang um die Breite dieses Schlitzes 36 zusammengezo- gen werden, so dass der Adapter 28 in die Innenkonusfläche 16 der Schale 10 eingesetzt werden kann. Nach dem Einsetzen wird der Adapter 28 freigegeben, so dass er sich wieder elastisch auseinander spreizt und mit seinem Außenwulst 34 in die Hinterschneidung 16 der Schale 10 eingreift und formschlüssig in der Schale 10 gehalten wird. Dabei hält die Elastizität des
Adapters 28 diesen mit seiner äußeren Mantelfläche 30 voll- flächig und stabil an der Innenkonusfläche 16 der Schale 10 anliegend. Umgekehrt kann der Adapter 28 aus der Schale 10 wieder entnommen werden, indem der Adapter 28 wieder um die Breite des Schlitzes 36 zusammengezogen wird.
In Figur 3 ist eine weitere Ausführung des Adapters 28 gezeigt. In dieser Ausführung ist der Adapter 28 nicht durchgehend geschlitzt, sondern weist in seinem inneren Rand mit dem kleineren Durchmesser eine Vielzahl von in gleicher
Winkelteilung gegeneinander versetzten axialen Einschnitten 38 auf. Durch die Einschnitte 38 wird der innere Rand des Adapters 28 in einzelne Federzungen unterteilt, die jeweils an ihrem freien Ende den Außenwulst 34 als Rastnase aufweisen. Beim Eindrücken des Adapters 28 in die Innenkonusfläche 14 der Schale 10 werden diese durch die Einschnitte 38 gebildeten Federzungen elastisch nach innen gedrückt, bis der Außenwulst
34 in die Hinterschneidung 16 gelangt und die Federzungen wieder auseinanderspreizen können, so dass der Adapter 28 formschlüssig in der Schale 10 gehalten wird. In dieser Ausführung ist der Adapter 28 unlösbar mit der Schale 10 verbun- den.
Es ist ohne weiteres ersichtlich, dass in die Schale 10 auch ein Einsatz aus Kunststoff, z. B. aus Polyethylen eingesetzt werden kann. In dieser Ausführung entspricht der Einsatz in seiner Form dem Einsatz 20.1 aus Keramik-Material. Da das Kunststoff-Material eine höhere elastische Verformbarkeit aufweist, kann an der äußeren Mantelfläche des Einsatzes aus Kunststoff ein Außenwulst angeformt sein, der dem Außenwulst 34 des Adapters 28 entspricht, so dass der Einsatz aus Kunst- stoff-Material mit diesem Außenwulst in die Hinterschneidung 16 der Schale 10 einschnappen kann. Der Einsatz aus Kunststoff wird somit formschlüssig gehalten, so dass keine selbsthemmende Konusverbindung notwendig ist.

Claims
1. Implantierbare Gelenkpfanne, insbesondere für Hüftgelenkendoprothesen, mit einer Schale (10) und einem mittels einer selbsthemmenden Konusverbindung in der Schale (10) gehaltenen Einsatz (20.1, 20.2), gekennzeichnet durch einen Adapter (28) , der zur Verkleinerung des Kegelwinkels der selbsthemmenden Konusverbindung in die Schale (10) einsetzbar ist und der die Form eines Kegelstumpfmantels hat, dessen äußere Man- telfläche (30) einen größeren Kegelwinkel aufweist als dessen innere Mantelfläche (32) .
2. Gelenkpfanne nach Anspruch 1, d a d u r c h g e k e n n z e i c h n e t, dass die Schale (10) eine Innenkonusfläche (14) aufweist, deren Kegelwinkel dem Selbsthemmungswinkel eines Einsatzes (20.1) aus Keramik-Material entspricht, dass der Kegelwinkel der äußeren Mantelfläche (30) des Adapters (28) dem Kegelwinkel der Innenkonusfläche (14) der Schale (10) entspricht und dass der Kegelwinkel der inneren Mantelfläche (32) des Adapters
(28) dem Selbsthemmungswinkel eines Einsatzes (20.2) aus Metall entspricht.
3. Gelenkpfanne nach Anspruch 2 , d a d u r c h g e k e n n z e i c h n e t, dass die Schale (10) aus Metall besteht, dass der Adapter (28) aus Metall besteht, dass der Kegelwinkel (Winkel zwischen zwei diametralen Mantellinien) der Innenkonusfläche (14) der Schale (10) und der Kegelwinkel der äußeren Mantelfläche (30) des Adapters (28) ca. 19° betragen und dass der Kegelwinkel der inneren Mantelfläche (32) des Adapters (28) ca. 10° beträgt.
4. Gelenkpfanne nach einem der vorhergehenden Ansprüche, d a d u r c h g e k e n n z e i c h n e t, dass der Adapter (28) in die Schale (10) formschlüssig einrastbar ist.
5. Gelenkpfanne nach Anspruch 4, d a d u r c h g e k e n n z e i c h n e t, dass der Adapter (28) einen Außenwulst (34) aufweist, der in eine Hinterschneidung (16) der Schale (10) eingreift.
6. Gelenkpfanne nach Anspruch 5, d a d u r c h g e k e n n z e i c h n e t, dass der Außenwulst (34) an dem Ende des Adapters (28) mit dem kleineren Durchmesser angeformt ist und dass sich die Hin- terschneidung (16) innen an die Innenkonusfläche (14) der Schale (10) anschließt.
7. Gelenkpfanne nach Anspruch 6, d a d u r c h g e k e n n z e i c h n e t, dass der Adapter (28) zumindest im Bereich des Außenwulstes (34) axial geschlitzt ist.
8. Gelenkpfanne nach Anspruch 7, d a d u r c h g e k e n n z e i c h n e t, dass der Adapter (28) einen axial durchgehenden Schlitz (36) aufweist .
